Abstract:Tumors are an important health concern in modern times. Breast cancer is one of the most prevalent causes of death for women. Breast cancer is rapidly becoming the leading cause of mortality among women globally. Early detection of breast cancer allows patients to obtain appropriate therapy, increasing their probability of survival. The adoption of 3-Dimensional (3D) mammography for the medical identification of abnormalities in the breast reduced the number of deaths dramatically. Classification and accurate detection of lumps in the breast in 3D mammography is especially difficult due to factors such as inadequate contrast and normal fluctuations in tissue density. Several Computer-Aided Diagnosis (CAD) solutions are under development to help radiologists accurately classify abnormalities in the breast. In this paper, a breast cancer diagnosis model is implemented to detect breast cancer in cancer patients to prevent death rates. The 3D mammogram images are gathered from the internet. Then, the gathered images are given to the preprocessing phase. The preprocessing is done using a median filter and image scaling method. The purpose of the preprocessing phase is to enhance the quality of the images and remove any noise or artifacts that may interfere with the detection of abnormalities. The median filter helps to smooth out any irregularities in the images, while the image scaling method adjusts the size and resolution of the images for better analysis. Once the preprocessing is complete, the preprocessed image is given to the segmentation phase. The segmentation phase is crucial in medical image analysis as it helps to identify and separate different structures within the image, such as organs or tumors. This process involves dividing the preprocessed image into meaningful regions or segments based on intensity, color, texture, or other features. The segmentation process is done using Adaptive Thresholding with Region Growing Fusion Model (AT-RGFM)". This model combines the advantages of both thresholding and region-growing techniques to accurately identify and delineate specific structures within the image. By utilizing AT-RGFM, the segmentation phase can effectively differentiate between different parts of the image, allowing for more precise analysis and diagnosis. It plays a vital role in the medical image analysis process, providing crucial insights for healthcare professionals. Here, the Modified Garter Snake Optimization Algorithm (MGSOA) is used to optimize the parameters. It helps to optimize parameters for accurately identifying and delineating specific structures within medical images and also helps healthcare professionals in providing more precise analysis and diagnosis, ultimately playing a vital role in the medical image analysis process. MGSOA enhances the segmentation phase by effectively differentiating between different parts of the image, leading to more accurate results. Then, the segmented image is fed into the detection phase. The tumor detection is performed by the Vision Transformer-based Multiscale Adaptive EfficientNetB7 (ViT-MAENB7) model. This model utilizes a combination of advanced algorithms and deep learning techniques to accurately identify and locate tumors within the segmented medical image. By incorporating a multiscale adaptive approach, the ViT-MAENB7 model can analyze the image at various levels of detail, improving the overall accuracy of tumor detection. This crucial step in the medical image analysis process allows healthcare professionals to make more informed decisions regarding patient treatment and care. Here, the created MGSOA algorithm is used to optimize the parameters for enhancing the performance of the model. The suggested breast cancer diagnosis performance is compared to conventional cancer diagnosis models and it showed high accuracy. The accuracy of the developed MGSOA-ViT-MAENB7 is 96.6 %, and others model like RNN, LSTM, EffNet, and ViT-MAENet given the accuracy to be 90.31 %, 92.79 %, 94.46 % and 94.75 %. The developed model's ability to analyze images at multiple scales, combined with the optimization provided by the MGSOA algorithm, results in a highly accurate and efficient system for detecting tumors in medical images. This cutting-edge technology not only improves the accuracy of diagnosis but also helps healthcare professionals tailor treatment plans to individual patients, ultimately leading to better outcomes. By outperforming traditional cancer diagnosis models, the proposed model is revolutionizing the field of medical imaging and setting a new standard for precision and effectiveness in healthcare.

