Abstract:Background: Telemedicine, a term that encompasses several applications and tasks, generally involves the remote management and treatment of patients by physicians. It is known as transversal telemedicine when practiced among health care professionals (HCPs). Objective: We describe the experience of implementing our telemedicine Eumeda platform for HCPs over the last 10 years. Methods: A web-based informatics platform was developed that had continuously updated hypertext created using advanced technology and the following features: security, data insertion, dedicated software for image analysis, and the ability to export data for statistical surveys. Customizable files called "modules" were designed and built for different fields of medicine, mainly in the ophthalmology subspecialty. Each module was used by HCPs with different authorization profiles. Implementation (results): Twelve representative modules for different projects are presented in this manuscript. These modules evolved over time, with varying degrees of interconnectivity, including the participation of a number of centers in 19 cities across Italy. The number of HCP operators involved in each single module ranged from 6 to 114 (average 21.8, SD 28.5). Data related to 2574 participants were inserted across all the modules. The average percentage of completed text/image fields in the 12 modules was 65.7%. All modules were evaluated in terms of access, acceptability, and medical efficacy. In their final evaluation, the participants judged the modules to be useful and efficient for clinical use. Conclusions: Our results demonstrate the usefulness of the telemedicine platform for HCPs in terms of improved knowledge in medicine, patient care, scientific research, teaching, and the choice of therapies. It would be useful to start similar projects across various health care fields, considering that in the near future medicine as we know it will completely change.

Abstract:This is the first article in a Series on collaborative projects between European countries, relevant for general practice/family medicine and primary healthcare. Telemedicine, in particular the use of the Internet, videoconferencing and handheld devices such as smartphones, holds the potential for further strides in the application of technology for the delivery of healthcare, particularly to communities in rural and remote areas within and without the European Union where this study is taking place. The Northern Periphery Programme has funded the 'Implementing Transnational Telemedicine Solutions' (ITTS) project from September 2011 to December 2013, led by the Centre for Rural Health in Inverness, Scotland. Ten sustainable projects based on videoconsultation (speech therapy, renal services, emergency psychiatry, diabetes), mobile patient self-management (physical activity, diabetes, inflammatory bowel disease) and home-based health services (medical and social care emergencies, rehabilitation, multi-morbidity) are being implemented by the six partner countries: Scotland, Finland, Ireland, Northern Ireland, Norway and Sweden. In addition, an International Telemedicine Advisory Service, created for the project, provides business expertise and advice. Community panels contribute feedback on the design and implementation of services and ensure 'user friendliness'. The project goals are to improve accessibility of healthcare in rural and remote communities, reducing unnecessary hospital visits and travel in a sustainable way. Opportunities will be provided for comparative research studies. This article provides an introduction to the ITTS project and how it aims to fulfil these needs. The ITTS team encourage all healthcare providers to at least explore possible technological solutions within their own context.

eHealth Literacy: From Theory to Clinical Application for Digital Health Improvement. Results from the ACCESS Training Experience

Abstract:Skills, knowledge, and awareness of digital and technological tools are essential to improve the state of well-being and health of older adults and also to mitigate the condition of social isolation in the aging process. For this reason, it is necessary to implement a social learning of electronic/digital tools for health of older people to support the achievement of eHealth and digital competences. The paper reports the results of an Italian innovative eHealth training for the European project ACCESS. The training has been based on blended didactical and interactive educational techniques, aimed at collecting as many points of view as possible from older adults. A total of 58 older adults were recruited to attend a four-week training program, which included five modules. The results showed a statistical significant difference between the eHealth Literacy Scale (eHEALS) mean value before and after the course. A significant negative correlation was found between eHEALS and positive/total Survey of Technology Use (SOTU), suggesting an inverse relationship between positive/total SOTU and eHEALS. There is a strong positive and statistically significant relationship between satisfaction with the training and eHEALS. The results indicate that the intervention increased the digital competences of participants connected to health.

