Abstract:A versatile, high-temperature superconducting (HTS) cable test facility has been designed and assembled at Oak Ridge National Laboratory (ORNL) in a joint program between ORNL and Southwire Company. The facility mission is to evaluate the performance of prototype HTS power transmission cables at lengths in the range of 1 to 5+ m. Power supplies are available for steady-state currents of 3000 A (DC) and 2000 A (AC) as well as up to 25000 A (DC) for pulsed loading. Cryogenic cooling is provided by an Air Products liquid nitrogen supply system with the capability of providing about a kilowatt of cooling at pressures up to 10 bar over a temperature range of about 67 to 77 K. Electrical and cryogenic data are taken by dedicated sensors scanned by ten-channel multimeters connected via cable and fiber optic to a personal-computer-based, data acquisition system using the LabVIEW program. These diagnostics allow measurement of the DC V-I characteristics and AC loss of the cable, dielectric integrity via a partial discharge measurement, and cryogenic performance at rated voltage (7.2-kV AC) and current (1250 A). Initial results from facility commissioning and early HTS cable testing are reported,.
Change in Social Participation of Adults with Spinal Cord Injury During the First Two Waves of the COVID-19 Pandemic in Canada

Abstract:Introduction: The change in environmental and social context measures during the COVID-19 pandemic affected daily activities of people with spinal cord injury (SCI), their inter-actions within the community, and consequently their social participation during the first wave of the pandemic. However, there is little information about the changes in social participation as the pandemic evolved in Canada. Objective: To describe the change in social participation of adults with SCI after the first two years of the COVID-19 pandemic in Canada. Methods: A follow-up from a previous study exploring the social participation of adults with SCI living during the first wave was conducted eight months later (second wave). Social participation was measured using the Assessment of Life Habits (LIFE-H 4.0) and Measure of Quality of the Environment (MQE)) among 18 adults with SCI. Results: Participants reported increases between both waves of COVID-19 in life habits categories, including mobility, personal care and health, nutrition, and recreation. New environmental factors were identified as facilitators, including the increased availability of businesses in the community. Conclusion: These findings suggest that people with SCI increased fulfillment and satisfaction of certain life habits. Reduced environmental barriers and increased facilitators improved social participation as the pandemic evolved.

Impact of health problems secondary to SCI one and five years after first inpatient rehabilitation

Abstract:Study design: This is a multicenter prospective cohort study. Objectives: The objective of this study was to describe and compare the impact of health problems secondary to spinal cord injury (SCI) on functioning at home and on social activities at 1 and 5 years after discharge from first inpatient rehabilitation. Setting: The study was conducted in a Dutch community. Methods: Participants with SCI who use a wheelchair for everyday mobility (N=110) completed a self-report questionnaire as part of a larger cohort study including four items on extra time needed (body care, bladder and bowel regulation, 'organization' and transportation) and impact of 10 health problems on functioning at home and on social activities. The 10 health problems include secondary health conditions (bladder regulation, bowel regulation, decubitus, pain, spasticity, gain in body weight and edema), psychosocial problems (sexuality, having difficulty with being dependent on help from others) and handicap management. Results: Median extra time needed for self-management and transportation was not significantly higher 1 year after discharge (16 (IQR 13.5) h per week) compared with 5 years after discharge (13 (IQR 17) h per week) (P=0.925). Participants reported slightly less impact, comparing the severity sum-score (range 10-50) of the 10 health problems on functioning at home and in social activities, 5 years post discharge (20 and 17, respectively) than 1 year post discharge (21 and 18, respectively; P<0.05). Most frequently mentioned health problems were handicap management, being dependent on help from others, bladder regulation, bowel regulation, pain and sexuality. Conclusions: The impact of health problems after SCI is considerable and hardly diminishes over time. These results emphasize the need for structured long-term care for people with SCI.

