Clinicopathological Features and Short-Term Prognosis of Gastrointestinal Stromal Tumor with Hemorrhage

Abstract:Gastrointestinal stromal tumors (GIST) are the most common mesenchymal tumor in the gastrointestinal tract. This study aimed to determine the characteristics and prognosis of GIST, especially those with gastrointestinal bleeding. A total of 131 GIST patients in Affiliated Drum Tower Hospital of Nanjing University Medical College between January 2016 and December 2017 were enrolled. Their data were collected from the case records and retrospectively analyzed. Follow-up was by telephone and outpatient review. Of the 131 patients 46 had gastrointestinal bleeding (bleeding group) and 85 did not have bleeding (no bleeding group). The two groups were comparable regarding mean age and gender composition. Significant differences were seen between the groups in the prevalence of comorbidities and the site of the GIST. Preoperative neutrophil count, lymphocyte count, hemoglobin level, platelet count, serum albumin, neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, positive fecal occult blood, CA724, and prothrombin time were significantly associated with bleeding. The National Institutes of Health (NIH) risk grade (P = 0.008), Dog1 positivity (P = 0.01), and type of surgery (P = 0.023) were also significantly different between the groups. On multivariate logistic regression analysis low hemoglobin (P < 0.001), high platelet count (P = 0.033), high platelet-to-lymphocyte ratio (P = 0.015), and NIH risk category “High” (P = 0.026) were independent risk factors for bleeding. Thus, we concluded that gastrointestinal bleeding is one of the clinical manifestations of GIST and low hemoglobin, high platelet count, high PLR appear to be independent risk factors for gastrointestinal bleeding in GIST patients.
