Abstract:The routine in our department for years used to be: prevention of bearing down during the end of the 2nd stage of labor in high myopic parturients, by forceps delivery, with the assumption that this will prevent increased intraocular pressure--thus preventing deterioration or increased damage to the eyes. The goal of this study was to challenge our theory and thus we decided to re-evaluate this approach. Various obstetrical data were recorded in 50 women with myopia from 4.5 to 15.0 diopters who were admitted in labor to our department. Fundus examination was performed in all of them before and after delivery. Various types of retinal degeneration and retinal breaks were observed in most of them at their arrival but no deterioration of these retinal defects was observed in any of the cases at the later examination. In view of our results, it is recommended to let high myopic patients deliver spontaneously.

Abstract:The review highlights aspects of the evolution of the views and opinions of ophthalmologists of different years on ophthalmological and obstetric approaches to the management of pregnancy and childbirth in women with high myopia. According to the literature, prevention of the risk of regmatogenic retinal detachment during childbirth since the early 1980s of the last century has been aimed at identifying peripheral vitreochorioretinal dystrophy during pregnancy as the leading risk factor for regmatogenic retinal detachment during childbirth. The main modern strategies for the management of pregnant women with the presence of peripheral vitreochorioretinal dystrophy during pregnancy and childbirth are considered; indications for natural delivery with high-grade myopia, absolute and relative indications for its implementation in a natural way are presented. This is reflected in the specific selection criteria for natural delivery. The effect of normal childbirth on eyes with abnormalities predisposing to rhegmatogenous retinal detachment

Abstract:Background: Pregnant women who have high myopia, a history of retinal detachment or retinal holes, or have known lattice degeneration are frequently referred to an ophthalmologist for advice concerning the management of pregnancy and labor, i.e. whether a spontaneous vaginal delivery can be allowed and whether prophylaxis for high-risk retinal pathology is indicated. Many obstetricians still believe that pregnant women with ocular abnormalities predisposing to rhegmatogenous retinal detachment should have an instrumental delivery, and a few even advocate cesarian section. Very little has been written about the management of pregnant women with high-risk retinal pathology, and opinions differ considerably. Patient data on this subject are scarce. Methods: We studied 10 women who had 19 deliveries (10 prospective and 9 retrospective) and who had a history of retinal detachment, had been diagnosed as having extensive lattice degeneration, or had been treated for symptomatic retinal holes or breaks. The women were followed from the third trimester of pregnancy through labor and delivery into the postpartum period, looking for changes in the retinal status. Results: We found no changes in the retinal status in the postpartum examination. Conclusion: We conclude that prenatal treatment of asymptomatic retinal pathology is not indicated and that spontaneous vaginal delivery may be allowed to take place in women with high-risk retinal pathology.

Abstract:The aim of the study was to investigate the effects of ephedrine intrathecal injection on maternal thermodynamics and spinal nerve block in cesarean section. A total of 107 patients undergoing elective cesarean section under combined spinal epidural anesthesia were randomly divided into two groups: group E: Bupivacaine (6 mg) combined with ephedrine (15 mg) and group C: Bupivacaine (6 mg). Systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), the incidence of hypotension, hypertension, tachycardia, nausea and vomiting were all recorded. The umbilical vein blood gases and noradrenaline, as well as maternal peripheral venous blood were analyzed. The time of high-quality spinal block was also recorded. Postoperatively, 1 day, 2 days bedside follow-up and 7, 21, 35 days telephone follow-up were carried out to determine any neurological deficit maternal performance. SBP and DBP decreased in group C but not in group E at 3 min after spinal anesthesia and 1 min after delivery (P<0.05). From 3 min post-anesthesia to the end of surgery, the HR levels in the C group were significantly higher than those of the E group (P<0.05). The incidence of hypotension, tachycardia, nausea and vomiting and neonate acidosis was low in group E compared with group C (P<0.05). The time of high-quality spinal block in group E was longer than that of group C (P<0.05). After spinal anesthesia, the concentration of noradrenaline from maternal peripheral venous blood was low in group C but high in group E compared with before spinal anesthesia (P<0.05). Ephedrine combined with low-dose bupivacaine intrathecal injection effectively maintained maternal hemodynamics and prolonged the time of sensory spinal block.
[Labor monitoring and indications for abortion and cesarean section in eye diseases--an overview]

