Abstract:Ectopic prostatic tissue is an underreported entity, which is found most commonly in the tower male genitourinary tract, and ectopic prostate tissue outside the urinary tract is even rarer. Our patient was a unique case of ectopic prostatic tissue within submucosa of the rectum. The patient presented with rectal bleeding, and a firm, round solid submucosa nodule found in the anterior rectum at digital rectal examination, it was 1cm in diameter and 5cm above the anal verge. The size and submucosa location of this nodule were confirmed by the colonoscopy and MRI. After being removed surgically, the histopathology of the specimen sections possessed typical prostatic acini and stroma, meanwhile the immunohistochemical staining for prostate specific antigen confirmed its' prostatic nature. It is the first case to date, which involves the mural of rectum. We hypothesizes that the etiologies of ectopic prostatic tissue within the submucosa rectum attribute to embryogenetic abnormality.

Abstract:A rare case of phyllodes tumour arising in the seminal vesicle is reported here. A 59-year old male presented with lower abdominal discomfort and symptoms of bladder outlet obstruction for 1 year. Transrectal ultrasonography and pelvic computed tomography and magnetic resonance imaging demonstrated a large mass posterior to the bladder and anterior to the rectum. Tumour excision and right seminal vesiculectomy were performed via a transvesical approach. The left seminal vesicle, bladder and prostate were not involved. The patient had an uneventful post-operative course and recovered well. There was no evidence of recurrence or metastasis during 22 months of regular follow up. The diagnosis, histological classification, treatment and prognosis of this case are presented together with a review of the literature. It is suggested that the behaviour of phyllodes tumour of the seminal vesicle is related to its histological parameters.

Abstract:Leiomyomas are benign tumors that can develop in any organ, with the uterus being the most common site. The prevalence of leiomyomas in women can reach as high as 70% to 80% by the age of 50, with the highest incidence among women of African descent. [ 1 ] Leiomyomas originating in the bladder are rare, representing only 0.05% of all bladder tumors, with approximately 250 cases reported globally, affecting men and women equally. Even fewer leiomyomas originate in the prostate, with fewer than 100 documented cases worldwide to date. Most leiomyomas are diagnosed postoperatively, though they may also present with symptoms of local irritation, such as urinary retention, irritative urethral symptoms, and hematuria. The prognosis for leiomyomas is generally positive, with a low recurrence rate. However, accurately assessing the tumor's origin, size, and boundaries via ultrasound, CT, or MRI is often challenging. Moreover, no clinical guidelines have been established for diagnosing and managing this condition or for distinguishing it from malignant bladder tumors. In this report, we describe a case of prostate leiomyoma in a 51-year-old male patient. Additionally, we review the available literature on prostate leiomyomas, covering their epidemiology, etiology, clinical presentation, imaging characteristics, and the diagnostic and therapeutic approaches used in the differential diagnosis and treatment of this condition in clinical practice.

Primary Primitive Neuroectodermal Tumor of the Urinary Bladder: a Case Report and Literature Review

Abstract:Primitive neuroectodermal tumor (PNET) arising in the urinary bladder is extremely rare. Here, we report a case of primary PNET of the urinary bladder in an aged Asian man that showed an aggressive clinical course. A 74-year-old man presented with frequency, dysuria, and gross hematuria. Pelvic computed tomography showed a mass in the urinary bladder. Microscopically, the tumor revealed sheets of uniform, small, round, blue cells, and immunohistochemical examination demonstrated primary PNET of the urinary bladder. Palliative chemotherapy was administered. After three courses of chemotherapy, his disease progressed and he died 4 months after diagnosis.

