X-RAY DIAGNOSIS OF THE SPINAL MOTOR SEGMENT INSTABILITY IN A PATIENT WITH OSTEOCHONDROSIS OF THE CERVICAL SPINE

Abstract:Introduction. In a healthy spine, all its elements, despite the strength of the connections between them, allow a person to perform various movements, which are flexion and rotation in the horizontal and vertical plane. The cervical spine in this regard has the greatest mobility. However, sometimes there comes a time when the spine loses its ability to hold itself in a certain position. This process occurs due to the fact that the vertebrae begin to slide slide relative to the other. In this case, the sliding direction can be any. This condition is called instability. Aim of the research. To study methods for diagnosing latent instability of the spinal motion segment in the cervical spine according to the data of the neurosurgical department of the Irkutsk Scientific Center for Surgery and Traumatology (ISCST). Materials and methods. Patient T., born on February 13, 1955, was admitted to the ISCST clinic, Neurosurgery Department. Admission diagnosis: widespread osteochondrosis with a predominant involvement of the cervical spine. Right vertebral artery syndrome. Chronic moderately severe cervivocranialgia syndrome (ICD M 42.1). Results. Based on the results of X-ray examinations, we have found that on the patient's functional spondylograms (on maximum flexion and maximum extension of the neck), an anterior displacement of the CIII body by 2 mm takes place; on extension, the posterior displacement of CIII body by 4 mm, with the instability index being 6.0 mm; and on spondylograms using the proposed method with weights, we have established that on flexion, the body of the CIII vertebra was displaced anteriorly by 3.0 mm; on maximum extension the body of the CIII vertebra was displaced posteriorly by 5.0 mm, the instability index was 8.0 mm. These changes were also confirmed using a mathematical formula. Conclusion. The proposed method for diagnosing instability of spinal motion segments in the cervical spine allows us to objectively identify hidden pathological mobility of the vertebrae and confirm the displacement using mathematical calculations. The developed diagnostic method makes it possible to timely determine treatment tactics and prevent the development of adverse events. The proposed mathematical calculation formula helps to clarify the indicator of instability of the spinal motion segment in the cervical spine and can be used in practical work.
