Abstract:Dysautonomic symptoms are associated with many disorders of the central nervous system, in some of which the autonomic dysfunction is particularly conspicuous or the major cause of disability. New neuroimaging procedures and microneurographic techniques have helped to clarify the underlying pathophysiology and to distinguish or, classify disorders that are clinically similar. Recent studies also suggest that the autonomic nervous system is involved more frequently or extensively than previously thought in certain neurological conditions such as multiple sclerosis or human immunodeficiency virus (HIV) infection, and have helped to define the nature of its involvement in other disorders.
The MMPI-A and 13-year-old inpatients: how young is too young?

Abstract:This study examined two questions in the use of the MMPI-A with 13-year-old inpatients: (a) are the profiles of 13-year-old inpatients markedly different from those produced by 14-year-old inpatients, and (b) what is the effect of scoring with standard MMPI-A norms versus Archer MMPI-A norms for 13-year-olds. Protocols from 56 13-year-old and 85 14-year-old psychiatric inpatients were analyzed. No significant differences were found for age in mean T scores, and no clear pattern of differences was found in percentage of elevations into the clinical range (T 65) for validity, clinical, content, or supplemental scales. A strong multivariate effect was found with the use of Archer MMPI-A norms resulting in lower T-score values than when standard MMPI-A norms are used. However, univariate differences based on norms were not routinely found with statistically significant differences in only 2 of the 38 scales for males and 7 of the 38 scales for females. The two major exceptions to this finding were that the use of Archer norms versus standard MMPI-A norms resulting in significantly higher mean T scores and a significantly higher percentage of cases falling within the clinical range for Scale 1 and the depression content scale.

Abstract:Since the Minnesota Multiphasic Personality Inventory (MMPI) has been shown to be useful in discriminating between adults who have organically-based somatic symptoms and other adults whose somatic symptoms are deemed to be functional, an investigation of its usefulness in this regard with adolescents (in the 13 through 16 age range) was undertaken. MMPI scores of 44 adolescents who came to a general hospital with somatic symptoms and who were found to have an organic disease were compared with the scores of 29 adolescents who came with similar symptoms for which no organic basis could be found and who were ultimately diagnosed as having a functional or psychogenic disorder. The groups were similar in terms of age, education, socioeconomic status, and race. Both boys and girls in the functional group had significantly higher mean scores on the first three scales of the MMPI ( Hs, D and Hy), and the girls had a higher mean score on Scale 7 ( Pt).

Future directions for the MMPI-A: research and clinical issues

Abstract:The Minnesota Multiphasic Personality Inventory-Adolescent (MMPI-A; Butcher et al., 1992), released in 1992, was developed specifically for use with adolescent respondents. The purpose of this article is to offer suggestions concerning 6 areas of productive research with this instrument. These areas include studies of the utility of codetype interpretation, issues related to profile elevation in clinical samples, and the identification of criterion for evaluating the usefulness of traditional and new MMPI-A scales. Further, these research issues also include establishing the optimal age ranges for use with the MMPI-A, detecting the effects of the release of a revised instrument on clinician's test use patterns with adolescents, and evaluating the optimal methods for the use of the MMPI-A Structural Summary interpretative approach. It is noted that these research recommendations are by no means exhaustive, and that many other research areas should also be investigated in developing a comprehensive research literature for this revised instrument.

