Abstract:ObjectiveTo study the Y-Chromosome microdeletion rate and type in azoospermia men before ICSI treatment and hence to assess the relationship between spermatogenic defects and Y-chromosome microdeletion.Methods15 Y-linked sequence tag sites(STS) were screened by means of PCR in 63 azoospermia men undergoing testicular diagnostic biopsy before ICSI treatment.ResultsA total of 12 (19.1%) cases of microdeletions were found in 63 azoospermia patients. 10 (15.8%) cases were involved in AZFc/DAZ microdeletions, 2 (3.2%) cases were involved in AZFb and AZFc, and no patients were involved in AZFa.ConclusionY-chromosome microdeltions is an important reason of azoospermia. PCR is a useful technique for detecting microdeletions. Screening of Y-chromosome microdeletions for azoospermia patients before ICSI treatment is essential.
Microdeletions of AZF in Y Chromosome and Spermatogenesis Failure in Infertile Men

Abstract:Objectives: To explore the relationship between AZF microdeletions in Y chromosomal and male spermatogenesis.Methods: The study recruited 1021 infertile Chinese men,including 343 men of azoospermia(177 with nearly normal spermatogenesis in tests and 166 cases with maturation arrest in tests),633 men of oligozoospermia and 45 men of asthenospermia.Y chromosomal microdeletions in AZFa,b,c regions were screened with a multiplex polymerase chain reaction(PCR).Results: In azoospermia patients,41(24.7%) cases with Y chromosomal microdeletions were found with spermatogenesis failure,while 3 cases were found to have normal spermatogenesis.There were 49 cases with AZF microdeletions found in oligozoospermics.No case was found in asthenospermics.There was a significant difference in Y chromosomal microdeletions between the patients with spermatogenesis failure and other infertile men(P0.01).Conclusion: Microdeletion of AZF was a genetic risk factor for spermatogenic failure and male infertility.Therefore,patients of zoospermia and oligozoospermia.should be screened for Y chromosome microdeletion.
AZF gene microdeletions: case series and literature review

Abstract:Objective: Aproximately 10% of patients with non-obstructive azoospermia and 5% with non-obstructive severe oligozoospermia carry AZF region microdeletions (AZoospermic Factor) in the Y chromosome. The aim of this study is to analize the clinical and pathological findings in this group of patients and compare them with the previous evidence. Material and methods: Retrospective study of 11 patients with diagnosis of azoospermia or oligozoospermia and the presence of AZFa, AZFb, AZFc microdeletions or any combination of them. Results: Microdeletions of AZFc region were found in 45% of cases, AZFa in 33% and a 10% showed a deletion of the three regions (a,b and c). 91% of them demonstrated azoospermia with low testicular volume in 62,5% cases. Conclusion: Microdeletions of AZF regions are associated with azoospermia and a low expectation of sperm retrieval in testicular biopsy. On the other hand, they seem not related with significative modifications on the hormone profile.

Abstract:To determine microdeletion loci and the characteristic of Y chromosome azoospermia factor(AZF) associated with Chinese idiopathic azoospermia or severe oligozoospermia and hence to provide a theoretic basis for gene diagnosis of AZF microdeletion in Chinese infertility men.The subjects of this study included 134 cases of azoospermia and 118 severe oligozoospermia, and 210 healthy male controls. With multiplex PCR technique, an analysis of 15 sequence tag sites(STS) in AZFa, AZFb and AZFc microdeletion was performed.No microdeletion was detected in controls whereas 13 STS loci microdeletions existed in cases, including sY84 and sY86 in AZFa; sY121, sY123, sY124, sY127, sY134 and sy133 in AZFb; sY152, sY242, sY254, sY255 and sY157 in AZFc. Five azoospermia patients were involved in AZFa microdeletions, 7 azoospermia and 3 severe oligozoospermia patients in AZFb, and 14 azoospermia and 18 severe oligozoospermia patients in AZFc. The prevalence rates of microdeletion in AZFa, AZFb and AZFc were 2.0%,4.0% and 12.7% respectively. The microdeletion rate showed statistically significant difference between the cases and controls.This study revealed an association between the microdeletion of 13 STS loci of AZF region and the idiopathic azoospermia or severe oligozoospermia in Chinese. There is no evidence to identify the genetic polymorphism of the above STS loci. The frequency and distribution characteristic of AZF microdeletion are similar to those of Caucasians. As candidate loci the 13 STS are useful in clinical gene diagnosis for the detection of AZF microdeletion in Chinese idiopathic azoospermia and severe oligozoospermia.

