Abstract:Venous thromboembolic disease remains a serious complication following lower extremity total joint arthroplasty. Postoperative thromboembolic complications, which include symptomatic deep vein thrombosis, bleeding, and pulmonary embolus, are recognized causes of patient morbidity, mortality, increased length of stay, and higher healthcare costs. Various thromboprophylaxis guidelines have been recommended to prevent and thereby reduce the incidence of such events. However, despite various studies exploring prophylaxis measures, the incidence of venous thromboembolic events has remained relatively unchanged over the past 10 years. We therefore aimed to: (1) evaluate the current 2011 American Academy of Orthopaedic Surgeons (AAOS) and the 2012 American College of Chest Physicians (ACCP) recommendations concerning venous thromboembolic disease after lower extremity joint arthroplasty; and (2) report on different modalities of prophylaxis, specifically, pharmacological agents, mechanical compression devices, and inferior vena cava filters. Both AAOS and ACCP guidelines recommend a combined approach with mechanical compression devices and pharmacological prophylaxis. A major difference is that the AAOS work group does not recommend specific pharmacological agents, whereas the ACCP guidelines give specific recommendations. Additionally, routine primary thromboprophylaxis with inferior vena cava filters is only recommended when there are contraindications to anticoagulation therapy.
Venous thromboembolic prophylaxis following total hip arthroplasty: a North American perspective

Abstract:Patients undergoing total hip arthroplasty are at risk for venous thromboembolic disease, including deep venous thrombosis, pulmonary embolism, and postphlebitic syndrome. The incidence of these complications has declined as a result of recent advances in surgical procedures, early postoperative patient mobilization, and prophylactic alternatives. This paper reviews the benefits and disadvantages of current mechanical and pharmacologic thromboprophylactic alternatives, as well as the prophylactic effects of different anesthesia techniques and of presurgical autologous blood donation. The pharmacologic agents that are widely used in North America today include aspirin, warfarin, and low-molecular-weight heparins. The low-molecular-weight heparins-enoxaparin and dalteparin-have recently gained increasing acceptance among orthopedic surgeons. The effectiveness and safety of extending prophylaxis beyond the time of hospital discharge are also discussed. It is concluded that the optimal thromboprophylactic regimen is one that is customized, adjusted according to each patient's risk factors and other characteristics.

Abstract:Venous thromboembolism (VTE) is a severe complication that can occur after major orthopedic procedures. As VTE-related morbidity and mortality are a significant concern for both medical professionals and patients, and preventative measures are typically employed. Multiple organizations, including the American College of Chest Physicians (ACCP) and the American Academy of Orthopedic Surgeons (AAOS), have developed guidelines for VTE prophylaxis specifically in patients undergoing joint replacement procedures. However, recently, the International Consensus Meeting (ICM) was convened, which brought together over 600 experts from 68 countries and 135 international societies. These experts, spanning a range of medical disciplines including orthopedic surgery, anesthesia, cardiology, hematology, vascular, and internal medicine, conducted a comprehensive review of the literature using a strict Delphi process to generate practical recommendations for VTE prophylaxis across all types of orthopedic procedures. This review article summarizes some of the recommendations of the ICM.
Venous thromboembolism is rare with a multimodal prophylaxis protocol after total hip arthroplasty