Abstract:The Minnesota Multiphasic Personality Inventory (MMPI) instruments have a longstanding history of clinical applications in neuropsychological settings, and many studies have supported the utility of MMPI-2-Restructured Form scores among specific neuropsychological populations. However, the MMPI-3, a new version of the MMPI, has yet to be studied in a clinical neuropsychological setting. To address this need, we investigated the psychometric properties of MMPI-3 scores using a sample of 197 independent practice outpatient neuropsychology examinees (56.9% women; 80.2% White). Internal consistency coefficients and standard errors of measurement were generally consistent with statistics reported for two comparison groups included in the MMPI-3 Technical Manual, supporting the reliability of MMPI-3 scores. Correlations with external criteria-including presenting problems, neurocognitive, psychological/behavioral, and psychiatric diagnostic variables-supported the convergent and discriminant validity of MMPI-3 scores. Finally, relative risk analyses showed meaningful and theoretically expected findings, supporting the utility of the MMPI-3 in a clinical neuropsychological setting. Abstract:This study examined the association between scores on the Minnesota Multiphasic Personality Inventory–2–Restructured Form (MMPI-2-RF) with static and dynamic risk assessment instruments, including the STATIC-99 and Level of Service Inventory–Revised (LSI-R). The sample included 304 male adults who were convicted of sexual offenses against children and were referred to a sex offender treatment program. On average, the sample had a Low-Moderate risk of re-offending according to the STATIC-99 and LSI-R. The results indicated that MMPI-2-RF scale scores in this setting are characterized by relatively high levels of under-reporting and externalizing psychopathology compared with the normative sample. We also found that scale scores in this sample produced reliability estimates that were similar to the normative sample. Finally, external correlations between the MMPI-2-RF scales and the risk assessment instruments indicated that the test was associated in expected ways with constructs measured by these instruments. Correlations were most robust among scales in the externalizing/behavioral dysfunction domain of the MMPI-2-RF. Overall, the results of the study support and guide use of the test in this population.
Relations Between Minnesota Multiphasic Personality Inventory–A Scales and Rorschach Variables With the Scope and Severity of Maltreatment Among Adolescents

Abstract:This study examined preexisting Rorschach (Exner, 2001) and Minnesota Multiphasic Personality Inventory-A (MMPI-A; Butcher et al., 1992) profiles to determine if selected MMPI-A scales and Rorschach variables would jointly associate with the number and severity of maltreatment subtypes (physical abuse, sexual abuse, neglect, and emotional maltreatment) of 157 adolescents (ages 14-17) with documented maltreatment histories. The Maltreatment Classification System was used to systematically code the maltreatment attributes. Six Rorschach variables (MOR, PER, Afr, SumY, SumC', Human Content) were significantly correlated with the number of maltreatment subtypes, but none of the anticipated MMPI-A scales were related. MMPI-A Scale 7 and Rorschach variables Ego, MOR, and PER were jointly associated with physical abuse severity. MMPI-A Scale 0 and Rorschach variables MOR, PER, SumY, SumC', PTI, Human Content, and Texture jointly associated with sexual abuse severity. This study supports the potential for certain MMPI-A scales and Rorschach variables to reflect the impact of adolescents' maltreatment experiences in terms of the number and severity of types of maltreatment experienced. Because both instruments captured different aspects of adolescents' maltreatment experiences, clinicians should consider using both when evaluating the impact of maltreatment on adolescents.

Abstract:The Minnesota Multiphasic Personality Inventory–3 (MMPI-3; Ben-Porath & Tellegen, 2020a) was developed to expand the content coverage of the MMPI-2–Restructured Form (MMPI-2-RF; Ben-Porath & Tellegen, 2008/2011) and to update the test's norms to better represent the U.S. population. Because most MMPI-3 items were carried over from the MMPI-2-RF, most MMPI-3 scales can be either fully scored or prorated from the MMPI-2/MMPI-2-RF item pool. We describe development of a procedure for deriving MMPI-3 scores from MMPI-2/MMPI-2-RF protocols and examine the psychometric properties of prorated MMPI-3 scores across various samples. Following Tarescavage et al. (2016), who developed a method for prorating most MMPI-2-RF scores from the original MMPI, prorated MMPI-3 scales had to meet three criteria: (a) sufficient representation in the MMPI-2-RF item pool, (b) association with the full-length corresponding MMPI-3 scale at r ≥ .90 in multiple samples, and (c) comparable T score means and standard deviations in multiple samples. These criteria resulted in the exclusion of the three MMPI-3 non-content-based Validity Scales, as well as nine Specific Problems Scales, yielding 16 MMPI-3 scales that can be fully scored and 24 for which psychometrically comparable prorated score estimates can be derived from responses to the MMPI-2 or MMPI-2-RF item pools. Evidence of the psychometric functioning of the resulting scale scores and their comparability with the full versions of these scales across samples is presented and discussed. Abstract:We examined associations between prehire Minnesota Multiphasic Personality Inventory-2–Restructured Form (MMPI-2-RF) scores and posthire performance ratings for a sample of 131 male police officers. Substantive scale scores in this sample were meaningfully lower than those obtained by the test’s normative sample and substantially range restricted, but scores were consistent with those produced by members of the police candidate comparison group (Corey & Ben-Porath). After applying a statistical correction for range restriction, we found several associations between MMPI-2-RF substantive scale scores and supervisor ratings of job-related performance. Findings for scales from the emotional dysfunction and interpersonal functioning domains of the test were particularly strong. For example, scales assessing low positive emotions and social avoidance were associated with several criteria that may be affected by lack of engagement with one’s environment and other people, including problems with routine task performance, decision making, assertiveness, conscientiousness, and social competence. Implications of these findings for assessment science and practice are discussed.

