The development of the field of visual prosthesis began with cortical prosthetic devices but since then, a variety of devices which target different sites along the visual pathway have been developed with the retinal prosthesis being the most advanced. We present a review of the history of these devices, an update on the current state of play and future prospects of this field.

Abstract:A retinal prosthesis, also known as a bionic eye, is a device that can be implanted to partially restore vision in patients with retinal diseases that have resulted in the loss of photoreceptors (e.g., age-related macular degeneration and retinitis pigmentosa). Recently, there have been major breakthroughs in retinal prosthesis technology, with the creation of numerous types of implants, including epiretinal, subretinal, and suprachoroidal sensors. These devices can stimulate the remaining cells in the retina with electric signals to create a visual sensation. A literature review of the pre-clinical and clinical studies published between 2017 and 2023 is conducted. This narrative review delves into the retinal anatomy, physiology, pathology, and principles underlying electronic retinal prostheses. Engineering aspects are explored, including electrode–retina alignment, electrode size and material, charge density, resolution limits, spatial selectivity, and bidirectional closed-loop systems. This article also discusses clinical aspects, focusing on safety, adverse events, visual function, outcomes, and the importance of rehabilitation programs. Moreover, there is ongoing debate over whether implantable retinal devices still offer a promising approach for the treatment of retinal diseases, considering the recent emergence of cell-based and gene-based therapies as well as optogenetics. This review compares retinal prostheses with these alternative therapies, providing a balanced perspective on their advantages and limitations. The recent advancements in retinal prosthesis technology are also outlined, emphasizing progress in engineering and the outlook of retinal prostheses. While acknowledging the challenges and complexities of the technology, this article highlights the significant potential of retinal prostheses for vision restoration in individuals with retinal diseases and calls for continued research and development to refine and enhance their performance, ultimately improving patient outcomes and quality of life.

Abstract:As the research hotspot of restoring vision for retinitis pigmentosa and age-related macular degeneration, visual prosthesis can form the visual perception and restore vision by capturing image with a microcamera, processing and encoding, transmitting the signal to the neural stimulator and delivering the current pulse to the neural system. Generally, three major types of visual prostheses are currently being developed: stimulation of the visual cortex using cortical implants, retinal devices implanted in the subretina or epiretina, and stimulation of the optic nerve. This paper will introduce the current progress and future challenge of visual prosthesis by C-sight group.

Abstract:Once the topic of folklore and science fiction, the notion of restoring vision to the blind is now approaching a tractable reality. Technological advances have inspired numerous multidisciplinary groups worldwide to develop visual neuroprosthetic devices that could potentially provide useful vision and improve the quality of life of profoundly blind individuals. While a variety of approaches and designs are being pursued, they all share a common principle of creating visual percepts through the stimulation of visual neural elements using appropriate patterns of electrical stimulation. Human clinical trials are now well underway and initial results have been met with a balance of excitement and cautious optimism. As remaining technical and surgical challenges continue to be solved and clinical trials move forward, we now enter a phase of development that requires careful consideration of a new set of issues. Establishing appropriate patient selection criteria, methods of evaluating long-term performance and effectiveness, and strategies to rehabilitate implanted patients will all need to be considered in order to achieve optimal outcomes and establish these devices as viable therapeutic options.

Abstract:This work presents a bunched of promising technologies to treat blind people: the bionic eyes. The strategy is to combine a retina implant with software capable to interpret the information received. Along this line of thinking, projects such as Retinal Prosthetic Strategy with the Capacity to Restore Normal Vision from Weill Medical College of Cornell University Project, Update on Retinal Prosthetic Research from The Boston Retinal Implant Project, and Restoration of Vision Using Wireless Cortical Implants from Monash Vision Group Project, have shown in a different context the use of technologies that commits to bring the vision through its use.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>To date, reviews of bionic eye have concentrated on implants which were used in human trials in the developed countries. This is the main restriction of this systematic review examines, however this review discusses worldwide advances in retinal prosthetic research, assesses engineering features and clinical progress of recent implant trials, and identifies potential future research areas in the field of bionic implants.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A literature review searching PubMed, Google Scholar, and IEEExplore was performed using the PRISMA Guidelines for Systematic Review. We included peer-reviewed papers in the review which demonstrated progress in human or animal trials and papers with described innovative bionic eye engineering design. For each trial, a characteristic of the device, engineering solution, and latest clinical outcomes were presented.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Eleven prosthetic projects fulfilled met our inclusion criteria and were ordered by stimulation location. Four have recently finished human trials, three are having conducted multi- or singlecenter human trials, and three are in preclinical animal testing stage. FDA has approved Argus II (FDA 2013, CE 2011); the Alpha-IMS (CE 2013) has been approved and obtained BCVA with Landolt-C test has taken into a multicenter clinical research. New approaches will be presented using alternating magnetic fields, low-intensity focused ultrasounds, optogenetics, implementing ionic gradients across neural cell membranes or influencing neurotransmitter levels will be presented in the review.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Several bionic eye have successfully achieved visual perception in animals and/or humans. However, many things need to be improved and engineering difficulties are to be resolved before bionic eye will be capable of fully and safely bring back vision functions. New approaches could improve medical outcome of future bionic eye.</p>

