Abstract:Objective: To evaluate the application of bar code scanning system for medicine dispensing in the outpatients of our hospital.Method: The main compositions,working process and application of the bar code scanning system for medicine dispensing was introduced.The effects of the system on the medicine dispensing error rate,window residence time of patients and the stress of pharmacists were investigated.Result: The medicine dispensing error rate was decreased.The window residence time of patients was shortened.The patient medicine education was strengthened,and the stress of pharmacists was relieved.Conclusion: Bar code scanning system can obviously decrease the medicine dispensing error rate,shorten the window residence time of patients and improve the quality of pharmaceutical care in outpatient pharmacy.The practicality of the system is promising which obtains widespread praise from pharmacists and patients.
Medication Administration Errors Made by Nurses Reflect the Level of Pharmacy Administration and Hospital Information Infrastructure.

Abstract:We read with great interest Kim's paper which highlights that medication administration guidelines are not always strictly followed by clinical nurses (Kim & Bates 2013). We are from a 2200-bed Joint Commission International (JCI)-accredited academic medical centre hospital in China. We agree with Kim's view that education, performance tracking, barcoding and electronic medication administration records help to reduce medication administration errors (MAEs). These are our perspectives on this issue. The depth, breadth and efficiency of cooperation between nurses, pharmacists and information engineers are pivotal to medication safety (Zhu et al. 2012). Appropriately labelled information in combination with a barcoding system has greatly improved medication administration safety in our hospital. Where appropriate, nurses must perform bedside barcode scanning prior to medication administration and immediately after the completion of infusion. Pharmacists can electronically trace whether infusion time is in accordance with the physicians prescription. Wrong time and drug omissions are common types of MAEs (Keers et al. 2013); however, our experience shows such MAEs can be prevented by automatic warnings and the presence of an electronic tracing system. A double-check policy for administering controlled medications is well implemented using personal digital assistant. Inpatient pharmacy services adhere to unit dose dispensing procedures and ensure each medication dispensed have a label containing barcode, patient name, identification number, drug information (name, dose, route, frequency and time) and warnings (drip rate, stability, infusion set type, light protection, signs of high-alert medications or medications that increase fall risk). We have an online query system of oral solid medication appearance (colour, size and shape) that provides a convenient way for nurses to check medication or identify medications temporarily discontinued by physician. We achieved zero occurrence of MAEs related with oral medications in 2012. In our hospital, regular multidisciplinary team meetings are regularly held in to ensure the medication system works optimally. These operation procedures were revised and updated during the journey to JCI accreditation. For example, pharmacists found that MAEs related with pipeline management were easily neglected by nurses. Therefore, a special protocol was drafted to address the issue; adherence to the guidelines was assessed using tracing methodology. We agree with the finding of Kim's study and believe it is vitally important to find solutions for MAEs, as opposed to blaming nurses who make these MAEs. Severity and frequency of MAEs reflect the level of pharmacy administration and hospital information infrastructure. In the clinical environment, teamwork and efforts to ensure high levels of quality assurance will help reduce the number of these errors. Abstract:Purpose To evaluate the effect of bedside barcode medication administration (BCMA) on specific tasks performed by nurses during medication administration in an intensive care unit (ICU). Methods A prospective observational study was conducted on paper-based medication administration (PBMA) and BCMA in an ICU. A prevalidated time and motion observation instrument was used to measure time spent by nurses on tasks associated with PBMA and BCMA. Frequency of each task performed and mean duration of time dedicated to each task were obtained using descriptive statistics. Analysis of covariance was conducted to assess the difference in time spent with various nursing tasks between 2 periods. Factors considered as covariates were age, gender, body weight, length of stay of the patients in the ICU, number of drugs administered, and number of nurses involved. Results A total of 101 PBMA and 151 BCMA were analyzed. The mean duration of total medication administration time was found to be significantly higher during BCMA(377.8 ± 232.8 s) than during PBMA(313.3 ± 224.3 s). During BCMA, nurses spent more time on the following medication administration tasks: drug preparation(159.4 vs 27.3 s; P < .05), assisting physician(5.2 vs. 0.4 s; P < .05), universal precautions(3.6 vs 0.0 s; P < .05), and conversing with patients(21.6 vs 15.0 s; P < .05). During BCMA, nurses spent less time on the following medication administration tasks: giving medications(24.7 vs 35.0 s; P < .05), documenting medications(11.3 vs 17.3 s; P < .05), and cross-checking drugs with orders(7.4 vs 7.6 s; P =.04). Conclusion During PBMA, nurses spent more time on documenting medications and less time conversing with patients. This changed significantly after implementation of BCMA. BCMA resulted in nurses spending less time on documentation and more time conversing with patients during medication administration process.