Abstract:Minnesota Multiphasic Personality Inventory-2-Restructured Form scores for 145 male police officer candidates were compared with supervisor ratings of field performance and problem behaviors during their initial probationary period. Results indicated that the officers produced meaningfully lower and less variant substantive scale scores compared to the general population. After applying a statistical correction for range restriction, substantive scale scores from all domains assessed by the inventory demonstrated moderate to large correlations with performance criteria. The practical significance of these results was assessed with relative risk ratio analyses that examined the utility of specific cutoffs on scales demonstrating associations with performance criteria.

Abstract:<span>This study investigated the relationship between symptom validity scales on the Clinical Assessment of Attention Deficit-Adult (CAT-A) and the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) in the context of Attention-Deficit/Hyperactivity Disorder (ADHD) evaluation. The sample comprised 140 consecutive patients referred for a neuropsychological evaluation of ADHD and were administered the CAT-A and the MMPI-2-RF and a battery of performance-based neurocognitive tests. Results indicated CAT-A/MMPI-2-RF symptom validity concordance of 51% between measures, with 38% concordant valid and 13% concordant invalid responses. Among those with discordance symptom validity results, rates of valid CAT-A/invalid MMPI-2-RF responding (41%) were more common than invalid CAT-A/valid MMPI-2-RF responding (8%). Results also indicated higher levels of ADHD symptoms among invalid responding within the CAT-A, whereas the MMPI-2-RF Cognitive Complaints scale did not differ by CAT-A validity status. Finally, symptom validity scales on both the CAT-A and MMPI-2-RF were largely discordant from neuropsychological test validity status per performance validity tests. Findings highlight the need for symptom validity testing when assessing ADHD and indicate that validity indices on broad personality assessments may assess different constructs than embedded validity indices in ADHD-specific measures.</span>
Examining the Psychometric Equivalency of MMPI-3 Scale Scores Derived From the MMPI-3 and the MMPI-2-RF-EX

Abstract:The current study evaluated the comparability of Minnesota Multiphasic Personality Inventory–3 (MMPI-3) scale scores derived from the 335-item MMPI-3 to MMPI-3 scale scores derived from the 433-item MMPI-2 restructured form–expanded version (MMPI-2-RF-EX), an enhanced version of the MMPI-2-RF that was used to develop and validate the MMPI-3. To that end, we examined data from 192 college undergraduates who completed both the MMPI-3 and MMPI-2-RF-EX 1 week apart using a counterbalanced design. Across versions, mean T-scores and standard deviations, estimates of internal consistency, and standard error of measurement values, were highly similar, indicating no clinically meaningful differences across versions. We also compared between-version test–retest comparability values with within-version values calculated using a sample of undergraduates ( N = 318) who completed the MMPI-2-RF-EX twice over the same time interval, finding only marginal differences across the two samples. Finally, we computed column-vector correlations between MMPI-3 scores from both versions and several criterion measures, where results reflected no effect of test version on external validity. Overall, we determined that scale scores derived from either booklet are psychometrically interchangeable, indicating that MMPI-3 scale scores obtained from an administration of the MMPI-2-RF-EX can be applied when using the 335-item MMPI-3.

Abstract:Overreporting is a common problem that complicates psychological evaluations. A challenge facing the effective detection of overreporting is that many of the identified strategies (e.g., symptom severity approaches; see Rogers & Bender, 2020) are not incorporated into broadband measures of personality and psychopathology (e.g., Minnesota Multiphasic Personality Inventory family of instruments). While recent efforts have worked to incorporate some of these newer strategies, no such work has been conducted on the MMPI-3. For instance, recent symptom severity approaches have been used to identify patterns of multivariate base rate "skyline" elevations on the BASC, and similar strategies have been adopted into the PAI to measure psychopathology (Multi-Feigning Index; Gaines et al., 2013) and cognitive symptoms (Cognitive Bias Scale of Scales; Boress et al., 2022b). This study used data from a simulation study ( n = 318) and an Active-Duty (AD) clinical sample ( n = 290) to develop and cross-validate such a scale on the MMPI-2-RF and MMPI-3. Results suggest that the MMPI SOS (Scale of Scales) scores perform equitably to existing measures of overreporting on the MMPI-2-RF and MMPI-3 and incrementally predict a PVT-classified "known-group" of Active Duty service members. Effects were generally large in magnitude. Classification accuracy achieved desired specificity (.90) and approximated expected sensitivity (.30). Implications of these findings are discussed, which emphasize how alternative overreporting detection strategies may be useful to consider for the MMPI. These alternative strategies have room for expansion and refinement.

