Abstract:Objective To observe the feature of airway inflammation in patients troubled by gastro-oesophageal reflux (GER) with asthma-like symptoms. Methods Thirty-four cases of GER with asthma symptoms, 12 with asthma and 11 healthy subjects were enrolled in this study. Sputum was induced by hypertonic saline aerosol inhalation in all patients and healthy subjects, and the sputum differential cell counts were determined. The concentration of IL-8, -6, and -5 in the supernatant of induced sputum was measured by using enzyme linked immunosorbert assay (ELISA) respectively. Results The sputum eosinophilic percentage in GERDa patients was significantly decreased (P ＜0. 05 ) and the sputum neutrophil percentage was increased as compared with those in asthmatic patients ( P ＜ 0. 05 ). The concentrations of IL-5 (52.86 ±20.25, 55.69 ± 17.72, 70.56 ± 12.63), IL-6 (22. 14 ±9. 18, 18.62 ±9.94, 15.63 ±13.61 ) in the sputum supernatant in all patients were significantly increased as compared with healthy controls (P ＜ 0. 05), and the concentration of IL-5 in asthmatic patients were higher than in GERDa patients (P ＜0. 05). The concentration of IL-8 in GERD patients (308. 33 ± 178.60, 234. 28 ± 130. 98) was significantly increase as compared with healthy controls ( 143.26 ± 32.71, P ＜ 0. 05 ). There was no significant difference in the concentration of IL-8 between asthmatic patients and healthy controls ( P ＞ 0. 05 ).Conclusion Asthma and gastroesophageal reflux are both characterized by airway inflammation. Differed with asthma obtained by airway eosinophilic inflammation and mainly elevated IL-5, the GER-triggered airway inflammation is characterized by a neutrophilic airway inflammation and mainly enhanced levels of IL-8. GER does aggravate pre-existing airway inflammation in asthmatic patients through related cytokines which is the linkage between them.

Abstract:OBJECTIVE:To investigate the characteristics of airway inflammation in severe asthma and the association with interleukin-17 (IL-17). METHODS:Sixteen patients with mild persistent asthma, 14 patients with moderate persistent asthma and 18 patients with severe persistent asthma, as well as 15 normal control subjects, were included in this study. At baseline, asthma symptom score was recorded, and lung function was measured. Induced sputum was obtained and processed for cell differential and the supernatant was assayed for the concentrations of IL-17 and IL-8 by sandwich enzyme-linked immunosorbent assay (ELISA), eosinophil cationic protein (ECP) by the immuno-CAP system, and neutrophil myeloperoxidase (MPO) by a colorimetric method, and the values were normalized to the protein content. Then 15 mild to moderate and 15 severe asthmatic patients received inhaled corticosteroid (Pulmicort, Astra-Zeneca) therapy for 4 weeks, and the above measurements were repeated. RESULTS:The percentage of sputum eosinophils and the level of ECP were significantly increased in mild [0.0670 +/- 0.0740, (155 +/- 91) x 10(-6) g/g protein], moderate [0.0830 +/- 0.0440, (180 +/- 83) x 10(-6) g/g protein] and severe [0.1240 +/- 0.1430, (191 +/- 87) x 10(-6) g/g protein] asthmatic patients, as compared to the normal controls [0.0000 +/- 0.0010, (44 +/- 25) x 10(-6) g/g protein, P < 0.01)]; but there was no significant difference among mild, moderate and severe asthma (P > 0.05). On the contrary, the percentage of sputum neutrophils was significantly increased in patients with severe asthma (0.589 +/- 0.203) as compared to patients with mild (0.455 +/- 0.154) and moderate (0.449 +/- 0.194) asthma and normal controls (0.313 +/- 0.134, P < 0.01). The level of MPO was also increased in severe asthma [(31 +/- 10 ) U/g protein] as compared to mild [(12 +/- 4) U/g protein] and moderate [(22 +/- 7) U/g protein] asthma and normal controls [(10 +/- 4) U/g protein, P < 0.01]. The sputum level of IL-17 in severe, moderate asthma [(264 +/- 137) x 10(-9) g/g protein, (172 +/- 65) x 10(-9) g/g protein] was significantly higher than those in mild asthma [(126 +/- 52) x 10(-9) g/g protein] and normal controls [(56 +/- 20) x 10(-9) g/g protein, P < 0.01]. As compared to the normal controls [(83 +/- 36) x 10(-9) g/g protein], the level of IL-8 in severe asthma [(531 +/- 321) x 10(-9) g/g protein] was significantly increased (P < 0.01), and those in mild [(410 +/- 181) x 10(-9) g/g protein] and moderate [(438 +/- 148) x 10(-9) g/g protein] asthma were also increased, although the difference was not statistically significant (P > 0.05). The level of IL-17 was correlated positively with IL-8, neutrophils and MPO (r = 0.525, 0.349, 0.602, all P < 0.01). After steroid therapy, the percentage of eosinophils, the levels of ECP, MPO, IL-17 and IL-8 decreased significantly in all patients combined, but the percentage of neutrophils remained unchanged (P > 0.05) and still significantly higher in severe asthmatic patients (0.642 +/- 0.157) as compared to mild and moderate asthmatic patients (0.394 +/- 0.147, P < 0.01). CONCLUSIONS:Airway neutrophilia is a feature of severe asthma. The IL-17/IL-8 pathway may be involved in the initial neutrophil influx into the airways.