Abstract:OBJECTIVE: (1) to assess prevalence and severity of socio-ecological barriers to leisure time physical activity (LTPA) in a sample of adults with spinal cord injury (SCI); (2) to examine the association of these barriers with sociodemographic characteristics and functional independence (FI); and (3) to explore which socio-ecological levels of barriers might be associated with LTPA.DESIGN: Cross-sectional study.SETTING: Neurorehabilitation Hospital specialized in SCI.PARTICIPANTS: 207 wheelchair users with SCI living in the community who attended a comprehensive check-up (22,7% female, 47.5 ± 10.7 mean age).INTERVENTIONS: Not applicable.MAIN OUTCOME MEASURES: The short version of the Barriers to Physical Activity Questionnaire for People with Mobility Impairments was used to measure socio-ecological barriers to LTPA. The Spinal Cord Independence Measure III was used to measure functional independence. The Physical Activity Recall Assessment for People with SCI was used to quantify LTPA. Several multiple linear regression models were computed to examine the associations between socio-ecological barriers and sociodemographic factors, LTPA and FI.RESULTS: A high prevalence of barriers to LTPA was found. Seven of these barriers (two intrapersonal, three organizational and two community) were present for >60% of the participants. Intrapersonal and organizational barriers had a higher impact on participants with lower FI and women. Interpersonal barriers were higher for older participants and with lower FI, while community barriers were higher for unemployed participants. Finally, intrapersonal and interpersonal barriers were negatively associated with LTPA, and FI revealed as a moderator of the intrapersonal barriers-LTPA relationship.CONCLUSIONS: Given their high prevalence and their association with LTPA, the development of interventions targeting socio-ecological barriers to LTPA in people with SCI becomes crucial. The associations of these barriers with FI and sociodemographic characteristics should also be considered for these interventions to be as specific and effective as possible.

Abstract:Improvements in care and rehabilitation have resulted in a higher proportion of people living with spinal cord injury (SCI), which calls for an increased focus on participation and autonomy. This observational cross-sectional study investigated the impact of SCI on autonomy and how it correlates to activity performance and upper extremity functioning. A total of 25 adults (mean age 58 years) with chronic cervical or thoracic SCI were included. Self-perceived autonomy was measured with Impact on Participation and Autonomy questionnaire, independence in activities of daily living (ADL) with Spinal Cord Independence Measure, upper extremity functioning with Action Research Arm Test (ARAT) and kinematic measures of the drinking task. The results showed that most participants perceived injury-related restrictions in outdoor autonomy (80%), family role (76%), and in indoor autonomy (72%). Independence in self-care (r = 0.72), mobility (r = 0.59) and upper extremity kinematics of movement time (r = 0.63) and smoothness (r = 0.49) were correlated to indoors autonomy. Social life autonomy was correlated to self-care (r = 0.50) and ARAT (r = 0.41). In conclusion, autonomy was perceived restricted after SCI in several major life areas and correlated with independence in ADL and upper extremity functioning. The aspects of autonomy should be considered more in goal setting and clinical decision-making.

Abstract:Background Regular leisure time physical activity (LTPA) has beneficial health effects in people with spinal cord injury (SCI). Yet, participation in LTPA is low, and little is known about changes many years after injury. Objectives To determine changes in LTPA in middle-aged and older adults with long-term SCI over six years, investigate associations with gender, age, injury characteristics and changes in secondary health conditions and activity limitations, and investigate factors related to being physically active or sedentary. Methods Part of the Swedish Aging with Spinal Cord Injury Study (SASCIS). LTPA was assessed twice over a six-year period with the Physical Activity Recall Assessment for people with Spinal Cord Injury (n=75; 32% women, mean age 67 years, mean time since injury 31 years, injury levels C1-L3, AIS A-D). Changes were assessed with paired t-tests, McNemar test and Wilcoxon signed-rank test, associations with multivariable regression and group comparisons with Mann-Whitney U-test and Chi Square test. Results On group level participation in LTPA did not change, but the variability was substantial on individual level. There were no significant associations between changes in LTPA and the investigated variables. Among sedentary participants, higher level and severity of injury were overrepresented and the activity limitations were greater. Conclusions Our findings indicate stability in LTPA over time, but with a large individual variation. In many participants LTPA was insufficient to reach positive health effects. Promoting participation in LTPA is therefore an important part of the long-term management of middle-aged and older adults aging with long-term SCI.

Abstract:Study design: Cohort study. Objective: To identify five-year longitudinal changes in employment, health, participation, and quality-of-life outcomes (QOL) among participants with chronic spinal cord injury (SCI) and to compare the amount of change in these outcomes between those surviving and those not surviving until follow-up. Methods: Participants were 1157 individuals from the SCI Longitudinal Aging Study, who have completed at least two self-report assessments separated by five-year intervals. The main outcome measures were 13 indicators related to employment, health, participation, and QOL/psychosocial indicators. Survival status measured at follow-up. Results: Those who survived to follow up had a history indicating a greater likelihood of employment, better health, participation, and QOL/psychosocial indicators. Among survivors, longitudinal declines were limited to the percent employed and participation indicators, whereas those deceased by follow-up had significant undesirable changes in employment, participation, health, and QOL/psychosocial indicators. More specifically, compared to the survivors, those deceased by follow-up experienced a greater increase in hospitalizations, decreases in nights away from home, and declines in global satisfaction over the five-year interval. Conclusions: Longitudinal declines in employment and some aspects of participation are common among long-term survivors and may be part of the natural course of outcomes after SCI. However, more dramatic increases in hospitalizations, fewer nights away from home, and declining satisfaction may be red flags for declining longevity.

