Abstract:Vascular calcification (VC), which is closely associated with significant mortality in cardiovascular disease, chronic kidney disease (CKD), and/or diabetes mellitus, is characterized by abnormal deposits of hydroxyapatite minerals in the arterial wall. The impact of oxidative stress (OS) on the onset and progression of VC has not been well described. Nicotinamide adenine dinucleotide phosphate (NADPH) oxidases, xanthine oxidases, myeloperoxidase (MPO), nitric oxide synthases (NOSs), superoxide dismutase (SOD) and paraoxonases (PONs) are relevant factors that influence the production of reactive oxygen species (ROS). Furthermore, excess ROS-induced OS has emerged as a critical mediator promoting VC through several mechanisms, including phosphate balance, differentiation of vascular smooth muscle cells (VSMCs), inflammation, DNA damage, and extracellular matrix remodeling. Because OS is a significant regulator of VC, antioxidants may be considered as novel treatment options.
What problem does this paper attempt to address?
The main issue this paper attempts to address is the exploration of the mechanisms by which oxidative stress (OS) influences the occurrence and development of vascular calcification (VC). Specifically, the paper focuses on the following aspects:
1. **Association between Oxidative Stress and Vascular Calcification**:
- Vascular calcification is a pathological process closely related to cardiovascular diseases, chronic kidney disease (CKD), and diabetes, characterized by abnormal deposition of hydroxyapatite minerals in the arterial wall.
- Oxidative stress promotes vascular calcification through various mechanisms, including phosphate balance, differentiation of vascular smooth muscle cells (VSMCs), inflammatory response, DNA damage, and extracellular matrix remodeling.
2. **Production and Clearance Systems of Oxidative Stress**:
- The paper details the production systems of oxidative stress, including NADPH oxidase (NOX), xanthine oxidase, myeloperoxidase (MPO), nitric oxide synthase (NOS), etc.
- Additionally, the paper discusses the clearance systems of oxidative stress, such as the roles of superoxide dismutase (SOD) and paraoxonase (PON).
3. **Molecular Mechanisms**:
- **Phosphate Balance**: High phosphate levels promote vascular calcification by activating the NF-κB signaling pathway and increasing ROS production.
- **Differentiation of Vascular Smooth Muscle Cells**: Persistent strong stimuli lead to the transformation of VSMCs into an osteoblast-like phenotype, accompanied by elevated expression of bone-related genes such as BMP-2 and Runx2.
- **Inflammation**: The inflammatory response participates in the process of vascular calcification by upregulating the expression of growth factors, chemokines, cytokines, matrix metalloproteinases (MMPs), and adhesion molecules.
4. **Potential Therapeutic Strategies**:
- Since oxidative stress is a significant regulator of vascular calcification, antioxidants may become new therapeutic options. The paper discusses the potential roles of various antioxidants (such as resveratrol, tempol, MnTMpyP, etc.) in inhibiting vascular calcification.
In summary, this paper aims to provide a comprehensive overview of the mechanisms by which oxidative stress influences vascular calcification and offers new insights for early prevention, diagnosis, and treatment.
