Abstract:We construct a household price index based on household-level scanner data (homescan data). Instead of the conventional Paasche type price index, in this paper, we apply the Geary-Khamis (GK) method that is commonly used to measure the values of different currencies in international trade. The mathematical properties of the GK price index help identify the complex factors behind household-level price differentials. We ?d that households that buy cereals, manufactured foods, and daily products at weekend bargain rates, reduce their price levels signi?antly. Further, households with children make purchases at lower prices than households without children through frequent bulk purchases at bargain sales. Certain types of shopping behaviors of the elderly lead them to make purchases at higher prices than the young. Speci?ally, shopping at specialized stores, not shopping at home improvement stores, spending less on Sundays, and purchasing the same goods repeatedly increase the price levels for the elderly.

Abstract:Objective To compare the effects ofbromocriptine (BCT) combined with clomiphene citrate (CC) on the outcome of ovulation-induction in patients with polycystic ovary syndrome (PCOS).Methods A prospective,randomized,controlled clinical trial was performed on 100 PCOS patients with infertility.Patients were randomly divided into two groups,control group received 50 mg CC from day 3 to day 7,test group (CC+ BCT) was given 50 mg of CC from day 3 to day 7 along with 2.5 mg of BCT daily for full cycle.Both the groups were treated for one cycle.Results The outcomes were measured by the hormonal status,follicular size,endometrial thickness,ovulation rate,and pregnancy outcomes.The baseline level of hormones between the two groups was not different before treatment.After intervention,the rate of ovulation in CC group was 72.0％ and in CC+ BCT group was 75.4％ (P＞0.05),the rate of ongoing pregnancy in CC+ BCT group (18.4％) was higher than that in CC group (8.0％).On hCG injection day,there were no statistically significant differences between two groups regarding to levels of follicle-stimulating hormone (FSH),estradiol (E2) and progesterone (P),but prolactin (PRL) and testosterone (T) was significantly decreased (respectively P=0.000,P=0.004),and LH level was also decreased but with no statistical significance (P=0.094).On hCG injection day 7,levels of P and E2 were not different between the two groups (respectively P=0.901,P=0.192),but PRL level was decreased (P=0.000) and the endometrial thickness was increased significantly (P=0.017).Conclusion Our results showed that BCT plus CC can improve the outcome of ovulation-induction of PCOS patients,reducing levels ofPRL,LH and T,and increasing the endometrial thickness of embryo implantation time.It can be concluded that use of BCT with CC improve the outcome of ovulation-induction more than use of CC alone for infertile PCOS patients,which may be brought about through dopamine agonists efficacy in reducing LH and PRL levels,vascular resistance and increasing uterine blood supply to the uterus.

Abstract:The aim of the research described herein was to develop and verify an efficient optimization-based aerodynamic / structural design tool for missile fin and configuration shape optimization. The developed software was used to design several missile fin planforms which were tested in the wind tunnel. Specifically, this paper addresses fin planform optimization for minimizing fin hinge moments, as well as aeroelastic design (flexible fin structures) for hinge moment control. The method is also capable of shape optimization of fin-body combinations with geometric constraints. The inclusion of aerodynamic performance, geometric constraints, and structural constraints within the optimization software facilitates multidisciplinary analysis and design. The results of design studies and wind tunnel tests are described.

