Abstract:Background: Nasal irrigation (also known as nasal douche, wash or lavage) is a procedure that rinses the nasal cavity with water or isotonic or hypertonic saline solutions. Nasal irrigation with saline (salt water) is common in modern and traditional therapy regimes. Delivered by bottle, spray, pump or nebuliser, the topical use of saline has been included as a supplement to most treatment protocols. Aim of the Study: The study aimed to evaluate the Efficacy of Saline Nasal Irrigation in Chronic Rhinosinusitis in a tertiary care hospital. Methods: This prospective observational study was conducted at the ENT outpatient department of Ent, Monno Medical College and Hospital, Manikganj, Bangladesh from January 2022 to December 2022. Results: Most patients (76%) strictly used SNI, while a smaller proportion (24%) only occasionally used it. After treatment with SNI, 46% of cases showed improvement in symptoms, while 54% showed no improvement. Statistical analysis revealed a significant difference in efficacy between improvement and no improvement (p<0.05). These findings suggest that SNI may effectively manage nasal congestion symptoms, particularly when used consistently. Conclusion: Chronic rhinosinusitis significantly negatively impacts the healthcare and economy of not only the patients but also society. Saline nasal irrigation is a practical yet easy therapy method for alleviating the symptoms of chronic rhinosinusitis and improving the quality of life in these chronic sufferers.

Abstract:BACKGROUND: Systematic reviews support nasal saline irrigation for chronic or recurrent sinus symptoms, but trials have been small and few in primary care settings. Steam inhalation has also been proposed, but supporting evidence is lacking. We investigated whether brief pragmatic interventions to encourage use of nasal irrigation or steam inhalation would be effective in relieving sinus symptoms.METHODS: We conducted a pragmatic randomized controlled trial involving adults (age 18-65 yr) from 72 primary care practices in the United Kingdom who had a history of chronic or recurrent sinusitis and reported a "moderate to severe" impact of sinus symptoms on their quality of life. Participants were recruited between Feb. 11, 2009, and June 30, 2014, and randomly assigned to 1 of 4 advice strategies: usual care, daily nasal saline irrigation supported by a demonstration video, daily steam inhalation, or combined treatment with both interventions. The primary outcome measure was the Rhinosinusitis Disability Index (RSDI). Patients were followed up at 3 and 6 months. We imputed missing data using multiple imputation methods.RESULTS: Of the 961 patients who consented, 871 returned baseline questionnaires (210 usual care, 219 nasal irrigation, 232 steam inhalation and 210 combined treatment). A total of 671 (77.0%) of the 871 participants reported RSDI scores at 3 months. Patients' RSDI scores improved more with nasal irrigation than without nasal irrigation by 3 months (crude change -7.42 v. -5.23; estimated adjusted mean difference between groups -2.51, 95% confidence interval -4.65 to -0.37). By 6 months, significantly more patients maintained a 10-point clinically important improvement in the RSDI score with nasal irrigation (44.1% v. 36.6%); fewer used over-the-counter medications (59.4% v. 68.0%) or intended to consult a doctor in future episodes. Steam inhalation reduced headache but had no significant effect on other outcomes. The proportion of participants who had adverse effects was the same in both intervention groups.INTERPRETATION: Advice to use steam inhalation for chronic or recurrent sinus symptoms in primary care was not effective. A similar strategy to use nasal irrigation was less effective than prior evidence suggested, but it provided some symptomatic benefit.TRIAL REGISTRATION: ISRCTN, no. 88204146.

