Abstract:Biochemical tests (serum glutamic pyruvic transaminase, serum glutamic oxaloacetic transaminase, alkaline phosphatase, gammaglutamyltranspeptidase, bilirubin, and serum amylase) were performed upon admission in 84 patients with suspected (36) or proven (48) acute pancreatitis at the time of the first episode of acute abdominal pain suspected clinically as acute pancreatitis. These parameters all increased significantly more in patients with gallstone pancreatitis. Among them, the SGPT was the most discriminant test between biliary and nonbiliary pancreatitis. The positive predictive value of SGPT was 92%, when the cutoff point was chosen at twice the upper limit of normal. In patients with increased SGPT, a SGOT-SGPT ratio less than 1 is the rule (88%) for those with gallstone pancreatitis. This enzymatic determination allowed us to select more accurately the patients suitable for morphological procedures to confirm the biliary origin of the pancreatitis.
Use of amylase and alanine transaminase to predict acute gallstone pancreatitis in a population with high HIV prevalence

Abstract:Background: The etiology of acute pancreatitis can be difficult to determine early in the course of the disease. The aim of the present study was to determine the relationship between biochemical parameters and the cause of acute pancreatitis. Methods: A prospectively collected database of patients admitted with acute pancreatitis between 2001 and 2008 was analyzed. The relationships between etiology and age, sex, admission serum amylase level, and admission alanine transaminase (ALT) level were evaluated. Results: Acute pancreatitis was diagnosed in 464 patients. The disease was related to alcohol in 275 cases, gallstones in 81 cases, HIV in 49 cases, dyslipidemia in 42 cases, and it was idiopathic in 17 cases. Alcoholic pancreatitis was more common in men, whereas the other identifiable causes were more common in women (P < 0.001). Mean age at presentation was 39 years with no difference in age in relation to etiology (P = 0.057). When compared to patients with non-biliary causes of pancreatitis, patients with gallstone pancreatitis had greater median (range) serum amylase activity [1,423 U/l (153-7,500 U/l) versus 589 U/l (58-11,144 U/l); P < 0.001] and ALT activity [153 U/l (8-13,233 U/l) versus 31 U/l (6-421 U/l); P < 0.001]. No significant differences in amylase or ALT activity were found between non-biliary etiologies (P > 0.05). Alanine transaminase was the only factor independent of sex to predict gallstone etiology, with activity >150 U/l having a specificity of 97 %. Conclusions: In patients with acute gallstone pancreatitis, the serum amylase and ALT activities were more than double those of other etiologies. An ALT level of >150 U/l was highly predictive of gallstone etiology independent of gender. Neither amylase nor ALT could differentiate non-biliary etiologies. The combination of amylase and ALT offered no improvement in predicting etiology over each alone.
Serum Cys C Predicts Acute Kidney Injury in Patients with Acute Pancreatitis: A Retrospective Study.

Abstract:Background and study aims: We investigated the value of the serum cystatin C level as a potential predictor of acute kidney injury (AKI) in patients with acute pancreatitis (AP).Patients and Methods: We retrospectively examined patients diagnosed with AP between January 2013 and December 2018. Patients were categorized into two groups based on their serum cystatin C levels after admission: the normal (n-Cys C group) and high serum cystatin C levels groups (h-Cys C group). Patients in the h-Cys C group demonstrated serum cystatin C levels >= 1.05 mg/L. Demographic parameters, laboratory data, and AP severity were compared between the two groups. Receiver operating curve (ROC) analysis was used to evaluate the efficacy of serum cystatin C in predicting persistent AKI.Results: A total of 379 patients with AP were enrolled: 319 in the n-Cys C group and 60 in the h-Cys C group. Serum cystatin C levels were significantly higher in patients with severe acute pancreatitis (SAP) compared to moderate acute pancreatitis (MAP) (P< 0.05). The h-Cys C group had a higher BISAP score (P < 0.001). Incidences of organ failure and SAP were significantly higher in the h-Cys C group (P < 0.05). ROC analysis indicated that a serum cystatin C cutoff point of 1.055 mg/L optimally predicted persistent AKI (AUC = 0.711). For internal validation, we selected 545 AP patients, treated at our center from 2019 to 2022, including 54 AKI patients. ROC analysis in this validation group yielded a sensitivity of 100% and specificity of 90.9% (AUC = 0.916, 95% CI: 0.894-0.937).Conclusion: Elevated serum cystatin C levels are sensitive indicators of adverse AKI prognosis in AP patients. The cystatin C level at admission can reflect a patient's initial renal function status.
Suboptimal Specificity of Asge High-Risk Group in Evaluation of Patients with Suspected Choledocholithiasis

