Abstract:BACKGROUNDAntibody-mediated rejection (AMR) constitutes an important cause of cardiac allograft loss; however, all current therapeutic strategies represent systemic applications with unsatisfactory efficacy. Previously, we successfully non-invasively detected C4d, a specific marker for AMR diagnosis, in allografts using C4d-targeted microbubbles (MBC4d). In this study, we extended this approach by incorporating nitric oxide (NO), as high NO levels manifest immunosuppressive and anti-thrombotic effects.METHODSWe designed novel MBC4d loaded with NO (NO-MBC4d). A rat model of AMR was established by pre-sensitization with skin transplantation. Contrast-enhanced ultrasound (CEUS) images were obtained and quantitatively analyzed following NO-MBC4d injection. Allograft survival and histologic features were analyzed to evaluate the therapeutic effect and underlying mechanism of NO-MBC4d toward AMR.RESULTSWe successfully obtained CEUS images following NO-MBC4d injection and demonstrated that the ultrasound signal intensity of the myocardial area and clearance time of NO-MBC4d both increased with increased C4d grade, thereby realizing non-invasive diagnosis of AMR. Furthermore, allograft survival was significantly prolonged, and rejection was obviously attenuated following NO-MBC4d injection through significant suppression of thrombosis and reduction of inflammatory cell infiltrates. Overall, the therapeutic efficacy was significantly improved in the NO-MBC4d group compared with the control NO-MB group, demonstrating that precise treatment could significantly improve the therapeutic efficacy compared with that afforded by systemic applications.CONCLUSIONSThis study presented a novel tool to provide simultaneous non-invasive diagnosis and precise treatment of AMR using NO-MBC4d CEUS imaging, which may be expected to provide a better option for recipients with AMR in clinic.
Identification of Candidate Biomarkers in Peripheral Blood for Cardiac Allograft Rejection Based on Bioinformatics Analysis

Abstract:BACKGROUND:Cardiac allograft rejection (AR) can cause graft dysfunction and even mortality, and an early noninvasive diagnosis of cardiac AR is required. This study aims to identify candidate biomarkers in peripheral blood for cardiac AR, which might benefit early diagnosis.MATERIAL AND METHODS:Gene expression profile (ID: GSE5967) of peripheral blood from cardiac allograft recipients was achieved from the Gene Expression Omnibus database, including 7 chips in rejection group, 7 chips in post-rejection group, and 7 chips in control group. After data preprocessing, limma package was used to screen the differentially expressed genes (DEGs) between these groups (|log2 fold change| ≥0.58, and p-value <0.05). Then, online software DAVID was utilized to study the pathways and functions involving these DEGs (p-value <0.05).RESULTS:Totally, 21 up-regulated and 16 down-regulated DEGs were identified between rejection and control groups, and up-regulated DEGs were mainly enriched in bio-functions about translation and ribosome. Furthermore, 3 up-regulated and 14 down-regulated DEGs were identified between post-rejection and control groups. The down-regulated DEGs in 2 contrast groups were mainly enriched in bio-functions about lipid biosynthesis and membrane.CONCLUSIONS:RPL7, RPL11, RPS23, RPS25, SCD5, CSF3R, and FPR1 were predicted as candidate biomarkers in peripheral blood for monitoring cardiac AR. The up-regulation of RPL7, RPS25, RPS23, and RPL11 might promote the translation of AR-related cytokines, and the down-regulation of SCD5, CSF3R, and FPR1 might reduce the stability of cell membrane, mediating cytokines secretion and the phagocytosis of macrophages. However, further studies are required to validate these predictions.

