Abstract:Objectives: The Aga Khan University in Karachi has a mission to educate leaders and to contribute to the development of health systems for Pakistan amid challenges of scarcity and complexity. Methods: Its key activities are (1) to design and test urban and rural health system prototypes, (2) to develop faculty in medical and nursing postgraduate community health sciences programs, and (3) to design and implement community-based undergraduate medical and nursing curricula. Results: The university has developed equity-based, cost-effective primary health care prototypes in Karachi slums. With government counterparts it has tested village-, facility-, and district-level interventions in a poor rural district. Federal policymakers have taken models from each for widespread replication. The university is training 49 medical and 19 nursing faculty for postgraduate programs in community health sciences. Most faculty retain institutional leadership positions, including teaching community-based, problem-solving, community health sciences as 20% of the medical and nursing undergraduate curriculum. Conclusions: The mission and experience of the Aga Khan University in population-based health systems design and health sciences education can guide universities in both developing and developed countries.

Established in 1983 as the first private university in Pakistan, AKU has evolved into a global institution with a focus on improving healthcare standards and addressing healthcare needs in the developing world. The article also discusses the undergraduate and postgraduate medical education programs at AKU Medical College, Pakistan, highlighting their unique features and pioneering approaches to medical education. The institution’s journey highlights its ability to adapt to the evolving healthcare landscape while maintaining a focus on quality and excellence, offering a model for other institutions striving to meet healthcare needs in low- and middle-income countries.

HISTORY MATTERS: THE ORIGINS OF BIOPHILIC DESIGN OF INNOVATIVE LEARNING SPACES IN TRADITIONAL ARCHITECTURE

Abstract:Recent empirical studies have shown a positive correlation between nature, the built environment and creativity in the human brain. During the medieval Islamic Golden Era, higher education buildings of non-medical ‘madrasa’ and medical ‘bimaristan’ institutions applied specific techniques and strategies so that human intellectual curiosity could flourish through direct and indirect contact with nature. In contrast, the architecture of modern universities has lessened students’ multisensory focus and engagement with nature. Several studies have addressed these institutions’ failure to foster the innovation-generation process. This systematic review summarises and synthesises previous studies, elaborating the characteristics of those spaces that can host Innovation-Generation Processes (IGPs) based on psychological and neurological investigation. The study analyses research outcomes that support the stimulative impact of nature on people’s cognitive capacities. This demonstrates that the biophilic design approach utilises natural conditions and elements within the built environment to enhance the physical, social, intellectual and psychological status of innovators. The findings of this study demonstrate a strong interrelationship between IGPs and the built environment in traditional higher education institutions based on the premise of biophilic design. Hence, we can adopt some lessons from these ‘timeless’ buildings to support the evolution of innovative university campuses today.
Enhancing medical training in conflict zones and remote areas through innovation: introducing the Canadian Virtual Medical University Initiative

Abstract:Background: The WHO projects a global shortage of 4.3 million physicians by 2030, with the largest deficits in developing and conflict-affected regions. Our aim is to train competent physicians rapidly and affordably through remote education programs. Methods: We developed an online medical training curriculum with four levels, focusing on different aspects of human body systems using a competency-based, student-centered approach. This study evaluates the first three levels; level four (internship) is outside this scope. The 105 medical students from eight Afghan universities were randomly assigned to nine groups. The curriculum includes Entrustable Professional Activities (EPA) for the cardiovascular system: level 1 covers basic medical sciences, level 2 pathology and basic clinical skills, and level 3 full clinical competencies. EPAs were delivered asynchronously online via Lecturio, CyberPatient, and Zoom. The 30-day intervention included 4 h of weekly online classes for formative assessment, collaborative learning, and evaluation, supervised by medical faculty members. Virtual pre- and post-intervention evaluations used multiple-choice questions and objective structured clinical examination (OSCE). We also conducted a satisfaction survey and open interview forum. Data triangulation from observations, surveys, and interviews validated curriculum effectiveness. The benchmarking method assessed cost-effectiveness. Findings: Pre- and post-intervention analysis showed a significant increase in clinical competencies and knowledge acquisition (P < 0.0001). The CyberPatient intervention improved clinical competency quality (P < 0.0001) and shortened decision-making time (P < 0.001). Cost analysis revealed that a virtual medical university would be 95% more cost-effective than traditional medical education. Interpretation: Integrating virtual technology with modern curriculum concepts in pre-internship years can effectively address healthcare training gaps and enhance education quality for healthcare workers at a low cost. Indigenous Preferences for Inpatient Rooms in Australian Hospitals: A Mixed-Methods Study in Cross-Cultural Design