Abstract:Introduction: Patients who are considered high-risk for choledocholithiasis after biochemical testing and abdominal ultrasound (AUS) are recommended to undergo endoscopic retrograde cholangiopancreatography (ERCP) with others to receive less invasive tests. The aim of the study is to determine whether high-risk patients by ASGE guidelines or other factors can accurately identify those with choledocholithiasis.Table 1: Diagnostic Performance of ASGE High-Risk Group and Other Test Characteristics in Patients with Suspected Choledocholithiasis ( N =2,726)Methods: Consecutive patients hospitalized (1/2011-12/2013) with suspected choledocholithiasis who received biochemical testing, AUS and definitive testing for choledocholithiasis (magnetic resonance cholangiopancreatography (MRCP), endoscopic ultrasound (EUS), ERCP, interventional radiology procedure and/or intraoperative cholangiogram (IOC)) at Sir Run Run Shaw Hospital were retrospectively identified. Patients with choledocholithiasis on AUS, bilirubin >4mg/dL, bilirubin >1.8mg/dL + dilated common bile duct (CBD) (>8mm and >6mm with or without prior cholecystectomy) and/or suspected cholangitis (presenting fever and/or leukocytosis) were considered high-risk per ASGE guidelines. Final diagnosis was based on choledocholithiasis identified on definitive testing. Results: Mean age of 2,726 patients with suspected choledocholithiasis was 55+15 yrs, 1,179 (43%) were male and 216 (8%) had prior cholecystectomy. On definitive testing (MRCP in 2,441 (90%), EUS in 69 (3%), interventional radiology procedure in 2, ERCP in 643 (23%), IOC in 189 (7%)), 1,076 (39%) patients had choledocholithiasis. A higher proportion of patients meeting ASGE high-risk criteria compared to other groups (770/1,271 (61%) vs. 306/1,455 (21%); OR=5.8, 95%CI 4.9-6.8) had confirmed choledocholithiasis. Diagnostic performance of test characteristics is shown (Table). On multivariate analysis, choledocholithiasis on AUS (AOR=17.4, 95%CI 12.6-23.9), bilirubin >4mg/dL (AOR=3.0, 95%CI 2.2-4.1) and bilirubin 1.8-4mg/dL (AOR=1.8, 95%CI 1.4-2.4) compared to < 1.8mg/dL and CBD dilation (AOR=1.8, 95%CI 1.5-2.2) increased the likelihood of choledocholithiasis. Conclusion: In patients with suspected choledocholithiasis, the specificity of high-risk group categorized by ASGE guidelines was suboptimal. Combination of factors that include choledocholithiasis on AUS and bilirubin >4mg/dL with or without CBD dilation had high specificities of >90% and moderate to high positive likelihood ratios. A revised algorithm with more stringent criteria may improve patient selection and reduce the risk of diagnostic ERCP.
Elevated Serum HbA1c Level, Rather Than Previous History of Diabetes, Predicts the Disease Severity and Clinical Outcomes of Acute Pancreatitis

