Abstract:Dynamic SPECT has the potential to provide absolute physiological parameter estimates. However, the low sensitivity of SPECT typically results in very noisy dynamic SPECT data. Filtering can reduce the noise, but at the expense of degrading the already poor resolution further. The effect of reconstruction parameters, post-reconstruction filtering and resolution recovery on kinetic parameter estimation bias and reliability was systematically investigated. Dynamic projection data were generated using Monte Carlo (MC) simulations of a mathematical brain phantom at 10 different levels of Poisson noise. The projection data were reconstructed with OSEM with varying numbers of iterations and subsets and were filtered with three-dimensional (3-D) Gaussian filters with varying FWHM. Bias and reliability of the main parameters of interest (K-1, V-d, and binding potential) for thalamus, cerebellum, and frontal cortex were estimated for the three-compartment model fits to the tissue time-activity curves derived from the reconstructed data. Reliability (standard deviation) of parameter estimates was obtained with the Bootstrap MC technique, which showed good agreement with conventional MC in a subset of data sets, but required only a small fraction of conventional MC computation time. Post-reconstruction filtering increased bias, without improving the reliability of parameter estimates and, hence, no post-reconstruction filtering is recommended. For reconstructions without resolution recovery, an effective number of 40 iterations overall provided the best tradeoff between bias and reliability for the range of noise levels studied. Resolution recovery achieved a modest reduction in bias.

Abstract:Dynamic positron emission tomography (PET) is capable of providing both spatial and temporal information of radio tracers in vivo. In this paper, we present a novel joint estimation framework to reconstruct temporal sequences of dynamic PET images and the coefficients characterizing the system impulse response function, from which the associated parametric images of the system macro parameters for tracer kinetics can be estimated. The proposed algorithm, which combines statistical data measurement and tracer kinetic models, integrates a dictionary sparse coding (DSC) into a total variational minimization based algorithm for simultaneous reconstruction of the activity distribution and parametric map from measured emission sinograms. DSC, based on the compartmental theory, provides biologically meaningful regularization, and total variation regularization is incorporated to provide edge-preserving guidance. We rely on techniques from minimization algorithms (the alternating direction method of multipliers) to first generate the estimated activity distributions with sub-optimal kinetic parameter estimates, and then recover the parametric maps given these activity estimates. These coupled iterative steps are repeated as necessary until convergence. Experiments with synthetic, Monte Carlo generated data, and real patient data have been conducted, and the results are very promising.

Abstract:Purpose Dynamic positron emission tomography (PET) is known for its ability to extract spatiotemporal information of a radio tracer in living tissue. Information of different functional regions based on an accurate reconstruction of the activity images and kinetic parametric images has been widely studied and can be useful in research and clinical setting for diagnosis and other quantitative tasks. In this paper, our purpose is to present a novel framework for estimating the kinetic parametric images directly from the raw measurement data together with a simultaneous segmentation accomplished through kinetic parameters clustering. Method An iterative framework is proposed to estimate the kinetic parameter image, activity map and do the segmentation simultaneously from the complete dynamic PET projection data. The clustering process is applied to the kinetic parameter variable rather than to the traditional activity distribution so as to achieve accurate discrimination between different functional areas. Prior information such as total variation regularization is incorporated to reduce the noise in the PET images and a sparseness constraint is integrated to guarantee the solution for kinetic parameters due to the over complete dictionary. Alternating direction method of multipliers (ADMM) method is used to solve the optimization problem. The proposed algorithm was validated with experiments on Monte Carlo-simulated phantoms and real patient data. Symbol error rate (SER) was defined to evaluate the performance of clustering. Bias and variance of the reconstruction activity images were calculated based on ground truth. Relative mean square error (MSE) was used to evaluate parametric results quantitatively. Result In brain phantom experiment, when counting rate is 1 x 10(6), the bias (variance) of our method is 0.1270 (0.0281), which is lower than maximum likelihood expectation maximization (MLEM) 0.1637 (0.0410) and direct estimation without segmentation (DE) 0.1511 (0.0326). In the Zubal phantom experiment, our method has the lowest bias (variance) 0.1559 (0.0354) with 1 x 10(5) counting rate, compared with DE 0.1820 (0.0435) and MLEM 0.3043 (0.0644). As for classification, the SER of our method is 18.87% which is the lowest among MLEM + k-means, DE + k-means, and kinetic spectral clustering (KSC). Brain data with MR reference and real patient results also show that the proposed method can get images with clear structure by visual inspection. Conclusion In this paper, we presented a joint reconstruction framework for simultaneously estimating the activity distribution, parametric images, and parameter-based segmentation of the ROIs into different functional areas. Total variation regularization is performed on the activity distribution domain to suppress noise and preserve the edges between ROIs. An over complete dictionary for time activity curve basis is constructed. SER, bias, variance, and MSE were calculated to show the effectiveness of the proposed method. Abstract:The primary goal of dynamic positron emission tomography (PET) is to quantify the physiological and biological processes through tracer kinetics analysis. However, the process is difficult and complicated because of the compromising imaging data quality, i.e. either longer scans with good counting statistics but poor temporal resolution, or noisy shorter scans with good temporal resolution. In this paper, we explore the usage of state space principles for physiological parameter estimation in dynamic PET imaging. The system equation is constructed from particular tracer kinetic models, with the number and relationship between tissue compartments dictated by the physiological and biochemical properties of the process under study. And the observation equation on measurement data is formed based on the specific types of imaging or image-derived data. Once the Poisson distributed PET data are converted to Gaussian ones through the Anscombe transformation, an extended Kalman filter is adopted to estimate the tracer kinetics parameters from the system and observations. More appropriate estimation strategies which better take care of the PET statistics are also under development. The framework is tested on simulated digital phantom data, and the results are of sufficient accuracy and robustness.
Spatial Auto-Regressive Analysis of Correlation in 3-D PET With Application to Model-Based Simulation of Data

