Abstract:Background Contemporary fixed orthodontic appliances are shifting from non‐customized pre‐adjusted appliances to custom‐designed and printed appliances with novel digital setup systems. We are one step closer to precision dentistry and orthodontics using personalized mechanics and custom appliances. However, despite the evidential enhancement and other improvements to fixed appliances, tooth movement is still limited to five degrees of freedom. Opening or closing spaces still requires manually placing elastomeric chains or coil springs. Aim In this article, we aimed to demonstrate how advancements in CAD/CAM technology, reverse engineering, and digital customization are helping orthodontics constantly evolve, enabling treatment with enhanced esthetics and minimal compliance. The clinical system (InBrace®, Irvine, CA) described in this article uses a patient‐specific, digitally designed multiloop NITI wire that delivers friction‐free, light, and continuous forces and activates automatically whenever the malocclusion deviates from the digital setup. Conclusion Through digital customization, InBrace allows for automated tooth movement in all six degrees of freedom, including space opening or closure, via programmed non‐sliding mechanics. Clinical Significance Precision orthodontics and personalized treatment have been significant developments in orthodontics recently. This article focuses on how a technologically advanced lingual appliance system could achieve targeted cosmetic results methodically via automation and personalization.

Innovations in Orthodontics: Skeletal Versus Dentoalveolar Expansion Utilizing Clear Aligners and TAD-Assisted Expanders

Abstract:Hypoplastic maxilla is a common skeletal anomaly that compromises function and esthetics. Beyond just a narrow-appearing smile, this abnormality presents significant restorative challenges in adult patients as it is often associated with crowding, compromised axial inclination of the teeth, lack of alveolar bone support, root proximity, and occlusal trauma.1 Recent research also confirms association of maxillary deficiency with nasal stenosis and a predisposition to compromised nasal airflow and pharyngeal collapse during sleep.2,3 Maxillary transverse skeletal deficiency is often but not always associated with posterior dental cross-bite. In most cases, maxillary posterior teeth are flared buccally and mandibular posterior teeth are excessively lingually inclined masking the underlying skeletal problem.4 Advances in 3D imaging in dentistry, namely ultra-low radiation cone-beam imaging technology, have significantly enhanced clinicians' ability to diagnose and subsequently treat a maxillary transverse deficiency.5.
Digital Planning to Enhance Diagnosis and Precision in Correcting Excessive Gingival Display in the Presence of Asymmetrical Maxillary Position: A Case Report

Abstract:This case report addresses the treatment of excessive gingival display (EGD) in the context of maxillary asymmetry, zenith irregularities, and occlusal plane inclination. Digital planning was pivotal in formulating a precise treatment strategy by incorporating facial photographs, digital models, and cone-beam computed tomography data. Parameters, including occlusal plane inclination, teeth position, and lip-to-gingival margin relationships, were considered to ensure treatment alignment with the patient's facial characteristics. Notably, during the planning phase, it was evident that the conventional approach using the cementoenamel junction as the apical limit for incisions would result in asymmetry. Consequently, the gingival margin position was defined in accordance with facial and lip features. The chosen treatment, flapless crown lengthening, was tailored to the patient's thin phenotype and guided by measurements derived from digital planning. Postsurgery, the patient experienced a swift and painless recovery. A harmonious smile with a stable gingival margin position was achieved at the 1-year follow-up, seamlessly complementing the patient's facial attributes. This case underscores the importance of personalized EGD treatment and the value of digital planning in enhancing diagnostic accuracy and precise treatment planning, ultimately facilitating optimal treatment strategies.