Abstract:Background and Objectives: Polycystic ovarian syndrome (PCOS) is a widespread endocrine disorder affecting 5-18% of females in their childbearing age. The aim of this study is to assess the efficacy of combining a low dosage of human chorionic gonadotropin (HCG) along with clomiphene citrate (CC) for stimulating ovulation in infertile women diagnosed with CC-resistant PCOS. Materials and Methods: A randomized controlled trial was carried out on 300 infertile CC-resistant PCOS women. All participants were assigned to two groups: the CC-HCG group and the CC-Placebo group. Subjects in the CC-HCG group were given CC (150 mg/day for 5 days starting on the 2nd day of the cycle) and HCG (200 IU/day SC starting on the 7th day of the cycle). Subjects in the CC-Placebo group were given CC and a placebo. The number of ovarian follicles > 18 mm, cycle cancellation rate, endometrial thickness, ovulation rate, clinical pregnancy rate, and occurrence of early ovarian hyper-stimulation syndrome were all outcome variables in the primary research. Results: Data from 138 individuals in the CC-HCG group and 131 participants in the CC-Placebo group were subjected to final analysis. In comparison to the CC-Placebo group, the cycle cancellation rate in the CC-HCG group was considerably lower. The CC-HCG group exhibited a substantial increase in ovarian follicles reaching > 18 mm, endometrial thickness, and ovulation rate. The clinical pregnancy rate was higher in the CC-HCG group (7.2% vs. 2.3%; CC-HCG vs. CC-Placebo). Upon adjusting for BMI and age, the findings of our study revealed that individuals in the CC-HCG group who had serum prolactin levels below 20 (ng/mL), secondary infertility, infertility duration less than 4 years, baseline LH/FSH ratios below 1.5, and serum AMH levels more than 4 (ng/mL) had a higher likelihood of achieving pregnancy. In the CC-Placebo group, there was a greater prediction of clinical pregnancy for those with serum AMH (<4), primary infertility, serum prolactin ≤ 20 (ng/mL), baseline LH/FSH < 1.5, and infertility duration < 4 years. Conclusions: The use of a small dose of HCG along with CC appeared to be an effective treatment in reducing cycle cancelation, improving the clinical pregnancy rate and ovulation rate in CC-resistant PCOS patients. Extended Letrozole Regimen for Treatment of Clomiphene Resistant Polycystic Ovary Syndrome Patients

Abstract:Background: Polycystic Ovary Syndrome (PCOS) is one of the most common causes of an-ovulatory, 4-7% of women, The aim of the study is to compare the extended letrozole regimen with the classical one in treatment of CC resistant PCO patients, Subjects and Methods: 60 infertile, PCO and CC resistant women were divided randomly into 2 groups, group I, (short letrozole therapy) received 2.5mg twice daily from 2nd day of cycle for 5 days and repeated for 3 cycles (classical regimen)and group II, (long letrozole therapy) treated by 2.5mg once daily from 2nd day of cycle for 10 days and repeated for 3 cycles (extended regimen). Follicular growth and endometrial thickness monitoring, in all patients was performed regulary by trans-vaginal ultrasound. On day 10 of the cycles. HCG injection (5,000 IU IM) was given when at least one follicle measured ≥ 18mm followed by timed intercourse. Results: ovulation after treatment among group I (56.7%) compared to group II (63.3%) (P=0.598). Total number of follicles during stimulation was significantly greater in group I (6.51 ±0.78) vs. (4.2 ±0.81) in group II; (P=0.001). The mean ± SD of endometrial thickness at HCG injection among group I and group II was 8.4±1.76 mm and 8.83±1.45 mm respectively without significant difference between them (P=0.076). Pregnancy rate in group I was (13.3%) while (26.7%) in group II showing significant statistical difference. (P = 0.017). Conclusion: Treatment with long letrozole protocol can produce more mature follicles and subsequently increased pregnancy rate than the short letrozole therapy