Abstract:OBJECTIVE:To compare the symptoms and lower airway inflammatory factors of patients with allergic rhinitis (AR), and to observe the effect of nasal irrigation in the treatment of allergic rhinitis. METHOD:Sixty-one cases diagnosed as AR after skin prick test (SPT)were selected and randomly divided into three groups: 17 patients in group A used nasal steroid spray; 21 cases in group B used nasal irrigation; 23 patients in group C combined ir rigation with nasal steroid. Before and after 3 months treatment, nasal visual analogue scale (VAS) score, rhino conjunctivtis quality of life questionnaire (RQLQ) score, fractional exhaled nitric oxide (FENO) values were observed and compared in each group. RESULT:Before treatment, there is no statistically difference between three groups (P > 0.05). After 3 months of treatment, VAS, RQLQ, FENO of all patients was significantly decreased (P < 0.05); VAS, RQLQ score was not significantly different among the three groups (P > 0.05), FENO value has no statistically significant difference between group A and group B (P > 0.05), but were less than that in group C (P < 0.05). CONCLUSION:Nasal irrigation can ameliorate nasal symptoms, improve quality of life, decrease lower airway inflammation of allergic rhinitis patients. Nasal irrigation is an effective treatment of allergic rhinitis. Nasal irrigation combined with nasal steroid can achieve more significant efficacy.

Abstract:OBJECTIVE Gestational rhinitis(GR) is a relatively common condition in women during pregnancy.Patients with GR often complain of nasal obstruction and rhinorrhea.The exact mechanism of GR are not clear.Safe and effective treatments for this disease have not been found to date.The aim of this study is to find an appropriate treatment method for GR.METHODS Thirty patients with GR were randomly divided into two groups.There were 15 patients in hypertonic saline group with a mean age of 28.73 years (range 24-31 years),and 15 patients in normal saline group with a mean age of 25.93 years (range 24-31 years).Hypertonic saline group was treated using 3.0％ saline with a temperature of 40℃ nasal irrigation,and normal saline group was treated with 0.9％ saline with a temperature of 40℃ nasal irrigation.The duration of the intervention period was 4 weeks.Visual Analog Scale(VAS) was used to evaluate the nasal symptoms including nasal obstruction and rhinorrhea,and the health-related quality of life was assessed with the 12-item Short Form Health Survey version 2.0(SF-12v2).Contents of histamine(HIS) and acetylcholinesterase(ACHE) in nasal lavage fluid(NLF) was assessed before and after 4-week treatment in the two groups in the study.RESULTS There were 28 patients completed the study.The total VAS scores of nasal symptoms including nasal obstruction and rhinorrhea decreased,and SF-12v2 score increased in the hypertonic saline group after 2-week,3-week and 4-week interventions.Furthermore,ACHE in NLF was also increased after 4-week treatment,but HIS showed no statistical changes.The VAS scores of nasal obstruction and rhinorrhea and SF-12v2 score after 2-week,3-week and 4-week interventions,and the contents of HIS and ACHE in NLF after 4-week treatment showed no statistical differences in the normal saline nasal irrigation group.There were statistical differences in the VAS scores of nasal obstruction and rhinorrhea,SF-12v2 score and ACHE in NLF after 4-week treatment,and no significant differences in the content of HIS in NLF between the 2 groups.CONCLUSION Hypertonic saline nasal irrigation is a safe and effective treatment for GR.
Double Blinded Randomized Controlled Trial Comparing Budesonide and Saline Nasal Rinses in the Post-operative Management of Chronic Rhinosinusitis

Abstract:Post-operative management of chronic rhinosinusitis is very crucial for outcomes following surgery, Normal saline nasal irrigation and steroid spray form the standard treatment of care in this period. However nasal irrigation may not be adequate and spray is usually started after 2 weeks of surgery which in any case does not deliver optimum dosage of drug to the paranasal sinus mucosa. Budesonide nasal irrigation in a high-volume low-pressure system could be the solution for a better outcome. A double blinded randomized control trial with 88 patients in 2 groups of 44 each received normal saline or Budesonide nasal irrigation (0.5 mg in 200 ml) twice daily. Patients were followed up at 2 weeks post-operatively and 3 months, a SNOT 22 and Lund Kennedy Endoscopic scores were assessed for subjective and objective assessment. Subset analysis of only CRS patients (55) were done, and results presented. Patient reported subjective score at 3 months post operatively, SNOT22 was significantly (p < 0.0001) improved with the use of Budesonide irrigation (26.69 ± 2.92) as compared to Normal saline (30.54 ± 2.81) and objective assessment score, LKES was significantly (p = 0.0031) better in Budesonide group (4.06 + 0.74) in comparison to Normal saline in the saline (4.50 + 0.67) respectively. The mean scores 3 months post op visit was significantly lower for both subjective SNOT (p < 0.001) and objective score LKES (p < 0.0001) in Budesonide groups. Budesonide nasal irrigation with positive pressure high volume device has better patient benefits and wound healing when compared to normal saline irrigation in the post-operative management of chronic rhinosinusitis.