Abstract:Aim: This mixed-method study aimed to establish preliminary evidence for spatial and design features that can improve the experience and participation of Indigenous inpatients in healthcare. Background: Disadvantaged across a range of health measures, a disproportionately high number of Indigenous people leave hospital without receiving appropriate medical care. Australian government policies to improve cultural safety of Indigenous patients have largely ignored physical settings and their potential to improve health outcomes. Despite increasing evidence on the potential of design to reduce patient stress, there is minimal research on cross-cultural design in health facilities, including for Indigenous Australians. Methods: A cross-sectional, area-based survey elicited design preferences of four healthcare settings from Indigenous participants ( n = 602). On the screen-based survey, participants selected from paired images that indicated their preferences for room layout and features of inpatient rooms. Semistructured in-depth interviews ( n = 55) explored meanings behind preferences. Results: Participants showed majority preferences for the two-bed patient room, for a balcony rather than a window only, Indigenous art, and view of a park over an urban environment. Analysis of qualitative data shifted the focus from Ulrich’s three supportive design components to cultural recognition, the desire for company of family members, and connection to life outdoors. Conclusions: Social and cultural factors were highly significant to patients for sense of control, family support, and positive distraction. In response, inpatient room size and type, views, access to outdoors and Indigenous art need to be considered in hospital design for Indigenous patients and families.

Abstract:PurposeToday’s hospitals are designed as collections of individual departments, with limited communication and collaboration between medical sub-specialties. Patients are constantly being moved between different places, which is detrimental for patient experience, overall efficiency and capacity. Instead, we argue that care should be brought to the patient, not vice versa, and thus propose a novel hospital architecture concept that we refer to as Patient Hub. It envisions a truly patient-centered, department-less facility, in which all critical functions occur in the same building and on the same floor.MethodsTo demonstrate the feasibility and benefits of our concept, we selected an exemplary patient scenario and used 3D software to simulate resulting workflows for both the Patient Hub and a traditional hospital based on a generic hospital template by Kaiser-Permanente.ResultsAccording to our workflow simulations, the Patient Hub model effectively eliminates waiting and transfer times, drastically simplifies wayfinding, reduces overall traveling distances by 54%, reduces elevator runs by 78% and improves access to quality views from 67 to 100% for patient rooms, from 0 to 100% for exam rooms and from 0 to 38% for corridors. In addition, the interaction of related medical fields is improved while maintaining the quality of care and the relationship between patients and caregivers.ConclusionWith the Patient Hub concept, we aim at rethinking traditional hospital layouts. We were able to demonstrate, alas on a proof-of-concept basis, that it is indeed feasible to place the patient at the very center of operations, while increasing overall efficiency and capacity at the same time and maintaining the quality of care.