Abstract:Background: Management of labor and indications for abortion in ophthalmologic diseases are discussed controversially in literature. Overtreatment is still frequent. In diseases like pseudotumor cerebri, high myopia, retinal detachment, retinopathy in EPH-gestosis, uveal melanoma and diabetic retinopathy, the question for optimal management of labor or interruption rises mainly from gynaecologists. Methods: In this review these conditions will be discussed with the literature--obtained by a medline-research. Conclusions: In the ophthalmologist's point of view--there are no indications for interruption or section caesarean in patients with above mentioned ophthalmologic diseases.
Effect of epidural anesthesia on the optic nerve sheath diameter in patients with pre-eclampsia: a prospective observational study

Abstract:Introduction Optic nerve sheath diameter (ONSD) reflects intracranial pressure and is increased in pre-eclampsia. Administrating a significant volume of epidural solution into the epidural space can potentially increase ONSD. We investigated the impact of epidural local anesthetic injection on ONSD in patients with pre-eclampsia. Methods Patients with pre-eclampsia (n=11) and normotensive pregnant women (n=11) received de novo epidural anesthesia for cesarean delivery. We administered 21 mL of an epidural solution containing 2% lidocaine and 50 μg fentanyl into the lumbar epidural space in incremental doses. ONSD was measured at baseline, 3, 10, and 20 min after completing the epidural injection, after delivery, and at the end of surgery. Primary outcome was the change in ONSD from baseline to 3 min after epidural injection in patients with pre-eclampsia and normotensive pregnant women. Serial changes in the ONSD were analyzed using a linear mixed model. Results At baseline and 3 min after epidural drug injection, ONSD was significantly larger in patients with pre-eclampsia than in normotensive mothers (5.7 vs 4.1 mm, p=0.001 and 5.4 vs 4.1 mm, p 0.999). Linear mixed model demonstrated that ONSD did not change after epidural anesthesia in either group (p=0.279 and p=0.347, respectively). Conclusions Despite a higher baseline ONSD in pre-eclampsia, epidural anesthesia did not further increase ONSD. Our findings indicate that epidural anesthesia can be safely administered in patients with pre-eclampsia at risk of increased intracranial pressure, without other intracranial pathology. Abstract:This review provides information on the features, incidence, pathogenetic mechanism, possibilities of conservative, laser and surgical treatment of glaucoma in pregnant women. The purpose of this literature review is to study the features of managing patients with glaucoma during pregnancy. Glaucoma is a large group of eye diseases characterized by a gradual development of visual impairments and atrophy of the optic nerve. The pathology is most common among the population over 40 years old, but due to the advancement of reproductive technologies, the availability of diagnostic methods, as well as high clinical awareness, the frequency of ophthalmologists managing glaucoma in pregnant women increases. Elevated intraocular pressure in pregnant women is becoming more common and, therefore, is an urgent problem. There is a trend towards an increase in the number of pregnant women who experienced elevated intraocular pressure for the first time. Previously identified pathology in childhood or secondary glaucoma due to previous ophthalmic diseases or severe somatic pathology is a special category of patients. Such patients are encountered regularly and require close supervision by both an ophthalmologist and a gynecologist. The evidence base for antiglaucoma drugs is small; there are clinical experiments on animals, which results do not always carry over to humans, and retrospective studies. The doctor's arsenal presently includes a number of drugs that have possible side effects for the fetus, as well as laser and surgical treatment. There is no standard treatment, so each case should be considered individually with an understanding of the physiology, mechanism of action of the drugs and their possible effects. Therefore, glaucoma during pregnancy and lactation is a serious medical problem, and its solution depends on interdisciplinary medical approach.

