Abstract:We tested the hypothesis that the production of prostaglandin E2 (PGE2) is impaired in duodenal ulcer disease and affected by treatment and healing. This was investigated by a study of maximal PGE2 synthesis rates in duodenal and antral mucosal biopsies obtained at endoscopy. The patients were divided into three groups. Group (a): endoscopically normal controls (n = 56); group (b): treatment controls (non-DU disease: gastric ulcer or oesophagitis treated by histamine H2 receptor antagonists) (n = 41); and group (c): patients with DU disease (n = 183) further subdivided into group (c1) active ulcer not on treatment (n = 47), (c2) treated active ulcer (n = 35), (c3) healed ulcer on treatment (n = 86), and (c4) healed ulcer not on treatment (n = 15). Group (a) synthesised (mean (SD] 106.6 (39.0) pg PGE2/mg wt of tissue from the duodenal bulb and 129.9 (56.9) from the second part of the duodenum. No difference was found between group (a) and (b) at either site. Group (c1) ulcer rim made 49.8 (22.7) and at all stages ulcer rim and scar made less than the control duodenal bulb (p less than 0.02). Uninvolved duodenal bulb form groups (c1) (63.4 (31.0], (c2) (83.6 (38.5], and (c3) (81.5 (31.1], however, also made significantly less than controls (p less than 0.02) and a similar though non-significant trend was seen in group (c4). Biopsies from the second part of the duodenum did not synthesise significantly less than the control group but a similar trend was noticed at each stage of ulcer treatment. Biopsies of control antrum synthesised 124.5 (32.2) but only 93.7 (44.2) in group (cl) (p < 0.005). All stages of duodenal ulcer healing were associated with a decreased capacity to synthesise the major prostaglandin PGE2 at the ulcer site and the uninvolved duodenal bulb and, in acute untreated duodenal ulcer, the uninvolved antrum. This decreased capacity may be the consequence of the disease process itself and not secondary to the treatment, indicating a basic pathophysiological abnormality which may explain the characteristic tendency of the disease to relapse.

Optimal suppression of gastric acid and ulcer healing in children with peptic ulcer

Abstract:Objective\r\nIt is well known that the suppression of gastric acidity is one of the most essential factors in promoting healing and preventing relapse of peptic ulcers. Little is known, however, about how to inhibit gastric acid secretion effectively in children with peptic ulcers. The present study aimed at understanding the optimal acid suppression by examining the relationship between ulcer healing and the suppression of gastric acid in children.\r\n\r\n\r\nMethods\r\nPatients with peptic ulcers diagnosed by endoscopy were randomized to receive either oral ranitidine (group A) or oral omeprazole (group B) for 4-6 weeks (mean 5 weeks) . Intragastric 24-hour pH monitoring was performed before treatment, and repeated three times: on day 1, weeks 1 and 5 post-dosing. Endoscopy was repeated on week 5 to evaluate the healing condition of ulcers.\r\n\r\n\r\nResults\r\nTwenty-seven patients followed up by endoscopy and pH monitoring were eligible for final analysis. The ulcers in 16 patients of group A and in all (11 cases) of group B were healed. No significant differences in the gastric mean pH, median pH and percentage of time with gastric pH \u003e 3 between the two groups were observed before treatment. Gastric pH increased significantly after treatment in both groups. These parameters in group A decreased significantly during weeks 1 and 5 as compared to those on day 1 post-dosing; the duration of drug action was also shortened significantly. The gastric mean pH and mean median pH in group B were significantly higher during weeks 1 and 5than on day 1 of post-dosing, the duration of drug action was also prolonged significantly. According to the result of endoscopic re-examination, the authors subdivided group A into unhealed (group C) and healed (group D) groups. The gastric acid suppressive effect in group C attenuated significantly by the end of the 5-week treatment and the differences of gastric mean pH and median pH, percentage of time with pH \u003e 3 between week 5 and pre-treatment level were not significant. While these parameters in group D were still significantly higher during week 5than those obtained before treatment. The gastric mean pH and median pH, percentage of time with pH \u003e 3 and the duration of drug action during week 5 were significantly higher in group D than in group C. The mean shortest duration of drug action in group B was 10 h, while it was 5 h in group D.\r\n\r\n\r\nConclusions\r\nThe onset of tolerance to ranitidine during the therapy might have affected healing of ulcers in the subjects. Effieacy of rabeprazole in freating patients with duodenal ulcer and its effect gastric parietal cell density

