Abstract:BACKGROUND Angiolipoma is a benign tumor and is generally found in subcutaneous tissues. Angiolipomas are rare in the gastrointestinal tract, including the stomach. Preoperative diagnosis of the tumor is difficult, although there are several radiological examinations such as computed tomography and endoscopic ultrasound. CASE SUMMARY We report a 24-year-old Chinese man with multiple gastric angiolipomas, with a positive stool occult blood examination. Endoscopic biopsy only showed nonspecific inflammation. Histological examination of the specimen by endoscopic snare resection showed that the tumor consisted of adipose tissues and blood vessels. We also performed a literature review. After the use of proton pump inhibitor, the fecal occult blood test was negative. Due to the difficulty of resecting multiple lesions in the stomach completely and the benign characteristics of angiolipoma, we chose to have regular upper gastrointestinal endoscopy evaluation of the lesion. No evidence of significant change in lesion size was detected after 3-years follow-up. CONCLUSION Gastric angiolipoma is rare, and benign neoplasm should be considered when lesions occur submucosally in the gastrointestinal tract.
Clinical and Pathological Features of Benign Ileocecal Ulcerative Lesions Discovered by Ileocolonoscopy: Analysis of 31 Cases

Abstract:OBJECTIVE:To investigate the clinical and pathological features of benign ileocecal ulcerative lesions proven by ileo-colonoscopy.METHODS:The clinical data of 31 patients, 17 males and 14 females, aged (47 +/- 17) (17 - 76) with distal ileal ulcers and/or ulcers of ileocecal valve, > or = 2 mm in diameter, confirmed by colonoscopy were analyzed.RESULTS:The clinical manifestations included abdominal pain, lower right abdominal pain, diarrhea, constipation, hematochezia, fever, and body weight loss. Abdominal pain was found in 20 patients (64.5%) and lower right abdominal pain in 6 (19.4%). 20 patients had the ulcers 0.2 - 1.0 cm in diameter and the ulcers of 11 patients were larger than 1.0 cm. The sites of ulcers included ileocecal valve (17 cases), anastomotic stoma after resection of ileocecal valve (3 cases), distal ileum (7 cases), and both ileocecal valve and distal ileum (4 cases). Single ulcer was seen in 17 patients and multiple in 14. The ulcers were sharply demarcated,without remarkable inflammation around or between the ulcers. Histopathological findings were mostly unspecific, such as chronic inflammation, active inflammation, ulceration, eosinophil infiltration, inflammatory granulation, granuloma, and thickness of vessel wall. Mostly, the causes of ulcer were of unknown origin (14 cases), especially for those of small size (12 cases). 10 cases (32.3%) were diagnosed as Crohn's disease. The other diagnoses were intestinal tuberculosis, ischemia, Behcet's disease, AIDS, and NSAIDs-associated ulcers.CONCLUSION:Most patients with benign ileocecal ulcers complain of abdominal pain, but lower right abdominal pain is not always present. The pathological findings are mostly not specific, but sometimes are critical for the diagnosis. The etiology was difficult to be confirmed and all the clinical data should be reviewed carefully.

Abstract:Ischemic colitis represents the most frequent form of intestinal ischemia occurring when there is an acute impairment or chronic reduction in the colonic blood supply, resulting in mucosal ulceration, inflammation, hemorrhage and ischemic necrosis of variable severity. The clinical presentation is variable and nonspecific, so it is often misdiagnosed. The most common etiology is hypoperfusion, almost always associated with generalized atherosclerotic disease. The severity ranges from localized and transient ischemia to transmural necrosis of the bowel wall, becoming a surgical emergency, with significant associated morbidity and mortality. The diagnosis is based on clinical, laboratory suspicion and radiological, endoscopic and histopathological findings. Among the radiological tests, enhanced-CT is the diagnostic investigation of choice. It allows us to make the diagnosis in an appropriate clinical setting, and to define the entity of the ischemia. MR may be adopted in the follow-up in patients with iodine allergy or renal dysfunctions, or younger patients who should avoid radiological exposure. In the majority of cases, supportive therapy is the only required treatment. In this article we review the pathophysiology and the imaging findings of ischemic colitis.