Abstract:Asthma heterogeneity complicates the search for targeted treatment against airway inflammation and remodeling. We sought to investigate relations between eosinophilic inflammation, a phenotypic feature frequent in severe asthma, bronchial epithelial transcriptome, and functional and structural measures of airway remodeling. We compared epithelial gene expression, spirometry, airway cross-sectional geometry (computed tomography), reticular basement membrane thickness (histology), and blood and bronchoalveolar lavage (BAL) cytokines of n = 40 moderate to severe eosinophilic (EA) and non-eosinophilic asthma (NEA) patients distinguished by BAL eosinophilia. EA patients showed a similar extent of airway remodeling as NEA but had an increased expression of genes involved in the immune response and inflammation (e.g., KIR3DS1), reactive oxygen species generation (GYS2, ATPIF1), cell activation and proliferation (ANK3), cargo transporting (RAB4B, CPLX2), and tissue remodeling (FBLN1, SOX14, GSN), and a lower expression of genes involved in epithelial integrity (e.g., GJB1) and histone acetylation (SIN3A). Genes co-expressed in EA were involved in antiviral responses (e.g., ATP1B1), cell migration (EPS8L1, STOML3), cell adhesion (RAPH1), epithelial–mesenchymal transition (ASB3), and airway hyperreactivity and remodeling (FBN3, RECK), and several were linked to asthma in genome- (e.g., MRPL14, ASB3) or epigenome-wide association studies (CLC, GPI, SSCRB4, STRN4). Signaling pathways inferred from the co-expression pattern were associated with airway remodeling (e.g., TGF-β/Smad2/3, E2F/Rb, and Wnt/β-catenin).

Abstract:Background: Although airway inflammation plays a major role in the pathophysiology of asthma, quantitative markers of airway inflammation are limited in clinical practice.Objective: To determine if the levels of noninvasive markers of eosinophil-catalyzed oxidation, lipid peroxidation, and nitric oxide (NO) production are associated with asthma.Methods: Participants were enrolled from academic medical centers participating in the Severe Asthma Research Program. Clinical characteristics, laboratory data, pulmonary function tests, and the levels of the following noninvasive markers were obtained: urinary bromotyrosine (BrTyr), a marker of eosinophil-catalyzed oxidation, urinary F-2-isoprostanes (F-2-IsoPs), markers of lipid peroxidation, and exhaled NO, a marker of airway inflammation.Results: Fifty-seven asthmatic participants and 38 healthy participants were enrolled. BrTyr, F-2-IsoPs, and exhaled NO were each significantly increased in asthmatic participants versus controls (p < 0.01). An elevated level (greater than the median) of any marker was associated with a significant 3- to 6-fold greater odds of having asthma. Participants with two or more elevated marker levels showed an 18-fold greater odds of having asthma. Relationships were also noted with airflow obstruction and bronchodilator response.Conclusion: The findings from this pilot study indicate that urinary levels of BrTyr and F-2-IsoPs, in addition to exhaled NO levels, are associated with asthma.
Characteristics of Airway Inflammation in Patients with the Emphysema Phenotype of Chronic Obstructive Pulmonary Disease