Abstract:Acute cardiac allograft rejection continues to be the cause of graft loss and contributes to the morbidity and mortality after cardiac transplantation. Repetitive endomyocardial biopsies are necessary to monitor the effects of immunosuppressants after cardiac transplantation. In this study, we investigate whether speckle tracking imaging (STI) is a valuable method in assessing acute cardiac rejection.Hearts from Brown Norway rats or Lewis rats were transplanted into other Brown Norway rats. Isografts and groups of allografts, either untreated or treated with cyclosporine A (CsA) at a low dose (3 mg x kg(-1) x d(-1)) or a high dose (10 mg x kg(-1) x d(-1)), were compared 7 days after transplantation. Echocardiography-derived left ventricular post wall thickness was increased only in untreated allografts.The left ventricular ejection fraction was significantly lower in the allografts compared with the isografts, but allografts treated without or with low-dose CsA showed similar results. The radial velocity and systolic radial strain rate showed a lower value in untreated allografts than other grafts, but there is no significant difference between allografts treated with high- or low-dose CsA and isografts. The circumferential strain and circumferential strain rate were comparable among the 4 groups. However, the radial strain exhibited a clear gradient in these groups (2.8 +/- 1.3 in untreated allografts, 5.2 +/- 10.9 in allografts treated with low-dose CsA, 6.3 +/- 1.8 in allografts treated with high-dose CsA, and 12.7 +/- 7.9 in isografts, P < 0.001).STI is able to offer a noninvasive method for detecting transplant allograft rejection.
Noninvasive and Quantitative Measurement of C4d Deposition for the Diagnosis of Antibody-Mediated Cardiac Allograft Rejection

Abstract:BACKGROUND:C4d is a specific biomarker for the diagnosis of antibody-mediated rejection (AMR) after cardiac transplantation. Although strongly recommended, routine C4d surveillance is hindered by the invasive nature of endomyocardial biopsy. Targeted ultrasound (US) has high sensitivity, and C4d is abundantly expressed within the graft of patients experiencing AMR, which makes it possible to visualize C4d deposition in vivo using targeted US.METHODS:We designed a serial dilution of C4d-targeted microbubbles (MBC4d) using a streptavidin-biotin conjugation system. A rat model of AMR with C4d deposition was established by pre-sensitization with skin transplantation before cardiac transplantation. MBC4d were injected into recipients and then qualitatively and quantitatively analyzed using the destruction-replenishment method with a clinical US imaging system and analyzed by software.FINDINGS:We successfully obtained qualitative images of C4d deposition in a wide cardiac allograft section, which, for the first time, reflected real-time C4d distribution. Moreover, normal intensity difference was used for quantitative analysis and exhibited an almost nearly linear correlation with the grade of C4d deposition according to the pathologic evidence. In addition, MBC4d injection did not affect the survival and aggravate injury, which demonstrates its safety.INTERPRETATION:This study demonstrates a noninvasive, quantitative and safe evaluation method for C4d. As contrast-enhanced US has been widely used in clinical settings, this technology is expected to be applied quickly to clinical practice. Non-invasive detection of acute renal allograft rejection by measurement of vascular endothelial growth factor in urine.

Abstract:Urinary vascular endothelial growth factor (VEGF) was determined by enzyme-linked immunosorbent assay in 199 renal allograft recipients and 80 healthy controls. Urinary VEGF level did not change significantly during the first 8 weeks after transplantation in 119 patients with stable renal function and there were no abnormal histological findings (No-AR). In 67 patients with acute rejection, urinary VEGF was significantly higher (28.57 +/- 6.21 pg/mu mol creatinine) than in the No-AR patients (3.05 +/- 0.45 pg/mu mol creatinine) and healthy controls (2.87 +/- 0.35 pg/mu mol creatinine). At a cut-off point of 3.26 pg/mu mol creatinine, sensitivity and specificity for diagnosis of acute rejection were 86.6 and 71.4%, respectively. The 13 patients with subclinical rejection excreted urinary VEGF (16.14 +/- 4.09 pg/mu mol creatinine) at a significantly higher level than No-AR patients (3.05 +/- 0.45 pg/mu mol creatinine). At a cut-off point of 4.69 pg/mu mol creatinine, sensitivity and specificity for diagnosis of subclinical rejection were 84.6 and 79.8%, respectively. In conclusion, monitoring VEGF in urine might offer a new non-invasive way to detect acute and subclinical rejection in renal transplant recipients.

