Abstract:【Objectives】To investigate relationship between the number of the occurrence of apoptosis of pneumocytes and ischemia /reperfusion injurance after transplant reperfusion.And to observe the expression of Fas and Bcl-2 which related to apoptosis.【Methods】The lung of 18 cases in human perfused with Raffinose-low-potassium dextran solution(R-LPD'S) was observed by light microscope,electrical microscope.Apoptosis of pneumocytes were stained by the terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling(TUNEL) technique.And immunohistochemistry for Fas and Bcl-2.【Results】Comparing with those taken 3 hours after cold ischemia,the lung tissue taken 0.5 hour and 1.5 hour after reperfusion showed strikingly increasing numbers of apoptotic cells quantified by apoptotic index(AI)(P 0.01).Significantly increasing expression of Fas and decreasing of Bcl-2.【Conclusion】 Apoptosis of pneumocytes happened during the process of lung transplsntation may be one of the features of early ischemia /reperfusion injury.The detection of apoptosis is important for the antiapoptosis treatment of preventing ischemia /reperfusion injury in lung transplantation.

Abstract:Lung transplantation is the most effective therapy for patients with end-stage lung disease. However, concern of donor lung damage and ischemia-reperfusion induced lung injury limits the use of "marginal" donor lungs. Recent transcriptomic studies have demonstrated that the enrichment of gene-clusters related to cell death and inflammation are the most profound signals during ischemia-reperfusion in human lung transplants. Herein, we focus on the relationship between inflammation and programmed cell death, especially necroptosis, mitochondrial permeability transition-initiated regulated necrosis, pyroptosis, ferroptosis, and autophagic cell death. Cell death-related molecules have been tested as potential biomarkers for donor lung assessment. Inhibitors for various types of cell death have been explored as therapeutics for ischemia reperfusion injury in lung transplantation. A deeper understanding of these mechanisms may help to improve donor management, organ preservation, prevention and treatment of primary graft dysfunction during and post transplantation. Moreover, evaluation and treatment of cell death and inflammation during ex vivo lung perfusion may be a game changer in donor organ management, assessment, repair, and reconditioning.

Abstract:OBJECTIVE:This study aimed to investigate the relationship between the severity of ischemia/reperfusion (I/R) injury and the acute rejection (AR) of allogeneic liver transplants in rats.METHODS:The experimental rats were divided in different groups: normal control group (sham group, group I); syngeneic liver transplant control group (similar gene group, group II); and allogeneic liver transplant groups (groups III to VI). The rats were humanely killed at 1, 3, 5, and 7 days after transplantation or sham operation to determine the severity of I/R injury, rejection classification, and hepatocyte apoptosis. Messenger RNA (mRNA) and protein expression levels of Fas, perforin, and granzyme B were assessed in the liver tissues using real-time polymerase chain reaction and immunohistochemistry, respectively.RESULTS:The rejection scores of the transplanted liver tissues gradually increased until these scores were proportional to the severity of I/R injury in groups III, IV, and V. The maximum scores were reached at 7 days after transplantation as the duration of transplantation was extended. The mRNA and protein expression levels of Fas, perforin, and granzyme B were significantly increased at 1, 3, 3, 5, and 7 days after liver reperfusion in groups III, IV, and V compared with those in groups I, II, and VI (P < .05).CONCLUSION:The occurrence of AR after allogeneic liver transplantation in rats was positively correlated with the severity of I/R injury. Given that I/R injury caused serious damage to the transplanted liver, the occurrence of AR consequently decreased.
The dynamic progress of cell death during donor lung preservation

