Abstract:Summary A footnote in Adolphe Quetelet's classic 1835 Treatise on Man described his algebraic analysis of how body weight (W ) varies with height (H ) in adult males and females. Using data on 12 short and 12 tall subjects of each sex, Quetelet established the rule that W is approximately proportional (∝ ) to H2 in adults; that is, W∝H2 when W≈αH2 for some constant α . Quetelet's Rule (W∝H2 ), transformed and renamed in the twentieth century to body mass index (BMI=W/H2 ), is now a globally applied phenotypic descriptor of adiposity at the individual and population level. The journey from footnote to ubiquitous adiposity measure traveled through hundreds of scientific reports and many more lay publications. The recent introduction of highly effective pharmacologic weight loss treatments has heightened scrutiny of BMI's origins and appropriateness as a gateway marker for diagnosing and monitoring people with obesity. This contemporary context prompted the current report that delves into the biological and mathematical paradigms that underlie the simple index BMI=W/H2 . Students and practitioners can improve or gain new insights into their understanding of BMI's historical origins and quantitative underpinning from the provided overview, facilitating informed use of BMI and related indices in research and clinical settings.

Abstract:Since laws of physics exist in nature, their possible relationship to terrestial growth is introduced. By considering the human body as a dynamic system of variable mass (and volume), growing under a gravity field, it is shown how natural laws may influence the vertical growth of humans. This approach makes sense because the non-linear percentile curves of different aspects of human physical growth from childhood to adolescence can be described in relation to physics laws independently of gender and nationality. Analytical relations for the dependence of stature, measured mass (weight), growth velocity (and their mix as the body mass index) on age are deduced with a set of common statistical parameters which could relate environmental, genetics and metabolism and different aspects of physical growth on earth. A relationship to the monotone smoothing using functional data analysis to estimate growth curves and its derivatives is established. A preliminary discussion is also presented on horizontal growth in an essentially weightless environment (i.e., aquatic) with a connection to the Laird-Gompertz formula for growth.

Abstract:Body-mass index, abbreviated as BMI and given by M/H2 with the mass M and the height H, has been widely used as a useful proxy to measure a general health status of a human individual. We generalise BMI in the form of M/Hp and pursue to answer the question of the value of p for populations of animal species including human. We compare values of p for several different datasets for human populations with the ones obtained for other animal populations of fish, whales, and land mammals. All animal populations but humans analyzed in our work are shown to have p ≈ 3 unanimously. In contrast, human populations are different: As young infants grow to become toddlers and keep growing, the sudden change of p is observed at about one year after birth. Infants younger than one year old exhibit significantly larger value of p than two, while children between one and five years old show p ≈ 2, sharply different from other animal species. The observation implies the importance of the upright posture of human individuals. We also propose a simple mechanical model for a human body and suggest that standing and walking upright should put a clear division between bipedal human (p ≈ 2) and other animals (p ≈ 3).

Abstract:Body mass index (BMI) is an anthropometric index that is commonly used in the medical setting and is a factor in assessing various disease risks but its origins are unknown by many. More importantly, BMI does not properly assess body fat percentage and muscle mass or distinguish abdominal fat from gluteofemoral fat, which is important to note because abdominal fat is associated with insulin resistance, metabolic disease, and cardiovascular complications. Using a less accurate index to assess the relationship between weight and disease risk is conceptually invalid because the use of BMI ultimately trickles into patient treatment, preventive medicine, and overall health outcomes. Several different anthropometric indices that more accurately assess abdominal adiposity through the incorporation of waist circumference exist and have been extensively studied, such as waist-to-hip ratio, waist-to-height ratio, and a body shape index. It is important that we consider replacing BMI's usage in the healthcare setting with a different anthropometric index: one that considers height, sex, and race differences, accounts for abdominal adiposity, and more accurately predicts the relationship between obesity, mortality, and diseases such as cardiovascular disease, hypertension, insulin resistance, and diabetes.

Abstract:Body mass index (BMI), a measurement based on a person's height and weight, allows the classification of individuals into categories such as obese or overweight. With these classifications, we can assess risk for hypertension, diabetes, cancer, hypercholesterolemia, and other chronic diseases. Furthermore, childhood BMI serves as a prediction method for health and disease later in life. Along with BMI, researchers also study waist circumference and waist-to-hip ratio in correlation with the above-mentioned chronic illnesses. This brief review explores the associations between body mass index, waist circumference, and the waist-hip ratio as measurements and their capability as predictors for persistent conditions like diabetes and hypertension.
The Statistical Mechanics of Human Weight Change