Abstract:OBJECTIVE:To study the morphology of Y chromosome and microdeletion of the correlated specific azoospermia factor(AZF) region on Y chromosome in cases of azoospermia and to identify the genetic diagnosis made for male infertility patients.METHODS:Peripheral blood samples were taken from two patients with azoospermia, and then were examined by use of G banding, C banding cytogenetic analysis and multiplex polymerase chain reaction (PCR) microdeletion analysis.RESULTS:The karyotypes of the two cases were 45, X, -Y, -22, +der(Y)t(Y;22)(q11.2;q11.2) and 46, XY, del(Y)(q11.2) respectively. In 12 sequence-tagged sites(STS) of AZFa, AZFb, AZFd, AZFc, only one was detected in the first case and two were detected in the other case.CONCLUSION:The cytogenetic analysis and the detection of AZF microdeletion on Y chromosome are essential to the final genetic diagnosis to be made for male infertility patients.

Abstract:To investigate the genetic causes of azoospermia and severe oligozoospermia.Cytogenetic analysis and multiplex polymerase chain reaction(PCR) analysis were done on the 148 patients with azoospermia and serious oligozoospermia.Eleven of the 148(7.4%) cases showed microdeletion of at least one STS. In fifteen STS of AZFa, AZFb,AZFd, AZFc, thirteen STS, eleven STS,two STS and one STS microdeletion were found in each case respectively, including two with 12 STS, five with 5 STS microdeletion.Seven cases had chromosomal morphologic changes(4.7%),four had deletion and one had deletion with translocation of long arm on Y chromosome. One had enlarged region one band two(q12) on long arm of Y chromosome and one had reciprocal translocation of autosomes.The findings indicated that AZF microdeletion and chromosomal abnormality should be important causes of male infertility.

Abstract:Background and objective: The presence of microdeletions in the Y-chromosome azoospermia factor (AZF) region (YCMs) is considered the most frequent genetic cause of male infertility along with Klinefelter syndrome. The objective of this study was to investigate the frequencies and type of YCMs in infertile men in Aragon and to analyze the relationship between sex hormones, sperm count and microdeletions in them. Patients and methods: Retrospective descriptive study of 644 men who during 2006-2019 were screened for YCMs using YChromStrip (Operón, Spain) by PCR+reverse hybridization, spermiogram, karyotype and quantification of sex hormones. Results: The frequency of YCMs was 3.88% (25/644), not being detected in any patient with mild or normospermic oligozoospermia, that is, in sperm counts higher than 5×106/mL. The group of azoospermic patients was the one that presented a higher frequency of YCMs (14.58%, 14/96). Deletions in the AZFc region were the most frequent (68%). 20% (5/25) of patients with YCMs also presented some type of karyotype abnormality that included aneuploidies, deletions, duplications and/or translocations. Sperm count was significantly lower and FSH and LH concentrations significantly higher in the group of patients with YCMs. Conclusions: YCMs screening is a key test in the diagnostic approach to male infertility. Obtaining an adequate result allows choosing suitable assisted reproduction techniques, preventing unnecessary treatments and the transmission of genetic defects to offspring.
Clinical Characteristics and Treatment of Azoospermia and Severe Oligospermia Patients with Y‐chromosome Microdeletions