A multimedia approach to raising awareness of information and communications technology amongst healthcare professionals

Abstract:This study is concerned with the application of advanced multimedia technology to the development of a programme aimed at raising awareness of information and communications technology (ICT) amongst health professionals in Ireland. The programme is delivered in the form of a symposium supplemented by a multimedia CD and associated web site. It examines how ICT can be used effectively in healthcare across all sectors - primary, secondary and tertiary - with a strong emphasis on supporting shared care. The aim is to empower users to make informed technological choices and to actively participate in the exploitation of ICT in the health sector. The programme was successfully completed and delivered to over 2300 health professionals across Ireland and follow-up activities include the active encouragement of leaders and champions within the sector. This will be supported by interactive web-based education and training material focused on specialised topics of particular interest within the broader context of continuing medical education (CME).
Systems of evidence-based healthcare and personalised health information: some international and national trends

Abstract:In Europe, North America and elsewhere, growing interest has focussed on evidence-based healthcare systems, incorporating the deployment of practice guidelines, as a field of application for health telematics. The clinical benefit and technical feasibility of common European approaches to this task has recently been demonstrated. In Europe it is likely that, building on recent progress in electronic health record architecture (EHRA) standards, a sufficient state of maturity can be reached to justify initiation within CEN TC251 of a prestandards process on guideline content formats during the current 5th Framework of EC RT&D activity. There is now a similar impetus to agree standards for this field in North America. Thanks to fruitful EC-USA contacts during the 4th Framework programme, there is now a chance, given well-planned coordination, to establish a global consensus optimally suited to serve the world-wide delivery and application of evidence-based medicine. This review notes three factors which may accelerate progress to convergence: (1) revolutionary changes in the knowledge basis of professional/patient/public healthcare partnerships, involving the key role of the Web as a health knowledge resource for citizens, and a rapidly growing market for personalised health information and advice; (2) the emergence at national levels of digital warehouses of clinical guidelines and EBM knowledge resources, agencies which are capable of brokering common mark-up and interchange media definitions between knowledge providers, industry and healthcare organizations; (3) the closing gap in knowledge management technology, with the advent of XML and RDF, between approaches and services based respectively on text mark-up and knowledge-base paradigms. A current project in the UK National Health Service (the National electronic Library of Health) is cited as an example of a national initiative designed to harness these trends.

Abstract:HealthTEK is a platform for hybrid learning addressed to both elderly people and intergenerational students, interested in engineering and medicine. The rationale between this combination is to create synergies between any age students and scholars from both fields in order to improve the knowlege in future health systems within the European Union, to allow them to be empowered in terms of integrated care to allow them to be more autonomous. The project is funded by the European Commission under the Erasmus+ project and it is based on an online architecture that allows students to access syllabi for the courses of introduction to biomedical engineering and anatomy, including as pedagogical strategy the use of gamified lessons about the project frAAgiLe in the case of biomedical engineering and about several case studies related to the treatment of different health issues. As for professors, families or caregivers they will have access to information about the performance of their multigenerational students and they will be able to modify and add contents to the different lessons. This interactive system will also allow the researchers behind its creation in the universities of Deusto, ESTIA, Bialystok, and Francisco de Vitoria, to collect data and implement AI algorithms that allow the personalisation and improvement of contents. Within the development of digital solutions for integrated care, this project does not only deal with an improvement and a closer relationship between healthcare and technology, it also works with students and scholars with reduced mobility, in order to make more contents accessible for them. The hybrid online solution that includes gamification is an optimised way to increase accessibility for those with disabilities, and also for those that, due to mental health problems, like anxiety, which is increasingly common in the post-COVID 19 panorama, struggle to get out of their homes or get into social situations like a classroom. Apart from contributing to the development of digital solutions, HealthTEK also deals with the Integrated Care pillar of improvement of health and health systems, integrated care and empowerment of elderly citizens in particular. Through an improved educative system with hybrid and digital approaches, as well as gamified inclusion of students and scholars with reduced mobility, the future of healthcare can be improved as well. The contents of the units included in HealthTEK have been created by health professionals with the newest methodologies, focusing on recreating real situations and training students in natural environments, showing them the processes of their future health-related jobs from the beginning until the conclusion of each activity. By always showing these activities from a first person perspective and making them become an active part of the gamified unit, the training ensures the collection of knowledge and skills in a natural manner. In sum, the HealthTEK is part of the digital development movement for the improvement of health and healthcare systems and its testing and implementation will prove the benefits of creating new synergies between engineering and medicine studies and the appropriate training of citizens to improve their health self-management.