Abstract:Abstract Background Polycystic ovary syndrome (PCOS) is considered one of the most common endocrine disorders affecting females in their reproductive age. In PCOS, an imbalance between androgens, anti-Müllerian hormone (AMH) and follicle stimulating hormone (FSH), cause a halt of follicular growth. Insulin resistance (IR) is another important component of PCOS. There is increasing evidence that vitamin D affects insulin and glucose metabolism, Vitamin D plays a vital role in the regulation of glucose-insulin homeostasis. Objectives to assess the efficacy of vitamin D supplementation in improving ovulation induction in PCOS women (1ry outcome) and increasing pregnancy rate (secondary outcome). Patients and Methods 120 women diagnosed PCOS according to Rotterdam criteria and were resistant to CC treatment were randomly allocated into equal two groups: Group V: number of 60 patients received cholecalciferol + Metformin for 2 months. Group C: 60 patients received Metformin 2 months. After two months from the start:Both study groups started induction with Clomiphene Citrate added every month starting from the 3rd month to the 5th month (3 cycles of ovulation induction) in addition to previous drugs mentioned. Results In our study results, regulation of menstrual cycle occured in 63.6% of women who recieved vitamin D + metformin (group V) and ovulation was confirmed in 34.5% of the total number (55 women). Later, 18% of them got pregnant after 3 months of treatment. Added to that 16 women out of 39 (41%) got pregnant by the end of the follow up period with cumulative pregnancy rate (53%). On the other hand menstrual regularity in (group C: metformin) recorded 61% with ovulation percentage of 26.9% (n = 54). Followed by 14.8% pregnancy rate at the 3rd month., which increased to 27.5% by the end of the study out of 47.5% ovulated women (n = 40). Cumulative pregnancy rate 40%. Conclusion From the current study we can conclude that there is no significant difference of adding vitamin D to CC resistant PCOS regarding ovulation and pregnancy rates. However, it can augment metformin action and improves its outcome.

Abstract:To evaluate the effectiveness of Letrozole and Clomiphene (CC) combination versus Letrozole alone for ovulation induction in infertile women with PCOS. This was an open label randomized controlled trial conducted between September 2020 and March 2023 at a tertiary level hospital. Women with a diagnosis of infertility and PCOS were included. Participants were randomized in a 1:1 ratio, to either the combination of 2.5 mg letrozole and 50 mg Clomiphene Citrate (CC) daily or 2.5 mg letrozole alone from 2nd to 6 th day of menstrual cycle for up to three treatment cycles. The primary outcome was ovulation rate per cycle. The secondary outcomes included ovulation rate per woman randomised, cumulative pregnancy and live birth rates. A total of the 120 participants, 59 women in Letrozole and CC arm (intervention) and 61 women in the Letrozole alone arm (control) were recruited. The woman per cycle ovulation rate following Letrozole and CC combination versus Letrozole alone (71/92 (77.2%) vs. 52/83 (62.6%), RR 1.43, 95%CI 0.99 to 2.06) but was not statistically significant. A per ITT analysis, the clinical pregnancy rate per woman randomized (16/61(26.2%) vs. 13/59(22%), RR 1.12, 95%CI 0.76 to 1.64) and live birth rate per woman randomized (10/61(16.4%) vs 11/59(18.6%), RR 0.92, 095%CI 0.57 to 1.50) did not differ significantly between Letrozole and CC versus Letrozole group. There was no significant improvement in ovulation, clinical pregnancy, or live birth rates with a combination of letrozole and CC versus letrozole alone in per woman randomized with PCOS.

Changes in brain catecholamine metabolism during bromocriptine treatment in polycystic ovary syndrome

Abstract:The role of dopaminomimetic drugs on the brain catecholamine metabolism in the neuroendocrine regulation of the polycystic ovary syndrome (PCO) was investigated. We measured, besides peptide hormones and sex steroids, urinary dopamine (DA), norepinephrine, epinephrine, vanillylmandelic acid, homovanillic acid (HVA), 3,4-dihydroxyphenylacetic acid (DOPAC) and total 3-methoxy-4-hydroxyphenylglycol (MHPG) levels by high-performance liquid chromatography with electrochemical detector in 10 women with PCO before and during long-term bromocriptine (BRC) administration. HVA and DOPAC concentrations were significantly lower (p < 0.001) in PCO patients compared with 12 control subjects in the early follicular phase, whereas MHPG concentrations were significantly higher (p < 0.01) in PCO patients. During BRC administration, HVA, DOPAC and MHPG levels increased significantly (p < 0.01 for HVA and DOPAC, and p < 0.05) for MHPG), prolactin levels dropped markedly (p < 0.01), whereas luteinizing hormone levels did not change (p = NS). These data show (1) a reduced DA activity in PCO which may be normalizable under BRC treatment, but also (2) no major effects of DA metabolism on the inappropriate gonadotropin secretion of the syndrome.

