Abstract:Objective To observe the changes of tip perfusion index(TPI),systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR),bispectral index(BIS),and blood noradrenaline(NE)and norepinephrine(E)under different stimulations,so as to evaluate the clinical value of TPI in monitoring the stress response during laparoscopic cholecystectomy.Methods Thirty patients of American Society of Anesthesiologists(ASA)grade Ⅰ-Ⅱ undergoing laparoscopic cholecystectomy under general anesthesia were enrolled in this study.The SBP,DBP,HR,TPI,BIS,the skin temperature of finger,NE and E levels were observed after the patients entering the operation room.Anesthesia was induced with midazolan 0.05 mg/kg,fentanyl 5 μg/kg and propofol effect compartment concentration 4.5 μg/mL by target-controlled infusion.Tracheal intubation was facilitated with vecuronium 0.12 mg/kg.Anesthesia was maintained with TCI of propofol(target effect-site concentration was set at 3.0 μg/mL)and remifentanil(0.25 μg·kg-1·min-1)was used for anesthesia maintenance.The patients were mechanically ventilated.The TPI,the skin temperature of finger,BIS,SBP,DBP,HR,NE and E were observed at the following time points:before induction of anesthesia(T0),after induction of anesthesia and before intubation(T1),immediately intubation(T2),5 min after intubation(T3),immediately skin incision(T4),1 min after pneumoperitoneum(T5)and 5 min after pneumoperitoneum(T6),immediately straw cholecy(T7),1 min after stopping pneumoperitoneum(T8),immediately extubation(T9),and 5 min after extubation(T10).Results Compared with the baseline level at T0,the SBP,DBP and HR values at T2,T5-T7 were significantly higher(P<0.05);and TPI values at T2,T4-T7 were significantly lower than that at T0(P<0.05).The skin temperatures were not significantly different at different time points(P>0.05).The BIS values at T1-T8 were significantly lower than that at T0(P<0.05),and it recovered to the level of T0 at T9 and T10.The plasma NE level at T1 was significantly lower than that at T0(P<0.05),and those at T2-T7 was significantly higher than that at T0(P<0.05).Plasma level of E at T5 was significantly higher than that at T1(P<0.05).We also found that TPI was negatively correlated with SBP,DBP,HR,plasma NE and E(r=-0.963,-0.908,-0.957,-0.845 and-0.324,respectively,P<0.05),and there were no relation of TPI with BIS and the skin temperature(r=-0.188,0.067,respectively,P>0.05).Conclusion TPI can be used to monitor the perioperative stress-induced changes of sympathetic tone in a noninvasive,real time,and continuous manner,and can it may serve as a convenient,effective indicator for clinical anesthesia management.