Abstract:IntroductionThe aim of our study is to explore the value of serum glycosylated hemoglobin A1c (HbA1c) in disease severity and clinical outcomes of acute pancreatitis (AP).Research design and MethodsPatients with AP were included from January 2013 to December 2020, retrospectively, dividing into normal serum HbA1c level (N-HbA1c) group and high serum HbA1c level (H-HbA1c) group according to the criteria HbA1c <6.5%. We compared patient characteristics, biochemical parameters, disease severity, and clinical outcomes of patients with AP in two groups. Besides, we evaluated the efficacy of serum HbA1c to predict organ failure (OF) in AP patients by receiver operating curve (ROC).ResultsWe included 441 patients with AP, including 247 patients in N-HbA1c group and 194 patients in H-HbA1c group. Serum HbA1c level was positively correlated with Atlanta classification, systemic inflammatory response syndrome, local complication, and OF (all p<0.05). Ranson, BISAP (bedside index of severity in acute pancreatitis), and CT severity index scores in patients with H-HbA1c were markedly higher than those in patients with N-HbA1c (all p<0.01). ROC showed that the best critical point for predicting the development of OF in AP with serum HbA1c is 7.05% (area under the ROC curve=0.79). Logistic regression analysis showed H-HbA1c was the independent risk factor for the development of OF in AP. Interestingly, in patients with presence history of diabetes and HbA1c <6.5%, the severity of AP was significantly lower than that in H-HbA1c group. Besides, there was no significant difference between with and without history of diabetes in N-HbA1c group.ConclusionsGenerally known, diabetes is closely related to the development of AP, and strict control of blood glucose can improve the related complications. Thus, the level of glycemic control before the onset of AP (HbA1c as an indicator) is the key to poor prognosis of AP, rather than basic history of diabetes. Elevated serum HbA1c level can become the potential indicator for predicting the disease severity of AP.
Non-ALT Biomarkers for Markedly Abnormal Liver Histology among Chinese Persistently Normal Alanine Aminotransferase-Chronic Hepatitis B Patients.

Abstract:AIM:To determine incidence and clinical biomarkers of marked necroinflammation and fibrosis characteristics among chronic hepatitis B (CHB) patients with persistently normal alanine aminotransferase (PNALT).METHODS:Liver biopsy was performed on 115 CHB patients with PNALT. Necroinflammation and fibrosis were graded by the Knodell histologic activity index and the Ishak fibrosis score, respectively. Correlations between the available clinical parameters and necroinflammation and fibrosis were analysed.RESULTS:Marked necroinflammation (Knodell activity index ≥ 7) and fibrosis (Ishak fibrosis score ≥ 3) were found in 36.5% and 15.5% of CHB patients with PNALT, respectively. Following a univariate logistic regression analysis, multiple logistic regression analysis indicated that aspartate transaminase (AST) (AUROC = 0.852, cut-off value = 22.5 U/L) serves as an independent predictor of notable liver inflammation, while platelet (PLT) count (AUROC = 0.905, cut-off value = 171.5 ×109/mL) and gamma-glutamyl transpeptidase (GGT) (AUROC = 0.909, cut-off value = 21.5 U/L) level serve as independent predictors of notable liver fibrosis.CONCLUSION:A considerable proportion of marked histological abnormalities existed in our cohort, who will benefit from optimal therapeutic strategies administered according to predictive indication by AST, PLT and GGT levels.
Analysis of Correlation Between Liver Function and Severity of Acute Pancreatitis