Abstract:Background: Social and community participation are major indicators to assess the adequacy of treatment and rehabilitation in patients with spinal cord injury (SCI). This study examined the relationship between functional independence, level of disability, and social and community participation among people with SCI in India. Materials and Method: In this cross sectional study, 110 persons with SCI, aged 18 years and above participated in a community setting, in Karnataka, India. Spinal Cord Independence Measure Version III-self-reporting (SCIM III-SR), Craig Handicap Assessment and Reporting Technique (CHART), and WHO Disability Assessment Schedule 2.0 (WHODAS) were the clinical outcome measures. Spearman’s correlation and stepwise multiple linear regression were done to determine association and identify the factors determining the community participation of people with SCI. Results: CHART physical independence had a positive correlation with SCIM self-care (R = 0.446) and SCIM mobility (r = 0.434). CHART cognitive independence (R = -0.38) and CHART mobility (R = -0.396) had a weak correlation with WHODAS. SCIM self-care and SCIM mobility (R 2 = 0.34) were determinants of CHART cognitive independence. SCIM respiratory and sphincter management and SCIM self-care (R 2 = 0.327) were determinants of CHART mobility. Conclusion: Self-care and mobility of people with SCI determine their ability to successfully reintegrate into the community, warranting a comprehensive community rehabilitation program.
Time Since Injury Is Key to Modeling Trends in Aging and Overall Functioning of Persons With Spinal Cord Injury: A Longitudinal Analysis of a Swiss Community Survey

Abstract:Objective: The aim of the study is to model the health indicator of functioning for persons with spinal cord injury or disorder living in Switzerland in the long-term context, considering chronological age, time since injury, and age at injury. Design: This is a longitudinal analysis of three waves of a community survey. An interval-scaled functioning metric was constructed using modern test theory. Random effects within-between mixed models were used to investigate trends in functioning over time. Results: Persons with complete injuries have lower levels of functioning than persons with incomplete injuries, and persons with complete tetraplegia have the lowest levels of functioning. The person's age when injured, the level and completeness of the injury, the presence of secondary health conditions, and various combinations of these factors significantly influenced how functioning changed over time. Conclusions: Our study shows that chronological age is not necessarily the best time variable for understanding the aging process of people with spinal cord injury in terms of changes in functioning over time and that modeling functioning over time since injury provides important insights into how functioning of people with complete or incomplete injuries declines with increasing time living with the injury.
Iron homeostasis after blood transfusion in stable preterm infants – an observational study

Abstract:Abstract Aim: To evaluate the short-term effects of blood transfusion on iron status [hemoglobin, ferritin, soluble transferrin receptor (sTfR), and reticulocyte count], hepcidin, and erythropoietin in stable preterm infants. Method: Sixty-three preterm infants treated with red blood cell transfusions (RBCTs) were included. Venous blood samples were collected before and within 24 h after each transfusion. Results: Hemoglobin concentration increased after RBCT (7.2±1.2 g/dL vs. 13.7±2.3 g/dL, P=0.02), as well as ferritin [131 (63–110.4) ng/mL vs. 211 (125.7–299.2) ng/mL, P=0.05); reticulocyte count decreased. sTfR did not change. Hepcidin serum levels increased from 37.5 (21.3–84.7) ng/mL to 72.6 (31.3–126.2) ng/mL, (P=0.04) and erythropoietin decreased (48±19 pg/mL vs. 29±17 pg/mL, P=0.06) after RBCT. A positive linear correlation was found (R2=0.76, P=0.0001) between hepcidin and ferritin levels of post-minus-pre RBCT. Hepcidin levels increased significantly in preterm infants who received RBCT after 1 month of age compared to those who received RBCT at <1 month (P=0.03). No correlation was found between gestational age, weight appropriate for age, or length of blood storage and hepcidin levels. Conclusion: Preterm infants can control iron levels by regulating hepcidin and decreasing erythropoietin. This ability varies with postnatal age.

