Assessing the Translation of Research and Innovation into Dental Practice

Abstract:Introduction: Over the past 100 y, there have been an unprecedented number of innovations that have improved oral health and functioning. Variations in lag times between discovery and clinical adoption are related to dentist education, the clinical availability of technology, and the perceived value of an innovation. Objectives: The purposes of this cross-sectional study were 1) catalogue research discoveries and innovations over the past 30 y and 2) from that list identify those which practicing dentists believe have maximally affected patient care. Methods: Thirty International Association for Dental Research leaders identified innovations over the past 30 y that have significantly affected dental care. The 30 most cited innovations were included in a questionnaire that was sent to a random sample of US dentists who graduated dental school during or before 1995 (before the recency of identified discoveries) and devoted ≥50% of their time to patient care. Eighty-two percent of respondents identified as general dentists and 18% as specialists. Respondents were asked to identify 5 to 7 items whose loss would have the most adverse effects on patient care. Results: The most cited advances were adhesive dental materials (74.5%), dental implants (71.9%), direct bonding systems (71.2%), dental loupes (54.7%), universal precautions for infection control (48.6%), and digital imaging (46.0%). There were differences in the ranking of responses between generalists and specialists. For the oral surgeons and periodontists (OMSPER, n = 51), top choices were dental implants (82%), cone beam computed tomography (CBCT) imaging (74%), regenerative dentistry and tissue engineering (68%), universal precautions (58%), digital imaging (56%), and dental loupes (48%). Conclusions: There was agreement among generalists and specialists about the importance of dental implants, digital imaging, use of dental loupes, and universal precautions in improving patient care. However, generalists also cited the importance of adhesive dental materials and bonding, and OMSPER cited CBCT and biological-based tissue restoration as major advances. Knowledge transfer statement: Many advances in dental patient care capitalize on innovations and technologies that have emerged after dentists graduate from dental school. Adoption of new technologies is influenced by information acquired from professional journals, continuing education, industry marketing activities, and interactions with colleagues. The results of this study suggest that innovations that are directly related to clinical procedures were rated most impactful. Future research should consider cost-effectiveness and patient perceptions on the impact of innovations and technologies.
Using dental care resources optimally: quality-efficiency trade-offs in a competitive private market

Abstract:Purpose: Modern lifestyle changes led to increased dental care needs in India. Consequently, there has been a sharp rise in dentist numbers. Karnataka state alone produces 2,500 dentists annually, who are engaged in the non-government sector owing to inadequate public sector opportunities. This article aims to assess Karnataka private dental clinic quality and efficiency. Design/methodology/approach: Dentists were interviewed using a close-ended, structured interview schedule and their clinics were assessed using a checklist adopted from guidelines for providing machinery and equipment under the National Oral Health Care Programme (NOHCP). Dental "hotel" and clinical quality were scored based on this checklist. Findings: Clinical quality was "excellent" in 12 per cent of clinics and poor in 49 per cent. Clinics with better infrastructure charged higher price (p < 0.05). Multi-chair clinics charging fixed rates were high (81 per cent). According to 59.5 per cent of dentists, competition did not improve quality while 27 per cent felt that competition increased price, not quality. About 30.9 per cent of the poor quality clinics, 41 per cent average quality clinics and 26 per cent good quality clinics were technically efficient. Practical implications: The multi chair clinics offered better quality at higher prices and single chair clinics provided poorer quality at lower prices. In other words, they had a sub-optimal price-quality mix. Therefore, there is a need to regulate price and quality in all clinics to arrive at an optimal price-quality mix so that clients are not overburdened financially even while receiving good quality dental care. Originality/value: The article advocates that resources are used optimally as a way to achieve value for money and to achieve break-even points thereby providing quality care in a competitive market. Factors that influence dental practitioner behaviour are evaluated.
Application of the lean manufacturing model as an effective way to improve the activities of a dental clinic.