Abstract:Aim: As a medical condition, pregnancy mandates the simultaneous treatment of both the mother and the fetus, making it a distinctive aspect of clinical medicine. Material and Method: We analyze the physiological changes occurring in the eyes and brain during pregnancy, as well as the neuro-ophthalmological manifestations that can occur during pregnancy. Studies published in both English and other languages, case reports, and reviews from 2011 to 2023 onwards were included. All surveys were acquired by exploring the databases. Results: We found a total of 2135 articles that showcase neuro-ophthalmic changes related to pregnancy: review and research articles (Science Direct 804, Web of Science 923, Scopus 345, and 63 Pub Med). In total, 86 studies were examined after applying the inclusion and exclusion criteria. Bilateral papilledema can be a warning sign for intracranial hypertension or cerebral venous sinus thrombosis. Additionally, when unilateral, it is important to differentially diagnose anterior ischemic optic neuropathy secondary to a hypercoagulant, compressive or inflammatory optic neuropathy, optic neuritis, or even orbital pseudotumor state. Severe eclampsia and preeclampsia can manifest as choroidal infarction, serous retinal detachment, and even cortical blindness. There can also be implications at the level of cranial nerves or transient Horner syndrome. Conclusions: Evaluating and treating a pregnant woman with neuro-ophthalmological manifestations is challenging. The obstetrician closely follows and has a medical relationship with the pregnant woman; hence he/she might be the first to be informed about the general condition of the pregnant woman or might request an ophthalmologic examination tailored to each specific case.

Abstract:Pregnancy causes changes in all body systems, including the eye. The eye can undergo physiological and pathological changes in pregnancy. Some changes exacerbate pre‐existing eye conditions while other conditions manifest for the first‐time during pregnancy. Early recognition and management are essential to prevent sight threatening complications. In addition, some obstetric complications can be associated with ophthalmic signs. Prompt recognition of these eye findings may be life saving for both the mother and the foetus. The aim of this article is to present potential ocular complications in pregnancy and outline the appropriate management to preserve sight and maintain maternal and foetal safety. The safety of the use of common ophthalmological medications will also be discussed.

Abstract:Meningioma in the parasellar region may lead to visual impairment, so intraoperative neurological monitoring is essential for enucleation surgery. However, intraoperative neurological monitoring in pregnant women is challenging, as the anesthesia management must consider the effects and risks to the fetus. Remimazolam is a newly introduced intravenous anesthetic that has little effect on blood pressure. However, the effects of remimazolam on intraoperative neuromonitoring are little known. We treated a pregnant patient with parasellar meningioma who developed visual impairment, using remimazolam for anesthesia and intraoperative neurophysiological monitoring of the visual evoked potential.

Abstract:<span class="a-plus-plus abstract-section id-a-sec1"><h3 class="a-plus-plus">Background</h3><p class="a-plus-plus">Hypertensive disorders of pregnancy lead to pathological changes in various organ systems of mother and fetus which contributes to maternal and fetal morbidity and mortality. It is a multisystem disorder which can involve end organs like kidneys, liver, eyes, hemopoietic system and placenta. Retinal involvement, though quite common, is rarely investigated. It is a unique site where the blood vessels can be directly observed. Observing retinal vasculature may provide clue to status of similar vessels in other parts of body including placental circulation. </p></span><span class="a-plus-plus abstract-section id-a-sec2"><h3 class="a-plus-plus">Methods</h3><p class="a-plus-plus">Hospital-based descriptive study which included 150 patients diagnosed as preeclampsia/eclampsia, who were admitted in the intensive care unit. Written consent was obtained from study subjects. Data were collected by history taking and examination of the subjects using pre-structured questionnaire. Ocular examination was carried out by ophthalmologist, which included anterior segment examination, visual acuity and dilated fundus examination. Multiple qualitative and quantitative parameters were studied. Data were analyzed by SPSS 14.0 software.</p></span><span class="a-plus-plus abstract-section id-a-sec3"><h3 class="a-plus-plus">Results</h3><p class="a-plus-plus">Ocular symptoms were seen in 22% of severe preeclampsia and in 100% of eclampsia patients. Blurred vision was the most common ocular complaint. Fundus changes were seen in 48.7% total study subjects. Arteriolar narrowing was the most common finding on fundus examination. Systolic blood pressure and serum creatinine were found predictive of changes in fundus (<em class="a-plus-plus">p</em> = .000). Incidence of fetal growth restriction was found to be significantly associated with fundus involvement (<em class="a-plus-plus">p</em> value .000).</p></span><span class="a-plus-plus abstract-section id-a-sec4"><h3 class="a-plus-plus">Conclusion</h3><p class="a-plus-plus">Ophthalmic examination including fundus examination should be a routine in the investigational armamentarium of hypertensive disorders of pregnancy.</p></span>

