Pattern Of Lymph Node Metastases And Its Implication In Radiotherapy In Patients With Thoracic Esophageal Squamous Cell Carcinoma

Abstract:Purpose/Objective(s)The patterns of lymph node metastases were studied in patients treated by radical esophagectomy with cervix, chest and abdomen lymph node regions dissection (three-field dissection). According to the lymph node metastatic patterns, we wish to find how to delineate CTV of lymph node in thoracic esophageal squamous cell carcinoma (ESCC) if these patients were treated by radical radiotherapy instead of surgery.Materials/MethodsTotal 414 thoracic ESCC patients who had undergone esophagectomy and lymphadenectomy with radical resection of the three field lymph node regions dissection by right thoracic inlet were retrospectively reviewed and the number of dissected lymph nodes was required to be > = 15 according to the requirement of radical lymph node regions dissection with thoracic ESCC in NCCN. In this study, the lymphatic drainage is divided into five areas. The cervical region includes supraclavicular lymph nodes, upper mediastinum including paratracheal, posterior mediastinal, para-aortic lymph nodes, mid-mediastinum including middle paraesophageal, subcarinal, pulmonary hilar nodes, lower mediastinum including lower paraesophageal, diaphragmatic, pulmonary ligament lymph nodes, perigastric region consisting of paracardial, left gastric, the celiac area nodes. The regional clusters of lymph node metastasis were analyzed using Chi-square test.ResultsThe overall rates of lymph node metastasis were 62.8% (260/414). There were no significantly differences among the lymph node metastasis rates in patients with upper, middle and lower thoracic tumors with 69.7% (46/66), 60.3% (161/267) and 65.4% (53/81), respectively (P = 0.317). The rates of lymph node metastases in the patients with upper thoracic tumors were 39.4% (26/66) in cervical region, 39.4% (26/66) in upper mediastinal, 10.6% (7/66) in middle mediastinal, 0% (0/66) in lower mediastinal, and 6.1% (4/66) in perigastric region, respectively. The different region lymph node metastases rates in patients with middle thoracic tumors were 28.8% (77/267), 24.4% (66/267), 24.3% (65/267), 10.5% (28/267), and 18.4% (49/267), respectively and in patients with lower thoracic tumors were 28.4% (23/81), 24.7% (20/81), 16% (13/81), 28.4% (23/81), and 42% (34/81), respectively.ConclusionsThe high rates of lymph node metastases were found in thoracic ESCC with no significantly difference among the tumors located in the different areas of esophagus. The regional clusters of lymph node metastasis were observed in the upper thoracic ESCC. CTV of lymph node in the upper thoracic ESCC should include the region of cervical, upper and middle mediastinal if these patients were treated by radical radiotherapy instead of surgery.