Abstract:Objective To evaluate the characteristics of airway inflammation in patients with the emphysema phenotype of chronic obstructive pulmonary disease (COPD).Methods Fifty-two outpatients with stable COPD were examined.For each subject,the clinical data were collected,and HRCT scanning and lung function tests were performed.Sputum cells were counted and the supernants were processed for measurement of IL-6,IL-8,TNF-α,VEGF,MMP-9 and TIMP-1.The subjects were divided into 2 groups:an emphysema group (n =29) and a non-emphysema group (n =23) based on the HRCT emphysema index (EI).Results There was no difference between the 2 groups in sex,age,body mass index,time course of cough and wheeze,and smoking (all P ＞0.05).FEV1 ％pred,DLCO％pred,and DLCO/VA％ pred were significantly decreased in the emphysema group as compared to the nonemphysema group [(48.21± 13.44)％ vs (57.04± 12.61)％,P =0.019; (48.57± 16.76)％ vs (65.64±12.48)％,P=0.001;(59.52±15.33)％ vs (82.99±16.31) ％,P＜0.001],but RV/TLC and RV％pred were significantly increased in the emphysema group [(53.86± 10.98)％ vs (44.93 ±6.72)％,P =0.003;(129.95±18.88)％ vs (118.77±13.16)％,P =0.037].The number and the percentage of sputum eosinophils were higher in the emphysema group [0.05 (0.00,0.13) × 109/L vs 0.00(0.00,0.01) × 109/L,P =0.032;0.50(0.00,2.00)％ vs 0.00(0.00,0.25)％,P =0.033].Moreover,the percentage of sputum eosinophils was positively correlated with emphysema index (B =1.248,P =0.007).However,there was no difference between the 2 groups in the number or percentage of sputum neutrophils,lymphocytes and macrophages.The sputum level of TNF-αtended to be higher in the emphysema group as compared to the non emphysema group [221.72(100.00,351.57) ng/L vs 144.85(54.51,269.16) ng/L,P =0.063],but the levels of IL-8,IL-6,VEGF,MMP-9,TIMP-1 and the ratio of MMP 9/TIMP-1 were not different between groups.Conclusions Patients with the emphysema phenotype of COPD in this study tended to have higher airway eosinophils and TNF-α,suggesting a differenct inflammatory profile which needed to be further explored.

Abstract:BACKGROUND:Severe asthma is a heterogeneous condition, as shown by independent cluster analyses based on demographic, clinical, and inflammatory characteristics. A next step is to identify molecularly driven phenotypes using "omics" technologies. Molecular fingerprints of exhaled breath are associated with inflammation and can qualify as noninvasive assessment of severe asthma phenotypes. OBJECTIVES:We aimed (1) to identify severe asthma phenotypes using exhaled metabolomic fingerprints obtained from a composite of electronic noses (eNoses) and (2) to assess the stability of eNose-derived phenotypes in relation to within-patient clinical and inflammatory changes. METHODS:In this longitudinal multicenter study exhaled breath samples were taken from an unselected subset of adults with severe asthma from the U-BIOPRED cohort. Exhaled metabolites were analyzed centrally by using an assembly of eNoses. Unsupervised Ward clustering enhanced by similarity profile analysis together with K-means clustering was performed. For internal validation, partitioning around medoids and topological data analysis were applied. Samples at 12 to 18 months of prospective follow-up were used to assess longitudinal within-patient stability. RESULTS:Data were available for 78 subjects (age, 55 years [interquartile range, 45-64 years]; 41% male). Three eNose-driven clusters (n = 26/33/19) were revealed, showing differences in circulating eosinophil (P = .045) and neutrophil (P = .017) percentages and ratios of patients using oral corticosteroids (P = .035). Longitudinal within-patient cluster stability was associated with changes in sputum eosinophil percentages (P = .045). CONCLUSIONS:We have identified and followed up exhaled molecular phenotypes of severe asthma, which were associated with changing inflammatory profile and oral steroid use. This suggests that breath analysis can contribute to the management of severe asthma.
Small-Airway Function Variables in Spirometry, Fractional Exhaled Nitric Oxide, and Circulating Eosinophils Predicted Airway Hyperresponsiveness in Patients with Mild Asthma.

