Abstract:ABSTRACT A systematic literature review on social determinations and consumption of psychoactive substances was realized. The research was accomplished in some databases, in Portuguese, English and Spanish, using the descriptors "Social Determinants in Health" and the Boolean descriptor AND for the term "Disorders Related to Substance Use". Then, 78 articles were selected, in which a concentration of studies was observed in the northern hemisphere, emphasizing on micro social factors. The most studied drugs were multiple substances (44.9%), alcohol (21.8%) and tobacco (15.4%), highlighting the determinants of income (35.9%), sex, family and territory (26.9% each). It is important to consider drug use as a complex and multifaceted biopsychosocial phenomenon, requiring greater production of evidence in developing countries, using different epistemological and methodological perspectives.
The consequences of childhood maltreatment on dual-diagnosis psychiatric conditions and clinical outcomes in substance use disorders: A systematic review and meta-analysis

Abstract:Background: Evidence suggests that the phenotypic expression of substance use disorders (SUD) may be influenced by exposure to childhood maltreatment (CM). Objective: To assess how CM could impact severity (relapse, treatment dropout, withdrawal), age of substance use onset, psychiatric dual diagnosis (depression, anxiety, PTSD, distress), and self-harm behavior/suicide attempts in adults with SUD, we performed a systematic review and meta-analyses. Methods: We searched PubMed, Web of Science, PsycINFO, and Embase to identify articles examining the association of maltreatment (physical, sexual, or emotional abuse or neglect) before age 18 years with clinical features and course of illness SUD. Multilevel random effects models were performed. Heterogeneity was quantified using the I2 statistic. Subgroup/meta-regression analyses were employed to investigate sources of heterogeneity. Risk of bias was assessed using an adapted version of the Newcastle-Ottawa Quality Assessment Scale. Results: Forty-five studies were included. CM was associated with all tested outcomes, such as anxiety (OR = 1.52; 95 % CI = 1.39, 1.65), depression (OR = 1.57; 95 % CI = 1.40, 1.77), PTSD (OR = 1.67; 95 % CI = 1.40, 1.99), psychological distress (OR = 1.49; 95 % CI = 1.24, 1.78), self-harm behavior and suicidal attempts (OR = 1.70; 95 % CI = 1.47, 1.96), SUD severity (OR = 1.15; 95 % CI = 1.04, 1.26), and younger age of onset use (OR = 0.76; 95 % CI = 0.69, 0.84). However, comparisons between outcomes revealed that the association was stronger for dual diagnosis symptoms/conditions than for measures of SUD severity itself. Meta-analyses for some outcomes had large statistical heterogeneity, though these were partly addressed through additional analyses. Gender, mean age of participants, polysubstance use pattern, substance of preference, and CM subtype were found to be moderators of these associations. Conclusions: The consequences of CM on individuals with SUD are more closely linked to co-occurring psychiatric manifestations than to the severity of common clinical indicators (relapse, treatment dropout, withdrawal), and this finding has important implications. Abstract:for both the individual and society. Given the high prevalence of mental disorders globally, it is necessary to triage resources to those who need them most. Although undertreatment receives considerable attention, previous studies suggest that between one-third and half of the patients who are treated for mental disorders do not meet formal criteria for such disorders and that 3–14% of people without a mental disorder receive treatment for problems with emotions or the use of substances in a given year. However, these data are far from conclusive in arguing that a high proportion of treatment going to patients with low need. This is true for several reasons. First, most studies failed to assess other indicators of need for treatment besides diagnoses (for example recent stressors or suicidal behaviours). This is important because mental disorders are not the only indicators of need for treatment. Second, most previous studies did not provide information about the sector in which patients who failed to meet full criteria for a disorder received their treatment. This is important because treatment of such patients in the human services sector (such as spiritual counselling during normal bereavement) or the self-help sector (such as participation in a trauma survivor support group) would not be seen as inappropriate. Third, most of these studies were carried out in a small set of countries (mostly the USA, but also Canada Germany, The Netherlands, Chile and Australia). As far as we are aware, no global data exist that examines the proportion of treatment for problems with emotional issues or substance use received by those patients who fail to meet full criteria for a mental disorder. Such a study is especially timely in an era when governments, policymakers and healthcare planners face competing priorities and economic constraints. Data are reported here on these issues based on an analysis of data collected in the World Health Organization (WHO) World Mental Health (WMH) surveys. The WMH surveys are general population surveys that used structured psychiatric interviews, validated for use worldwide, to measure the presence of mental disorders as well as treatment for problems with emotions or for substance use. The current study builds on earlier WMH studies reporting on the crossnational treatment of mental disorders by investigating treatment of people without mental disorders. The specific aims of the study were to investigate the proportion of patients in treatment for problems with emotions or substance use at anytime in the 12 months before interview who failed to meet criteria for a DSM-IV disorder at any time during that 12-month period, to investigate other indicators of need among these patients, to examine the sectors of treatment as well as the frequency and duration of their use of services, and to examine their self-reported reasons for seeking treatment.