Abstract:Objective: To evaluate the clinical and pathological characteristics and treatment of ductal adenocarcinoma of the prostate. Methods: A case of ductal adenocarcinoma of the prostate gland was reported, and the relevant literature was reviewed to investigated the diagnosis and treatment of this disease. A 59-year-old male suffered from hemospermia and hematuria. A soft nodule in the right lobe of the prostate was found by digital rectal examination (DRE). His serum prostatic specific antigen (PSA) level was 0.7ng/ml. The prostate biopsy result indicated prostate cancer and radical prostatectomy was performed on him. Results: The postoperative pathology showed a primary ductal adenocarcinoma in the right lobe of prostate, which infiltrated the right vesicle. Gleason score was 5+4 (Sum=9) and the pathologic stage was pT3b. With immunohistochemical staining, the tumor cells were positive for PSA and prostatic acid phosphatase, but negative for androgen receptor. Postoperatively, the patient recovered well with no hemospermia or hematuria. During a follow-up of 7 years, the patient was alive. Conclusion: Ductal adenocarcinoma of the prostate is a clinically rare subtype of prostate cancer and difficult to be found in early stage. The diagnosis depends on histopathologic and immunohistochemical studies. Radical prostatectomy is recommended for this disease.

Abstract:Prostatic-type polyps are uncommon lesions in the urinary tract. They are sometimes found in the lower urinary tract, particularly on the posterior urethra, but are rarely found in the bladder. We report a case of 15-year-old boy who presented with dysuria. Routine ultrasonography showed a mass in the bladder arising near the internal orifice of urethra. Further inspection with cystoscopy followed by transurethral resection and pathology confirmed the lesion to be a prostatic-type polyp. An overview of other similar case studies showed that the pathogenesis of this condition is controversial, haematuria and dysuria are common clinical symptoms and endoscopic transurethral resection is the best treatment option. Since the polyp is benign, recurrence and progression of this disorder is unlikely to occur.

Abstract:Leiomyosarcomas are rare, highly aggressive tumors of the urinary bladder. With approximately 200 cases reported in the literature, there is limited data on the prognosis and treatment of these neoplasms. Curative treatment approaches are primarily characterized by radical surgery, especially radical cystectomy. However, this procedure is associated with significant impairments in the quality of life for patients. This circumstance forms the basis for considering a curative treatment approach with partial cystectomy for a 19-year-old man with leiomyosarcoma of the urinary bladder.
Transurethral Resection of Prostate Treatment for Recurrence of a Multilocular Prostatic Cystadenoma: a Case Report

Abstract:Multilocular prostatic cystadenoma (MPC) is a rare benign tumor that originates from the prostate itself. MPC is usually characterized by large multilocular cysts located between the rectum and bladder. The clinical presentation includes obstructive voiding symptoms, such as poor stream, intermittency, sensation of incomplete emptying, acute urinary retention and sometimes constipation symptoms due to mechanical compression of the lower intestine. Most of the previously reported patients with MPC underwent open surgery. Although the natural history of MPC remains unknown, surgical excision may not always be necessary. Here we report the case of a 49-year-old male, treated by transurethral electroresection of prostate (TURP) for prostate cyst one and half years before．His biopsy of TURP showed benign prostatic tissue with no evidence of malignancy. However, the symptoms of urinary tract obstruction were obviously aggravated after the operation. Acute urinary retention occurred intermittently 3 times. In our hospital, his total prostate specific antigen (tPSA) was 5.440 μg/L, free prostate specific antigen (fPSA) was 1.528 μg/L. After examination, it was considered as benign lesions clearly. In the operation of TURP, we found that the tumor was multilocular cystic. Histologically, the cell was mucus. Concerning the immunophenotype, CK5/6(+) , p40(+), PSA(+), P504S(+), PAX-2(-), PAX-8(-), MUC1(+), MUC5ac(+), the results of special staining were as follows: AB(+), PAS(+). At the end of the follow up 3 months later, the routine semen analysis results showed that his semen volume was 3 mL and the sperm density and sperm mobility were normal. At the end of the follow up eight months later, the patient remained free of lower urinary tract symptoms and there were no signs of recurrence. His international prostate symptom score (I-PSS) had dropped from 32 to 4, and quality of life score (QOL) had dropped from 6 to 2. MPC is a rare benign tumor originating from the prostate. TURP may aggravate the symptoms of lower urinary tract obstruction in patients with MPC, and may be temporarily observed for some asymptomatic young and middle-aged patients.