Abstract:Objective: To evaluate the efficacy of xylitol nasal irrigation (XNI) treatment on chronic rhinosinusitis (CRS) and to investigate the effect of XNI on nasal nitric oxide (NO) and inducible nitric oxide synthase (iNOS) mRNA in maxillary sinus.Materials and methods: Patients with CRS were enrolled and symptoms were assessed by Visiral Analog Scale (VAS) and Sino-Nasal Outcome Test 22 (SNOT-22). Nasal NO and iNOS mRNA in the right maxillary sinus were also examined. Then, they were treated with XNI (XNI group) or saline nasal irrigation (SNI, SNI group) for 30 days, after which their symptoms were reassessed using VAS and SNOT-22, and nasal NO and iNOS mRNA in the right maxillary sinus were also reexamined.Results: Twenty-five out of 30 patients completed this study. The scores of VAS and SNOT-22 were all reduced significantly after XNI treatment, but not after SNI. The concentrations of nasal NO and iNOS mRNA in the right maxillary sinus were increased significantly in XNI group. However, significant changes were not found after SNI treatment. Furthermore, there were statistical differences in the assessments of VAS and SNOT-22 and the contents of nasal NO and iNOS mRNA in the right maxillary sinus between two groups.Conclusions: XNI results in greater improvement of symptoms of CRS and greater enhancement of nasal NO and iNOS mRNA in maxillary sinus as compared to SNI. Abstract:Previous studies have shown that whole royal jelly, a fraction from royal jelly (10-hydroxy-2-decenoic acid), and certain closely related dicarboxylic acids, some of which are also found in royal jelly, will inhibit the development of transplantable AKR leukemia when the pH is below 5.6.The ester of 10-hydroxy-2-decenoic acid from royal jelly was found to be just as effective against AKR leukemic cells as the acid itself, with the added advantage that it could be used at neutrality.Through the testing of a series of mono- and di-carboxylic acids, as well as other closely related compounds, the activity has been shown to be associated mainly with 9- and 10-carbon straight chain monocarboxylic acids either saturated or unsaturated. Slight variations in the structure either reduce or destroy the activity.
A Novel Powered Nasal Irrigation After Endoscopic Sinus Surgery in Patients With Chronic Rhinosinusitis: A Preliminary Study