Abstract:Purpose: Patients with variable symptoms suggestive of asthma but with normal forced expiratory volume in 1 second (FEV1) often fail to be diagnosed without a bronchial provocation test, but the test is expensive, time-consuming, risky, and not readily available in all clinical settings. Patients and Methods: A cross-sectional study was performed in 692 patients with FEV1 >= 80% predicted; normal neutrophils and chest high-resolution computed tomography; and recurrent dyspnea, cough, wheeze, and chest tightness. Results: Compared with subjects negative for AHR (n=522), subjects positive for AHR (n=170) showed increased FENO values, peripheral eosinophils (EOS), and R5-R20; decreased FEV1, FEV1/Forced vital capacity (FVC), and forced expiratory flow (FEFs) (P <=.001 for all). Small-airway dysfunction was identified in 104 AHR(+) patients (61.17%), and 132 AHR(-) patients (25.29%) (P<0.001). The areas under the curve (AUCs) of variables used singly for an AHR diagnosis were lower than 0.77. Using joint models of FEF50%, FEF75%, or FEF25%-75% with FENO increased the AUCs to 0.845, 0.824, and 0.844, respectively, significantly higher than univariate AUCs (P < 0.001 for all). Patients who reported chest tightness (n=75) had lower FEFs than patients who did not (P<0.001 for all). In subjects with chest tightness, the combination of FEF50% or FEF25%-75% with EOS also increased the AUCs substantially, to 0.815 and 0.816, respectively (P < 0.001 for all versus the univariate AUCs). Conclusion: FENO combined with FEF50% and FEF25%-75% predict AHR in patients with normal FEV1. FEF25%-75%, FEF50%, or FEF25%-75% together with EOS also can potentially suggest asthma in patients with chest tightness.
[A Study of Airway Reactivity and Nature of Chronic Asthmatic Bronchitis].

Abstract:OBJECTIVE:To observe the airway reactivity with methacholine provocation and exercise challenge of chronic asthmatic bronchitis and to assess preliminarily the nature of chronic asthmatic bronchitis.METHODS:42 patients with bronchial asthma, 29 with simple chronic bronchitis and 27 with chronic asthmatic bronchitis were selected. The study protocol consisted of inhalation challenge with methacholine and exercise challenge. Count of blood eosinophil cells and level of blood total IgE were studied. Serum eosinophil cationic protein (ECP) concentration was measured in the asthmatic bronchitis group.RESULTS:The positive ratio to methacholine response was 77.8% in chronic asthmatic bronchitis group, whereas the ratios were 100.0% and 41.4% in bronchial asthma and simple chronic bronchitis groups respectively. According to the response to exercise challenge, patients with chronic asthmatic bronchitis were divided into positive and negative groups. The cumulative dose of methacholine producing a 20% fall in FEV(1) (PD(20)-FEV(1)) was different between the two groups. In the positive group (9 patients), none was negative with methacholine response and PD(20)-FEV(1) being similar to the bronchial asthma group. The blood total IgE and ECP levels were significantly higher in the positive than in the negative group.CONCLUSION:The characteristics of airway reactivity in chronic asthmatic bronchitis are midway between bronchial asthma and simple chronic bronchitis. The nature of chronic asthmatic bronchitis may be different in different patients.

Abstract:Rationale: Extracellular DNA (eDNA) and neutrophil extracellular traps (NETs) are implicated in multiple inflammatory diseases. NETs mediate inflammasome activation and IL-1β secretion from monocytes and cause airway epithelial cell injury, but the role of eDNA, NETs, and IL-1β in asthma is uncertain. Objectives: To characterize the role of activated neutrophils in severe asthma through measurement of NETs and inflammasome activation. Methods: We measured sputum eDNA in induced sputum from 399 patients with asthma in the Severe Asthma Research Program-3 and in 94 healthy control subjects. We subdivided subjects with asthma into eDNA-low and -high subgroups to compare outcomes of asthma severity and of neutrophil and inflammasome activation. We also examined if NETs cause airway epithelial cell damage that can be prevented by DNase. Measurements and Main Results: We found that 13% of the Severe Asthma Research Program-3 cohort is "eDNA-high," as defined by sputum eDNA concentrations above the upper 95th percentile value in health. Compared with eDNA-low patients with asthma, eDNA-high patients had lower Asthma Control Test scores, frequent history of chronic mucus hypersecretion, and frequent use of oral corticosteroids for maintenance of asthma control (all P values <0.05). Sputum eDNA in asthma was associated with airway neutrophilic inflammation, increases in soluble NET components, and increases in caspase 1 activity and IL-1β (all P values <0.001). In in vitro studies, NETs caused cytotoxicity in airway epithelial cells that was prevented by disruption of NETs with DNase. Conclusions: High extracellular DNA concentrations in sputum mark a subset of patients with more severe asthma who have NETs and markers of inflammasome activation in their airways.