Abstract:We report an unusual case of retrovesical ectopic prostate tissue in a 73-year-old man with primary prostate cancer. The man’s prostate-specific antigen was 24.66 ng/ml.Transabdominal ultrasonography, pelvic computed tomography,and pelvic magnetic resonance imaging demonstrated a heterogeneous 8.5 × 8.0 × 7.0 cm mass in contact with the posterior wall of the urinary bladder. The patient underwent a retropubic radical prostatectomy and resection of tumor. Pathological examination of prostate revealed a prostatic adenocarcinoma, Gleason score of 4 + 5 = 9, and the retrovesical tumor was confirmed to be a benign prostate tissue.

Abstract:Abstract Background Most bladder tumors are derived from the urothelium. Benign mesenchymal tumors are rare. Leiomyoma of the bladder is the most common benign neoplasm. We present a case of leiomyoma of the bladder presenting with acute urinary retention in a female patient and report on the post-operative change in urodynamic findings. To our knowledge, few cases of this kind have been reported. Case Presentation A 56-year-old woman presented with acute urinary retention. Evaluations including ultrasound, magnetic resonance imaging, cystoscopy, and urodynamics contributed to a diagnosis of leiomyoma of the bladder. Various medications were ineffective for solving her lower urinary tract symptoms; therefore, a transurethral resection was performed. The final pathological report was leiomyoma. After the operation, her symptoms resolved; this improvement was confirmed by an urodynamic analysis. The postoperative urodynamics demonstrated a lower frequency of detrusor overactivity during filling cystometry and an increase in the uroflow rate, with reduced detrusor pressure in a pressure flow study. Conclusions Leiomyoma of the bladder can cause female outlet obstruction. A review of the literature and disease management is discussed.

[A Case of Primary Adenocarcinoma Mucinous Subtype of the Bladder]

Abstract:A 48-year-old man who presented with asymptomatic gross hematuria in July 202X had been followed up without treatment. In January 202X, he was referred to our department due to the exacerbation of his hematuria. Contrast-enhanced magnetic resonance imaging revealed bladder cancer suggested bilateral seminal vesicle and prostate invasion, and enlarged right internal and external iliac lymph nodes. The pathological diagnosis was mucinous bladder adenocarcinoma. Prostate biopsy results were negative. Upper and lower gastrointestinal examinations were unremarkable. We suspected bladder cancer cT4aN2M0. In March 202X＋1, the patient underwent robotic-assisted laparoscopic total bladder resection, pelvic lymph node dissection, and intracorporeal urinary tract modification (ileal conduit creation). The final diagnosis was primary mucinous adenocarcinoma pT4aN2M0 of the bladder. Given the heightened risk of recurrence, the patient was administered a three-month course of oxaliplatin and capecitabine (XELOX) as adjuvant postoperative chemotherapy. The patient remains free of progression at 8 months postoperatively. Adenocarcinoma of the bladder is an exceedingly rare entity, with no established chemotherapeutic protocols. Primary mucinous adenocarcinoma of the bladder is even more exceptional. Presently, only regimens similar to those for colorectal cancer or adenocarcinoma of unknown primary, including 5-fluorouracil, are considered. In our particular case, we elected to pursue XELOX therapy, aligning with the principles governing the management of colorectal cancer.

Abstract:Abstract Background: Leiomyomas are the most common pelvic tumors in fertile women and rare conditions of them have extrauterine locations. Ovarian Leiomyoma accounts only for 0.5 to 1% of all benign ovarian tumors with fewer than 60 cases reported in a manner of case report. Here we present a series of eight patients over a six-year period(2012-2018). The clinico-pathological features, diagnosis and management were discussed. Case presentation: We had experienced eight cases of ovarian leiomyoma.The clinical features,pathologic findings,diagnosis, treatment were reviewed. The mean age of these patients was 38.5 years. The majority of these patients may be asymptomatic and usually diagnosed incidentally during pelvic examination or pathologic examination after surgery. Six cases presented only with pelvic mass sized from 2 to 20 cm and even some patients persisted for more than twenty years while the other two were diagnosed due to four years of primary infertile or three months of irregular vagina bleeding. Three patients coincident with uterine leiomyoma while the others not. Three had ovarian leiomyoma degenerated and one case had a evidence of atypical 4/10. Conclusions : ovarian leiomyoma was very rare but it should be considered in the diagnosis of pelvic or ovarian solid masses. Magnetic resonance imaging was provital in its different diagnose while fast frozen pathology during operation was very necessary for its surgical decision. A proper surgical decision should be made according to patients age.

