Abstract:Smith and Mick identify four basic problems with the theory the present writers developed to explain organizational responses (in this case the behavior of hospitals) to regulation. They challenge the basic assumption regarding autonomy, disagree with the implied cause and effect relations between organizational response and regulation, criticize the omission of goals, and claim that the theory has only limited generality. In so doing they state that their primary concern is with "improving our understanding of the limitations and benefits of the theory." Each of the four topics they raise for consideration will receive comment.

Abstract:This paper presents a general theory of organizational response to regulation, a theory that integrates adaptation and mutual selection perspectives. Two major forms of regulation in the hospital industry, certificate of need and rate review, are examined. Hypotheses are derived concerning the nature and timing of the various adjustments hospitals make both in internal organizational arrangements and in patterns of interorganizational activity in the face of regulatory constraints. Suggestions and data sources for testing the theory are presented.

Abstract:As the transition from planned economy to socialist market economy, certain state-owned hospitals are still run by the government directly and require corresponding governance structure to guarantee the right behavior. Conclusion of this paper shows, based on economic theory, ownership reform is neither the sufficient nor necessary conditions for performance improvement, which is determined by the purpose of public hospital operation. Therefore, the "Priority of Ownership Reform Theory" for public hospitals is denied in this paper, which otherwise provides a theoretical support for the further reform in governance structure of public hospitals. In addition, this paper first raises the "information paradox" inside asymmetrical information firms in theory, which can be applied not only to reform in hospitals but also to reform in firms with similar information characteristics.

Abstract:Objective This paper explores how hospital complaints managers react to recommendations for systemic quality reforms by health complaints commissions in response to complaints by patients in Queensland and New South Wales. Methods Semi-structured qualitative interviews were conducted with complaints managers in 17 hospitals. Interview transcripts were then thematically analysed and data on responses to health complaint commissions was organised in relation to Valerie Braithwaite's typology of motivational postures. Results Respondents supported involvement by an independent authority where patients had serious complaints about the services they received in hospital, but wanted more negotiation with commissions on service improvement recommendations. Conclusions Hospital complaints managers mostly responded as virtuous or rational actors to the symbolic power of complaints commissions. This may be context dependent because Australian health commissions operate within a pro-reform context as a result of recent publicity around health system failures. What is known about the topic? Little is known about regulatory relationships between complaints commissions and hospitals. There has been no Australian research considering how complaints managers respond to commission recommendations for quality improvements and reforms to hospital services. What does the paper add? The paper uses a novel theoretical framework based on regulatory theory to understand and describe the reactions of complaints managers to commission recommendations. What are the implications for practitioners? Commissions should seek commentary from complaints managers through open dialogue before making final recommendations. This will ease the progress of reforms and make recommendations more acceptable and 'genuine' in the specific context of the hospital.

Abstract:Theory production has been a central focus of management research for decades, mostly because theory legitimizes both management research and, through its application, management practice as professional endeavors. However, such an emphasis on theory glosses over one of its constraining and particularized roles in scientific explanation, namely that theory codifies predictive knowledge. Committing to a ‘traditional’ or ‘critical’ understanding of theory thus amounts to embracing the view that prediction is achievable within a circumscribed field of study. Such an embrace is non-controversial in natural science. However, within the realm of management studies, it necessitates and smuggles in a strawman view of human existence, one which does not accommodate freedom and responsibility. This limitation of management theory explains its inadequate utility. This article argues that alternative avenues for management research exist. JEL CLASSIFICATION: M10

Abstract:This chapter explores the motivational dynamics of the promotion and prevention systems outlined in regulatory focus theory (Higgins, 1997). It includes a review of the core tenets of the theory—identifying and responding to important and frequently asked questions—in discussing significant research of the past two decades since the theory made its debut. In particular, the chapter includes a discussion of what defines each system, how regulatory focus orientations are commonly measured and manipulated, what differentiates promotion and prevention motivation from approach and avoidance motivation, what characterizes the trade-offs of each system, and newer developments in research on regulatory fit, group dynamics, and motivational flexibility. Throughout, avenues for future research are suggested.