Abstract:Laryngeal views obtained during direct laryngoscopy with and without manual in-line neck stabilization (MILNS) and during video-assisted intubation with MILNS using the angulated video-intubation laryngoscope were assessed in 100 paediatric patients (aged 0.25-17.3 yr). Visualization of the larynx (Cormack and Lehane score) as well as time taken for video-assisted tracheal intubation by six nurses and four resident anaesthetists not experienced in the technique were recorded. Cormack and Lehane scores were significantly worse during direct laryngoscopy when MILNS was applied. Video-assisted visualization of the larynx during MILNS produced scores, which were as good or better than those observed during direct laryngoscopy alone. Intubation times ranged from 19-75 s (mean 35 (SD 13.4); median 32).

Abstract:Pregnancy introduces a multitude of changes in the body, including hormonal fluctuations and metabolic changes, which can lead to atypical ocular signs and symptoms. Ocular manifestations range from fluctuations in vision, to microstructural changes in the retina and choroid, to dry eye disease. This narrative review highlights the range of pregnancy-related effects on the eye and vision that are likely to present in the context of routine eyecare. Specifically, physiological ocular changes and pathological ocular changes that manifest for the first time, or are exacerbated, in uncomplicated pregnancy are discussed. The literature has evolved from simply noting differences in the eye between pregnant and non-pregnant groups, to refining knowledge of the proposed underlying pathophysiology with the advent of newer technologies in eyecare. A particular focus of this review is navigating when pregnancy changes in the eye occur or peak during the gestational period, and whether the changes are short-lived or might extend past pregnancy. While many pregnancy-associated changes are temporary and resolve post-partum, it is also recognised that some changes persist after pregnancy, with a notable absence of literature on ocular changes with loss or termination of pregnancy. Currently or previously pregnant women (or those planning to become pregnant), and other health professionals, should be educated about the importance of seeking eyecare before, during and after pregnancy.

Abstract:Objectives: The article aims at gaining in-depth knowledge of physiological and anatomical changes in pregnancy. Methods: A randomized controlled trial was done among 200 pregnant woman in Department of Obstetrics and Gynecology at Tertiary care institute and hospital for the period of 6 months (November 2021 to April 2022). Random Number table was used for selection of patients who were followed for pregnancy in the department. All demographic details and Pre-Anesthetic checkups were done. Statistical analysis was done using the software Statistical Package for the Social Sciences (SPSS) 22.0 version. This consecutive series of original research papers were reviewed for the reporting of ethics committee approval and patient consent. Results: Respiratory physiology continues to change throughout pregnancy. Airway resistance decreases 50% due to the effects of progesterone on bronchial smooth muscle. With Minute ventilation, RR, Tidal volume, and Alveolar ventilation are increased. Anatomically, diaphragm elevates with cephalad pressure of the growing gravid uterus, and the chest wall diameter increases in the anteroposterior and transverse diameters. This leads to unchanged Total Lung Capacity, Vital Capacity, and CC, while Expiratory Reserve Volume, Residual volume, and Functional residual capacity seen a decreased in 20% in all patients. Blood volume increases early in gestation (at approximately 12 weeks), continues to increase rapidly during the second trimester and more slowly during the remainder of pregnancy. Cardiac output increases in early pregnancy with changes measurable by 8 weeks gestation. Conclusion: Minute ventilation, RR, Tidal volume, and Alveolar ventilation are increased. Cardiac output increases in the early pregnancy with changes measurable by 8 weeks gestation. Cardiac output increases most rapidly (to approximately 35% above baseline) during the second trimester, then remains about the same until term. Output increases substantially with labor contractions; each contraction increases cardiac output an additional 10–25%.