Training Service Users in the Use of Telehealth: Scoping Review

Abstract:Background: The use of telehealth has rapidly increased, yet some populations may be disproportionally excluded from accessing and using this modality of care. Training service users in telehealth may increase accessibility for certain groups. The extent and nature of these training activities have not been explored. Objective: The objective of this scoping review is to identify and describe activities for training service users in the use of telehealth. Methods: Five databases (MEDLINE [via PubMed], Embase, CINAHL, PsycINFO, and Web of Science) were searched in June 2023. Studies that described activities to train service users in the use of synchronous telehealth consultations were eligible for inclusion. Studies that focused on health care professional education were excluded. Papers were limited to those published in the English language. The review followed the Joanna Briggs Institute guidelines for scoping reviews and was reported in line with the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews) guidelines. Titles and abstracts were screened by 1 reviewer (EG). Full texts were screened by 2 reviewers (EG and JH or SC). Data extraction was guided by the research question. Results: The search identified 8087 unique publications. In total, 13 studies met the inclusion criteria. Telehealth training was commonly described as once-off preparatory phone calls to service users before a telehealth visit, facilitated primarily by student volunteers, and accompanied by written instructions. The training content included guidance on how to download and install software, troubleshoot technical issues, and adjust device settings. Older adults were the most common target population for the training. All but 1 of the studies were conducted during the COVID-19 pandemic. Overall, training was feasible and well-received by service users, and studies mostly reported increased rates of video visits following training. There was limited and mixed evidence that training improved participants' competency with telehealth. Conclusions: The review mapped the literature on training activities for service users in telehealth. The common features of telehealth training for service users included once-off preparatory phone calls on the technical elements of telehealth, targeted at older adults. Key issues for consideration include the need for co-designed training and improving the broader digital skills of service users. There is a need for further studies to evaluate the outcomes of telehealth training activities in geographically diverse areas.

Abstract:BACKGROUND The presence of tumor metastases in the mediastinum is one of the most important elements in determining the optimal treatment strategy in patients with non-small cell lung cancer. This review is aimed at examining the current strategies for investigating lymph node metastases corresponding to an "N2" classification delineated by The International Staging Committee of the International Association for the Study of Lung Cancer (IASLC). METHODS Extensive review of the existing scientific literature related to the investigation of mediastinal lymph node metastases was undertaken in order to summarize and report current best practices. CONCLUSIONS N2 disease is very heterogeneous requiring multiple modalities for thorough investigation. New research is now focusing on better identifying, defining, and classifying lymph node metastases in the mediastinum. Molecular staging and sub-classifying mediastinal lymph node metastases are being actively researched in order to provide better prognostic value and to optimize treatment strategies. Non-invasive imaging, such as PET/CT and minimally invasive techniques such as endobronchial and endoscopic ultrasound guided biopsy, are now the lead investigative methods in evaluating the mediastinum for metastatic presence.

Abstract:1. Placental blood flow rate and calcium transfer rate were measured at 61 d of pregnancy in guinea-pigs carrying between one and eight foetuses. 2. Placental blood flow rate was significantly correlated with foetal weight. Ca transfer rate was related to placental size. Irrespective of litter size the mean amount of Ca transferred across a placenta was between 0.22 and 0.34 mg/h per g placental tissue. 3. It was concluded that there was a limit to the rate of transfer which was produced by a combination of limitations in placental blood flow rate, maternal plasma Ca concentration and placental tissue transfer capacity.
Research on cooperative learning and achievement: What we know, what we need to know.