Critical Perspectives on Accounting

Abstract:Existing agency theory fails as a positive theory since it does not provide a theory of accounting regulatory development. Furthermore, the arguments against market failure theories in Watts and Zimmerman (1986) are inconsistent with positive theory in that they evaluate rather than predict the theories. In fact, the market for excuses arguments used by Watts and Zimmerman (1979) are capable of predicting the existence of market failure theories. That these arguments were not extended in Watts and Zimmerman (1986) seems to indicate an ideological bias. If Watts and Zimmerman wish to argue normatively, they should not do so under the guise of positive theory.The failure of agency theory to provide a prediction of accounting regulatory development stems from its reliance on a rationality assumption which is on one hand narrow and on the other hand broad and inconsistent. Richer theories relying on either a better specified rationality assumption or broader views of organizational behaviour are needed to predict and explain the development of accounting regulatory structures. A confluence of theories is most likely to provide the richest description.

Abstract:This article examines organization, management, and leadership theory development and evolution from the perspectives of legitimacy and efficacy. Based on this examination, we support rigorous testing and grounded theory approaches, advocate the use of evidence-based theory development, and offer the 5P's Model of Strategic Leadership as an example of a model that originated from evidence-based theory development. We suggest that researchers, faculty members, and business practitioners should be able to contemplate and debate the usefulness of various theories and models with the understanding that application is essential for substantiation of their worth. In this article, we analyze existing organizational theory, but we also advocate the introduction of new organization theories. This can occur if researchers in the various educational institutions, business organizations, professional organizations, and media outlets are more open to the creation of a variety of models and theories, the legitimacy and usefulness of which can later be substantiated through application. Introduction The objectives of this article are: (1) to examine the history of various organization theories and their development from the perspective of legitimacy and efficacy, (2) to analyze selected debates regarding theory development and various theories, (3) to advocate the use of evidence-based theory development, and (4) to provide an example of a model and related theories that we created after reviewing existing theories and determining gaps in terms of implementation and alignment. We are not rejecting the need for rigorous testing of theories. We are simply advocating the proposal of conceptual theories and models for debate and potential usefulness. Then through application of the theories, they can be tested in terms of validity and the extent to which they are useful. This paper suggests that many very useful new organizational theories and models as well as extensions and adaptations of other theories and models have been, and are being, developed. The difficulties in getting these theories and models considered for legitimacy and usefulness range from realistic questions to superciliousness as struggles and debates continue among theorists, practitioners, and empirical scientists. Walsh, Meyer, and Schoonhoven (2006: 657) suggest other potential problems including "the irrelevance of organization theory," the debates among two groups-those who "argue for closer ties to business practice" and those who "argue for maintaining our academic distance," and the fact that organization theorists "find ourselves so self-critical and . . . fundamentally, adrift." We would not go so far as to say that we are adrift, but we are perhaps so self-critical that we do not allow theories to be presented so that they can be debated. We believe that an understanding of, and appreciation for, the history of management and organization theory is valuable. Yet we as management and organization theorists, scholars, and researchers are also aware that times have changed, and continue to change, dramatically. Business planning horizons are much shorter and competitive pressures (often intense) lead to a more rapid rate of business innovation than ever before. We propose that we, as the academicians studying these phenomena, should be thinking innovatively about the theory development process. We suggest that the originators of new theories and models and extensions of existing theories and models should be able to present in the literature for debate and later testing for application potential. To facilitate organization theory development for introduction and application in today's rapidly changing environment, we advocate the evidence-based approach and other innovative approaches to the creation of new, and the extension of existing, theories and models. Abstract:Objective To explore the key factors of public hospital regulation and the impact factors for such regulation,and to provide reference for improvement.Methods Multi-input precedence chart was used to identify the key factors,and decision making trial and evaluation laboratory was used to analyze impact factors on the regulation.Results Such preference charts as medical behavior,quality and patient safety,and admittance scored over 60 points,thus ranking key factors of regulation on public hospitals.Those factors affecting the outcomes of regulation,namely"defective legal system","poor management system", and "diversification and fragmentation of supervision entities", as well as"incapable supervisors" scored greater than 0,ranking as the cause factors.Conclusions The key content and factors related to effect of regulation could be used to improve regulation.In the course of reform of the regulation mechanism,the regulation outcomes could be improved with focus on key regulation factors and main cause factors.

Abstract:This paper describes a mean field approach to defining and implementing policy-based system administration. The concepts of regulation and optimization are used to define the notion of maintenance. These are then used to evaluate stable equilibria of system configuration, that are associated with sustainable policies for system management. Stable policies are thus associated with fixed points of a mapping that describes the evolution of the system. In general, such fixed points are the solutions of strategic games. A consistent system policy is not sufficient to guarantee compliance; the policy must also be implementable and maintainable. The paper proposes two types of model to understand policy driven management of Human-Computer systems: (i) average dynamical descriptions of computer system variables which provide a quantitative basis for decision, and (ii) competitive game theoretical descriptions that select optimal courses of action by generalizing the notion of configuration equilibria. It is shown how models can be formulated and simple examples are given.