Abstract:Objective To explore the clinical value of liver function indicators in predicting the severity of disease in patients with acute pancreatitis (AP),and to provide a reference for clinical treatment.Methods A retrospective study was conducted and 142 patients with acute pancreatitis were included.The disease severity was evaluated according to the RANSON score and the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score,and severe pancreatitis was used as the outcome index.Univariate logistic regression analysis and receiver operating characteristic (ROC) curve analysis were performed on single indicators that were statistically significant between mild and severe patients to determine the distinguishing value for disease severity.Results The RANSON score was graded:43 cases of severe patients (RANSON≥3) and 99 cases of mild patients (RANSON ＜3);the difference in albumin (ALB) levels between the two groups was statistically significant (P ＜ 0.05).Univariate logistic regression analysis and ROC curve analysis were performed on ALB (OR =0.88,P ＜0.001).The area under the ROC curve and its 95％ CI were 0.73 (0.63,0.83).APACHE Ⅱ score classification:94 cases of severe patients (APACHE Ⅱ score≥8) and 48 cases of mild patients (APACHE Ⅱ score ＜8).The difference in alanine aminotransferase (ALT) levels in the two groups were statistically significant (P ＜0.05).Univariate factor logistic regression analysis and ROC curve analysis were performed on ALT (OR =1.001,P =0.314).The area under the ROC curve and its 95％ CI were 0.61 (0.50,0.71).Conclusions ALB and ALT are valuable biomarkers for predicting the severity of AP.
Diagnostic value of hematological parameters in acute pancreatitis

Abstract:Background: Acute pancreatitis (AP) is an inflammatory disease with rapid onset and rapid progression. In severe cases, it can cause systemic inflammatory response syndrome (SIRS), multiple organ failure (POF) and death. In order to explore the effective index of early diagnosis of AP, we have detected the levels of serum lipase (LPS), serum amylase (S-AMY) and C-reactive protein (CRP). Meanwhile, cost evaluation was performed on these biochemical parameters for AP patients. Methods: In this study, 212 patients were admitted with acute abdominal pain, divided into AP group (n=107) and non-AP group (n=105), including cholelithiasis (n=8), intestinal obstruction (n=34), acute cholangitis (n=14), acute cholecystitis (n=20), acute appendicitis (n=20) and pancreatic cancer (n=9). Demographic data and laboratory examination results were collected, the sensitivity and specificity of LPS, S-AMY and CRP in the diagnosis of AP were calculated respectively, as well as the costs of single or multiple tests were evaluated. Results: The serum level of S-AMY and LPS in AP group were significantly higher than those in nonAP group (P<0.01), The serum level of CRP in AP group was higher than that in non-AP group (P<0.05). When the critical value of the normal reference value is three times as the critical value, the sensitivity and specificity of the LPS in diagnosing AP were 91.6% and 90.5%, S-AMY was 70.1% and 91.4%. And CRP was 59.8% and 56.2%, respectively. Compared with the trend of serum LPS expression, the expression trend of S-AMY was positively correlated (r=0.81), while there was no correlation with the trend of CRP expression. Conclusions: The ability of LPS to diagnose AP is significantly better than that of LPS + CRP, as well as the combined detection of LPS + CRP + S-AMY, which can be used as the first choice for laboratory examination of AP.

Abstract:When a cylindrical pure electron plasma is displaced from the center of the trap, it performs a bulk circular orbital motion known as the l=1 diocotron mode. The slow application of a perturbing potential to a patch on the trap wall distorts the orbit into a noncircular closed path. Experiments and a simple theoretical model indicate that the area enclosed by the loop is an adiabatic invariant. Detailed studies are made of the breaking of the invariant when perturbations are rapidly applied. Abstract:Research in schizophrenia illustrates that the field of psychophysiology lies at the interface of clinical science, cognitive science, and neuroscience. Electrodermal abnormalities in schizophrenic patients have been found reliably across laboratories, and data reported here demonstrate that these abnormalities are at least partially dependent upon the clinical state of the patients. Both tonic and phasic electrodermal activity increased when schizophrenic patients entered into a psychotic episode, and preliminary evidence suggests that the increases may serve as early prodromal signs of an impending episode. Significance of the electrodermal abnormalities for understanding cognitive and attentional deficits is suggested by associations with secondary reaction time and startle blink probe findings. Neuroscience implications are highlighted by the correlation of the electrodermal abnormalities with regional brain metabolic activity indexed by positron emission tomography. An important task for the future is to interrelate measures from these various domains. The field of psychophysiology is particularly well positioned for this task.
Changes in the Velocity of Blood in the Portal Vein in Mild Acute Pancreatitis—A Preliminary Clinical Study