Abstract:When a scanner is installed and begins to be used operationally, its actual performance may deviate somewhat from the predictions made at the design stage. Thus it is recommended that routine quality assurance (QA) measurements be used to provide an operational understanding of scanning properties. While QA data are primarily used to evaluate sensitivity and bias patterns, there is a possibility to also make use of such data sets for a more refined understanding of the 3-D scanning properties. Building on some recent work on analysis of the distributional characteristics of iteratively reconstructed PET data, we construct an auto-regression model for analysis of the 3-D spatial auto-covariance structure of iteratively reconstructed data, after normalization. Appropriate likelihood-based statistical techniques for estimation of the auto-regression model coefficients are described. The fitted model leads to a simple process for approximate simulation of scanner performance-one that is readily implemented in an R script. The analysis provides a practical mechanism for evaluating the operational error characteristics of iteratively reconstructed PET images. Simulation studies are used for validation. The approach is illustrated on QA data from an operational clinical scanner and numerical phantom data. We also demonstrate the potential for use of these techniques, as a form of model-based bootstrapping, to provide assessments of measurement uncertainties in variables derived from clinical FDG-PET scans. This is illustrated using data from a clinical scan in a lung cancer patient, after a 3-minute acquisition has been re-binned into three consecutive 1-minute time-frames. An uncertainty measure for the tumor SUVmax value is obtained. The methodology is seen to be practical and could be a useful support for quantitative decision making based on PET data.

Abstract:Positron emission tomography (PET) images are corrupted by noise. This is especially true in dynamic PET imaging where short frames are required to capture the peak of activity concentration after the radiotracer injection. High noise results in a possible bias in quantification, as the compartmental models used to estimate the kinetic parameters are sensitive to noise. This paper describes a new post-reconstruction filter to increase the signal-to-noise ratio in dynamic PET imaging. It consists in a spatio-temporal robust diffusion of the 4D image based on the time activity curve (TAC) in each voxel. It reduces the noise in homogeneous areas while preserving the distinct kinetics in regions of interest corresponding to different underlying physiological processes. Neither anatomical priors nor the kinetic model are required. We propose an automatic selection of the scale parameter involved in the diffusion process based on a robust statistical analysis of the distances between TACs. The method is evaluated using Monte Carlo simulations of brain activity distributions. We demonstrate the usefulness of the method and its superior performance over two other post-reconstruction spatial and temporal filters. Our simulations suggest that the proposed method can be used to significantly increase the signal-to-noise ratio in dynamic PET imaging.

