Abstract:Objective To investigate the prevalence of glucocorticoid-induced osteoporosis (GIOP) and the associated risk factors.Methods Patients with rheumatic disease were investigated in several hospitals in Shanghai and Zhejiang.We investigated the demographic data,the usage of glucocorticoids and other medications,the laboratory tests results including blood routine test,biochemical profiles,bone metabolism biomarkers as well as bone mineral density (BMD) were collected.The prevalence of GIOP and the relationship between the category of rhematic diseases,disease course,the dose of glucocorticoids used,other medications and therapy for osteoporosis.Results Six hundred and five patients were investigated,including patients with systemic lupus erythematosus (SLE),rheumatiod arthritis (RA),Sj(o)gren's syndrome (SS),vasculitis and other connective tissue diseases.Among them,39.0％ (236 cases) patients had osteoporosis (OP),41.7％(252 cases) patients had osteopenia,which was significantly higher than general population (19.3％,117 cases).The patients with abnormal BMD accounted for 80.3％ (159 cases) and 84.6％ (148 cases) of them,respectively.The high risk factors of GIOP included age,the course of disease,height,menopause and history of fracture.In our study,45.5％(275 cases) patients did not accept any GIOP prevention medication.Conclusion Patients who have long-term glucocorticoids therapy tend to suffer from OP.The incidence of GIOP increases with the course of the disease.Therefore,we should pay more attention to the prevention of GIOP.

Abstract:OBJECTIVETo investigate the use of glucocorticoids in patients with severe community-acquired pneumonia (SCAP) in the intensive care unit (ICU) of Hospitals in Zhejiang Province and to provide a reference for guiding clinical use of SCAP patients.METHODSTo draw up a questionnaire with reference to the Chinese and international guidelines, and to investigate the knowledge of community-acquired pneumonia (CAP) related guidelines and the use of glucocorticoids in patients with SCAP by doctors in hospitals above secondary level in Zhejiang Province by Email. Then the valid questionnaire was analyzed.RESULTSIn June 2016, 340 questionnaires were distributed, and all were returned after 2 months, with 333 of valid; 333 doctors from 45 ICUs in Zhejiang Province participated in the survey. (1) The knowledge of CAP-related guidelines in ICU doctors: 79.58% (265/333) of the doctors had read the CAP guidelines, and those who work over 10 years had a higher reading rate than those with 1-5 years and 6-10 years [93.07% (94/101) vs. 74.00% (111/150), 73.17% (60/82), both P < 0.05]. Post-graduates and above had higher reading rates than undergraduates [85.35% (134/157) vs. 74.43% (131/176), P < 0.05]. Senior doctors had higher reading rates than the junior and intermediate doctors [93.07% (94/101) vs. 71.43% (80/112), 75.83% (91/120), both P < 0.05]. The rate of understanding the clinical application of glucocorticoids was 13.81% (46/333). The doctors who work over 10 years and the seniorshad a relatively high awareness rate, 23.76% (24/101) and 20.79% (21/101) respectively. However, there was no significant difference in the awareness rate between doctors with different degrees and different levels of hospitals. (2) For the use of glucocorticoids in different causes of pneumonia, 44.74% (149/333) of doctors routinely used glucocorticoids in severe viral pneumonia. The proportion of glucocorticoids used in severe bacterial pneumonia, severe fungal pneumonia, severe pneumocystis pneumonia, chronic obstructive pulmonary disease (COPD) and severe pneumonia were 22.82% (76/333), 9.31% (31/333), 22.52% (75/333) and 18.32% (61/333), respectively. (3) The way of glucocorticoid usage: 79.58% (265/333) of doctors chose methylprednisolone, 4.20% (14/333) chose hydrocortisone, 1.20% (4/333) chose dexamethasone, and 15.02% (50/333) had not use glucocorticoids. The proportion of physicians who chose to use glucocorticoids within 24 hours of admission and 1-7 days after admission were 52.65% (149/283) and 47.35% (134/283), respectively. Glucocorticoids were used more in doctors with lower academic qualifications and hospitals within 24 hours. The undergraduate degree was 61.39% (97/158), and the second-grade class hospital was 67.50% (27/40). Among the doctors who chose methylprednisolone, 60.75% (161/265) prescribe the dose ≤ 80 mg/d; 79.15% (224/283) chose the course of ≤ 7 days. The number of years of work, education, professional title and hospital grade had no significant effect on the choice of methylprednisolone and the course of treatment.CONCLUSIONSICU doctors of 45 hospitals in Zhejiang Province have a high degree of heterogeneity in the understanding of the use and guidelines of glucocorticoids in SCAP. It is necessary to strengthen the ICU doctor's study of clinical guidelines at home and abroad and to develop a glucocorticoid use plan according to the specific conditions of patients, so that SCAP patients can benefit more.
A cross-sectional survey of glucocorticoids application in patients with rheumatoid arthritis