Abstract:Increasing numbers of hospitals are implementing bar-coding systems to prevent errors in patient identification. In the present case, a diabetic patient admitted to a teaching hospital was mistakenly given the bar-coded identification wristband of another patient who was admitted at the same time. When a laboratory result that documented the diabetic patient's severe hyperglycemia was entered into the other patient's electronic medical record, the latter patient seemed to have a very high glucose level and was almost given what could have been a fatal dose of insulin. This near miss shows that computer systems, although having the potential to improve safety, may create new kinds of errors if not accompanied by well-designed, well-implemented cross-check processes and a culture of safety. Moreover, computer systems may have the pernicious effect of weakening human vigilance, removing an important safety protection. Researchers should continue to study real-world implementation of computerized systems to understand their benefits and potential harms, and administrators and providers should seek ways to anticipate these harms and mitigate them.

Abstract:Objective: This study aims to investigate how barcode-assisted medication administration systems (BCMAs) can affect medication administration errors (MAEs) in non-unit-dose dispensing settings, since unit dose dispensing system can be a confounding variable affecting MAE rates. Materials and Methods: We conducted a systematic review of articles on MEDLINE, EMBASE, EMCARE, CINAHL and Scopus. Studies are meticulously examined to exclude those with unit-dose dispensing setting. Results: We included 4 papers in the review. The categories of MAEs reported among these studies are heterogeneous. 2 studies give weak evidence and 1 study give moderate evidence that BCMA can lower some categories of MAEs. 1 studies gives weak evidence that BCMA increase the wrong administration time error. Discussion: Studies provide weak to moderate evidence that barcode-assisted medication administration can lower certain categories of medication administration errors. However, some reported findings are minimal. Conclusion: More multi-ward multi-hospital studies need to be conducted to provide stronger evidence on BCMA's impact on MAEs, especially on dosage-related MAEs, in settings without unit dose dispensing systems.

Abstract:Paramount to patient safety is the ability for nurses to make clinical decisions free from human error. Yet, the dynamic clinical environment in which nurses work is characterized by uncertainty, urgency, and high consequence, necessitating that nurses make quick and critical decisions. The aim of this study was to examine the influence of human and environmental factors on the decision to administer among new graduate nurses in response to alert generation during bar code–assisted medication administration. The design for this study was a descriptive, longitudinal, observational cohort design using EHR audit log and administrative data. The study was set at a large, urban medical center in the United States and included 132 new graduate nurses who worked on adult, inpatient units. Research variables included human and environmental factors. Data analysis included descriptive and inferential analyses. This study found that participants continued with administration of a medication in 90.75% of alert encounters. When considering the response to an alert, residency cohort, alert category, and previous exposure variables were associated with the decision to proceed with administration. It is important to continue to study factors that influence nurses' decision-making, particularly during the process of medication administration, to improve patient safety and outcomes.

Abstract:Medicine serialization seems to be a key to both counterfeiting and the complex supply chain of the pharmaceutical industry, including recalls and returns. Though there are countries that use serialization as a legal necessity, it is still a developing topic within the industry. Serialization is not just for anti-counterfeit technology but also to use supply chain and stock management more proficiently. The design of the Barcode is a fundamental requirement as per Healthcare Distribution Alliance (HDA) guidelines for serialized products. Encoding Barcodes on pharmaceutical drugs with critical product attributes is necessary for tracking and validating product sources. Since the serialization guidelines are regulated, small-scale pharmaceutical companies are struggling to comply with regulations due to heavy investments. Printing Serialized labels with barcodes required specialized third-party systems and software. The cost of third-party systems that integrate with ERP and Serialized systems for exchanging data is very expensive. On the other hand, the operational and sustainability costs of these systems will be an additional year-to-year cost for the company. Aim: This Study investigated the feasibility of designing a barcode printing solution for drug traceability. Material and Method: A case study has been carried out at a USA-based generic pharmaceutical company to print serialized labels using their enterprise Resource planning (ERP) integrated with the packaging line. Result: This dynamic solution for serialized label printing saves the company a significant number of resources and money, as they do not need to buy any specialized third-party system to print the label or send back the product to the packaging line. Conclusion: In this paper, we implement and validate a dynamic solution to print 2D data matrix and 1D Linear barcodes successfully within an ERP system by eliminating the need for a specialized third-party printing system.