Abstract:Objective To observe the dynamic progress of apoptosis and necrosis of pneumocytes during donor lung preservation, we tried to build a model to accurately assess graft function. Methods Experiments were performed with 42 male inbred (250 to 350 g) Sprague-Dawley rats, which were divided into 7 groups based on preservation times (0,2,4,6,12,18,24 h). The heart-lung block was then removed and stored in iced low potassium dextran(LPD) solution at 4 ℃ for different times. All lungs were flushed with 20 mL of a 500 mmol/L trypan blue solution through the main pulmonary artery, and then with 20 mL of 0.9% normal saline and 10 mL of 4% paraformaldehyde. The formalin-fixed lung tissues were embedded in paraffin and cut into 4-μm-thick tissue slices. We observed trypan blue staining with light microscopy. Detection of apoptosis through in situ TUNEL was undertaken, using in situ cell death detection kit according to the manufacturer’s instructions. Hoechst 33258 was used to show all the nucleated cells. Under fluorescent microscopy, cell counts were done by two researchers in a blinded fashion. Results The number of cell death increased and the type of the cell death changed by prolonged cold preservation time of the donor lungs. After cold ischemic storage, minimal cell death was seen in the 4 h groups. Significant cell death was detected only in lungs after 18 h of cold storage as determined through trypan blue-positive staining. On the contratry, apopotic cell begins in the 2 h groups, reach about 9% of all the nucleated cells in 12 h groups, and diminished in 18 h groups. Conclusions The study demonstrates a significant association among cold preservation time, extent and mode of cell death, suggesting new potential strategies to accurately assess graft function.

Abstract:Objective To study the effect of aprotinin on improving graft function and inhibiting cell apoptosis of cells of donor lung after transplantation and to in vestigate the relation between post-transplant lung function and apoptosis.Methods On an isolated,whole blood-perfused,ventilated rabbit lung transplantation model,24 rabbits were divided into two groups:in group A the donor lung block was flushed with LPD solution through the pulmonary artery,and in group B aprotinin in 150KIU/ml was added to the pulmonary flushing fluid.Oxygenation capacity(PaO2,PaCO2)and pulmonary compliance(PaP,PawP,CL)were continuously monitored during the one-hour cyclic perfusion.The structural and ultrastructural changes in the donor lung were examined before and after transplantation.Terminal deoxynucleotidyl transferase-uridine nucleotide end-labeling(TUNEL-Fluorescein)and propidium iodide(PI)double staining techniques were used to detect apoptosis of donor lung before and after transplantation.Results The donor lung in group B had significantly better oxygenation capacity during 60 minutes of reperfusion(PaO2>112.33mmHg).PaO2 of group B was significantly higher than those of group A after 30 minutes of reperfusion(54.42-118.87mmHg vs 112.33-183.73mmHg,P<0.01).PaCO2 in group A was significantly higher than that in group B(50.96mmHg vs 41.42mmHg,P<0.01).PaP and PawP in group A were higher than that in group B after 20min and 30min of reperfusion(P<0.001).CL of group B was higher than that of group A(0.156 vs 0.120cm3·cmH2O-1,P<0.01).Electron microscopy revealed that ultrastructure of donor lung in group B was well preserved after 60min of reperfusion.TUNEL-Fluorescein and PI double staining revealed that almost no evidence of apoptosis was found in specimens after cold ischemic periods.Significant increase in the numbers of cells undergoing apoptosis were observed after graft reperfusion.And there were significantly more apoptotic cells in groups A(24.67%)compared with groups B(15.50%)after transplantation(P<0.01).The percentage of apoptosis cells were inversely correlated with post-transplant donor lung function(r=-0.895,P<0.001).Conclusion The addition of aprotinin to LPD solution may improve donor lung function by inhibiting apoptosis in the early reperfusion period.The apoptosis might contribute to ischemia-reperfusion injury during the early phase of graft reperfusion and be responsible for severe organ dysfunction.