Intelligent Breast Cancer Diagnosis with Heuristic-assisted Trans-Res-U-Net and Multiscale DenseNet using Mammogram Images

Abstract:Breast cancer (BC) significantly contributes to cancer-related mortality in women, underscoring the criticality of early detection for optimal patient outcomes. A mammography is a key tool for identifying and diagnosing breast abnormalities; however, accurately distinguishing malignant mass lesions remains challenging. To address this issue, we propose a novel deep learning approach for BC screening utilizing mammography images. Our proposed model comprises three distinct stages: data collection from established benchmark sources, image segmentation employing an Atrous Convolution-based Attentive and Adaptive Trans-Res-UNet (ACA-ATRUNet) architecture, and BC identification via an Atrous Convolution-based Attentive and Adaptive Multi-scale DenseNet (ACA-AMDN) model. The hyperparameters within the ACA-ATRUNet and ACA-AMDN models are optimised using the Modified Mussel Length-based Eurasian Oystercatcher Optimization (MML-EOO) algorithm. Performance evaluation, leveraging multiple metrics, is conducted, and a comparative analysis against conventional methods is presented. Our experimental findings reveal that the proposed BC detection framework attains superior precision rates in early disease detection, demonstrating its potential to enhance mammography-based screening methodologies.

Abstract:Breast cancer is widespread worldwide and can be cured if diagnosed early. Using digital mammogram images and image processing with artificial intelligence can play an essential role in breast cancer diagnosis. As many computerized algorithms for breast cancer diagnosis have significant limitations, such as noise handling and varying or low contrast in the images, it can be difficult to segment the abnormal region. These challenges could be overcome by proposing a new pre-processing model, exploring its impact on the post-processing module, and testing it on an extensive database. In this research work, the three-step method is proposed and validated on large databases of mammography images. The first step corresponded to the database classification, followed by the second step, which removed the pectoral muscle from the mammogram image. The third stage utilized new image-enhancement techniques and a new segmentation module to detect abnormal regions in a well-enhanced image to diagnose breast cancer. The pre-and post-processing modules are based on novel image processing techniques. The proposed method was tested using data collected from different hospitals in the Qassim Health Cluster, Qassim Province, Saudi Arabia. This database contained the five categories in the Breast Imaging and Reporting and Data System and consisted of 2892 images; the proposed method is analyzed using the publicly available Mammographic Image Analysis Society database, which contained 322 images. The proposed method gives good contrast enhancement with peak-signal to noise ratio improvement of 3 dB. The proposed method provides an accuracy of approximately 92% on 2892 images of Qassim Health Cluster, Qassim Province, Saudi Arabia. The proposed method gives approximately 97% on the Mammographic Image Analysis Society database. The novelty of the proposed work is that it could work on all Breast Imaging and Reporting and Data System categories. The performance of the proposed method demonstrated its ability to improve the diagnostic performance of the computerized breast cancer detection method.

Abstract:Breast cancer is the most common cancer and is the leading cause of cancer death among women worldwide. Detection of breast cancer, while it is still small and confined to the breast, provides the best chance of effective treatment. Computer Aided Detection (CAD) systems that detect cancer from mammograms will help in reducing the human errors that lead to missing breast carcinoma. Literature is rich of scientific papers for methods of CAD design, yet with no complete system architecture to deploy those methods. On the other hand, commercial CADs are developed and deployed only to vendors' mammography machines with no availability to public access. This paper presents a complete CAD; it is complete since it combines, on a hand, the rigor of algorithm design and assessment (method), and, on the other hand, the implementation and deployment of a system architecture for public accessibility (system). (1) We develop a novel algorithm for image enhancement so that mammograms acquired from any digital mammography machine look qualitatively of the same clarity to radiologists' inspection; and is quantitatively standardized for the detection algorithms. (2) We develop novel algorithms for masses and microcalcifications detection with accuracy superior to both literature results and the majority of approved commercial systems. (3) We design, implement, and deploy a system architecture that is computationally effective to allow for deploying these algorithms to cloud for public access.

