Abstract:The crisis of medical response caused by catastrophic events might significantly affect emergency response, and might even initiate more serious social crisis. Therefore, early identification and timely blocking the formation of crisis in the early phase after a major disaster will improve the efficiency of medical response in a major disaster and avoid serious consequences. In the present paper, we described the emergency strategy to crisis management of medical response after a major disaster. Major catastrophic events often lead to various crises, including excess demand, the crisis of response in barrier and the structural crisis in response. The corresponding emergency response strategies include: (i) shunt of catastrophic medical surge; (ii) scalability of medical surge capacity; (iii) matching of the structural elements of response; (iv) maintaining the functions of support system for medical response and maximising the operation of the integrated response system; and (v) selection of appropriate care 'standard' in extreme situations of overload of disaster medical surge. In conclusion, under the impact of a major catastrophic event, medical response is often complex and the medical surge beyond the conventional response capacity and it is easy to be in crisis. In addition to the current consensus of disaster response, three additional aspects should be considered. First, all relevant society forces led by the government and military should be linkages. Second, a powerful medical response system must be based on a strong support system. Third, countermeasures of medical surge should be applied flexibly to the special and specific disaster environment, to promote the effective medical response force.

Abstract:Currently, some 50% of the US population has a chronic disease, creating an epidemic, and 86% of health care costs are attributable to chronic disease. The medical profession and its leadership did not recognize or respond appropriately to the rising prevalence of chronic disease. As a consequence, a health care crisis emerged, with inadequate access to care and quality of care together with excessive costs. In the years since the 1950s, when the chronic disease prevalence grew, the clinical literature did not follow. It remained preoccupied with acute disease. Similarly, medical education did not change. Studies and critiques gave little or modest attention to the rising dominance of chronic disease and neglected elements of good care. Recently, some health services responding to their growing number of patients with chronic illness have designed and tested new ways of providing care. They have found that, as a result, the patient's health outcomes were improved, costs of care were lower, and patient satisfaction was higher. These results and experiences provide examples of what can be done. The health care crisis and the emergence of a chronic disease epidemic coincided to a substantial degree. Although the epidemic did not cause the crisis, it contributed significantly. Now, the medical profession and its leadership are confronted by the responsibility to build a practice of medicine and a health care system that better meet the needs of patients with chronic illness and reduces the health care crisis.
Eradicating and eliminating infectious diseases: past, present and future

Abstract:During the past 60 years, a number of infectious diseases have been targeted for eradication or elimination, with mixed results. While smallpox is the only one successfully eradicated so far, campaigns on yaws and malaria brought about a dramatic reduction in the incidence in the beginning of the campaign but ultimately could not achieve the desired goal. There is again a renewed interest in disease eradication. The World Health assembly in May 2010 passed a resolution calling for eradication of measles by 2015; the target of polio eradication still remains elusive. In view of these developments, it is appropriate time to revisit the concept of disease eradication and elimination, the achievements and failures of past eradication programmes and reasons thereof, and possibly apply these lessons while planning for the future activities. This paper based on the Dr. A.L.Saha Memorial Oration describes various infectious diseases that have been targeted for eradication or elimination since 1950s, the potential direct and indirect benefits from disease eradication, and the issues and opportunities for the future.
Next-generation healthcare: a strategic appraisal