Abstract:OBJECTIVE:This study aimed to investigate the relationship between the severity of ischemia/reperfusion (I/R) injury and the acute rejection (AR) of allogeneic liver transplants in rats.METHODS:The experimental rats were divided in different groups: normal control group (sham group, group I); syngeneic liver transplant control group (similar gene group, group II); and allogeneic liver transplant groups (groups III to VI). The rats were humanely killed at 1, 3, 5, and 7 days after transplantation or sham operation to determine the severity of I/R injury, rejection classification, and hepatocyte apoptosis. Messenger RNA (mRNA) and protein expression levels of Fas, perforin, and granzyme B were assessed in the liver tissues using real-time polymerase chain reaction and immunohistochemistry, respectively.RESULTS:The rejection scores of the transplanted liver tissues gradually increased until these scores were proportional to the severity of I/R injury in groups III, IV, and V. The maximum scores were reached at 7 days after transplantation as the duration of transplantation was extended. The mRNA and protein expression levels of Fas, perforin, and granzyme B were significantly increased at 1, 3, 3, 5, and 7 days after liver reperfusion in groups III, IV, and V compared with those in groups I, II, and VI (P < .05).CONCLUSION:The occurrence of AR after allogeneic liver transplantation in rats was positively correlated with the severity of I/R injury. Given that I/R injury caused serious damage to the transplanted liver, the occurrence of AR consequently decreased.
Prediction of Subclinical Renal Allograft Rejection by Vascular Endothelial Growth Factor in Serum and Urine.

Abstract:BACKGROUND:Until now subclinical renal allograft rejection has only been recognized through a protocol biopsy. The aim of this study was to assess whether measurement of vascular endothelial growth factor (VEGF) in serum and urine could be adopted as a new noninvasive tool to predict subclinical rejection.METHODS:Concentration of VEGF in serum and urine was determined by ELISA in 132 recipients of a renal allograft with stable renal transplant function who were to undergo protocol biopsy and 80 healthy controls. A conventional receiver operating characteristic (ROC) curve was used to determine the sensitivities and specificities for patients with subclinical rejection.RESULTS:Levels of VEGF in serum (126.96 +/- 20.13 pg/mL; 95% confidence interval [95% CI], 83.10-170.83) and urine (16.14 +/- 4.09 ng/mmol creatinine; 95% CI, 7.21-25.06) of 13 patients with subclinical rejection significantly differed from those of 119 patients with no allograft rejection (No-AR) and health controls. The areas under the ROC curve were 0.771 (95% CI, 0.0.64-0.901) and 0.819 (95% CI, 0.662-0.976), respectively. Levels of VEGF in serum and urine after antirejection therapy (50.45 +/- 6.58 pg/mL and 2.60 +/- 0.83 ng/mmol creatinine, respectively) were lower than those at the time of protocol biopsy. No difference in urinary and serum VEGF expression was observed between cyclosporine and tacrolimus treatment.CONCLUSION:It is first reported that the monitoring of VEGF in serum and urine might be a new noninvasive approach to supplement a protocol biopsy for detection of subclinical rejection.
Evaluating acute rejection after heterotopic cardiac transplantation in rats by speckle tracking imaging

STI offers promise as a noninvasive method for detecting transplant allograft rejection.
Non‐invasive parameters for detection of cardiac allograft rejection

Abstract:Six non‐invasive parameters for detection of allograft rejection were evaluated in a series of 51 consecutive heart transplant recipients. All patients were treated with a triple‐drug regimen for immunosuppression, consisting of steroids, azathioprine and cyclosporine A. Three immunological and three functional parameters were compared with the results of endomyocardial biopsies (EMB). The number of activated lymphocytes in the peripheral blood was assessed by cytoimmunological monitoring (CIM) and correlated with EMB in 727 instances. In 154 of these, acute rejection was present, the sensitivity of CIM was 74.1%. Serum neopterine levels were assessed in 246 samples, 56 of which were obtained during acute rejection. The sensitivity of this assay was 50%. Antimyolcmmal antibodies (AMLA) were studied in 89 blood samples. Correlation with acute rejection on EMB disclosed a sensitivity of 67% for IgM and 65% for IgG subclasses of AMLA. Of the functional parameters studied, both intramyocardial electrogram (telemetry pacemaker system), and left ventricular ejection fraction (radionuclide ventriculography) showed poor correlations with EMB (47% and 39%, respectively). M‐mode echocardiography, by contrast, more reliably predicted acute rejection when assessing the isovolmetric relaxation period (sensitivity 78%). With the combined use of CIM and echocardiography, no acute rejection was missed in 9 instances (sensitivity 100%). It is concluded that more than one non‐invasive parameter is needed to accurately predict cardiac allograft rejection. The combined use of functional and immunological parameters appears advantageous.