Abstract:Background: Ischemia-reperfusion injury related to lung transplantation is a major contributor to early postoperative morbidity and mortality. We hypothesized that donation after cardiac death donor lungs experience warm ischemic conditions that activate different injurious mechanisms compared with donor lungs that undergo prolonged cold ischemic conditions. Methods: Rat donor lungs were preserved under different cold ischemic times (CIT) (12 hours or 18 hours), or under warm ischemia time (WIT) (3 hours) after cardiac death, followed by single left lung transplantation. Lung function was analyzed during the 2-hour reperfusion period. Microscopic injury, cell death, energy status, and inflammatory responses were assessed. Results: Pulmonary oxygenation function was significantly worse in both 18hCIT and WIT groups, accompanied by higher peak airway pressure, acute lung injury scores, and expression of cell death markers compared with the 12hCIT control group. In lung tissue, reperfusion induced increased expression levels of interleukin (IL)-1α, IL-1β, IL-6, and chemokines CCL2, CCL3, CXCL1, and CXCL2 in CIT lungs. Notably, these changes were much lower in the WIT group. Additionally, plasma levels of IL-6, IL-18, CCL2, and vascular endothelial growth factor were significantly higher, and adenosine triphosphate levels were significantly reduced in warm versus cold ischemic lungs. Conclusions: Compared with 12hCIT, posttransplant pathophysiology deteriorated similarly in both 18hCIT and WIT groups. However, tissue adenosine triphosphate levels and inflammatory profiling differed between warm versus cold ischemic donor lungs. These differences should be carefully considered when developing specific therapeutic strategies to reduce ischemia-reperfusion injury in lung transplantation.

Abstract:OBJECTIVE To investigate A(1), A(2A), A(2B), and A(3) adenosine receptors in lymphocytes and neutrophils from patients with early rheumatoid arthritis (ERA) as well as from RA patients treated with methotrexate (MTX) or anti-tumor necrosis factor alpha (anti-TNFalpha), as compared with those in age-matched healthy controls, and to examine correlations between the status and functionality of adenosine receptors and TNFalpha release and NF-kappaB activation. METHODS Adenosine receptors were analyzed by saturation binding assays and Western blot analyses. We investigated the potency of typical A(2A) and A(3) agonists in the production of cAMP in control subjects, ERA patients, and RA patients treated with MTX or anti-TNFalpha. In a separate cohort of RA patients, TNFalpha release and NF-kappaB activation were evaluated in plasma and nuclear extracts, respectively. RESULTS In ERA patients, we found a high density and altered functionality of A(2A) and A(3) receptors. The binding and functional parameters of A(2A) and A(3) receptors normalized after anti-TNFalpha, but not MTX, treatment. TNFalpha release was increased in ERA patients and in MTX-treated RA patients, whereas in anti-TNFalpha-treated RA patients, release was comparable to that in the controls. NF-kappaB activation was elevated in ERA patients and in MTX-treated RA patients. Anti-TNFalpha treatment mediated decreased levels of NF-kappaB activation. CONCLUSION A(2A) and A(3) receptor up-regulation in ERA patients and in MTX-treated RA patients was associated with high levels of TNFalpha and NF-kappaB activation. Treatment with anti-TNFalpha normalized A(2A) and A(3) receptor expression and functionality. This new evidence of A(2A) and A(3) receptor involvement opens the possibility of exploiting their potential role in human diseases characterized by a marked inflammatory component.
Acinar Cell Apoptosis Induced by Early Ischemia/reperfusion Injury in Miniature Swine Pancreas Transplantation

Abstract:Objective By establishing a model of swine pancreas transplantation, we sought to clarify the occurrence of apoptosis of pancreatic acinar cell during early ischemia/reperfusion injury.Methods Experiments of pancreas transplantation with enteric drainage were performed in 20 miniature swine. Biopsies were taken from the pancreatic tail before perfusion in situ, after pancreas removal, 2h after cold ischemia and 1h after reperfusion. Target parameters were measured, including apoptosis detected by TUNEL, morphological damage examined by light and transmission electron microscopy, immunohistochemistry for Bcl-2 and Bax protein. Blood samples were taken exactly 0 min, 30 min, and 1h after reperfusion for measuring the serum amylase and lipase.Results The pancreatic tissue taken after pancreas removal, 2h after cold ischemia, 1h after reperfusion versus before perfusion in situ showed strikingly increasing numbers of apoptotic cells quantified by AI(P0.01), significantly higher damage scores (P0.01), significantly decreasing levels of expression of Bcl-2 (P0.01) and increasing expression of Bax (P0.01). The levels of serum amylase and lipase significantly increased 30 min, 1h after reperfusion (P0.01). Conclusion Apoptosis of pancreatic cells happened during the process of pancreas transplantation may be one of the specific features of early ischemia/reperfusion injury. Antiapoptosis treatment should be emphasized for preventing ischemia/reperfusion injury in pancreas transplantation.
Ischemia-Reperfusion Injury in a Simulated Lung Transplant Setting Differentially Regulates Transcriptomic Profiles between Human Lung Endothelial and Epithelial Cells