Abstract:Background and aim: Despite their flaws, the low-cost but powerful economical solutions can ensure everyone has access to health. The main aim of this study is to extract characteristics of frugal innovation (FI) and social innovation (SI) for Primary Health Centers (PHCs) in low resource settings (LRS) for sustainable development. We will use the gained insights to design the mobile primary healthcare infrastructure using FI and SI strategies. There is a lack of methodology to design sustainable healthcare infrastructure for LRS. There is a gap in the literature about building sustainable infrastructure to provide basic healthcare facilities essential to the community. This article studies several factors necessary for designing sustainable infrastructure from the lens of FI, SI, and sustainability to develop a mobile healthcare infrastructure for last-mile people. Methods: Started with purposive sampled case studies to find out factors and criteria that most affect the success for an innovation to be frugal, social, and sustainable. The established criteria were used to design, develop, and deploy the mobile Primary Health Center (mPHC). Moving forward, we tested the system designed with stakeholders to gather insights. At this stage we found the feedback loop from the stakeholders and the role of interdisciplinary discussions between experts, medical officers, nurses, patient, and other staff of PHCs during the design, development, deployment, and test stage to be useful in taking design decisions efficiently. Results: The designed healthcare infrastructure of mPHC through the aspects of FI and SI proves to be efficient in providing key healthcare services to LRS. Conclusion: Focusing on essential capabilities and optimizing performance with technology, methodologies, and processes reduces costs in an innovation. Focus on socially inclusive and rebalancing power disparities, overcome societal challenges and improve human capabilities will create a sustainable and novel solution.
Innovative Models of Care for Hospitals of the Future

Abstract:New ways of providing acute care outside of traditional hospital building complexes, such as virtual care or hospital in the home, are becoming more common. Despite this, many hospitals are still conceived as "bricks and mortar" centralised constructions, and few health service infrastructure organisations meet intensively with consumers or clinicians prior to conceptualising hospital design. Our study sought to understand the needs and expectation of community members and healthcare providers, and co-design innovative models of acute care to inform development of a new metropolitan hospital in Australia. Our study used a three-step approach, consisting of academic and grey literature reviews; a demographic analysis of the hospital catchment population; and a series of 20 workshops and 6 supplementary interviews with community members and local healthcare providers. We found that care should be tailored to the healthcare needs and expectations of each consumer, with consumers cared for in the community where possible and safe. We propose an innovative model of care for hospitals of the future, consisting of fully integrated acute care underpinned by appropriate digital architecture to deliver care that is community focussed. It is vital that new hospitals build in sufficient adaptability to leverage future innovation and meet the needs of growing and changing communities.

Abstract:Vernacular buildings across the globe provide instructive examples of sustainable solutions to building problems. Yet, these solutions are assumed to be inapplicable to modern buildings. Despite some views to the contrary, there continues to be a tendency to consider innovative building technology as the hallmark of modern architecture because tradition is commonly viewed as the antonym of modernity. The problem is addressed by practical exercises and fieldwork studies in the application of vernacular traditions to current problems. This study investigates some aspects of mainstream modernist design solutions and concepts inherent in the vernacular of Asia, particularly that of the Chittagong Hill Tracts (CHT). This work hinges on such ideas and practices as ecological design, modular and incremental design, standardization, and flexible and temporal concepts in the design of spaces. The blurred edges between the traditional and modern technical aspects of building design, as addressed by both vernacular builders and modern architects, are explored.
A community healthcare clinic in Baltimore: healing environment, design criteria, and assessment metrics

Abstract:Purpose The purpose of this research is to identify quick, effective and affordable architectural design solutions to improve the health of patients, visitors and staff, in an underfunded community healthcare center (CHC) in Baltimore. Design/methodology/approach Both qualitative (individual and Focus group interviews) and quantitative methods (space syntax analysis and questionnaire survey) are used to assess the healing environment. The questionnaires cover interior and exterior environment related questions. Findings The questionnaire identifies three issues related to wayfinding, interior design, and access to natural light. The findings demonstrate that the waiting area, exam room and laboratory need to be upgraded. Incorporation of some other interior components including, wall art, plants, music, signage are also suggested by the respondents. Originality/value Very few studies in the USA have been done to understand the effects of architectural design to create a healing environment on the community scale.

