Abstract:Importance: Glaucoma is a significant cause of global blindness and there are, as yet, no effective means of screening. Objective: To assess the potential role of frequency-doubling technology (FDT) perimetry in screening for glaucoma using data collected as part of the National Health and Nutrition Examination Survey (NHANES). Design, setting, and participants: Reanalysis of cross-sectional data of 6797 participants in the 2005-2008 cycles of the NHANES, which evaluated a sample of the noninstitutionalized US population with at least light-perception vision. A subset of optic nerve photographs were regraded by 3 glaucoma specialists in December 2012. Each participant underwent visual field testing, including FDT perimetry screening, and had fundus photographs taken. Main outcomes and measures: Sensitivity and specificity of FDT perimetry to detect glaucoma, macular disease, or decreased visual acuity. Results: A total of 5746 NHANES participants had optic images originally graded. We regraded 1201 images of 1073 eyes of 548 participants with initial cup-disc ratio (CDR) of 0.6 or greater and 423 images of 360 eyes of 180 randomly selected participants with initial CDR less than 0.6. Diagnoses of glaucoma by disc photograph were 1.6% (3 of 180) in the CDR less than 0.6 group and 31.4% (172 of 548) in the CDR of 0.6 or greater group. The sensitivity of FDT was 33% (95% CI, 0%-87%) and specificity was 77% (95% CI, 71%-84%). For the group with at least 1 CDR of 0.6 or greater, sensitivity of FDT was 66% (95% CI, 59%-73%) and specificity was 70% (95% CI, 66%-75%). When analyzed to give FDT credit for identifying glaucoma, macular disease, or decreased visual acuity, the sensitivity of the test was 80% (95% CI, 77%-83%) and the specificity was 83% (95% CI, 82%-84%). Approximately 25% of the NHANES population was not able to successfully complete FDT testing, representing screening failures and decreasing specificity. Conclusions and relevance: Using the 2005-2008 waves of the NHANES as a model of population-based screening for eye disease, FDT perimetry lacks both sensitivity and specificity as a means of screening for glaucoma, the presence of retinal disease, or decreased acuity in a population-based setting. Given that no single test of glaucoma has yet been shown to be appropriate in a screening setting, to our knowledge, investigators should consider novel methods of detecting glaucoma or combinations of tests that might work better in a screening setting.

Abstract:PURPOSE. To study the diagnostic effectiveness of an improved software version of the well-known authors' modification of Frequency Doubling Technology (FDT) perimetry by comparing it with the previous software version and standard automated perimetry on the same contingent of healthy individuals and patients with primary open-angle glaucoma (POAG). METHODS. The study included 56 patients (105 eyes) (mean age 61±13.2 years) with POAG. Patients were divided into three groups depending on the stage of glaucoma: group 1 — early stage (39 eyes), group 2 — moderate (25 eyes), and group 3 — advanced (9 eyes) stage. The fourth (control) group included 32 eyes of 16 healthy people (average age 48.2±6.2 years). All study subjects underwent standard automated perimetry (SAP) on the Humphrey III visual field analyzer and non-standard perimetry in the form of our own modification of FDT perimetry and its improved software version — FDT VR version based on the new portable automatic perimeter "Stimulus" (OOO Total Vizhen, Russia). The optic nerve disc was evaluated on the Heidelberg Retina Tomograph 3. RESULTS. The software version of the authors' modification of FDT perimetry has been improved for liquid crystal monitors and adapted to the portable perimeter "Stimulus". When comparing diagnostic effectiveness, both variants of FDT perimetry are not inferior to SAP in sensitivity (87.6; 87.8 and 86.7%, respectively), and in terms of specificity (98.2; 98.1 and 84.4%, respectively) they are superior. A strong (r=0.99988) and reliable (p<0.05) correlation was found between the average values of light sensitivity for both variants of FDT perimetry. CONCLUSION. The introduction of FDT perimetry based on the new portable automatic perimeter "Stimulus" into widespread ophthalmological practice can contribute to improving the effectiveness of screening and early diagnosis of glaucoma.