Abstract:Background and Objectives We aimed to compare the efficacy of a novel powered irrigation system with that of the manual bottle-squeeze method for postoperative healing after endoscopic sinonasal surgery (ESS).Subjects and Method In this prospective randomized clinical trial, 29 patients were enrolled for nasal irrigation (NI) with either NOSSHA® (Womens Care Co., Ltd.) powered irrigation system (NOSSHA® group, n=14) or manual irrigation (control group, n=15). Objective findings were evaluated using the modified Lund-Kennedy scores. Subjective outcomes were assessed using the total nasal endoscopic score (TNES), total nasal symptom score (TNSS), visual analog scale (VAS), and quality of life (QOL) questionnaires in each group at baseline, and 1, 2, 4, 6, and 8 weeks after ESS. We compared the postoperative changes between both groups.Results The mean TNES of patients were significantly higher in the NOSSHA® group than in the control group (p=0.015); however, the improvement in TNES was achieved 2 weeks earlier in the NOSSHA® group. The improvement in TNES (p<0.001) and TNSS (p<0.001) was statistically significant in both groups. The improvement in QOL was statistically significant in the NOSSHA® (p<0.001) and control group (p=0.007). The improvement in the TNSS and QOL was earlier in the NOSSHA® group by 4 and 7 weeks, respectively; no early improvement occurred in the NOSSHA® group for the VAS score.Conclusion We validated the usefulness of postoperative NI using a powered device, which may be useful for patients who cannot tolerate manual NI.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Objectives: We investigated the effectiveness of different types of nasal irrigation sprays in adult allergic rhinitis (AR) patients. Methods: A total of 1700 patients with AR (866 males and 834 females) were assigned to: Group 1: Hypertonic nasal spray group (n = 600) (Sinomarin® hypertonic nasal spray); Group 2: Algae-containing hypertonic nasal spray group (n = 600) (Sinomarin Plus Algae ENT); and Group 3: Isotonic saline nasal spray group (n = 500). All patients underwent an otolaryngological examination, continued their standard AR treatment, and received the assigned nasal spray additionally (1 spray to each nostril, 3 times a day, for 3 weeks). Allergic symptom scores, turbinate examination, total symptom scores, and quality of life (QoL) scores were evaluated during pre- and post-treatment periods. Results: In groups 1 and 2, symptom scores and turbinate color and edema, total symptom scores, and QoL scores increased after treatment (P < .05). In the saline group, there were no significant differences in symptom scores and total symptom scores after treatment; however, improvement was detected in turbinate color and edema values after treatment. QoL scores increased after treatment. When comparing the 3 groups, the total symptom scores of groups 1 and 2 were significantly lower, and the QoL scores of groups 1 and 2 were considerably higher than those of the saline group. There were no significant differences between groups 1 and 2. Conclusion: Algae-containing and hypertonic nasal spray may be added to the standard AR treatment to increase QoL and decrease total symptom scores.

Daily Saline Nasal Douching for Chronic Allergic Rhinosinusitis: An Autobiographical Case Report

Abstract:Chronic allergic rhinosinusitis is a debilitating condition in a group of susceptible individuals with multifactorial etiology. I present my case of suffering from chronic allergic rhinosinusitis for 25 years treated with multiple drugs and surgeries with no relief. Adjunctive daily saline nasal douching has provided significant relief for three years to date. Genetic susceptibility with defective ciliary clearance, mucosal metaplasia with increased goblet cells secreting thick and inspissated mucus blocking the normal drainage, and stasis of secretions with inadequate mucosal immunity predisposes to polymicrobial infections, including fungal infections, antimicrobial resistance, and recurrent episodes of rhinosinusitis. Medical and surgical strategies often fail to manage this condition appropriately in a few cases. Adjunctive daily saline nasal douching is a safe and effective alternative to manage the debilitating symptoms of chronic allergic rhinosinusitis.

Abstract:We report the case of a 60-year-old female with a history of hypertension who was admitted with an acute inferior myocardial infarction. She received rescue percutaneous transluminal coronary angioplasty/stenting of an occluded right coronary artery for failed thrombolysis with a good initial result. However, this was immediately complicated by cardiogenic shock characterized by left ventricular outflow tract (LVOT) gradient. She was treated with intravenous fluids and adrenaline. Predischarge echocardiography showed no LVOT gradient and features of left ventricular hypertrophy that mainly affected the septum.
Nasal irrigation for the treatment of vasomotor rhinitis: a pilot study