Abstract:Abstract Bioinspired bionic eyes should be self-driving, repairable and conformal to arbitrary geometries. Such eye would enable wide-field detection and efficient visual signal processing without requiring external energy, along with retinal transplantation by replacing dysfunctional photoreceptors with healthy ones for vision restoration. A variety of artificial eyes have been constructed with hemispherical silicon, perovskite and heterostructure photoreceptors, but creating zero-powered retinomorphic system with transplantable conformal features remains elusive. By combining neuromorphic principle with retinal and ionoelastomer engineering, we demonstrate a self-driven hemispherical retinomorphic eye with elastomeric retina made of ionogel heterojunction as photoreceptors. The receptor driven by photothermoelectric effect shows photoperception with broadband light detection (365 to 970 nm), wide field-of-view (180°) and photosynaptic (paired-pulse facilitation index, 153%) behaviors for biosimilar visual learning. The retinal photoreceptors are transplantable and conformal to any complex surface, enabling visual restoration for dynamic optical imaging and motion tracking.

Abstract:Since Brindley evoked a visual sensation in a blind patient by electrical stimulation of the visual cortex, some groups have been committing to develop visual prosthesis based on electrical stimulation over the remaining structures of the visual pathway to restore lost vision caused by retinitis pigmentosa (RP) and age-related macular degeneration (ARMD). As the widespread availability of micro-manufacturing being introduced into the building of novel interfaces to the nervous system, there are four approaches to a visual prosthesis being currently pursued, including visual cortex, epi-retinal, sub-retinal and optic nerve prostheses.

Abstract:Visual prostheses based on a stimulating microelectrode array to restore vision offer a promising approach for the blind and has become a rapidly growing scientific field in neurorehabilitation engineering. A number of research groups from major developed countries lead the research activities in this field. The goal of the C-Sight Project is to develop an implantable microelectronic medical device that will restore useful vision to blind patients. Retinitis pigmentosa (RP) and age-related macular degeneration are the two leading causes for blindness, for which there have been no effective treatments, both surgically and biologically, until now.The ultimate goal of our project is to develop a completely implantable visual prosthesis based on a penetrating microelectrode array at the optical nerve; to implement this, it requires many technical advances. However, in our study, an implantable microcamera, the wearable information processor, and the multichannel neurostimulator are investigated. Electrophysiological experiments were also performed to provide evidence for the feasibility of our approach. Some related psychophysical studies including simulated phosphene positioning and recognition of pixelized images are also reported in this article.

Abstract:There are several research projects going on around the world, which are attempting to develop a prosthetic device to restore sight to the blind. This paper describes the efforts of Second Sight of New York, Inc. The device being developed is called an Artificial epi-Retinal Prosthesis (AeRP), which is basically a small optical computer that fits into the intraocular space of the eye. The AeRP is designed to draw light into the device by specially designed fibre optics. The light is ‘digitized’ by the fibre optic system and then directed to individual photodiode cells making up concentric cylinders thus providing several hundred photodiode cells in the device. The produced electrical stimulation from each cell is then delivered to the retinal ganglion cells by a specially designed delivery system utilizing electrically conducting polymer strands (ECP), which sit on an ‘umbrella’ at the back of the device. The retinal ganglion cells receive the electrical stimulation, which would then be transmitted through the visual system of the brain. There are several innovations in this approach as compared to the other projects. They include, first the design, which will allow for a high number of PC to produce electrical stimulation that will stimulate multiple RGC per PC; the use of the ECP strands has not been used in such an approach before this. Tests have revealed that nerve cells have a good affinity for the material of the ECP. The use of the ECP as well as the fact that the AeRP is completely photovoltaic, with no external power sources, implies that there will not be high heat build-up in the back of the eye, which might damage RGC. A smaller version of the AeRP called the Mini epi-Retinal Prosthesis (MeRP) is the subject of a complimentary paper. It is being built now and will be tested in cell culture studies to determine the efficacy of the design and materials. No actual implants have been performed yet.