Abstract:Research on cooperative learning is one of the greatest success stories in the history of educational research. While there was some research on this topic from the early days of this century, the amount and quality of that research greatly accelerated in the early 1970's, and continues unabated today, a quarter-century later. Hundreds of studies have compared cooperative learning to various control methods on a broad range of measures, but by far the most frequent objective of this research is to determine the effects of cooperative learning on student achievement. Studies of the achievement effects of cooperative learning have taken place in every major subject, at all grade levels, in all types of schools in many countries. Both field studies and laboratory studies have produced a great deal of knowledge about the effects of many types of cooperative interventions and about the mechanisms responsible for these effects. Further, cooperative learning is not only a subject of research and theory; it is used at some level by millions of teachers. A recent national survey (Puma, Jones, Rock, & Fernandez, 1993) found that 79% of elementary teachers and 62% of middle school teachers reported making some sustained use of cooperative learning.
Disseminating Research Outputs: The CrowdHEALTH Project

Abstract:Introduction: Dissemination benefits come from the outputs integration and implementation by the key audience, who will also determine the relevance and usability of the disseminated content. Aim: One of the CrowdHEALTH project's objectives is the transition from patient health records towards the Holistic Health Records (HHRs) and Social HHR. The CrowdHEALTH project aims at integrating high volumes of health-related data collected from various sources to support policy-making decisions. Methods: The European Federation for Medical Informatics (EFMI) supports the development of an effective Communication and Collaboration Plan identifying the messages, the tools and channels in disseminating the project and its outcomes to the target audience based on the McGuire approach. Results: The process for defining the dissemination strategy is a cyclic one as shown in the following figure involving review of each step periodically The next step was to define the four dimension dissemination approach based on McGuire attributes of persuasive communication. The objectives, target groups, key messages, the tools and channels where defined at this stage. Conclusion: The CrowdHEALTH project and its outcomes were disseminated with a variety of tools and channels such as scientific journals, conferences, exhibitions and social media communication.
TELEHEALTH CURRICULUM AT HEALTH CARE HIGHER EDUCATION: AN INTERNATIONAL EDELPHI STUDY

Abstract:Background Considering the increasingly frequent use of telehealth, the lack of training of health professionals in telehealth care, it becomes essential to train and specialize future health professionals, with a formal and structured education at a higher level in the use of telehealth. There is no guideline on the core competencies in telehealth at health care higher education, therefore, a consensus is of paramount importance to externally validate these findings, compiling the core competencies in telehealth. Objective To identify the core competencies in telehealth needed in the curriculum at higher education. Methods International modified eDelphi study conducted in 3 rounds. The panel was made up of expert's researchers in telehealth selected on the Expertscape and Pubmed platform, in addition to clinicians, professors, administrators and higher-level coordinators for the snowball sample. The questions were developed by an international steering committee and tested in a pilot test. The first round was presented to the participants with closed questions, with 47 competences distributed in 11 domains, and an open question, the participants judged the degree of agreement on the competencies and suggested new competencies. The consensus was defined with the competencies that reached high agreement (>75%) at the end of the third round. Results Total of 100 participants, from 18 different countries, responded to first round suggesting 2 new competencies and 1 new domain. At the end of the third round with 80 participants, we reached a consensus (>75%) with 47 core competencies in a telehealth curriculum distributed in 12 domains: principles of telehealth; care planning and management; assessment, diagnosis, and treatment; adequacy of the environment; professionalism; legal aspects; patient privacy; patient safety; access and equity; patient preference; technology; applicability of telehealth. Conclusion Our framework describes the core competencies distributed in different domains necessary in the telehealth curriculum in health care higher education, recommended by a panel of international experts, clinicians and professors. Future research on implementation and effectiveness needs to be carried out to investigate whether the structure provided in this study covers the need to train future professionals. Implications This curriculum is a first step towards promoting higher education and training of future health professionals in telehealth care. The core competencies in a telehealth curriculum make it possible to guide teaching institutions and health course coordinators, regarding the training of students and contribution in the disciplines that use or may use the telehealth resource, being a first step in the educational development of international telehealth.

