Abstract:Adverse demographics in Korea impinges on its growth potential and fiscal outlook. Accordingly, this study examines the current demographic situation and recent projections related to the impacts of population aging in Korea, particularly on the looming fiscal imbalance. The focal conclusion is that a two-way effect exists from population aging. First is the anticipated stress placed on government finances due to increasing welfare expenditure for the elderly. Second is sluggish economic growth and thus the inability to collect sufficient government revenues. The prospect of large and growing deficits is therefore immediate and potentially long lasting as governments will be faced with rising spending demands and sluggish tax revenues arising simultaneously from an aging population.

Abstract:Korea is one of the fastest-aging societies in the world. According to the Korea National Statistical Office, the number of those aged 65 years and older will dramatically increase from 9.1% in 2005 to 37.3% in 2050. It is very evident that Korea must urgently prepare itself for new times. The government insurance policy for elderly people, now called long-term care insurance, is ultimately designed for providing home- and facility-based supports to seniors with geriatric diseases and dementia, as well as for reducing the support burden on other family members. This article is to introduce the long-term care insurance of Korea and thereby to advocates the necessity of international discussion of the prospects for developing health care for aging populations; its aim is to encourage the sharing of differing national experiences concerning care for the elderly.

Remembering sacrifices: attitude and beliefs among second-generation Korean Americans regarding family support

Abstract:Korean immigration peaked in the mid-1980s, so that large cohorts of post-1965 immigrants are now approaching or entering retirement. As the baby boomer generation ages, few studies have examined how the lack of retirement savings and eldercare plans combined with cultural expectations such as filial piety may pose challenges for aging Korean immigrants and their adult children. This exploratory study examines attitudes and beliefs among 1.5 and 2nd generation Korean American adults regarding filial expectations and support for aging immigrant parents. In-depth interviews conducted with 124 adult children of immigrants show that their attitudes and beliefs around filial care were primarily motivated by feelings of gratitude and a strong sense of responsibility toward their parents. In addition, because Korean immigrant parents often face language and financial barriers, adult children were preparing themselves for future support of their parents' finances, health care and long-term care needs. Although both adult sons and daughters expressed a desire to care for their parents, adult daughters often discussed in detail their concerns and worries about future care of their parents. The findings of this paper illustrate how the intersections of gender, culture, and class inform attitudes and beliefs regarding aging and family support among Korean American families.
Public Support, Family Support, and Life Satisfaction of the Elderly: Evidence from a New Government Old-Age Pension in Korea

Abstract:Population aging is a global phenomenon occurring both in developed and less developed countries. While families are still playing an important role in providing support for elderly people, governments are also expanding their public old‐age support programs in many societies. Public pensions are one of the major policy tools geared to social protection of the elderly. However, little is known about how the programs affect elders particularly in terms of their subjective well‐being. Such effectiveness depends in part on the extent to which public pension income displaces or 'crowds out' family elder support. Using the introduction of the Basic Old‐Age Pension (BOAP), a non‐ contributory old‐age pension in Korea, this dissertation examines the relationship among public support, family support, and life satisfaction of the elderly. As an introductory chapter, chapter 1, " The continuing importance of children in relieving elder poverty: evidence from Korea, " describes the actual financial status of elderly Koreans and the amount of financial support they receive from children. Analysis of the 2006 Korean Longitudinal Study of Ageing shows that almost 70 percent of Koreans aged 65 or more years received financial transfers from children and that the transfers accounted for about a quarter of the average elder's income. While over 60 percent of elders would be poor without private transfers, children's transfers substantially mitigate elder poverty, filling about one quarter of the poverty gap. v Furthermore, children's transfers to low‐income parents tend to be proportionally larger, so elder income inequality is reduced by the transfers. Over 40 percent of elders lived with a child and co‐residence helps reduce elder poverty. Using the introduction of BOAP, chapter 2, " Does money buy happiness?: Evidence from a new government pension in Korea, " assesses how the program affected elders' life satisfaction. Notably, this study adds valuable evidence to the literature of whether money buys happiness, a question of great interest but notoriously difficult to answer given the difficulties associated with isolating income as a causal factor. To make the causal inference, this chapter utilizes a difference‐in‐difference research design and analyzes longitudinal data from the Korean Retirement and Income Study (KReIS). Results show that BOAP did raise elders' life satisfaction: annual benefits of 1,000 kW, approximately 1,000 U.S. dollars, raised elders' life satisfaction by 0.16‐0.22 of one standard deviation and the effect was statistically significant. Monitoring healthy ageing for the next decade: South Korea's perspective

