Abstract:Silicosis is associated with preventable but irreversible lung damage. Early quantifiable assessment of silicosis workers would promote early interventional steps to reduce health deterioration. The objectives of this study were to correlate spirometry findings and post-six-minute walk test oxygen desaturation (post-6MWT OD) in chronic simple silicosis with age, duration of silica exposure (DSE), smoking pack years (SPY), occupation, and mean pulmonary artery pressure. Based on occupational exposure to silica and radiologic confirmation of chronic simple silicosis, 104 patients (all males) were enrolled and grouped based on SPY (nil, 1-10, 11-20, and >20) and occupation (drillers and dressers). They were further investigated with spirometry, post-6MWT OD, and transthoracic echocardiography (TTE), and findings were statistically analyzed. Abnormal spirometry findings were seen in 62.5% of total cases (65/104), with the highest percentage in the >20 SPY group (84%; 21/25) and drillers (68.3%; 28/41). The post-6MWT OD was seen in 50.96% of cases (53/104) with the highest percentage in the >20 SPY group (56%; 14/25) and drillers (63.4%; 26/41). Normal and restrictive patterns were predominant among <20 SPY groups, while obstructive and mixed patterns were prevalent in the >20 SPY group. Normal, obstructive, and restrictive patterns were predominant among dressers, while mixed patterns were in drillers. Mean age and mean DSE were higher for >20 SPY group and dressers; obstructive and mixed patterns; and patients with post-6MWT OD. Pulmonary hypertension was significantly associated with the presence of abnormal spirometry patterns (69.3%; 45/65) and post-6MWT OD (79.3%; 42/53). Mean forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) was significantly lower for the >20 SPY group. Mean FEV1 had an inverse relation with SPY, and mean FVC was lower for drillers than dressers. Spirometry, post-6MWT OD, and TTE assessment give a complete overview of the cardiopulmonary exercise capacity of chronic silicosis patients and facilitate early interventions with special consideration for workers involved in machine-based mining activity.

Abstract:Introduction: Pneumoconiosis is defined as a diffuse interstitial lung disease secondary to inhalation of mineral particles. Haemoptysis is common in pneumoconiosis. But it is usually secondary to bronchiectasis, bronchitis or pulmonary tuberculosis occurring with silicosis. Observation: A 74-year-old Portuguese man, known to have chronic respiratory failure secondary to chronic silicosis, was treated for moderate haemoptysis. CT angiography revealed bilateral fibrosing peri-hilar masses, inferior lobe nodules and previously known mediastinal lymphadenopathy, without active bleeding. Fibreoptic bronchoscopy showed an anthracotic appearance with lesions typical of silicosis on pathological examination of trans-bronchial biopsies and needle aspiration of mediastinal adenopathy by endobronchial ultrasound. Conclusion: After a full aetiological assessment, no cause other than silicosis has been identified. Haemoptysis is frequently observed in silicosis, but it results from silicosis complications. It is not a classic complication of isolated silicosis.

Abstract:Seventy‐two patients with soft tissue sarcoma arising in the oral cavity, oropharynx, larynx, parotid region, cheek, scalp, and neck, were entered on the Intergroup Rhabdomyosarcoma Study (IRS)‐I, and could be analyzed for survival. Sixty‐three (88%) attained complete remission (CR), of whom 13 subsequently relapsed (78% actuarial 5‐year retained CR rate). The patients with primary tumor arising in sites other than the neck had a retained CR rate exceeding 90%. In contrast, 11 relapses occurred in the 26 patients with neck primaries (54% retained CR rate). Females, and infants younger than age 24 months were more likely to relapse. Prognostic factors with little or no influence on relapse included tumor size, histology, regional lymph node status, clinical group, and treatment arm. Five of the 6 patients with failure at the primary site had either no radiotherapy (2 patients) or an insufficient dose (<3000 rad). No patient required major organ sacrifice such as laryngectomy or pharyngectomy. Isolated failure in regional nodes did not occur. Children with nonorbital, nonparameningeal head and neck soft tissue sarcoma treated in accordance with the IRS protocol have an excellent rate of local control and survival. Primary tumors arising in the neck are more likely to relapse locally or distantly.
Continued exposure to silica after diagnosis of silicosis in Brazilian gold miners