Abstract:I read with great interest the recent study by Escorpizo et al. published in Spinal Cord [1], which investigates the connection between employment status, quality of life (QoL), and self-perceived health (SPH) in individuals with spinal cord injury (SCI). While the study provides valuable insights, there are specific aspects that could benefit from further attention and exploration by the authors. Firstly, the presentation of the main results poses challenges for readers in terms of interpretation. Path analysis was conducted separately for each quartile, categorized based on a country's gross domestic product (GDP). Within each path, variables such as the independent variable, dependent variables, and covariates were examined. The independent variable examined was "employment status", while the dependent variables encompassed QoL and SPH. Covariates included multiple factors such as age, gender, education, married/partner status, years since SCI, tetraplegia, complete SCI, and years unemployed after SCI. The findings indicated a positive association between employment and favorable ratings for both QoL and SPH. Conversely, being employed reduced the likelihood of reporting poor outcomes, as reflected by negative coefficients. However, it is important to note that these associations did not achieve statistical significance across all models. Additionally, covariates with direct effects on QoL and SPH were randomly distributed across the GDP quartiles. The complexity of multiple variables and paths contributed to the difficulty in interpreting the results. Upon examination of Figure 1 itself [1], readers may encounter challenges in discerning the intended message conveyed by the figure.

Abstract:In the proposed system for naming organic compounds, the structure to be named is divided into homogeneous segments made of ring systems, heteroatoms, or acyclic carbon chains which, together with their substituents, are then numbered and named, independently of each other, by current organic nomenclature. The set of names of segments makes the segment name of the structure. To select the direction of citation in segment names, seniority rules for all the types of segments are used. The segment nomenclature obviates selection of parent structure and principal group, ambiguous numbering of acyclic carbon chains, and use of a hierarchy of enclosing marks. The segment nomenclature generates unique names which are simpler and more comprehensible than current names, especially for complex structures containing besides acyclic carbon chains several ring systems and/or heteroatoms. The perceived benefits and barriers of sport in spinal cord injured individuals: a qualitative study

Abstract:Purpose: This study explores what individuals with spinal cord injury (SCI) perceive to be the benefits of becoming involved in organized sport and identifies the barriers to participation. Method: Seven SCI athletes from a variety of sports were interviewed in a semi-structured format. Results: 20 benefits and 18 individual barriers were identified and categorized into sub-themes: benefits were categorized into sub-themes of "socialization," "self-worth," "physical challenge" and "emotional." Barriers were grouped into "organization," "medical," "emotional," "a lack of available information" and "views held by others." Conclusion: A variety of benefits from participating in sport were identified, including socialization, the acquisition of knowledge from others, the development of greater awareness of health and well-being issues, weight maintenance, functional development and independence. Barriers identified included financial constraints, the lack of information regarding medical complications and sporting opportunities, and the need for able-bodied support. The findings have implications for the rehabilitation of individuals with SCI.
Experienced inclusion and recognition amongst people with a spinal cord injury: A comparative study in Norway, The Netherlands, and Australia

Abstract:Background The aim of this article is to study inclusion and recognition experienced amongst people with a Spinal Cord Injury (SCI) in Norway, The Netherlands, and Australia. This is approached both from the perspectives of an interest in the impact of mobility limitations versus social attitudes, and from a consideration of differences between societies. Methods The data derive from the core questionnaire of International Spinal Cord Injury Community Survey with extended national modules on the attitudes and values of respondents from Norway, The Netherlands, and Australia. The data gathered in 2017-18 include 2,450 participants aged 18 years or older. The data are analysed and presented with descriptive statistics and OLS regression analyses. In order to explore our main questions, we run regression controlling for country effects in addition exploring within country effects. Results Mobility limitations are a substantially weaker predictor of self-perceived inclusion and recognition than experiences of negative attitudes towards disabled people. Stereotypical attitudes and norms in society are shown to have various impacts on inclusion in the three countries. The Norwegian respondents report overall better results on inclusion and recognition than respondents in Australia and The Netherlands, illustrating the importance of national contexts. Conclussions Challenges associated with inclusion, recognition, and respect after SCI need to be defined in a language broader than mobility limitations and stereotypical attitudes towards people with an SCI. The main road to both inclusion and recognition in society is primarily linked to job and educational status, in addition to family and friends. The results are of particular interest as measures to support reintegration into society, including a strengthening of labour market integration programs.