Abstract:Substance use is a major cause of disability globally. This has been recognized in the recent United Nations Sustainable Development Goals (SDGs), in which treatment coverage for substance use disorders is identified as one of the indicators. There have been no estimates of this treatment coverage cross‐nationally, making it difficult to know what is the baseline for that SDG target. Here we report data from the World Health Organization (WHO)'s World Mental Health Surveys (WMHS), based on representative community household surveys in 26 countries. We assessed the 12‐month prevalence of substance use disorders (alcohol or drug abuse/dependence); the proportion of people with these disorders who were aware that they needed treatment and who wished to receive care; the proportion of those seeking care who received it; and the proportion of such treatment that met minimal standards for treatment quality (“minimally adequate treatment”). Among the 70,880 participants, 2.6% met 12‐month criteria for substance use disorders; the prevalence was higher in upper‐middle income (3.3%) than in high‐income (2.6%) and low/lower‐middle income (2.0%) countries. Overall, 39.1% of those with 12‐month substance use disorders recognized a treatment need; this recognition was more common in high‐income (43.1%) than in upper‐middle (35.6%) and low/lower‐middle income (31.5%) countries. Among those who recognized treatment need, 61.3% made at least one visit to a service provider, and 29.5% of the latter received minimally adequate treatment exposure (35.3% in high, 20.3% in upper‐middle, and 8.6% in low/lower‐middle income countries). Overall, only 7.1% of those with past‐year substance use disorders received minimally adequate treatment: 10.3% in high income, 4.3% in upper‐middle income and 1.0% in low/lower‐middle income countries. These data suggest that only a small minority of people with substance use disorders receive even minimally adequate treatment. At least three barriers are involved: awareness/perceived treatment need, accessing treatment once a need is recognized, and compliance (on the part of both provider and client) to obtain adequate treatment. Various factors are likely to be involved in each of these three barriers, all of which need to be addressed to improve treatment coverage of substance use disorders. These data provide a baseline for the global monitoring of progress of treatment coverage for these disorders as an indicator within the SDGs.

Abstract:BACKGROUND:Previous research suggests that many people receiving mental health treatment do not meet criteria for a mental disorder but are rather 'the worried well'.AIMS:To examine the association of past-year mental health treatment with DSM-IV disorders.METHOD:The World Health Organization's World Mental Health (WMH) Surveys interviewed community samples of adults in 23 countries (n = 62 305) about DSM-IV disorders and treatment in the past 12 months for problems with emotions, alcohol or drugs.RESULTS:Roughly half (52%) of people who received treatment met criteria for a past-year DSM-IV disorder, an additional 18% for a lifetime disorder and an additional 13% for other indicators of need (multiple subthreshold disorders, recent stressors or suicidal behaviours). Dose-response associations were found between number of indicators of need and treatment.CONCLUSIONS:The vast majority of treatment in the WMH countries goes to patients with mental disorders or other problems expected to benefit from treatment.