Abstract:Objective To study the differences of clinical and histopathologic features between ischemic colitis (IC) and ulcerative colitis (UC), which can make for the differential diagnosis and treatments. Methods Comparative analysis were focused on the clinical and histopathologic data of IC patients (20 cases) and UC patients (30 eases). Results The onset duration of IC [(5±7) d] was significantly shorter than that of UC [(953±1354) d]. IC patients tended to have history of cadiovascular diseases or abdominal surgery (65%). The typical clinical manifestations of IC were sudden onset of abdominal pain (85%), hematochezia (60%)and diarrhea (50%), but UC usually presented with abdominal pain (83%), diarrhea (63%), bloody mucopurulent stool (57%) and tenesmus (20%). Colonoscopy showed longitudinal ulceration (60%)on the mesenteric side, the segmental lesions usually involved only 1/4-1/2 of the perimeter of the lumen (80%), and the boundary of lesions was clear (75%) in IC patients. On the contrary, the ulcer of UC was usually map-like (43%), the lesions usually involved the entire lumen (90%), and the pseudopolyp (37%) was another colonoscopic feature of UC. The histopathologic study revealed that dilation and hyperaemia of vessels (90%) were common in IC. Severe edema of mucosa (95%) was usually presented in IC, and the thickened vascular wall (50%) was the histopathologic characteristic of IC.Crypt abscesses (47%) usually appeared in UC. Conclusions The histopathologic characteristics and differences of colonoscopic findings and clinical features provide strong bases for the differentialdiagnosis between IC and UC.

Abstract:The research problem investigated in this article emerged from a year-long ethnographic study of the interdisciplinary process of teams evaluating children for mental retardation/developmental disabilities. The researchers noticed that when a discipline was not present at team meetings, others attempted to speak for it. This article examines the team process and case consequences of such a "missing voice." Data collected through participant observation and videotaping were triangulated with transcripts, written and audiovisual records, and an interview with the missing team member. This process revealed a trail of ambiguous findings and uncertainty on the part of team members of disciplines related to that of the missing member. The "missing" member identified the knowledge, assessment skills, and theoretical perspective she could have contributed if consulted. The findings demonstrate that (a) team members bring differing knowledge and observational perspectives and (b) team deliberations appear to be a function of who is present and what is negotiated.
Colonic Mucosal Pseudolipomatosis: Diagnosis and Etiology

Abstract:Background: Colonic pseudolipomatosis is a rare condition and its pathogenesis is still not well-known. Several mechanisms were proposed to explain its pathogenesis. This report describes and discuss colonic pseudolipomatosis incidentally found in a series of 8 patients undergoing routine colonoscopy during a 5 days period. Methods: During a 5-days period (16-20 August 2014), we performed 25 colonoscopies and noticed the sudden appearance of colonic pseudolipomatosis in eight of them. Results: All underwent routine colonoscopy and found to have sudden appearance of instantaneous white blanching during colonoscopy. These lesions were either seen during insertion or withdrawal of the colonoscope. Biopsy specimen was taken from all and the biopsies showed colonic pseudolipomatosis. The indications for colonoscopy and colonoscopic findings are shown in table 1. The sudden appearance of this cluster of cases stimulated us to investigate and look for a possible etiology including the help of the central sterile services department (CSSD). We found that the automated endoscope disinfectant machine was not functioning during this week period and the scopes disinfection was done manually. The disinfectant used contained hydrogen peroxide. Mucosal colonic pseudolipomatosis is a rare colonoscopic finding with distinct endoscopic and histological findings. It is a benign and selflimiting condition. The exact etiology of colonic pseudolipomatosis is not known. We like others feel that endoscopic chemical disinfectant with hydrogen peroxide is important in the pathogenesis of colonic pseudolipomatosis rather than air-pressure related mechanical injury during colonoscopy. Awareness of this is important as colonic pseudolipomatosis may be confused with other pathological conditions. Endoscopists unfamiliar with this condition must keep this in mind as it is self-limiting and biopsies will confirm the diagnosis.

Diffuse Cavernous Hemangioma of the Colon

Abstract:A 70-year-old man was admitted to our clinic with a history of rectal bleeding and constipation, his colonoscopy revealed varicosities and bluish nodular lesions of the rectum (Figure 1). Abdominal CT showed multiple nodular lesions beginning from the distal descending colon and extending to the rectum, calcifications suggesting phleboliths were also seen in these lesions. A contrast enhanced pelvic MRI demonstrated multiple tubular lesions showing hyperintensity on T2-weighted images and hypointensity on T1-weighted images, consistent with the affected areas on the CT scan (Figure 2). It was a diffuse cavernous hemangioma, which mostly affects the rectosigmoid colon in the gastrointestinal tract, and can clinically mimic internal hemorrhoids, ulcerative colitis or cancer (1). Gastrointestinal hemangioma is a rare benign vascular neoplasm, and might be associated with a congenital disorder like Osler-Weber-Rendu disease, Maffucci's syndrome, Klippel-Trénaunay syndrome, or the congenital blue rubber bleb nevus syndrome (2). Even though there are different medical treatment options targeting VEGF and FGF-mediated pathways such as bevacizumab and thalidomide, and endoscopic approaches like sclerotherapy and electrocautery; complete resection of the hemangioma is the only curative treatment method (1, 3). Therefore, the patient was referred to department of surgery for a definitive treatment, and lost to follow-up.