Abstract:The objective of this study is to evaluate the efficacy of the clomiphene stair-step protocol to induce ovulation in women with polycystic ovarian syndrome (PCOS) compared to traditional protocol. This single center randomized controlled trial was undertaken. A 140-patients who met all of the inclusion criteria were divided into two main groups and induction of ovulation for both protocols was performed. Follow up of follicular maturation is done by transvaginal ultrasound. The time to ovulation with the stair step protocol was 21-28 days as compared with the traditional protocol which was 42-70 days. The dose dependent ovulation rate was 43% at 100 mg with the stair step protocol compared with 25.3% with the traditional regimen and the ovulation rate at 150 mg was 21.6% with the stair step protocol compared with 14.7% with the traditional protocol while the clomiphene citrate resistance rate was higher with the traditional protocol 38.7% as compared to the stair step method which was 20% which are statistically significant. This randomized controlled trial suggests that clomiphene stair step protocol decreases the time to ovulation and may improve ovulation rates in clomiphene-resistant women.

Abstract:Objective To investigate the clinical efficacy of individualized letrozole therapy for polycystic ovary syn -drome ( PCOS) patients resistant to clomiphene .Methods A total of 52 PCOS patients resistant to clomiphene were en-rolled in the current study .They were administrated with individualized letrozole therapy , while their follicle growth was strictly monitored.Then, the following information was recorded to analyze clinical efficacy:the weekly dosage of letro-zole, the number of dominant follicles , hCG injection date , endometrial thickness , and the rates of ovulation and preg-nancy.Results The individualized letrozole therapy was applied for 102 cycles.The average dosage of letrozole was (42.73 ±13.73) mg [(17.09 ±5.49) tablets].The rates of ovulation and pregnancy were 91.17% and 65.38%, respectively .No ovarian hyperstimulation syndrome was reported .Conclusion Individualized letrozole therapy can ef-fectively improve the rates of ovulation and pregnancy for PCOS patients resistant to clomiphene under strictly monitoring conditions.
Clomiphene citrate and dexamethazone in treatment of clomiphene citrate-resistant polycystic ovary syndrome: a prospective placebo-controlled study

Abstract:Background: The aim of this work was to evaluate the efficacy of adding dexamethazone (DEX) (high dose, short course) to clomiphene citrate (CC) in CC-resistant polycystic ovary syndrome (PCOS) with normal dehydroepiandrosterone sulphate (DHEAS) in induction of ovulation. Methods: Eighty infertile women with CC-resistant PCOS were randomly assigned into two groups. Group I: Clomiphene citrate 100 mg/day was given from day 3 to day 7 of the cycle and DEX 2 mg/day from day 3 to day 12 of the cycle. Group II: Same protocol of CC combined with placebo (folic acid tablets) was given from day 3 to day 12 of the cycle. The main outcome was ovulation. Secondary measures included number of follicles >18 mm endometrial thickness and pregnancy rate. Ovarian follicular response was monitored by transvaginal ultrasound. HCG 10,000 U was given when at least one follicle measured 18 mm, and timed intercourse was advised. Results: There were no statistically significant differences between groups as regards age, duration of infertility, BMI, waist-hip ratio (WHR), menstrual pattern, hirsutism, serum DHEAS or day of HCG administration. The mean number of follicles>18 mm at the time of HCG administration and the mean endometrial thickness were significantly higher in the DEX group than in the placebo group (P<0.05). Similarly, there were significantly higher rates of ovulation (75 versus 15%) (P<0.001) and pregnancy (40 versus 5%) (P<0.05) in the DEX group. Dexamethazone was very well tolerated as no patients complained of any side effect. There was a significant difference between the responders and non-responders in the presence of oligomenorrhea, amenorrhea or hirsutism. Conclusion: Induction of ovulation by adding DEX (high dose, short course) to CC in CC-resistant PCOS with normal DHEAS is associated with no adverse anti-estrogenic effect on the endometrium and higher ovulation and pregnancy rates in a significant number of patients. Induction with DEX appears to be independent on age, period of infertility, BMI or WHR.

