Qualitative study on the problems and potential solution strategies for part-time clinical pharmacists clinical services work in a tertiary hospital in China

Abstract:Objective To provide an initial understanding of problems and potential solution strategies for part-time clinical pharmacist work in China, and provide references for the training of part-time clinical pharmacists. Methods The study was conducted in a tertiary teaching hospital in China, and the project lasted 6 months. Phenomenological methods were used to guide the research design. Research data were obtained by conducting one-to-one semistructured interviews with part-time clinical pharmacists, and interview data were coded and analysed through thematic analysis. Results A total of 21 pharmacists were interviewed in a semistructured manner, and the results showed that following problems exist in the work of part-time clinical pharmacists: the existing professional knowledge is not adequate to meet the demands of clinical service; the career orientation of part-time clinical pharmacists is not clear; lack of professional self-confidence in clinical pharmacy practice; there is no suitable entry point to carry out pharmacy service work; it is difficult to communicate effectively, and for in addition, 17 potential solution strategies are proposed for the current problems, which can provide reference for the development of part-time clinical pharmacists' work. Conclusions The work performed by part-time clinical pharmacists is currently immature and the strategies derived from this study may serve as potential solutions to resolve the part-time clinical pharmacy practice challenges.

Their understanding of diseases and comprehensive clinical application of drugs are relatively insufficient, and they are unable to carry out pharmaceutical care work using a systematic clinical thinking mode. 2. **The professional orientation of part - time clinical pharmacists is not clear**: - Since the main work of part - time clinical pharmacists is still concentrated on ensuring drug supply, clinical pharmacy services are often interrupted due to participation in drug - checking and other work, and the work content of part - time clinical pharmacists lacks a unified standard. This leads to an unclear professional orientation for them, and doctors, nurses and patients also have a relatively vague understanding of this profession, with a low sense of professional identity. 3. **Lack of professional confidence in clinical pharmacy practice**: - Part - time clinical pharmacists generally lack professional confidence during the preparation period and at work, which is manifested as fear of unknown clinical problems, lack of confidence in their existing knowledge structure, communication anxiety with medical staff and patients, and lack of professional identity. These problems affect the effectiveness of their work. 4. **Lack of appropriate entry points for pharmaceutical services**: - Part - time clinical pharmacists are all transformed from hospital pharmacists and have no experience in patient - centered pharmaceutical services. Therefore, they have not yet formed a fixed working mode in China. In terms of working hours, they need to coordinate drug dispensing and clinical pharmacy services; in terms of working methods, they lack appropriate service entry points, which is not conducive to the development of part - time clinical pharmacists' careers and the continuous improvement of the team. 5. **Lack of communication skills and difficulty in effectively communicating with doctors, nurses and patients**: - Part - time clinical pharmacists lack clinical practice experience and usually have not received communication skills training. Therefore, they are unable to communicate effectively when providing clinical pharmacy services, resulting in the failure to achieve the expected results of pharmaceutical services. There is an urgent need to strengthen the training of pharmacists' communication skills in the future. To address these problems, the study proposed 17 potential solution strategies, which can provide references for the systematic training of part - time clinical pharmacists. Among them, the three most popular strategies are: - **Increase the introductory training of clinical pharmacy services** (Strategy 10) - **Develop training in communication skills and methods** (Strategy 16) - **Increase the service teaching by experienced clinical pharmacists** (Strategy 9) Through these strategies, the study hopes to provide valuable references for the training and development of part - time clinical pharmacists in China.