Abstract:Successful next-generation healthcare must deliver timely access and quality for an aging population, while simultaneously promoting disease prevention and managing costs. The key factors for sustained success are a culture with aligned goals and values; coordinated team care that especially engages with physicians and patients; practical information that is collected and communicated reliably; and education in the theory and methods of collaboration, measurement and leadership. Currently, optimal population health is challenged by a high prevalence of chronic disease, with large gaps between best and usual care, a scarcity of health human resources - particularly with the skills, attitudes and training for coordinated team care - and the absence of flexible, reliable clinical measurement systems. However, to make things better, institutional models and supporting technologies are available. In the short term, a first step is to enhance the awareness of the practical opportunities to improve, including the expansion of proven community-based disease management programs that communicate knowledge, competencies and clinical measurements among professional and patient partners, leading to reduced care gaps and improved clinical and economic outcomes. Longer-term success requires two additional steps. One is formal inter-professional training to provide, on an ongoing basis, the polyvalent human resource skills and foster the culture of working with others to improve the care of whole populations. The other is the adoption of reliable information systems, including electronic health records, to allow useful and timely measurement and effective communication of clinical information in real-world settings. A better health future can commence immediately, within existing resources, and be sustained with feasible innovations in provider and patient education and information systems. The future is now.
The use of technology to transform the home into a safe-haven

Abstract:On June 14, 2006 three reports were published by the Institute of Medicine (IOM) in regards to "THE FUTURE OF EMERGENCY CARE IN THE UNITED STATES HEALTH SYSTEM". The three combined reports: Hospital-Based Emergency Care at the Breaking Point, Emergency Medical Services at the Crossroads and Emergency Care for Children Growing Pains, are a clear reflection of the state we currently face, even without a major disaster. Some key findings drawn from all three reports showed that the emergency care system is ill-prepared to handle a major one. For example, many of the 41 million citizens who do not have medical insurance end up using the Emergency Departments (ED) as their source of "regular" care and many of these EDs are at or over capacity, there is little surge capacity for a major event, whether it takes the form of a natural disaster, disease outbreak, or terrorist attack. If we had during the major disaster event, a "contagion" element, i.e. pandemic flu, then the problem would be even more complicated, since the "regular" hospital patient population would need to be isolated from these patients. If we add to this equation the length of time involved in the "current" process of vaccine creation and production (i.e. the volume of vaccines that would be required to be provided to the citizens of the world), the scenario does not look to promising. A new model is needed then to address these requirements. In the developed world we have a number of devices (e.g., radio, TV, Computers, telephones, mobile devices, etc.) and infrastructure (e.g., cable, wireless networks, etc.) that are already supplying the homes and the individuals with a large number of independent applications and different types of information. These stovepipes or independently developed family that include: tele-banking, Telehealth, tele-education, e-commerce, entertainment on demand, etc. when "connected" as an integrated set, may provide an ideal environment, where families may stay at home for a long period of time (quarantine) and would have all the mechanisms in place for getting food and water from supermarkets, drugs from the pharmacy, the children would be able to go to school from home (in turn their school grounds may become temporary hospitals), adults could telecommute to work and minor conditions could be consulted and treated through these systems (with the help of a Telehealth platform that would include electronic health records), etc.

Abstract:Chronic critical illness (CCI) is an inevitable result of overpopulation and aging, as well as the development of medicine. The number of CCI patients will constantly increase and become an unaffordable economic burden for families, societies and countries. CCI could be prevented by multiple measures. Firstly, doctors must know about the pathophysiology and etiology of the disease. When providing organ function support for CCI patient, we have to know and treat the cause of the disease as early as possible. Secondly, we need to precisely monitor the insults caused by the disease and/or improper host response to the disease, evaluate the organ reserve function, and predict the outcomes and life quality after discharging from hospital. In addition, it is necessary to strengthen the humanity training of health care workers, publicize the correct thanatopsis in the whole society that every life is "born to die", and define the core role of medicine as "to comfort always".
Reflecting on the safety zoo: Developing an integrated pandemics barrier model using early lessons from the Covid-19 pandemic