Abstract:BACKGROUND:Acute rejection (AR) and acute tubular necrosis (ATN) are main causes of early renal allograft dysfunction. Blood oxygen level-dependent magnetic resonance imaging (BOLD MRI) and Diffusion weighted (DW) MRI can provide valuable information about changes of oxygen bioavailability and water diffusion by measuring R2* or apparent diffusion coefficient (ADC) respectively. We aimed to determine the value of BOLD MRI and DW MRI in detecting causes for early allograft dysfunction in renal allograft recipients. METHODS:Fifty patients received renal allografts from deceased donors were analyzed, including 35 patients with normal renal function (control group), 10 AR patients and 5 ATN patients. Cortical R2* (CR2*) and medullary R2* (MR2*) were measured by BOLD MRI. Ten diffusion gradient b values (0, 5, 10, 20, 50, 100, 200, 400, 800, 1200s/mm2) were used in DW MRI. ADC values were measured in renal cortex (CADC) and medulla (MADC). CADCl and MADCl were measured under low b values (b ≤ 200 s/mm2), while CADCh and MADCh were measured under high b values (b > 200 s/mm2). RESULTS:MR2* was significantly lower in AR group (18.2 ± 1.5/s) than control group (23.8 ± 5.0/s, p = 0.001) and ATN group (25.8 ± 5.0/s, p = 0.004). There was a tendency of lower levels on CADCl, MADCl, CADCh or MADCh in AR group than in control group. There were no differences on ADC values between AR group and ATN group. CONCLUSIONS:BOLD MRI was a valuable method in detection of renal allografts with acute rejection.

Abstract:Background. Organs transplantation is an effective treatment for end-stage organ failure. Despite the use of modem immunosuppressants to decrease its incidence, acute rejection episodes (ARE), still present a problem for diagnosis, resolution, and prediction of long-term outcomes due to the absence of sufficiently robust biomarkers. Methods. Using an heterotopic heart transplantation model using Dark Agouti to Lewis rats, and sirolimus (rapamycin, Rapa) treatment by gavage, we divided recipients into four groups: controls, ARE, Rapa-14, and Rapa-7. We evaluated recipients by hematoxylin and eosin staining of grafts and reverse transcription polymerase chain reactions. Levels of plasma metabolites were quantified using gas chromatography/time-of-flight mass spectrometry. Data were evaluated employing partial least-squares discriminant analysis (PLS-DA), the area under the receiver operating characteristic curves with negative predictive values (NPV) and positive predictive values (PPV). Results. The graft survival was prolonged by Rapa. Plasma levels of 10 metabolites differed significantly between the ARE and the Rapa-14 groups as illustrated by the total ion current. According to PLS-DA, proline, glycine, serine, phenylalanine, and isocitrate showed the greatest effects with areas under the curve of 0.944, 0.917, 1.0, 0.861, and 0.944 respectively. The NPV values were 85.7%, 85.7%, 100%, 83.3%, and 85.7% and PPV values, 100%, 100%, 100%, 83.3%, and 100% respectively. Therefore, these metabolites may be used to predict the occurrence and progression of ARE. Conclusion. The trend of changes suggested that plasma metabolites correlated with the immune state of recipients. Therefore, metabonomics may provide new biomarkers for graft injury in the early phases of ARE.

