Abstract:States play a principal role in designing and implementing publicly funded long term care programs. They are in a key position to determine what setting, from whom, and under what philosophy of care individuals can access long term care. State long term care systems have evolved over the past 25 years to support increased availability of community based care for older individuals. States have pursued three broad strategies to increase availability. One set of strategies aims to increase capacity through expanding the supply, public funding and eligibility for a wide array of home and community based services (HCBS). A second set of strategies seeks to constrain institutional growth in order to increase the dollars available for HCBS. Managed/capitated long term care is a third strategy that is a hybrid of the other two approaches. Empirically, increased capacity through use of Medicaid and Medicare resources and growth in the supply of community based care providers, combined with moderation of institutional bed supply, are associated with enhanced access and expenditures for community based long term care. The effectiveness of capitated or managed systems of care varies by model and in some cases is still under study. State fiscal resources are key to the ability to support community based care for the elderly. Targeted federal support may be critical to the continued expansion of community based care, given heterogeneity in state resources, coupled with an increasing demand for long term care.
Attorney-case manager collaboration in the conservatorship of older adults

Abstract:The complex issues of long-term care are everchanging, with no one discipline having the corner on all the intricate options, regulations, and ethical choices that often confront an aging person. This is most critical when a person's incompetence requires the appointment of a conservator or guardian. When the probate court finds it necessary to appoint a conservator/guardian for an older adult, and family or other informal caregivers are unavailable or unable to assume the role, the responsibility is often delegated to an attorney. However, an attorney's expertise is in the field of law, not necessarily in the complex and intricate long-term health care system. Maintaining a mentally incompetent and functionally disabled person in the safest and least restrictive environment is demanding. It requires a thorough knowledge and understanding of the physical and emotional changes associated with the aging process, an area of expertise of long-term care case managers. Combining the clinical proficiency of the case manager with the legal and financial expertise of the attorney benefits the conserved older adult. This article illustrates this collaborative approach to long-term care through the use of several case histories.
Contradictions in case management: client-centered theory and directive practice with frail elderly

Abstract:New long-term care legislation and managed care legitimate case management for gatekeeping and advocacy for frail elderly. Qualitative in-depth interviews with case managers who are social workers and nurses were used to address the question: To what extent is client-centered theory reflected in case management practice? Differences between reported client-centered theory and directive practice were identified by five themes: (a) client wishes versus system constraints; (b) the paradox of working to keep clients home versus the perceived inevitability of nursing home placement; (c) client centeredness versus the case manager's care plan; (d) client self-determination versus strategies of persuasion; (e) informing the client about case management versus the reality of practice. Ethical implications, competing demands, and environmental constraints are discussed. Suggestions are made to better integrate client-centered theory with directive practice.

Abstract:This article argues for a fresh look at how we provide long-term care (LTC) for older persons. Essentially, LTC offers a compensatory service that responds to frailty. Policy debate around LTC centers on costs, but we are paying for something we really don't want. Building societal enthusiasm (or even support) for LTC will require re-inventing and re-branding. LTC has three basic components: personal care, housing, and health care (primarily chronic disease management). They can be delivered in a variety of settings. It is rare to find all three done well simultaneously. Personal care (PC) needs to be both competent and compassionate. Housing must provide at least minimal amenities and foster autonomy; when travel time for PC raises costs dramatically, some form of clustered housing may be needed. Health care must be proactive, aimed at preventing exacerbations of chronic disease and resultant hospitalizations. Enhancing preferences means allowing taking informed risks. Payment incentives should reward both quality of care and quality of life, but positive outcomes must be defined as slowing decline. Paying for services but not for housing under Medicaid would automatically level the playing field between nursing homes (NH) and community-based services. Regulations should achieve greater parity between NH and community care and include both positive and negative feedback. Providing post-acute care should be separate from LTC. Using the tripartite LTC framework, we can create innovative flexible approaches to providing needed services for frail older persons in formats that are both desirable and affordable. Such care will be more socially desirable and hence worth paying for.