Abstract:Current understanding of mechanisms of ischemia-reperfusion-induced lung injury during lung preservation and transplantation is mainly based on clinical observations and animal studies. Herein, we used cell and systems biology approaches to explore these mechanisms at transcriptomics levels, especially by focusing on the differences between human lung endothelial and epithelial cells, which are crucial for maintaining essential lung structure and function. Human pulmonary microvascular endothelial cells and human lung epithelial cells were cultured to confluent, subjected to different cold ischemic times (CIT) to mimic static cold storage with preservation solution, and then subjected to warm reperfusion with a serum containing culture medium to simulate lung transplantation. Cell morphology, viability, and transcriptomic profiles were studied. Ischemia-reperfusion injury induced a CIT time-dependent cell death, which was associated with dramatic changes in gene expression. Under normal control conditions, endothelial cells showed gene clusters enriched in the vascular process and inflammation, while epithelial cells showed gene clusters enriched in protein biosynthesis and metabolism. CIT 6 h alone or after reperfusion had little effect on these phenotypic characteristics. After CIT 18 h, protein-biosynthesis-related gene clusters disappeared in epithelial cells; after reperfusion, metabolism-related gene clusters in epithelial cells and multiple gene clusters in the endothelial cells also disappeared. Human pulmonary endothelial and epithelial cells have distinct phenotypic transcriptomic signatures. Severe cellular injury reduces these gene expression signatures in a cell-type-dependent manner. Therapeutics that preserve these transcriptomic signatures may represent new treatment to prevent acute lung injury during lung transplantation.
Effects of prolonged cold-ischemia on autophagy in the graft lung in a rat orthotopic lung transplantation model

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Introduction</h3><p>Ischemia-reperfusion (I/R) injury causes present challenges in the field of graft transplantation which is also a major contributor to early graft dysfunction or failure after organ transplantation. The study focuses on the effects of prolonged cold-ischemia (CI) on the autophagic activity in the graft lung in a rat orthotopic lung transplantation model.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Material and methods</h3><p>Donor lungs were preserved under CI conditions for different periods. An orthotopic lung transplantation model was developed, and the lung tissues from donor lungs subjected to CI preservation and reperfusion were harvested. We evaluated the effects of different CI periods on autophagy, reactive oxygen species (ROS) and glucose consumption. Additionally, the mechanism by which prolonged CI affected autophagy was investigated through determination of the molecules related to the mTOR pathway after treatment with 3-Methyladenine (3-MA), rapamycin and an adenosine triphosphate (ATP) synthase inhibitor oligomycin (OM).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Prolonged CI led to increased activities of key glycolytic enzymes, glucose consumption and lactic acid production. Autophagy, ROS and glucose consumption were induced in the graft lung after I/R, which reached peak levels after 6 h and was gradually decreased. Most importantly, the perfusion treatment of 3-MA or OM decreased ROS level and autophagy, but increased the extent of mTOR phosphorylation, while the perfusion treatment of rapamycin induced ROS and autophagy.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Taken together, autophagy mediated by a prolonged CI preservation affects the glucose consumption and ROS production in the graft lung <em>via</em> the mTOR signaling pathway.</p>

Pulmonary cell death in warm ischemic rabbit lung is related to the alveolar oxygen reserve