Abstract:Purpose: To determine whether frequency-doubling technology (FDT) perimetry detects visual field loss in perimetrically normal eyes of patients with open-angle glaucoma (OAG) and whether these visual field defects subsequently are detected by standard automated perimetry (SAP), and to explore the relating factors of the progression from abnormalities based on FDT to visual field loss based on SAP.Design: Prospective cohort study.Participants: Sixty-eight OAG patients with unilateral field loss detected by SAP (Octopus, G2 program; Interzeig, Schlieren, Switzerland).Methods: Perimetrically normal eyes of participants were examined with the FDT N-30 threshold program (Humphrey Instruments, Welch-Allyn, Skaneateles, NY). The visual field examination was followed by a series of SAP examinations administered over 3 years.Main Outcome Measures: The relationship between FDT and subsequent SAP results in perimetrically normal eyes was analyzed. Glaucomatous optic neuropathy (GON), visual field indices, intraocular pressure (IOP), and central corneal thickness (CCT) were compared between converters (eyes with subsequent SAP abnormality) and nonconverters within perimetrically normal eyes with abnormal FDT results. Finally, the SAP test points were matched to the abnormal FDT sectors. The relative risk (RR) of subsequent SAP abnormality corresponding to FDT abnormal sectors was calculated.Results: Sixty perimetrically normal eyes of 60 participants had complete data and a qualifying follow-up. Baseline FDT results were abnormal in 65%. Of the eyes with abnormal FDT results, 51% developed abnormal SAP results after 4 to 27 months, whereas none of the eyes with normal FDT results developed abnormal SAP results (P<0.05). In perimetrically normal eyes with abnormal FDT results, converters had a greater cup-to-disc ratio, more eyes with GON, larger and deeper cups, and worse FDT mean deviation than nonconverters (P<0.05). The IOP and CCT did not differ between the 2 groups. The RR of subsequent SAP abnormality corresponding to abnormal FDT sectors was 5.38 (95% confidence interval, 3.61-8.04; P<0.05).Conclusions: In perimetrically normal eyes of OAG patients, FDT detected visual field loss in almost 2 of every 3 of these eyes and also predicted to some extent future visual field loss on SAP. Frequency-doubling Perimetry in Examination of Open-Angle Glaucoma with Hemifield Defect

Abstract:OBJECTIVE:To determine whether frequency-doubling perimetry (FDP) can find visual field defect of early glaucoma which can not be found by conventional perimetry.METHODS:Forty-nine eyes of 49 patients with open-angle glaucoma and hemifield defect detected by Humphery Field Analyzer (HFA) were tested by both FDP (threshold test C-20) and Heidelberg Retina Tomograph (HRT). For each hemifield, its FDP result was evaluated. According to the FDP results in intact HFA hemifields, the patients were divided into FDP normal group and FDP abnormal group. The HRT parameters of half disk corresponding to the intact HFA hemifields were compared between these two groups.RESULTS:There was a strong correlation between the mean sensitivity of FDP and mean deviation of HFA results (r = 0.58, P < 0.01). In intact HFA hemifields, 41% (20/49) of eyes showed visual loss in FDP tests, while in defective HFA hemifields, only 2% (1/49) of eyes was normal in FDP test. For the HRT parameters of half disk corresponding to the intact HFA hemifields, cup area and cup/disk area ratio were statistically bigger and rim area and rim volume were statistically smaller in FDP abnormal group than in FDP normal group.CONCLUSION:FDP is able to detect early glaucomatous visual field damage which can not be detected by HFA, but can be explained by the optic nerve glaucomatous changes in HRT parameters.
Comparison of Standard Automated Perimetry, Short-Wavelength Automated Perimetry, and Frequency-Doubling Technology Perimetry to Monitor Glaucoma Progression