Abstract:Spatial Kerr solitons, typically associated with the standard paraxial nonlinear Schro dinger equation, are shown to exist to all nonparaxial orders as exact solutions of Maxwell's equations in the presence of the vectorial Kerr effect. More precisely, we prove the existence of azimuthally polarized, spatial, dark soliton solutions of Maxwell's equations, while exact linearly polarized (2 + 1)D solitons do not exist. Our ab initio approach predicts the existence of dark solitons up to an upper value of the maximum field amplitude, corresponding to a minimum soliton width of about one-fourth of the wavelength.

Abstract:A common gynecological disease in the world is breast cancer that early diagnosis of this disease can be very effective in its treatment. The use of image processing methods and pattern recognition techniques in automatic breast detection from mammographic images decreases human errors and increments the rapidity of diagnosis. In this paper, mammographic images are analyzed using image processing techniques and a pipeline structure for the diagnosis of the cancerous masses. In the first stage, the quality of mammogram images and the contrast of abnormal areas in the image are improved by using image contrast improvement and a noise decline. A method based on color space is then used for image segmentation that is followed by mathematical morphology. Then, for feature image extraction, a combined gray-level cooccurrence matrix (GLCM) and discrete wavelet transform (DWT) method is used. At last, a new optimized version of convolutional neural network (CNN) and a new improved metaheuristic, called Advanced Thermal Exchange Optimizer, are used for the classification of the features. A comparison of the simulations of the proposed technique with three different techniques from the literature applied on the MIAS mammogram database is performed to show its superiority. Results show that the accuracy of diagnosing cancer cases for the proposed method and applied on the MIAS database is 93.79%, and sensitivity and specificity are obtained 96.89% and 67.7%, respectively.

Abstract:Breast cancer significantly impacts women's health, with ultrasound being crucial for lesion assessment. To enhance diagnostic accuracy, computer-aided detection (CAD) systems have attracted considerable interest. This study introduces a prospective deep learning architecture called "Multi-modal Multi-task Network" (3MT-Net). 3MT-Net utilizes a combination of clinical data, B-mode, and color Doppler ultrasound. We have designed the AM-CapsNet network, specifically tailored to extract crucial tumor features from ultrasound. To combine clinical data in 3MT-Net, we have employed a cascaded cross-attention to fuse information from three distinct sources. To ensure the preservation of pertinent information during the fusion of high-dimensional and low-dimensional data, we adopt the idea of ensemble learning and design an optimization algorithm to assign weights to different modalities. Eventually, 3MT-Net performs binary classification of benign and malignant lesions as well as pathological subtype classification. In addition, we retrospectively collected data from nine medical centers. To ensure the broad applicability of the 3MT-Net, we created two separate testsets and conducted extensive experiments. Furthermore, a comparative analysis was conducted between 3MT-Net and the industrial-grade CAD product S-detect. The AUC of 3MT-Net surpasses S-Detect by 1.4% to 3.8%.
Advancing Breast Cancer Detection: Enhancing YOLOv5 Network for Accurate Classification in Mammogram Images

Abstract:Recent advances in artificial intelligence (AI), notably deep learning, have sparked widespread curiosity with bioinformatics, particularly the challenges presented by medical imaging. It's been really helpful in enabling the Computer Aided Diagnosis CAD system to provide precise outcomes. Nonetheless, it is still a difficult task to identify breast cancer in mammography images. The purpose of this effort is to lower False Positive Rate FPR and False Negative Rate FNR and increase Matthews's correlation coefficient MCC value. Two highly tailored object detection models, YOLOv5 and Mask R-CNN, are utilized to get the job done. YOLOv5 is able to detect the mass and determine whether it is benign or malignant. However, YOLOV5's limited real estate necessitates certain tweaks to the original model in order to get the desired effects. Tumor borders and size are both identified by Mask RCNN as it traverses breast parenchyma in search of malignancies. Stages of cancer are based on the magnitude of the patients' tumours. This model employs YOLOv5+Mask RCNN and is trained on the INbreast, CBIS-DDSM, and BNS dataset. The proposed model is compared against the baseline version of YOLOv5 to determine how well it performs. The proposed method improves performance, with an FPR of 0.049%, a FNR of 0.029%, and a high MCC value of 92.02%. Based on the results of the studies, combining YOLOv5 with Mask RCNN improves accuracy by 0.06 percentage points compared to using either method alone. Furthermore, this effort may aid in determining the patient's prognosis and allowing clinicians to be more accurate and predictable in the diagnosing process at an early stage.