Abstract:An era of managerialism in health care delivery systems is now well ensconced throughout the nations of the OECD. This development has occurred, in large part, as a response to funding pressures in institutionally based health care delivery imposed by principal third party insurers. In the case of publicly funded hospitals, the more traditional concerns for stewardship and appeasement of professional groups is being replaced by a greater emphasis on cost consciousness and corporate-style leadership as these organizations seek to reposition themselves in new funding and regulatory environments. While institutional theory and strategic management perspectives help illuminate these issues, this paper argues that a place-based perspective is also needed to understand the changes currently underway in health care delivery and publicly funded human services more generally. This is illustrated with reference to developments in the strategic management of public hospitals in the province of Ontario. Evidence from a survey of senior administrators of public hospitals, distributed at the height of these policy reform initiatives, is examined to shed light on local level management responses to changing policy and fiscal pressures. The data suggest that the latest policy directions in the province of Ontario will 'encourage' hospital executives in particular community settings to steer their organizations in very unfamiliar directions. The findings suggest a need for greater attention to context and setting in health services research and policy.

Abstract:Abstract The use of behavioral economics in policy-making and public administration has neglected the heuristics that affect those who must comply with regulations. These heuristics affect the decision to comply with a regulation, and therefore they affect the effectiveness of regulations, and more generally the attitude of the regulated community toward government. In a survey and in interviews, small business owners described their compliance activities and their attitudes toward government. The business owners demonstrated the availability heuristic in their hatred of paperwork requirements, the bandwagon effect in seeking out reinforcement of negative reactions toward regulation, the anchoring of attitudes in particularly unpleasant interactions with government, and the impact of their economic well-being on their reaction to regulation. They also demonstrated the ineffectiveness of some information disclosure requirements. Incorporation of these behaviors and heuristics into the evaluation and enforcement of regulation may improve compliance and business relations with regulatory agencies.

Population ecology is used as a framework for explaining both the direct and indirect effects of regulatory activity on entry, exit, and market structure. This framework is then used to derive specific propositions about regulatory effects on entry, exit, and market structure in the health maintenance organization industry.
Theory X/Y in the Health Care Setting: Employee Perceptions, Attitudes, and Behaviors

Abstract:Douglas McGregor's conceptualization of Theory X and Theory Y has influenced management practices for almost six decades, despite the relative paucity of empirical support. This empirical study examined the relationships between health care employees' perceptions of (1) manager Theory Y and Theory X orientations; (2) work unit psychological safety, organizational citizenship behavior, and service quality; and (3) the employing entity. The study used survey data from more than 3500 employees of a large US health care system and analyzed them using confirmatory factor and hierarchical regression analyses. Results indicate that McGregor's conceptualization is best considered as two separate constructs-Theory Y and Theory X-rather than as one-dimensional X/Y construct. This study's three dependent variables were positively related to Theory Y and negatively related to Theory X, with larger Theory Y effect sizes. Psychological safety partially mediated the relationship between Theory Y and the dependent variables Y. Practical implications are presented.

Abstract:Hospital system formation has recently accelerated. Executives emphasize scale economies that lower operating costs, a claim unsupported in academic research. Do systems achieve lower costs than freestanding facilities, and, if so, which system types? We test hypotheses about the relationship of cost with membership in systems, larger systems, and centralized and local hub-and-spoke systems. We also test whether these relationships have changed over time. Examining 4,000 U.S. hospitals during 1998 to 2010, we find no evidence that system members exhibit lower costs. However, members of smaller systems are lower cost than larger systems, and hospitals in centralized systems are lower cost than everyone else. There is no evidence that the system’s spatial configuration is associated with cost, although national system hospitals exhibit higher costs. Finally, these results hold over time. We conclude that while systems in general may not be the solution to lower costs, some types of systems are.