Abstract:Objective To evaluate the efficacy, safety and feasibility of letrozole ( LTZ) combined with low dose highly purified human menopausal gonadotropin ( Hp-HMG ) in clomiphene citrate ( CC )-resistant infertile women with polycystic ovary syndrome ( PCOS ) . Methods Totally 120 infertile women with PCOS with CC resistance were enrolled for ovulation induction by LTZ combined with low dose Hp-HMG, together with 237 cycles. The primary study endpoints were the clinical pregnancy rate, the ovulation rate and the percentage of monofollicular development. The secondary efficacy endpoints included the number of ovulated follicles, serum level of estradiol and luteinizing hormone ( LH) and endometrial thickness on the day of gonadotropin releasing hormone agonist ( GnRH-a) triggering. The main safety parameters were the incidence of adverse events, including the early abortion, ovarian hyperstimulation syndrome ( OHSS ) , and multiple pregnancy. Results The rate of cycle ovulation was 96. 2% (228/237), the percentage of monofollicular development was 70%(166/237), cycle cancellation rate was 1. 3% (3/237) and anovulation rate was 3. 8% (9/237);237 cycles achieved the periodic single pregnancy rate was 32. 9% (75/228), the early abortion rate was 5. 3% (12/228). Two twin gestation and 1 ectopic pregnancy; In 120 patients, the rate of cumulative clinical pregnancy was 65% (78/120). No severe OHSS, multiple pregnancy, local or systemic side effects were seen. Conclusion Letrozole combined with low dose Hp-HMG maybe an effective and safe choice for reducing hyperstimulation and increasing pregnancy rate for ovulation induction in CC-resistant women with PCOS.

Abstract:Background: Both clomiphene (CC) and letrozole are commonly used oral ovulation induction agents, with different mechanism of action. Apart from letrozole or CC, options for ovulation induction in polycystic ovarian syndrome (PCOS) patients are limited and thus leaving the use of gonadotropin injections as the only option of ovulation induction in these patients. The objective of this study is to evaluate the efficacy of combined therapy of letrozole and CC compared to the use of letrozole alone to achieve ovulation in infertile women with PCOS. Methods: This will be a single-centre, double arm, triple-blind randomized controlled trial. The study was conducted after taking approval from institutional ethics committee and was prospectively registered with the clinical trials registry- India. Women were randomly assigned to receive a combination of 2.5 mg letrozole and placebo daily or a combination of 2.5 mg letrozole and 50 mg CC daily on cycle days 3–7 for one treatment cycle. Conclusions: If combination of clomiphene and letrozole is proved to be more efficacious than letrozole alone, there will be several breaks through advantages in the management of infertility. The novel method of ovulation induction with combined letrozole and clomiphene will bring down overall cost of infertility treatment. Trial registration: The study is prospectively registered with the clinical trials registry- India CTRI/2020/09/028012.