Abstract:In the context of the global obesity epidemic, it is important to know who becomes obese and why. However, the processes that determine the changing shape of Body Mass Index (BMI) distributions in high-income societies are not well-understood. Here we establish the statistical mechanics of human weight change, providing a fundamental new understanding of human weight distributions. By compiling and analysing the largest data set so far of year-over-year BMI changes, we find, strikingly, that heavy people on average strongly decrease their weight year-over-year, and light people increase their weight. This drift towards the centre of the BMI distribution is balanced by diffusion resulting from random fluctuations in diet and physical activity that are, notably, proportional in size to BMI. We formulate a stochastic mathematical model for BMI dynamics, deriving a theoretical shape for the BMI distribution and offering a mechanism to explain the ongoing right-skewed broadening of BMI distributions over time. The model also provides new quantitative support for the hypothesis that peer-to-peer social influence plays a measurable role in BMI dynamics. More broadly, our results demonstrate a remarkable analogy with drift-diffusion mechanisms that are well-known from the physical sciences and finance.

Abstract:Abstract In this work, statistical formulae, with depend on anthropometric measurement and the age, are used to estimate the weight of the lower limb of 20 young, healthy, adults. Then, four clinical characteristics, the body mass index (BMI), the blood volume (BV), basal metabolic rate (BMR) and skin surface, also based on statistical formulae, are estimated for this sample population. Furthermore, the four clinical characteristics are estimated for 20 fictitious patients, with one lower limb amputated. From the sample population, the averaged results are the lower limb mass is 12.05 ± 2.79 kg, the BMI 22.89 ± 3.93 kg/m 2 , BV 3.99 ± 0.82 l, BMR 1,543.32 ± 236.98 kCal and body surface (BS) 1.66 ± 0.20 m 2 . While for the fictitious patients, the results are the BMI of 18.42 ± 3.17 kg/m 2 , BV of 3.60 ± 0.74 l, BMR of 1,399.07 ± 183.38 kCal and BS of 1.51 ± 0.18 m 2 . The clinic characteristics of the sample population, healthy-young adults showed, for instance, that the BV vary from 2.84 l and up to 5.67 l. The lower limb mass is approximately 20% of the total mass of the individual, while the BV, after the amputation, decreased by an amount of 10%. By having equations, depending on anthropometric measurements and age provides valuable information on the clinical characteristics of the patient. Thus, the clinicians, adding their experience to these results, may be more confident to the treatment and the evolution of the patient.

Abstract:Meaningful laws of nature must be independent of the units employed to measure the variables. The principle of similitude (Rayleigh 1915) or dimensional homogeneity, states that only commensurable quantities (ones having the same dimension) may be compared, therefore, meaningful laws of nature must be homogeneous equations in their various units of measurement, a result which was formalized in the $\rm \Pi$ theorem (Vaschy 1892; Buckingham 1914). However, most relations in allometry do not satisfy this basic requirement, including the `3/4 Law' (Kleiber 1932) that relates the basal metabolic rate and body mass, which it is sometimes claimed to be the most fundamental biological rate (Brown et al. 2004) and the closest to a law in life sciences (West \& Brown 2004). Using the $\rm \Pi$ theorem, here we show that it is possible to construct a unique homogeneous equation for the metabolic rates, in agreement with data in the literature. We find that the variations in the dependence of the metabolic rates on body mass are secondary, coming from variations in the allometric dependence of the heart frequencies. This includes not only different classes of animals (mammals, birds, invertebrates) but also different exercise conditions (basal and maximal). Our results demonstrate that most of the differences found in the allometric exponents (White et al. 2007) are due to compare incommensurable quantities and that our dimensionally homogenous formula, unify these differences into a single formulation. We discuss the ecological implications of this new formulation in the context of the Malthusian's, Fenchel's and the total energy consumed in a lifespan relations.

Abstract:The purpose of this study is to investigate the association between anthropometric characteristics and performance in all track and field running events and assess Body Mass Index (BMI) as a relevant performance indicator. Data of mass, height, BMI and speed were collected for the top 100 international men athletes in track events from 100 m to marathon for the 1996-2011 seasons, and analyzed by decile of performance. Speed is significantly associated with mass (r = 0.71) and BMI (r = 0.71) in world-class runners and moderately with height (r = 0.39). Athletes, on average were continuously lighter and smaller with distance increments. In track and field, speed continuously increases with BMI. In each event, performances are organized through physique gradients. « Lighter and smaller is better » in endurance events but « heavier and taller is better » for sprints. When performance increases, BMI variability progressively tightens, but it is always centered around a distance-specific optimum. Running speed is organized through biometric gradients, which both drives and are driven by performance optimization. The highest performance level is associated with narrower biometric intervals. Through BMI indicators, diversity is possible for sprints whereas for long distance events, there is a more restrictive aspect in terms of physique. BMI is a relevant indicator, which allows for a clear differentiation of athletes' capacities between each discipline and level of performance in the fields of human possibilities.