Abstract:This study examined the utility of the Minnesota Multiphasic Personality Inventory-3 (MMPI-3) in the assessment of personality disorders (PDs). Participants included undergraduate students who were administered the MMPI-3 and the Personality Inventory for the DSM-5-Short Form (PID-5-SF) as a measure of dimensional pathological personality traits. We examined associations between the MMPI-3 and the Alternative Model for Personality Disorders (AMPD) of the DSM-5, with a particular emphasis on the MMPI-3 Personality Psychopathology Five (PSY-5) scales. Results demonstrated MMPI-3 scales were largely convergent with both the PID-5 domain and facet scores. Given their conceptual overlap, we examined the joint factor structure of the MMPI-3 PSY-5 scales and the PID-5-SF trait facets and found strong empirical convergence of these conceptual models. The results provide a necessary update to personality assessment with the MMPI-3 and provide further insight into the use of the MMPI instruments in assessing personality psychopathology in the AMPD.

Abstract:The purpose of the present study was to revise and update the MMPI-2-RF personality disorder (PD) syndrome scales for the Minnesota Multiphasic Personality Inventory–3 (MMPI-3). Study 1 describes the development of the MMPI-3 PD syndrome scales in three separate samples of community participants (n = 1,591), university students (n = 1,660), and outpatient mental health patients (n = 1,537). The authors independently evaluated each of the 72 new MMPI-3 items and rated them for appropriateness for scale inclusion and used various statistical procedures for final item selection. Ultimately, all 10 scales were revised, with nine incorporating items that were new to the MMPI-3. In Study 2, we subsequently validated the new MMPI-3 PD Syndrome scales against measures of traditional PD measures, trait measures of the Alternative Diagnostic and Statistical Manual of Mental Disorders, fifth edition-5 model of personality disorders (AMPD) and the five-factor model (FFM) of personality, and specific criterion measures of externalizing, psychopathy, narcissism, emotional dysregulation, and self-harm, in two samples of university students (ns = 489 and 645). With some exceptions, the results were generally supportive of the convergent and discriminant validities of the MMPI-3 PD Syndrome scales. The Histrionic PD scale in particular was associated with questionable results and diverged most strongly from the theoretical construct it was originally meant to reflect. Comparing adult and adolescent transsexuals: an MMPI-2 and MMPI-A study

Abstract:Sex, sexual orientation and age have been shown to be important in relation to psychological functioning in transsexuals. However, only few studies to date took these factors into account and not earlier have adolescent transsexuals participated. In this study the Minnesota Multiphasic Personality Inventory (MMPI-2 or MMPI-A, respectively) was administered to 293 adults (207 male to female transsexuals (MtFs), mean age 38.04 (range 18.56-65.62) and 86 female to male transsexuals (FtMs), mean age 33.26 (range 18.95-64.30)) and 83 adolescents (43 MtFs, mean age 15.70 (range 13.16-18.70) and 40 FtMs, mean age 15.64 (range 13.05-18.56)) with a gender identity disorder (GID). Of adult MtFs, 33% were categorized as "homosexuals" and 66% as "non-homosexuals". Of adult FtMs, 77% were categorized as "homosexuals" and 33% as "non-homosexuals". Adult FtMs functioned significantly better than MtFs on three clinical scales. Contrary to what is often assumed, no differences in psychological functioning were found in the adult transsexuals with regard to sexual orientation, except on one clinical scale. Most remarkably, significantly more adults with GID scored in the clinical range on two or more clinical scales than adolescents with GID. Therefore, early medical intervention may be recommendable for adolescents with GID.