Abstract:Background: If lungs could be retrieved for transplantation from non-heart-beating cadavers, the shortage of donors might be significantly alleviated. Methods: We studied the effect of different postmortem lung conditions on pulmonary cell death. Lungs from 208 New Zealand white rabbits were flushed with trypan blue vital dye solution at intervals after circulatory arrest, fixed, and mounted for histologic examination. Pulmonary cells were judged to be viable on the basis of their ability to exclude trypan blue dye. In the control group, lungs were excised immediately after death and immersed in cold (4 degrees C) saline solution. In the other groups, cadavers were left at room temperature with lungs deflated, ventilated with room air or 100% oxygen or 100% nitrogen, or inflated with room air or 100% oxygen. Results: There was a gradual increase in percentage (mean +/- SEM) of nonviable cells in the control group from 2.5%+/-0.9% (preischemic value) to 18.1%+/-2.8% at 24 hours after death (p < 0.001). In cadavers with lungs deflated, 79.7%+/-2.1% of cells were nonviable at 24 hours after circulatory arrest (p < 0.001 versus control group). In contrast, room air-ventilated cadavers showed only 21.4%+/-2.7% nonviable cells at this interval (p < 0.001 versus deflated group; not significant versus control group). Values in oxygen-ventilated animals were similar. Nitrogen-ventilated cadavers, however, had significantly more nonviable lung cells (73.8%+/-3.2%; p < 0.001 vs room air and oxygen-ventilated group, not significant vs deflated group). Oxygen-inflated lungs showed a parallel decrease in cell viability up to 4 hours after death when compared with room air-inflated cadaveric lungs, but thereafter more cells became nonviable in the latter group (11.1%+/-0.7% vs 19.6%+/-3.2% at 6 hours and 48.7%+/-7.2% vs 75.5%+/-4.6% at 24 hours, respectively; p < 0.01). Conclusions: Postmortem room air ventilation is as good as oxygen ventilation in delaying pulmonary cell death, and its effect is comparable to cold storage; nitrogen ventilation, however, is ineffective and not different from deflation; oxygen inflation will preserve ischemic cells for longer intervals as opposed to room air inflation. Therefore the alveolar oxygen reserve seems to be the critical factor to protect-the lung parenchyma from warm ischemic damage.
Altered purine metabolism at reperfusion affects clinical outcome in lung transplantation

Abstract:Introduction: Lung transplantation is an established treatment for patients with end-stage lung disease. However, ischaemia reperfusion injury remains a barrier to achieving better survival outcomes. Here, we aim to investigate the metabolomic and transcriptomic profiles in human lungs before and after reperfusion, to identify mechanisms relevant to clinical outcome. Methods: We analysed 67 paired human lung tissue samples collected from 2008 to 2011, at the end of cold preservation and 2 hours after reperfusion. Gene expression analysis was performed with R. Pathway analysis was conducted with Ingenuity Pathway Analysis. MetaboAnalyst and OmicsNet were used for metabolomics analysis and omics data integration, respectively. Association of identified metabolites with transplant outcome was investigated with Kaplan-Meier estimate and Cox proportional hazard models. Results: Activation of energy metabolism and reduced antioxidative biochemicals were found by metabolomics. Upregulation of genes related to cytokines and inflammatory mediators, together with major signalling pathways were revealed by transcriptomics. Purine metabolism was identified as the most significantly enriched pathway at reperfusion, based on integrative analysis of the two omics data sets. Elevated expression of purine nucleoside phosphorylase (PNP) could be attributed to activation of multiple transcriptional pathways. PNP catabolised reactions were evidenced by changes in related metabolites, especially decreased levels of inosine and increased levels of uric acid. Multivariable analyses showed significant association of inosine and uric acid levels with intensive care unit length of stay and ventilation time. Conclusion: Oxidative stress, especially through purine metabolism pathway, is a major metabolic event during reperfusion and may contribute to the ischaemia reperfusion injury of lung grafts.

Abstract:Apoptosis has been associated with several events in solid organ transplantation, including ischemia/reperfusion (IR) injury and acute rejection. To determine whether apoptosis-profiles may distinguish these two conditions, we analyzed apoptosis rates in a rat orthotopic small bowel transplant (SBT) model. SBT was performed in Lewis rats with either freshly harvested or preserved (4 h, in UW at 4°C) syngeneic and allogeneic (Brown-Norway) grafts. Bowel samples were collected 2 h after reperfusion and on small bowel transplant postoperative days (POD) 1, 4, and 7. Apoptosis was detected by measuring levels of histone-associated DNA fragments and caspase 3 expression, and by determining apoptotic body counts. All markers measured 2 h after reperfusion increased profoundly in association with preservation. After a significant decrease on POD 1, apoptosis rates rose again between POD 4 and 7 only in allogeneic grafts. Subnormothermic ex vivo lung perfusion attenuates graft inflammation in a rat transplant model