Abstract:Dynamic PET image reconstruction is a challenging issue due to the low SNR and the large quantity of spatio-temporal data. We propose a robust state-space image reconstruction (SSIR) framework for activity reconstruction in dynamic PET. Unlike statistically-based frame-by-frame methods, tracer kinetic modeling is incorporated to provide physiological guidance for the reconstruction, harnessing the temporal information of the dynamic data. Dynamic reconstruction is formulated in a state-space representation, where a compartmental model describes the kinetic processes in a continuous-time system equation, and the imaging data are expressed in a discrete measurement equation. Tracer activity concentrations are treated as the state variables, and are estimated from the dynamic data. Sampled-data H(infinity) filtering is adopted for robust estimation. H(infinity) filtering makes no assumptions on the system and measurement statistics, and guarantees bounded estimation error for finite-energy disturbances, leading to robust performance for dynamic data with low SNR and/or errors. This alternative reconstruction approach could help us to deal with unpredictable situations in imaging (e.g. data corruption from failed detector blocks) or inaccurate noise models. Experiments on synthetic phantom and patient PET data are performed to demonstrate feasibility of the SSIR framework, and to explore its potential advantages over frame-by-frame statistical reconstruction approaches.

Abstract:Recent advances in preclinical SPECT instrumentation enable non-standard multi-isotope acquisitions at the edge of physical feasibility to improve efficiency of pharmaceutical research. Due to the variety of applications, optimization of imaging hardware, acquisition protocols and reconstruction algorithms is a central and recurring task. For this purpose, we developed a Monte Carlo simulation model of a preclinical state-of-the-art multi-pinhole SPECT system, the NanoSPECT/CTPLUS, with emphasis on high accuracy for multi-isotope experiments operating near the system range limits. The GATE/ GEANT4 model included an accurate description of multi-pinhole collimators and all substructures of the detector back compartment. The readout electronics was modeled with a variety of signal processors partially extended to incorporate non-simplified measured response functions. The final model was able to predict energy spectra, planar images and tomographic reconstructions with high accuracy for both standard and non-standard multi-isotope experiments. Complex activity distributions could be reproduced for a wide range of noise levels and different modes of angular undersampling. Using the example of a dual-isotope triple-tracer experiment, the model has proven to be a powerful tool for protocol optimization and quantitative image correction at the performance range limits of multi-isotope multi-pinhole SPECT.

Patient-adaptive lesion metabolism analysis by dynamic PET images.

Abstract:Dynamic PET imaging provides important spatial-temporal information for metabolism analysis of organs and tissues, and generates a great reference for clinical diagnosis and pharmacokinetic analysis. Due to poor statistical properties of the measurement data in low count dynamic PET acquisition and disturbances from surrounding tissues, identifying small lesions inside the human body is still a challenging issue. The uncertainties in estimating the arterial input function will also limit the accuracy and reliability of the metabolism analysis of lesions. Furthermore, the sizes of the patients and the motions during PET acquisition will yield mismatch against general purpose reconstruction system matrix, this will also affect the quantitative accuracy of metabolism analyses of lesions. In this paper, we present a dynamic PET metabolism analysis framework by defining a patient adaptive system matrix to improve the lesion metabolism analysis. Both patient size information and potential small lesions are incorporated by simulations of phantoms of different sizes and individual point source responses. The new framework improves the quantitative accuracy of lesion metabolism analysis, and makes the lesion identification more precisely. The requirement of accurate input functions is also reduced. Experiments are conducted on Monte Carlo simulated data set for quantitative analysis and validation, and on real patient scans for assessment of clinical potential.

Abstract:Monte Carlo simulation is an essential tool to assist in the design of new medical imaging devices for emission tomography. This paper modeled the full 3D whole body PET scanner and 2D animal PET scanner with septa using Monte Carlo simulation code GATE based on Geant4, analyzed the scatter fraction, scatter distribution, multiple scatter events, and scatter events from outside of FOV under 3D acquisition mode and scatter events from the septa under 2D acquisition mode. For the critical problems of 3D scatter correction: multiple scatter events and scatter events from outside of FOV, we designed additional experiments, got the fitted relationship between the percentage of multiple scatter events and the cross sectional area of the phantom, and the effects of the scatter events from outside of FOV at different ring positions; for the 2D scatter correction, we separated the scatter events from septa for analysis, and provided a quantitative reference for the elimination of these events or introduction into the procedure of reconstruction.
Constructing Reliable Parametric Images Using Enhanced GLLS for Dynamic SPECT