Abstract:We have computed the excitation probabilities of the T/sub 2/ molecules in the crystal surrounding the ..beta..-decaying tritium. The influence of this effect on the neutrino mass determination has been studied. For a neutrino mass of the order of a few eV the effect has been shown to be very important. Its importance decreases, however, with increasing neutrino mass. In addition we consider the influence of the high-energy portion (E>164 eV) of the probability distribution on the neutrino mass.
Effect of Na adsorption on the structural and electronic properties of Si(111)3×3?> -Au surface

Abstract:Adsorption of ∼0.1 ML of Na onto the Si(111)3×3?> -Au surface held at 300 °C has been found to induce pronounced changes in its structural and electronic properties. Domain wall networks, characteristic of the pristine surface, are removed completely, leading to the formation of a highly ordered homogeneous surface. The original atomic arrangement of the Si(111)3×3?> -Au is preserved and Na atoms occupy T4 adsorption sites at the centers of surface Si trimers. Upon Na adsorption, a pronounced metallic S1 surface-state band develops. It is characterized by a large spin splitting (momentum splitting at the Fermi level Δk∥ = 0.027 Å−1 and consequent energy splitting ΔEF = 110 meV), large electron filling (on the order of 0.5 electrons per 3×3?> unit cell) and small effective electron mass of (0.028 ± 0.006)me. The natural consequence of the latter properties is a high surface conductivity of the Si(111)3×3?> -(Au, Na) surface.

Abstract:Alteration in the expression of microRNAs (miRNAs) is associated with oncogenesis and cancer progression. In this review we aim to suggest that elevated levels of estrogens and their metabolites inside the lungs as a result of cigarette smoke exposure can cause widespread repression of miRNA and contribute to lung tumor development. Anti-estrogenic compounds, such as the components of cruciferous vegetables, can attenuate this effect and potentially reduce the risk of lung cancer (LC) among smokers.

Abstract:Acute renal allograft rejection (ARAR) after kidney transplantation associated with reduced graft survival and eventual graft failure is poorly diagnosed in hospitals. Here, we report the development of Artificial bioMarker Probes (AMPros) for sensitive urinalysis of ARAR in murine models. AMPros spontaneously go to the kidneys after systemic administration, specifically react with the prodromal immune biomarkers to activate their near-infrared fluorescence signals to report cell-mediated rejection, and efficiently undergo renal excretion into urine. Thus, AMPros enable convenient optical urinalysis that detects ARAR prior to histological manifestation of rejection, which is also earlier than current diagnostic methods measuring proinflammatory cytokines and peripheral blood lymphocyte mRNAs. Due to the high kidney specificity, AMPros-based urinalysis discriminates allograft rejection against other non-alloimmune specific diseases, which is unattainable by measurement of serological biomarkers. Such a noninvasive and sensitive urine test holds great promise in continuous monitoring of renal allograft conditions at low resource settings for timely clinical interventions.

Abstract:Objective Methotrexate and rapamycin demonstrate an additive effect in prolonging cardiac allograft survival in a major histocompatibility complex mismatched rat model. The present study aimed to identify functional proteins involved in the allograft-protective effects of these two agents and reveal potential diagnostic markers for treating rejection. Methods Serum samples from heterotopic heart-transplanted LEW(RT-11) rats (either without immunosuppressive treatment or treated with methotrexate alone, rapamycin alone, or methotrexate and rapamycin combined) were analysed by surface-enhanced laser desorption/ionization time-of-flight mass spectrometry. Protein profiles obtained using a weak cation exchange ProteinChip® CM-10 array were then analysed using ProteinChip® Software. Results Of 28 rejection-related proteins identified, isoelectric point and mass information from two potential candidate proteins matched information from the UniProtKB/Swiss-prot database, suggesting them to be complement component C3f fragment and complement component 4A (C4A, anaphylatoxin). Conclusions Proteomic analysis revealed 28 proteins as potential diagnostic markers of tissue rejection. Of these, 11 proteins may represent targets relating to the additive effects of methotrexate and rapamycin. Two protein peaks, with mass-to-charge ratios of 1950 Da and 8577 Da, may have potential for use in post-transplant diagnosis of rejection.
C4d as a significant predictor for humoral rejection in renal allografts.