Abstract:Objective To study the clinical effectiveness and the changes of gastric parietal cell density in the patients with duodenal ulcer(DU) after rabeprazole therapy.Methods(1)We studied 32 patients with DU diagnosed endoscopically and a control group of 24 patients with chronic superficial gastritis.Endoscopy of the upper digestive tract was done early in the morning after an overnight fast.Two biopsies were obtained in the midgastric corpus(one on the lesser curvature,the other on the greater curvature) for light microscopy;one was obtained in the antrum 3 cm from the pylorus in the greater curvature for HP culture.(2)HP positive patients with DU received triple regimen(rabeprazole plus two antibiotics) for 1 week,followed by rabeprazole 20 mg once daily for 3 weeks.After the treatment,all the patients received endoscopy again.Results The parietal cell density in DU group(44.41±4.16) was significantly higher than in controls(42.37±4.51)(P<0.05),but there was no significant difference between HP positive(44.35±3.69) and HP negative DU subjects(44.67±6.25) or between HP positive controls(42.90±4.39) and HP negative gastritis controls(42.75±4.68)(P>0.05).Parietal cell density(44.41±4.16) was significantly lower after the rabeprazole treatment(42.44±4.72)(P<0.05).Rabeprazole could cure DU and rapidly relieve the symptoms of abdominal pain,acid reflux,and heartburn.The cure rate,healing rate,and effective rate after 4 weeks' treatment were 40.6%,90.6%,and 100%,respectively.Conclusions The parietal cell density in DU patients is significantly higher than that in chronic superficial gastritis patients,and is markedly reduced after the rabeprazole treatment.Rabeprazole is effective and safe in the treatment of DU.It can rapidly relieve the symptoms and obtain a high healing rate of DU.
Polaprezinc Combined with Triple Therapy for Helicobacter Pylori Associated Gastritis: A Prospective, Multicenter, Randomized Clinical Trial

Abstract:Background: Helicobacter pylori is one of the most prevalent global pathogens and treatment failure is increasing due to rising antibiotic resistance.Polaprezinc is a gastric mucosal protective agent complex of zinc and L-carnosine, shown to improve efficacy when combined with triple therapy in Japan.Aim: To compare the clinical efficacy and safety of the polaprezinc combined to triple therapy versus standard triple therapy in the eradication of H. pylori.Methods: This is a randomized, parallel-group controlled, prospective multicenter study in 11 cities of China.Treatment-naïve patients with H. pylori-associated gastritis were randomly assigned to the following groups: Arm A which consists of triple-therapy (omeprazole 20mg, amoxicillin 1g, and clarithromycin 500mg twice daily) plus polaprezinc 75mg twice daily, Arm B which consists of the same triple-therapy plus polaprezinc 150mg twice daily, or Arm C which is the standard triple-therapy for 14 days.Eradication was considered successful if C 13 or C 14 urea breath test was negative 4 weeks after completion of therapy as the primary outcome.Digestive symptoms improvement 7, 14, 28 days after completion of intervention and rates of adverse effects were the secondary outcomes.Results: 332 enrolled, and 303 patients completed the study, with 106, 96, and 101 patients in Arm A, Arm B, and Arm C. In the intention-to-treat (ITT) analysis, the rate of H. pylori eradication was significantly higher for Arm A (77.0%) and Arm B (75.9%) vs Arm C (58.6%) (p<0.01),whereas there was no difference between Arm A and Arm B (P=0.90) (Table 1, Figure 1).In the per-protocol (PP) analysis, the rate of H. pylori eradication was significantly higher for Arm A (81.1%) and Arm B (83.3%) vs Arm C (61.4%) (P<0.01), while there was no difference between Arm A and Arm B (P=0.62) (Table 1, Figure 1).In all three groups, there were significant symptomatic improvement including abdominal pain, acid reflux, belching, heartburn, bloating, nausea, and vomiting at 7 days, 14 days and 28 days after treatment compared to baseline (P<0.0001).The adverse event rate of Arm B was higher than Arm A (P=0.04) and Arm C (P=0.02), including mild leukopenia, elevated liver enzymes, elevated serum uric acid, and high blood pressure, but there were no serious adverse events.Conclusions: Standard dose polaprezinc combined with standard triple therapy can significantly improve H. pylori eradication rate without an increase in adverse event and is well-tolerated.H. pylori eradication rate in the ITT and PP populations
[The characteristics of duodenogastric reflux in peptic ulcer and chronic gastritis]