Abstract:Abstract Leading innovation for excellence in occupational health in Moroccan universities on the eve of major world-class projects involves several key steps and considerations. University leaders are responsible for fostering a culture of innovation within the institution. They should encourage and support innovative teaching methods, research approaches, and administrative practices. A strategic approach to meet this objective can include the following elements: • Form a multidisciplinary task force consisting of experts in occupational health, medicine, public health, environmental science, engineering, and administration. This team will lead and coordinate efforts to innovate and improve occupational health practices. • Conduct a comprehensive assessment of the current occupational health practices and infrastructure in Moroccan universities. • Gather data on prevalent occupational health issues among university staff and students thanks a SWOT analysis. • Engage key stakeholders including university administrators, faculty members, staff representatives, students, and relevant government bodies. • Develop and implement comprehensive policies and guidelines for occupational health and safety tailored to the specific needs and challenges of Moroccan universities. • Ensure compliance with local regulations and international standards. • Invest in infrastructure upgrades and resources necessary for ensuring a safe and healthy working and learning environment. • Establish occupational health clinics or centers within universities equipped with necessary facilities for medical examinations, diagnostics, and treatment. • Revise academic curricula to integrate contemporary concepts of occupational health and safety across relevant disciplines such as medicine, engineering, environmental science, and management. Abstract:Globally and historically, Indigenous healthcare is efficacious, being rooted in Traditional Healing (TH) practices derived from cosmology and place-based knowledge and practiced on the land. Across Turtle Island, processes of environmental dispossession and colonial oppression have replaced TH practices with a colonial, hospital-based system found to cause added harm to Indigenous Peoples. Growing Indigenous health inequities are compounded by a mental health crisis, which begs reform of healthcare institutions. The implementation of Indigenous knowledge systems in hospital environments has been validated as a critical source of healing for Indigenous patients and communities, prompting many hospitals in Canada to create Traditional Healing Spaces (THSs). After ten years, however, there has been no evaluation of the effectiveness of THSs in Canadian hospitals in supporting healing among Indigenous Peoples. In this paper, our team describes THSs within the Center for Addiction and Mental Health (CAMH), Canada's oldest and largest mental health hospital. Analyses of 22 interviews with hospital staff and physicians describe CAMH's THSs, including what they look like, how they are used, and by whom. The results emphasize the importance of designating spaces with and for Indigenous patients, and they highlight the wholistic benefits of land-based treatment for both clients and staff alike. Transforming hospital spaces by implementing and valuing Indigenous knowledge sparks curiosity, increases education, affirms the efficacy of traditional healing treatments as a standard of care, and enhances the capacity of leaders to support reconciliation efforts.

Abstract:This research developed an assessment tool for enhancing the healing environment in healthcare buildings, especially interior finishing materials, based on sustainability standards, i.e., Leadership in Energy and Environmental Design (LEED) standards and WELL building standards. The purpose of this tool is to help decision-makers, interior designers, and client committees to identify the compatible interior materials' specifications with the healing and non-infection environment in order to create a better experience for patient, staff, and visitors. The current study adopts a sustainability-oriented approach to bring more objectivity for assessing the architectural finish in the healing environment at the healthcare projects through a case study of King Faisal University's (KFU) hospital project with a 182,000 m2 built area. Following the consensus of different experts, the assessment tool integrated two main international standards: LEED healthcare and WELL building standards, in addition to chromotherapy method. This tool assesses interior finishing materials, which affect the healing environment and sustainability in healthcare buildings. Each standard has credits, including items and scores for each item. The developed assessment tool was adopted in the KFU Hospital project, which has gained high acceptance among all stakeholders, including decision-makers. The developed tool is an integrated instrument (based on LEED standards, WELL standards, and chromotherapy method) for assessment; instead of using different tools for assessing finishing materials in healthcare buildings, the assessment tool supports all stakeholders in analyzing interior finishing material to achieve the optimum healing environment and sustainability.