Abstract:The generalized linear least square (GLLS) method can successfully construct unbiased parametric images from dynamic positron emission tomography data. Quantitative dynamic single photon emission computed tomography (SPECT) also has the potential to generate physiological parametric images. However, the high level of noise, intrinsic in SPECT, can give rise to unsuccessful voxelwise fitting using GLLS, resulting in physiologically meaningless estimates. In this paper, we systematically investigated the applicability of our recently proposed approaches to improve the reliability of GLLS to parametric image generation from noisy dynamic SPECT data. The proposed approaches include use of a prior estimate of distribution volume ( V d), a bootstrap Monte Carlo (BMC) resampling technique, as well as a combination of both techniques. Full Monte Carlo simulations were performed to generate dynamic projection data, which were then reconstructed with and without resolution recovery, before generating parametric images with the proposed methods. Four experimental clinical datasets were also included in the analysis. The GLLS methods incorporating BMC resampling could successfully and reliably generate parametric images. For high signal-to-noise ratio (SNR) imaging data, the BMC-aided GLLS provided the best estimates of K 1, while the BMC-Vd-aided GLLS proved superior for estimating V d. The improvement in reliability gained with BMC-aided GLLS in low SNR image data came at the expense of some overestimation of V d and increased computation time.

Abstract:Recently introduced stationary dedicated cardiac SPECT scanners provide new opportunities to quantify myocardial blood flow (MBF) using dynamic SPECT. However, comparing to PET, the low sensitivity of SPECT scanners affects MBF quantification due to the high noise level, especially for <sup>201</sup> Thallium (<sup>201</sup>Tl) due to its typically low injected dose. The conventional indirect method for generating parametric images typically starts by reconstructing a time series of frame images followed by fitting the time-activity curve (TAC) for each voxel or segment with an appropriate kinetic model. The indirect method is simple and easy to implement; however, it usually suffers from substantial image noise that could also lead to bias. In this paper, we developed a list mode direct parametric image reconstruction algorithm to substantially reduce noise in MBF quantification using dynamic SPECT and allow for patient radiation dose reduction. GPU-based parallel computing was used to achieve more than 2000-fold acceleration. The proposed method was evaluated in both simulation and in vivo canine studies. Compared with the indirect method, the proposed direct method achieved substantially lower image noise and variability, particularly at large number of iterations and at low-count levels.

Tracer Kinetic Models as Temporal Constraints during DCE-MRI reconstruction

Abstract:Purpose: To apply tracer kinetic models as temporal constraints during reconstruction of under-sampled dynamic contrast enhanced (DCE) MRI. Methods: A library of concentration v.s time profiles is simulated for a range of physiological kinetic parameters. The library is reduced to a dictionary of temporal bases, where each profile is approximated by a sparse linear combination of the bases. Image reconstruction is formulated as estimation of concentration profiles and sparse model coefficients with a fixed sparsity level. Simulations are performed to evaluate modeling error, and error statistics in kinetic parameter estimation in presence of noise. Retrospective under-sampling experiments are performed on a brain tumor DCE digital reference object (DRO) at different signal to noise levels (SNR=20-40) at (k-t) space under-sampling factor (R=20), and 12 brain tumor in- vivo 3T datasets at (R=20-40). The approach is compared against an existing compressed sensing based temporal finite-difference (tFD) reconstruction approach. Results: Simulations demonstrate that sparsity levels of 2 and 3 model the library profiles from the Patlak and extended Tofts-Kety (ETK) models, respectively. Noise sensitivity analysis showed equivalent kinetic parameter estimation error statistics from noisy concentration profiles, and model approximated profiles. DRO based experiments showed good fidelity in recovery of kinetic maps from 20-fold under- sampled data at SNRs between 10-30. In-vivo experiments demonstrated reduced bias and uncertainty in kinetic mapping with the proposed approach compared to tFD at R>=20. Conclusions: Tracer kinetic models can be applied as temporal constraints during DCE-MRI reconstruction, enabling more accurate reconstruction from under- sampled data. The approach is flexible, can use several kinetic models, and does not require tuning of regularization parameters.

Abstract:BackgroundKinetic analysis of dynamic PET data requires an accurate knowledge of available PET tracer concentration within blood plasma over time, known as the arterial input function (AIF). The gold standard method used to measure the AIF requires serial arterial blood sampling over the course of the PET scan, which is an invasive procedure and makes this method less practical in clinical settings. Traditional image-derived methods are limited to specific tracers and are not accurate if metabolites are present in the plasma.ResultsIn this work, we utilise an image-derived whole blood curve measurement to reduce the computational complexity of the simultaneous estimation method (SIME), which is capable of estimating the AIF directly from tissue time activity curves (TACs). This method was applied to data obtained from a serotonin receptor study (11C-SB207145) and estimated parameter results are compared to results obtained using the original SIME and gold standard AIFs derived from arterial samples. Reproducibility of the method was assessed using test-retest data. It was shown that the incorporation of image-derived information increased the accuracy of total volume of distribution (V T) estimates, averaged across all regions, by 40% and non-displaceable binding potential (BP ND) estimates by 16% compared to the original SIME. Particular improvements were observed in K1 parameter estimates. BP ND estimates, based on the proposed method and the gold standard arterial sample-derived AIF, were not significantly different (P=0.7).ConclusionsThe results of this work indicate that the proposed method with prior AIF information obtained from a partial volume corrected image-derived whole blood curve, and modelled parent fraction, has the potential to be used as an alternative non-invasive method to perform kinetic analysis of tracers with metabolite products.