Abstract:To compare the therapeutic efficacy of clomiphene citrate (CC) and letrozole (LE) on ovulation, pregnancy, and live birth in women with polycystic ovary syndrome (PCOS); and to ensure if LE can replace CC as the first-line therapy for ovulation induction in these women. This is a prospectively, randomized, controlled trial in the tertiary hospital. Two-hundred and sixty-eight anovulatory PCOS patients were treated by CC or CC plus metformin and LE or LE plus metformin for three continuous cycles or conception; their ovulation rates, pregnancy rates, and live birth rates were calculated and compared. No significant difference was noted among the four groups regarding to the baseline data of clinical manifestations, serum sex hormone levels, and serum insulin levels. A total of 240 patients completed the therapies. The ovulation rate was significantly higher in the group LE than the group CC; however, no significant difference was noted between the groups LE and CC, CC, and CC + MET, or LE and LE + MET in the pregnancy rate, abortion rate, and live birth rate. No birth defect was found in the total of 63 newborns. CC regimen was still recommended to be the first-line therapy of ovulation induction for PCOS.

Abstract:Background: Oligo-astheno-teratospermia (sperm of low concentration, reduced motility and increased abnormal morphology)of unknown cause is common and the need for treatment is felt by patients and doctors alike. As a result, a variety of empirical, non-specific treatments have been used in an attempt to improve semen characteristics and fertility.Whilst bromocriptine treatment for reducing prolactin levels in hyperprolactinaemic males (as in females), and, in the treatment of hypogonadotropic hypogonadism with hyperprolactinaemia, is beneficial, it has also been used for oligospermic men in the absence of any endocrinopathy. Prolactin may play a direct role in spermatogenesis and hormone production. It has also been claimed that in oligospermic men with normal gonadotrophins mean prolactin levels are higher and that hyperprolactinaemia is more common compared to fertile men. It has been proposed that the administration of bromocriptine under these circumstances might counteract a prolactin-induced block on the action of gonadotrophins on the testicles and, subsequently, that the reduction in prolactin levels might lead to an improvement in semen parameters and fertility. Although it is not licensed for use in male infertility, bromocriptine has been used for normogonadotrophic individuals with oligospermia and normal or slightly elevated prolactin levels. This review considers the available evidence of the effect of bromocriptine therapy for normoprolactinaemic males with idiopathic oligo and/or asthenospermia. Objectives: The objective of this review was to assess the effects of bromocriptine on pregnancy rates among couples where subfertility has been attributed to idiopathic oligo- and/or asthenospermia. Search strategy: The Cochrane Subfertility Review Group specialised register of controlled trials was searched". Selection criteria: Randomised trials of oral bromocriptine versus placebo or no treatment for couples with subfertility attributed to male factor. Data collection and analysis: Data were extracted by one reviewer and any disagreements were resolved by discussion with other reviewers. Main results: Four studies were included. The method of randomisation was not specified in any of the trials, which were all of crossover design. Compared with placebo, bromocriptine was associated with a significant reduction in serum prolactin levels (weighted mean difference -195.3 micro international units per litre, 95% confidence interval -276.5 to -114). No effects on sperm parameters were seen. There was also no effect on pregnancy rates observed between bromocriptine and placebo (0.70 odds ratio, 95% confidence interval 0.15 to 3.24). Authors' conclusions: Bromocriptine appears to reduce prolactin levels in subfertile men with normal gonadotrophic function. There is not enough evidence to show that bromocriptine is helpful in improving fertility.[This abstract was prepared centrally.].
Comparative Study between Stair-step Protocol of Clomiphene Citrate and Combined Clomiphene and Gonadotropins for Induction of Ovulation in Women with Polycystic Ovary Syndrome