Abstract:Our current predicament, the Covid-19 pandemic is first of all a health crisis. However, social disruption and economic damage are becoming visible some 7 months after the Wuhan City outbreak early December 2019. The authors wondered what could have been done better in prevention and repression of the Covid-19 pandemic from a safety management and risk control point of view. Within a case study framework, the authors gathered literature on pandemics, about country response effectiveness, and about human behaviour in the face of danger. The results consist of a safety management oriented narrative about the current pandemic, several critical observations about the current paradigms and shortcomings of preparation, and a number of opportunities for improvements of countermeasures. Many of the proverbial animals in the safety zoo, representing typical behaviours, were observed in action. Based on well proven risk analysis methods - risk management, event tree, scenarios, bowtie - the authors then analyse the generic sequence of events in a pandemic, starting from root causes, through prevention, via the outbreak of a pathogen, through mitigation to long term effects. Based on this analysis the authors propose an integrated pandemics barrier model. In this model the core is a generic pandemic scenario that is distinguishing five risk controllable sequential steps before an outbreak. The authors contend that the prevention of pandemics via safety management based biohazard risk control is both possible and of paramount importance since it can stop pandemic scenarios altogether even before an outbreak.

Abstract:Resource support between individuals is of particular importance in controlling or mitigating epidemic spreading, especially during pandemics. However, there remains the question of how we can protect ourselves from being infected while helping others by donating resources in fighting against the epidemic. To answer the question, we propose a novel resource allocation model by considering the awareness of self-protection of individuals. In the model, a tuning parameter is introduced to quantify the reaction strength of individuals when they are aware of the disease. And then, a coupled model of resource allocation and disease spreading is proposed to study the impact of self-awareness on resource allocation and its impact on the dynamics of epidemic spreading. Through theoretical analysis and extensive Monte Carlo simulations, we find that in the stationary state, the system converges to two states: the whole healthy or the completely infected, which indicates an abrupt increase in the prevalence when there is a shortage of resources. More importantly, we find that too cautious and too selfless for the people during the outbreak of an epidemic are both not suitable for disease control. Through extensive simulations, we locate the optimal point, at which there is a maximum value of the epidemic threshold, and an outbreak can be delayed to the greatest extent. At last, we study further the effects of the network structure on the coupled dynamics. We find that the degree heterogeneity promotes the outbreak of disease, and the network structure does not alter the optimal phenomenon in behavior response. Based on the results of this study, a constructive suggestion is that in the face of a global pandemic, individuals or countries should strengthen mutual support and cooperation while doing their own prevention to suppress the epidemic optimally.

Abstract:Multiple, accelerating and interacting ecological crises are increasingly understood as constituting a major threat to human health and well-being. Unconstrained economic growth is strongly implicated in these growing crises, and it has been argued that this growth has now become "uneconomic growth", which is a situation where the size of the economy is still expanding, but this expansion is causing more harm than benefit. This article summarises the multiple pathways by which uneconomic growth can be expected to harm human health. It describes how health care systems—especially through overuse, low value and poor quality care—can themselves drive uneconomic growth. Health economists need to understand not only the consequences of environmental impacts on health care, but also the significance of uneconomic growth, and pay closer attention to the growing body of work by heterodox economists, especially in the fields of ecological and feminist economics. This will involve paying closer heed to the existence and consequences of diminishing marginal returns to health care consumption at high levels; the central importance of inequalities and injustice in health; and the need to remedy health economists' currently limited ability to deal effectively with low value care, overdiagnosis and overtreatment.

[Effect of major epidemics on cultural awareness]

Abstract:Mankind has been stricken with "major" epidemic diseases throughout its history. The most serious among them immediately threaten man's life e.g. plague, cholera, smallpox, typhus, and dysentery, besides, there are others which take a slower course e.g. lues, leprosy, leishmaniasis, tuberculosis, and malaria. Yet, the "lesser" epidemic diseases like diphtheria, scarlet fever, mumps, pneumococcosis, influenza, and most recently AIDS may also turn into "major" ones. Originally, man exclusively depended on his genetic makeup for protection, and being particularly prone to attacks of disease he was subject to natural selection. Thus, only one human species survived, the homo sapiens. Interbreeding achieved biologic adaptation and created a balanced genetic polymorphism. Advancing in his degree of civilization, man formed groups, developed clothing, fire, houses, and tools, and his increasing cultural awareness allowed him to migrate from the tropical climates to more temperate, and less disease-infested zones. Immigration and wars, and the accompanying infections jeopardized and diminished entire populations and eradicated highly developed cultures like that of the American Indians. The plague, coming from Asia, and lues, from America, as well as cholera, influenza, and smallpox spread around the whole globe. Fear and terror led to irrational conclusions and triggered persecutions. The attitude of accepting disease as a God-sent fate (Hiob), or a God-sent punishment suppressed reasonable measures against disease. The necessary official measures have increasingly restricted liberty, and this patronizing treatment needs to be opposed with a higher sense of responsibility. Medical art has developed from more healing towards prophylactic and predictive medicine, which prognosticates the individual susceptibility to particular infections, and other risk factors.