Abstract:We evaluated the safety and efficacy of a multimodal approach for prophylaxis of thromboembolism after total hip arthroplasty, which includes preoperative discontinuation of procoagulant medication; autologous blood donation; hypotensive epidural anesthesia; intravenous administration of heparin during surgery and before femoral preparation; aspiration of intramedullary contents; pneumatic compression; knee-high elastic stockings; and early mobilization and chemoprophylaxis for 4 to 6 weeks (aspirin 83%; warfarin 17%). One thousand nine hundred forty-seven consecutive, nonselected patients (2032 total hip arthroplasties) who received this multimodal prophylaxis were observed prospectively for 3 months. The incidence of asymptomatic deep vein thrombosis assessed by ultrasound in the first 171 patients was 6.4%. The incidence of clinical deep vein thrombosis in the subsequent 1776 patients was 2.5%. Symptomatic pulmonary embolism occurred in 0.6% (12 of 1947; nine in patients receiving aspirin and three in patients receiving Coumadin), none of them fatal. One patient died of a myocardial infarct. This multimodal approach is safe and efficacious and compares favorably with those reported in the literature and with our historic controls. If these preventive measures are strictly observed during the perioperative period, postoperative chemoprophylaxis does not need to be aggressive in the patient without predisposing factors. Our low rate of deep vein thrombosis and pulmonary embolism do not support routine anticoagulation prophylaxis with drugs that increase risk of bleeding. Abstract:Background Human papillomavirus (HPV) is an oncogenic virus causing oropharyngeal cancers and resulting in a favorable outcome after the treatment. The role of HPV in oral cavity squamous cell carcinoma (OSCC) remains ambiguous. Objective This study aimed to examine the effect of HPV infection on disease control among patients with OSCC following radical surgery with radiation-based adjuvant therapy. Patients and Method We prospectively followed 173 patients with advanced OSCC (96% were stage III/IV) who had undergone radical surgery and adjuvant therapy between 2004 and 2006. They were followed between surgery and death or up to 60 months. Surgical specimens were examined using a PCR-based HPV blot test. The primary endpoints were the risk of relapse and the time to relapse; the secondary endpoints were disease-free survival, disease-specific survival, and overall survival. Results The prevalence of HPV-positive OSCC was 22%; HPV-16 (9%) and HPV-18 (7%) were the genotypes most commonly encountered. Solitary HPV-16 infection was a poor predictor of 5-year distant metastases (hazard ratio, 3.4; 95% confidence interval, 1.4–8.0; P = 0.005), disease-free survival (P = 0.037), disease-specific survival (P = 0.006), and overall survival (P = 0.010), whereas HPV-18 infection had no impact on 5-year outcomes. The rate of 5-year distant metastases was significantly higher in the HPV-16 or level IV/V metastasis group compared with both the extracapsular spread or tumor depth ≥11-mm group and patients without risk factors (P<0.001). Conclusions HPV infections in advanced OSCC patients are not uncommon and clinically relevant. Compared with HPV-16-negative advanced OSCC patients, those with a single HPV-16 infection are at higher risk of distant metastases and poor survival despite undergoing radiation-based adjuvant therapy and require a more aggressive adjuvant treatment and a more thorough follow-up.

Abstract:INTRODUCTION The quality of life in patients with heart failure is diminished by symptoms of disease, affected social connections, frequent hospitalisations, side effects caused by medication, and costs of treatment borne by the patient himself. OBJECTIVE The objective of this research was to measure the quality of life of patients with heart failure and assess any potential changes during a six-month period. METHOD This research is a follow-up study, and comprised 56 patients who were treated from September 1998 to August 1999 at the Institute for Cardiovascular Diseases of the Clinical Centre of Serbia. During the first hospitalisation, an initial baseline measurement of quality of life was conducted. The measurement was then repeated after three and six months, during check-ups. The measurement was conducted by using a special questionnaire for patients suffering from heart disorders, the "Minnesota Living with Heart Failure Questionnaire." RESULTS The results showed a considerable improvement in the overall quality of life between the baseline and second measurements (F = 6.263; p = 0.020), as well as considerable improvement in the physical dimension of quality of life between the baseline and first measurements (F = 6.797; p = 0.016) and between the baseline and second measurements (F = 5.351; p = 0.030). CONCLUSION Thanks to the application of the special Minnesota questionnaire, it was possible to measure small but considerable changes in the patients' quality of life.

Abstract:Deep venous thrombosis (DVT) is one of the most common complications after total joint replacement, which is also one of the most concerned problems for clinicians.Domestic research data shows that the incidence of DVT in patients without thrombotic prophylaxis after joint replacement surgery are 20.6％-40.0％.The occurrence mechanism of DVT is explained by the Virchow theory, that is blood stasis, the blood vessel wall damage and blood coagulation state.The diagnostic rate of DVT is not satisfactory.The diagnosis of symptomatic DVT depends mainly on clinical symptoms and auxiliary examination.The diagnosis of asymptomatic DVT is mainly based on the auxiliary examination.The prevention and treatment of DVT after artificial joint replacement is currently mainly concentrated in the aspects of new oral anticoagulant agents, drug prevention method, and time limit.