Abstract:Detection of progression is paramount to the clinical management of glaucoma. Our goal is to compare the performance of standard automated perimetry (SAP), short-wavelength automated perimetry (SWAP), and frequency-doubling technology (FDT) perimetry in monitoring glaucoma progression.Longitudinal data of paired SAP, SWAP, and FDT from 113 eyes with primary open-angle glaucoma enrolled in the Diagnostic Innovations in Glaucoma Study or the African Descent and Glaucoma Evaluation Study were included. Data from all tests were expressed in comparable units by converting the sensitivity from decibels to unitless contrast sensitivity and by expressing sensitivity values in percent of mean normal based on an independent dataset of 207 healthy eyes with aging deterioration taken into consideration. Pointwise linear regression analysis was performed and 3 criteria (conservative, moderate, and liberal) were used to define progression and improvement. Global mean sensitivity (MS) was fitted with linear mixed models.No statistically significant difference in the proportion of progressing and improving eyes was observed across tests using the conservative criterion. Fewer eyes showed improvement on SAP compared to SWAP and FDT using the moderate criterion; and FDT detected less progressing eyes than SAP and SWAP using the liberal criterion. The agreement between these test types was poor. The linear mixed model showed a progressing trend of global MS overtime for SAP and SWAP, but not for FDT. The baseline estimate of SWAP MS was significantly lower than SAP MS by 21.59% of mean normal. FDT showed comparable estimation of baseline MS with SAP.SWAP and FDT do not appear to have significant benefits over SAP in monitoring glaucoma progression. SAP, SWAP, and FDT may, however, detect progression in different glaucoma eyes.

Abstract:PURPOSE:To evaluate the potential of stratus optical coherence tomography (OCT) for detecting early glaucoma.PATIENTS AND METHODS:Fifty-two open-angle glaucoma patients with unilateral visual field loss detected by standard automatic perimetry were tested using stratus OCT and frequency doubling technology (FDT) N-30. Fifty-two perimetrically normal eyes were enrolled and divided into 2 groups: eyes with normal OCT results and eyes with abnormal OCT results. FDT parameters and scores were compared between the 2 groups. We evaluated the topographic relationship of the OCT-measured retinal nerve fiber layer (RNFL) defects with the FDT-measured visual field loss and glaucomatous optic neuropathy.RESULTS:In eyes with abnormal OCT results, the percentage of eyes with abnormal FDT results was significantly higher (P<0.001); FDT mean deviation, pattern standard deviation, and scores were significantly worse than those with normal OCT results (P=0.005, P<0.001, P<0.001, respectively); and the OCT-measured RNFL defects showed good topographic agreement with the FDT-measured visual field loss and glaucomatous optic neuropathy.CONCLUSIONS:RNFL defects detected by stratus OCT were associated with FDT visual field loss. Stratus OCT can detect early glaucoma sooner than standard automatic perimetry.
Focal Loss Analysis of Peripapillary Nerve Fiber Layer Reflectance for Glaucoma Diagnosis

Abstract:Purpose: To evaluate nerve fiber layer (NFL) reflectance for glaucoma diagnosis using a large dataset. Methods: Participants were imaged with 4.9mm ONH scans using spectral-domain optical coherence tomography (OCT). The NFL reflectance map was reconstructed from 13 concentric rings of optic nerve head(ONH) scan, then processed by an azimuthal filter to reduce directional reflectance bias due to variation of beam incidence angle. The peripapillary thickness and reflectance maps were both divided into 96 superpixels. Low-reflectance and low-thickness superpixels were defined as values below the 5th percentile normative reference for that location. Focal reflectance loss was measured by summing loss, relative to the normal reference average, in low-reflectance superpixels. Focal thickness loss was calculated in a similar fashion. The area under receiving characteristic curve (AROC) was used to assess diagnostic accuracy. Results: Fifty-three normal, 196 pre-perimetric, 132 early perimetric, and 59 moderate and advanced perimetric glaucoma participants were included from the Advanced Imaging for Glaucoma Study. Sixty-seven percent of glaucomatous reflectance maps showed characteristic contiguous wedge or diffuse defects. Focal NFL reflectance loss had significantly higher diagnostic accuracy than the best NFL thickness parameters (both map-based and profile-based): AROC 0.80 v. 0.75 (p<0.004) for distinguishing glaucoma eyes from healthy control eyes. The diagnostic sensitivity was also significantly higher at both 99% and 95% specificity operating points. Conclusions: Focal NFL reflectance loss improved glaucoma diagnostic accuracy compared to the standard NFL thickness parameters.