Psychosocial Consequences of Spinal Cord Injury: A Narrative Review

Abstract:Consequences of a spinal cord injury (SCI) entail much more than damage to the spinal cord. The lives of people with SCI, along with those around them, experience profound long-lasting changes in nearly every life domain. SCI is a physical (biological) injury that is inextricably combined with various psychological and social consequences. The objective of this review is to present psychosocial challenges following SCI through the biopsychosocial model, beginning with acknowledgement of the larger societal effects of ableism and stigma before addressing the many unique psychosocial aspects of living with SCI. Included in this review are qualitative studies and systematic reviews on current psychosocial outcomes and consequences. This paper attempts to structure this information by dividing it into the following sections: relationships and family; changes in finances and employment; issues related to the person's living situation; community reintegration; factors associated with mood and coping (e.g., depression, anxiety, substance use, and PTSD); self-harm behaviors (ranging from nonadherence to suicide); effects of traumatic brain injury; considerations regarding sexual health; aging with SCI; and concludes with a brief discussion about post-traumatic growth. Cultivating an understanding of the unique and interrelated psychosocial consequences of people living with SCI may help mitigate the psychosocial aftermath and serve as a reminder to providers to maintain a person-centered approach to care.
Are Social Relationships an Underestimated Resource for Mental Health in Persons Experiencing Physical Disability? Observational Evidence From 22 Countries

Abstract:Objectives: As advancing evidence on modifiable resources to support mental health in persons experiencing physical disabilities is of particular importance, we investigate whether structural and functional social relationships relate to mental health in people with spinal cord injury (SCI). Methods: Data from 12,330 participants of the International SCI community survey (InSCI) from 22 countries were analyzed. Structural (partnership status, living situation) and functional aspects of social relationships (belongingness, relationship satisfaction, problems with social interactions) were regressed on the SF-36 mental health index (MHI-5), stratified by countries and for the total sample using multilevel models. Results: Functional aspects of social relationships were consistently related to clinically relevant higher MHI-5 scores and lower risk of mental health disorders (MHI-5 >56). Structural social relationships were inconsistently associated with mental health in our sample. Conclusion: This study provides evidence that functional aspects of social relationships are important resources for mental health. Interventions to establish and maintain high quality relationships should be considered in public health interventions and rehabilitation programs to reduce long-term mental health problems in persons experiencing physical disabilities.

Abstract:This study aimed to identify self-care ability, quality of life (QOL), and their related factors in individuals with spinal cord injury (SCI). A cross-sectional design and a convenience sample of 152 individuals with SCI from four SCI units and clinics in Jordan were used. Modified Barthel Index of Activities of Daily Living and the World Health Organization Quality of Life-BREF instruments were used. About 55.3% of participants reported moderate dependence on others to support their self-care ability, 48.0% reported good QOL, 65.8% were satisfied with their health after SCI, and physical and environmental domains received the highest scores. Having incomplete injury or paraplegia significantly predicted higher self-care ability. Being male, having a higher level of education, incomplete injury, paraplegia, and no pressure injury significantly predicted higher QOL ( p < .000). Collaboration is needed among health institutions, families, and communities to improve self-care ability and QOL after SCI.

Cardiovascular Risk Factors Among Older Adults With Long-Term Spinal Cord Injury

Abstract:Background: Individuals with spinal cord injury (SCI) now live longer, which increases the risk of cardiovascular disease. Knowledge of cardiovascular risk factors amenable to intervention are therefore needed to support their healthy aging. Objective: To describe the occurrence of cardiovascular risk factors among older adults with long-term SCI and investigate the association with sociodemographics and injury characteristics. Design: Cross-sectional descriptive cohort study. Setting: Home settings. Participants: In total, 123 individuals (71% men, injury levels C1-L5, American Spinal Injury Association Impairment Scale A-D), mean age 63 years, mean time since injury 24 years. Methods: Data from the Swedish Aging with Spinal Cord Injury Study (SASCIS), collected through interviews and assessments during home visits and from medical records. Main outcome measures: Anthropometric measurements, blood pressure, fasting plasma glucose and blood lipids, and data on cardiovascular comorbidity and tobacco use. Results: One third had a previous diagnosis of hypertension, and 55% presented with a blood pressure ≥ 140/90 mm Hg at the time of assessment. Sixteen percent had a history of diabetes and in 15% fasting glucose levels were ≥ 7 mmol/L. Dyslipidemia was present in 76%, whereas 16% had prediagnosed dyslipidemia. Mean body mass index (BMI) was 27 kg/m2 and mean waist circumference was 101 cm. When SCI-adjusted BMI cut-off values were used, 93% were considered overweight (BMI ≥22 kg/m2 ), and 60% had a waist circumference associated with cardiometabolic risk. A total of 16% smoked regularly. The median number of cardiovascular risk factors was 3. No significant associations were found between the total number of risk factors and sociodemographics and injury characteristics. Conclusions: The high occurrence of cardiovascular risk factors among older adults with long-term SCI can pose additional consequences to their health. Regular assessments and interventions targeting cardiovascular risk in this population are therefore warranted. Further research is needed to identify modifiable factors associated with their risk profile. Level of evidence: III.