Abstract:Deep vein thrombosis (DVT) after total hip or knee arthroplasty is relatively common and is associated with significant morbidity and mortality, presenting a large clinical burden. The objective of this study was to investigate the incidence and risk factors for DVT after total joint arthroplasty. This retrospective review was conducted in a single institution. The patients received postoperative DVT chemoprophylaxis (Fraxiparine or Rivaroxaban), followed by venography to check for DVT on the third to ninth postoperative day. The clinical characteristics were summarized and analyzed using SPSS version 17.0. From March 2010 to July 2013, 963 patients were enrolled in the study. DVT chemoprophylaxis was administered to all of the 963 patients. DVT occurred in 173 subjects (17.96%). Univariate analysis showed that age (P<0.001), BMI (P<0.01), cholesterol (P<0.001), high-density lipoprotein (P<0.001), apolipoprotein A (P<0.01), history of tumor (P<0.05), visual analogue scale (VAS) pain score (P<0.001), and operative duration (P<0.05) were significant risk factors for DVT. The incidence of DVT was 17.96% after total joint arthroplasty with prophylactic antithrombotic therapy. Age, BMI, cholesterol, high- density lipoprotein, apolipoprotein A, history of tumor, VAS score, and operative duration were significant risk factors for DVT. Copyright (C) 2017 The Author(s). Abstract:As the population ages and medical care, along with a functional life style, continues to improve the health of this group, the number of joint replacement surgeries will continue to rise each year in the United States. This growing volume carries with it the risk of venous thromboembolism associated with joint replacement surgery. The American College of Chest Physicians and the American Academy of Orthopedic Surgery have provided guidelines for preventing this complication with the use of warfarin, low molecular weight heparins, pentasaccharides, and aspirin. These agents have had variable efficacy and safety preventing postoperative and out-of-hospital venous thromboembolism. New classes of oral agents, which inhibit Factor II or Factor X, have been shown to be an effective and safe class of anticoagulants that do not require monitoring or have food and drug interactions. This paper will review the current data on the new oral anticoagulants in joint replacement surgery.

Abstract:Aims: We investigated the efficacy and safety profile of commonly used venous thromboembolism (VTE) prophylaxis agents following hip and knee arthroplasty. Methods: A systematic search of PubMed, Embase, Cochrane Library, Web of Science, and OrthoSearch was performed. Prophylaxis agents investigated were aspirin (< 325 mg and ≥ 325 mg daily), enoxaparin, dalteparin, fondaparinux, unfractionated heparin, warfarin, rivaroxaban, apixaban, and dabigatran. The primary efficacy outcome of interest was the risk of VTE, whereas the primary safety outcomes of interest were the risk of major bleeding events (MBE) and wound complications (WC). VTE was defined as the confirmed diagnosis of any deep vein thrombosis and/or pulmonary embolism. Network meta-analysis combining direct and indirect evidence was performed. Cluster rank analysis using the surface under cumulative ranking (SUCRA) was applied to compare each intervention group, weighing safety and efficacy outcomes. Results: Of 86 studies eligible studies, cluster rank analysis showed that aspirin < 325 mg daily (SUCRA-VTE 89.3%; SUCRA-MBE 75.3%; SUCRA-WC 71.1%), enoxaparin (SUCRA-VTE 55.7%; SUCRA-MBE 49.8%; SUCRA-WC 45.2%), and dabigatran (SUCRA-VTE 44.9%; SUCRA-MBE 52.0%; SUCRA-WC 41.9%) have an overall satisfactory efficacy and safety profile. Conclusion: We recommend the use of either aspirin < 325 mg daily, enoxaparin, or dabigatran for VTE prophylaxis following hip and knee arthroplasty.

Abstract:ABSTRACT The overall risk of venous thromboembolism (VTE) after hip arthroscopy (HA) is reported to range from 0.2% to 9.5%, but a clear set of recommendations for VTE prophylaxis in HA patients remains scarce. The aim is to survey high-volume hip arthroscopists about their current trends regarding VTE prophylaxis use. A combination of two consensus group methods was used in this study: nominal group technique (NGT) and modified Delphi. A preliminary questionnaire was prepared, and rounds of discussion were completed between NGT members. The final version of the survey was administered to 35 high-volume hip surgeons. Delegates’ mean volume of annual hip arthroscopic surgery was 109. Approximately 22% of their patients are revision HA procedures. A total of 91.4% of delegates use chemoprophylaxis, 28.6% use sequential compression devices and 91.4% believed that chemoprophylaxis is necessary for more prolonged and complex procedures (strong consensus). Aspirin was the choice for all participants, and the duration was 2–3 weeks (31.4%), 1 month (65.7%) and 2–3 months (2.9%). History of VTE, hypercoagulable status, and malignancy were considered risk factors. No consensus was achieved for the discontinuation of oral contraceptive and smoking preoperatively. However, the optimal length of VTE prophylaxis is unclear. A total of 97.1% of the experts responded that they administer aspirin between 2 and 4 weeks. High-volume arthroscopic surgeons do consider VTE prophylaxis to be important and warranted in the postoperative setting. Aspirin is the mainstay of chemoprophylaxis, although the appropriate duration is unknown.