District nurses' views on and experiences with a telemedicine educational programme in palliative care

Abstract:Background: Medical advancements, limited resources and shifting demographics have increased the number of patients with palliative care needs in primary care. To address educational needs, the specialised palliative care team of South Jutland, Denmark, created a telemedicine educational programme in palliative care to empower district nurses. Aim: The study aimed to explore district nurses' views on and experiences with a telemedicine educational programme in palliative care. Research methods: A qualitative explorative study based on interpretive description was conducted. Data collection consisted of four focus group interviews with district nurses from three municipalities, supplemented by participant observations and a focus group interview with teachers from the specialised palliative care team. Data were analysed using predominately inductive thematic analysis. Results: District nurses (n = 15) who participated in the programme and members of the specialised palliative care team (n = 6) who taught the programme were included. Analysis revealed the following advantages: reaffirming and updating existing knowledge, reduced professional isolation and creation of a forum to promote knowledge dissemination. A disadvantage was limited interaction between teachers and district nurses, questioning suitability for teaching complex palliative care. Initial technical problems affected motivation to participate. Organisational support differed between participating municipalities resulting in varying degrees of programme integration. Despite advantages of IT-expert-led sessions, key-nurse-led sessions in smaller groups proved more beneficial, suggesting a combination of IT support and key-nurse management to maximise benefits. Conclusion: The use of an inter-professional telemedicine educational programme to teach palliative care to district nurses is beneficial. However, programmes should be designed for interactivity and address varying educational needs. Key-nurse roles require managerial and IT support to optimise knowledge dissemination. Further research on implementation of telemedicine education in palliative care is needed.

Abstract:Purpose of Review:This article addresses the occurrence of acute symptomatic seizures in the setting of many medical illnesses. Recent Findings:Many medical illnesses can cause seizures. Metabolic derangements, such as disorders of serum glucose metabolism, cause seizures, as well as other neurologic manifestations. Many medicines (such as antibiotics, antivirals, antidepressants, and antipsychotics), whether used at typical doses or in overdosage, can lead to acute symptomatic seizures. Summary:Acute symptomatic seizures occur most often in illnesses that directly injure the brain. Trauma, stroke, CNS tumors, and CNS infection very commonly cause seizures. However, many medical illnesses do not directly injure the brain but lead to neurologic signs and symptoms, such as seizures. Recognizing these effects, especially in critically ill patients, is important for optimizing medical care.

Abstract:In organizations that use DevOps practices, software changes can be deployed as fast as 500 times or more per day. Without adequate involvement of the security team, rapidly deployed software changes are more likely to contain vulnerabilities due to lack of adequate reviews. The goal of this paper is to aid software practitioners in integrating security and DevOps by summarizing experiences in utilizing security practices in a DevOps environment. We analyzed a selected set of Internet artifacts and surveyed representatives of nine organizations that are using DevOps to systematically explore experiences in utilizing security practices. We observe that the majority of the software practitioners have expressed the potential of common DevOps activities, such as automated monitoring, to improve the security of a system. Furthermore, organizations that integrate DevOps and security utilize additional security activities, such as security requirements analysis and performing security configurations. Additionally, these teams also have established collaboration between the security team and the development and operations teams.

Abstract:<sec>PurposeInterest is growing in integrated systems of e-care for the frail elderly. The present project promotes cognitive training and assessment using ICT tools, stimulates the socialization, shares and monitors the situation of the elderly by different professionals, care givers and relatives.</sec><sec>ContextWithin the frame of the European Commission funded SOCIABLE Project, 348 elderly are participating in the trial.</sec><sec>Data sourcesThe approach was pilot in seven different sites, across Italy (2 Geriatrics Units and 1 Municipality), Greece (1 Memory Clinic and 1 Municipality), Spain (1 University) and Norway (1 Municipality).</sec><sec>Case descriptionSOCIABLE pilot study is a multi-national, multicenter, non-randomized, placebo-controlled efficacy study. Its primary objective is to evaluate the effects of a computer-based cognitive training and social activation program on the cognition, the affection and the functional abilities of cognitively intact elderly, patients with MCI and patients with mild AD. Conclusions: Two types of models of integrated care delivery for the frail elderly have been tested; one was characterized by group sessions in the pilot center assisted by caregiver or psychologist, another was stipulated in the use of ICT tools by the elderly at home. The first results of the trial will be illustrated.</sec><sec>DiscussionIntegrated care delivery can be achieved in various ways. Irrespective of which model is adopted were detected high levels of satisfaction and performance by all groups and an important ability to monitor results by medical experts and care givers.</sec>