Abstract:Objective To observe the therapeutic effect of simple 3.0％ saline nasal irrigation and combined treatment of 3.0％ saline nasal irrigation and budesonide nasal spray for vasomotor rhinitis (VMR),and explore the long-term effect for VMR.Through examination of levels of substance P (SP) and mucin (MUC)5B in nasal lavage fluid,the mechanisms of nasal irrigation treatment for VMR was discussed.Methods One hundred and one patients from Department of Otorhinolaryngology Head and Neck Surgery,Huashan Hospital of Fudan University with VMR were randomly divided into 4 groups.The number of patients was 24 in control group,25 in budesonide nasal spray treatment group (budesonide group),25 in nasal irrigation treatment group (nasal irrigation group) and 27 in budesonide nasal spray + nasal irrigation group (combined treatment group).Control patients were left untreated.Budesonide group was under budesonide nasal spray treatment,nasal irrigation group was treated using 3.0％ saline with a temperature of 40℃ and combined treatment group was given both treatments.The duration of the intervention period was 3 months (90 days).Visual Analog Scale (VAS) was used to evaluate nasal symptoms,and the healthrelated quality of life was assessed using the 12-item Short Form Health Survey version 2.0 (SF-12v2).Enzyme-linked immunosorbent assay (ELISA) was used to assess the contents of SP and MUC5B in nasal lavage fluid before and after 3-month treatments in budesonide and nasal irrigation group in the study.MUC5B in nasal lavage fluid after the SP challenge and anticholinergic drug intervention in control group were also evaluated with ELISA.Results Nighty out of 101 patients completed the study.In the budesonide and combined treatment group after relevant interventions,the total VAS score of nasal symptoms decreased (5.91 ±0.21 vs 3.82 ±0.15,6.18±0.17 vs 3.92 ±0.15,t value was 8.193,10.060,respectively,all P ＜ 0.05) and SF-12v2 score increased (146.00 ± 1.23 vs 152.30 ± 0.97,146.00 ± 1.08 vs155.40 ± 0.90,t value was 3.982,6.697,respectively,all P ＜ 0.05),with both scores showed no significant differences in the nasal irrigation group (5.96 ± 0.17 vs 5.72 ± 0.15,146.10 ± 1.17 vs 147.00 ±0.94,t value was 1.038,0.607,respectively,all P ＞ 0.05) after the first month.In the budesonide and combined treatment group after relevant interventions,the total VAS score of nasal symptoms decreased (5.91 ±0.21 vs 5.05 ±0.15,6.18 ±0.17 vs 5.10 ±0.12,t value was 3.374,5.351,respectively,all P ＜ 0.05) and SF-12v2 score increased (146.00 ± 1.23 vs 150.90 ± 0.76,146.00 ± 1.08vs 153.60 ± 0.94,t value was 3.373,5.343,respectively,all P ＜ 0.05),with both scores showed no significant differences in the nasal irrigation group (5.96 ± 0.17 vs 5.78 ± 0.17,146.10 ± 1.17 vs 148.10 ±0.80,t value was 0.716,1.438,respectively,all P ＞0.05) after the second month.By the end of the third month,in nasal irrigation and combined treatment group,the VAS score was diminished (5.96±0.17 vs 4.80 ±0.12,6.18 ±0.17 vs 4.44 ±0.13,t value was 5.485,8.264,respectively,all P ＜0.05) and SF-12v2 score was elevated (146.10 ± 1.17 vs 150.80 ±0.96,146.00 ± 1.08 vs 152.90 ±0.85,t value was 3.163,5.008,respectively,all P ＜ 0.05),but there were no significant differences in budesonide group (5.91 ±0.21 vs 5.68 ±0.18,146.00 ± 1.23 vs 148.40 ±0.85,t value was 0.819,1.587,respectively,all P ＞ 0.05).Additionally,SP in nasal lavage fluid decreased and MUC5B showed no statistical changes in budesonide group after three months,however,SP showed no any changes and MUC5B reduced significantly in nasal lavage fluid in nasal irrigation group.Furthermore,the anticholinergic drug could not decrease the concentration of MUC5B after the SP challenge in nasal cavity in control group.Conclusions The therapeutic effect of simple nasal irrigation with 3.0％ saline or combined treatment of 3.0％ saline nasal irrigation and nasal corticosteroids is superior to simple nasal corticosteroids.Nasal corticosteroids plays a role in the inhibition of sensory nerve endings in nasal mucosa,but neurotransmitter plays a limited role in the pathogenesis of VMR.
Effect of hypertonic saline on mucociliary clearance in patients with chronic rhinosinusitis