A Hybrid Deep Learning Classification of Perimetric Glaucoma Using Peripapillary Nerve Fiber Layer Reflectance and Other OCT Parameters from Three Anatomy Regions

Abstract:Precis: A hybrid deep-learning model combines NFL reflectance and other OCT parameters to improve glaucoma diagnosis. Objective: To investigate if a deep learning model could be used to combine nerve fiber layer (NFL) reflectance and other OCT parameters for glaucoma diagnosis. Patients and Methods: This is a prospective observational study where of 106 normal subjects and 164 perimetric glaucoma (PG) patients. Peripapillary NFL reflectance map, NFL thickness map, optic head analysis of disc, and macular ganglion cell complex thickness were obtained using spectral domain OCT. A hybrid deep learning model combined a fully connected network (FCN) and a convolution neural network (CNN) to develop and combine those OCT maps and parameters to distinguish normal and PG eyes. Two deep learning models were compared based on whether the NFL reflectance map was used as part of the input or not. Results: The hybrid deep learning model with reflectance achieved 0.909 sensitivity at 99% specificity and 0.926 at 95%. The overall accuracy was 0.948 with 0.893 sensitivity and 1.000 specificity, and the AROC was 0.979, which is significantly better than the logistic regression models (p < 0.001). The second best model is the hybrid deep learning model w/o reflectance, which also had significantly higher AROC than logistic regression models (p < 0.001). Logistic regression with reflectance model had slightly higher AROC or sensitivity than the other logistic regression model without reflectance (p = 0.024). Conclusions: Hybrid deep learning model significantly improved the diagnostic accuracy, without or without NFL reflectance. Hybrid deep learning model, combining reflectance/NFL thickness/GCC thickness/ONH parameter, may be a practical model for glaucoma screen purposes.

Abstract:BACKGROUND:Fundus changes associated with high myopia (HM) may mask those associated with primary open-angle glaucoma (POAG). This study aim to determine the characteristics of RNFL thickness changes in patients with both POAG and HM and compare these to changes in patients with only HM. The diagnostic capabilities of both OCT and GDxVCC in this subset of patients are also evaluated.METHODS:Twenty-two eyes with POAG and HM (spherical equivalent (SE) between -6.0 and -12.0 D) were evaluated, and 22 eyes with HM were used for comparison. Characteristic retinal nerve fiber layer (RNFL) thickness profiles in patients with POAG and HM were examined using optical coherence tomography (OCT) and scanning laser polarimetry with variable corneal compensation (GDxVCC), and the diagnostic capabilities of these imaging modalities were compared. RNFL parameters evaluated included superior average (Savg-GDx), inferior average (Iavg-GDx), temporal-superior-nasal- inferior-temporal (TSNIT) average, and nerve fiber indicator (NFI) on GDxVCC and superior average (Savg-OCT), inferior average (Iavg-OCT), nasal average (Navg-OCT), temporal average (Tavg-OCT), and average thickness (AvgThick-OCT) on OCT (fast RNFL scan). Visual field testing was performed and defects were evaluated using mean defect (MD) and pattern standard deviation (PSD).RESULTS:The RNFL parameters (P < 0.05) significantly different between groups included Savg-GDx, Iavg-GDx, TSNIT average, NFI, Savg-OCT, Iavg-OCT, Tavg-OCT, and AvgThick-OCT. Significant correlations existed between TSNIT average and AvgThick-OCT (r = 0.778), TSNIT average and MD (r = 0.749), AvgThick-OCT and MD (r = 0.647), TSNIT average and PSD (r = -0.756), and AvgThick-OCT and PSD (r = -0.784). The area under the receiver operating characteristic curve (AUROC) values of TSNIT average, Savg-GDx, Iavg-GDx, NFI, Savg-OCT, Iavg-OCT, Navg-OCT, Tavg-OCT, and AvgThick-OCT were 0.947, 0.962, 0.973, 0.994, 0.909, 0.917, 0.511, 0.906, and 0.913, respectively. The NFI AUROC was the highest value.CONCLUSIONS:RNFL thickness was significantly lower in all but the nasal quadrant in patients with POAG and HM, compared to patients with only HM. Measurements with OCT and GDxVCC were well-correlated, and both modalities detected RNFL thickness changes. However, GDxVCC was better than OCT in detecting POAG in HM patients.