Abstract:Objective To investigate the application of glucocorticoids in patients with rheumatoid arthritis(RA) in China.Methods Totally 1 095 cases of RA from 21 hospitals in China from January 2009 to August 2010 were retrospectively enrolled.Clinical data,laboratory data and application of glucocorticoids were analyzed.Results In 1 095 patients with RA,417 patients(38.1％)were treated with glucocorticoids,detailed dosages of glucocorticoid were available in 359 cases of them.In these 359 cases,219 cases(61.0％) were treated with lowdose glucocorticoid(≤ 10 mg/d prednisone equivalent dose),119 cases(33.1％) were treated with medium-dose glucocorticoid(＞ 10 mg/d and ≤30mg/d prednisone equivalent dose) and 21 cases(5.9％) were treated with high-dose glucocorticoid(＞ 30mg/d prednisone equivalent dose);107 cases(30.3％) had treatments shorter than 6 months and 246 cases(69.7％) were treated longer than 6 months.All patients were divided into glucocorticoid group (417 cases) and non-glucocorticoid group(678 cases).The total course of RA,the course of diagnosis,the number of abnormal joints,the proportion of extra articular symptoms and the health assessment questionaire(HAQ)score in glucocorticoid group were all significantly higher than those in non-glucocorticoid group [87.6(32.4,176.4) months vs 58.2 (17.7,139.2) months;4.1 (0.0,27.4) months vs 2.0 (0.0,12.9) months;2(0,6) vs 1(0,4);31.2％(130/417) vs 20.4％ (138/678);0.8(0.1,1.6)scores vs 0.5(0,1.4)scores] (all P ＜ 0.05).Proportions of antirheumatic drug,biologics and non-steroidal anti-inflammatory drug applications had no significant differences between groups (P ＞ 0.05).Conclusion At present,glucocorticoids is wildly used in severe RA patients in China,but the application needs to be standardized.
Effects of the working experience, educational background, professional titles, and hospital grades of intensive care unit doctors on clinical glucocorticoid use in acute respiratory distress syndrome

Abstract:Although glucocorticoids are commonly used for patients with acute respiratory distress syndrome in the intensive care unit, the exact attitudes of different intensive care unit (ICU) doctors about glucocorticoid usage are largely unknown. Herein, we investigated the practice of glucocorticoid application for acute respiratory distress syndrome (ARDS) by ICU doctors in China. Questionnaires were developed and sent to ICU doctors at 45 hospitals to perform statistics and analysis. ICU doctors with more working experience and professional titles had more knowledge of ARDS. Glucocorticoids were more likely to be used for ARDS caused by chemical inhalation. Doctors with longer working experience, better educational background, and higher professional titles used fewer glucocorticoids. In addition, 97.2%of the doctors considered using methylprednisolone or hydrocortisone first, 50.9% used glucocorticoids within 24hours of onset, and 37.1% insisted that steroid therapy should last 3 to 5days. Although ICU doctors with more working experience and professional titles have a better understanding of glucocorticoid use in ARDS, the majority of clinical practices and attitudes are similar among different doctors regardless of working experience, educational background, professional titles, or hospital grades.
Efficacy and Safety of Glucocorticoids in Treatment of COVID-19: a Retrospective Study.

Abstract:Background: Novel coronavirus pneumonia is a novel kind of highly contagious disease without any specific drugs. Considering the successful experience of antiviral therapy combined with glucocorticoids (GCs) in severe acute respiratory syndrome, this study was designed to evaluate the clinical efficacy of GCs in treating patients with coronavirus disease 2019 (COVID-19). Methods: A cohort of 42 patients with COVID-19 admitted to The First Hospital of Jiaxing from January 4, 2020, to February 16, 2020, were included and grouped into a test group (n=20) and control group (n=22) based on their therapeutic regimens. There were no significant differences in baseline characteristics between patients in the two groups. Conventional treatment (antiviral therapy) was given to patients in both groups, while an additional hormone drug (GCs) was used in patients in the test group. Indices including body temperature, blood routine indices [white blood cell (WBC), lymphocyte, monocyte, and C-reactive protein (CRP)], blood biochemical indices [alanine aminotransferase (ALT) and aspartate aminotransferase (AST)], and complications were recorded during the treatment. Time to achieve negative virus nucleic acid (nCoV-RNA) testing, and hospital stays were also observed and compared between the two groups. Results: All included patients completed the trial. After treatment, superior therapeutic efficacy was achieved in patients in the test group, with body temperature dropping more significantly with a much shorter recovery time compared to the control group (P=0.0412). Simultaneously, the percentage of patients with abnormal blood routine indices (WBC), monocyte, and (CRP) in the test group was reduced more sharply, while no noticeable difference was observed in the number of patients who developed abnormal blood biochemical indices during treatment between the two groups. Additionally, a shorter duration of hospital stays was found in the test group relative to the control group (14.84 +/- 8.76 vs. 18.25 +/- 7.42 days, P>0.05). Patients who received GCs had a shorter recovery time for body temperature and inflammation. Conclusions: Hormonotherapy with GCs can accelerate the recovery time for body temperature as well as inflammation in patients with COVID-19. It deserves promotion and application in the clinical treatment of coronavirus disease as a form of adjuvant medicine. The ongoing focus of research is on long-term adverse events in GCs.