Abstract:Over the past decade the importance of governing boards as policy-making setting and oversight units within organizations has increased dramatically. Although this is true for both corporate- and private-sector organizations (Bacon and Brown 1977; Gelman 1988), it is particularly relevant to the health sector. Hospital governing boards, long considered inconsequential in hospital management, have recently become subject to closer scrutiny. The role of governing boards in decisions affecting hospital strategy and hospital performance is once again a topic of some interest in boardrooms and hospital trade journals. Impressive evidence of the renewed interest in governance is provided by the funding of an instructional consortium by the S.K. Kellogg Foundation to help strengthen trusteeship and governing board decision making, and to improve education for health services managers in the area of governance. Members of the consortium include the Hospital Research and Educational Trust, the American Hospital Association, the American College of Healthcare Executives, and the Association for University Programs in Health Administration. Among the activities being undertaken by this consortium is the development of a self-assessment tool/methodology for boards, a bibliography and reference guide on effective governance for practicing trustees, research workshops for faculty in health administration programs, and a teaching guide on governance and trustee leadership. Despite this interest, the question with which we began this article persists. Do governing boards make a difference? In the course of our review of previous work on governance we found that, more often than not, that question has been transformed into: how do boards influence hospital performance? And very often that question has been further narrowed into: which board structure leads to better hospital performance? We have argued for a respecification of the initial question. Rather than pursuing a definition of the maximally performing governing board, we should perhaps shift our focus back to a fuller understanding of board structure and function, and its influence on hospital change. The model developed here combines four essential, and very basic, questions: 1. What are the basic dimensions that underlie structural variation in different types of governing boards? 2. How do these board types influence structural change in hospitals? 3. How is the effect of board influence on change itself likely to change over time as a function of the hospital's general pattern of growth, decline, stability, or instability?(ABSTRACT TRUNCATED AT 400 WORDS)
Examining Public Hospital Service Failure: The Influence of Service Failure Type, Service Expectations, and Attribution on Consumer Response

Abstract:This study investigates three independent variables; types of service failure, service expectations, and attribution on postfailure responses in healthcare. A between-subjects 3 × 2 × 2 experimental design using written scenarios was used. The findings demonstrate that customer responses to service failures in hospitals are extensively influenced by the type of service failure, the level of service expectation of the provider, and, to a lesser extent, the controllability of the cause of the failure. Core service failures lead to greater increases in negative responses for satisfaction, emotional, and behavioral responses than supplementary service failure with high service expectations protecting the provider against overall dissatisfaction, negative word-of-mouth, and switching behaviors. Interestingly, perceived high controllability leads to greater dissatisfaction but not to increased negative emotional or behavioral responses. The study applies attribution theory to explain the results. The article concludes with managerial implications.

Abstract:This study attempts to fill the gap in research on institutional logics by studying how multiple institutional logics, with their conflicting guidelines for organizational attention, shape organizational decision-making. We propose that the corporate governance attributes filter the influence of different institutional logics and help organizations to process the institutional complexity. We test our argument in the empirical context of how Chinese publicly listed firms respond to the central government’s regulations on issuing CSR reports. We find support for our argument by analysing listed firms between 2005 and 2010. Our study contributes to the literature on institutional logics, research on how organizations respond to institutional pressures, and CSR in transitional economies.

Abstract:Background: For almost a decade, public and private organizations have pressured hospitals to improve their patient safety records. Since 2008, the Centers for Medicare & Medicaid Services has no longer been reimbursing hospitals for secondary diagnoses not reported during the point of admission. This ruling has motivated some hospitals to engage in safety-oriented programs to decrease adverse events. Purpose: This study examined which hospitals may engage in patient safety solutions and whether they create these patient safety solutions within their structures or use suppliers in the market. Methodology: We used a theoretical model that incorporates the key constructs of resource dependence theory and transaction cost economics theory to predict a hospital's reaction to Centers for Medicare & Medicaid Services "never event" regulations. We present propositions that speculate on how forces conceptualized from the resource dependence theory may affect adoption of patient safety innovations and, when they do, whether the adopting hospitals will do so internally or externally according to the transaction cost economics theory. Findings: On the basis of forces identified by the resource dependence theory, we predict that larger, teaching, safety net, horizontally integrated, highly interdependent, and public hospitals in concentrated, high public payer presence, competitive, and resource-rich environments will be more likely to engage in patient safety innovations. Following the logic of the transaction cost economics theory, we predict that of the hospitals that react positively to the never event regulation, most will internalize their innovations in patient safety solutions rather than approach the market, a choice that helps hospitals economize on transaction costs. Practice implications: This study helps hospital managers in their strategic thinking and planning in relation to current and future regulations related to patient safety. For researchers and policy analysts, our propositions provide the basis for empirical testing.