Impact of Attenuation, Scatter and Depth Dependent Collimator Response Correction in 3-D Cardiac SPECT Imaging: a Monte Carlo Investigation

Abstract:Cardiac SPECT images are degraded by several physical effects, including attenuation, Compton scatter and collimator geometric response. We proposed a method, analytical SPECT modeling (ASM), for efficiently and accurately modeling the spatially varying SPECT point response function in nonuniform attenuating objects. A quasi Monte Carlo integration method was employed in the model, which resulted in an 1 to 2 orders of magnitude speed up in modeling process. Monte Carlo studies were done on simulated heart images from the NCAT phantom, and our model was implemented in a 3-D OSEM algorithm. The system transition matrix was generated based on the patient-specific attenuation map before reconstruction, which included several parts: 1) the primary matrix which modeled depth dependent collimator response of primary photons; 2) the attenuation factor matrix which modeled nonuniform attenuation of primary photons; 3) the scattering matrix which modeled attenuation, scatter and collimator response of scattered photons. An unmatched projector/backprojector was used to accelerate the reconstruction. Reconstruction results in which different degrading effects were modeled were compared in terms of global quantitative accuracy, contrast and noise level. Results showed that the more complete the degrading effects were modeled in reconstruction, the higher quality and quantitative accuracy could be obtained.

Abstract:Tracer-kinetic analysis of dynamic contrast-enhanced magnetic resonance imaging data is commonly performed with the well-known Tofts model and nonlinear least squares (NLLS) regression. This approach yields point estimates of model parameters, uncertainty of these estimates can be assessed e.g. by an additional bootstrapping analysis. Here, we present a Bayesian probabilistic modeling approach for tracer-kinetic analysis with a Tofts model, which yields posterior probability distributions of perfusion parameters and therefore promises a robust and information-enriched alternative based on a framework of probability distributions. In this manuscript, we use the Quantitative Imaging Biomarkers Alliance (QIBA) Tofts phantom to evaluate the Bayesian Tofts Model (BTM) against a bootstrapped NLLS approach. Furthermore, we demonstrate how Bayesian posterior probability distributions can be employed to assess treatment response in a breast cancer DCE-MRI dataset using Cohen's d. Accuracy and precision of the BTM posterior distributions were validated and found to be in good agreement with the NLLS approaches, and assessment of therapy response with respect to uncertainty in parameter estimates was found to be excellent. In conclusion, the Bayesian modeling approach provides an elegant means to determine uncertainty via posterior distributions within a single step and provides honest information about changes in parameter estimates.

Abstract:The recently developed total-body PET scanner enables high temporal resolution in dynamic imaging. Due to the much improved temporal resolution and large field of view, delay and the dispersion effects in the image-derived input function, which vary for different tissues and organs, may affect accuracy in parametric imaging. In this paper, the delay effect was studied using the early kinetics of an FDG scan, which may be approximated using a 1-tissue compartment model. Dynamic reconstructed frames were acquired using the total-body PET scanner with 1-second frames for the first 30 seconds and 2 seconds for the subsequent 60 seconds. The image-derived input function was acquired from the reconstructed dynamic sequence using volumes of interest in the ascending and descending aorta. Voxel-specific delay times for the plasma input function were also modeled within the 1-tissue compartment model. A total of 4 parametric images were generated. Image-based parametric image generation was achieved with a maximum likelihood estimation method. Parametric images with and without the modeling of delay time in the input function were compared. Additional image denoising techniques including Gaussian denoising and non-local-mean denoising were employed. Quantitative evaluation was achieved by the calculation of the Akaike Information Criterion (AIC). The voxel-specific parameters of the 1-tissue compartment together with the delay time were successfully reconstructed using the proposed method. The estimated delay time showed variations as large as 40 seconds. The non-local-mean filter was shown to be able to reduce the image noise of the generated parametric images. Various image artifacts were observed when no delay time model was included. We have shown that with the use of total-body PET and the increased sensitivity, it is possible to estimate parametric images using the very early stages of the FDG injection. The combined effects of delay and dispersion will be studied in the future.
Inhibition of proteolysis at harvest using heat in alfalfa silages: effects on silage composition and digestion by sheep.