Abstract:Patients with gastroduodenal ulcer, chronic gastritis (CG) and healthy controls (124, 57 and 24 subjects, respectively) were examined for intensity and duration of duodenogastric reflux (DGR). These indices were assessed by concentrations of gastric juice bilirubin. It is found that DGR occurred in 29% of the examinees free of gastroduodenal diseases, does not correlate with morphological CG-induced changes and Helicobacter pylori contamination degree. The reflux in CG seems to be secondary in pathogenesis of this disease. In patients with aggravated duodenal and gastric ulcer the reflux in the basal stage is 2-2.5 and 5-6 times higher than in the controls, respectively. DGR parameters correlated with the height of the ulcer defect. Retrograde entrance of the duodenal content into the stomach in duodenal ulcer enhances acid production in the stomach through gastrin mechanisms.
The Symptomatic Improvement and Safety of Rabeprazole on Peptic Ulcer

Abstract:Objective To observe the symptomatic improvement and safety of rabeprazole in treatment of patients with peptic ulcer. Methods One thousand one hundred and fifty nine patients with peptic ulcer identified by gastroscopy were recruited from 13 hospitals in this multi-center open clinical trial. All patients were given rabeprazole 10mg d for 5 days, and the symptoms of epigastric pain, acid regurgitation and heart burn as well as side-effects were recorded daily during the therapy. Results The alleviating rate of all symptoms was 67.5%,98.3% and 99.7% in the 1st,2nd and 5th day, while the dissolving rate of symptoms was 23.3%, 51.8% and 83.6%, respectively. Alleviating rate and dissolving rate in the 2nd and 5th day were much higher than the 1st day (P0.01). Conclusion Rabeprazole is highly effective and safe in relieving the symptom of peptic ulcer.

Abstract:AIM: To evaluate the quality of gastric ulcer healing after different antiulcer treatment by endoscopic ultrasonography (EUS).METHODS: The patients were divided into three groups, and received lansoprazole, amoxicillin and clarithromycin for 1 wk. Then group A took lansoprazole combined with tepreton for 5 wk, group B took lansoprazole and group C took tepreton for 5 wk. Endoscopy and EUS were performed before and 6 wk after medication.RESULTS: There was no significant difference in cumulative healing rate to S stage between the groups (89%, 82% vs 83%, P>0.05). The rate of white scar formation was significantly higher in group A than in groups B and C (67%, 36%, 50%, P<0.05). The average contraction rates of the width of ulcer crater, length of disrupted muscularis propria layer and hypoechoic area were higher in group A than in groups B and C (0.792 +/- 0.090, 0.660 +/- 0.105 vs 0.668 +/- 0.143, P<0.05). The hypoechoic area disappeared in four cases of group A, one of group B and two of group C. The percentage of hypoechoic area disappearance was higher in group A than in the other two groups (44%, 9% vs 17%, P<0.05). Gastric ulcer healing was better in group A.CONCLUSION: The combined administration of proton-pump inhibitors and mucosal protective agent can improve gastric ulcer healing. Peptic ulcer and gastritis in uraemia, with particular reference to the effect of Helicobacter pylori infection