Abstract:Obesity rates have been increasing over recent decades, causing significant concern among policy makers. Excess body fat, commonly measured by body mass index, is a major risk factor for several common disorders including diabetes and cardiovascular disease, placing a substantial burden on health care systems. To guide effective public health action, we need to understand the complex system of intercorrelated influences on body mass index. This paper, based on all eligible articles searched from Global health, Medline and Web of Science databases, reviews both classical and modern statistical methods for body mass index analysis. We give a description of each of these methods, exploring the classification, links and differences between them and the reasons for choosing one over the others in different settings. We aim to provide a key resource and statistical library for researchers in public health and medicine to deal with obesity and body mass index data analysis.

Abstract:Obesity interventions can result in weight loss, but accurate prediction of the bodyweight time course requires properly accounting for dynamic energy imbalances. In this report, we describe a mathematical modelling approach to adult human metabolism that simulates energy expenditure adaptations during weight loss. We also present a web-based simulator for prediction of weight change dynamics. We show that the bodyweight response to a change of energy intake is slow, with half times of about 1 year. Furthermore, adults with greater adiposity have a larger expected weight loss for the same change of energy intake, and to reach their steady-state weight will take longer than it would for those with less initial body fat. Using a population-averaged model, we calculated the energy-balance dynamics corresponding to the development of the US adult obesity epidemic. A small persistent average daily energy imbalance gap between intake and expenditure of about 30 kJ per day underlies the observed average weight gain. However, energy intake must have risen to keep pace with increased expenditure associated with increased weight. The average increase of energy intake needed to sustain the increased weight (the maintenance energy gap) has amounted to about 0·9 MJ per day and quantifies the public health challenge to reverse the obesity epidemic.

To guide effective public health action, we need to understand the complex system of intercorrelated influences on BMI. This paper will review both classical and modern statistical methods for BMI analysis, highlighting that most of the classical methods are simple and easy to implement but ignore the complexity of data and structure, whereas modern methods do take complexity into consideration but can be difficult to implement. A series of case studies are presented to illustrate these methods and some potentially useful new models are suggested.

Modeling transitions in body composition: the approach to steady state for anthropometric measures and physiological functions in the Minnesota human starvation study

Abstract:BackgroundThis study evaluated whether the changes in several anthropometric and functional measures during caloric restriction combined with walking and treadmill exercise would fit a simple model of approach to steady state (a plateau) that can be solved using spreadsheet software (Microsoft Excel®). We hypothesized that transitions in waist girth and several body compartments would fit a simple exponential model that approaches a stable steady-state.MethodsThe model (an equation) was applied to outcomes reported in the Minnesota starvation experiment using Microsoft Excel's Solver® function to derive rate parameters (k) and projected steady state values. However, data for most end-points were available only at t = 0, 12 and 24 weeks of caloric restriction. Therefore, we derived 2 new equations that enable model solutions to be calculated from 3 equally spaced data points.ResultsFor the group of male subjects in the Minnesota study, body mass declined with a first order rate constant of about 0.079 wk-1. The fractional rate of loss of fat free mass, which includes components that remained almost constant during starvation, was 0.064 wk-1, compared to a rate of loss of fat mass of 0.103 wk-1. The rate of loss of abdominal fat, as exemplified by the change in the waist girth, was 0.213 wk-1.On average, 0.77 kg was lost per cm of waist girth. Other girths showed rates of loss between 0.085 and 0.131 wk-1. Resting energy expenditure (REE) declined at 0.131 wk-1. Changes in heart volume, hand strength, work capacity and N excretion showed rates of loss in the same range. The group of 32 subjects was close to steady state or had already reached steady state for the variables under consideration at the end of semi-starvation.ConclusionWhen energy intake is changed to new, relatively constant levels, while physical activity is maintained, changes in several anthropometric and physiological measures can be modeled as an exponential approach to steady state using software that is widely available. The 3 point method for parameter estimation provides a criterion for testing whether change in a variable can be usefully modelled with exponential kinetics within the time range for which data are available.

Abstract:Body composition is a key component for maintaining good general health and longevity. It can be influenced by a variety of factors, including genetics, environment, and lifestyle choices. The assessment of body composition is an essential tool for nutrition specialists to effectively evaluate nutritional status and monitor progression during dietary interventions. As humans age, there is a natural increase in fat mass coupled with a gradual decline in lean mass, specifically in bone and muscle mass. Individuals with a high body fat percentage are at a greater risk of cardiovascular diseases, type 2 diabetes, several types of cancer, and early mortality. Significant decreases in bone mineral density signify osteopenia and osteoporosis, while reductions in skeletal muscle mass increase the risk of developing sarcopenia. Moreover, undernutrition exacerbates the effects of many medical conditions and is important to address. Though weight tracking and calculation of BMI are used commonly by clinicians and dietitians, these measures do not provide insight on the relative contributions of fat mass and fat-free mass or the changes in these compartments that may reflect disease risk. Therefore, it is important that healthcare professionals have a critical understanding of body composition assessment and the strengths and limitations of the methods available.