Abstract:BACKGROUND Optical coherence tomography (OCT) is a high resolution noncontact imaging modality which can quantitatively detect the optic disc and retinal structure. This study was designed to evaluate the diagnostic capability of parameters of the optic disc, retinal nerve fiber layer thickness, and ganglion cell complex (GCC) using a new technology called Fourier-domain OCT (FD-OCT) for early primary open angle glaucoma (POAG) patients. METHODS Two groups of patients, early perimetric damage POAG and normal subjects were included in this observational cross-sectional study. All patients underwent FD-OCT and visual field examination in addition to full ophthalmic examinations. Receiver operating characteristic curves (ROC) were studied for all parameters. The sensitivity and specificity for distinguishing between normal and early glaucomatous eyes, the areas under the receiver operating characteristic curves (AROC) and positive, negative likelihood ratios were evaluated for all the single parameters and selected combined parameters using arbitrary cutoffs. RESULTS Thirty-four eyes of 34 early POAG patients and 42 eyes of 42 normal subjects were analyzed. Cup/disc (C/D) vertical ratio presented the best sensitivity and positive likelihood ratio for selected specificities (95% and 85%) which were 79.4% and 88.2%, 33.4 and 7.4, respectively. Among all single parameters, the C/D vertical ratio demonstrated the highest AROC which was at 0.930. The average thickness of circumpapillary RNFL on 3.45 mm showed the highest AROC among all of the peripapillary RNFL parameters. The sensitivity at selected specificity and AROC of GCC were not as high as C/D vertical ratio and RNFL AT on 3.45 mm. When the C/D vertical ratio, RNFL AT on 3.45 mm, and rim area were combined using a logistical diagnostic model, the AROC was raised to 0.949 but not significantly different from the top single parameter, C/D vertical ratio. CONCLUSIONS The key parameters obtained by FD-OCT were able to show the significant differences of optic discs, thickness of RNFL and GCC between POAG patients and normal subjects. According to sensitivity, specificity, likelihood ratio and AROC, the top three parameters from FD-OCT for early diagnosis of POAG were C/D vertical ratio, RNFL AT on 3.45 mm, and the rim area.

Abstract:Background: Diagnosis of early glaucomatous damage as well as the detection of glaucomatous change are still difficult tasks. During the last years numerous new diagnostic techniques have been investigated and some of them have been introduced to the market. Methods: This paper reviews the different aims of diagnostic technologies in the field of glaucoma. Methods appearing suitable for a large-scale use by ophthalmologists will be judged according to their suitability to meet the goals of early diagnosis, objective diagnosis, progression analysis and screening. Conclusions: The new diagnostic techniques have not yet greatly influenced our diagnostic procedure. This is mainly due to the fact that, for the most difficult borderline cases of glaucoma, these methods are of limited value, or have not yet been sufficiently investigated. Nevertheless, the Heidelberg retina tomograph (HRT), the nerve fiber analyzer GDX, short wavelength perimetry (SWAP), and the frequency doubling test (FDT) may play a role in these cases. For follow-up, HRT and GDx have proven valuable, especially in early stages of the disease. The very short testing time of FDT together with good specificity qualifies this test for glaucoma screening.

Abstract:Purpose: Evaluation of ganglion cell-inner plexiform layer thickness in the diagnosis of pre-perimetric glaucoma (PPG) and comparison to retinal nerve fiber layers. Methods: This study was a prospective hospital-based study. A total of 30 PPG and control patients were studied for retinal nerve fiber layer thickness (RNFL) and ganglion cell-inner plexiform layer complex (GC-IPL) by spectral-domain optical coherence tomography. PPG was defined as eyes with a normal visual field and one or more localized RNFL defects that were associated with a typical glaucomatous disc appearance. Diagnostic abilities of GC-IPL, optic nerve head (ONH), and RNFL parameters were computed using area under receiver-operating curve (AUROC), sensitivity, and specificity. Results: GC-IPL parameters showed significant changes in PPG cases as compared to normal subjects in each region ( P value < 0.001). RNFL parameters also differed significantly from normal subjects in all quadrants ( P value 0.003 to < 0.001). Within GC-IPL parameters, the superotemporal region had the maximum area under the curve (AUC), followed by inferior, superior, and inferotemporal regions. Within RNFL parameters, the inferior quadrant had the maximum AUC, followed by superior and nasal quadrants. the GC-IPL parameters in PPG showed that the AUC of the GC-IPL parameters was much higher than those of the ONH and RNFL values. Conclusion: Although both the parameters RNFL and GC-IPL showed significant changes in PPG patients compared to healthy subjects, a higher AUC of GC-IPL points toward the higher sensitivity of GC-IPL than RNFL for detecting glaucoma in early stages.
Hemifield-based analysis of pattern electroretinography in normal subjects and patients with preperimetric glaucoma