Abstract:Objective: To explore the effect of hypertonic saline on nasal mucociliary clearance in patients with chronic rhinosinusitis.Methods: The saccharin test was used to determine the mucociliary clearance before irrigation and after normal saline or hypertonic saline irrigation.On the following days,an alternative solution was used to repeat the treatment and retest the mucociliary clearance time.Results: The normal saccharin clearance time of patients with chronic rhinosinusitis was 19.62 minutes,and it changed to 17.74 and 13.22 minutes after the normal saline and hypertonic saline irrigation,respectively.Compared with the normal group,the mucociliary clearance was not significantly changed in the normal saline group(P>0.05),but significantly decreased in the hypertonic saline group(P<0.01).Conclusion: Hypertonic saline can significantly improve the nasal mucociliary clearance time,while normal saline has no such effect.
Clinical study of nasal irrigation and budesonide nasal spray in the treatment of vasomotor rhinitis

Abstract:Objective Vasomotor rhinitis (VMR) is a common disease that is unrelated to allergy and infection. The exact pathogenesis of VMR is not clear. Effective long-term treatments for VMR have not been found to date. This paper compared the curative effect of nasal irrigation and budesonide nasal spray in the treatment of vasomotor rhinitis, aimed to select a long-term and effevtive treatment. Methods 101 patients with VMR were randomly divided into 4 groups. The mean age was 40.75±13.46 years old for the control group (range, 19~61 years old; 13 male, 11 female; n=24), 41.16 ±13.56 years old for the budesonide nasal spray treatment group (budesonide group) (range, 18~66 years old;13 male, 12 female;n=25), 43.88 ±9.27 years old for the nasal irrigation treatment group (nasal irrigation group) (range, 19~66 years old; 14 male, 11 female; n=25) and 43.59 ±11.40 years old for the budesonide nasal spray + nasal irrigation group (combined treatment group) (range, 20~64 years old; 13 male, 14 female; n=27). The control group patients were left untreated. Budesonide group took budesonide nasal spray treatment (each 2 sprays per nostril, 1 times daily), nasal irrigation group used 3.0% saline with a temperature of 40°C irrigate the nasal. Combined treatment group were given both treatments. The duration of the intervention period was 3 months (90 days). Visual Analog Scale (VAS) was used to evaluate nasal symptoms and the health-related quality of life was assessed with the 12-item Short Form Health Survey version 2.0 (SF-12v2). Results 90 VMR patients completed the study. At the end of the third month, the VAS score (t=5.690, P<0.0001) (t=7.054, P<0.0001) was diminished and SF-12v2 score (t=3.660, P<0.001) (t=4.971, P<0.0001) was elevated in nasal irrigation and combined treatment group versus control group, but there were no significant differences (t=1.247, P>0.05, t=1.910, P>0.05) between budesonide and control group. In addition, there were statistically significant differences between nasal irrigation (t=4.055, P<0.001, t=2.233, P<0.05) or combined (t=5.460, P<0.0001, t=3.714, P<0.001) treatment group and budesonide group, but no statistical differences (t=1.814, P>0.05, t=1.257, P>0.05) were found between nasal irrigation and combined treatment group. Conclusion Simple budesonide nasal spray is not an appropriate long-term treatment for VMR. Nasal irrigation or combined treatment of budesonide with nasal irrigation may be an appropriate therapy.
Baby shampoo nasal irrigations for the symptomatic post-functional endoscopic sinus surgery patient

Abstract:Background: Symptoms of postnasal drainage and thickened mucus are commonly seen in patients with chronic rhinosinusitis (CRS) recalcitrant to sinus surgery and conventional medical therapies. Chemical surfactants can act as a mucolytic by reducing water surface tension and have the potential to serve as an antimicrobial agent. Baby shampoo is an inexpensive, commercially available solution containing multiple chemical surfactants. This is an in vitro study of its antimicrobial effects on Pseudomonas biofilms with translation to a clinical study for use as an adjuvant nasal wash in patients with CRS who remain symptomatic despite adequate sinus surgery and conventional medical therapies. Methods: In vitro testing was performed to determine the optimal concentration of baby shampoo that disrupted preformed bacterial biofilms and inhibited biofilm formation. This concentration was then used in a prospective study of symptomatic post-functional endoscopic sinus surgery (FESS) patients who irrigated twice a day for 4 weeks. Validated outcome forms and objective smell testing was performed before and after therapy. Results: One percent baby shampoo in normal saline was the optimal concentration for inhibition of Pseudomonas biofilm formation. Baby shampoo had no effect on the eradication of preformed Pseudomonas biofilms. Eighteen patients with CRS with an average of 2.8 surgeries were studied after irrigating with 1% baby shampoo solution. Two patients discontinued use because of minor nasal and skin irritations; 46.6% of patients experienced an overall improvement in their subjective symptoms, and 60% of patients noted improvement in specific symptoms of thickened mucus and postnasal drainage. Conclusion: Baby shampoo nasal irrigation has promise as an inexpensive, tolerable adjuvant to conventional medical therapies for symptomatic patients after FESS. Its greatest benefit may be in improving symptoms of thickened nasal discharge and postnasal drainage.
Determination of the effect of topical application of N-acetylcysteine on the recovery rate of patients with chronic rhinosinusitis after surgical treatment, based on SNOT-22 test scores