Abstract:Background and objective: Portal vein thrombosis is associated with a decrease in the main blood velocity in this vessel. While most studies examine etiological factors of portal vein thrombosis after its occurrence, we aimed to evaluate portal vessels and assess whether mild acute pancreatitis affects blood flow in the portal vein and increases the risk of thrombosis. Materials and methods: This prospective single centered follow-up study enrolled 66 adult participants. Fifty of them were diagnosed with mild acute pancreatitis based on the Revised Atlanta classification, and 16 healthy participants formed the control group. All participants were examined three times. The first examination was carried out at the beginning of the disease and the next two at three-month intervals. Blood samples were taken and color Doppler ultrasound performed the first time, whereas ultrasound alone was performed during the second and third visits. Mean and maximal blood velocities and resistivity index in the main portal vein and its left and right branches were evaluated. Results: Mean velocity of the blood flow in the main portal vein and its right and left branches was not significantly different from healthy individuals during the acute pancreatitis phase: 23.1 ± 8.5 cm/s vs. 24.5 ± 8.2 cm/s (p = 0.827); 16.4 ± 7.9 cm/s vs. 16.4 ± 8.1 cm/s (p = 1.000); and 8 ± 3.4 cm/s vs. 7.4 ± 2.5 cm/s (p = 0.826), respectively. The same was observed when comparing the maximal blood flow velocity: 67.9 ± 29 cm/s vs. 67.5 ± 21 cm/s (p > 0.05); 45.4 ± 27 cm/s vs. 44 ± 23.8 cm/s (p = 0.853); and 22.2 ± 9.8 cm/s vs. 20 ± 7.3 cm/s (p = 0.926), respectively. Changes in venous blood velocities were not significant during the follow-up period in separate study groups. Conclusions: Portal blood flow velocities do not change during mild acute pancreatitis in the inflammatory and postinflammatory periods. This observation suggests that mild acute pancreatitis does not increase the risk of portal vein thrombosis.
Accuracy of prediction of fresh gas flow requirements during spontaneous breathing with the T-piece.

Abstract:In 40 spontaneously breathing children (7.3-47.9 kg) anaesthetized with halothane for minor surgical procedures the fresh gas flow (FGF) at onset of rebreathing (FGFr) was determined and end-tidal CO2 concentration (ETCO2), minute ventilation (VE), tidal volume (VT) and respiratory rates (f) were registered. The accuracy of predicting the FGFr from 2 X VE, 3 X VE and from two formulae (FGF = 15 X kg X f and FGF = 3 X (1000 + 100 X kg) was evaluated. FGFr ranged from 3.5 to 10 l min-1. FGF calculated from 2 X VE was inadequate. Calculations of FGF from 3 X VE and with the two formulae gave an adequate FGF in more than 80% of the children. No serious under-estimations were found. In a few cases FGF level was overestimated by more than 150%. It is suggested that when the theoretical calculation of FGF results in flow rates well over 10 l min-1 an upper flow rate limit of 10 l min-1 may be used in children weighing less than 30 kg since no child required a FGF over this rate.
Factors affecting measurement of urinary amylase after bladder‐drained pancreas transplantation