Abstract:There is an increase in the need of the population, regardless of age, for dental care. At the same time, today there is a personnel shortage, insufficient staffing of the material and technical base of medical organizations providing dental care in outpatient settings. In this regard, it is necessary to search for new organizational technologies in the activities of these organizations to improve the availability and quality of medical care and meet the growing needs of patients. The purpose of the study: to evaluate the effectiveness of the developed model for the use of lean manufacturing technologies in dental practice. Materials and methods. In basic dental polyclinics, data on the timing of time spent by patients at the registry, while waiting for an appointment with a dentist–therapist, the time spent by a dentist–therapist in the treatment of medium caries of permanent teeth, taking into account the filling out of medical documentation before and after the use of lean manufacturing technologies, were analyzed. The assessment of the existing navigation in polyclinics, routing schemes for patients, methods of making an appointment was carried out by visualization method. In addition, the data of the survey of patients and employees of the dental clinic in the previously conducted study were analyzed. Results. After the application of lean manufacturing technologies, the time spent by the patient in the registry was reduced by 12–16 minutes. The total number of actions of the registrar decreased by 31.0%, and the distance traveled by 38.0–40.0%. The time of adding value at the reception of patients by a doctor increased by 20.0% (SP No. 1) and 24.3% (SP No. 2). The time of appointment of a dentisttherapist decreased by 33.8% in joint venture No. 1 and by 32.3% in joint venture No. 2. 72.7% of medical workers noted a positive effect in the activities of their polyclinics. The proportion of patients satisfied with the organization of medical care in dental clinics increased by 33.0%. On average, 88.5% of them noted positive changes in the registry, 73.0% – in the appointment of a dentist-therapist, 74.5% – in the new navigation, 75.0% – in the appointment. Findings. The use of lean manufacturing methods helped to reduce the time spent by patients at the reception desk, at the dentisttherapist’s appointment, increased the availability of an appointment, increased the satisfaction of both medical workers and patients. Thus, the use of the lean manufacturing technology implementation model has shown its effectiveness.
Top Veterinary Practice Issues that Negatively Affect Culture, Retention, and Performance

Abstract:With over 70 years of experience and having consulted with nearly 2000 veterinary hospitals in the United States and abroad, this top 10 list has been consistent through the years with some different approaches since the pandemic. Beyond consulting and advising, we teach practice management for veterinary students at Washington State University (Rick) and University of Georgia (Jeff). As a part of the 4th year curriculum, we conduct on-site evaluations for practices where the veterinary students observe appointments, interpret practice and financial data, and make recommendations for improvement to the owners. From these reports, often over 100 pages long and from debrief meetings with owners, we offer the following as our list of the TOP 10challenges we see in practices today. For each issue, we identify the issue, describe common findings, and then suggest some changes that practices can institute to either treat or avoid these issues for their practices.
Marketing dentistry: a pilot study in Dudley

Abstract:This study was an attempt to persuade non-users or irregular users of dental services to change their behaviour and visit the dentist more regularly. The task was divided into two main parts. One was a promotion campaign undertaken in the Borough of Dudley in the West Midlands and the other a professional development programme for the dentists practising in the same area. The professional development campaign was designed to make the practitioners aware of the barriers to dental care which are perceived by patients. It was undertaken through a series of workshops on various aspects of practice development and promotion to which the staff of all the dental practices in Dudley were invited. Thirty-nine out of a total of forty-one accepted the invitation and attended some or all of the programme. In addition to the workshops members of the research team visited each of the participating practices regularly to discuss aspects of the programme and to record progress. The practitioners had varied attitudes to marketing and varied learning styles, and therefore they had different expectations of the nature and purpose of professional development. The majority attended solely to learn about the promotion campaign. There were also varying perceptions of patient recruitment and retention. None saw this as a serious problem. However, even before the new National Health Service contract for general dental practitioners there was the growing realisation that in the future there would be competition between practices and that this could be a reason for considering recruitment and the retention of patients more carefully. This was considered by many of the dentists, but not all, to be a problem of patient behaviour, linked with a degree of fatalism, and that nothing much could be done about it. There was a reluctance to change and the fear of the risks involved led a number of the general dental practitioners to identify and emphasise the difficulties in making new developments rather than their benefits. Only one quarter of the practices made any changes and they tended to prefer quick solutions rather than basic development. The experience gained in this part of the study indicated that in the future professional development should be considered in terms of the dentist's perception of the costs and the benefits; direct costs, opportunity costs, and the prospect of effecting change.(ABSTRACT TRUNCATED AT 400 WORDS)
A survey for endodontists in today's economy: exploring the current state of endodontics as a profession and the relationship between endodontists and their referral base