Abstract:South Korea is the fastest ageing country among OECD countries. Unlike the older generation growing up in the aftermath of the Korean war, the first and second baby boomer generations have heightened expectations regarding public services. In addition to the demand in higher quality of both social and health services by these newer older population, there is a concomitant increased quantitative demand. It is imperative that Korea reimagines their health, social welfare and economic policies to reflect the rapidly changing needs of such generations. One way to do this is to mainstream and continually monitor healthy ageing in all aspects of future policies. In 2021, the Korean Longitudinal Healthy Aging Study was launched in this context, to better understand the needs of the new-older age generation and to produce evidence to support formulation of better tailored policies that could promote healthy ageing. However, Korea is only in its early stage in developing a monitoring system that looks into the performance level of policies that support healthy ageing. As a country that is preparing for such rapid demographic transition and has already commenced developing its healthy ageing indicators, it will be important to assess and monitor uniformly the level of healthy ageing from the framework perspective of WHO. Korea welcomes WHO's development of an internationally applicable M&E framework for healthy ageing. We hope that WHO's M&E framework on healthy ageing will help Korea align to the international standards in its journey through the UN Decade of Healthy Ageing 2021-2030 and beyond.
Inclusive Aging in Korea: Eradicating Senior Poverty

Abstract:Poverty for the elderly is one of the most urgent social problems when discussing the social problems facing Korean society. The purpose of this study is to identify the causes of elderly poverty problems and to seek countermeasures. According to a systematic analysis of the economic difficulties of the elderly population that applies a socioecological model, the cause of elderly poverty is complicated by the specificity of the labor market and pension system in Korean society. This is compounded by the lack of a public support system that can overcome insufficient family care and a lack of individual preparation. To alleviate elderly poverty, this paper recommends three policy alternatives. First, a robust multipillar retirement income security system must be established. To secure a minimal retirement income for the elderly in poverty, who have been marginalized from the public pension system design, the basic pension should be raised for the bottom 70% of senior citizens. Second, in order to tackle labor market duality and early retirement, the seniority-oriented wage system should be reformed into a job-based wage system. Third, to minimize unemployment and promote quality among re-employment jobs, the government should strengthen vocational skills development by expanding programs tailored to older people.

Abstract:Until recently in South Korea, the central dilemma facing children with ageing parents was how and by whom their parents should be cared for. In accordance with the norm of filial piety, the eldest son used to take responsibility. However, with the recent proliferation of long-term care hospitals, this arrangement is changing. These institutions, which play the combined role of rehabilitative hospital, long-term care centre, and nursing home, admit elderly people who do not require active medical intervention. The government’s promotion of these hospitals, centred on deregulation, ambiguity around their function, and the lack of alternative care facilities, has led to an expansion of the sector and consequently to the ‘nursing hom(e)fication’ of many of these institutions. While these hospitals ease the pressures associated with an ageing population, their mainstreaming has had an impact on healthcare, medicine, and the lives of elderly people. The hospital field has become commercialised, medical practice is being transformed, and the dignity of elderly people is being lost through hospitalisation. In this new care regime, filial piety itself is undergoing transformation—from an ideology underpinning the domestication of care, to the market idiom of service compliance. In this article, I introduce these hospitals and investigate how their growth has brought about a Korean style of elderly care commodification, revealing the undercurrents of healthcare privatisation and the neoliberalisation of welfare.
Determinants of Healthy and Active Ageing in Korea