Abstract:Background: There is a paucity of studies analyzing the effect of continued silica exposure after the onset of silicosis with regard to disease progression. The present study investigates differences in clinical and radiological presentation of silicosis among former workers with a diagnosis of silicosis, and compares workers who continued to be exposed to silica with those who stopped silica exposure after having received their diagnosis. Methods: A sample of 83 former gold miners with a median of 21 years from the first diagnoses of silicosis, had their clinical and occupational histories taken and underwent both chest radiography (International Labor Organization standards) and spirometry. Their silica exposure was assessed and an exposure index was created. The main outcome was the radiological severity of silicosis and tuberculosis (TB). The statistical analysis was done by multiple logistic regression. Results: Among the 83 miners, 44 had continued exposed to silica after being diagnosed with silicosis. Continuation of silica exposure was associated with advanced radiological images of silicosis (X-ray classification in category 3, OR = 6.42, 95% CI = 1.20-34.27), presence of coalescence and/or large opacities (OR = 3.85, CI = 1.07-13.93), and TB (OR = 4.61, 95% CI = 1.14-18.71). Conclusions: Differential survival is unlikely to explain observed differences in silicosis progression. Results reinforce the recommendation that silica exposure should be halted at an early stage whenever X-ray is suggestive of the disease.

Abstract:OBJECTIVE:To investigate the selection of recipients, operative technique, and perioperative management of lung transplantation for silicosis.METHODS:Lung transplantations (LTx) were performed for five end-stage silicosis in our hospital who were diagnosed in accordance with recommendations of the local Prophylactic Therapeutic Institution for Occupational Diseases. The chest roentgenogram and high resolution CT showed somewhat pulmonary interstitial fibrosis, pulmonary emphysema and massive opacities. The mean pulmonary artery pressure (mPAP) was > 30mmHg, NYHA III or IV. Two patients received thoracic surgery prior to LTx, one patient was ventilator-dependent. One patient received bilateral sequence lung transplantation (BSLT) under extracorporeal membrane oxygenation (ECMO) support. Four patients received single lung transplantation (SLT), 3 under ECMO support.RESULTS:Patient five died of multiple organ failure on postoperative day 8, the remaining four patients were discharged from hospital. During follow up, patient three died of severe infection 7 month postoperatively, the remaining three patients were alive for 5 years, 3 years and 2 years respectively, and lived good quality of life, especially with lower mPAP and improved lung function. Although our patients suffered low-grade chronic rejection with the manifestation of bronchiolitis obliterative syndrome (BOS).CONCLUSION:Lung transplantation is a viable option for patients with end-stage silicosis, providing acceptable quality of life and survival. Both SLT and BSLT are satisfactory approach for end-stage silicosis,and long-term survival requires further investigations.

Abstract:Objective: To evaluate whether lung transplantation conferred acceptable survival compared with conventional treatment for end-stage silicosis. Methods: We retrospectively analyzed data for five consecutive patients with silicosis between September 2002 and December 2010, four underwent single lung transplantation and one bilateral lung transplantation. Results: There was no perioperative mortality. Extracorporeal membrane oxygenation was required in four patients, three underwent single lung transplantation and one received bilateral lung transplantation, three of them were successfully weaned. One developed primary graft dysfunction 2 days after transplant and died of multiple organ failure on postoperative day 8. The remaining four patients were discharged from hospital. During follow-up, one recipient died of severe infection 7 months after transplant. All remaining patients returned to work and had a good quality of life after 5,3 and 2 years, respectively. Conclusions: Lung transplantation offers effective therapy for patients with end-stage silicosis.

Abstract:BACKGROUND Pneumoconiosis is a chronic progressive fibrotic interstitial pneumonia for which the pathogenesis and treatment remain unclear. Previous studies showed that sodium ferulate (SF) may have a therapeutic effect, and this study explored the mechanism underlying SF-related improvement. MATERIAL AND METHODS In this study, a silicosis mouse model and primary cultured mouse lung fibroblasts were established. Hematoxylin-eosin staining, western blot analysis, quantitative real-time polymerase chain reaction, and Masson staining were used to observe the lung injury, expression of vimentin, and the degree of pulmonary fibrosis. The extracted lung fibroblasts were identified by immunofluorescence. The expression of fibrosis-related genes encoding transforming growth factor-ß1 (TGF-ß1), neutrophil alkaline phosphatase 3 (NALP3), collagen-1, alpha-smooth muscle actin (alpha-SMA), and phosphorylated p38 (p-p38) and p38 proteins were detected by western blot. The effects of SF and the TGF-ß pathway agonist SRI-011381 on cell proliferation and the expression of fibrosis-related protein in mouse lung fibroblasts were measured by Cell Counting Kit-8, immunofluorescence, and western blot as needed. RESULTS SF reduced the lung lesions in silicosis mice and inhibited the expression of vimentin and fibrosis-related genes, while having no effect on body weight. Vimentin expression was positive in the extracted cells. In vitro experiments showed that SF inhibited the proliferation of lung fibroblasts and the expression of fibrosis-related proteins. In addition, SF partly reversed the opposite regulatory effect of SRI-011381 on lung fibroblasts. CONCLUSIONS SF inhibited lung injury and fibrosis in silicosis mice through the NALP3/TGF-ß1/alpha-SMA pathway.