Abstract:Venous thromboembolism (VTE) is a notable contributor to the morbidity and mortality of patients with orthopaedic trauma. Several associations have published guidelines on VTE prophylaxis, with a strong predilection toward low-molecular-weight heparin for chemoprophylaxis. However, previous recommendations may be revisited because recent high-level evidence demonstrated aspirin to be noninferior to low-molecular-weight heparin in preventing serious complications of VTE. Direct oral anticoagulants are emerging as safe alternatives to injectable medication and may offer improved patient compliance. There is no consensus on postinjury duration of chemoprophylaxis as an outpatient. Mechanical prophylaxis remains an important adjunctive VTE preventive measure for inpatients with pelvis and extremity fractures. Proteomics and novel laboratory measurements may be able to predict high-risk patients and detect early thrombus formation. Despite high-quality prospective research over the past decade, we still have much to learn about patient- and injury-specific risk factors.

Abstract:The aim of the present study was to investigate the perioperative and postoperative incidence of deep vein thrombosis (DVT) and validate the effectiveness of our own preventive treatment protocol for venous thromboembolism (VTE) occurrence in lower extremity arthroplasty patients. The subjects were 1,054 patients (mean age: 74.3 years) who underwent total hip arthroplasty (THA) or total knee arthroplasty (TKA) at our institutions between April 2014 and March 2017. We examined the frequencies of pre- and post-operative DVT by lower extremity Doppler images, and the incidence rate at proximal or distal regions as well as that according to preoperative DVT status were evaluated. Preoperative DVT was detected in 6.5% (69 cases) of our cohort and those were located 1.4% (15 cases) at proximal and 5.1% (54 cases) at distal regions. A significantly higher rate of postoperative DVT development was observed in preoperative DVT+ THA patients ( P = 0.0075), but not in TKA patients only with a higher tendency ( P = 0.56). The overall incidence of DVT up to 2 weeks after surgeries was 27.3% (288 cases); however, the rate in proximal femur regions was suppressed to 2.8% (30 cases), and there was no symptomatic pulmonary thromboembolism (PTE) case. The results demonstrated the importance of regular Doppler examination for early detection of postoperative DVT occurrence and the following immediate treatment initiation. Our own VTE preventive treatment protocol could reduce the development of proximal DVT, and the periodic monitoring as well as prompt treatment might prevent the fatal PTE. osteoarthritis (OA), rheumatoid arthritis (RA).

Abstract:KEYWORDS: Venous thromboembolism (VTE) following primary total knee arthroplasty (TKA) is multifactorial [ Citation 1 , Citation 2 ]. TKA is a commonly performed, appealing broad research; nevertheless, evidence-based international consensus and high-level recommendations on the gold standard prophylaxis of VTE are lacking. Without prevention, the prevalence of VTE after primary TKA varies from 41 to 85% [ Citation 3 ]. 60% and 14% of patients develop, respectively, asymptomatic and symptomatic VTE after lower extremities orthopedic surgery within two weeks [ Citation 4 ]. Despite a four to six weeks prophylaxis is recommended in every patient following primary TKA, VTE still occur [ Citation 5 ]. Despite prophylaxis, a fourth of patients who have undergone TKA report evidence of VTE, and 2% to 4% of the evidence symptoms of deep vein thrombosis (DVT) or pulmonary embolism (PE) [ Citation 6 , Citation 7 ]. Considering the high number of TKAs performed worldwide, refining indications and protocols of VTE prophylaxis is necessary. The International Consensus Meeting on Venous Thromboembolism (ICM-VTE) offers key recommendations based on extensive evidence and expert opinions [ Citation 5 ]. Anticoagulants or aspirin are recommended, and the choice depends on patient-specific factors, adherence and risk stratification [ Citation 5 ]. Compression devices, early mobilization, monitoring, education, and proper prophylaxis timing are crucial to balance the prevention of VTE with the risk of bleeding [ Citation 5 ].