Abstract:Heat treatment at harvest was used to investigate the effects of proteolysis on silage composition and digestion by sheep. Four alfalfa (Medicago sativa) silages were prepared, two from mid-bloom and two from pre-bloom crops from the same field. Mid-bloom alfalfa was conserved with formic acid as two unwilted silages, either without (unwilted control; UWC) or after heat treatment (unwilted heated; UWH) applied as steam for 1 min. Pre-bloom alfalfa was ensiled either after 24 h wilting (wilted control; WC) or after heating in a crop dehydrator for 2 min (wilted control; WH). Heated treatments were inoculated with Lactobacillus plantarum. Eight wethers, cannulated in the rumen and duodenum, were given the silages to determine the effects of heat treatment on digestion. Heat treatment inhibited protease activity and reduced protein catabolism in the silo. In unwilted silages, heat treatment had no effects (P greater than .05) on OM or N digestion, but it reduced (P less than .05) CP degradability in the rumen. In wilted silages, heat treatment reduced (P less than .05) apparent OM digestion in the rumen and increased (P less than .05) the proportion of N intake flowing to the intestines as non-ammonia N (NAN). Efficiency of microbial protein synthesis also was increased (P less than .01). Absorption of N posterior to the duodenum was increased (P less than .05) in WH compared to WC, but there was no effect (P greater than .05) of heat treatment on apparent total tract N digestibility.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:BackgroundTo perform kinetic modelling quantification, PET dynamic data must be acquired in short frames, where different critical conditions are met. The accuracy of reconstructed images influences quantification. The added value of Time-Of-Flight (TOF) and Point Spread Function (PSF) in cardiac image reconstruction was assessed.MethodsA static phantom was used to simulate two extreme conditions: (i) the bolus passage and (ii) the steady uptake. Various count statistics and independent noise realisations were considered. A moving phantom filled with two different radionuclides was used to simulate: (i) a great range of contrasts and (ii) the cardio/respiratory motion. Analytical and iterative reconstruction (IR) algorithms also encompassing TOF and PSF modelling were evaluated.ResultsBoth analytic and IR algorithms provided good results in all the evaluated conditions. The amount of bias introduced by IR was found to be limited. TOF allowed faster convergence and lower noise levels. PSF achieved near full myocardial activity recovery in static conditions. Motion degraded performances, but the addition of both TOF and PSF maintained the best overall behaviour.ConclusionsIR accounting for TOF and PSF can be recommended for the quantification of dynamic cardiac PET studies as they improve the results compared to analytic and standard IR.

Novel Method to Estimate Kinetic Microparameters from Dynamic Whole-Body Imaging in Regular-Axial Field-of-View PET Scanners

Abstract:For whole-body (WB) kinetic modeling based on a typical PET scanner, a multi-pass multi-bed scanning protocol is necessary given the limited axial field-of-view. Such a protocol introduces loss of early-dynamics in time-activity curves (TACs) and sparsity in TAC measurements, inducing uncertainty in parameter estimation when using least-squares estimation (LSE) (i.e., common standard) especially for kinetic microparameters. We present a method to reliably estimate microparameters, enabling accurate parametric imaging, on regular-axial field-of-view PET scanners. Our method, denoted parameter combination-driven estimation (PCDE), relies on generation of reference truth TAC database, and subsequently selecting, the best parameter combination as the one arriving at TAC with highest total similarity score (TSS), focusing on general image quality, overall visibility, and tumor detectability metrics. Our technique has two distinctive characteristics: 1) improved probability of having one-on-one mapping between early and late dynamics in TACs (the former missing from typical protocols), and 2) use of multiple aspects of TACs in selection of best fits. To evaluate our method against conventional LSE, we plotted tradeoff curves for noise and bias. In addition, the overall SNR and spatial noise were calculated and compared. Furthermore, CNR and TBR were also calculated. We also tested our proposed method on patient data (18F-DCFPyL PET scans) to further verify clinical applicability. Significantly improved general image quality performance was verified in microparametric images (e.g. noise-bias performance). The overall visibility and tumor detectability were also improved. Finally, for our patient studies, improved overall visibility and tumor detectability were demonstrated in micoparametric images, compared to use of conventional parameter estimation.
