Abstract:Left ventricular hypertrophy is a leading risk factor for cardiovascular morbidity and mortality. Although reverse ventricular remodelling was long thought to be irreversible, evidence from the past three decades indicates that this process is possible with many existing heart disease therapies. The regression of pathological hypertrophy is associated with improved cardiac function, quality of life and long-term health outcomes. However, less than 50% of patients respond favourably to most therapies, and the reversibility of remodelling is influenced by many factors, including age, sex, BMI and disease aetiology. Cardiac hypertrophy also occurs in physiological settings, including pregnancy and exercise, although in these cases, hypertrophy is associated with normal or improved ventricular function and is completely reversible postpartum or with cessation of training. Studies over the past decade have identified the molecular features of hypertrophy regression in health and disease settings, which include modulation of protein synthesis, microRNAs, metabolism and protein degradation pathways. In this Review, we summarize the evidence for hypertrophy regression in patients with current first-line pharmacological and surgical interventions. We further discuss the molecular features of reverse remodelling identified in cell and animal models, highlighting remaining knowledge gaps and the essential questions for future investigation towards the goal of designing specific therapies to promote regression of pathological hypertrophy.

Age-dependent implications of left ventricular hypertrophy regression in patients with hypertension

Abstract:Left ventricular hypertrophy (LVH) is a significant risk factor for cardiovascular mortality and morbidity in patients with hypertension. However, the effect of age on LVH regression or persistence and its differential prognostic value remain unclear. Therefore, we investigated the clinical implications of LVH regression in 1847 patients with hypertension and echocardiography data (at baseline and during antihypertensive treatment at an interval of 6–18 months) according to age. LVH was defined as a left ventricular mass index (LVMI) > 115 g/m 2 and >95 g/m 2 in men and women, respectively. LVH prevalence at baseline was not different according to age (age < 65 years: 42.6%; age ≥65 years: 45.7%; p = 0.187), but LVH regression was more frequently observed in the younger group (36.4% vs. 27.5%; p = 0.008). Spline curves and multiple linear regression analysis showed a significant relationship between reductions in systolic blood pressure and LVMI in the younger group (β = 0.425; p < 0.001), but not the elderly group (β = 0.044; p = 0.308). LVH regression was associated with a lower risk of the study outcome (composite of cardiovascular death and hospitalization for heart failure) regardless of age. In conclusion, the association between the reduction in blood pressure and LVH regression was prominent in patients with age < 65 years, but not in those with age ≥65 years. However, an association between LVH regression and lower risk of cardiovascular death and hospitalization for heart failure was observed regardless of patient age, suggesting the prognostic value of the LVH regression not only in the younger patients but also in elderly patients.

Abstract:Objective To investigate the relationship between plasma B-type natriuretic peptide(BNP), serum procollagen type Ⅲ (PC Ⅲ) levels and left ventricular geometry in essential hypertension. Methods One hundred twenty-seven essential hypertensive patients and 30 control subjects were enrolled. Hypertensive patients were classified into four groups according to echocardiographic findings which were: normal geometry(n=25), concentric remodeling(n=38), eccentric hypertrophy(n=36), or eccentric hypertrophy(n=28). Serum PC Ⅲ was measured by radioimmunoassay, plasma BNP level was determined by a point-of-care immunoassy. Results (1)The levels of plasma BNP and serum PC Ⅲ in essential hypertensive patients were higher than those in normotensive control subjects BNP:80±44 vs 53±29 pg/mL; PC Ⅲ: 99.9±47.4 vs 76.4±31.6 μg/L, respectively (P<0.05). The levels of plasma BNP and serum PC Ⅲ in eccentric hypertrophy and concentric hypertrophy groups were higher than those in other groups (BNP:eccentric hypertrophy 93±43 vs concentric hypertrophy 102±54 vs concentric remodeling 64±26 vs nomal geometry 61±27 pg/mL; PC Ⅲ:eccentric hypertrophy 116.2±49.5 vs concentric hypertrophy 113.7±58.8 vs concentric remodeling 86.1±32.5 vs nomal geometry 62.6±35.1 μg/mL ; respectively P<0.05). (2) Both plasma BNP and serum PC Ⅲ were significantly correlated left ventricular mass index (LVMI), relative left wall thickness(RWT), interventricular septal thickness(IVST), left ventricular posterior wall thickness(LVPWT); plasma BNP levels were correlated with serum PC Ⅲ levels. Conclusion Plasma BNP and serum PC Ⅲ may contribute to left ventricular remodeling, and may be served as a reliable objective markers for monitoring the developement or reversal of left ventricular hypertrophy in patients with essential hypertension.