Abstract:This paper examines the roots and consequences of the “medicalization” of aging in America. The present set of programs under Medicare, Medicaid and Title XX are described, with particular attention given to the unproductive “dichotomy” created between health and medical needs (addressed primarily under Medicare and Medicaid) and the interrelated social needs of the elderly, provided for, albeit inadequately, through Title XX. The present medically and institutionally based approach to care for the elderly in America is contrasted with approaches that seek to create environments that increase self‐reliance for the elderly. Programmatic redesign strategies providing a full continuum of care are premised on such concepts as “normalization” and the “least restrictive alternative.” Particular attention is focused on the area of community based home health and social services. The experiences of several states in providing more comprehensive community based services are explored. One such program—California's recently initiated Multi‐Purpose Senior Services Project—is described. It potentially provides a wide range of services that previously were unavailable through more narrowly designed categorical programs. The importance of broader programs of community based care for the elderly, and important research directions, are examined.

Abstract:OBJECTIVE The goal of this study was to evaluate the quality of images obtained with a prototype imaging technique, multiacquisition variable-resonance image combination (MAVRIC), compared with fast spin-echo (FSE) images in the evaluation of patients who have undergone hip, shoulder, or knee arthroplasty. MATERIALS AND METHODS MRI with metal-artifact reduction FSE and MAVRIC sequences was performed in the care of 122 patients who had undergone 74 hip, 27 shoulder, and 21 knee arthroplasties. The FSE and MAVRIC images were subjectively graded for visualization of the synovium, prosthesis-bone interface, and hip abductors or supraspinatus tendon. The presence of synovitis, osteolysis, or supraspinatus tendon tear was recorded. RESULTS Visualization of the synovium was significantly better on MAVRIC images than on FSE images of the hip (p < 0.0001), shoulder (p < 0.01), and knee (p < 0.01). Synovitis was detected only on the MAVRIC images of nine subjects (12%) who had undergone hip arthroplasty and five subjects (18%) who had undergone shoulder arthroplasty. Visualization of the periprosthetic bone was significantly better on MAVRIC images of the hip (p < 0.0001), shoulder (p < 0.0001), and knee (p < 0.01). Osteolysis was detected only on the MAVRIC images of 12 subjects (16%) who had undergone hip arthroplasty, six (22%) who had undergone shoulder arthroplasty, and five (24%) who had undergone knee arthroplasty. Visualization of the supraspinatus tendon was significantly better on MAVRIC images (p < 0.0001). Supraspinatus tendon tears in 12 subjects (44%) were detected only on MAVRIC images. CONCLUSION MAVRIC complements the information on FSE images after arthroplasty and is a useful additional sequence, particularly when there is concern about synovitis, periprosthetic osteolysis, or the presence of a supraspinatus tendon tear.

Abstract:Objectives: To describe a demonstration program that uses master's-level care managers (care advocates) to link Medicare managed care enrollees to home- and community-based services, testing whether referrals to noninsured services can reduce service usage and increase member satisfaction and retention. Design: Using an algorithm designed to target frail, high-cost users of Medicare insured healthcare services, the program partners PacifiCare's Secure Horizons and four of its medical groups with two social service organizations. Setting: Three care advocates located in two community-based social services agencies using telephone interviews to interact with targeted elders living in the community. Participants: Three hundred ninety PacifiCare members aged 69 to 96 receiving care from four PacifiCare-contracted medical groups. Intervention: The 12-month intervention provides telephone assessment, links to eight types of home- and community-based services, and monthly follow-up contacts. Measurements: Sociodemographic characteristics of intervention participants, types of service referrals, and acceptance rates. Results: Lessons learned included the importance of building a shared vision among partners, building on existing relationships between members and providers, and building trust without face-to-face interactions. Conclusion: The program builds on current insured case management services and offers a practical bridge to community-based services.