Automated assessment of task-based performance of digital mammography and tomosynthesis systems using an anthropomorphic breast phantom and deep learning-based scoring

Abstract:Purpose: Conventional metrics used for assessing digital mammography (DM) and digital breast tomosynthesis (DBT) image quality, including noise, spatial resolution, and detective quantum efficiency, do not necessarily predict how well the system will perform in a clinical task. A number of existing phantom-based methods have their own limitations, such as unrealistic uniform backgrounds, subjective scoring using humans, and regular signal patterns unrepresentative of common clinical findings. We attempted to address this problem with a realistic breast phantom with random hydroxyapatite microcalcifications and semi-automated deep learning-based image scoring. Our goal was to develop a methodology for objective task-based assessment of image quality for tomosynthesis and DM systems, which includes an anthropomorphic phantom, a detection task (microcalcification clusters), and automated performance evaluation using a convolutional neural network. Approach: Experimental 2D and pseudo-3D mammograms of an anthropomorphic inkjet-printed breast phantom with inserted microcalcification clusters were collected on clinical mammography systems to train a signal-present/signal-absent image classifier based on Resnet-18 architecture. In a separate validation study using simulations, this Resnet-18 classifier was shown to approach the performance of an ideal observer. Microcalcification detection performance was evaluated as a function of four dose levels using receiver operating characteristic (ROC) analysis [i.e., area under the ROC curve (AUC)]. To demonstrate the use of this evaluation approach for assessing different technologies, the method was applied to two different mammography systems, as well as to mammograms with re-binned pixels emulating a lower-resolution X-ray detector. Results: Microcalcification detectability, as assessed by the deep learning classifier, was observed to vary with the exposure incident on the breast phantom for both DM and tomosynthesis. At full dose, experimental AUC was 0.96 (for DM) and 0.95 (for DBT), whereas at half dose, it dropped to 0.85 and 0.71, respectively. AUC performance on DM was significantly decreased with an effective larger pixel size obtained with re-binning. The task-based assessment approach also showed the superiority of a newer mammography system compared with an older system. Conclusions: An objective task-based methodology for assessing the image quality of mammography and tomosynthesis systems is proposed. Possible uses for this tool could be quality control, acceptance, and constancy testing, assessing the safety and effectiveness of new technology for regulatory submissions, and system optimization. The results from this study showed that the proposed evaluation method using a deep learning model observer can track differences in microcalcification signal detectability with varied exposure conditions.

Abstract:Cancer-related deadly diseases affect both developed and underdeveloped nations worldwide. Effective network learning is crucial to more reliably identify and categorize breast carcinoma in vast and unbalanced image datasets. The absence of early cancer symptoms makes the early identification process challenging. Therefore, from the perspectives of diagnosis, prevention, and therapy, cancer continues to be among the healthcare concerns that numerous researchers work to advance. It is highly essential to design an innovative breast cancer detection model by considering the complications presented in the classical techniques. Initially, breast cancer images are gathered from online sources and it is further subjected to the segmentation region. Here, it is segmented using Adaptive Trans-Dense-Unet (A-TDUNet), and their parameters are tuned using the developed Modified Sheep Flock Optimization Algorithm (MSFOA). The segmented images are further subjected to the breast cancer detection stage and effective breast cancer detection is performed by Multiscale Dilated Densenet with Attention Mechanism (MDD-AM). Throughout the result validation, the Net Present Value (NPV) and accuracy rate of the designed approach are 96.719% and 93.494%. Hence, the implemented breast cancer detection model secured a better efficacy rate than the baseline detection methods in diverse experimental conditions.