Abstract:169 male silicosis patients were selected as case group and 96 healthy male miners exposed to silica dust were selected as the controls.All the subjects were invited to perform a questionnaire and lung function examination at the same time.The pulmonary dysfunction was classified according to the ratios of tested and predicted values.Results showed that all parameters of lung function declined significantly in case group compared with the control group,the ratios of FEV_(1.0),FEV_(1.0)/FVC and MEF_(75%) showed obvious difference between category Ⅰ and category Ⅱ + Ⅲ of the silicosis patients in case group.The main types of pulmonary dysfunction were restrictive and mixed ventilation disorders in silicosis patients.The unconditional Logistic regression model suggested that the main risk factors of pulmonary dysfunction were various category of silicosis,medium or heavy smoking,long-term exposure to silica(30 years)and pulmonary tuberculosis.It is suggested that the incidence of pulmonary dysfunction would be increased with the progress of silicosis,in addition,the risk factors of pulmonary dysfunction also involve smoking,long duration exposure to silica,and complications of silicosis etc.

Abstract:Silicosis, characterized by irreversible pulmonary fibrosis, remains a major global public health problem. Nowadays, cumulative studies are focusing on elucidating the pathogenesis of silicosis in order to identify preventive or therapeutic antifibrotic agents. However, the existing research on the mechanism of silica-dust-induced pulmonary fibrosis is only the tip of the iceberg and lags far behind clinical needs. Idiopathic pulmonary fibrosis (IPF), as a pulmonary fibrosis disease, also has the same problem. In this study, we examined the relationship between silicosis and IPF from the perspective of their pathogenesis and fibrotic characteristics, further discussing current drug research and limitations of clinical application in silicosis. Overall, this review provided novel insights for clinical treatment of silicosis with the hope of bridging the gap between research and practice in silicosis.

Persistent radiological changes following miliary tuberculosis in miners exposed to silica dust

Abstract:Setting: Silicosis leads to increased susceptibility to tuberculosis, but it has also been suggested that tuberculosis may interact with intra-pulmonary silica to exacerbate fibrotic lung disease. Objectives: To investigate the possibility that silicosis developed due to or was exacerbated by tuberculosis. Methods: In a case series of 15 miners presenting with culture-positive miliary tuberculosis, serial radiographs taken premorbidly, at presentation, and after 2 and 6 months of standard anti-tuberculosis treatment were graded for nodularity using the International Labour Organization system. Results: Increased nodule profusion (compared to premorbid film) remained in 13 (87%) and eight (53%) patients after 2 and 6 months of treatment, respectively, despite clinical improvement in all and documented bacteriological cure in eight (53%). These phenomena, observed irrespective of human immunodeficiency virus (HIV) status, were most pronounced in men with minor premorbid changes. Abnormal pulmonary collagenisation related to silica particles was apparent at post-mortem in two men who died of HIV-associated cryptococcosis after completing TB treatment. Conclusions: Previous silica exposure appears to result in delayed and potentially incomplete radiological resolution of miliary TB. We postulate that the immune response in tubercles may evoke a 'bystander' fibrotic response, as cytokines play a central role in the pathogenesis of both TB and silicosis.
Association of preserved ratio impaired spirometry with mortality and airflow obstruction in the silicotics: a longitudinal cohort study