Abstract:Ischemic colitis (CI) is a common form of ischemic injury, which evolves as a consequence of decreased arterial blood flow to the colon. In general, CI is attributed to an elderly with multiple comorbidities; however, it may also occur in young or middle-aged individuals. The etiology of CI is multifactorial and the clinical presentation varies upon the severity of deprivation of the intestinal blood flow, and the development of the microvasculature plexus. Multiple case reports have associations with medications, vascular disorders, pathogens and hematologic diseases. Occlusive and nonocclusive diseases are the major mechanisms, which are simultaneously the causative factor of intestinal ischemia. In this review, we discuss major factors predisposing to occurrence of CI and analyze the mechanisms of action of several classes of medications currently used. We also suggest possible therapies and discuss the latest reports, which may lead to the discovery of novel pharmacological targets for future anti-CI drugs to be used in the clinical treatment.

Abstract:Objective: To investigate the characteristics and value of ultrasound in the diagnosis of ischemic colitis. Methods: Fundamental imaging, native tissue harmonic imaging and color Doppler imaging were performed in 12 patients with acute abdominal pain and rectal bleeding. The probe frequencies were 3.5-5.0 MHz and 7.0-12.0 MHz. We analyzed the location, approximate length, colon wall thickness and color Doppler flow imaging of the involved colonic segment. Results: All the diagnosis were confirmed by colonoscopy or treament montoring and follow-up. The location of the ischemic colitis could be shown correctly in 12 of 12 patients (100%). Ischemic colitis was identified on the right side in one patient (8.4%), in the left side in seven patients (58.4%), in the transverse colon in two patients (16.6%), in the splenic flexure in two patients (16.6%). The length of involvement varied from 10.0-20.0 cm (mean length, 15.0 cm). The symmetric thickness of the colon wall ranged from 0.5 to 2.0 cm (mean, 1.3 cm). Little color Doppler flow was observed in the wall of the affected colon segment in four patients (33.3%). Flow was absent in the eight patients (66.7%). There was no thrombus demonstrated in the superior mesenteric vein or superior mesenteric artery. Conclusion: Ultrasound could correctly estimate location, approximate length, wall thickness and color Doppler flow imaging of the involved colon. Ultrasound plays an important role in diagnosis, follow-up and monitor of ischemic colon disease.

Abstract:Colonic angiomyolipomas (CA) are very rare benign tumors arising from perivascular epithelioid cells. CA are most often found either during screening colonoscopies or as an incidental finding during abdominal imaging. However, some rare cases of CA are found to present with abdominal pain and hematochezia. In this article, we report a case of a 62-year-old man who presented with intermittent hematochezia and constipation who was found to have an angiomyolipoma in the sigmoid colon. The lesion was successfully removed endoscopically with no recurrence of bleeding and no complications within the first 30 days after the procedure.

Abstract:Bauer first described colonic lipomas (CLs) in 1757. [ 1 ] The prevalence of this lesion ranges from 0.2% to 4.4%, representing 1.8% of all benign colonic lesions, [ 2 ] after hyperplastic and adenomatous polyps. Many authors have described a slight predominance of this lesion in females in their 5th or 6th decade of life, [ 3 ] primarily localized to the right colon. [ 4 ] The lesion is usually segmental, although there have been cases of diffuse lesions reported in the literature, which are typically small and asymptomatic. [ 5 ] Therefore, in most cases, they are discovered incidentally. Multiple lipomas have been shown in 10% to 20% of patients, notably if a lipoma was found in the cecum. The challenge for every physician is to differentiate this lesion from malignant colonic lesions, at the outset. Various diagnostic methods are available, including ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), and endoscopic ultrasonography (EUS). In this review, we provide the necessary elements to preoperatively suspect this lesion. Both surgical and endoscopic resections (ERs) have good outcomes with no known recurrence after complete removal. Endoscopy is an effective diagnostic and therapeutic tool for treating small lesions. Surgery is the only method available for large lesions or those involving the muscular or serosal layers. Colotomy excision or segmental colon resection is indicated when the diagnosis is definite. In contrast, segmental resection, hemicolectomy, or subtotal colectomy are recommended when the diagnosis is questionable or when a complication occurs. Indeed, only a histopathological analysis of the tumor can provide a definitive diagnosis. However, this lesion sometimes presents as an acute abdominal condition owing to the occurrence of intussusception with intestinal obstruction. In such cases, time is limited and not all means are available to direct the diagnosis towards the suspicion of a lipoma; therefore, in these cases, it is advisable to consider this tumor as malignant.

Clinicopathological Features of Phlebosclerotic Colitis.