Digital mammography: a world without film?

Abstract:Objectives: eDiaMoND is a next generation Internet ("Grid") multidisciplinary research project funded by the UK e-Science Programme with the following objectives; the development of a next generation Internet enabled prototype to demonstrate the potential benefits of a national infrastructure to support digital mammography; the exploration of potential benefits for digital mammography systems, with particular emphasis being placed on selected applications, namely, screening, training, computer-aided detection and appropriate support for epidemiological studies. Methods: EDiaMoND has worked in conjunction with selected clinical partners to enable the collection of valuable mammography information and the design of applications based upon extensive requirements gathering exercises. The clinical partners validated both the immediate needs and assisted with defining future needs of such an architecture to support the UK Health Service. Results: The project has succeeded in invoking the interest of clinical partners and representatives of the UK NHS Breast Screening Programme in our vision of a world without film, albeit a long way off. The project has also succeeded in identifying the barriers to adopting this approach with the current limitations within the NHS, and has developed a blueprint for working towards this strategy. Conclusions: A UK national digital mammography archive has the potential to provide major benefits for the UK. For example, such an archive could: ensure that previous mammograms are always available, and could link up seamlessly the screening, assessment and symptomatic clinics; it could provide a huge teaching and training resource; it could be a huge resource for epidemiological studies.

Abstract:Breast cancer is a considerable prime cause which affects the lives of women’s and leads to death all around the world. A tumor is said to be malignancy if a number of cancerous cells can spread to other organs. There has been great impact on the field of medical imaging by the advancement of the computer technology as new and improved techniques of data acquisition, analysis, processing and visualization has evolved. A Digital Breast Tomosynthesis is used for the detection of breast cancer. This is also called as three-dimensional mammography, which eliminates the overlapping tissue problem. Digital Breast Tomosynthesis (DBT) provides information about potential abnormal tissues necessary for medical follow up and diagnosis. DBT gets additional importance in medical science as it is the only preliminary method of diagnosing a breast cancer with the dense breast. Early and accurate diagnosis can be sufficient in resolving various complications and guiding the patient with timely and proper treatment. By considering the above factors there is a great requirement to explore for DBT. The proposed work develops a computer aided methodology for automatic tumor detection and diagnosing in tomosynthesis Patient’s image. This method is incredibly helpful for doctors or the radiologist automatically locates the tumor space within the breast image for further surgery.

Abstract:Teleradiology refers to the electronic transmission of radiological images, such as X-rays, Computed Tomograms (CT's), and Magnetic Resonance Images (MRI's) across geographical locations for the purposes of interpretation and consultation. The digital radiological images are typically transmitted using standard telephone lines, satellite connections, or wide area networks (WANs). Teleradiology is an empowering technology and a facilitator for enhanced medical care. Teleradiology enables a single radiologist to simultaneously provide services to several hospitals independent of their location and also allows the exploitation of global time differences to provide emergency night coverage by personnel in a different time zone working a day shift. Additionally, the quality of care delivered by a wide awake, alert physician working a day shift is far superior to that provided by a radiologist who is up all night. Subspecialty opinions delivered to locations where the expertise is otherwise unavailable are an added benefit. In this paper, we describe the current state-of-art in teleradiology, the benefits of the clinical practice of teleradiology, and the technical, regulatory and security issues related to teleradiology. We begin by providing a historical background describing the evolution of global teleradiology.

