Abstract:Congenital heart disease (CHD) is a prevalent congenital cardiac malformation, which lacks effective early biological diagnosis and intervention. MicroRNAs, as epigenetic regulators of cardiac development, provide potential biomarkers for the diagnosis and treatment of CHD. However, the mechanisms underlying miRNAs-mediated regulation of cardiac development and CHD malformation remain to be further elucidated. This study aimed to explore the function of microRNA-20b-5p (miR-20b-5p) in cardiac development and CHD pathogenesis.

Although some progress has been made in the research of CHD in recent years, its early biological diagnosis and intervention means are still insufficient. MicroRNA (miRNA), as an epigenetic regulator, plays an important role in heart development and may become a potential biomarker for CHD diagnosis and treatment. However, the specific regulatory mechanism of miRNA in heart development and CHD malformations still needs to be further elucidated. ### Main findings 1. **Expression changes of miR - 20b - 5p**: - During the 12 - day heart differentiation process, the expression of miR - 20b - 5p in hESCs was significantly down - regulated, while it was significantly up - regulated in plasma samples from ASD patients. 2. **miR - 20b - 5p inhibits heart differentiation**: - miR - 20b - 5p inhibits the expression of key cardiac transcription factors (such as GATA4, NKX2.5, TBX5, MYH6 and cTnT) by targeting TET2 and 5hmC, thereby inhibiting heart differentiation derived from hESCs. 3. **The role of TET2 in heart differentiation**: - Knockdown of TET2 significantly inhibits heart differentiation, and the miR - 20b - 5p inhibitor can partially restore this inhibitory effect, further confirming that miR - 20b - 5p regulates heart differentiation by targeting TET2. ### Conclusion In summary, this study provides strong evidence that miR - 20b - 5p plays an inhibitory role in heart differentiation derived from hESCs by targeting TET2, highlighting its importance as a potential biomarker for ASD.