Abstract:Introduction: The purpose of this study was to assess the perceptions, referral trends, and practice patterns of practicing endodontists in the United States and any effect the recent economy may have had on these. Methods: A 24-question survey was formulated and sent via www.surveymonkey.com to 3255 active members of the American Association of Endodontists. Overall, 875 participants completed the survey, a response rate of 26.9%. Results: The average number of treatment cases per day was 5.7. Average work hours per week were 34.3 for men and 30.7 for women (P < .05). Among all treatment cases, 46% were nonsurgical retreatment, and 7.2% were apical surgical procedures. Procedural misadventure accounted for 10.8% of all treatment cases, with the most common referral reason being unable to locate canals (75.0%) followed by separated instruments (15.3%). Of all respondents, 49.9% performed regenerative endodontic procedures, and 7.7% placed implants. Among endodontists who practice in urban areas, 69.7% believed there were too many endodontists, and 50% have delayed their retirement plans because of recent economic impact, compared with their suburban and rural counterparts at 66.1% and 38%, 25.9% and 33.1%, respectively (P < .05). Fifty-nine percent of respondents were optimistic about the future of endodontics as a specialty, but those who have practiced more than 20 years were more pessimistic than those with less experience (P < .05). Conclusions: Recent economic impacts appear to have had an effect on the perceptions of active endodontists regarding practice success, the future of the specialty, and their retirement plans. Those who have been in practice longest (>20 years), practice in urban settings, and practice in a solo environment are most significantly affected.
An Assessment Model for Evaluating Outcomes in Federally Qualified Health Centers' Dental Departments: Results of a 5 Year Study

Abstract:Purpose: The purpose of this report was to establish baseline data on 10 oral health performance indicators over 5 fiscal years (2007 to 2008 through 2011 to 2012) for an Iowa health center. The baseline data provides an assessment model and reports outcomes based on the use of the model. Performance indicators show evidence of provider performance, accountability to stakeholders and provide the benchmarks required for dental management to develop future goals to improve oral health outcomes for atrisk populations. Methods: Using descriptive statistic, this report extrapolated data from the Iowa Health Center's computer management systems software, HealthPro, and Centricity electronic medical records, and analyzed using IBM® SPSS® 19. This report describes the change in utilization for number and type of visits for uninsured and Medicaid patients over 5 fiscal years (a fiscal year is measured from November 1 through October 31). Results: The number of patients receiving at least 1 dental visit in a measurement year showed n=81,673 procedures with 21% (17,167) being unduplicated patients. Preventive averaged 46%, restorative 18%, urgent care 22% and other procedures 14%. Conclusion: Federally qualified health centers (FQHCs) with a dental component serve populations with the greatest health disparities. This population includes ethnic and racial minorities, uninsured, underinsured, rural residents, Medicaid and Medicare. Establishing baseline data for FQHCs provides a foundational tool that will allow dental management to analyze successes as well as deficiencies in the goal to provide increased utilization to oral health care for at-risk populations.
Optimizing the Oral Health of Patients Through Risk Assessment and Prevention

Abstract:Dental caries, gingivitis, periodontitis, and peri-implant diseases are all initiated by dysbiotic dental plaque biofilm. For this reason, a critical component for the prevention and treatment of these common dental diseases is effective delivery of oral hygiene by practitioners and patients. To enhance biofilm disruption and adequately assess risk for oral disease development and progression, dental healthcare practitioners should educate patients and utilize tools that allow for stratification of patients based on their individual risk profiles for oral diseases. This article seeks to highlight emerging trends to enhance disease prevention and promote wellness for dental healthcare providers in clinical practice.

A comprehensive data collection approach involved the extraction and analysis of relevant statistics, including the distinction of new versus established patients, the distribution of different diagnoses among these groups, and the overall number of patients who underwent procedures. By examining the interventions, and recommendations provided to patients following diagnosis, the research aims to enhance our understanding of the longitudinal care strategies employed by oral pathologists.