Abstract:Abstract: Urinary amylase activity (UA) was evaluated in 14 diabetic patients receiving bladder‐drained whole pancreas transplants. Amylase was dependent upon pH with optimal activity at pH 7. Increasing acidity led to a substantial loss of measured UA (45% remaining at pH 4, 73% at pH 5, 86% at pH 6, p < 0.001); however, UA was stable at room temperature if the pH was kept constant. On the day after transplantation, a transient peak in UA was followed by a trough lasting a median of 3 days. Thereafter the UA rose steadily, with the rate of rise being faster if the pancreas donor was less than 30 years of age (p < 0.05). Maximal UA values were observed between 3‐6 months after transplantation. High protein and fat liquid test meals, designed to stimulate exocrine secretion, produced an 83% rise in UA (p = 0.02), which was not observed when water was given (n = 8, p = 0.007, differences at 1 hour). Fasting for 17.6 ± 2.1 h (mean ± SEM) produced a 29 ± 6% fall in 24‐h UA, without any measurable change in other parameters of renal or pancreatic function. A fall of 50% in 24‐h UA over 7 d was compared with pancreas biopsy (n = 40) for predicting rejection, and showed a sensitivity of 60% and a specificity of 93%. In 3 patients a false‐negative result occurred in the first few days after transplantation at a time when the UA failed to rise as normally expected. Understanding and eliminating some of the factors which cause variability in UA may improve the interpretation of 24‐h UA measurements in bladder‐drained pancreas allografts.

Abstract:Background: Occult biliary stones escape detection on conventional investigations, and clinico-biochemical systems proposed for predicting biliary pancreatitis has low predictive values. Aim: To evaluate the accuracy of clinico-biochemical parameters for prediction of biliary pancreatitis in patients undergoing endoscopic ultrasonography. Methods: Early endoscopic ultrasonography was performed on 139 patients presenting with acute pancreatitis within 24 h of admission. The aetiologies of all patients were determined after complete evaluations, and clinico-biochemical characteristics of patients with a biliary cause (biliary group) and non-biliary causes (non-biliary group) were compared. Results: Biliary pancreatitis was diagnosed in 107 patients and 32 patients had non-biliary causes. The biliary group belonged to a significantly older age group, had a female predominance, significantly more derangement of liver function and a higher incidence of severe attack of acute pancreatitis. On multivariate analysis, female sex, age >58 years and serum alanine aminotransferase >150 U/L were independent predictive factors for biliary cause of acute pancreatitis. Using these three factors for prediction of biliary cause, the sensitivity was 93% and overall accuracy was 85%. Conclusion: Clinico-biochemical prediction for biliary cause of acute pancreatitis improves in the era of endoscopic ultrasonography with a higher sensitivity and overall accuracy. In centres where endoscopic ultrasonography is inaccessible or local expertise is unavailable, clinico-biochemical prediction of biliary cause of acute pancreatitis may provide a useful alternative in the initial management of this group of patients.

Abstract:BACKGROUND: Early detection of pancreatic necrosis allows better management of the disease. Contrast-enhanced computed tomography (CT) as the gold standard for detecting pancreatic necrosis is expensive.AIM OF THE STUDY: This study was to evaluate for the first time whether blood glucose estimation on hospital admission--a simple, cheap, readily available laboratory parameter--may detect pancreatic necrosis and have prognostic value in acute pancreatitis.METHODS: Single blood glucose estimation upon hospital admission was evaluated prospectively for detecting pancreatic necrosis and as a prognostic indicator. The study included 241 nondiabetic patients with a first attack of acute pancreatitis. All underwent CT within 72 h of admission.RESULTS: High blood glucose (> 125 mg/dl) correlated significantly with complex high clinical and biochemical prognostic scores (Ranson, Imrie), a high Balthazar score, pancreatic pseudocysts, and a long hospital stay, but not with organ failure, indication for artificial ventilation, dialysis, surgery, length of intensive care, and mortality. Pancreatic necrosis detection sensitivity of high blood glucose was 83%, specificity 49%, positive predictive value 28%, and negative predictive value 92%.CONCLUSION: A patient with normal blood glucose on admission is unlikely to have pancreatic necrosis. Contrast-enhanced CT would not be needed unless the patient fails to improve.