Abstract:Medication management is both an economic challenge and a patient safety issue. In German hospitals between 19 per cent and 35 per cent of all failures causing harm to patients are attributable to medication errors. As a consequence, an estimated 15,000 patients die each year. In addition, about 14 per cent of the average length of stay is assignable to unplanned drug interactions. Independent of patient risks, health impairments and outcome deficits, every non-fatal medication error leads to additional costs totalling €3,000 on average. Nurses, in particular, who typically suffer from work overload, are affected by error-prone medication logistics. Between 26 per cent and 44 per cent of all failures occurring in the medication administration process result from activities in which nurses are substantially involved. International best-in-class hospitals like the Mayo Clinic (Phoenix), Scripps (San Diego), OLVG (Amsterdam) and Guy’s and St. Thomas’ (London) have been using automated dispensing systems (smart cabinets) for medication safety reasons for many years. Moreover, in the USA, automated medication dispensing cabinets are ubiquitous in various types of hospitals and have an implementation rate of 89 per cent. But in German hospitals, this technology is still nowhere to be seen. In 2020, the German government, via legal act, decided that all hospitals must implement a closed-loop medication administration system in combination with a unit dose drug delivery by January 2025 in order to minimise medication errors. Otherwise, a penalty of 2 per cent of the total hospital revenue would have to be paid. Derived from best-practice reports in the literature, this paper explores the benefits of smart cabinets in terms of economic efficiency, patient safety, reduced work burden and employee acceptance. Furthermore, the reasons why many German hospital decision makers refuse to implement smart cabinets are presented and analysed. Finally, based on all this information, a generic medication administration process has been developed to provide a blueprint for a successful implementation of smart cabinets as an electronically based backbone in a closed-loop medication administration system.

Abstract:Engineered macromolecules offer compelling means for the therapy of conventionally undruggable interactions in human disease. However, their efficacy is limited by barriers to tissue and intracellular delivery. Inspired by recent advances in molecular barcoding and evolution, we developed BarcodeBabel, a generalized method for the design of libraries of peptide barcodes suitable for high-throughput mass spectrometry proteomics. Combined with PeptideBabel, a Monte Carlo sampling algorithm for the design of peptides with evolvable physicochemical properties and sequence complexity, we developed a barcoded library of cell penetrating peptides (CPPs) with distinct physicochemical features. Using quantitative targeted mass spectrometry, we identified CPPS with improved nuclear and cytoplasmic delivery exceeding hundreds of millions of molecules per human cell while maintaining minimal membrane disruption and negligible toxicity in vitro. These studies provide a proof of concept for peptide barcoding as a homogeneous high-throughput method for macromolecular screening and delivery. Abstract:Abstract Introduction Patient misunderstanding of instructions on medication labels is a common cause of medication errors and can result in ineffective treatment. One way to better improve patient comprehension of medication labels is by optimizing the content and display of the information. Objectives To review comparative studies that have evaluated the design of a medication label to improve patient knowledge or safety. Methods Studies were selected from systematic computerized literature searches performed in PubMed, Embase (Elsevier), Cochrane Central (EBSCO), Cumulative Index to Nursing and Allied Health Literature-CINAHL (EBSCO), and Web of Science (Thomson Reuters). Eligible studies included comparative studies that evaluated the design of a medication label to improve patient knowledge or safety. Results Of the 246 articles identified in the primary literature search, 14 studies were selected for data abstraction. Thirteen of these studies significantly impacted the patient understanding of medication labels. Three studies included a measure of patient safety in terms of medication adherence and dosing errors. The utilization of patient-centered language, pictograms/graphics, color/white space, or font optimization was seen to have the most impact on patient comprehension. Conclusion It is essential to present medication information in an optimal manner for patients. This can be done by standardizing the content, display, and format of medication labels to improve understanding and medication usage. Evidence-based design principles can, therefore, be used to facilitate the standardization of the structure of label content for both print and electronic devices. However, more research needs to be done on validating the implications of label content display to measure its impact on patient safety. Implementation challenges of electronic blood transfusion safety systems: Lessons from an international, multi-site comparative case study

Abstract:Background: Severe transfusion reactions resulting from errors in matching the correct blood with the correct patient are considered never events. Despite the relative technical simplicity of barcode scanning for patient-blood bag matching, the adoption and universal application of this safety measure are by no means universal. This study highlights the logistical and institutional challenges associated with spreading, scaling up, and sustaining such IT-supported safety measures in healthcare. Study design and methods: We report findings from a 5-year, prospective, multi-site case study conducted across one hospital in England and three hospitals in the Netherlands. Ethnographic methods, including interviews and observations, were used at each site to investigate the implementation of barcode scanning-supported safety pathways for blood transfusions. Results: Significant variation was observed across the sites in the adoption and implementation of barcode scanning-supported safety pathways. Despite the potential for reducing transfusion errors, the introduction of this innovation was met with varying levels of success in different settings. Discussion: This study highlights the critical role of inter-hospital learning and flexible system design in successfully implementing barcode scanning-supported safety pathways for blood transfusions. A more structured, national-level network for knowledge sharing could enhance the spread and sustainability of such innovations across healthcare settings.