Abstract:Alcohol is a major cause of pre-mature death and individual suffering worldwide, and the importance of diagnosing and treating AUD cannot be overstated. Given the global burden and the high attributable factor of alcohol in a vast number of diseases, the need for additional interventions and the development of new medicines is considered a priority by the World Health Organization (WHO). As of today, AUD is severely under-treated with a treatment gap nearing 90%, strikingly higher than that for other psychiatric disorders. Patients often seek treatment late in the progress of the disease and even among those who seek treatment only a minority receive medication, mirroring the still-prevailing stigma of the disease, and a lack of access to effective treatments, as well as a reluctance to total abstinence. To increase adherence, treatment goals should focus not only on maintaining abstinence, but also on harm reduction and psychosocial functioning. A personalised approach to AUD treatment, with a holistic view, and tailored therapy has the potential to improve AUD treatment outcomes by targeting the heterogeneity in genetics and pathophysiology, as well as reason for, and reaction to drinking. Also, the psychiatric co-morbidity rates are high in AUD and dual diagnosis can worsen symptoms and influence treatment response and should be considered in the treatment strategies.

Development and Implementation of Substance Use Training for Allied and Behavioral Health Professionals: Addressing the Epidemic and Improving the Outcomes

Abstract:Allied and behavioral health professionals (ABHP) interact with clients who may be at higher risk for substance misuse or substance use disorders (SUD) due to mental health problems, relational problems, and acute/chronic pain due to injury/illness. These problems can lead to more substance misuse and affect daily life activities and overall health and wellbeing. Unfortunately, not enough individuals seek treatment due to stigma associated with substance use, partially because of the lack of properly trained and educated healthcare professionals. The researchers developed an interprofessional training program using the Screening, Brief Intervention, and Referral to Treatment (SBIRT) to develop learning modules focused on understanding the science of addiction, the stigma related to addictions, and the basic interviewing and assessment techniques to support appropriate substance use intervention and referral. The goal was to prepare ABHP students to appropriately address substance use as part of their future professional practice. The SBIRT learning modules used pre/post test quizzes to collect data on the effectiveness. 140 occupational and physical therapy and mental health counselling students completed the training and the results showed a significant increase in participants knowledge supporting the value of the SBIRT training.

Abstract:There is recognition that biomedical research into the causes of mental disorders and their treatment needs to adopt new approaches to research. Novel biomedical techniques have advanced our understanding of how the brain develops and is shaped by behaviour and environment. This has led to the advent of stratified medicine, which translates advances in basic research by targeting aetiological mechanisms underlying mental disorder. The resulting increase in diagnostic precision and targeted treatments may provide a window of opportunity to address the large public health burden, and individual suffering associated with mental disorders. While mental health and mental disorders have significant representation in the "health, demographic change and wellbeing" challenge identified in Horizon 2020, the framework programme for research and innovation of the European Commission (2014-2020), and in national funding agencies, clear advice on a potential strategy for mental health research investment is needed. The development of such a strategy is supported by the EC-funded "Roadmap for Mental Health Research" (ROAMER) which will provide recommendations for a European mental health research strategy integrating the areas of biomedicine, psychology, public health well being, research integration and structuring, and stakeholder participation. Leading experts on biomedical research on mental disorders have provided an assessment of the state of the art in core psychopathological domains, including arousal and stress regulation, affect, cognition social processes, comorbidity and pharmacotherapy. They have identified major advances and promising methods and pointed out gaps to be addressed in order to achieve the promise of a stratified medicine for mental disorders.

Abstract:Substance dependence represents a pervasive global concern within the realm of public health. Presently, it is delineated as a persistent and recurrent neurological disorder stemming from drug-triggered neuroadaptations in the brain's reward circuitry. Despite the availability of various therapeutic modalities, there has been a steady escalation in the mortality rate attributed to drug overdoses. Substantial endeavors have been directed towards the exploration of innovative interventions aimed at mitigating cravings and drug-induced repetitive behaviors. Within this review, we encapsulate the most auspicious contemporary treatment methodologies, accentuating meta-analyses of efficacious pharmacological and non-pharmacological approaches: including gabapentin, topiramate, prazosin, physical exercise regimens, and cerebral stimulation techniques.