Abstract:Abstract Issue After Covid-19, Italy approved the National Recovery and Resilience Plan (NRRP) to boost public health, sustainability and digital innovation, which promotes ecological and digital transition reforms. The strategic health reform redesigned territorial healthcare network by introducing new models of proactive care, as Territorial Coordination Centres (TCCs) and Telenursing. Description of the problem TCC is an innovative organisational model, that coordinates taking care by linking services and professionals involved in different care settings. In TCCs are involved nursing staff, that performs a telemedicine service called Telenursing, a way of delivering nursing care directly to the patient's home, through digital health tools. How will TCCs be implemented, using NRRP funds? How will Telenursing impact on home care? Results By 2024, 600 TCCs are planned to be implemented and fully operational, across the Country, financed by NRRP (280 bln €). The standard set by the NRRP reform is 1 TCC each 100,000 inhabitants, with 4-6 primary nurses involved (2,400-3,600 nationally). TCCs are organised to be interconnected with every healthcare network setting, including home care. Telenursing, within the TCCs, aims to guarantee proactive, personalised and proximity care, enhancing the home as a privileged place of care. Nurses of TCCs perform health monitoring, assessment, promotion, prevention and education by Telenursing, in order to reduce inappropriate hospital admissions and to increase patient satisfaction, safety and empowerment. By 2026, at least 200,000 people will be assisted through telemedicine tools, as planned in NRRP reform (funding 1 bln €). Lessons Italy, pursuing better health outcomes for the population, is investing in strengthening public primary care services and digitalization of services. New strategy aims to improve health of population and decrease health inequalities, while reducing geographical variations. Key messages • Telenursing performed by nurses working in TCCs is aimed at improving proactive, personalised and proximity care, supporting patients and caregivers at home. • By 2024, TCCs will be fully operating to guarantee an innovative system of public primary care, that uses digital tools to manage population in home care, reducing inappropriate hospitalization.

Abstract:Telehealth is defined as the provision of health care services over a distance. Major health systems, including outpatient clinics and check-ups alike, turned to telehealth and teleconsultation amid the COVID19 pandemic. There are many recognized advantages of telehealth and its increasing implementation, however, not much is known about the specific impact of remote consultation on the quality of non-consultant hospital doctors (NCHD) training experiences in medicine. This study aimed to gain rich descriptive insights into the specific impact of remote consultation on the quality of medical NCHD training experiences and their perceptions, through purposeful sampling. Data was collected via semi-structured virtual interviews of fifteen NCHD participants. The interviews were recorded, transcribed and analysed using thematic analysis. The collective transcripts were analysed using NVIVO data software for common themes. The themes identified suggested that there were both advantages and disadvantages to telehealth and its impact on training across a spectrum of entities including; doctor-patient relationships, peer relationships, service provision, work morale, working hours, clinical skills experience and general feedback as a trainee. NCHD perspectives expressed were mixed across all themes. Further research is recommended to advance understanding of the potential impact of telehealth on current and prospective doctors in training. With a greater understanding of general insights, future changes, including telehealth training programmes, could be implemented to build confidence and familiarity with telehealth utilization and potentiate its conceivable benefits.

Abstract:Communication among medical informatics communities can suffer from fragmentation across multiple forums, disciplines, and subdisciplines; variation among journals, vocabularies and ontologies; cost and distance. Online communities help overcome these obstacles, but may become onerous when listservs are flooded with cross-postings. Rich and relevant content may be ignored. The American Medical Informatics Association successfully addressed these problems when it created a virtual meeting place by merging the membership of four working groups into a single listserv known as the "Implementation and Optimization Forum." A communication explosion ensued, with thousands of interchanges, hundreds of topics, commentaries from "notables," neophytes, and students--many from different disciplines, countries, traditions. We discuss the listserv's creation, illustrate its benefits, and examine its lessons for others. We use examples from the lively, creative, deep, and occasionally conflicting discussions of user experiences--interchanges about medication reconciliation, open source strategies, nursing, ethics, system integration, and patient photos in the EMR--all enhancing knowledge, collegiality, and collaboration.