Abstract:The evolution of healthcare industry management corresponds to management trends in the global economy. Reforming medicine within the framework of the concepts of sustainable development, value-oriented approach, person-centeredness, as well as challenges due to the pandemic provide a number of relevant tasks and subjects for management consulting. A comparison of the market volumes of domestic and foreign medical management consulting shows a significant difference in the demand for this field of activity. It is necessary to explore the objective and subjective reasons for the restrictions on the development of domestic management consulting. The purpose of the study is to identify problems of management consulting in dental practice. The authors set the task of reviewing foreign and domestic sources of scientific literature and analytical reporting on the management consulting market, clarifying the main economic trends in domestic and foreign consulting services in healthcare, and identifying indicators and performance factors for management consulting in dental institutions. The methodology of content analysis of scientific information made it possible to draw conclusions about the state of the industry and formulate recommendations for its scaling. The results of the study confirmed the relevance of management consulting for the management and staff of dental organizations and the need for the development of the industry. The challenges of modern management increase the need to attract experts and professional management consultants to dental organizations.
An Analysis of Scientific Research Trends in Oral Implantology Between 2016 and 2022

Abstract:The present study aims to evaluate the trends of oral implant–related research in design and topics between the range of 2016 to the end of 2022. The electronic search was conducted in MEDLINE via Pubmed. Papers published in Clinical Oral Implant Research, Clinical Implant Dentistry and Related Research, and International Journal of Oral Implantology as well as previously European Journal of Oral Implantology, International Journal of Oral and Maxillofacial Implants, and Journal of Oral Implantology between January 1, 2016, and December 30, 2022, were retrieved. Articles were classified according to their study design and major subjects. The Joinpoint regression model was used to determine changes in the trends of study designs and topics. Statistical significance was defined as a p-value <.05. A total of 3382 articles were analyzed in this study. In the specified period, in vivo, prospective cohort, retrospective case control, randomized clinical studies in the design and prosthetic complications, peri-implant hard tissue studies in the topic experienced a significant decreasing pattern in the number of published articles as well as the total number of articles. Case reports and series, retrospective cohort, nonrandomized clinical studies in the design and outcomes of implant-related treatment plans, immediate implant placement, and peri-implantitis in the topic experienced a significant decreasing pattern followed by a significant increasing pattern with a turning point between 2017 and 2020. Considering the limitations of this scientific topical trend analysis, it can be concluded that the recent pandemic affected the research path in oral implantology in many ways.

Perceptions of mental health screening and referral to treatment in National Dental‐Practice Based Research Network practices: A qualitative study

Abstract:Objectives Dental practices can have additional positive impacts on public health by implementing mental health screening and referral to treatment in dental care workflows. In this study, we examined how dental practices identify and address adult patient mental health concerns, attitudes about implementing mental health screening and referral, and potential barriers and facilitators to treatment. Methods We conducted semi‐structured focus groups with 17 dentists, 10 dental hygienists, and 5 dental assistants/office staff in the South‐Central region of the National Dental Practice‐Based Research Network. Transcribed interviews were analyzed using thematic analysis. Results We identified five main themes from dental practitioners and office staff responses. Practitioners and office staff: (1) discover patient mental health concerns through record review, patient/caregiver disclosure, and patient observation; (2) respond to patients' mental health concerns by making the patient more comfortable, documenting the concern in the patient's chart, and directly addressing the mental health concern; (3) want a systematic process for mental health screening and referral to treatment in their dental office; (4) recognize potential barriers in implementing health screening and referral to treatment processes; (5) desire training on mental health matters. An overarching theme emerged: developing a trusting relationship with patients. Conclusions Participants noted the importance of implementing systematic procedures for mental health screening and referral to services into dental practices, while also recognizing the potential barriers for integrating such processes. They also expressed a desire for quality training and resources that can support better care for patients with mental health concerns.