Abstract:Successful visual prostheses require stable, long-term attachment. Epiretinal prostheses, in particular, require attachment methods to fix the prosthesis onto the retina. The most common method is fixation with a retinal tack; however, tacks cause retinal trauma, and surgical proficiency is important to ensure optimal placement of the prosthesis near the macula. Accordingly, alternate attachment methods are required. In this study, we detail a novel method of magnetic attachment for an epiretinal prosthesis using two prostheses components positioned on opposing sides of the retina. The magnetic attachment technique was piloted in a feline animal model (chronic, nonrecovery implantation). We also detail a new method to reliably control the magnet coupling force using heat. It was found that the force exerted upon the tissue that separates the two components could be minimized as the measured force is proportionately smaller at the working distance. We thus detail, for the first time, a surgical method using customized magnets to position and affix an epiretinal prosthesis on the retina. The position of the epiretinal prosthesis is reliable, and its location on the retina is accurately controlled by the placement of a secondary magnet in the suprachoroidal location. The electrode position above the retina is less than 50 microns at the center of the device, although there were pressure points seen at the two edges due to curvature misalignment. The degree of retinal compression found in this study was unacceptably high; nevertheless, the normal structure of the retina remained intact under the electrodes.

Abstract:Brain-machine interfacesBrain-machine interface (BMIs) of the future will be used to treat diverse neurological disorders and augment human capabilities. However, to realize this futuristic promise will require a major leap forward in how electronic devices interact with the nervous system. Current BMIs provide coarse communicationCommunication with the target neural circuitry, because they fail to respect its cellular and cell-type specificity. Instead, they indiscriminately activate or record many cells at the same time and provide only partial restoration of lost abilities. A future BMI that may pave the path forward is an artificialRetinaretinaArtificial retina—a device that can restore vision to people blinded by retinal degeneration. Because the retina is relatively well understood and easily accessible, it is an ideal neural circuit in which to develop a BMI that can approach or exceed the performance of the biological circuitry. This chapter summarizes the basic neuroscienceNeuroscience of vision, identifies the requirements for an effective retinal interface, and describes some of the necessary circuits and systems. Based on these ideas and the lessons from first-generation retinal prosthesesBionic, a novel neuroengineering approach is proposed: the first BMI that will interact with neural circuitry at cellular and cell-type resolution.

Despite recent advances in the development of visual neuroprostheses, the quality of current prosthetic vision is still rudimentary and does not differ much across different device technologies.Approach.Rather than aiming to represent the visual scene as naturally as possible, aSmart Bionic Eyecould provide visual augmentations through the means of artificial intelligence-based scene understanding, tailored to specific real-world tasks that are known to affect the quality of life of people who are blind, such as face recognition, outdoor navigation, and self-care.Main results.Complementary to existing research aiming to restore natural vision, we propose a patient-centered approach to incorporate deep learning-based visual augmentations into the next generation of devices.Significance.The ability of a visual prosthesis to support everyday tasks might make the difference between abandoned technology and a widely adopted next-generation neuroprosthetic device.

Abstract:Purpose To develop and evaluate a photovoltaic, wireless wide‐field epiretinal prosthesis for the treatment of retinitis pigmentosa. Methods A mosaic array of thinned silicon‐based photodiodes with integrated thin‐film stimulation electrodes was fabricated with a flexible polyimide substrate film to form a film‐based miniaturized electronic system with wireless optical power and signal transmission and integrated electrostimulation. Manufactured implants were characterized with respect to their optoelectronic performance and biocompatibility following DIN EN ISO 10993. Results A 14 mm diameter prosthesis containing 1276 pixels with a maximum sensitivity at a near infrared wavelength of 905 nm and maximized stimulation current density 30–50 μm below the electrodes was developed for direct activation of retinal ganglion cells during epiretinal stimulation. Fabricated prostheses demonstrated mucosal tolerance and the preservation of both metabolic activity, proliferation and membrane integrity of human fibroblasts as well as the retinal functions of bovine retinas. Illumination of the prosthesis, which was placed epiretinally on an isolated perfused bovine retina, with infrared light resulted in electrophysiological recordings reminiscent of an a‐wave (hyperpolarization) and b‐wave (depolarization). Conclusions A photovoltaic, wireless wide‐field epiretinal prosthesis for the treatment of retinitis pigmentosa using near infrared light for signal transmission was designed, manufactured and its biocompatibility and functionality demonstrated in vitro and ex vivo.