Abstract:Medical image analysis places a significant focus on breast cancer, which poses a significant threat to women's health and contributes to many fatalities. An early and precise diagnosis of breast cancer through digital mammograms can significantly improve the accuracy of disease detection. Computer-aided diagnosis (CAD) systems must analyze the medical imagery and perform detection, segmentation, and classification processes to assist radiologists with accurately detecting breast lesions. However, early-stage mammography cancer detection is certainly difficult. The deep convolutional neural network has demonstrated exceptional results and is considered a highly effective tool in the field. This study proposes a computational framework for diagnosing breast cancer using a ResNet-50 convolutional neural network to classify mammogram images. To train and classify the INbreast dataset into benign or malignant categories, the framework utilizes transfer learning from the pretrained ResNet-50 CNN on ImageNet. The results revealed that the proposed framework achieved an outstanding classification accuracy of 93%, surpassing other models trained on the same dataset. This novel approach facilitates early diagnosis and classification of malignant and benign breast cancer, potentially saving lives and resources. These outcomes highlight that deep convolutional neural network algorithms can be trained to achieve highly accurate results in various mammograms, along with the capacity to enhance medical tools by reducing the error rate in screening mammograms.

Artificial intelligence is helpful with this process by allowing us to rapidly start the necessary protocol for treatment in the early stages of disease development. Artificial intelligence is a major contributor to the improvement of medical treatment for patients. In order to prevent and foresee this problem on the individual, family, and generational levels, Monitoring the patient's therapy and recovery is crucial. This study's objective is to outline a non-invasive method for using mammograms to detect breast abnormalities, classify breast disorders, and identify cancerous or benign tumor tissue in the breast. We used classification models on a dataset that has been pre-processed so that the number of samples is balanced, unlike previous work on the same dataset. Identifying cancerous or benign breast tissue requires the use of supervised learning techniques and algorithms, such as random forest (RF) and decision tree (DT) classifiers, to examine up to thirty features, such as breast size, mass, diameter, circumference, and the nature of the tumor (solid or cystic). To ascertain if the tissue is malignant or benign, the examination's findings are employed. These features are mostly what determines how effectively anything may be categorized. The DT classifier was able to get a score of 95.32%, while the RF satisfied a far higher 98.83 percent.

Abstract:Using computer-aided design (CAD) systems, this research endeavors to enhance breast cancer segmentation by addressing data insufficiency and data complexity during model training. As perceived by computer vision models, the inherent symmetry and complexity of mammography images make segmentation difficult. The objective is to optimize the precision and effectiveness of medical imaging. The study introduces a hybrid strategy combining shape-guided segmentation (SGS) and M3D-neural cellular automata (M3D-NCA), resulting in improved computational efficiency and performance. The implementation of Shape-guided segmentation (SGS) during the initialization phase, coupled with the elimination of convolutional layers, enables the model to effectively reduce computation time. The research proposes a novel loss function that combines segmentation losses from both components for effective training. The robust technique provided aims to improve the accuracy and consistency of breast tumor segmentation, leading to significant improvements in medical imaging and breast cancer detection and treatment. This study enhances breast cancer segmentation in medical imaging using CAD systems. Combining shape-guided segmentation (SGS) and M3D-neural cellular automata (M3D-NCA) is a hybrid approach that improves performance and computational efficiency by dealing with complex data and not having enough training data. The approach also reduces computing time and improves training efficiency. The study aims to improve breast cancer detection and treatment methods in medical imaging technology.

Abstract:Computed tomography image (CTI) sequence is essentially a time-series data that typically consists of a large amount of nearby and similar CTIs. Due to the high communication and computational costs, it is difficult to perform a progressive distributed similarity retrieval of the large CTI sequence (CTIS)s, particularly in resource-constraint mobile telemedicine network (MTN)s. In this paper, we present a Dprs method—progressive distributed and parallel similarity retrieval scheme for the CTISs in the MTN. To the best of our knowledge, there is little research on the Dprs processing, especially in the MTN. Four supporting techniques (i.e., (1) PCTI-based similarity measurement, (2) lightweight privacy-preserving strategy, (3) SSL-based data distribution scheme, and (4) the UDI framework) are developed. The experimental evaluation indicates that our proposed Dprs method is more progressive than the state of the art, with a significant reduction in response time.