Abstract:OBJECTIVE:To determine the diagnostic and clinical significance of C4d accumulation in renal allografts followed by acute rejection. METHODS:A total of 158 graft biopsies performed from December 1997 to December 2002 were classified, according to the Banff-97 criteria, into hyperacute rejection (HAR, three cases), acute vascular rejection (AVR, 27), acute cellular rejection (ACR, 24), borderline rejection (BR, 38), acute tubular necrosis (ATN, five), stable graft function (SGF, 30) and baseline kidney (31). Immunohistochemical technique was used to determine the C4d deposition level. RESULTS:The percentages of C4d positive in HAR, AVR, ACR, BR, ATN, SGF and baseline kidney groups were 100% (3/3), 77.8% (21/27), 37.5% (9/24), 23.7% (9/38), 0% (0/5), 3.3% (1/30), 0% (0/31), respectively. In acute rejection patients, the peak serum creatinine (sCr) level in C4d(ptc)-positive group (41 cases) was 334.82 +/- 238.37 micromol/L, with that of C4d(ptc)-negative group (47 cases) being 220.20 +/- 176.94 micromol/L (p < 0.01). After treatment, the trough sCr level in C4d(ptc)-positive group and C4d(ptc)-negative group were 176.87 +/- 111.80 and 121.75 +/- 34.59 micromol/L (p < 0.01), respectively. In each AVR, ACR and BR subgroups, the peak sCr level, the trough sCr level, after 3 or 6 months of AR, the sCr level in C4d(ptc)-positive subgroup was higher than that of C4d(ptc)-negative subgroup. There were more resistance against steroid therapy [65.9% (27/41) vs. 36.2% (17/47), p = 0.005] and a higher rate of graft loss [29.3% (12/41) vs. 6.4% (3/47), p = 0.001] in C4d(ptc)-positive group than those of C4d(ptc)-negative group. In each C4d(ptc)-positive subgroup of AVR, ACR and BR the complete reversion was 57.1, 56 and 66.7%, respectively, it is almost same. CONCLUSION:The C4d deposition level is of great value in diagnosis of acute rejection caused by humoral immune components. It is a significant predictor of graft survival and will be of great help when treating acute rejection.

Abstract:Objective: To investigate the expression of intercellular adhesion molecule-1 (ICAM-1) and vascular cell adhesion molecule-1 (VCAM-1) in allograft with chronic rejection (CR) after heart transplantation. Methods: Seventy-two SD rats receiving transplantation of the heats of 71 Wistar rats were divided into 4 equal groups: Group A, undergoing heterotopic cervical heart transplantation (transplanted with donor's heart subcutaneously at the neck) and receiving none intervention, and with the transplanted hearts taken out 3 d after transplantation to the end of CR; Group B, injected intravenously with the splenocytes (SPCs) of the donors on day 0, injected with cyclophomide (CP) on d2, transplanted with the donor hearts on d15, with the transplanted hearts taken out 15 - 120 days after transplantation; Group C, transplanted with the donor's heart and injected intraperitoneally with cyclosporine A (Cs A) 10 mg/kg every other day for 8 - 10 times, and with the transplanted hearts taken out 60 d after transplantation to the end of CR; and Group D, injected intravenously with the SPCs of the donors on day 0, injected with CP on d2, transplanted with the donor hearts on d15, with the transplanted hearts taken out 150 - 420 days after transplantation. Immunohistochemical method was used to detect the expression of ICAM-1 and VCAM-1 in the allografts. Results: The cardiac allograft survival time of Group B was the longest. Less ICAM-1 and VCAM-1 expression and lymphocyte infiltration were observed in SPC and CP-pretreated group. Reversely, protein expression levels of ICAM-1 and VCAM-1 were both high and significant lymphocyte infiltration was seen in Groups B and D. There was no significant difference in the expression of ICAM-1 and VCAM-1 between Groups A and C (P > 0.05). Conclusion: The expression levels of ICAM-1 and VCAM-1 are associated with the occurrence and development of rejection and have positive correlation with the severity of graft rejection. Determination of ICAM-1 and VCAM-1 can predict the function of allograft and provide evidence for early diagnosis and prevention of CR.
Ultrasound Molecular Imaging of Lymphocyte-endothelium Adhesion Cascade in Acute Cellular Rejection of Cardiac Allografts.