Abstract:Abstract Background Myocardial fibrosis is observed in multiple cardiac conditions including hypertension and aortic stenosis. Excessive fibrosis is associated with adverse clinical outcomes, but longitudinal human data regarding changes in left ventricular remodelling and fibrosis over time are sparse because of the slow progression, thereby making longitudinal studies challenging. The purpose of this study was to establish and characterize a mouse model to study the development and regression of left ventricular hypertrophy and myocardial fibrosis in response to increased blood pressure and to understand how these processes reverse remodel following normalisation of blood pressure. Methods We performed a longitudinal study with serial cardiovascular magnetic resonance (CMR) imaging every 2 weeks in mice ( n = 31) subjected to angiotensin II-induced hypertension for 6 weeks and investigated reverse remodelling following normalisation of afterload beyond 6 weeks ( n = 9). Left ventricular (LV) volumes, mass, and function as well as myocardial fibrosis were measured using cine CMR and the extracellular volume fraction (ECV) s. Results Increased blood pressure (65 ± 12 vs 85 ± 9 mmHg; p < 0.001) resulted in higher indices of LV hypertrophy (0.09 [0.08, 0.10] vs 0.12 [0.11, 0.14] g; p < 0.001) and myocardial fibrosis (ECV: 0.24 ± 0.03 vs 0.30 ± 0.02; p < 0.001) whilst LV ejection fraction fell (LVEF, 59.3 [57.6, 59.9] vs 46.9 [38.5, 49.6] %; p < 0.001). We found a strong correlation between ECV and histological myocardial fibrosis (r = 0.89, p < 0.001). Following cessation of angiotensin II and normalisation of blood pressure (69 ± 5 vs baseline 65 ± 12 mmHg; p = 0.42), LV mass (0.11 [0.10, 0.12] vs 0.09 [0.08, 0.11] g), ECV (0.30 ± 0.02 vs 0.27 ± 0.02) and LVEF (51.1 [42.9, 52.8] vs 59.3 [57.6, 59.9] %) improved but remained impaired compared to baseline ( p < 0.05 for all). There was a strong inverse correlation between LVEF and %ECV during both systemic hypertension (r = − 0.88, p < 0.001) and the increases in ECV observed in the first two weeks of increased blood pressure predicted the reduction in LVEF after 6 weeks (r = − 0.77, p < 0.001). Conclusions We have established and characterized angiotensin II infusion and repeated CMR imaging as a model of LV hypertrophy and reverse remodelling in response to systemic hypertension. Changes in myocardial fibrosis and alterations in cardiac function are only partially reversible following relief of hypertension.