Abstract:Objective: Ex vivo lung perfusion has emerged as a novel technique to safely preserve lungs before transplantation. Recent studies have demonstrated an accumulation of inflammatory molecules in the perfusate during ex vivo lung perfusion. These proinflammatory molecules, including damage-associated molecular patterns and inflammatory cytokines, may contribute to acute and chronic allograft dysfunction. At present, ex vivo lung perfusion is performed clinically at normothermic temperature (37°C). The effect of lowering temperature to the subnormothermic range during ex vivo lung perfusion has not been reported. In this study, we hypothesized that lower ex vivo lung perfusion temperature will lead to a reduction in allograft inflammation and result in improved post-transplant graft function. Methods: Lewis rat heart-lung blocs underwent 4 hours of ex vivo lung perfusion in 3 temperature groups: 37°C (MP37), 30°C (MP30), and 25°C (MP25). In the control group, lung grafts were preserved by static cold storage before transplantation. After ex vivo lung perfusion or static cold storage, the left lung was transplanted for 2 hours before the animal was killed. Sera and tissue were collected and analyzed. Results: There were no differences in partial pressure of arterial oxygenation to fraction of inspired oxygen ratios during 4 hours of ex vivo lung perfusion between temperature groups. Tumor necrosis factor α significantly increased in the MP37 group during ex vivo lung perfusion, whereas this was not seen at lower temperatures. Extracellular DNA and high-mobility group box 1 perfusate concentrations increased significantly during ex vivo lung perfusion in all groups, but the rate of increase was diminished at lower temperature. Two hours post-transplant, there were no significant differences in partial pressure of arterial oxygenation to fraction of inspired oxygen ratios of the lung graft or serum damage-associated molecular pattern levels among groups. On histologic grading after transplantation, greater injury was observed in the MP30 and MP37 groups, but not MP25, when compared with static cold storage. Conclusions: Subnormothermic ex vivo lung perfusion at 25°C reduces the production of inflammatory mediators during ex vivo lung perfusion and is associated with reduced histologic graft injury after transplantation.

Abstract:Leukocytes have been implicated in ischemia-reperfusion (IR) injury of the lung, but the individual role of T cells has not been explored. Recent evidence in mice suggests that T cells may play a role in IR injury. Using a syngeneic (Lewis to Lewis) rat lung transplant model, we observed that recipient CD4(+) T cells infiltrated lung grafts within 1 h of reperfusion and up-regulated the expression of CD25 over the ensuing 12 h. Nude rats (rnu/rnu) and heterozygous rats (rnu/+) were used to determine the role of T cells in IR injury. No significant difference in lung function was observed between nude and heterozygous recipient rats after 2 h of reperfusion. However, after 12 h of reperfusion, recipient nude rats had significantly higher oxygenation and lower peak airway pressure than recipient heterozygous rats. This was associated with significantly lower levels of IFN-gamma in transplanted lung tissue of recipient nude rats. Reconstitution of recipient nude rats with T cells from heterozygous rats restored IR injury after 12 h of reperfusion. The effect of T cells was independent of neutrophil recruitment and activation in the transplanted lung. These results demonstrate that recipient T cells are activated and mediate IR injury during lung transplantation in rats.

Abstract:Papillary and surface micropuncture in Munich-Wistar rats was used to assess the role of proximal segments of superficial and juxtamedullary (JM) nephrons, the distal tubule of superficial nephrons, and the terminal collecting duct in acid excretion. The relative role of these segments in ammonium production, bicarbonate reclamation, and net acid formation was assessed under hydropenic conditions and after a chronic acid load. In these two settings the proximal segment of both kinds of nephrons is the major site of ammonium production and bicarbonate reclamation. However, this segment's contribution to net acid formation was only significant during acidosis. On the other hand, segments beyond the distal tubule appear to be the major site of acid formation. In situ pH measurements were lower in these nephron segments and fell even more after the induction of an acidosis. Ammonia appears to enter fluid between the end of the distal tubule and the base of the collecting duct. In vivo pH measurements made near the bend of Henle's loop of JM nephrons were more alkaline than near the end of the proximal tubule of superficial nephrons. It is postulated that this difference in pH allows ammonium to dissociate, permitting the movement of ammonia out of the tubule lumen and into collecting duct fluid where it is protonated and, therefore, reentrapped. This process is enhanced by the ingestion of a chronic acid load.