Abstract:Phlebosclerotic colitis (PC) is a rare chronic ischemic colitis caused by venous reflux disorder. It is also called idiopathic mesenteric phlebosclerosis (IMP) due to unknown etiology. The disease is characterized by sclerosis of mesenteric vein and its branches as well as fibrosis, hyaline degeneration, calcification, thickening of colon wall. CT images show linear calcification in the colon mucosa as well as mesenteric vein and its branches. Endoscopy shows purple-blue mucosa with multiple erosion and ulceration. Microscopically, the colon mucosa shows fibrosis, hyaline degeneration and extensive thickening. The most characteristic lesion is fibrosis and calcification of the vessels especially the veins. Arteries in all layers of colon are also involved, but the injury is significant mild and less. We collected 10 confirmed patients from 2012 to 2019 in our hospital, studied their clinical histories in detail, summarized typical changes of CT images, endoscopic images and pathological sections, and made a detail follow-up. In addition to typical pathological changes, we also found that gardenia or its metabolites may be the pathogenic factor. Probablely, geniposide which is metabolized to genipin by β-glucosidase of colon flora in proximal colon, results in venous sclerosis. PC is occult onset and irreversible without special symptoms in the early stage, but it will also be stable after removing the pathogenic ingredient. Most of patients may be "cured" by appropriate conservative medication and stopping drinking. Contrary, inappropriate surgery may "trigger" the acute ischemia which results in obstruction rapidly. We hope our colleagues pay attention to the unique lesion and make early diagnosis and treatment.

Abstract:A man in his 50s was referred to hospital by his general practitioner with a suspected rectal lesion after rectal examination. He had also experienced intermittent constipation without rectal pain for five years, which he had managed with self-administered simple enemas once a week for the past three years, but these were not effective at relieving the constipation. Case series of Cryptogenic Multifocal Ulcerating Stenosing Enteritis (CMUSE)

Abstract:Background and study aims: Small bowel ulceration poses a limited, but difficult differential diagnosis. The most common causes are Crohn's disease (CD), NSAID-associated enteritis, lymphoma, cytomegaly virus infection and tuberculosis. A less known and relatively novel differential diagnosis is cryptogenic multifocal ulcerative stenosing enteritis (CMUSE). Patients and methods: ive patients referred for balloon-assisted enteroscopy for various reasons showed endoscopic features of CMUSE. These findings and, when available, medical imaging were reviewed in order to increase general knowledge on CMUSE. Results: Five patients, 3 males and 2 females, with a mean age of 39±5 years, underwent balloon-assisted enteroscopy. Typical short, circular, ulcerative stenoses were detected in the jejunum in 2 and in the ileum in 3 patients. The number of stenoses ranged from 1 to 7 per patient. Histopathology revealed nonspecific granulocyte inflammation without specific CD findings. Stenoses were often missed on pre-enteroscopy CT or MRI enteroclysis due to their short length. Treatment consisted of endoscopic balloon dilation in 3, corticosteroids in 3, azathioprine in 1 and anti-TNFα biologicals in 3 patients. 3 patients needed additional surgery because of ongoing symptomatic small bowel stenosis or retained wireless videocapsule. Conclusion: In patients with short, ulcerative small intestinal stenoses CMUSE is an important but often neglected differential diagnosis. The pathophysiology and relationship to CD are subject of ongoing debate, but specific endoscopic characteristics, different histopathological findings and lack of clear abnormalities on CT or MRI enterography suggest that CMUSE is a distinct albeit rare chronic inflammatory bowel disease.

Abstract:Ulcerative colitis (UC) is a chronic inflammatory bowel disease of unknown aetiology affecting the colon and rectum. Multiple factors, such as genetic background, environmental and luminal factors, and mucosal immune dysregulation, have been suggested to contribute to UC pathogenesis. UC has evolved into a global burden given its high incidence in developed countries and the substantial increase in incidence in developing countries. An improved understanding of the mechanisms underlying UC has led to the emergence of new treatments. Since the early 2000s, anti-tumour necrosis factor (TNF) treatment has significantly improved treatment outcomes. Advances in medical treatments have enabled a paradigm shift in treatment goals from symptomatic relief to endoscopic and histological healing to achieve better long-term outcomes and, consequently, diagnostic modalities have also been improved to monitor disease activity more tightly. Despite these improvements in patient care, a substantial proportion of patients, for example, those who are refractory to medical treatment or those who develop colitis-associated colorectal dysplasia or cancer, still require restorative proctocolectomy. The development of novel drugs and improvement of the treatment strategy by implementing personalized medicine are warranted to achieve optimal disease control. However, delineating the aetiology of UC is necessary to ultimately achieve disease cure.

Abstract:Atherosclerotic disease affecting the mesenteric vasculature can result in chronic ischemia throughout different regions of the gastrointestinal system, although it rarely impacts the stomach. We present an unusual presentation of chronic mesenteric ischemia (CMI) with extensive gastric ulceration, a large cecal ulcer, and ischemic liver lesions.