Abstract:The rapid evolution of telehealth, or telemedicine, has spurred crucial technological advancements aimed at addressing the early stages of complex cancer conditions, where conventional diagnostic methods face challenges. This research introduces a cancer detection system that utilizes Internet of Things (IoT)-based patient records and machine learning. The primary objective is to automate real-time breast cancer monitoring and detection in residential institutions and smart hospitals, thus enhancing the delivery of quality cancer healthcare. Background: Traditional diagnostic methods, particularly physical inspection, exhibit inherent limitations in identifying breast cancer at early stages. This research responds to this challenge by leveraging innovative technologies, such as IoT and deep learning-based techniques, to overcome the constraints of conventional approaches. Objective: The primary goal of this study is to develop and implement a cancer detection system that integrates IoT-based patient records and machine learning for real-time breast cancer monitoring in residential and healthcare settings. Method: The research employs a synergistic combination of IoT technology for collecting images of residential users and Convolutional Neural Network (CNN), a deep learning technique, for early cancer prediction. The focus lies on contributing to the overall well-being of individuals who may unknowingly be living with cancer. Result: Simulated outcomes after 25 epochs are presented, emphasizing the training accuracy of the model and its validation accuracy using the proposed VGG16 classifier. Graphical representations of the results indicate consistent performance metrics, with both validation and training accuracy exceeding 99%. Specifically, the training accuracy measures at an impressive 99.64%, while the validation accuracy stands at 99.12%. Main Findings: The study demonstrates the effectiveness of the integrated IoT and deep learning techniques in achieving high accuracy rates for early breast cancer prediction. The findings affirm the potential of this approach to assist dermatologists in identifying breast malignancies at treatable stages. Conclusion: This research establishes a foundational framework for the integration of IoT and deep learning techniques, presenting a promising avenue for advancing early cancer detection in smart healthcare systems. The proposed cancer detection system holds significant potential for improving healthcare outcomes and contributing to the overall well-being of individuals at risk of breast cancer.
High Performance Breast Cancer Diagnosis From Mammograms Using Mixture of Experts With EfficientNet Features (MoEffNet)

Abstract:As breast cancer is a leading cause of death for women globally, there is a critical need for better diagnostic tools. To address this challenge, we propose MoEffNet, a cutting-edge framework that offers high-performance breast cancer diagnosis. MoEffNet is characterised by its innovative hybrid integration of EfficientNet and Mixture of Experts (MoEs), using two powerful techniques developed to enhance accuracy and efficiency. EfficientNet, known for its robust feature extraction capabilities, utilises compound scaling and depth-wise separable convolutions to capture image features across multiple levels of abstraction. This is combined with MoEs framework, which employs specialised expert networks to analyse distinct aspects of mammograms. MoEffNet analyses features at various levels: low-level for basic patterns, mid-level for detailed analyses, and high-level for complex contents. Features extracted from various EfficientNet model stages are assigned to specialised experts to optimise diagnostic precision. A dynamic gating mechanism (EffiGate) is introduced to ensure that the most relevant experts contribute to each diagnostic decision, by dynamically adjusting their influence based on input data characteristics. This approach ensures that the most effective experts are utilised for each case, resulting in superior accuracy. The scalability of MoEffNet is highlighted by its ability to adapt to various computational constraints and accuracy requirements, using EfficientNet's architecture, which ranges from B0 to B7 models. We have validated MoEffNet's effectiveness on three mammographic datasets (MIAS, CBIS-DDSM, and INbreast) achieving outstanding results (AUC >0.99 across all datasets), outperforming existing methods. Particularly, EfficientNet B1 and B2 models with three or four experts achieved the highest accuracy, demonstrating MoEffNet's potential as a robust diagnostic tool for early breast cancer detection. Through its innovative hybrid model, robust feature extraction, dynamic gating, and specialised expert networks, MoEffNet sets a new benchmark in automated mammogram analysis, offering a powerful tool for more accurate and reliable breast cancer diagnosis.