In many instances this is because the clinical aspect of case management is performed by personnel lacking in clinical experience. Over the next few years federal and state legislators will be looking at the issue of case management and who provides it. Many groups will be putting in their bid. The home care community must be prepared to fight, support, or activate its own case management program.

Abstract:Families are the bedrock of long-term care, but policymakers have traditionally considered them "informal" caregivers, as they are not part of the formal paid caregiving workforce. As chronic and long-term care systems have become more complex and as more demanding tasks have been shifted to families, this view is no longer sustainable. The care transition process offers a critical opportunity to treat family caregivers as important care partners. Enhancing their involvement, training, and support will contribute to reducing unnecessary rehospitalizations and improving patient outcomes. The contributions and experiences of family caregivers should be considered in gathering information to shape policies and practice; training health care professionals; developing programs; and reforming financing.

Abstract:BACKGROUND Acculturation may influence women's perceptions of health care experiences and may explain the epidemiologic paradox, whereby foreign-born women have lower rates of adverse birth outcomes than United States (US)-born women. We evaluated the relationship between maternal acculturation and specific dimensions of prenatal interpersonal processes of care (IPC) in ethnically diverse women. METHODS Cross-sectional analysis of 1243 multiethnic, postpartum women who delivered at Kaiser Permanente Medical Center in Walnut Creek or San Francisco General Hospital. Women retrospectively reported on their experiences in seven domains of IPC during their pregnancy pertaining to communication, decision making, and interpersonal style. The primary independent variables were four measures of maternal acculturation: birthplace, English language proficiency, the number of years residing in the US, and age at immigration to the US. Generalized linear models, stratified by infant outcome, measured the association between each maternal acculturation measure and specific IPC domains while adjusting for type of health insurance, demographic, and reproductive factors. RESULTS Approximately 60% of the sample was foreign-born, 36% reported low English proficiency, 43% had resided in the US <10 years, and 35% were age 20 years or older when they immigrated to the US. Over 64% of the women reported having public insurance during pregnancy. In adjusted analyses among women who delivered term and normal birth weight infants, less acculturated women and women with non-private health insurance were more likely to have higher mean IPC scores when compared to more acculturated or US-born women and women with private health insurance, respectively. CONCLUSION In a large and ethnically diverse sample of childbearing women in Northern California, less acculturated pregnant women reported better prenatal care experiences than more acculturated and US-born women, another dimension of the "epidemiologic paradox." However, the relationship between acculturation and IPC, as reported during the postpartum period, differed according to infant outcomes.

Abstract:Abstract Home modification (HM) can promote older adults’ functioning as their needs change, reduce fall risks, and support caregivers. A supportive home environment is increasingly important as homes become healthcare delivery sites for home and community-based services (HCBS). HM is funded and administered by disparate agencies, often hindering access to HM services for at-risk older adults who need them the most. The Aging Network (State Units on Aging (SUAs), Area Agencies on Aging (AAAs), and Title VI organizations serving Native American older adults) plays an important but not well understood role in HM. To address this lack of research, the USC Leonard Davis School of Gerontology, ADvancing States, and the National Association of Area Agencies on Aging in cooperation with Scripps Gerontology Center conducted three national surveys, with support from the Administration for Community Living: 1) directors of the 56 SUAs with an 89% response rate; 2) directors of the 618 AAAs with a 79% response rate; and 3) directors of 276 Title VI programs with an 84% response rate. Exemplary practices included HM advocacy through interagency coalitions; state and local plan priority setting; creative HM financing with housing, disability, and health care sectors, including partnerships with Medicaid agencies; and integration of HMs into state and local HCBS, including nursing home transition and caregiver support programs. Findings on the types of HM activities, service delivery barriers, funding sources, collaborations, and targeted populations will inform HM policy and practice for the Aging Network’s critical state and local agencies serving low-income older adults.