Abstract:Both aging and high blood pressure (BP) are associated with a risk of left ventricular concentricity and hypertrophy. We hypothesized that optimal BP management improves left ventricular remodeling beyond aging. Among 558 hypertensive patients on continuous antihypertensive treatment and without concurrent heart disease who were referred to a cardiology clinic with echocardiography and ambulatory BP monitoring data, 142 patients' echocardiographic data was available after 10 years. Baseline BP and changes in left ventricular geometry were evaluated. Mean age at baseline was 71.0 years old. Baseline daytime BP was 129.9/72.4 ± 17.1/10.2 mmHg and nighttime BP was 122.5/67.1 ± 16.9/9.1 mmHg. After 10 years, left ventricular mass index (LVMI) and relative wall thickness (RWT) significantly decreased from 104.5 ± 26.3 to 97.9 ± 26.4 g/m2, p = 0.003 and 0.51 ± 0.09 to 0.47 ± 0.09, p < 0.001, consecutively. Among patients with hypertrophic geometry at baseline, 17.2% reverted to normal geometry at follow-up. Daytime systolic BP (136.9 ± 18.5 mmHg vs 126.2 ± 16.5 mmHg, p = 0.03), nighttime systolic BP (126.2 ± 17.7 mmHg vs 116.3 ± 16.0 mmHg, p = 0.038) and daytime pulse pressure (63.5 ± 17.3 mmHg vs 53.1 ± 14.9 mmHg, p = 0.022) at baseline were higher in patients who remained hypertrophic than those without hypertrophy at follow-up. On logistic regression analysis, daytime, nighttime systolic BP, and daytime pulse pressure were significantly related to the regression of hypertrophy adjusted for age, sex, eGFR, BMI, LVMI, and RWT at baseline. For conclusion, antihypertensive treatment for 10 years improved LV geometry despite aging. Ambulatory BP and pulse pressure at baseline predicted the change of LV geometry after 10 years.

Abstract:Purpose of review Left ventricular (LV) hypertrophy (LVH) is a well recognized target organ adaptation to longstanding uncontrolled hypertension and other cardiovascular risk factors. It is also a strong and independent predictor of many cardiovascular disorders. Recent findings This focused review explores the current concepts in screening, diagnosis, prevention, and treatment of LVH in patients with hypertension. Currently, the primary screening and diagnostic tools for LVH are ECG and 2D echocardiography. Implementing machine learning in the diagnostic modalities can improve sensitivity in the detection of LVH. Lifestyle modifications, blood pressure control with antihypertensive therapy, and management of comorbidities aid in preventing and reversing LV remodeling. Summary LVH is a common and often silent complication of hypertension. Prevention and reversal of LV remodeling are crucial for cardiovascular risk reduction in patients with hypertension.

Abstract:Circulation: Cardiovascular Imaging, Volume 17, Issue 8, Page e016489, August 1, 2024. BACKGROUND:Left ventricular (LV) hypertrophy occurs in both aortic stenosis (AS) and systemic hypertension (HTN) in response to wall stress. However, differentiation of hypertrophy due to these 2 etiologies is lacking. The aim was to study the 3-dimensional geometric remodeling pattern in severe AS pre- and postsurgical aortic valve replacement and to compare with HTN and healthy controls.METHODS:Ninety-one subjects (36 severe AS, 19 HTN, and 36 healthy controls) underwent cine cardiac magnetic resonance. Cardiac magnetic resonance was repeated 8 months post-aortic valve replacement (n=18). Principal component analysis was performed on the 3-dimensional meshes reconstructed from 109 cardiac magnetic resonance scans of 91 subjects at end-diastole. Principal component analysis modes were compared across experimental groups together with conventional metrics of shape, strain, and scar.RESULTS:A unique AS signature was identified by wall thickness linked to a LV left-right axis shift and a decrease in short-axis eccentricity. HTN was uniquely linked to increased septal thickness. Combining these 3 features had good discriminative ability between AS and HTN (area under the curve, 0.792). The LV left-right axis shift was not reversible post-aortic valve replacement, did not associate with strain, age, or sex, and was predictive of postoperative LV mass regression (R2=0.339,P=0.014).CONCLUSIONS:Unique remodeling signatures might differentiate the etiology of LV hypertrophy. Preliminary findings suggest that LV axis shift is characteristic in AS, is not reversible post-aortic valve replacement, predicts mass regression, and may be interpreted to be an adaptive mechanism.