Abstract:This prospective cross-sectional study investigated the visual function of preperimetric glaucoma (PPG) patients based on hemifield (HF) pattern electroretinogram (PERG) amplitudes. Thirty-two (32) normal subjects and 33 PPG patients were enrolled in control and PPG groups, respectively. All of the participants had undergone full ophthalmic examinations, including spectral-domain optical coherence tomography (SD-OCT), visual field (VF) examination and pattern electroretinography (PERG). The PERG parameters along with the HF ratios of SD-OCT and PERG were compared between the control and PPG groups. Pairwise Pearson's correlation coefficients and linear regression models were fitted to investigate the correlations. The PERG N95 amplitudes were significantly lower in the PPG group ( P < 0.001). The smaller/larger HF N95 amplitude ratio of the PPG group was found to be smaller than that of the control group (0.73 ± 0.20 vs. 0.86 ± 0.12; P = 0.003) and showed positive correlations with affected HF average ganglion cell-inner plexiform layer (GCIPL) thickness (r = 0.377, P = 0.034) and with average GCIPL thickness (r = 0.341, P = 0.005). The smaller/larger HF N95 amplitude ratio did not significantly change with age (β = − 0.005, P = 0.195), whereas the full-field N95 amplitude showed a negative correlation with age (β = − 0.081, P < 0.001). HF analysis of PERG N95 amplitudes might be particularly useful for patients with early glaucoma.

Abstract:Purpose. The purpose of this study was to investigate the utility of steady state pattern electroretinogram (ss-PERG) in detecting retinal ganglion cell (RGC) dysfunction in glaucoma suspects (GS) who had normal 24-2 Humphrey Visual Fields (HFA). Materials and Methods. This was a prospective cohort study of GS patients who were identified based on optic disc appearance with normal HFAs. Patients received a complete eye examination, standard automated perimetry (SAP), optical coherence tomography (OCT), and ss-PERG measurements. The ss-PERG parameters, Magnitude (Mag), Magnitude D (MagD), and MagD/Mag ratio, were examined, along with their relationships between HFA and OCT measurements. Results. Twenty-five patients were included in this study, with a total of 49 eyes. Fifteen eyes had abnormal ss-PERG parameters and when compared to GS eyes with normal ss-PERG parameters, there were significant differences in HFA 24-2, retinal nerve fiber layer (RNFL) thickness, and ganglion cell layer and inner plexiform layer (GCL + IPL) thickness. All ss-PERG parameters were significantly correlated with 24-2 VF mean deviation (MD) and visual field index (VFI), as well as 10-2 VF MD after controlling for age, sex, intraocular pressure, central corneal thickness, and spherical equivalent. When controlled for age, spherical equivalent, and IOP, MagD/Mag ratio significantly contributed to the variance in average GCL + IPL thicknesses, whereas 24-2 VF MD and 10-2 VF MD did not. MagD/Mag ratio also significantly accounted for variance in all macular GCL + IPL sectors, while 10-2 VF MD did not. Conclusions. ss-PERG has significant correlations with HFA global indices and was predictive of GCL + IPL thickness in GS patients. Clinical Significance. ss-PERG may serve as a useful functional tool for detecting and measuring RGC dysfunction in GS. It appears to be more sensitive than HFA in the detection of early changes in GCL + IPL thicknesses and may be helpful to use in conjunction with current diagnostic studies to improve the ability of monitoring GS progression.