Abstract:SUMMARYClinical characteristics, testicular pathology, serum levels of reproductive hormones, and genetic analysis were compared among 100 azoospermic, 20 oligozoospermic cases with azoospermia factor (AZF) microdeletion, and 50 fertile males to evaluate the relationship between the AZF microdeletion regions and the azoospermia phenotype. AZF microdeletion region, testicular volume, and serum reproductive hormone levels of patients were compared against histological examination of testicular biopsies. The number of cases of AZFa, AZFb, AZFc, AZFb + c, and AZFa + b + c microdeletion was respectively 2 (1.7%), 15 (12.5%), 77 (64.2%), 24 (20.0%), and 2 (1.7%). The testicular volume of patient with AZF microdeletion was smaller (P < 0.01), while luteinizing hormone (LH) and follicle‐stimulating hormone (FSH) levels were significantly higher than that of fertile individuals (P < 0.01 and P < 0.05, respectively). Among the patients, testicular volume with AZFb‐microdeletion cases was larger compared to patients with AZFc or AZFb + c microdeletions (P < 0.05 and P < 0.01, respectively), whereas FSH levels were significantly lower than that of AZFc or AZFb + c microdeletions (P < 0.05). The Johnsen score of patients with an AZFb + c microdeletion was lower than that of patients with AZFb and AZFc microdeletions, but no significant difference was observed. Pathological findings of testicular biopsies poorly correlated with the pattern of AZF deletion, with the AZFc microdeletion exhibiting the most varied phenotypes. In subsequent assisted reproductive treatments, sperm from patients with an AZFc microdeletion that was obtained by testicular sperm aspiration (TESA) or microdissection testicular sperm extraction (m‐TESE) were more likely to result in pregnancy. Combined with testis pathology pattern, the specific region of AZF microdeletion and hormonal assessments provide reliable prognostic information on the chance of successful sperm retrieval for assisted reproductive technologies. Abstract:Recently, work has shown that azoospermia factor (AZF) microdeletions result from homologous recombination between almost identical blocks in this gene region. These microdeletions in the Y chromosome are a common molecular genetic cause of spermatogenetic failure leading to male infertility. After completion of the sequencing of the Y chromosome, the classical definition of AZFa, AZFb, and AZFc was modified to five regions, namely AZFa, P5/proximal-P1, P5/distal-P1, P4/distal-P1, and AZFc, as a result of the determination of Y chromosomal structure. Moreover, partial AZFc deletions have also been reported, resulting from recombination in their sub-ampliconic identical pair sequences. These deletions are also implicated in a possible association with Y chromosome haplogroups. In this review, we address Y chromosomal complexity and the modified categories of the AZF deletions. Recognition of the association of Y deletions with male infertility has implications for the diagnosis, treatment, and genetic counseling of infertile men, in particular candidates for intracytoplasmic sperm injection.

Abstract:OBJECTIVE: To evaluate the frequency of complete and partial AZFa Y-chromosome microdeletions among infertile Israeli men. To review the published frequencies and histologic findings of AZFa deletions.DESIGN: Retrospective study.SETTING: Academic medical center.PATIENT(S): A total of 1,260 infertile Israeli men. Literature review (2000-2010) of reports on men with AZFa deletions and their testicular findings.INTERVENTION(S): The DNA of 1,260 infertile men was evaluated for AZF microdeletions. The DNA of 657 of them with undetected microdeletions was analyzed for partial AZFa deletion in the USP9Y and DDX3Y genes using sequence-tagged sites beyond EAA/EMQN recommendations.MAIN OUTCOME MEASURE(S): The frequency of complete and partial AZFa microdeletions. Availability of sperm cells for intracytoplasmic sperm injection in men with complete/partial microdeletions.RESULT(S): Two men had complete AZFa deletion (a frequency of 0.28% among nonobstructive azoospermic men). None had partial AZFa deletions.CONCLUSION(S): The likelihood of finding sperm cells in men with complete AZFa deletions is negligible. Complete AZFa deletion is rare and usually associated with azoospermia and absence of sperm cells in testicular tissue. The low frequency of partial AZFa deletions and the inconsistent prospects for spermatogenesis reported in the literature question the need for routine assessment of microdeletions in genes, such as USP9Y or DDX3Y.

The effect of azoospermia factor microdeletions on intracytoplasmic sperm injection results in azoospermia patients