Abstract:Technological progress cannot be hindered because today, education is entering the industrial era 4.0 characterized by education that uses digital technology in the learning process. With the use of this technology, the learning process is not limited to space and time. The terms religious knowledge and general science should not exist. Science is one unit because its source is from the Koran. This study aims to explain technological innovation in the integration and interconnection of science in Islamic higher education. This research is a research library. Therefore, what is done is exploring several data by examining in-depth reference sources from national and international journal articles and reading books relevant to the research. The data collection method that researchers use is by collecting books, articles, journals, and opinions which reveal and study the Interconnection Integration. The study results indicate that the integration and interconnection of knowledge can be achieved if learning refers to the principles of integration between Hadarah an-Nash, Hadarah al-Ilm, and Hadarah al-Falsafah. Technological advances can be used to achieve these goals, such as e-learning, digital learning resources, and teleconferencing.
The Role of Patients' Psychological Comfort in Optimizing Indoor Healing Environments: A Case Study of the Indoor Environments of Recently Built Hospitals in Sulaimani City, Kurdistan, Iraq

Abstract:Objective: This study investigates how patients and medical staff assess the physical environments of two recently built hospitals in Sulaimani City to understand the extent to which indoor environmental factors influence the creation of an optimal healing environment. Background: A contemporary healing environment may be recognized by the quality of an indoor environment in positively influencing patients' psychological comfort and supporting their recovery. Method: Post-occupancy evaluations were conducted in Shar General Hospital and Faruk Medical City wards. A 43-item questionnaire was distributed to 312 patients, doctors, and nurses to gather their perspectives. In total, 175 valid questionnaires were retrieved. Results: The results show that the quality of the indoor environments met patients' psychological needs and expectations, and as a result, they were generally satisfied with the indoor healing environments. The findings from the patient and medical staff surveys suggest three essential factors in creating a healing environment, which are (1) Interior appearance, (2) Privacy, and (3) Comfort and control. Significant negative correlations of some demographic characteristics, such as educational attainment and age, with patient satisfaction were observed. Conclusions: This study shows that exploring patients' and medical staff's experiences can reveal factors that positively influence patients' satisfaction levels, which may vary depending on their sociocultural perspectives and personal characteristics. Additionally, the findings emphasize the role of the tested factors in increasing patients' satisfaction levels, optimizing healing environments, and informing design decisions.

The smart ICU comprehensively and electronically integrates the patient in the ICU with all aspects of care, displays data in a variety of formats, converts data to actionable information, uses data proactively to enhance patient safety, and monitors the ICU environment to facilitate patient care and ICU management. The keys to success in this complex informatics design process include an understanding of advanced informatics concepts, sophisticated planning, installation of a robust infrastructure capable of both connectivity and interoperability, and implementation of middleware solutions that provide value. Although new technologies commonly appear compelling, they are also complicated and challenging to incorporate within existing or evolving hospital informatics systems. Therefore, careful analysis, deliberate testing, and a phased approach to the implementation of innovative technologies are necessary to achieve the multilevel solutions of the smart ICU.

Abstract:The field of engineering is increasingly appreciating the value of diversity for innovative design solutions. Successful engineering depends on our ability to explore constrained parameter spaces for finding the best solutions, and more diverse minds and experiences enable us to explore the entire potential solution space more thoroughly, more quickly, and more creatively. With a goal to expand the diversity of experiences and mindsets in our undergraduate bioengineering curricula, Arusha Technical College (ATC) in Arusha, Tanzania and Clemson University (CU) in Clemson, South Carolina, U.S., have partnered together over the past 5 years to provide intercontinental educational opportunities for undergraduate students, graduate assistants, and faculty. In 2018, CU and ATC collaborated on an international design course targeting undergraduate students in biomedical engineering focused on global health solutions for resource poor communities. Undergraduate students from ATC and CU collaborated on design projects through formal videoconferenced group meetings, e-mail, and various social media platforms. The year ended with a joint design symposium in Arusha where the students presented on their work in a public poster forum. This successful ATC-CU Global Health Design Collaboration pilot year provides a solid model upon which to build. Students reported overall positive experiences and plans to continue in their curriculum to graduation, as well as some ATC and CU students changing their career direction to include global health initiatives.