Intensive Blood Pressure Lowering Improves Left Ventricular Hypertrophy in Older Patients with Hypertension: The STEP Trial

Abstract:Hypertension, Ahead of Print. Background:Intensive systolic blood pressure (SBP) lowering has been increasingly used; however, its effect on cardiac remodeling remains not fully understood. This secondary analysis of the Strategy of Blood Pressure Intervention in the Elderly Hypertensive Patients trial aims to determine the changes in left ventricular hypertrophy (LVH) that occur in the context of intensive SBP lowering.Methods:A total of 7141 older patients with hypertension were randomly assigned to intensive treatment (SBP target, 110–130 mm Hg) or standard treatment (130 to 150 mm Hg). LVH was defined according to the Peguero-Lo Presti criteria on a standard 12-lead ECG.Results:At baseline, the prevalence of LVH (16.6% versus 16.5%) and the mean Peguero-Lo Presti value (1811 versus 1808 μV) were comparable between the treatment groups. During a median follow-up of 3.24 years, intensive SBP lowering was associated with a significantly lower risk of new LVH occurrence (hazard ratio, 0.76 [95% CI, 0.66–0.89];P=0.001) and slower progression of the mean Peguero-Lo Presti index value by −23.47 μV/y (95% CI, −34.93 to −12.01;P=0.000). However, the rates of regression of baseline LVH did not differ significantly. Of note, the beneficial effect of intensive SBP lowering in terms of cardiovascular events (hazard ratio, 0.75 [95% CI, 0.59–0.97]) was not markedly attenuated after adjusting for LVH as a time-varying covariate (hazard ratio, 0.76 [95% CI, 0.59–0.97]).Conclusions:Intensive SBP lowering protects against LVH development in older hypertensive patients, however, this favorable effect could not explain most of the reduction in cardiovascular events associated with intensive SBP lowering.

Left Ventricular Pattern and Cardiac Function in Hypertension

Abstract:Aim To explore the relationship between the remodelling pattern of left ventricle and cardiac function in patients of hyertension. Methods 2 DE and M mode echocardiography were used to study the left ventricular patterns in 209 essential hypertensive patients and correlated it with the left ventricular function. Results The percentage of functional class in patients with normal geometry and concentric remodeling were 75.4% and 60.0% respectively, whereas in patients with concentric and eccentric hypertrophy class Ⅱ function were only 12.7% and 11.8%, respectively. The size of left atrium was larger in concentric hypertrophy (indicating the decreased left ventricular compliance). The left ventricular systolic function was injured apparently in eccentric hypertrophy. Conclusion The different methods of clinical treatment should be taken according to the different pattern of left ventricle.

Abstract:Background: High blood pressure is a major risk factor for cardiac remodeling and left ventricular hypertrophy, increasing cardiovascular risk and leading to heart failure with preserved ejection fraction (HFpEF). Since renal sympathetic denervation (RDN) reduces blood pressure in the long term, we aimed to investigate the long-term effect of RDN in patients with HFpEF in the present analysis. Methods: Patients previously enrolled in a local RDN registry who underwent high-frequency RDN with the use of the Symplicity Flex® renal denervation system between 2011 and 2014 were followed up. The patients were assessed by 24-h ambulatory blood pressure measurement, transthoracic echocardiography, and laboratory tests. We used the echocardiographic and biomarker criteria of the Heart Failure Association (HFA)-PEFF (Pre-test assessment, Echocardiography and Natriuretic Peptide Score, Funkctional testing, and Final aetiology) score to identify patients with HFpEF. Results: Echocardiographic assessment was available for 70 patients at a 9-year long-term follow-up. Of these patients, 21 had HFpEF according to the HFA-PEFF score. We found a significant reduction of the HFA-PEFF score from 5.48 ± 0.51 points at baseline to 4.33 ± 1.53 points at the 9-year follow-up ( P < 0.01). This decrease was due to a greater reduction in morphological and biomarker subcategories [from 1.95 ± 0.22 to 1.43 ± 0.51 points ( P < 0.01) and from 1.52 ± 0.52 to 0.90 ± 0.63 points ( P < 0.01), respectively] than in the functional one. Morphologically, there was a reduction in left ventricular hypertrophy and left atrial dilation. Conclusions: The present analysis suggests that RDN may lead to a regression of the extent of HFpEF beyond a reduction in blood pressure and thus possibly contribute to an improvement in prognosis. More detailed information will be provided by ongoing randomized sham-controlled trials.