Abstract:Background: Measurement of airway inflammation is an important step to determine phenotype of asthma and allergic rhinitis (AR). Objective: To assess the level of nitric oxide in exhaled air (FeNO), nasal fraction of nitric oxide (nasal NO), their relationship with clinical control and blood eosinophils in patients with steroid-naive mild and moderate asthma and AR. Methods: One hundred forty-seven patients (65 men), ages 26‐49.5 years (mean age, 32 years) with AR (n = 81) or AR and concomitant asthma (n = 46) and 20 healthy subjects were included in a single-center cohort study. All the patients underwent spirometry with reversibility test. Control of asthma and AR was assessed by using the Asthma Control Questionnaire and the visual analog scale, respectively. Levels of FeNO and nasal NO were measured by chemiluminescent analyzer, peripheral blood eosinophils were counted by automatic analyzer. Results: The FeNO level was significantly elevated in the patients with asthma and concomitant AR compared with the healthy subjects and was associated with control of both asthma and AR. There was no correlation between nasal NO and control of AR. Receiver operating characteristic analysis revealed that the level of eosinophils of 150 cells/μL may be a cutoff for lower airway eosinophilic inflammation. Blood eosinophils count was unable to distinguish eosinophilic and non-eosinophilic upper airway inflammation. Conclusion: We confirm that FeNO but not nasal NO is a marker of eosinophilic airway inflammation in patients with mild-moderate steroid-naive AR and concomitant asthma. A blood eosinophil level of ≥150 cells/μL may be a simple marker of eosinophilic airway inflammation in patients with asthma. However, its low specificity requires repeated measurements and use in combination with other biomarkers.

Abstract:Paired long‐read metagenomic sequencing and proteomics in sputum and nasal lavage identified a single dominant respiratory pathogen (H. influenzae more than M. catarrhalis and S. pneumoniae) in 23% of severe asthma sputum samples. Nasal and sputum microbiomes were highly discordant. H. influenzae load correlated with ECP, elastase and IL‐10 suggesting eosinophil activation and a potential immune evasion mechanism. Background The airway microbiome in severe asthma has not been characterised at species‐level by metagenomic sequencing, nor have the relationships between specific species and mucosal immune responses in 'type‐2 low', neutrophilic asthma been defined. We performed an integrated species‐level metagenomic data with inflammatory mediators to characterise prevalence of dominant potentially pathogenic organisms and host immune responses. Methods Sputum and nasal lavage samples were analysed using long‐read metagenomic sequencing with Nanopore and qPCR in two cross‐sectional adult severe asthma cohorts, Wessex (n = 66) and Oxford (n = 30). We integrated species‐level data with clinical parameters and 39 selected airway proteins measured by immunoassay and O‐link. Results The sputum microbiome in health and mild asthma displayed comparable microbial diversity. By contrast, 23% (19/81) of severe asthma microbiomes were dominated by a single respiratory pathogen, namely H. influenzae (n = 10), M. catarrhalis (n = 4), S. pneumoniae (n = 4) and P. aeruginosa (n = 1). Neutrophilic asthma was associated with H. influenzae, M. catarrhalis, S. pneumoniae and T. whipplei with elevated type‐1 cytokines and proteases; eosinophilic asthma with higher M. catarrhalis, but lower H. influenzae, and S. pneumoniae abundance. H. influenzae load correlated with Eosinophil Cationic Protein, elastase and IL‐10. R. mucilaginosa associated positively with IL‐6 and negatively with FGF. Bayesian network analysis also revealed close and distinct relationships of H. influenzae and M. catarrhalis with type‐1 airway inflammation. The microbiomes and cytokine milieu were distinct between upper and lower airways. Conclusions This species‐level integrated analysis reveals central, but distinct associations between potentially pathogenic bacteria and airways inflammation in severe asthma.