Abstract:Based on a framework developed by the World Health Organization, we construct an individual-level percentage measure of healthy and active ageing employing the results of a unique survey of ten thousand elderly Korean respondents conducted in 2020 and relate its values to the senior respondent’s physical, lifestyle, and socio-economic characteristics. We find that the median value of our healthy and active ageing index is approximately 40%, suggesting significant room for improvement. An important role in interpreting our empirical results is played by the apparent role of Korea’s senior employment as a means of “making ends meet” rather than a way of improving the quality of one’s ageing, suggesting an important direction for government policy development. Our results underscore the importance of promoting higher-quality employment opportunities for senior citizens as opposed to creating these opportunities per se. This appears especially important given the fast pace of Korea’s ageing.

Abstract:Abstract Background A thorough examination of the morbidity and comorbidity profiles among the elderly and an evaluation of the related factors are required to improve the delivery of health care to the elderly and to estimate the cost of that care. In South Korea where the aged population is rapidly increasing, however, to date only one study using a limited sample (84 subjects) has provided information on morbidity and related factors among the elderly. Using a large, stratified, random sample (2,767 subjects) from the population-based Ansan Geriatric study, the present study sought to assess the morbidity and comorbidity, and to determine the relationships of these variables with sociodemographic and health characteristics in elderly people in South Korea. Methods A total of 2,767 subjects (1,215 men and 1,552 women) aged 60–84 years were randomly selected from September 2002 to August 2003 in Ansan, South Korea. Data on sociodemographic and health characteristics, and clinical diagnosis were collected using questionnaires. When available, the medical records and medications taken by the subjects were also cross-checked. Results Of the total subjects, 78.0% reported diagnosed disease, 11.0% had been cured, and 46.8% had been diagnosed with more than two diseases. The mean number of morbidities per person among elderly Koreans was 1.62 ± 1.35 (mean ± standard deviation), and women had a greater number of diseases per person than did men. The most common morbidities were chronic diseases such as hypertension, arthritis, and diabetes mellitus. In women, osteoporosis and arthritis were the second and third most prevalent diseases, respectively. Morbidity was significantly associated with gender, employment, household income, alcohol intake, self-assessed health status, and worries about health. Conclusion These data will enhance understanding of the patterns of health problems among elderly Koreans and will contribute to the application of appropriate intervention strategies.

Population Aging and Its Impact on Economic Growth -Implications for Korea

Abstract:In this paper, we systematically assess the impact of population aging on economic growth using Penn World Table 9.0’s newly available national-accounts growth rates, which is recommended by PWT for comparisons of growth rates across countries. Our empirical specification is an extended partial adjustment model which allows us to estimate the effects of demographic change in both the short run and long run. We use both five and ten year intervals for the period from 1960 to 2014. We find that population aging hampers economic growth in both short run and long run. We also find that elderly participation in the labor force has a positive influence on economic growth, which suggests that the harmful effect of aging can be mitigated by more active participation of the elderly in the labor force. Interestingly, we also find that the future level of population aging, not just the past level, has a detrimental effect on economic growth. Future aging may raise concerns about future growth prospects and thus adversely affect current economic activity. In addition, we project how aging will affect Korea’s future growth prospects. The projected growth rates decline continuously to －2.7％ twenty years from the last year of the sample（ in 2034）, which is even lower than －1.6％, Japan’s fitted growth rate for the most recent year. One possible explanation for why aging has a bigger impact on Korea than Japan is that aging is progressing even more rapidly in Korea than it did in Japan. Korea’s economic future may thus be even gloomier than Japan’s present because its demographic prospects are even worse. Lastly, we offer a number of policy options for Korea to mitigate the pronounced impact of population aging on economic growth. The Korean government has already actively taken a wide range of policy measures to address the aging challenge. Those measures are centered on efforts to boost fertility, but also include support for the elderly. Unfortunately, those measures have not been effective so far, as evidenced by the continued rapid progress of population aging. Going forward, one especially promising area of government intervention, in light of our empirical findings, is raising the participation of women in the workforce through a holistic package of policies that will enable women to better balance work and family. The Role of NGOs in Enabling Elderly Activity and Care in the Community: a Case Study of Silver Wings in South Korea

