Abstract:Abstract Introduction Obstructive sleep apnoea syndrome (OSAS) is an increasingly common disorder characterized by episodes of a complete or partial collapse of the upper airway during sleep, leading to oxygen desaturation and disrupted sleep. OSAS is related to increased risk of cardiovascular and cardiometabolic diseases, as well as increased morbidity and mortality (i.e. increase of road accidents and work injuries). Due to the growing number of subjects with suspected OSAS, the need for simple and effective diagnostic methods becomes urgent. The aim of the study was to apply the new machine learning algorithms (MLA) in the clinical evaluation of OSAS. Methods 429 patients with suspected OSAS underwent anamnesis, nocturnal cardio-respiratory monitoring and a medical examination. The severity of OSAS was assessed through the Apnea Hypopnea Index (AHI). To verify the relationships between AHI, comorbidities, anthropometric and socio-demographic characteristics, we performed a multivariate analysis using MLA (logistic regression, Classification Tree and Random Forest). Results The study shows that certain clinical phenotypes (overweight or obese subjects) are more frequently associated with OSAS and that this association is even more significant in male subjects. Discussion Our study confirmed that sleep deprivation due to OSAS is closely linked to obesity and modification of the anthropometric characteristics that must be evaluated in the case of patients/workers with suspected OSAS. Indeed poor sleep increases hunger hormones. It may be due to higher sympathetic nerve activity, the stress response, caused by poor sleep. Conclusion Further studies will be needed to understand the use of MLA in diagnostic, prognostic and therapeutic decision-making of OSAS.