All of these aspects have seen significant developments and improvements over the years, all geared toward increased clinical success and longevity. Highlighting some of these key developments, this article provides an update on resin-based composites, curing lights, and matrix bands.
Job Satisfaction, Burnout, and Intention to Leave among Dental Hygienists in Clinical Practice

Abstract:Purpose: There is limited research about the job satisfaction (JS), burnout (BO), and intention to leave (ITL) amongst dental hygienists in clinical practice providing patient care. The purpose of this study was to explore current trends and the factors influencing JS, BO, and ITL among dental hygienists in their current positions.Methods: A cross-sectional research study was conducted with a convenience sample of dental hygienists recruited via social media sites. The web-based survey consisted of three previously validated instruments (Job Satisfaction Survey, Oldenburg Burnout Inventory, and Turnover Intention Scale). Descriptive statistics were used to analyze the data.Results: The survey completion rate was 77% (n=554). Job satisfaction and burnout were associated with five factors related to ITL: frustration, achieving personal-work related goals, considering leaving, accepting another job, job satisfying personal needs, and looking forward to another day at work. Findings indicated that higher levels of JS (β=-0.95, p<0.001) predicted decreased ITL while disengagement (β=0.79, p<0.001) and exhaustion (β=0.29, p<0.001) predicted an increase in ITL (F(3, 554)=141.63, R2=0.44, p<0.001). Increased JS predicted a decrease in willingness to accept another job (β=-0.55, p<0.001). Disengagement predicted a higher willingness to accept another job (β=0.60, p<0.001) however exhaustion did not (β=0.09, p<0.001; F(3, 554)=46.89, R2=0.20, p<0.001 ).Conclusion: Findings suggest there is overall job satisfaction amongst dental hygienists in clinical practice with the exception of the lack of fringe benefits and opportunities for promotion. Employers may need to identify ways to address these concerns to retain qualified dental hygienists. In addition, employers need to be proactive in addressing factors impacting burnout and dental hygienists' intent to leave their positions.

Practice Trends and Job Satisfaction of Dental Therapists in Canada: Results from a National Survey

Abstract:The objective of this research was to evaluate the practice trends, clinical services and job satisfaction of dental therapists in Canada. Licenced Canadian dental therapists were recruited to participate in this cross-sectional study. A total of 124 dental therapists completed the survey (~68% response rate), with 57.3% of respondents being over the age of 50. Most respondents were actively engaged in full-time clinical practice in private dental offices. Indigenous dental therapists were significantly more likely to work outside of private dental offices providing care for Indigenous communities. Just over half of respondents were compensated by an annual salary, with the highest proportion of full-time practitioners earning between $75,000 and $99,000 per year. Dental therapists who were active in clinical practice performed a wide range of preventive, diagnostic, and treatment services consistent with their scope of practice. This research demonstrates that Canadian dental therapists are highly engaged and satisfied with their profession. Dental therapists can facilitate improved access to oral health care in less accessible areas of Canada; however, compensation packages and incentives to work in these less accessible areas must be addressed, as well as legal and regulatory requirements to ensure that dental therapists are licenced providers throughout Canada.

Abstract:Background: Periodontitis is one of the most prevalent oral diseases with significant implications for systemic health. This study aims to explore themes influencing successful long-term outcomes in periodontal treatment through a historical lens, focusing on various factors influencing the longevity of periodontal health and dentition stability. Methods: Utilizing an inductive qualitative thematic analysis approach, this study utilized a retrospective chart review of 19 patient records spanning, on average, 40 years. Results: Four major themes contributing to periodontal stability were identified: (1) adequate patient plaque control; (2) regular periodontal maintenance; (3) collaboration between hygienists and periodontists; and (4) the application of various periodontist/dentist-performed procedures. Conclusion: Current findings underscore the importance of these factors in preserving patients' periodontal health by emphasizing conservative treatment approaches in the maintenance and retention of the natural dentition. Key points: Adequate patient plaque control is essential for maintaining a patient's periodontal health and dentition long-term. A regular periodontal maintenance schedule should be assigned and modified as necessary for each individual patient's care. Collaborative care with hygienist colleagues and ensuring there are flexible treatment options for patients can lead to successful treatment outcomes when hygiene alone is not sufficient. This study examined patient records to identify factors contributing to long-term periodontal stability and maintenance of teeth. By analyzing 19 patient charts over an average of 40 years using a qualitative approach, four key themes were identified in successful patient treatments: effective patient plaque control, regular periodontal maintenance, collaboration between dental hygienists and periodontists, and the addition of other approaches by periodontists and dentists when necessary. The study underscores the importance of these factors in preserving periodontal health and retaining natural teeth with conservative treatment approaches. This research highlights the critical role of sustained, multifaceted dental care and professional collaboration in achieving successful long-term oral and systemic health outcomes.