Multimodal thromboprophylaxis following primary hip arthroplasty: the role of adjuvant intermittent pneumatic calf compression

Abstract:We report a retrospective review of the incidence of venous thromboembolism in 463 consecutive patients who underwent primary total hip arthroplasty (487 procedures). Treatment included both total hip replacement and hip resurfacing, and the patients were managed without anticoagulants. The thromboprophylaxis regimen included an antiplatelet agent, generally aspirin, hypotensive epidural anaesthesia, elastic compression stockings and early mobilisation. In 258 of these procedures (244 patients) performed in 2005 (cohort A) mechanical compression devices were not used, whereas in 229 (219 patients) performed during 2006 (cohort B) bilateral intermittent pneumatic calf compression was used. All operations were performed through a posterior mini-incision approach. Patients who required anticoagulation for pre-existing medical problems and those undergoing revision arthroplasty were excluded. Doppler ultrasonographic screening for deep-vein thrombosis was performed in all patients between the fourth and sixth post-operative days. All patients were reviewed at a follow-up clinic six to ten weeks after the operation. In addition, response to a questionnaire was obtained at the end of 12 weeks post-operatively. No symptomatic calf or above-knee deep-vein thrombosis or pulmonary embolism occurred. In 25 patients in cohort A (10.2%) and in ten patients in cohort B (4.6%) asymptomatic calf deep-vein thromboses were detected ultrasonographically. This difference was statistically significant (p = 0.03). The regimen followed by cohort B offers the prospect of a low incidence of venous thromboembolism without subjecting patients to the higher risk of bleeding associated with anticoagulant use.

Abstract:Most database query optimizers use cost models to identify good query execution plans. Inaccuracies in the cost models can cause query optimizers to select poor plans. In this paper, we consider the problem of accurately estimating the I/O costs of database access methods, such as index scans. We present some experimental results which show that existing analytical I/O cost models can be very inaccurate. We also present a simple analysis which shows that larger cost estimation errors can cause the query optimizer to make larger mistakes in plan selection. We propose the use of an adaptive black-box statistical cost estimation methodology to achieve better estimates.

Efficacy and safety of aspirin in prevention of venous thromboembolism after total joint arthroplasty

Abstract:Objective: To study the efficacy and safety of aspirin in prophylaxis of venous thromboembolism (VTE) after total joint arthroplasty. Methods: 240 patients who received total joint arthroplasty, 157 undergoing total knee arthroplasty (TKA) and 83 undergoing total hip arthroplasty (THR), were divided into 2 basically matched groups, Group A (n = 100), receiving aspirin enteric-coated tablets 100 mg/day since the first day after operation till discharge 10 ∼ 14 days after operation, and Group B (n = 140), receiving subcutaneous injection of low molecular weight heparin (LMWH) once daily for 10 d. The effects and safety were evaluated. Results: 13 patients (13.0%) were diagnosed with deep venous thrombosis (DVT) in Group A, 7 of them were symptomatic, 3 were suspected of pulmonary embolism, and 2 suffered from cardio-cerebrovascular event. In Group B, 10 patients (7.1%) had DVT, 4 of them were symptomatic; suspicious pulmonary embolism and cardio-cerebrovascular event were diagnosed in 3 and 8 patients respectively. There were no statistic significances between the two groups in all these aspects. The quantity of bleeding, decrease of hemoglobin, hematoma rate, and infection rate of Group A were (693.4 ± 480.1) ml, (32.9 ± 18.0) g/L,1%, and 0% respectively, all not significantly different from those of Group B (648.9 ± 521.1) ml, (36.4 ± 21.9) g/L, 2.1%, and 1.4% respectively, all P > 0.05. Conclusion: Aspirin is as effective as LMWH in venous thromboembolism prophylaxis after total joint arthroplasty. In addition, aspirin is cheap, administered orally, well tolerated, without necessity for surveillance, and with good compliance and potential of prevention of cardio-cerebrovascular events.