Abstract:Rationale Preserved ratio impaired spirometry (PRISm), defined as an impaired forced expiratory volume in one second (FEV1) with a preserved ratio of FEV1 to forced vital capacity (FVC), is associated with increased risk of airflow obstruction (AFO) and mortality in the general population. However, evidence is limited among the individuals with silicosis, an old occupational disease with an ongoing outbreak in some developed countries. Objectives To investigate the association of PRISm with the risk of mortality and incident AFO in a cohort of workers with silicosis. Methods A total of 4315 workers aged 18-80 years and diagnosed with silicosis at the Pneumoconiosis Clinic, Tuberculosis and Chest Service during 1981-2019 were enrolled in this study and followed up for a median of 12.3 years till 31 December 2019. Spirometry was included in the diagnostic examination of silicosis and follow-up reassessments. Lung function categories of participants were classified as normal spirometry (FEV1/FVC ≥ 0.7, FEV1 ≥ 80% predicted), PRISm (FEV1/FVC ≥ 0.7, FEV1 < 80% predicted), and AFO (FEV1/FVC < 0.7). The hazard ratio (HR) and 95% confidence intervals (95% CI) were estimated using Cox proportional hazards models adjusting for age, body mass index, tuberculosis history, smoking, and radiographic characteristics. Measurements and Main Results During the follow-up period, a total of 2399 (55.6%) subjects died, 1359 of whom died from respiratory-related diseases, and 780 subjects developed AFO. Subjects with PRISm had significantly increased multivariable-adjusted risk of all-cause death (adjusted HR=1.63, 95% CI 1.44-1.85) and respiratory-related mortality (adjusted HR=1.74, 95% CI 1.48-2.05) as compared with the those with normal spirometry. Besides, there was a higher risk of developing AFO in subjects with PRISm than in those with normal spirometry (adjusted HR=1.46, 95% CI 1.22-1.75). No significant interaction was observed between PRISm and smoking status in the risk of all-cause mortality and incident AFO. Conclusions PRISm is significantly associated with increased all-cause and respiratory-related mortality and a greater risk of progression to AFO among the individuals with silicosis.

Abstract:BACKGROUND Characteristics, practice patterns, and clinical outcomes of secondary spontaneous pneumothorax (SSP) with heritable connective tissue disorders (Marfan, Ehlers-Danlos, and Birt-Hogg-Dubé syndromes) are unclear. METHODS A nationwide Japanese inpatient database, which included data on 524 patients with SSP (884 hospitalizations) and 137821 with primary spontaneous pneumothorax (PSP) between July 2010 and March 2020, was used in this study. Hospitalization for SSP (n=884) was categorized into surgical (n=459) and non-surgical (n=425) groups, and the patient characteristics, treatment, and outcomes were compared between the groups. Multivariable analyses were performed to evaluate risk factors for pneumothorax recurrence. We also compared the patient characteristics among those with different underlying heritable connective tissue disorders. RESULTS Compared to the non-surgical group, the surgical group had less frequent readmission for pneumothorax (26% vs. 44%; hazard ratio, 0.47; 95% confidence interval, 0.38–0.58). Young patients (hazard ratio, 2.46; 95% confidence interval, 1.83–3.32) or those with Birt-Hogg-Dubé syndrome (2.53; 1.77–3.63) had a high risk of recurrence. Pneumothorax occurred frequently in teenagers with Marfan syndrome, in those aged 20–39 years with Ehlers-Danlos syndrome, and in those aged ≥40 years with Birt-Hogg-Dubé syndrome. CONCLUSIONS Detailed information on the characteristics and clinical course of SSP in heritable connective tissue disorders will aid the clinical decision-making process.

This article explores the current guidance, controversies, and recent advances in the management of this condition.

Abstract:Silicosis is a serious occupational disease characterized by lung fibrosis that is caused by long-term inhalation of silica-containing fine particles. Lysophosphatidic acid (LPA) and LPA(1/3) plays a role in lung fibrosis. Until recently, there has been little research investigating the role of LPA and LPA receptors (LPAR) in silica-induced development of pulmonary fibrosis. In this study, we evaluated the hypothesis that LPA and LPA(1/3) may play a role in silicosis pathogenesis using rat silicosis models induced by intratracheal instillation of silica, and randomly divided into control, silica, and VPC-12249 groups. LPA serum and bronchoalveolar lavage fluid (BALF) levels were quantified by ELISA. alpha-smooth muscle actin (alpha-SMA), type I and III collagen protein expression was quantified by western blotting (WB), and type I and III collagen mRNAs detected by reverse transcriptase-polymerase chain reaction (RT-PCR). Lung hydroxyproline (HYP) levels were detected using alkaline hydrolysis, with hematoxylin and eosin (H&E) and picrosirius red staining used for pathological examination. In vitro experiments showed that LPA stimulated fibroblasts proliferated in a time and dose-dependent manner and promoted expression of alpha-SMA, and type I and III collagen. Moreover, LPA serum and BALF levels increased in silica-instilled rats. In vivo and in vitro experiments revealed that alpha-SMA expression and collagen deposition reduced significantly after VPC-12249 treatment, and histopathological results show VPC-12249 alleviates silicosis progression. In conclusion, our findings suggest that LPA promotes the proliferation, transformation, and collagen synthesis of fibroblasts, and that LPA-LPA(1/3) are involved in the development of silicosis and may serve as novel therapeutic targets for treatment.