Abstract:Eosinophils play a key role in airway inflammation in many diseases, such as allergic and non-allergic asthma, chronic rhinosinusitis with nasal polyps, and chronic obstructive pulmonary disease. In these chronic disabling conditions, eosinophils contribute to tissue damage, repair, remodeling, and disease persistence through the production a variety of mediators. With the introduction of biological drugs for the treatment of these respiratory diseases, the classification of patients based on clinical characteristics (phenotype) and pathobiological mechanisms (endotype) has become mandatory. This need is particularly evident in severe asthma, where, despite the great scientific efforts to understand the immunological pathways underlying clinical phenotypes, the identification of specific biomarkers defining endotypes or predicting pharmacological response remains unsatisfied. In addition, a significant heterogeneity also exists among patients with other airway diseases. In this review, we describe some of the immunological differences in eosinophilic airway inflammation associated with severe asthma and other airway diseases and how these factors might influence the clinical presentation, with the aim of clarifying when eosinophils play a key pathogenic role and, therefore, represent the preferred therapeutic target.

Abstract:OBJECTIVE:To investigate the distribution of airway inflammation phenotype in patients with bronchial asthma (asthma), and to analyze clinical characteristics, inflammatory cytokines, pulmonary small vessels remodeling and small airway wall remodeling in patients with neutrophilic asthma.METHODS:Sixty-three patients with asthma were enrolled from January 2015 to December 2015 in Peking University Third Hospital. Clinical data including gender, age, body mass index (BMI), pulmonary function tests (PFTs), asthma control test (ACT) were recorded. All the patients underwent sputum induction. The cellular composition of the sputum was evaluatedand the concentration of active MMP-9 in the sputum tested. Blood routine tests were done and the concentration of IgE, periostin, and TGF-beta1 levels were measured in serum by enzyme-linked immunosorbent assay (ELISA). Small airway wall remodeling was measured in computed tomography (CT) scans, as the luminal diameter, luminal area, wall thickness and wall area % adjusted by body surface area (BSA) at the end of the 6th generation airway, in which the inner diameter was less than 2 mm. Small vascular alterations were measured by cross-sectional area (CSA), and the total vessel CSA < 5 mm2 was calculated using imaging software.RESULTS:The distributions of airway inflammatory phenotypes of the asthmatic patients were as follows: neutrophilic asthma (34.9%, 22/63), eosinophilic asthma (34.9%, 22/63), mixed granulocytic asthma (23.8%, 15/63), and paucigranulocytic asthma (6.3%, 4/63). The neutrophilic subtype patients had a significantly higher active MMP-9 level in sputum compared with the eosinophilic phenotypepatuents, as 179.1 (74.3, 395.5) vs. 50.5 (9.7, 225.8), P<0.05. Sputum neutrophil count was negatively correlated with FEV1%pred (r=-0.304,P<0.05), and positively correlated with active MMP-9 level in sputum (r=-0.304, P<0.05), and positive correlation trend with airway wall thickness (r=0.533, P=0.06). There was a significantly negative correlation of active MMP-9 level in sputum with FEV1%pred (r=-0.281, P<0.05), in positive correlation with small airway wall area (%)(r=0.612, P<0.05), and inpositive correlation trend with airway wall thickness (r=0.612, P=0.06). Neutrophils count in peripheral blood was positively correlated with neutrophil counts in sputum.CONCLUSION:Neutrophil count in airway is related to lung function in asthmatic patients. Neutrophils may accelerate small airway wall remodeling through the release of active MMP-9. Neutrophil count in peripheral blood is related to neutrophils count in sputum, which may be used as a substitute for evaluating inflammatory phenotype.

Abstract:Two recent articles by the same research group documented that patients with severe eosinophilic asthma exhibit an increased proportion of a subtype of eosinophils, namely CD62L low inflammatory eosinophils (iEos) and identified an intriguing correlation between such iEos and asthma control scores. Moreover, CD62L low iEos were reduced after treatment with the anti-IL-5 monoclonal antibody mepolizumab. In the future, we believe that eosinophil subtypes could represent a useful biomarker in severe eosinophilic asthma, helping clinicians characterize patient endotypes and monitoring the response to biological drugs. Patients with severe eosinophilic asthma (SEA) have an increased proportion of a subtype of eosinophils, CD62L low inflammatory eosinophils (iEos), which are reduced after mepolizumab treatment. iEos might represent a novel useful biomarker in SEA.