Abstract:Case management (CM) is an intervention for improving integrated care for patients with complex care needs. The implementation of this complex intervention often raises opportunities for change and collective leadership has the potential to optimize the implementation. However, the application of collective leadership in real-world is not often described in the literature. This commentary highlights challenges faced during the implantation of a CM intervention in primary care for people with complex care needs, including stakeholders' buy-in and providers' willingness to change their practice, selection of the best person for the case manager position and staff turnover. Based on lessons learned from PriCARE research program, this paper encourages researchers to adopt collective leadership strategies for the implementation of complex interventions, including promoting a collaborative approach, fostering stakeholders' engagement in a trusting and fair environment, providing a high level of communication, and enhancing collective leadership attitudes and skills. The learnings from the PriCARE program may help guide researchers for implementing complex healthcare interventions.

Abstract:Substantial investments were made in population based biobanks over the past decade. Ethnocultural community members are both sponsors and beneficiaries of biobanks. In addition, the success of biobank projects depends on community support and participation. Yet there are few empirical data on views, perceptions and interests of ethnocultural communities on biobanks. This silent gap in genomics, ethics and policy literatures has to be addressed. We conducted a qualitative research study with in-depth interviews of ethnocultural community leaders (e.g., members of the Canadian Parliament, school commissioners) on their perspectives concerning population specific genomics research and biobanks. An equal partnership model where public is not only informed, but also involved in decision-making processes was perceived as an essential democratic requisite. These empirical data on ethnocultural community leaders’ views, interests and perceptions identify several key socio-cultural and ethical factors that can be decisive for effective and sustainable community involvement in biobanks.
Parathyroid hormone and bone mass after kidney transplantation.

Abstract:Metabolic bone disorder develops during chronic renal failure and chronic dialysis treatment, continues after successful kidney transplantation, and is further aggravated by corticosteroids and immunosuppressants. The recognized risk factors for bone loss, i.e. parathyroid hormone (PTH) secretion, age, duration of hemodialysis, sex and post-transplantation period, were examined in this cross-sectional study of 91 stable kidney transplant recipients. Patient age was 21-67 years, time spent on dialysis 1-216 months, and post-transplantation period 1-228 months. Bone mineral density was measured by dual energy x-ray absorptiometry at the lumbar spine, femoral neck and radius. T-scores (bone mineral density expressed as standard deviation of healthy young population) were used in statistical analyses to avoid sex differences in bone mass. PTH was measured by a commercial kit (9.07 +/- 11.81 pmol/L). The femoral neck and radius correlated negatively with PTH and hemodialysis duration both in simple correlations and multiple regression, and femoral neck additionally with age. The lumbar spine correlated negatively only with PTH. Post-transplantation time did not correlate significantly with the three densitometry sites. Also, the frequency of osteoporosis or osteopenia for the femoral neck and spine did not differ between the first 12 months and subsequent period, but was positively influenced by hemodialysis duration. There was no sex difference for the variables analyzed in the study. The results indicated that prolonged hemodialysis treatment and consequently increased PTH secretion had unfavourable effect on the femoral neck and radius bone in kidney transplant recipients. Regarding predictors in this study, only PTH was found to adversely affect the lumbar spine. Further deterioration of change in the skeletal status could not be demonstrated, which might be explained by the reduction in PTH secretion and possibly by improvement of the bone disorder.
Flying beneath the radar of health reform: the community living assistance services and supports (CLASS) act

Abstract:The Patient Protection and Affordable Care Act attempts to address prevailing deficiencies in long-term care (LTC) financing through the Community Living Assistance Services and Supports (CLASS) Act, a national voluntary LTC insurance program administered by the Federal government. The CLASS Act is intended to supplement rather than supplant assistance received from other payers. Furthermore, its reliance on a cash benefit allocated by beneficiaries with the assistance of counseling services makes it consistent with the consumer-directed philosophy increasingly favored by the LTC advocacy community. Largely due to inadequate take-up, however, particularly among better than average risks, it is unlikely that implementation of the CLASS Act will fundamentally alter the current public-private partnership for LTC financing. Instead, voluntary enrollment combined with a lack of medical underwriting could lead to disproportionate numbers of high-cost enrollees. This could result in premium increases that further discourage participation on the part of the broader population. Barring making the program mandatory, there are a number of comparatively minor changes policymakers could make to strengthen the risk pool, though doing so will involve a trade-off between attracting better-off risks while eschewing those likely to need the benefit most. Thus, although the CLASS Act may provide a meaningful benefit for those who enroll, its impact on improving the affordability of LTC for most Americans will likely be limited. Most will continue to rely on substantial unpaid care, out-of-pocket payments when formal care is required, and Medicaid when all other money has run out.
Aging, Pensions and Long-term Care: What, Why, Who, How? Comment on "Financing Long-term Care: Lessons From Japan"