Abstract:Knowledge of human body composition at the dawn of the twentieth century was based largely on cadaver studies and chemical analyses of isolated organs and tissues. Matters soon changed by the nineteen twenties when the Czech anthropologist Jindřich Matiegka introduced an influential new anthropometric method of fractionating body mass into subcutaneous adipose tissue and other major body components. Today, one century later, investigators can not only quantify every major body component in vivo at the atomic, molecular, cellular, tissue-organ, and whole-body organizational levels, but go far beyond to organ and tissue-specific composition and metabolite estimates. These advances are leading to an improved understanding of adiposity structure-function relations, discovery of new obesity phenotypes, and a mechanistic basis of some weight-related pathophysiological processes and adverse clinical outcomes. What factors over the past one hundred years combined to generate these profound new body composition measurement capabilities in living humans? This perspective tracks the origins of these scientific innovations with the aim of providing insights on current methodology gaps and future research needs.

Relationship between body mass, lean mass, fat mass, and limb bone cross-sectional geometry: Implications for estimating body mass and physique from the skeleton

Abstract:Objectives: Estimating body mass from skeletal dimensions is widely practiced, but methods for estimating its components (lean and fat mass) are poorly developed. The ability to estimate these characteristics would offer new insights into the evolution of body composition and its variation relative to past and present health. This study investigates the potential of long bone cross-sectional properties as predictors of body, lean, and fat mass. Materials and methods: Humerus, femur and tibia midshaft cross-sectional properties were measured by peripheral quantitative computed tomography in sample of young adult women (n = 105) characterized by a range of activity levels. Body composition was estimated from bioimpedance analysis. Results: Lean mass correlated most strongly with both upper and lower limb bone properties (r values up to 0.74), while fat mass showed weak correlations (r ≤ 0.29). Estimation equations generated from tibial midshaft properties indicated that lean mass could be estimated relatively reliably, with some improvement using logged data and including bone length in the models (minimum standard error of estimate = 8.9%). Body mass prediction was less reliable and fat mass only poorly predicted (standard errors of estimate ≥11.9% and >33%, respectively). Discussion: Lean mass can be predicted more reliably than body mass from limb bone cross-sectional properties. The results highlight the potential for studying evolutionary trends in lean mass from skeletal remains, and have implications for understanding the relationship between bone morphology and body mass or composition.

Abstract:The most widely used measure of adiposity is to express weight adjusted for height using the body mass index (BMI). However, its limitations such as its inability to distinguish muscle weight from fat weight are well known, leading public health authorities in the UK and US to recommend measuring waist circumference as a complementary diagnostic tool for obesity. Recent attention placed on the syndrome referred to as 'normal weight obesity' – individuals with normal BMI but high body fat content – emphasizes the need for a more comprehensive diagnostic tool for obesity. Based on the NHANES III data, we utilize a semi-parametric spline approach to depict graphically the relationship between BMI, waist circumference and percent body fat. In this note, we propose that percent body fat charts that incorporate information from three anthropometric dimensions supersede the one-size-fits-all obesity diagnostic approach based on power-type indices such as the BMI.

Abstract:Abstract The assessment of body composition has important applications in the evaluation of nutritional status and estimating potential health risks. Bioelectrical impedance analysis (BIA) is a valid method for the assessment of body composition. BIA is an alternative to more invasive and expensive methods like dual-energy X-ray absorptiometry, computerized tomography, and magnetic resonance imaging. Bioelectrical impedance analysis is an easy-to-use and low-cost method for the estimation of fat-free mass (FFM) in physiological and pathological conditions. The reliability of BIA measurements is influenced by various factors related to the instrument itself, including electrodes, operator, subject, and environment. BIA assumptions beyond its use for body composition are the human body is empirically composed of cylinders, FFM contains virtually all the water and conducting electrolytes in the body, and its hydration is constant. FFM can be predicted by BIA through equations developed using reference methods. Several BIA prediction equations exist for the estimation of FFM, skeletal muscle mass (SMM), or appendicular SMM. The BIA prediction models differ according to the characteristics of the sample in which they have been derived and validated in addition to the parameters included in the multiple regression analysis. In choosing BIA equations, it is important to consider the characteristics of the sample in which it has been developed and validated, since, for example, age- and ethnicity-related differences could sensitively affect BIA estimates.