Real-time imaging as visual biofeedback in active second stage of labor among nulliparas: a randomized controlled trial

Abstract:Background A prolonged second stage of labor increases the risk of severe perineal laceration, postpartum hemorrhage, operative delivery, and poor Apgar score. The second stage is longer in nulliparas. Maternal pushing during the second stage of labor is an important contributor to the involuntary expulsive force developed by uterine contraction to deliver the fetus. Preliminary data indicates that visual biofeedback during active second stage hastens birth. Objective The objective of this study was to evaluate if visual feedback of the perineum reduced the length of active second stage of labor in comparison to control. Material and Methods A randomized controlled trial was conducted in University Malaya Medical Center from December 2021 to August 2022. Nulliparous women about to commence active second stage, at term, with singleton gestation, reassuring fetal status and no contraindication for vaginal delivery were randomized to live-viewing of maternal introitus (intervention) or maternal face (sham/placebo control) as visual biofeedback during their pushing. A video camera Bluetooth-linked to a tablet computer display screen were used: in the intervention arm the camera was focused on the introitus and for the control arm, on the maternal face. Participants were instructed to watch the display screen during their pushing. The primary outcomes were the intervention to delivery interval and maternal satisfaction with the pushing experience assessed using 0-10 visual numerical rating scale. Secondary outcomes included mode of delivery, perineal injury, delivery blood loss, birthweight, umbilical cord arterial blood pH and base excess at birth, Apgar score at 1 and 5 minutes and neonatal admission. Data were analyzed with the t-test, Mann-Whitney U test, Chi square test and Fisher exact test as appropriate. Results 230 women were randomized (115 to intervention arm and 115 to control arm). The active second stage duration (intervention to delivery interval) was median[interquartile range] 16[11-23] vs. 17[12-31] minutes p=0.289 and maternal satisfaction with pushing experience was 9[8-10] vs 7[6-7] P<0.001 for intervention and control arm respectively. Women randomized to intervention arm were more likely to agree to recommend their management to a friend: 88/115(76.5%) vs. 39/115(33.9%) RR 2.26 (95%CI 1.72-2.97), p<0.001 more likely to sustain a pattern of less severe perineal injury (p=0.018). Conclusion Real-time viewing of the maternal introitus as visual biofeedback during pushing resulted in higher maternal satisfaction compared with sham control of viewing the maternal face but the time to delivery was not significantly shortened.

[Chemistry and antitumor activity of hainanensine analogues].

Abstract:A synthetic analogue of hainanensine 2a was further modified in order to improve its antitumor activity. Six compounds 3-8, were obtained, and among these compounds 8 showed significant activity. The diastereomer (+/-)8B was resolved by (+) tartaric acid and (-)benzoyl-tartaric acid to the corresponding enantiomers (-)8B and (+)8B, respectively and their optical purities were determined by isotope dilution method as 93% and 96%. The antitumor activity of the newly obtained compounds were tested in vitro, and (-)8B was found to have an IC50 of 1.9 mol/L, eighteen times higher than the original compound 2a.

Abstract:Pre-eclampsia/eclampsia during pregnancy and the puerperium are major risk factors for maternal and perinatal morbidity and mortality worldwide. Early diagnosis followed by appropriate treatment can prevent neurological disorders, considered one of the most serious sequelae of the disease. The detection of increased intracerebral pressure through the use of ocular ultrasonography could be considered an effective method for its diagnosis, since it has the advantage of being a noninvasive technique, easy to perform at the patient's bedside and with a high sensitivity and specificity for the detection of intracranial hypertension.