Abstract:Aims: To determine: (i) the prevalence of histological gastritis and peptic ulcer; and (ii) the clinical features of peptic ulcer, in patients with end-stage renal failure. Methods: Upper endoscopy was performed by a single observer in 268 patients with end-stage renal failure over a 6-year period. Gastric histology and Helicobacter pylori status were studied in 40 consecutive subjects in whom there were no contraindications for gastric biopsy and who had not used antibacterial drugs in the preceding 4 weeks. As there are only limited data for healthy volunteers in Singapore, 33 age-, sex- and race-matched patients with functional dyspepsia from an earlier drug trial and 18 healthy volunteers who were not age-matched were used as controls. The clinical features of 43 consecutive uraemic patients with peptic ulcer were compared with those of 118 consecutive non-uraemic peptic ulcer patients seen by the same author. Results: Among uraemic patients, histological gastritis was less common, compared with healthy volunteers and functional dyspepsia patients. Helicobacter pylori infection as assessed by histology was also less common among uraemic patients compared with functional dyspepsia patients, but the difference was not statistically significant on serological assessment. Uraemic patients with ulcer had an equal sex ratio, in contrast to a male preponderance among peptic ulcer patients with normal renal function. Uraemic patients with ulcer were more likely to be pain-free, to present with haemorrhage, to have multiple ulcers and postbulbar duodenal ulcers, but were less likely to have H. pylori infection. Among uraemic subjects, the prevalence of H. pylori infection was similar whether or not peptic ulcer was present. Conclusions: The prevalence of histological gastritis was lower in uraemic patients when compared with patients with functional dyspepsia and healthy volunteers. Peptic ulcers in uraemic subjects have different clinical characteristics from peptic ulcer in non-uraemic subjects.
Different dose of new generation proton pump inhibitors for the treatment of Helicobacter pylori infection: A meta-analysis

Abstract:The evidence on whether high-dose new generation proton pump inhibitors (PPIs) including rabeprazole and esomeprazole achieve a higher eradication rate of Helicobacter pylori has not been assessed. The primary comparison was eradication and adverse events (AEs) rate of standard (esomeprazole 20 mg bid, rabeprazole 10 mg bid) versus high-dose (esomeprazole 40 mg bid, rabeprazole 20 mg bid) PPIs. Sub-analyses were performed to evaluate the eradication rate between Asians and Caucasians, clarithromycin-resistance (CAM-R) strains, and clarithromycin-sensitivity (CAM-S) strains of different dose PPIs. We conducted a literature search for randomized controlled trials comparing high-with standard-dose esomeprazole and rabeprazole for H. pylori eradication and AEs. A total of 12 trials with 2237 patients were included. The eradication rate of high-dose PPIs was not significantly superior to standard-dose PPIs regimens: 85.3% versus 84.2%, OR 1.09 (0.86–1.37), P = 0.47. The high dose induced more AEs than those of the standard dose, but didn’t reach statistical significance (OR 1.25, 95% CI: 0.99–1.56, P = 0.06). Subgroup analysis showed that the difference in eradication rate of PPIs between high- and standard-dose groups were not statistically significant both in Asians (OR 0.99, 95% CI 0.75–1.32, P = 0.97) and Caucasians (OR 1.27, 95% CI 0.84–1.92, P = 0.26). Furthermore, there were similar eradication rates in CAM-S (OR 1.2; 95% CI 0.58–2.5; P = 0.63) and CAM-R strains (OR 1.08; 95% CI 0.45–2.56; P = 0.87) between the standard-and high-dose groups. High and standard dosages of new generation of the PPIs showed similar H. pylori eradication rates and AEs as well as between Asian versus Caucasian populations, with or without clarithromycin-resistance. However, further studies are needed to confirm.

A Randomized Controlled Multicenter Trial of Rebamipide on Chronic Non-Atrophic Gastritis

Abstract:Objective To investigate the effect of rebamipide on chronic non-atrophic gastritis (NAG) with erosion and its protection of gastric mucosa from Helicobacter priori(Hp) associated lesions.Methods Patients(n=452)with endoscopically confirmed NAG with erosion from 11 hospitals in China were enrolled and randomly assigned at a ratio of 3:1 to receive either rebamipide(100 mg t.i.d.)or sucralfate(1.0 t.i.d.)for 8 weeks.Hp infected patients received eradication treatment before randomization.Symptoms,endoscopic scores and histological changes were recorded before and after therapy.Concentrations of serum prostaglandin E(PGE:)and oxygen free radical(MDA)were measured in patients from 2 centers.Results Per-protocol analysis(n=415)showed that the dyspeptic symptom score in rebamipide group decreased significantly after eight weeks of treatment. The endoscopic inflammation score in rebamipide group also decreased from 2.65 ±0.09 to 0.60±0.10(P<0.001),which was,significantly better than that of sucralfate group(P<0.001).Histological findings were consistent with the endoscopic findings.There Was a significant elevation(P=0.002)in PGE2 concentration in mucesa from rebamipide-treated subjects [(225.4±18.3) pg/g vs.(266.7±14.7)Pg/g] compared with that in sucralfate group.The concentration of MDA significantly decreased from(325.9±65.6)mmoL/g to(216.5±61.5)mmol/g,which is markedly different from that of sucralfate group(P=0.046).No statistical difference was found between Hp eradication group,Hp infection group and Hp negative group,regarding the effect of Rebamipide.Conclusion Compared to sucralfate,Rebamipide demonstrates a superior effect on improvement of dyspepsia symptom and endoscopic findings in erosive NAG,which is not influenced by Hp infection.
Gastro-protecting Effect of Gefarnate on Chronic Erosive Gastritis with Dyspeptic Symptoms.