Abstract:Considerable research documents the health consequences of psychosis and co-occurring substance use disorders. Results of randomized controlled trials assessing the effectiveness of psychosocial interventions for persons with dual diagnoses are equivocal but encouraging. Many studies are hampered by small, heterogeneous samples, high attrition rates, short follow-up periods, and unclear description of treatment components. The treatments available for this group of patients (which can be tailored to individual needs) include motivational interviewing, cognitive-behavioral therapy, contingency management, relapse prevention, case management, and skills training. Regardless of whether services follow integrated or parallel models, they should be well coordinated, take a team approach, be multidisciplinary, have specialist-trained personnel (including 24-hour access), include a range of program types, and provide for long-term follow-up. Interventions for substance reduction may need to be further developed and adapted for people with serious mental illnesses. Further quality trials in this area will contribute to the growing body of data of effective interventions.

Abstract:Abstract Introduction: The use of psychoactive substances in patients diagnosed with mental illness is an increasing public health problem; the purpose of this work was to carry out a study to identify the most prevalent psychoactive drug abuse patterns in hospitalized patients in a psychiatric hospital, to estimate the severity of substance abuse, as well as to determine the factors associated with drug consumption using multivariate statistical modeling. Methods An analytical, retrospective study was carried out through the review of patient records discharged during 2018. Sociodemographic data, diagnoses, clinical follow-up information, clinimetric score of the alcohol consumption, Tobacco and Substances Detection Test (ASSIST 2.0) were obtained as well as the toxicological profile results. Results A representative sample of 268 participants was obtained, 61.6% men and 38.4% women, 86.6% unemployed, 67.9% single, with an average age of 36 years and 8 years of schooling. The most frequent diagnoses were Schizophrenia (42%), Substance-Induced Psychotic Disorder (22%), and Bipolar Disorder (17.9%). The most frequent substances detected in urine toxicology test were: Cannabis 21.8% (n = 45) and methamphetamines 10.7% (n = 22). The substances with the most frequent consumption in the last 3 months were tobacco, ethanol, cannabis, and methamphetamines. The highest score in the severity of consumption were methamphetamines and cannabis. The most important predictors for ethanol consumption were tobacco and cannabis abuse, for cannabis abuse were schooling years and ethanol consumption, and for methamphetamine consumption, the most important predictors were cocaine and cannabis abuse. Conclusions Young, male, unemployed subjects, with lower educational level, a psychiatric condition, and psychoactive consumption represent an identifiable "sociodemographic phenotype" that has an increased risk of requiring hospital services on a more recurrent basis and that could represent a burden to the health system. Methamphetamines abuse continues to increase and is becoming a significant mental health and public health issue. Cannabis and methamphetamine use are more severe and have more serious repercussions in terms of psychiatric morbidity. Correct public policies must be designed to intervene in this population.
Dopamine D2/3 receptor availability in alcohol use disorder and individuals at high risk: Towards a dimensional approach

Abstract:Alcohol use disorder (AUD) is the most common substance use disorder worldwide. Although dopamine‐related findings were often observed in AUD, associated neurobiological mechanisms are still poorly understood. Therefore, in the present study, we investigate D2/3 receptor availability in healthy participants, participants at high risk (HR) to develop addiction (not diagnosed with AUD), and AUD patients in a detoxified stage, applying 18F‐fallypride positron emission tomography (18F‐PET). Specifically, D2/3 receptor availability was investigated in (1) 19 low‐risk (LR) controls, (2) 19 HR participants, and (3) 20 AUD patients after alcohol detoxification. Quality and severity of addiction were assessed with clinical questionnaires and (neuro)psychological tests. PET data were corrected for age of participants and smoking status. In the dorsal striatum, we observed significant reductions of D2/3 receptor availability in AUD patients compared with LR participants. Further, receptor availability in HR participants was observed to be intermediate between LR and AUD groups (linearly decreasing). Still, in direct comparison, no group difference was observed between LR and HR groups or between HR and AUD groups. Further, the score of the Alcohol Dependence Scale (ADS) was inversely correlated with D2/3 receptor availability in the combined sample. Thus, in line with a dimensional approach, striatal D2/3 receptor availability showed a linear decrease from LR participants to HR participants to AUD patients, which was paralleled by clinical measures. Our study shows that a core neurobiological feature in AUD seems to be detectable in an early, subclinical state, allowing more individualized alcohol prevention programs in the future.