Focal Loss Analysis of Nerve Fiber Layer Reflectance for Glaucoma Diagnosis

Abstract:Purpose: To evaluate nerve fiber layer (NFL) reflectance for glaucoma diagnosis.Methods: Participants were imaged with 4.5 × 4.5 mm volumetric disc scans using spectral-domain optical coherence tomography. The normalized NFL reflectance map was processed by an azimuthal filter to reduce directional reflectance bias caused by variation of beam incidence angle. The peripapillary area of the map was divided into 160 superpixels. Average reflectance was the mean of superpixel reflectance. Low-reflectance superpixels were identified as those with NFL reflectance below the fifth percentile normative cutoff. Focal reflectance loss was measured by summing loss in low-reflectance superpixels.Results: Thirty-five normal, 30 preperimetric, and 35 perimetric glaucoma participants were enrolled. Azimuthal filtering improved the repeatability of the normalized NFL reflectance, as measured by the pooled superpixel standard deviation (SD), from 0.73 to 0.57 dB (P < 0.001, paired t-test) and reduced the population SD from 2.14 to 1.78 dB (P < 0.001, t-test). Most glaucomatous reflectance maps showed characteristic patterns of contiguous wedge or diffuse defects. Focal NFL reflectance loss had significantly higher diagnostic sensitivity than the best NFL thickness parameter (from map or profile): 77% versus 55% (P < 0.001) in glaucoma eyes with the specificity fixed at 99%.Conclusions: Azimuthal filtering reduces the variability of NFL reflectance measurements. Focal NFL reflectance loss has excellent glaucoma diagnostic accuracy compared to the standard NFL thickness parameters. The reflectance map may be useful for localizing NFL defects.Translational Relevance: The high diagnostic accuracy of NFL reflectance may make population-based screening feasible.

Abstract:Objective To explore the potential of frequency-doubling technology perimetry (FDP) for predicting the risk of future visual field loss on standard automated perimetry (SAP) for preperimetric openangle glaucoma(OAG).Methods A prospective cohort study.OAG patients with unilateral field loss detected by SAP (Octopus,G2 program) were examined with the FDP N-30 threshold program (Humphrey Instruments) at baseline.53 of the SAP test points were matched to the 19 of FDP sectors as SAP-on-FDP sectors.Visual field examinations were followed by a series of SAP examinations for the perimetrically normal eyes administered over 8 years.The relative risk (RR) of subsequent SAP abnormality corresponding to FDP abnormal sectors in perimetrically normal eyes of participants at baseline was calculated finally.Results Forty-eight perimetrically normal eyes of 48 participants had complete data and a qualifying follow-up.At baseline,there were 912 FDP sectors in the 48 eyes with normal SAP results,from which 21％ (196/912) were abnormal.During the 8-year-follow-up,26％(51/196) for FDP abnormal sectors developed an SAP abnormality in the SAP-on-FDP sector in 4 to 90 months after baseline,and the RR of subsequent SAP abnormality corresponding to abnormal FDP sectors was 5.50 (95％ confidence interval,3.53～8.58;P ＜0.001).Conclusions In preperimetric OAG,FDP could exactly predict the risk of future visual field loss on SAP to some extent.