Abstract:Background The role of gastro-protecting agents on symptomatic chronic gastritis is unclear. This multicenter, open, randomized trial was designed to compare the comprehensive effects of gefarnate with sucralfate on erosive gastritis with dyspeptic symptoms.Methods Totally 253 dyspepsia patients confirmed with erosive gastritis were enrolled from six centers in China. They randomly received either daily 300 mg gefarnate or 3 g sucralfate for six weeks. The primary endpoint was the effective rate of both treatments on endoscopic erosion at week six.Results Gefarnate showed an effective rate of 72% and 67% on endoscopic score and dyspeptic symptom release, which is statistically higher than sucralfate (40.1% and 39.3%, P <0.001, intension-to-treat). For histological improvement, gefarnate showed both effective in decreasing mucosal chronic inflammation (57.7% vs. 24.8%, P <0.001, intension-to-treat) and active inflammation (36.4% vs. 23.1%, P <0.05, intension-to-treat) than the control. A significant increase of prostaglandins and decrease of myeloperoxidase in mucosa were observed in gefarnate group. Severity of erosion is non-relevant to symptoms but Helicobacter pylori (H. pylon) status does affect the outcome of therapy.Conclusions Gefarnate demonstrates an effective outcome on the mucosal inflammation in patients with chronic erosive gastritis. Endoscopic and inflammation score should be the major indexes used in gastritis-related trials. Chin Med J 2012;125(16):2878-2884

Abstract:BACKGROUND Gastric ulcers (GUs) have a high risk of clinical morbidity and recurrence, and further exploration is needed for the prevention, diagnosis, and treatment of the disease. AIM To investigated the effects of a diet plan on pepsinogen (PG) I, PG II, gastrin-17 (G-17) levels and nutritional status in patients with GUs. METHODS A total of 100 patients with GUs treated between May 2022 and May 2023 were enrolled, with 47 patients in the control group receiving routine nursing and 53 patients in the experimental group receiving dietary nursing intervention based on a diet plan. The study compared the two groups in terms of nursing efficacy, adverse events (vomiting, acid reflux, and celialgia), time to symptom improvement (burning sensation, acid reflux, and celialgia), gastric function (PG I, PG II, and G-17 levels), and nutritional status [prealbumin (PA) and albumin (ALB) levels]. RESULTS The experimental group showed a markedly higher total effective rate of nursing, a significantly lower incidence of adverse events, and a shorter time to symptom improvement than the control group. Additionally, the experimental group’s post-intervention PG I, PG II, and G-17 levels were significantly lower than pre-intervention or control group levels, whereas PA and ALB levels were significantly higher. CONCLUSION The diet plan significantly reduced PG I, PG II, and G-17 levels in patients with GUs and significantly improved their nutritional status.