Abstract:To prepare for the 2030 "baby-boomer challenge", some governments have begun to implement healthcare reforms over the past two decades. These reforms have led healthcare information systems (HIS) to evolve into a major research area in our discipline. This research area has an increasing individual, organizational, and economic impact. Due to the 2030 "baby-boomer challenge", the number of elderly individuals continues to increase, and they may have chronic illnesses, such as eye problems and Alzheimer's disease. Given the practical need for HIS that support chronic care, we decided to conduct a literature synthesis and identify opportunities for HIS research. Specifically, we present the chronic care model and analyze how IS researchers have discussed HIS to address the needs of patients with chronic illness. Further, we identify research gaps and discuss the research topics on HIS that future work can extend and customize to support these patients. Our results stimulate and guide future research in the HIS area. This paper has the potential to strengthen the body of knowledge on HIS.
DANGER! Crisis Health Workers at Risk

Abstract:The occupational hazards of health workers (HWs) in standard work environments have been well defined in both the developed and developing world during routine working conditions. Less defined are the hazards to HWs during pandemics, epidemics, natural disasters, wars, conflicts, and other crises. How do crises affect the infrastructure of medical systems? What are the distinct needs of the patient population during crises? What are the peculiarities of the Crisis Health Worker (CHW)? What are the known CHWs' occupational risks? What are the protective factors? By means of a PubMed search, we synthesized the most relevant publications to try to answer these questions. Failures of healthcare infrastructure and institutions include CHW shortages, insufficient medical supplies, medications, transportation, poorly paid health workers, security concerns, and the absence of firm guidance in health policy. Healthcare needs affecting the patient population and CHWs include crisis-induced injury and illness, hazardous exposures, communicable diseases, mental healthcare, and continuity of care for pre-crisis medical conditions. CHWs' occupational hazards include supply deficiencies, infectious disease transmission, long working hours, staff shortages, financial reimbursements, mental fatigue, physical exhaustion, and inconsistent access to clean water, electricity, and Internet. CHWs suffer from injuries and illnesses that range from immediate, debilitating injuries to chronic, unforeseen effects like mental fatigue, physical exhaustion, anxiety, burnout, and even post-traumatic stress syndrome (PTSD). Protective factors include personal traits such as adaptability and resilience as well as skills learned through structured education and training. Success will be achieved by constructively collaborating with local authorities, local health workers, national military, foreign military, and aid organizations.

Abstract:The global phenomenon of an aging population, compounded by the pervasive pandemic of abdominal obesity and metabolic diseases stemming from unhealthy lifestyles, presents formidable challenges for governments and institutions worldwide. These trends not only escalate healthcare costs but also strain the sustainability of existing public healthcare systems. However, simply increasing spending is not a viable solution. Despite healthcare expenditure skyrocketing from 4.1 trillion in 2023, the USA has witnessed a unique and disturbing decline in both lifespan and healthspan. 1 In 2019, the life expectancy gap between the USA and other leading industrialized nations was 5.43 years for men and 5.96 years for women. And even within the USA, marked disparities in mortality rates exist, with Asians and Hispanics showing cardiometabolic disease death rates for men 40%–60% lower and for women 30%–40% lower than African Americans. 2 Moreover, nearly 90% of healthcare costs in the USA, as in other developed nations, are directed towards treating chronic diseases retroactively, after they have already manifested. 1 This underscores the urgent need to transition towards preventative healthcare and educational models, rather than predominantly addressing chronic conditions reactively within hospital settings.