Abstract:BACKGROUND: The dynamic changes of pneumocyte apoptosis and aspartate-specific cysteine proteases-3 (caspase-3) expression in lung tissue of rats during the process of lung ischemia/reperfusion (I/R) injury and the possible action mechanisms remain unclear.OBJECTIVE: This study was to observe the dynamic changes of pneumocyte apoptosis and caspase-3 expression in the rat lung tissue during the process of lung I/R injury, and to analyze the role of pneumocyte apoptosis and the possible action mechanism.DESIGN: A randomized controlled animal experiment.SETTING: Emergency Center, First Hospital, Nanjing Medical University.MATERIALS: This study was carried out in the Animal Laboratory of the First Hospital of Nanjing Medcial University and Nanjing Center for Radioimmunity between April 2006 and September 2006. Twenty-eight male healthy SD rats of clean grade, with body weight of 250 to 350 g, aged 49 to 76 days, were provided by the Experimental Animal Center of Nanjing Medical University. The involved rats were randomized into experimental group and control group, with 14 rats in each.METHODS: ①Experimental intervention: Rats in the experimental group were created into models of lung I/R injury according to the method of Eppinger et al. They were occluded for 45 minutes at the porta of lung (no systolic and diastolic reactions in lung tissue being considered as successful occlusion), and then they were reperfused (recovery of systolic and diastolic function being considered as successful reperfusion); After that, lung tissues were harvested at 3 and 6 hours after lung I/R injury, 7 rats at each time point. Each rat in the control group was subjected to a thoracotony only, but lung tissues were isolated at the same time point by the same method. ②Experimental evaluation: Apoptotic cells in the lung tissue were detected with a flow cytometer by Annexin-V-PI staining, and apoptosis rate was calculated. Caspase-3 expression in the lung tissue was observed by immunohistochemical method and image analysis. Wet to dry weight ratio(W/D) of lung tissue of rats in the two groups was calculated; the number of injured pulmonary alveoli at I/R 3 hours/that at I/R 6 hours was calculated for quantitative evaluation of injured lung tissue; Patho-morphological changes of lung tissue were observed by haematoxylin & eosin staining under an optical microscope.MAIN OUTCOME MEASURES: ①Pneumocyte apoptosis rate and caspase-3 expression in the lung tissue. ②W/D of lung tissue and quantitative evaluation of injured lung tissue. ③Patho-morphological changes of lung tissue.RESULTS: Twenty-eight rats were involved in the final analysis, without deletion. ①Pneumocyte apoptosis rates in the experimental group at I/R 3 and 6 hours were significantly increased as compared with control group (P＜0.01). In the experimental group, pneumocyte apoptosis rate was decreased a little at I/R 6 hours than at I/R 3 hours (P＜0.05). ②Caspase-3 expression in the lung tissue of rats of experimental group reached its top at I/R 3 hours, and was decreased a little at I/R 6 hours. At each time point, caspase-3 expression in the experimental group was increased as compared with control group (P＜0.01). ③In the experimental group, the number of injured pulmonary alveoli at I/R 3 hours/that at I/R 6 hours and W/D ratios of lung tissues were significantly increased as compared with control group (P＜0.01). In the experimental group, two ratios at I/R 6 hours were higher than those at I/R 3 hours (P＜0.05).④In the experimental group, the structure of pulmonary alveoli was destructed, collapsed and disappeared; lots of inflammatory cell infiltration was found; Patho-morphological changes of injured lung tissue at I/R 6 hours were severer than those at I/R 3 hours. No obvious changes were found in the control group.CONCLUSION: At the early stage of lung I/R injury, the alteration of caspase-3 maybe activate pneumocyte apoptosis and induce the apoptosis of lung tissue, and thereby leads to lung injury.