Abstract:Background: The pharmacokinetic and clinical behaviors of many proton pump inhibitors (PPIs) in peptic ulcer treatment are altered by CYP2C19 genetic polymorphisms. This non-inferiority study evaluated the efficacy and safety of the novel PPI anaprazole compared with rabeprazole. We also explored the influence of Helicobacter pylori ( H. pylori ) infection status and CYP2C19 polymorphism on anaprazole. Methods: In this multicenter, randomized, double-blind, double-dummy, positive-drug parallel-controlled, phase III study, Chinese patients with duodenal ulcers were randomized 1:1 to receive rabeprazole 10 mg + anaprazole placebo or rabeprazole placebo + anaprazole 20 mg once daily for 4 weeks. The primary efficacy endpoint was the 4-week ulcer healing rate assessed by blinded independent review. Secondary endpoints were the proportion of patients with improved overall and individual duodenal ulcer symptoms at 4 weeks. Furthermore, exploratory subgroup analysis of the primary endpoint by H. pylori status and CYP2C19 polymorphism was conducted. Adverse events were monitored for safety. Non-inferiority analysis was conducted for the primary endpoint. Results: The study enrolled 448 patients (anaprazole, n = 225; rabeprazole, n = 223). The 4-week healing rates were 90.9% and 93.7% for anaprazole and rabeprazole, respectively (difference, -2.8% [95% confidence interval, -7.7%, 2.2%]), demonstrating non-inferiority of anaprazole to rabeprazole. Overall duodenal ulcer symptoms improved in 90.9% and 92.5% of patients, respectively. Improvement rates of individual symptoms were similar between the groups. Healing rates did not significantly differ by H. pylori status or CYP2C19 genotype for either treatment group. The incidence of treatment-emergent adverse events was similar for anaprazole (72/220, 32.7%) and rabeprazole (84/219, 38.4%). Conclusions: The efficacy of anaprazole is non-inferior to that of rabeprazole in Chinese patients with duodenal ulcers. Registration: ClinicalTrials.gov, NCT04215653.

Abstract:Areas of the body accessible to gastric secretions, such as the stomach and duodenum, are most commonly damaged by circumscribed lesions of the upper gastrointestinal tract mucosa. Peptic ulcer disease is the term for this illness (PUD). About 80% of peptic ulcers are duodenal ulcers, with stomach ulcers accounting for the remaining 20%. Duodenal ulcers are linked to the two primary results about Helicobacter pylori infection and COX inhibitor users. Additional causes might include drinking, smoking, stress, and coffee consumption. The indications and symptoms of a duodenal ulcer depend on the patient's age and the lesion's location. For duodenal ulcers, proton pump inhibitors (PPIs) are the usual course of treatment. This comprehensive study included an in-depth literature search in the literature and methods section using electronic databases such as PubMed, ScienceDirect, and Google Scholar. The search method included publications published from the inception of the relevant database to the present. Inclusion criteria included studies investigating different treatment options for duodenal ulcer disease, including traditional pharmacotherapy and naturopathic treatments. Data mining includes information on treatment techniques, treatment outcomes, and possible synergies between conventional and herbal treatments. In addition, this review critically examines the available information on the effectiveness, safety, and possible side effects of different treatments. The inclusion of conventional and herbal treatments is intended to provide a comprehensive overview of the many treatment options available for duodenal ulcer disease. A more comprehensive and personalized treatment plan can be achieved by incorporating dietary changes, lifestyle modifications, and, if necessary, herbal therapies to complement other treatments normally.

Abstract:Despite the generally accepted fact that Helicobacter pylori is the main cause of gastric and duodenal ulcers, its role is not always decisive. It was found that this pathogen does not have a sufficient level of invasiveness, and in about 70% of cases does not lead to morphological disorders of the gastric and intestinal mucosa. Speaking about the possible causes of the development of peptic ulcer, one cannot but take into account the influence of stressful situations, aggravated heredity, and nutritional errors. One of the urgent problems of modern medicine is to increase the effectiveness of peptic ulcer treatment and prevent recurrence of the disease, and the main direction of antiulcer therapy is the eradication of Helicobacter pylori with the parallel administration of antisecretory drugs. Meanwhile, it should be noted that the lack of certainty in matters of pathogenesis over a number of years led to the unreasonable prescription of a large number of various medications, ranging from antacids and antispasmodics to sedatives and tranquilizers. Such a wide variety of therapeutic drugs used indicates that for a long time it was not possible to confirm the real effectiveness and clinical efficacy of most antiulcer drugs, which once again confirmed the correctness of the phrase said by the famous therapist V.Kh. Vasilenko regarding the fact that "gastroduodenal ulcers heal due to treatment, despite treatment and even in spite of treatment".