Abstract:Background &objective: Y chromosome abnormalities are common in male patients with severe oligo-azoospermia. In studies with karyotype analysis and cytogenetic methods, the importance of the Y chromosome in spermatogenesis has been well understood. Deletions in the azoospermia factor (AZF) localized at the distal end of the Y chromosome adversely affect the spermatogenesis process. Our objective was to determine the frequency of AZF microdeletion in azoospermia patients who underwent microTESE. Methods: In this retrospective cohort study, 806 azoospermic men attending the In Vitro Fertilization (IVF) Center for infertility treatment between 2010 and 2022 were included. AZF deletion screening was conducted in all patients included in the study. Azoospermic patients with and without Y microdeletion were matched with the female's age, cause of infertility, number of oocytes retrieved and number of metaphase II (MII) oocytes produced and compared. The primary outcome was the live birth rate (LBR). Pregnancy rate (PR) and clinical pregnancy rates (CPR) were secondary outcomes. Results: We detected Y microdeletion in 55 (6.82%) of 806 infertile azoospermic men and 35 of them included in the study. Although the required gonadotropin dose and the total number of retrieved oocytes were similar, clinical pregnancy rates and live birth rates were found to be significantly lower in the microdeletion patient group (21.6% vs. 43%, p<0.05; and 18.9% vs. 36%, p<0.05, respectively). Conclusions: Poor sperm quality in AZF microdeletion patients complicates the selection of appropriate sperm for ICSI. Therefore, it leads to a decrease in embryonic development, fertilization and pregnancy results. In order to select the best sperm for the use in ICSI procedure in this patient population, intracytoplasmic morphologically selected sperm injection (IMSI) method can be preferred to improve the cycle outcomes.
Tracking microdeletions of the AZF region in a patrilineal line of infertile men

Abstract:Male infertility is considered to be a difficult-to-treat condition because it is not a single entity, but rather reflects a variety of different pathologic conditions, thus making it difficult to use a single treatment strategy. Structural alterations in the Y chromosome have been the principal factor responsible for male infertility. We examined 26 family members of 13 patients with male infertility who showed deletions in the AZF region. In family 1, the father and a brother did not show microdeletions. However, a son showed a microdeletion in AZFa (sY84) and an azoospermic sperm analysis, but another son had a microdeletion in AZFa (sY84) and AZFb (sY127) and a normal sperm analysis. The father of family 2, with severe oligozoospermia, had a microdeletion in the AZFa region (sY84) and his son, conceived by intracytoplasmic sperm injection, also showed the same microdeletion. In the other families, only the men with an altered sperm analysis had a microdeletion. It is possible that in family 1, the father and brother who did not show microdeletions in this study, could have microdeletions in regions upstream or downstream of the one analyzed. The treatment with intracytoplasmic sperm injection can result in vertical transmission of microdeletions of the AZF region and can also cause the expansion of a de novo mutation. This finding reinforces the necessity of an investigation of microdeletions of the Y chromosome in individuals who are candidates for assisted reproduction, as well as genetic counciling and follow-up.
AZF gene expression analysis in peripheral leukocytes and testicular cells from idiopathic infertility

Abstract:The aim of this study was to assess the frequency of AZF microdeletions in peripheral leukocytes and testicular cells in Chinese men with idiopathic infertility. Expression in testicular cells was also determined. In this study, we screened 62 idiopathic infertile patients, in whom karyotype, sperm count and hormonal parameters were evaluated. Genomic DNA was extracted from the peripheral leukocytes. Molecular analysis was performed by two multiplex polymerase chain reactions (PCR) using a set of eight sequence tagged sites (STS) from 3 different regions of the Y chromosome. Total cellular RNA was extracted from the testicular tissue using a Trizol-method. Reverse Transcription (RT) reactions were performed to synthesize cDNA. Amplification of DFFRY, RBM and DAZ genes was performed to analyze their expression in testicular cells. In this cohort, we found 12 submicroscopic deletions (12/62, 19.4%). Nine patients (9/33, 27.2%) were detected in the azoospermic group and three (3/29, 10.3%) in the severe oligozoospermic group. RT-PCR analysis from testicular cells gave normal amplifications for SRY and DFFRY mRNA in 62 idiopathic patients; two patients were negative for RBM expression; no RBM and DAZ were detected for a case; 12 patients had no expression in the AZFc region involving the DAZ gene. Of 12 cases, three patients with normal PCR analysis of DAZ gene on genomic DNA showed no RT-PCR amplification for DAZ mRNA. The use of RT-PCR of specific spermatid expressed genes in conjunction with examining microdeletions using peripheral leukocytes is suggested to avoid the transmission of the Y chromosomal microdeletions from a father to a son via testicular sperm aspiration (TESE), intracytoplasmic sperm injection (JCSI).