Abstract:Abstract Drug-induced liver injury (DILI) is a challenge in clinical medicine and drug development. Highly sensitive novel biomarkers have been identified for detecting DILI following a paracetamol overdose. The study objective was to evaluate biomarker performance in a 14-day trial of therapeutic dose paracetamol. The PATH-BP trial was a double-blind, placebo-controlled, crossover study. Individuals (n = 110) were randomized to receive 1 g paracetamol 4× daily or matched placebo for 2 weeks followed by a 2-week washout before crossing over to the alternate treatment. Blood was collected on days 0 (baseline), 4, 7, and 14 in both arms. Alanine transaminase (ALT) activity was measured in all patients. MicroRNA-122 (miR-122), cytokeratin-18 (K18), and glutamate dehydrogenase (GLDH) were measured in patients who had an elevated ALT on paracetamol treatment (≥50% from baseline). ALT increased in 49 individuals (45%). All 3 biomarkers were increased at the time of peak ALT (K18 paracetamol arm: 18.9 ± 9.7 ng/ml, placebo arm: 11.1 ± 5.4 ng/ml, ROC-AUC = 0.80, 95% CI 0.71–0.89; miR-122: 15.1 ± 12.9fM V 4.9 ± 4.7fM, ROC-AUC = 0.83, 0.75–0.91; and GLDH: 24.6 ± 31.1U/l V 12.0 ± 11.8U/l, ROC-AUC = 0.66, 0.49–0.83). All biomarkers were correlated with ALT (K18 r = 0.68, miR-122 r = 0.67, GLDH r = 0.60). To assess sensitivity, biomarker performance was analyzed on the visit preceding peak ALT (mean 3 days earlier). K18 identified the subsequent ALT increase (K18 ROC-AUC = 0.70, 0.59–0.80; miR-122 ROC-AUC = 0.60, 0.49–0.72, ALT ROC-AUC = 0.59, 0.48–0.70; GLDH ROC-AUC = 0.70, 0.50–0.90). Variability was lowest for ALT and K18. In conclusion, K18 was more sensitive than ALT, miR-122, or GLDH and has potential significant utility in the early identification of DILI in trials and clinical practice.