Abstract:South Korea is now one of the most rapidly aging societies in the world and meeting the elderly’s needs requires a systemic and paradigm change. In this study I argue that community-based care of the elderly in the community should be seen as complementary to, and arguably more fundamental than, enhancing long-term care services per se. Based on qualitative interviews and observations, this paper presents a case study of Silver Wings, the community-based initiative of local non-governmental organization (NGO) Saerom to provide older people’s self-help groups with support from HelpAge Korea. The study analyzes the activities and capacities of local NGOs in promoting “aging-in-community” and the findings suggest that such initiatives can certainly contribute to improving well-being and also to reducing long-term care needs. Lessons and challenges of the initiative, as well as policy suggestions, are also discussed.

Abstract:Abstract Age-friendliness studies in Korea have investigated a few subsets of the local community environments for older adults’ well-being. Little is known about comprehensively understanding which aspects in community contexts can help older individuals living alone improve their life satisfaction. To address the knowledge gaps, this study utilized the raw data from the 2020 National Survey of Older Koreans, and responses of 3,112 older adult single-person households were investigated. From an ecological perspective, hierarchical regression analysis was performed by distinguishing the community environments into three categories of the World Health Organization’s age-friendly environments: physical, social, and service environments. The results showed that the perception of the physical environment (housing, living facilities, and space) was positively associated with life satisfaction. Second, the social environment (social involvement, neighbor interaction, respect for older individuals, and political activity) was positively related to life satisfaction. Third, perceived service environment (difficulty in utilizing service) was negatively related to life satisfaction. Based on the findings, we proposed political and practical recommendations. Specifically, as a physical environment aspect, effective budget allocation and policy attention should be required for the autonomy and independence of older adults living alone in their daily lives by ensuring that housing and circumstances are suitable for aging-in-place. As a societal component, initiatives must be established to promote participation in decision-making processes that result in a more favorable social perception through social inclusion. Finally, for the service environment, we must advocate for increased accessibility to community supportive services (mobility, health care, food delivery, communication, and information).
Unmet healthcare needs of elderly people in Korea

Abstract:BackgroundElderly people often have more complicated healthcare needs than younger adults due to additional functional decline, physical illness, and psychosocial needs. Unmet healthcare needs increase illness severity, complications, and mortality. Despite this, research on the unmet healthcare needs of elderly people is limited in Korea. This study analysed the effect of functional deterioration related to aging on unmet healthcare needs based on the Korea Health Panel Study.MethodsThis cross-sectional study used data from the 2011–2013 survey of 8666 baseline participants aged 65 years and older. Unmet healthcare needs were calculated using a complex weighted sample design. Group differences in categorical variables were analysed using the Rao-Scott Chi-square test. Using logistic regression analysis, the association between unmet healthcare needs and aging factors was analysed.ResultsThe prevalence of unmet healthcare needs in Korean elderly was 17.4%. Among them, the leading reason was economic hardship (9.2%). Adjusting for sex, age, socioeconomic characteristics, and health-related characteristics, the group with depression syndrome was 1.45 times more likely to have unmet healthcare needs than that without depression syndrome (95% CI = 1.13–1.88). The group with visual impairment was 1.48 times more likely to have unmet healthcare needs than that without it (95% CI = 1.22–1.79). The group with hearing impairment was 1.40 times more likely to have unmet healthcare needs than that without it (95% CI = 1.15–1.72). The group with memory impairment was 1.74 times more likely to have unmet healthcare needs than that without it (95% CI = 1.28–2.36).ConclusionsThe unmet medical needs of the elderly are more diverse than those of younger adults. This is because not only socioeconomic and health-related factors but also aging factors that are important to the health of the elderly are included. All factors were linked organically; therefore, integrated care is needed to improve healthcare among the elderly. To resolve these unmet healthcare needs, it is necessary to reorganize the healthcare system in Korea to include preventive and rehabilitative services that address chronic diseases in an aged society and promote life-long health promotion.