Abstract:The effects of blockade of the renin-angiotensin system at three different sites on regression of left ventricular hypertrophy (LVH) were investigated in spontaneously hypertensive rats (SHRs). Angiotensin-converting-enzyme (ACE) inhibitor, angiotensin II type 1 (AT1) receptor antagonist, renin inhibitor, or the combination of hydralazine and hydrochlorothiazide was administered for 7 days to SHRs to obtain the same decrease in blood pressure. Treatment with ACE inhibitor, AT1 receptor antagonist, or renin inhibitor significantly decreased left ventricular mass. On the other hand, hydralazine administration failed to diminish LVH in SHRs. These findings indicate that blockade of the renin-angiotensin system regressed LVH irrespective of the site of the blockade and independent of hemodynamic changes.

Abstract:Objective: Left ventricular hypertrophy in end-stage renal disease patients undergoing renal replacement is linked to an increased risk for cardiovascular diseases. Dialysis does not completely prevent or correct this abnormality, and the evidence for kidney transplantation varies. This analysis aims to explore the relationship between kidney transplantation and left ventricular hypertrophy. Design and method: MEDLINE and Scopus were systematically searched in October 2023. All cross-sectional and longitudinal studies that fulfilled our inclusion criteria were included. Outcome was left ventricular mass index. We conducted a meta-analysis using a frequentist random effects model. Meta-regression was applied to examine the left ventricular mass index changes dependent on various covariates. Sensitivity analysis was employed to handle outlying or influential studies and address publication bias. All analyses were conducted using R version 4.3.2. Results: From 7416 records, 46 studies met the inclusion criteria with 4122 included participants in total. Longitudinal studies comparing LVMI in renal transplant recipients before transplantation and thereafter demonstrated an improvement of left ventricular mass index after kidney transplantation - 0.44 g/m2 [-0.60; -0.28] (SMD [95% confidence interval]). By meta-regression analysis blood pressure was identified as predictor of LVMI change. A younger age at the time of kidney transplantation and well-controlled anemia were also associated with regression of left ventricular hypertrophy. In studies longitudinally comparing kidney transplant recipients with patients staying on dialysis no difference was detected -0.09 g/m2 [-0.33; 0.16] (SMD [95% confidence interval]). Meta-regression using changes of systolic blood pressure as covariate showed an association between higher blood pressure and an increase in left ventricular mass, regardless of the modality of renal replacement treatment. Conclusions: In conclusion, our results indicated a potential cardiovascular benefit, defined as the regression of left ventricular hypertrophy, after kidney transplantation. This benefit was primarily attributed to improved blood pressure control rather than the transplantation itself.

Abstract:AbstractAimsHeart failure with preserved ejection fraction (HFpEF) develops in response to hypertensive left ventricular (LV) hypertrophy and is associated with increased cardiovascular events. Although the progression to systolic heart failure is a known consequence of LV hypertrophy and HFpEF, few data are available on the LV geometry change and frequency of deterioration to systolic dysfunction in this population.Methods and resultsWe evaluated the baseline and follow‐up characteristics in 680 patients with LV hypertrophy and HFpEF in this prospective cohort study. The primary endpoint was 5 year all‐cause mortality. The changes of LV geometry and heart failure transition were analysed. Systolic dysfunction [left ventricular ejection fraction (LVEF) < 50%] occurred in 182 patients (26.8%) during a 5 year follow‐up. Patients with LVEF deterioration were associated with a lower survival rate. Beta‐blocker prescription was a protective factor for preserved LVEF. And concentric LV geometry shifted to eccentric hypertrophy was uncommon (10.6%) during a 5 year follow‐up.ConclusionsA quarter of patients with hypertensive LV hypertrophy and HFpEF progresses to systolic dysfunction during a 5 year follow‐up, which was accompanied by poor clinical outcomes. And beta‐blocker therapy might play a protective role for preserved LVEF in this population.
Association of Endothelial and Mild Renal Dysfunction with the Severity of Left Ventricular Hypertrophy in Hypertensive Patients