Abstract:Background: The developed world responds to new and re-emerging diseases through the discovery of medications. Disease can be transmitted around the world in a day, but the development of medications does not occur at this rate. The world has one environment and the focus in health care must be on identifying factors in this environment that coalesce to produce disease. Aim: The aim of this paper is to introduce the integrative model of environmental health and explore its potential to illuminate the Toronto SARS experience. Discussion: SARS affected people on three continents in a matter of days. Response to this new disease varied from one area to another and was dependent upon the level of integration of health services and communication across services. The present focus of the health care system is on treating the results of disease rather than the causative factors. Reacting to a new disease had grave social and economic consequences. The time for a new global environmental approach to health is now. The Toronto SARS experience was examined using the integrative model of environmental health and the upstream perspective as exemplars to interrupt the traditional approach to disease. All health care providers share the responsibility to learn about and to understand how our environment creates disease. This knowledge comes through research on topics such as; chemicals, pesticides, soil erosion, killing of forests, contamination of water, destabilization of climate, and social disruption from wars. Conclusions: Health care systems in the developed world continue to focus on the treatment of disease. A global ecological initiative for an integrated disease prevention system must be negotiated among nations.
Coping with the Challenges of COVID-19 Using the Sociotype Framework: A Rehearsal for the Next Pandemic

Abstract:The world, as a global village, is currently taking part in a real-time public health, medical, socio-cultural, and economic experiment on how best to combat the COVID-19 pandemic. Extraordinary times demand extraordinary measures. Depending on the time from the outbreak, strategies have ranged from minimal intervention to mitigation by quarantine for high-risk groups (elderly with chronic illnesses) to containment and lockdown. Adherence to such restrictions have depended on the individual and national psyche and culture. One can understand and forgive governments for being over-cautious, but not for being ill-prepared. It seems that Singapore after SARS (2003) and South Korea after MERS (2015) learnt from their experiences and have fared relatively well with minimal disruption to daily routines. Coping with the challenge of COVID-19 is an urgent global task. We use the Sociotype ecological framework to analyze different coping responses at three levels: Context (government and leadership, social context, health services, and media); Relationships; and the Individual. We describe the many negative outcomes (e.g. mortality [obviously], unemployment, economic damage, food insecurity, threat to democracy, claustrophobia) and the positive ones (e.g. new, remote teaching, working, and medical routines; social bonding and solidarity; redefining existential values and priorities) of this surreal situation, which is still evolving. We highlight the importance of humor in stress reduction. Regular and reliable communication to the public has to be improved, acknowledging incomplete data, and learning to deal with fake news, misinformation, and conspiracy theories. Excess mortality is the preferred statistic to follow and compare outcomes. When the health risks are over, the economic recovery responses will vary according to the financial state of countries. If world order is to be reshaped, then a massive economic aid plan should be launched by the rich countries-akin to the Marshall plan after the Second World War. It should be led preferably by the USA and China. The results of the tradeoffs between health and economic lockdowns will only become apparent in the months to come. The experiences and lessons learned from this emergency should be used as a rehearsal for the next epi-/pandemic, which will surely take place in the foreseeable future.
The Lebanese Healthcare Crisis: An Infinite Calamity

Abstract:In the past, a number of events rocked Lebanon, a small region of the previously prestigious Phoenician civilization. Whether it was mandates, wars, or economic compromises, the country always seemed to rise up again to a prominent stature in the Middle East. Once known as Switzerland of the East, Lebanon was torn apart by the works of sectarian battles during the civil war from 1975 to 1990. Since then, the country has never been the same with the turmoil left and right. Despite all of that, the healthcare sector has been one of the most prominent in the Middle East and the entire Arab world with accomplished physicians returning from immigration to serve their country. Lebanon excelled in holding first-time international conferences, performing medical interventions, and offering one of the best healthcare education and training to its juniors. The most recent setbacks since late 2019 have, however, held Lebanon back and subsequently handcuffed the healthcare system, leading to the impactful demise of the once glorious care. Nevertheless, the healthcare system remains one of the top-tier domains fighting against the coronavirus disease 2019 (COVID-19) pandemic and the failings of the rocked state.