Abstract:There is a real need for innovation in health care delivery, as well as in medicine, to address related challenges of access, quality, and affordability through new and creative approaches. Health care environments must foster innovation, not just allowing it but actively encouraging it to happen anywhere and at every level in health care and medicine-from the laboratory, to the operating room, bedside, and clinics. This paper reviews the essential elements and environmental factors important for health-related innovation to flourish in academic health systems.The authors maintain that innovation must be actively cultivated by teaching it, creating "space" for and supporting it, and providing opportunities for its implementation. The authors seek to show the importance of these three fundamental principles and how they can be implemented, highlighting examples from across the country and their own institution.Health innovation cannot be relegated to a second-class status by the urgency of day-to-day operations, patient care, and the requirements of traditional research. Innovation needs to be elevated to a committed endeavor and become a part of an organization's culture, particularly in academic health centers.
Identifying the Most Important Strategies for Development Health Tourism in Affiliated Regions of Abadan University of Medical Sciences

Abstract:Background: Since Abadan University of Medical Sciences is situated near Arabic Countries and has similar cultural and linguistic characteristics to these countries, it must improve its health infrastructures to compete in this highly profitable market. Objectives: This study aimed to identify strategies for the development of health tourism at Abadan University of Medical Sciences. Methods: This was a qualitative study carried out in 2022. Study participants included all (22 person) experts and thinkers from Abadan University of Medical Sciences. The research tool was a checklist made by the researcher. The data collection tool consisted of several questions regarding health tourism development strategies. The interviewing process continued until saturation had been reached. The content analysis method was then used to analyze the data. Results were categorized and analyzed using MAXQDA version 2021 software. Results: Abadan tourism development strategies for health tourism included five major themes such as (administrative and executive, economic, law and regulation reform, marketing, and information) with 46 sub-themes. The most important administrative and executive strategy was encouraging travel agencies that are active in attracting foreign patients. The most important economic strategies were to reduce customs tariffs on medical equipment and attract direct foreign investment in the medical sector. The most important strategies were to amend and revise the laws and regulations of the nations and peoples of the valley. Conclusions: It is recommended that policymakers and decision-makers in the health tourism industry of Abadan University of Medical Sciences take steps to strengthen and develop health tourism based on the priorities set.

Abstract:High performance adaptive solutions are capable of responding to the dynamic nature of users and context. These innovative and dynamic systems are steadily gaining ground over ubiquitous ‘best fit’ static models. These architectural elements often exist beyond the scope of mainstream building standards and traditional methods for data representation or communication. This presents major challenges to a highly standardized and compartmentalized industry in which ‘innovation’ is limited to a few signature practices that design iconic yet expensive structures, which often prioritize aesthetics over performance. This paper offers an overview of the benefits that integrated dynamic systems bring to buildings. Through an examination of an applied practice, this paper offers guidelines for communicating complex geometry in a clear design language across interdisciplinary collaborations. The use of diagrammatic grammar to translate underlying algorithmic rules into instructions for design allows complex, innovative solutions to be realized more effectively. The ideas presented here are based on the design principles of the competition-winning scheme of the Al-Bahr Towers. As lead consultant in Innovation Design & Research at AHR (former Aedas-UK), Abdulmajid Karanouh designed and spearheaded this project in close collaboration with Arup. The buildings won the Best Innovation Award 2012 by the Council for Tall Buildings and Urban Habitat (CTBUH). The pair of towers won recognition for its performance-driven form, and dynamic facade that operates following the movement of the sun.