Abstract:Background. Acute cellular rejection is one of the main reasons for graft failure after heart transplantation. A precise diagnosis at the early stage of acute cellular rejection is a big challenge for clinicians. Given the importance of the interaction between T cells and graft endothelia in initiating rejection, we developed T cell-microbubble complexes (cell-MBs) as ultrasound molecular imaging probes to monitor the lymphocyte-endothelium adhesion cascade in cardiac acute cellular rejection. Methods. Cell-MBs were fabricated by incubating lymphocytes with anti-CD4 antibody-conjugated MBs (MBCD4). The potential of cell-MBs as probes for detecting acute cardiac rejection was examined. Donor hearts from Brown Norway or Lewis rats were transplanted into Lewis recipients. Ultrasound molecular imaging was performed on allografts of untreated or cyclosporin A (CsA)-treated recipients, and isografts on posttransplantation day 3. Histology was used to assess rejection grades. Results. We detected a significantly stronger ultrasound molecular imaging signal of cell-MBs than that of MBCD4 or plain MBs in allografts of untreated and CsA-treated recipients. No signal enhancement was observed in isografts with cell-MBs. The signal of cell-MBs in allografts of the untreated group was significantly higher than that in the CsA-treated group, and the signal in the CsA-treated group was higher than that in isografts. Histology confirmed grade 3R rejection in the untreated group, grade 2R rejection in CsA-treated group, and no rejection in isografts. Conclusions. Our study suggests that cell-MBs can function as a promising probe to image the dynamic lymphocyte-endothelium adhesion cascade for noninvasive diagnosis of cardiac acute cellular rejection.

Abstract:The in vitro generation of a bioartificial cardiac construct (CC) represents a promising tool for the repair of ischemic heart tissue. Several approaches to engineer cardiac tissue in vitro have been conducted. The main drawback of these studies is the insufficient size of the resulting construct for clinical applications. The focus of this study was the generation of an artificial three-dimensional (3D), contractile, and suturable myocardial patch by combining a gel-based CC with decellularized porcine small intestinal submucosa (SIS), thereby engineering an artificial tissue of 11 cm² in size. The alignment and morphology of rat neonatal cardiomyocytes (rCMs) in SIS-CC complexes were investigated as well as the re-organization of primary endothelial cells which were co-isolated in the rCM preparation. The ability of a rat heart endothelial cell line (RHE-A) to re-cellularize pre-existing vessel structures within the SIS or a biological vascularized matrix (BioVaM) was determined. SIS-CC contracted spontaneously, uniformly, and rhythmically with an average rate of 200 beats/min in contrast to undirected contractions observed in CC without SIS support. rCM exhibited an elongated morphology with well-defined sarcomeric structures oriented along the longitudinal axis in the SIS-CC, whereas round-shaped and random-arranged rCM were observed in CC. Electric coupling of rCM was demonstrated by microelectrode array measurements. A dense network of CD31⁺/eNOS⁺ cells was detected as permeating the whole construct. Superficial supplementation of RHE-A cells to SIS-CC led to the migration of these cells through the CC, resulting in the re-population of pre-existing vessel structures within the decelluarized SIS. By infusion of RHE-A cells into the BioVaM venous and arterial pedicles, a re-population of the BioVaM vessel bed as well as distribution of RHE-A cells throughout the CC was achieved. Rat endothelial cells within the CC were in contact with RHE-A cells. Ingrowth and formation of a network by endothelial cells infused through the BioVaM represent a promising step toward engineering a functional perfusion system, enabling the engineering of vascularized and well-nourished 3D CC of dimensions relevant for therapeutic heart repair.