Abstract:This thesis addresses several aspects of glaucoma diagnostics from both a clinical and a screening perspective. New instruments for diagnosing glaucoma have been developed over the past years, but little information is available regarding their performance as screening methods and their usefulness in ordinary clinical practice. PURPOSE OF THE RESEARCH UNDERLYING THIS THESIS: The objectives of this research were as follows: to compare the accuracy of results of analysis of the optic nerve head (ONH) achieved by computerized imaging using the Heidelberg Retina Tomograph (HRT) and by subjective assessment performed by physicians with different degrees of experience of glaucoma (paper III); to evaluate the effect of a continuous medical education (CME) lecture on subjective assessment of the ONH for diagnosis of glaucoma (paper II); to investigate subjective assessment of perimetric test results by physicians with varying knowledge of glaucoma with a trained artificial neural network (ANN) and to compare the certainty of the classifications (paper IV); and to compare the diagnostic performance of time-domain Stratus optical coherence tomography (OCT) with that of spectral-domain Cirrus OCT (paper I), frequency doubling technology (FDT) screening perimetry and scanning laser polarimetry with the GDx variable corneal compensator (VCC) in a random population-based sample and in patients with glaucoma of varying disease severity. Methods and results: In evaluation of the ONH, use of the HRT statistical tools, Moorfields regression analysis (MRA) and the Glaucoma Probability Score (GPS) was compared with subjective assessment performed by 45 physicians. Optic nerve head images and photographs from 138 healthy and 97 glaucoma subjects were included. The sensitivity of MRA was higher (87-94%) than that of the average physician (62-82%), considerably greater than that of ophthalmologists with subspecialties other than glaucoma (53-77%) and non-significantly better than that of glaucoma experts (72-88%). Sensitivity achieved by GPS (79-93%) was also greater than that of the average physician. MRA correctly classified all eyes with advanced glaucomatous visual field defects, a result that was not achieved by GPS or even by the glaucoma experts. In eyes with small discs, MRA sensitivity (88%) was comparable with that of glaucoma experts (85%) and much better than that of GPS (50%). Also, the group comprising all physicians provided specificity (75-92%) similar to that of both MRA (69 - 86%) and GPS (72-94%) (Andersson et al. 2011a). A 1-hr CME lecture on ONH assessment led to a significant improvement in sensitivity (from 70% to 80%) and a significant decrease in uncertain assessments (from 22% to 13%), whereas specificity remained unchanged (68%) (Andersson et al. 2011b). A rise in sensitivity was seen in all subgroups of physicians, including glaucoma experts. Thirty physicians assessing standard automated perimetry (SAP) test results as Humphrey Field Analyzer single-field analysis printouts with full StatPac information from 99 patients with glaucoma and 66 healthy subjects were compared with a trained ANN regarding diagnostic performance. ANN reached significantly higher sensitivity (93%) than the average physician (83%), whereas specificity was similar for these two groups (91% and 90%, respectively). Diagnostic accuracy was similar among the different groups of physicians and seemingly rather independent of experience. Sensitivity ranged from 82% in the subgroup of other subspecialists to 87% in the glaucoma expert group, and specificity ranged from 88% among general ophthalmologists to 91% for glaucoma experts. The ANN attained certainty of classification that was in parity with that provided by the glaucoma experts and did not make any completely incorrect classifications of the visual fields (i.e. erroneous classifications were in the borderline zone) (Andersson et al. 2012). From a population-based randomly selected sample (n=308) of older subjects (aged ≥ 50 years) living in southern Sweden, 170 subjects underwent a comprehensive examination that included Stratus OCT, Cirrus OCT, an FDT screening programme and the GDx VCC. The same test protocol was applied to 138 randomized clinical patients with different stages of glaucoma. In the population-based sample, both Stratus and Cirrus OCT showed high diagnostic accuracy with area under the receiver-operating curve (aROC) values close to 1.0 (Bengtsson et al. 2012). Both OCT instruments correctly classified all of the clinical glaucoma patients with advanced disease. FDT screening showed high sensitivity (91%) but erroneously gave normal test results for some eyes with advanced disease. GDx VCC had lower sensitivity (73-92%) and also led to a large proportion of examinations with an atypical retardation pattern that is known to affect the diagnostic efficiency of this instrument. Conclusions: The HRT MRA performed better than most physicians and was consistent with the glaucoma experts. These results suggest that MRA can be a valuable tool for diagnosing glaucoma in ordinary practice, particularly when only a few glaucoma experts are available. Even though MRA provided 100% sensitivity in eyes with advanced glaucoma, it probably does not offer sufficient specificity to make it suitable as a screening method. Continuing medical education on ONH analysis had a small, but positive effect on diagnostic accuracy for glaucoma. An ANN trained to classify visual fields seemed to perform at least as well as most of the participating physicians, whose performances were remarkably similar regardless of their level of experience. This indicates that available tools for interpreting SAP findings are helpful in assessments of visual field test results. However, SAP is associated with learning effects (Heijl et al. 1989) that may entail low specificity for untrained subjects, and hence, it is not an ideal screening method for glaucoma. By comparison, the screening test of FDT is rapid and easy, but it is probably less suitable for screening purpose, because some eyes with advanced glaucoma were missed in this investigation. GDx VCC images for a relatively large number of eyes could not be analysed and is thus not appropriate for screening. The OCT instruments offer both high sensitivity and high specificity, and all eyes with advanced disease were correctly classified as glaucomatous in this evaluation. However, these instruments are still expensive and require special operator skills. Additional development to obtain OCT instrument that is more compact, easier to use and less expensive might render such tomography suitable as a screening tool for glaucoma.