Abstract:BACKGROUNDThe association between impaired renal function and increase left ventricular mass was shown to be related to increase in arterial stiffness, which indicates that vascular homeostasis and remodeling may impact the left ventricular hypertrophy (LVH) in patients with renal dysfunction.METHODSWe measured the peripheral arterial reactive hyperemia index (RHI) and estimated glomerular filtration rate (eGFR) in 317 hypertensive patients comprising 115 normal RHI (RHI > 1.67) and normal eGFR (eGFR >= 90ml/min per 1.73 m(2)), 136 low RHI (RHI <= 1.67), 27 low eGFR (60 <= eGFR < 90ml/min per 1.73 m(2)) and 39 low RHI combined with low eGFR.RESULTSMultivariate logistic regression analysis identified lg RHI (odds ratio (OR): 0.001, 95% confidence interval (CI): 10(-6) to 0.426, P = 0.024) and lg eGFR (OR: 0.009, 95% CI: 10(-4) to 0.414, P = 0.016) as independent factors correlated with LVH respectively in hypertensive patients. Compared with normal RHI and eGFR patients, the extent of LVH in patients with either low RHI (OR: 1.224 95% CI: 0.451 to 3.327, P = 0.691) or low eGFR (OR: 0.593 95% CI: 0.070 to 5.037, P = 0.632) did not significantly increase, while it increased significantly in patients with low RHI combined with low eGFR (OR: 4.629 95% CI: 1.592 to 13.458, P = 0.005).CONCLUSIONSThe concurrence of endothelial and mild renal dysfunction was significantly associated with the severity of LVH in hypertensive patients.

Abstract:Aim:To investigate the left ventricular hypertrophy and myocardial fibrosis in spontaneously hypertensive rats(SHRs),and the protective role of enalapril and losartan. Methods:Enalapril(20 mg·kg 1 ·d 1 ) or losartan(25 mg·kg 1 ·d 1 ) or their combination(enalapril 10 mg·kg 1 ·d 1 ,losartan 12.5 mg·kg 1 ·d 1 ) was administered from 10 to 16 weeks of age of SHRs( n= 30).Male,age matched untreated SHR and Wistar Kyoto(WKY) rats were served as controls.Systolic blood pressure(SBP), left ventricular mass and left ventricular mass index(LVMI) were determined.Left ventricular collagen contents were measured by spectrophotometric method.Cardiomyocytes size,the parameter and content of collagen on pathological tissue slices were assessed with computerized video processing. Results:Compared with WKY rats,the SBP,LVMI,left ventricular collagen contents,cardiomyocytes size,collagen volume fraction(CVF) and perivascular circumferential collagen area(PVCA) were significantly increased in SHRs( P 0.001),and substantially reduced in treated SHRs than that in untreated SHRs( P 0.01).There were no difference in these parameters between enalapril group and losartan group.Except the SBP, the combination of enalapril with losartan reduced all markers more pronouncedly than enalapril or losartan alone. Conclusion:Enalapril and losartan decrease blood pressure,reverse early left ventricular hypertrophy and myocardial fibrosis in SHR. Little difference of effects has been found between them. The combination of enalapril with losartan is more effective than monotherapy except effect on the SBP.

Abstract:Objective To investigate the effect of losartan on left ventricular hypertrophy and left ventricular diastolic function in hypertension.Methods Thirty-two essential hypertension patients(WHO classⅡ)were treated with losartan for eight weeks.Plasma renninactivity,angiotensin Ⅱ(AngⅡ),aldosterone and procolagen Ⅲ were determined.The left ventricular mass and ventricular function were monitored by color Doppler cardiography.Results Levels of aldosterone were decreased significantly.The ratio of peak velocity was increased at rapid filling(E) and atrial contraction(A).The thickness of interventricular septum(IVS)and posterior wall,left ventricular mass index did not alter.Conclusion Blood pressure,procolagen Ⅲ and the diastolic function are improved after the treatment by losartan.