Abstract:Over the past year, historians of medicine have found our discipline invested with a new sense of relevance. In trying to make sense of epidemics past and present, many of us have been substantially influenced by Charles Rosenberg's 1989 Daedalus essay, "What Is an Epidemic? AIDS in Historical Perspective." Writing in the middle of another unfolding global pandemic, Rosenberg suggested that all epidemics possessed similar forms of social choreography, and that applying a narrative framework could help to understand their sequence, structure, and social impact. This issue of the Bulletin offers contributions from thirteen scholars working in various geographic, chronological, and thematic areas that engage with Rosenberg's fundamental historical question about what defines an epidemic, although the question takes on different forms, and different forms of urgency, in each of their works.

Abstract:The COVID-19 pandemic has caught many nations by surprise and has already caused millions of infections and hundreds of thousands of deaths worldwide. It has also exposed a deep crisis in modeling and exposed a lack of systems thinking by focusing mainly on only the short term and thinking of this event as only a health crisis. In this paper, authors from several of the key countries involved in COVID-19 propose a holistic systems model that views the problem from a perspective of human society including the natural environment, human population, health system, and economic system. We model the crisis theoretically as a feedback control problem with delay, and partial controllability and observability. Using a quantitative model of the human population allows us to test different assumptions such as detection threshold, delay to take action, fraction of the population infected, effectiveness and length of confinement strategies, and impact of earlier lifting of social distancing restrictions. Each conceptual scenario is subject to 1000+ Monte-Carlo simulations and yields both expected and surprising results. For example, we demonstrate through computational experiments that maintaining strict confinement policies for longer than 60 days may indeed be able to suppress lethality below 1% and yield the best health outcomes, but cause economic damages due to lost work that could turn out to be counterproductive in the long term. We conclude by proposing a hierarchical Computerized, Command, Control, and Communications (C4) information system and enterprise architecture for COVID-19 with real-time measurements and control actions taken at leach level.

Abstract:Abstract Telecommunications technologies and the Internet offer a revolution in management of global healthcare systems and sustainable development. This paper explores emergent telecommunications infrastructures and their consequences for the future of healthcare management using a scenario methodology. A world systems view from evolutionary economics provides a unique perspective on sustainable development. System performance on the dimensions of universal access, equitable resource allocation and consumer participation are evaluated in light of ideological, political and cultural considerations of governance. Alternative scenarios vary according to market dynamics led by healthcare institutions (push) or by consumer demand (pull), and according to system control mechanisms that may be technologically embedded, institutional or behavioural. The scenario analysis suggests that telecommunications technologies and the Internet may contribute significantly to improve global healthcare system performance, to manage ideological diversity and to reduce the wide inequities that separate the industrialized nations from the developing world.

Abstract:The term sustainability has in recent years rightfully received increasing attention in relation to environmental and planetary sustainability. The field of medicine has not been left untouched by this trend, although the great threat of global warming calls for a more significant change in mindset if meaningful progress is to be made [ Citation 1 ]. Planetary health is, however, only one aspect of sustainability of the healthcare system. Increased longevity and changing demographics with a growing population of the elderly and a rising prevalence of frailty and multimorbidity alongside an expanding arsenal of expensive treatment options cannot forever be met by the proceeding trend of enlarging healthcare budgets. Continuing expansion of the healthcare system with rising expenditure is not a sustainable development and there is an obvious necessity of prioritising our resources better. Furthermore, there is a worldwide shortage of healthcare personnel which puts further restrictions to the capabilities of healthcare systems to meet these increasing demands.