Abstract:Degeneration of photoreceptors in the retina is a leading cause of blindness, but commonly leaves the retinal ganglion cells (RGCs) and/or bipolar cells extant. Consequently, these cells are an attractive target for the invasive electrical implants colloquially known as "bionic eyes." However, after more than two decades of concerted effort, interfaces based on conventional electrical stimulation approaches have delivered limited efficacy, primarily due to the current spread in retinal tissue, which precludes high-acuity vision. The ideal prosthetic solution would be less invasive, provide single-cell resolution and an ability to differentiate between different cell types. Nanoparticle-mediated approaches can address some of these requirements, with particular attention being directed at light-sensitive nanoparticles that can be accessed via the intrinsic optics of the eye. Here we survey the available known nanoparticle-based optical transduction mechanisms that can be exploited for neuromodulation. We review the rapid progress in the field, together with outstanding challenges that must be addressed to translate these techniques to clinical practice. In particular, successful translation will likely require efficient delivery of nanoparticles to stable and precisely defined locations in the retinal tissues. Therefore, we also emphasize the current literature relating to the pharmacokinetics of nanoparticles in the eye. While considerable challenges remain to be overcome, progress to date shows great potential for nanoparticle-based interfaces to revolutionize the field of visual prostheses.

Abstract:A high-optical-resolution artificial retina system that accurately communicates with the optic nerve is the main challenge in the modern biological science and bionic field. Here, we developed a bionic artificial retina possessing phototransduction "cells" with measurements even smaller than that of the neural cells. Using the technique of micrometer processing, we constructed a pyramid-shape periodic microarray of a photoreceptor. Each "sensing cell" took advantage of polythiophene derivative/fullerene derivative (PCBM) as a photoelectric converter. Because folic acid played an essential role in eye growth, we particularly modified the polythiophene derivatives with folic acid tags. Therefore, the artificial retina could enlarge the contact area and even recognize the nerve cells to improve the consequence of nerve stimulation. We implanted the artificial retina into blinded rats' eyes. Electrophysiological analysis revealed its recovery of photosensitive function 3 months after surgery. Our work provides an innovative idea for fabricating a high-resolution bionic artificial retina system. It shows great potential in artificial intelligence and biomedicine.
Visual prosthesis-a new hope for vision restoration

Abstract:Severe retinal pathologies such as age-related macular degeneration and retinitis pigmentosa are still uncurable so far. A visual prosthesis is a device that can electrically stimulate a certain site on the visual pathway to replace the function of the diseased neurons, which has been proved a feasible approach to partially restore vision for blind patients. To date a number of various technical schemes by various groups in the world have been proposed and are currently at different stages of basic research or clinical trials. In this article we present recent progresses in visual prosthesis development of foreign research groups as well as that of the Chinese Project for Sight (C-sight) group in China. A breif introduction of progress in commercialization of visual prosthesis was also presented.

Abstract:Severe visual impairment can result from retinal degenerative diseases such as retinitis pigmentosa, which lead to photoreceptor cell death. These pathologies result in extensive neural and glial remodelling, with survival of excitable retinal neurons that can be electrically stimulated to elicit visual percepts and restore a form of useful vision. The Phoenix99 Bionic Eye is a fully implantable visual prosthesis, designed to stimulate the retina from the suprachoroidal space. In the current study, nine passive devices were implanted in an ovine model from two days to three months. The impact of the intervention and implant stability were assessed using indirect ophthalmoscopy, infrared imaging, and optical coherence tomography to establish the safety profile of the surgery and the device. The biocompatibility of the device was evaluated using histopathological analysis of the tissue surrounding the electrode array, with a focus on the health of the retinal cells required to convey signals to the brain. Appropriate stability of the electrode array was demonstrated, and histological analysis shows that the fibrotic and inflammatory response to the array was mild. Promising evidence of the safety and potential of the Phoenix99 Bionic Eye to restore a sense of vision to the severely visually impaired was obtained.