Abstract:Japan has been aging faster than other industrialized nations, and its experience offers useful lessons to others. Japan has been willing to expand its welfare state with a long-term care (LTC) insurance to finance home care and nursing home care for frail elderly. As Ikegami shows, it created new facilities and expanded specialized staffing for home care, developed a country-wide assessment system and shifted responsibilities between the central and local authorities over that assessment and the determination of co-payments for LTC. Faced with rapid growth in demand for LTC, the government felt the need for new cost control measures. The Japanese experience illustrates that new social policies take time to develop. There is often a need to adjust. But there are also other lessons. The main one is that there is no direct relation between the degree of population aging and total health spending. While aging requires adjustments in the organization of care, and expanding LTC for frail elderly, international studies show there is no need to worry about the 'unaffordability' of aging. In this commentary, we have framed four "What, Why, Who, and How" questions about LTC to (re-)define the borderlines between public and private responsibilities for the range of activities for which some (but certainly not all) frail elderly as well as many non-elderly require support in daily life.

Abstract:Long-term care (LTC) is one of the most pressing public policy challenges today. Implementing policies to meet the population's demands becomes relevant in many countries, particularly in a context of rapid population aging. Both the technical complexities and the financial burden of implementing LTC policies discourage policy makers' actions in this area. In this environment, targeted policies arise as a solution to reduce the cost of implementing LTC policies. This article presents several arguments in favor of implementing universal—vs targeted—LTC initiatives. Arguments are divided into general arguments against targeting public policies, using categories proposed by Amartya Sen, and LTC-specific arguments, based on the concept of LTC as social security. Information shows that despite the financial arguments in favor of targeted policies, in the case of LTC, its costs may overcome the benefits. These results provide important lessons for policy makers, particularly regarding the design of (universal) LTC policies, warning that the allegedly simple solution of targeting benefits needs to be revisited, and replaced for policies that could balance universalism and resource constraints. This message is particularly important today for countries that face the challenge of increasing LTC needs and tighter resource constraints.

Public-private partnership in care provision for ageing-in-place: A comparative study in Guangdong, China

Abstract:Objective: This paper investigates how the public-private partnership (PPP) functions in the care provision for older people in Guangdong, China, particularly for ageing-in-place programs. Methods: Three cities with diversified PPP forms of care provision for older people were chosen as case studies. Focus groups were conducted in these cities for the local authorities and care homes involved in the partnerships. Results: Three forms of PPP, including private sector involvement for the ownership, construction, and operation of the facilities delivering care services, were identified. The public-private co-constructed and privately run model with greater private sector involvement may lead to higher quality care in one specific program. The publicly constructed and privately run program is widely used in many cities, advancing the expansion of ageing-in-place programs in more communities. These PPPs reflect flexible and practical collaboration between the public and private sectors at the local level in the Chinese context. Conclusions: PPP have accelerated the development of a better care system for older people, mainly in terms of more affordable and higher-quality care options for the public. They are also a feasible solution for the local government due to more cost-effective and sustainable care provisions than previous publicly provided programs. The specific forms adopted are primarily associated with the scale of home care provision for ageing-in-place and the local government's financial capacity. Despite increasing private sector involvement, the public sector retains its crucial role in delivering care services targeting ageing in China.