Super Aging in South Korea Unstoppable but Mitigatable: A Sub-National Scale Population Projection for Best Policy Planning

Abstract:Abstract This research portrays the spatial and temporal progression of super-aging in regions throughout South Korea. Using a single-year population projection considering gross domestic migration, this research identifies which regions will shortly become a super-aged society. A cohort-component method with a migrant pool model is applied. The county-level national population registration data (2000–2018) are aggregated into 37 regions for the model run. In 2020, 16 rural regions will become super-aged societies. By 2029, all 37 regions, including the metropolitan areas, will join the group, with Sejong, the administrative capital, being the last to enter. In brief, the rural areas become super-aged earlier than the metropolitan areas, and within a decade, those 65 years old or older will make up the majority of the national population. Among all the metropolitan areas, Busan, the largest harbor city, will be the first to be super-aged in 2023. Sejong will experience the most radical change between 2020 and 2050. The research outcomes demonstrate that demographic changes in the rural and metropolitan areas are different; hence, the recent population policies, such as promoting fertility, may not work in the rural areas as they have already lost their population momentum due to the extreme and on-going urbanization throughout the nation. The unstoppable aging will pose adverse effects on future citizens (who are mostly senior) both financially and medically. An increase in health care expenditure and a nationwide blood shortage for transfusion are anticipated, for example.

Abstract:Korean elderly persons who immigrated to America tend to live apartfrom their adult children, contrary to the assumption that theywould adhere to their old custom of intergenerational family livingprevalent in Korea where the tradition of filial piety is stillinfluential. This paper explores factors associated with theelderly immigrants‘ living separate from their adult children andelicits the reasons they give for this residential pattern. Over100 elderly Korean-Americans were interviewed. Abstract:Frailty has been previously studied in Western countries and the urban Korean population; however, the burden of frailty and geriatric conditions in the aging populations of rural Korean communities had not yet been determined. Thus, we established a population-based prospective study of adults aged ≥ 65 years residing in rural communities of Korea between October 2014 and December 2014. All participants underwent comprehensive geriatric assessment that encompassed the assessment of cognitive and physical function, depression, nutrition, and body composition using bioimpedance analysis. We determined the prevalence of frailty based on the Cardiovascular Health Study (CHS) and Korean version of FRAIL (K-FRAIL) criteria, as well as geriatric conditions. We recruited 382 adults (98% of eligible adults; mean age: 74 years; 56% women). Generally, sociodemographic characteristics were similar to those of the general rural Korean population. Common geriatric conditions included instrumental activity of daily living disability (39%), malnutrition risk (38%), cognitive dysfunction (33%), multimorbidity (32%), and sarcopenia (28%), while dismobility (8%), incontinence (8%), and polypharmacy (3%) were less common conditions. While more individuals were classified as frail according to the K-FRAIL criteria (27%) than the CHS criteria (17%), the CHS criteria were more strongly associated with prevalent geriatric conditions. Older Koreans living in rural communities have a significant burden of frailty and geriatric conditions that increase the risk of functional decline, poor quality of life, and mortality. The current study provides a basis to guide public health professionals and policy-makers in prioritizing certain areas of care and designing effective public health interventions to promote healthy aging of this vulnerable population.