Abstract:Objective:To study the pathological characteristics and clinical features of leiomyoma of the seminal vesicle and to improve the diagnosis and treatment.Methods:Retrospective analysis of 1 case of leiomyoma of the seminal vesicle were described,and then reviewed the related literature.Results:The patient was found a tumour with composite resonance which below the bladder and dorsiprostate by the B ultrasound;A cystic-solid mass was discovered in the left area of seminal vesicle through CT,and no intumescent lymphaden was found in cavitas pelvis.His tumor markers in blood-serum: PSA,CEA,CA125,CA199,SCC and AFP were all normal.At last,the tumor was diagnosised as Leiomyoma of the Seminal Vesicle after complete ablating.The patient recovered quickly after operation.Conclusions:Leiomyoma of the Seminal Vesicle is a scarce benign tumor of uropoiesis and reproductive system,and the clinical situation has no specificity.The diagnosis of this disease was depended on the histological characteristic and the immunohistochemistry marker primally,and needs to differential diagnosis with leiomyosarcoma and interstitialoma.Complete ablating by operation was the main treatment,and the prognosis was well and no recurrence commonly.
Combination of laparoscopy and open technique in management of large extravesical urinary bladder leiomyoma; a case report

Abstract:Introduction and importance: Leiomyoma is a rare benign bladder tumor, classified into intravesical, intramural and extravesical types according to the location. Because of the difficulty of accurate preoperative diagnosis, resection is performed in the majority of the cases. Case presentation: A 37-year-old Japanese man presented to the hospital with a chief complaint of abdominal swelling. Abdominal computed tomography (CT) revealed a large solid mass (20 cm in size) from the abdominal wall to bladder. The tumor was successfully removed by a combination of laparoscopic and open surgery. The histological diagnosis was compatible with leiomyoma, and the patient remained free from recurrence at 3 years after surgery. Clinical discussion: The possibility of urachal carcinoma could not be ruled out preoperatively because of the location and internal heterogeneous findings by contrast CT. Although imaging is useful in the diagnosis of leiomyoma, the need for histological examination for a conclusive diagnosis has been noted. Therefore, surgical intervention is reported as a major treatment option. In the present case, laparoscopic approach was performed in accordance with partial cystectomy. The procedure was useful for observation of the positional relationship between the tumor and adjacent intestinal organs, and antegrade resection was performed without incident. Conclusion: Laparoscopic approach may be a useful and safe procedure for the resection of extravesical bladder leiomyoma.

Abstract:Children can normally be expected to copy a circle at 3, a square at 4, and a triangle at 5 years of age on standard 81/2 x 11-inch paper. This study hypothesized that the shape of the background paper influences these norms. 53 nursery children copied each of these geometric forms on various-shaped background paper. The results indicated that copying a triangle on triangular paper was just as easy for these children as copying a square on a square piece of paper.

Abstract:A leiomyoma is a benign neoplasm composed of smooth muscle cells that commonly occurs in the genitourinary and gastrointestinal tracts. Primary intracranial leiomyoma, however, is extremely rare and only a few cases have been reported to date. In this study, we present two cases of primary intracranial leiomyomas in middle-aged men. Magnetic resonance images (MRI) showed lesions with homogeneous low signals on T1-, T2-, diffusion-weighted, and FLAIR sequences. Tumors were totally removed and there was no evidence of recurrence in the follow-up study. Pathological analysis with immunohistochemistry revealed that tumors had characteristics of benign smooth muscles. A review of relevant literature has been conducted.