Abstract:Background and Aim The development of acute pancreatitis (AP) is strongly linked to blood clotting and fibrinolysis issues. Modern clinical practices now utilize advanced blood markers like thrombin–antithrombin III complex (TAT), plasmin‐α2‐plasmin inhibitor complex, thrombomodulin (TM), and tissue plasminogen activator–inhibitor complex (t‐PAIC) to assess thrombosis risk. Our study used a highly sensitive chemiluminescence technique to measure these markers in AP patients, aiming to determine their early predictive value for AP severity. Methods There were 173 patients with AP, all of whom developed symptoms within 72 h; 102 individuals had onset symptoms within 48 h. The biomarkers were measured upon admission before determining the severity of AP. Results The levels of TAT, plasmin‐α2‐plasmin inhibitor complex, TM, and t‐PAIC were significantly higher in the severe acute pancreatitis (SAP) group compared with the mild acute pancreatitis and moderate severe acute pancreatitis groups. For the patients within 72 h of onset, TAT, TM, and t‐PAIC predicted the occurrence of SAP. For the patients within 48 h of onset, TAT and t‐PAIC predicted the occurrence of SAP. Abstract:BackgroundAmylase (AMY) is a biomarker that comes mainly from the pancreas and salivary glands. Macroamylasemia is a condition often accompanied by malignant tumors and clinical features that include: (i) recurrent abdominal pain, no abnormality in the salivary glands, serum amylase (AMY) level ≥2 to 3 times higher than the normal value and poor patient response to treatments for pancreatitis; (ii) normal serum creatinine and urea levels, increased serum AMY level, and urine AMY level within the normal range or slightly lower. Because macroamylasemia may cause abnormally increased serum AMY levels for a long period of time, misdiagnosis of pancreatitis and delayed diagnosis/treatment of the disease may occur. We report a case of lung adenocarcinoma with macroamylasemia that was co-diagnosed using wet chemistry with kinetic method and dry chemistry to measure AMY.MethodsA 47-year-old male patient was previously diagnosed with "acute pancreatitis and lung infection" after initial admission to a local hospital with paroxysmal cough and abdominal pain without obvious cause. To further diagnose the patient, we determined the blood AMY level by using the wet chemistry with kinetic method and the Vitros® dry chemistry test, in addition to conducting a physical exam, abdominal and chest computed tomography (CT), and other lab tests. Two methods, calculation of amylase/creatinine clearance ratio (Cam/Ccr ratio) and protein precipitation by polyethylene glycol (PEG), were also performed to validate assumed interference by serum macroamylase (MAMS) in the wet chemistry method compared to the dry chemistry method, which has technology to capture large molecules and minimize the impact of interferents. MAMS is a high-molecular-weight AMY complex that is synthesized by normal serum AMY bound with macromolecular substances (eg, immunoglobulins and carbohydrates).ResultsThe final diagnosis was a stage IIB cT3N0M0 adenocarcinoma of the upper lobe of the right lung and macroamylasemia. Symptoms and radiologic findings were atypical in terms of pancreatitis. Serum AMY was 2,178U/L when measured with the wet chemistry with kinetic method. When the specimen was re-tested with Vitros dry chemistry, the AMY level was 369 U/L. After precipitation with 25% PEG 6000 solution, the blood AMY level from wet chemistry decreased from 2395.1 U/L to 246.7 U/L, yielding a precipitation rate of 89.7%. Cam/Ccr ratio was calculated as 0.23% (<1%). The calculated PEG precipitation rate and Cam/Ccr ratio from this study were consistent with the ranges considered for macroamylasemia, which are ≥73% and Cam/Ccr ratio <1%, respectively.ConclusionsHigh serum AMY levels do not necessarily indicate pancreatitis or parotitis; wet chemistry results can be impacted by the presence of serum macroamylase. Instead of relying solely on blood AMY in diagnosing acute pancreatitis, clinicians should also consider the correlations of blood AMY with urine AMY and blood lipase. In patients with prolonged blood AMY elevation but normal lipase and urine AMY, re-testing of AMY with a more interference-resistant methodology (e.g. Vitros dry chemistry) may be requested. Accordingly, Cam/Ccr ratio calculation and PEG precipitation can be performed to validate the diagnosis of macroamylasemia.

Abstract:The aim of the present study was to investigate the expression of microRNA (miRNA)-372 in the serum of patients with hyperlipidemic acute pancreatitis (HTGAP), and its clinical significance. Patients with a serum lipid concentration ≥11.3 or 5.65-11.3 mmol/l with chylous serum were included in group A (n=40). The remaining patients did not have HTGAP and were included in group B (B). A further 25 patients with hyperlipidemia, but not AP (group C), and 30 healthy volunteers (group D) were recruited as controls. The level of miR-372 in the serum of group A (4.76±2.60) was significantly increased compared with groups B (0.98±0.80), C (0.85±0.62) and D (0.76±0.44); however, there was no significant difference in the expression of miR-372 between groups B, C and D. The expression level of miR-372 was significantly increased in the severe HTGAP group (6.45±2.20) compared with the mild HTGAP group (3.08±1.74). Further experiments suggested that the expression level of miR-372 was positively correlated with the level of triacylglycerol (r=0.666; P<0.001) but not with the level of amylase (r=-0.145; P>0.05). ROC analysis indicated that the combined use of miR-372 expression levels and Acute Physiology and Chronic Health Evaluation II scoring improved the diagnostic value for HTGAP. In summary, the expression of miR-372 in HTGAP was significantly upregulated and increased with the severity of the disease. The results of the present study may provide a novel strategy for the diagnosis and severity assessment of HTGAP in the clinic.
