The moderating role of area level disadvantage in the relation between individual adversity, immune activation and psychopathology

Abstract:Introduction Exposure to early adversity has been associated with the development of psychopathology, and particularly internalizing symptoms in later life. While early adversity has also been associated with an activation of the immune system, it remains unclear whether this mediates the association between adversity and internalizing symptoms. Methods We evaluated 158 young adolescents (mean age 13SD 0.97; 53% females) from the eBRAIN study. We used the Strength and Difficulty Questionnaire (SDQ) to evaluate psychopathology. Inflammatory biomarkers levels (circulating levels of IL-2, IL-6, IL- 8, IFN-gamma, TNF-alpha and CRP) were estimated in plasma samples. Results 57.6% (n=97) of participants reported at least one type of Individual-level adversity, and those with more than one adversity were more likely to have an SDQ internalizing symptom score ≥ 9 (U=1286, p<.001). The number of Individual-level adversity was also positively associated with levels of CRP (B=0.11, 230 SE=0.05, p=0.04) but not with any other inflammatory marker. Interestingly, area-level deprivation did moderate the link between Individual- level adversity and CRP levels (B=-0.32, SE=0.11, p<.01). Specifically, a higher CRP level was only seen in those adolescents exposed to adversity who lived in less deprived areas. Conclusion Systemic inflammation in individuals exposed to early adversity may become more evident in later stages of life. These findings suggest that neighbourhood characteristics could moderate the activation of the immune system, and particularly that adolescents living in more deprived areas already present a state of immune activation that is independent from additional experiences of individual adversity.