Abstract:Abstract To explore the clinical characteristics of systemic sclerosis complicated with silicosis. The systemic sclerosis patients treated in the Guangxi Workers’ Hospital and the People's Hospital of Guangxi Zhuang Autonomous Region from January 2000 to December 2020 were divided into the systemic sclerosis with silicosis group and the systemic sclerosis without silicosis group. Survival analysis was performed using Kaplan–Meier estimates the Cox proportional hazards model. A propensity score matching was applied in order to avoid the selection bias. Over the past 20 years, 72 systemic sclerosis patients with silicosis and 238 systemic sclerosis patients without silicosis were treated in the two hospitals. The systemic sclerosis patients with silicosis group had more males ( P < 0.000),lower mean age at onset of SSc ( P < 0.000), more frequent occurrence of weight loss ( P = 0.028), smoking ( P < 0.000), tuberculosis ( P < 0.000), cardiac involvement ( P < 0.000), ILD ( P = 0.017), pulmonary hypertension ( P = 0.024), elevated BNP ( P < 0.000). With regards to the multivariate Cox regression analysis, silicosis was related with a higher overall mortality before (HR = 3.666, 95% CI = 1.440–11.234, p = 0.025) and after the propensity score matching analysis (HR = 2.817, 95% CI = 1.196–10.764, p = 0.014). Independent risk factors for overall mortality were Gangrene (HR = 3.003, 95% CI = 1.343–9.431), Cardiac involved (HR = 5.370, 95% CI = 1.910–15.472), Scl-70 (HR = 3.569, 95% CI = 1.333–10.869), Elevated BNP (HR = 2.135, 95% CI = 1.293–9.564). Concomitant silicosis worsens systemic sclerosis patients’ prognoses. Gangrene, Scl-70, elevated BNP and cardiac involvement are independent risk factors for overall mortality. Key Points • Concomitant silicosis worsens SSc patients’ prognoses. • For individuals with occupational exposure, close observation of the symptoms of SSc, early diagnosis, and interruption of exposure may improve the prognosis. • Gangrene, Scl-70, elevated BNP and cardiac involvement are independent risk factors for overall mortality.

Abstract:Purpose of ReviewPneumoconiosis due to silicosis is still present due to modern industries such stone carving, garment industry, and road construction. We aim to discuss how to accurately diagnose silicosis, appropriately risk stratify patients with silicosis, and discuss potential new pharmacotherapies that may be of potential use in the future.Recent FindingsAcute silicosis, chronic silicosis, and accelerated silicosis are 3 distinct clinical categories. Acute silicosis is the least common form and occurs with high exposures of silica dust in a short time frame. Patients with prolonged exposures at low concentrations have the highest risk of developing chronic silicosis. Chronic silicosis is the classic presentation of silicosis with a slow onset of symptoms. Accelerated silicosis has an earlier onset than chronic silicosis and presents within 5 to 10 years of exposure. Chronic silicosis and accelerated silicosis can lead to progressive massive fibrosis, which is silica nodules merging together with associated fibrosis. Based on cumulative dose, patients with silica exposure had an independent increased risk of developing lung cancer, with an additional cumulative risk among smokers. Lung transplant should be considered in patients with silicosis who develop severe lung disease. There is no current role for any pharmacological agents in silicosis; however, there is ongoing research to identify possible pharmacological agents.SummaryThe diagnosis of silicosis is made based on occupational history, appropriate radiographic findings, and exclusion of other potential etiologies. Prevention of silica dust exposure remains crucial to reduce damage to the lungs.