Abstract:Background Psychological testing is used in some presurgical psychological evaluations of patients seeking metabolic and bariatric surgery (MBS). The Minnesota Multiphasic Personality Inventory-3 (MMPI-3) contains new norms, updated item content, and new scales such as Eating Concerns and Impulsivity, relevant to the assessment of patients seeking MBS. Objectives The goal of this investigation was to establish convergent and discriminant properties of the MMPI-3 scales with relevant clinical interview and medical record data from Electronic Medical Records. Setting An academic medical center in the Midwest. Materials & Methods A sample of 790 consecutive patients who completed a presurgical psychological evaluation and took the MMPI-3 were included. Data from medical records and the assessment report were coded by a trained research assistant. Results MMPI-3 scale scores demonstrated good convergent and discriminant validity. For instance, the Emotional/Internalizing Dysfunction scales correlated with depression and anxiety disorder, suicide history, sexual abuse history, psychotropic medications, and eating behaviors. Behavioral/Externalizing Dysfunction scales correlated with alcohol, nicotine, and substance use and eating behaviors, such as loss of control over eating. The Eating Concerns scale on the MMPI-3 demonstrated the highest correlational patterns with various eating behaviors, such as loss of control over eating, binge eating, and stress eating. Conclusions MMPI-3 scale scores perform as well as their MMPI-2-RF counterparts. Newer scales, such as Eating Concerns and Impulsivity perform particularly well in the assessment of various eating behaviors.

Abstract:MMPI-2 scores from a parent competency sample (N = 136 parents) are compared with a previously published data set of MMPI-2 scores for child custody litigants (N = 508 parents; Bathurst et al., 1997). Independent samples t tests yielded significant and in some cases substantial differences on the standard MMPI-2 clinical scales (especially Scales 4, 8, 2, and 0), with the competency sample obtaining higher clinical scores as well as higher scores on F, FB, VRIN, TRIN, and L, but lower scores on K, relative to the custody sample. Despite the higher scores in the competency sample, MMPI-2 mean scores did not exceed the clinical cutoff (T > 65). Moreover, the present competency sample essentially replicates the MMPI-2 scores of a previously published competency sample, suggesting that the present findings are representative of that population. The present findings suggest that separate reference groups be used when conducting child custody vs. parental competency evaluations, as these appear to be distinct populations despite there being similarities in the testing circumstances.

Abstract:This study evaluated the incremental validity of the Minnesota Multiphasic Personality Inventory-3 (MMPI-3) and the NEO Personality Inventory-3 (NEO PI-3) scores in predicting California Commission on Peace Officer Standards and Training dimension (i.e., the POST-10) ratings in public safety ( n = 754) pre-employment psychological evaluations (PPEs). Although research has supported the use of the MMPI and NEO PI instruments separately, no study has evaluated the use of these abnormal- and normal-range tests in conjunction, consistent with the POST Manual requirement of use of clinical and normal-range personality measures. The MMPI-3 and NEO PI-3 were completed during PPE. Problem ratings in the POST-10 dimensions were rated using information from the assessing psychologist’s report. MMPI-3 and NEO PI-3 scores incremented one another in nearly every hierarchical ordinal logit regression model conducted to predict the ratings for five dimensions. Clinical implications and limitations, including potential criterion contamination especially for NEO PI-3 scores, are discussed.

Abstract:The purpose of this study was to investigate the predictive validity of the Minnesota Multiphasic Personality Inventory–2–Restructured Form (MMPI-2-RF) in a sample of law enforcement officers. MMPI-2-RF scores were collected from preemployment psychological evaluations of 136 male police officers, and supervisor ratings of performance and problem behavior were subsequently obtained during the initial probationary period. The sample produced meaningfully lower and less variant substantive scale scores than the general population and the MMPI-2-RF Police Candidate comparison group, which significantly affected effect sizes for the zero-order correlations. After applying a correction for range restriction, MMPI-2-RF substantive scales demonstrated moderate to strong associations with criteria, particularly in the Emotional Dysfunction and Interpersonal Functioning domains. Relative risk ratio analyses showed that cutoffs of 45 T and 50 T maintained reasonable selection ratios because of the exceptionally low scores in this sample and were associated with significantly increased risk for problematic behavior. These results provide support for the predictive validity of the MMPI-2-RF substantive scales in this setting. Implications of these findings and limitations of these results are discussed.