Abstract:Peptic ulcer disease (PUD) involves ulceration of the mucosa in the stomach and/or proximal duodenum. The main causes are Helicobacter pylori infection and nonsteroidal anti-inflammatory drug (NSAID) use. PUD occurs in 5% to 10% of people worldwide, but rates have decreased by more than half during the past 20 years. This reduction is thought to be because of H pylori management, more conservative use of NSAIDs, and/or widespread use of proton pump inhibitors (PPIs). Common symptoms include postprandial abdominal pain, nausea, vomiting, and weight loss. These symptoms have broad overlap with those of other conditions, making clinical diagnosis difficult. Endoscopy is the gold standard for diagnosis, especially in older patients and those with alarm symptoms, but a test-and-treat strategy (noninvasive test for H pylori and treat if positive) can be used for younger patients with no alarm symptoms. Numerous treatment regimens are available, all of which include PPIs plus antibiotics. As an alternative to PPIs, a new triple therapy with vonoprazan (which blocks acid production) plus antibiotics has been approved and appears to be superior to conventional therapy with PPIs plus antibiotics. At least 4 weeks after treatment, repeat testing for H pylori should be obtained to confirm cure. When possible, NSAIDs should be discontinued; when not possible, antisecretory cotherapy should be considered.

Abstract:Introduction: In recent years, the frequency of idiopathic peptic ulcers (IPUs) has increased. However, the clinicopathological characteristics of IPU have not been fully elucidated and treatment methods for recurrent and refractory cases have not yet been established. Case presentation: A man in his forties complained of epigastric discomfort. Esophagogastroduodenoscopy revealed a gastric ulcer in the lesser curvature of the gastric angle. After Helicobacter pylori was eradicated, the gastric ulcer recurred despite the administration of a potassium competitive acid blocker (PCAB), and a diagnosis of IPU was made. Twenty-four-hour intragastric pH monitoring revealed insufficient gastric acid suppression. Misoprostol was added to the patient's treatment. Subsequently, the ulcer healed and recurrence was not observed. Conclusion: For refractory IPU, the evaluation of pathophysiological function through 24-h gastric pH monitoring may lead to the selection of an appropriate treatment. If a proton pump inhibitor and PCAB do not improve the IPU, combination treatment with misoprostol may be considered as an option.

Abstract:The effect of intermittent dosage with omeprazole on basal and pentagastrin stimulated gastric acid secretion and fasting plasma gastrin was assessed in eight duodenal ulcer subjects who were in remission. Omeprazole (20 mg daily) was given for a three day 'weekend' each week for two months. Twenty four hours after the first and eighth weekend, basal and peak acid output were still markedly suppressed (greater than 50%) compared with pretreatment. After the treatment free four days, however (just before the eighth weekend), peak acid output had returned to pretreatment values; basal acid output was still somewhat reduced (mean 3.6 mmol/l) but the difference from baseline was not statistically significant. Fasting plasma gastrin concentration increased slightly but significantly, from a baseline median of 17 pmol/l to 25 and 31 pmol/l respectively, 24 hours after the first and eighth weekends. All but two values (of 16) remained within the reference range. Before the fourth and eighth weekends, and again at 12 days and three months after treatment, gastrin values were not significantly different from baseline. Thus a 'weekend therapy' regimen with this long acting antisecretory compound produces substantial acid suppression, but for only part of the week, with modest and reversible changes in fasting plasma gastrin. It should therefore be suitable for efficacy testing for prevention of recurrence of peptic ulcer or reflux oesophagitis.

Abstract:BACKGROUND With the current interest in toric intraocular lenses (IOLs), it is critical to establish whether these implants rotate following cataract surgery. If there is continual rotation of the implant with capsular bag contraction, then the orientation of the astigmatic correction would be unstable and thus not clinically useful. METHODS Twenty-eight eyes were followed for up to 6 months postoperatively. These consecutive eyes were selected on the basis of demonstrating adequate dilation following surgery, such that the points of haptic to optic insertion could be visualized with the slit-lamp microscope. Sequential photographs and comparisons of the orientation of the lenses postoperatively were performed. RESULTS With the exception of one lens implant, none of the 28 IOLs demonstrated significant rotation. Of the 16 eyes followed for 6 months, none demonstrated rotation of greater than 4 degrees, which was within experimental error. CONCLUSIONS Continued rotation of PMMA IOLs following implantation in the capsular bag seldom occurs. Thus, the concept of toric IOLs appears to be quite usable.