Abstract:Background: Polycystic ovary syndrome affects 7 to 8% of women and may be the most common cause of female infertility. Anovulation, early pregnancy loss has all been implicated in the low fecundity. The aim of this study was to compare between the use of clomiphene citrate by stair-step protocol and use of combined clomiphene citrate and gonadotropins in patients with polycystic ovary syndrome. Methods: This randomized non blinded controlled clinical study was carried out on 60 Patients who was selected from outpatient clinic of Tanta university hospitals from September 2019 to September 2020. Selected patients were allocated into two groups with 1:1 ratio. Group (A): was subjected to clomiphene citrate stair-step stimulation protocol. Group (B): was subjected to combined clomiphene and gonadotropins stimulation protocol. Results: Mann-Whitney U test was used to compare quantitative data because it is not normally distributed (Age, BMI and duration of infertility). Significance defined by p < 0.05. Chi-square analysis was used for the categorical variable (number of MGF). Mann-Whitney U test was used to compare quantitative data because it is not normally distributed (Size of larger follicle and Endometrial thickness), Chi-square analysis was used for the categorical variables (Ovulation rate and Pregnancy rate). Mann-Whitney U test was used to compare quantitative data which are not normally distributed (FSH and LH). Student t test was used for the quantitative data which are normally distributed (Prolactin and TSH). Conclusions: In conclusion, the CC stair‐step protocol is a safe, simple option for the management of PCOS patients, having a larger number of mature Grafian follicle, lower endometrial thickness and higher ovulation rate compared to combined clomophine and gonadotrophin protocol.

Abstract:Background: Polycystic ovarian syndrome (PCOS) is the most common endocrine disorder among women in the reproductive period. It is one of the leading causes of infertility. The manifestations of PCOS include irregular or no menstrual periods, excess body weight and facial hair, acne, heavy periods and pelvic pain. The aim of this study is to determine the efficacy of stair step protocol compared to traditional protocol in ovulation induction of polycystic ovarian syndrome (PCOS) patients in terms of increasing rate of ovulation and pregnancy. Methods: This study is non-blinded, multicenter, randomized controlled study and was carried out on infertile women attending the Fertility Clinic of Tanta University Hospital and Hurghada General Hospital. Two Hundred infertile patients with PCOS criteria and no other causes of infertility were enrolled according to inclusion and exclusion criteria. Results: Student t test was used for the continuous variables (FSH, LH, Prolactin, AMH, TSH, Free testosterone). Student t test was used for the continuous variables (Endometrial thickness, Time to ovulate (d)). Chi-square analysis was used for the categorical variables (Ovulation rate, Pregnancy rate). Chi-square analysis was used for the categorical variables (Ovulation rate per cycle). There was no significant difference between two groups as regards mild and severe side effects. Conclusions: This study concluded that stair step regimen improves the ovulation rate and pregnancy rate without any detrimental side effects compared to traditional regimen. It helps to know the sensitivity and resistance of an individual to clomiphene citrate much earlier and helps to plan ahead with alternative treatment for desired outcome. The advantage of shorter treatment period with similar side effects makes the stair step protocol suitable for use in routine clinical practice.

Abstract:OBJECTIVES:To evaluate different oral contraceptive pill (OCP) pretreatment associated differential in-vitro fertilization (IVF)/intracytoplasmic sperm injection (ICSI) outcomes of polycystic ovary syndrome (PCOS) patients and explore enhanced hormonal balance induced by the pretreatment.METHODS:This retrospective study included 500 PCOS women and 565 normal ovulating counterparts undergoing IVF/ICSI. The PCOS patients were divided into three groups based on the OCP pretreatment regimens: non-OCP (without OCP pretreatment), unsuccessive OCP (the period of successive pretreatment ≤2 months) and successive OCP (the period of successive pretreatment ≥3 months) groups. Comprehensive hormonal and ultra-sonographic assessments were performed before/after IVF pretreatment. Confounding factors affecting pregnancy outcomes were analyzed with logistic regression.RESULTS:PCOS patients with significant endocrine disorders had reduced implantation and pregnancy rates and increased miscarriage rate. Successive, not unsuccessive OCP pretreatment, significantly improved the implantation and pregnancy rates, and reduced the incidence of monotocous small-for-gestational age infants, which was accompanied by remarkably decreased hyperandrogenism and antral follicles.CONCLUSION:PCOS is an independent risk factor for poor IVF outcome. Successive, not unsuccessive, OCP cyclical pretreatment could improve pregnancy outcome of PCOS patients, associated with reduction of hyperandrogenism and antral follicle excess.