Abstract:Introduction Depressive, anxiety and adjustment disorders are highly prevalent among mental health outpatients. The lack of funding for mental health problems produces inefficient results and a high burden of disease. New cost-effective group interventions aimed at treating these symptoms might be an appropriate solution to reduce the healthcare burden in mental health units. Mindfulness-based interventions (MBIs) have shown significant reductions in anxious, depressive and adjustment symptomatology. Recent research highlights the influence of compassion as a key mechanism of change. However, MBIs only address compassion implicitly, whereas compassion-based protocols consider it a core aspect of psychotherapy. In this randomised controlled trial, we hypothesise that the provision of attachment-based compassion therapy (ABCT), which is a compassion-based protocol, will be more effective than mindfulness-based stress reduction (MBSR), which is a conventional MBI programme, for the treatment of depressive, anxious and adaptive symptoms in patients in mental health settings. Methods and analysis Approximately 90 patients suffering from depressive, anxious or adjustment disorders recruited from Spanish mental health settings will be randomised to receive 8 weekly 2 hours group sessions of ABCT, 8 weekly 2.5 hours group sessions of adapted MBSR (with no full-day silent retreat) or treatment as usual (TAU), with a 1:1:1 allocation rate. Patients in the ABCT and adapted MBSR groups will also receive TAU. The main outcome will be general affective distress measured by means of the ‘Depression Anxiety Stress Scales-21’ at post-test as primary endpoint. Other outcomes will be quality of life, mindfulness, self-compassion and the use of healthcare services. There will be a 6-month follow-up assessment. Intention-to-treat analysis will be conducted using linear mixed models. Per-protocol and secondary outcome analyses will be performed. A data monitoring committee comprising the trial manager, the ABCT and MBSR teachers and an independent clinical psychologist will monitor for possible negative side effects. Ethics and dissemination Approval was obtained from the Ethics Committee of the General University Hospital of Castellón, Spain. The results will be submitted to peer-reviewed specialised journals, and brief reports will be sent to participants on request. Trial registration number NCT03425487

Abstract:Drug use disorders are increasingly recognized as the main cause of public health issues worldwide. The current analysis aims to provide the most comprehensive, updated estimates of the burden from drug use disorders at global, regional, and national levels during the past three decades. Prevalence, incidence, deaths, and disability-adjusted life-years (DALYs) were estimated from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 by age and sex for drug use disorder as well as its four main categories (opioid use disorders, cocaine use disorders, amphetamine use disorders, and cannabis use disorders) in 204 countries and territories between 1990 and 2019. DisMod-MR 2.1, and Bayesian meta-regression were used to analyze prevalence and incidence, while the Cause of Death Ensemble model (CODEm) was used to estimate death of diseases. Globally, the burden of drug use disorders, as measured by the average annual percentage change (AAPC) of deaths and DALYs, continues to increase. The patterns by regions of DALYs due to drug use disorders varied significantly, and it is mainly in developed countries and concentrated among young people and males. Programs for drug use disorders management should be improved, particularly in opioid use disorders. Governments will face increasing demand for treatment and support services, and effective prevention as well as control strategies are required to reduce the burden from these causes.

Abstract:Abstract Objective The Mental and Substance Use Disorders Prevalence Study (MDPS) builds upon previous epidemiologic studies to provide estimates of prevalence and treatment rates of mental and substance use disorders among adults aged 18–65 in the U.S. The study background and methods are described. Method The MDPS employed novel techniques such as the inclusion of household, prison, homeless and state psychiatric hospital populations, a semi‐structured clinical interview administered by trained clinical interviewers to assess disorders, the assessment of both past year and lifetime schizophrenia spectrum disorder (SSD) using full Diagnostic and Statistical Manual 5 criteria, as well as other mental and substance use disorders, and video‐based interviewing. Population specific and combined sample weights were developed to estimate nationally representative prevalence and treatment rates. Results Data collection was conducted between October 2020 and October 2022 resulting in 5679 clinical interviews. The statistical weighting and analytic plan are described. Weighted response rates and reasons for non‐response are provided for each study population. Conclusions The MDPS successfully developed and employed novel techniques to estimate the prevalence and treatment rates of mental and substance use disorders in both household and non‐household populations, including some of the most impairing disorders such as SSD.