Abstract:endonasal endoscopic sinus surgery ( FESS ) is the gold standard for the treatment of patients with chronic rhinosinusitis worldwide. The task of the otorhinolaryngologist is aimed at restoring the function of the osteomyetal complex . However, the tactics of the postoperative period, especially the topical form of treatment, remain an extremely important issue. The purpose of the study: Determination of the influence of the topical form of N-acetylcysteine on the speed of recovery of patients after FESS, compared to standardized topical treatment, based on the sino-nasal outcome test. The study was carried out at the Department of Otorhinolaryngology of Bogomolets National Medical University on the basis of the Oleksandriv Clinical Hospital of Kyiv together with the CNME "Kremenchuk City Hospital of Planned Treatment", Kremenchuk. A population of 96 patients aged 18 to 60 with a diagnosis of chronic rhinosinusitis after surgical treatment. The randomization method was two groups of patients were formed. 1st group (study) - 47 patients who received irrigation of the topical form of N-acetylcysteine. 2nd group (control) - 49 patients who received topical therapy using irrigation of 0.9% NaCl solution. The volume of surgical intervention of both groups of the study: Endonasal endoscopic maxillotomy, septoplasty , bilateral vasotomy of the lower nasal turbinates. To participate in the study, all participants had to sign an informed consent. All patients were surveyed using the SNOT22 test on the 1st day before surgery treatment and on the 3rd, 10th and 28th days after surgical treatment.According to the obtained results, the effectiveness of both methods of topical treatment was revealed (p=0.001). However, the use of topical N -acetylcysteine in patients of the study group has more effective dynamics of symptoms, according to the SNOT -22 questionnaire on the 3rd (p=0.034), 10th (p=0.002) and 28th day (p=0.001) after surgical treatment. Therefore, the topical form of N-acetylcysteine has an effective effect on the speed of recovery of the mucous membrane of the nasal cavity and paranasal sinuses and improves the quality of life in patients with chronic rhinosinusitis after FESS.
Effect of nasal irrigation in adults infected with Omicron variant of COVID-19: A quasi-experimental study

Abstract:Objective: To investigate the effect of nasal irrigation on the duration of symptoms and nucleic acid conversion in adults infected with the Omicron variant of COVID-19. Methods: This quasi-experimental study enrolled patients diagnosed with asymptomatic, mild, or moderate Omicron infection at the Shandong Public Health Clinical Center between April 1, 2022 and May 1, 2022. Patients were divided into two groups to receive Lianhua Qingwen granules and traditional Chinese medicine (TCM) prescriptions (conventional group) and 3% hypertonic saline nasal irrigation based on conventional treatment (nasal irrigation groups), respectively. Primary outcomes were symptom disappearance time and nucleic acid negative conversion time. Secondary outcomes were peripheral blood white blood cell (WBC), lymphocyte (LYM) count, neutrophil (NEU) count, C-reactive protein (CRP) level, and chest CT examination findings. Results: Eighty patients were included (40 patients/group). Multiple linear regression analysis showed that, after adjustment for comorbidities, smoking history, LYM count, and Ct values of N gene, the patients in the nasal irrigation group were more likely to get lower nucleic acid negative conversion time (β = -11.052, 95% CI: -8.277-13.827, P < 0.001) compared with the conventional group. The symptom disappearance time showed no significant improvement (P > 0.05). Subgroup analysis for treatment-naïve patients in the nasal irrigation group showed similar nucleic acid negative conversion time improvement (P = 0.038). Conclusion: Early nasal irrigation shortens the nucleic acid negative conversion time in adults infected with the Omicron variant but without improvements in symptom disappearance time.
Efficacy of mometasone furoate nasal spray in the treatment of acute rhinosinusitis