Abstract:Objective Skin cancer is among the most common cancers worldwide.Skin cancer screening relies primarily on visual inspection by dermatologists,and largely depends on their experience.However,imbalances in regional development have led to an uneven distribution of medical resources worldwide.Telemedicine is an effective approach to alleviate this dilemma,and a major branch of this field is teledermatology.Dermatologists can remotely view clinical images and medical histories of skin cancer patients in various ways and provide remote diagnosis and treatment suggestions.However,teledermatology relies on medical experts and network conditions,and patients who cannot access the Internet in remote areas cannot enjoy the convenience of remote consultation.Some portable devices based on automatic diagnostic algorithms have made up for some shortcomings of traditional teledermatology;however,because they can only judge the type of disease,these devices have limited clinical applicability.To compensate for the shortcomings of the current research,our team design and build a skin tumor artificial intelligence-enhanced teleconsultation system,which has both offline automatic skin tumor screening and online remote consultation and preoperative planning functions. Methods In this study,we build a skin tumor intelligent teleconsultation system with two typical application scenarios:1)skin tumor self-screening without network conditions.Patients or doctors who lack experience at the local site can use the dermatoscope in the system to obtain images of skin lesions and then use the deep learning algorithm in the system to generate automatic diagnosis results.2)Remote skin tumor consultation under network conditions.Remote consultation for skin tumors involves real-time interactions among inexperienced doctors at a local site and experienced medical experts at a remote site.First,the doctor at the local site uses a dermatoscope or ordinary camera in the system to obtain skin images of patients with skin tumors.Then,the deep learning algorithm in the system generates automatic diagnosis results based on the images.Finally,the remote expert confirms the disease diagnosis result and recommends a corresponding treatment plan based on network-transmitted images and algorithmic cues.For patients requiring surgery,both parties can use the system for preoperative planning.Instructive annotations drawn by remote doctors on the screen are transmitted over the network to the local site and projected onto the body surface of the patient in situ.To achieve the above functions,we first train a RegNetY-800M model based on 7-point dataset and deploy it on Raspberry Pi,also using a neural network computing accelerator to accelerate neural network computation,then use camera,laser projector,and beam splitter to form a co-axial projective imaging design that can project instructive annotations made by remote experts with high accuracy onto patient's body surface,finally design a visual software interface for easy use by doctors.To characterize the system,we first design a benchtop experiment to quantify the achievable accuracy of the system,then design a control experiment to verify whether the system can improve the applicability and efficiency of the traditional teleconsultation system.Finally,a clinical experiment is designed to verify the clinical applicability of the system. Results and Discussions The benchtop experimental results show that the maximum projection error of the system is less than 1.5 mm(Fig.4),and the CIEDE2000 value after color correction is less than 2,which can accurately restore colors in the scene(Fig.5).The control experimental results show that there is no significant difference between the algorithm in the system and dermatologists;dermatologists perform better with AI prompts.Clinical experimental results show that using this system for automatic diagnosis and remote consultation of skin tumors is feasible. Conclusions The intelligent teleconsultation system for skin tumors designed and built by our team has both automatic disease screening without a network and remote consultation with the network.The control experimental results show that the diagnostic results of the algorithm deployed in the system are comparable to those of dermatologists and that the algorithm can help dermatologists make more efficient and accurate decisions.The experimental results show that the system has clinical applicability.Compared to traditional remote consultation systems,this system has the following three advantages.1)It has a wider application range.In remote areas without a network,the system can serve as a supplement to dermatologists,compensating for defects in which traditional teleconsultation systems cannot be used without a network.2)Better performance.Automatic diagnosis results in the system can assist specialists in a more efficient and accurate screening of skin tumors.3)Intuitive display.This system can project annotations made by remote experts onto body surfaces of patients with high precision,thereby making remote guidance in preoperative consultations and surgical planning processes more intuitive and precise.This system can help patients in areas with scarce medical resources perform early screening for various diseases,such as skin tumors.