Abstract:OBJECTIVE: Eosinophilic inflammation in the respiratory tract is a hallmark of bronchial asthma. In naïve cases, the inflammatory profile is associated with disease severity and reactivity to inhaled corticosteroids (ICS). Sustained airway eosinophilia has been reported during ICS treatment. However, the immunological characteristics of these cases are not known and it is unclear if this situation contributes to asthma control. This study was performed to determine the answer of these questions.METHODS: To compare phenotypes of eosinophilic and non-eosinophilic asthma (EA and NEA, respectively) under ICS treatment, clinical data were obtained from asthmatic subjects (n = 22) and healthy controls (n = 10), and the leukocyte compositions of induced sputum and peripheral blood were determined. T lymphocyte profiles in systemic blood were assessed by flow cytometry.RESULTS: A higher frequency of emergency room visits was observed in the NEA group, which had a higher neutrophil count relative to the total inflammatory cell population in induced sputum than the EA group (59.5 versus 36.6%; p < 0.01). The fraction of helper T (Th)17 lymphocytes as well as the ratio of Th17 to regulatory T cells (Treg) in the peripheral blood was higher in the NEA than in the EA group (0.24 versus 0.13; p < 0.05).CONCLUSIONS: Th17 were more prevalent than Treg cells in the peripheral blood of NEA patients under ICS treatment, corresponding to neutrophil-dominant airway inflammation and a severe asthmatic phenotype. Thus, an imbalance in Th17/Treg may be associated with the pathogenesis of NEA in patients undergoing ICS treatment.

Abstract:Background: The release of various inflammatory mediators into the bronchial lumen is thought to reflect both the type and degree of airway inflammation, eosinophilic Th2, and Th9, or neutrophilic Th1, and Th17, in patients with asthma. Aims: We investigated whether cytokines and chemokines differed in sputum from subjects with more severe compared with milder asthma and whether unbiased factor analysis of cytokine and chemokine groupings indicates specific inflammatory pathways. Methods: Cell-free supernatants from induced sputum were obtained from subjects with a broad range of asthma severity (n = 158) and assessed using Milliplex® Cytokines/Chemokine kits I, II and III, measuring 75 individual proteins. Each cytokine, chemokine or growth factor concentration was examined for differences between asthma severity groups, for association with leucocyte counts, and by factor analysis. Results: Severe asthma subjects had 9 increased and 4 decreased proteins compared to mild asthma subjects and fewer differences compared to moderate asthma. Twenty-six mediators were significantly associated with an increasing single leucocyte type: 16 with neutrophils (3 interleukins [IL], 3 CC chemokines, 4 CXC chemokines, 4 growth factors, TNF-α and CX3CL1/Fractalkine); 5 with lymphocytes (IL-7, IL-16, IL-23, IFN-α2 and CCL4/MIP1β); IL-15 and CCL15/MIP1δ with macrophages; IL-5 with eosinophils; and IL-4 and TNFSF10/TRAIL with airway epithelial cells. Factor analysis grouped 43 cytokines, chemokines and growth factors which had no missing data onto the first 10 factors, containing mixes of Th1, Th2, Th9 and Th17 inflammatory and anti-inflammatory proteins. Conclusions: Sputum cytokines, chemokines and growth factors were increased in severe asthma, primarily with increased neutrophils. Factor analysis identified complex inflammatory protein interactions, suggesting airway inflammation in asthma is characterized by overlapping immune pathways. Thus, focus on a single specific inflammatory mediator or pathway may limit understanding the complexity of inflammation underlying airway changes in asthma and selection of appropriate therapy.

Abstract:INTRODUCTION: Eosinophils play an important regulatory and immunomodulatory role in airway mucosa and have antiparasitic and antiviral properties as well as pro-inflammatory effects that may also cause persistence of inflammation with tissue remodeling. The number of eosinophils and the detection of specific mediators in biological samples from, e.g., blood, nasal secretions, and bronchial fluid can serve as biomarkers that reflect the underlying pathophysiology of certain diseases, predict treatment success, and detect therapy effects.MATERIALS AND METHODS: A literature search was conducted to determine the immunologic basis, mode of action, clinical significance, and available evidence for therapeutic approaches using eosinophil-targeted monoclonal antibodies by searching Medline, Pubmed, and the national and international trial database (ClinicalTrials.gov) and guideline registries as well as the Cochrane Library. Human studies published on the topic in the period up to and including 10/2023 were considered.RESULTS: Based on the international literature and previous experience, the results are summarized, and recommendations are given.CONCLUSION: The important role of eosinophils in immunological processes in the airway mucosa is comprehensively analyzed and can serve as a basis for current and future treatment approaches.