Abstract:Abstract Background Acute rhinosinusitis (ARS) refers to a group of disorders characterized by inflammation of the respiratory epithelium of the nose and paranasal sinuses lasting from 7 to 28 days. In the treatment of ARS in addition to an antibiotic, intranasal corticosteroids hasten the clearance of bacteria, decrease the frequency and severity of disease recurrence, and reduce the duration of infection. The purpose is to compare the efficacy of the combination of mometasone furoate nasal spray (MFNS) with amoxicillin and amoxicillin alone in the treatment of acute rhinosinusitis. A total of 120 patients (≥ 12 years) were randomized into 2 groups: group A ( N : 60) receiving amoxicillin 500 mg thrice daily alone and group B ( N : 60) receiving amoxicillin 500 mg thrice daily and MFNS 200 μg twice daily for 7 days. Patients were followed up after 7 days. The Sino-Nasal Outcome Test-22 (SNOT-22) questionnaire was taken before and after. The total score of SNOT-22 was compared between the groups. Results There was a reduction in the mean total SNOT score in both groups from 21.32 ± 11.29 to 9.37 ± 6.55 in group A and from 26.68 ± 11.97 to 3.07 ± 3.46 in group B which were statistically significant ( p < 0.001) in both groups. The posttreatment mean score with the amoxicillin group was 9.31 ± 6.55 and that of the amoxicillin and mometasone furoate group was 3.07 ± 3.46, and their mean difference was 6.3 ± 0.95. In comparison, MFNS with amoxicillin was significantly ( p < 0.001) superior than amoxicillin alone. Conclusion Patients receiving amoxicillin alone or amoxillin with MFNS, both showed improvement of symptoms in ARS. However, amoxicillin with MFNS showed significantly higher improvement and relief of symptoms in ARS than amoxicillin alone.

Abstract:ABSTRACT Three asparagine synthetase genes, asnB,asnH, and asnO (yisO), were predicted from the sequence of the Bacillus subtilisgenome. We show here that the three genes are expressed differentially during cell growth. In a rich sporulation medium, expression ofasnB was detected only during exponential growth, that ofasnH was drastically elevated at the transition between exponential growth and stationary phase, and that of asnOwas seen only later in sporulation. In a minimal medium, bothasnB and asnH were expressed constitutively during exponential growth and in stationary phase, while the expression of asnO was not detected in either phase. However, when the minimal medium was supplemented with asparagine, only the expression ofasnH was partially repressed. Transcription analyses revealed that asnB was possibly cotranscribed with a downstream gene, ytnA, while the asnH gene was transcribed as the fourth gene of an operon comprisingyxbB, yxbA, yxnB, asnH, and yxaM. The asnO gene is a monocistronic operon, the expression of which was dependent on one of the sporulation sigma factors, sigma-E. Each of the three genes, carried on a low-copy-number plasmid, complemented the asparagine deficiency of anEscherichia coli strain lacking asparagine synthetases, indicating that all encode an asparagine synthetase. In B. subtilis, deletion of asnO or asnH, singly or in combination, had essentially no effect on growth rates in media with or without asparagine. In contrast, deletion ofasnB led to a slow-growth phenotype, even in the presence of asparagine. A strain lacking all three genes still grew without asparagine, albeit very slowly, implying that B. subtilismight have yet another asparagine synthetase, not recognized by sequence analysis. The strains lacking asnO failed to sporulate, indicating an involvement of this gene in sporulation.