Abstract:Objective: To screen the Y chromosome microdeletion in infertile men with idiopathic azoospermia and oligospermia. Methods: Eighty-nine samples(56 oligospermia and 33 azoospermia patients) were screened for 6 STS loci from AZFa,b and c regions by multi-PCR. The single STS,RBMY1A1 and DAZ gene in those with microdeletion were further verified with single PCR. Results: With the screening and verification,6 infertile men were identified as Y chromosome microdeletion from 89 patients (7%). Among them,AZFc/DAZ deletion occurred in 2 oligospermia and 3 azoospermia men,respectively; another azoospermia man showed a deletion of AZFb+c/DAZ /RBMY1A1. Conclusions: After optimization,89 infertile men were screened and 6 Y chromosome microdeletions were identified. The patients with Y chromosome deletions also had gene deletions. Maybe these genes’ deletion resulted in the male infertility.

Abstract:Objective:To investigate the association among idiopathic azoospermia,severe oligospermia or oligospermia in infertility men and microdeletion of azoospermia factor (AZF) gene on the Y chromosome.Methods:Multiplex-PCR technique was used to detect microdeletions of AZF gene in 448 infertile men and 192 normal male controls with fifteen sequence-tagged sites (STSs).Results:There were no microdeletions in 192 normal male controls but 41 microdeletions in 448 infertile men.The rates of microdeletion were 9.2% (41/448) in all,and were 12.0% (19/158),10.8% (17 /157) and 3.8% (5/133) in azoospermia,severe oligozoospermia and oligozoospermia respectively.The rates of AZF microdeletion in azoospermia and severe oligozoospermia were significant higher than that of the oligozoospermia.Compared with the six STS locis method recommended by European Academy of Andrology (EAA),14 % more deletions were detected based on fifteen STS locis in the AZF region.Conclusion:Microdeletion of AZF gene on Y chromosome is one of the major risks leading to idiopathic azoospermia,severe oligospermia or oligospermia.Increasing the number of sequence-tagged sites used for screening can improve the detection rates of Y chromosome AZF microdeletion.

Abstract:Objective:To investigate the correlation of male infertility with abnormality of chromosomal construction and quantity and with the microdeletion of azoospermia factor (AZF) gene on the Y chromosome.As well as the relationship between the partial deletions in the AZFc region of Y chromosome and male infertility.Methods:Multiplex-PCR technique was used to detect microdeletions of AZF gene in 494 infertile men and 236 fertile controls with fifteen sequence-tagged sites (STSs).Chromosome karyotype analyses were performed in the 494 patients.Results:In the azoospermia,severe oligozoospermia and oligozoospermia patients,the incidences of chromosomal abnormality were 11.86％ (21/177) 、3.39％ (6/177) 、2.08％ (3/144) . The rates of AZF microdeletion in azoospermia and severe oligozoospermia were significant higher than that of the oligozoospermia.there were significant differences in the b2/b3 deletion between patients with spermatogenic failure and serious oligozoospermia and controls (both P＜0.05).However,there was no difference for the gr/gr deletion between patients and controls.Conclusion:There was a high frequency of chromosomal construction and quantity abnormality and microdeletion of AZF gene in patients with azoospermia,severe oligospermia and oligospermia,which suggested that chromosomal abnormality and microdeletion of AZF gene might be the important genetic causes of male infertility;There are several types of partial deletion in the AZFc region on Y chromosome,gr/gr deletion might slightly affect male spermatogenesis and exist as a kind of genomic polymorphism.b2/b3 deletion is the high risk factor to spermatogenic failure of male infertility.
Screening of the Microdeletions in the Y Chromosome of Idiopathic Oligospermia or Azoospermia

Abstract:Objective To assess the relationships between idiopathic oligospermia or azoospermia and microdeletions in the Y chromosome. Methods 18 Y-linked sequence-tagged sites in AZF region were screened by means of multiplex PCR in 38 idiopathic oligospermia or azoospermia, including 11 azoospermia, 9 severe oligospermia, 18 oligospermia. Results Microdeletions in the genomic DNA were observed in 6 of 38 cases, 2 being in men with azoospermia,1 in severe oligospermia and 3 in oligospermia. The two formers were AZFd(DYS 237) +AZFc (DAZ+DYS 240),the latter was AZFd(DYS 237) in the deletion types.The total deletion rate was 16%(6/38). Conclusions Microdeletions of the Y chromosome is an important reason of idiopathic oligospermia or azoospermia . Multiplex PCR is a useful technique for detecting microdeletions.