Abstract:Background:EULAR and ACR guidelines recommend a treat-to-target approach for patients with RA including regular assessments of disease activity. Glucocorticoids are commonly used to control inflammation associated with uncontrolled disease. However, patients using glucocorticoids may develop short- and long-term side effects.Objectives:To examine the real-world use of glucocorticoids among patients with RA who are disease-modifying antirheumatic drug (DMARD)-naïve or failing their first conventional synthetic DMARD (csDMARD) or biologic DMARD (bDMARD).Methods:From a large US health claims database, this study included adults with ≥2 RA claims ≥30 days apart who started (index date [ID], 1/1/2012–3/31/2017) a first DMARD (DMARD-naïve) or patients who newly initiated a csDMARD and then switched to or added another DMARD (csDMARD switchers), and patients who initiated a first bDMARD and then switched to another bDMARD or Janus kinase inhibitor (JAKi; bDMARD switchers). All patients had continuous enrollment 1-year before and ≥1 year after ID and were evaluated for pre- and post-ID use of glucocorticoids (oral or injectable), prednisone equivalent dose (PED), and duration of exposure ≥30 days.Results:The study included 28,201 patients in the DMARD-naïve cohort, 7,816 csDMARD switchers, and 4,656 bDMARD switchers (median age 54 years for all, 73%–78% female).Among DMARD-naïve patients, 66.5% used glucocorticoids during the pre-ID period (Figure 1) and 61.2% had >7.5 mg/day PED, 21.2% had >30 mg/day PED, and 21.2% had ≥30 days of exposure to glucocorticoids (Figure 2). Post-ID, 69.4% of patients used glucocorticoids, while 54.7% had >7.5 mg/day PED, 13.5% had >30 mg/day PED, and 44.9% had ≥30 days of exposure to glucocorticoids.Among csDMARD switchers, 84.5% of patients used glucocorticoids during the pre-ID period (Figure 1), and 73.4% had >7.5 mg/day PED, 16.0% had >30 mg/day PED, and 56.4% had ≥30 days of exposure to glucocorticoids (Figure 2). During the post-ID treatment, 74.1% of patients used glucocorticoids, 56.2% had >7.5 mg/day PED, 14.4% had >30 mg/day PED, and 45.8% had ≥30 days of exposure to glucocorticoids.Among bDMARD switchers, 85.1% of patients used glucocorticoids in the pre-ID period (Figure 1), and 70.2% had >7.5 mg/day PED, 17.4% had >30 mg/day PED, and 55.2% had ≥30 days of exposure to glucocorticoids (Figure 2). During post-ID treatment, 75.4% of patients used glucocorticoids and 59.7% of patients had >7.5 mg/day PED, 16.7% had >30 mg/day PED, and 45.6% had ≥30 days of exposure to glucocorticoids.Conclusion:Real world glucocorticoid use was high in all categories of DMARD-treated RA patients, at baseline and during their next treatment, suggesting ongoing medical needs. Glucocorticoid doses exceeded 7.5 mg/day for most patients. A Survey to Glucocorticoid Induced Oseoporosis Management in Rheumatologists in China

Abstract:Objective To investigate the management protocols about glucocorticoid induced osteoporosis by rheumatology physicians of China.Methods A questionnaire survey was conducted among 136 rheumatology physicians.Results 101 effective questionnaires were collected.58.4% responders have had continuous medical education(CME)about GIOP;67.3% respondents considered dual X-ray absorptiometry the preferred technique to assess bone mineral density,but 84.2% preferred radiation in daily practice.In dealing with postmenopausal patients starting long-term steroid therapy,87.1% rheumatologists considered base bone mass should be measured,and calcium+ vitamin D was recommended by 89.1% rheumatologists,bisohosphonates or HRT were recommended by 27.8%rheumatologists only.In dealing with young women patients starting long-term steroid therapy,only 42.6% rheumatologists considered base bone mass should be measured,47.5% preferred bisohosphonates to treat GIOP.Only 35.6% rheumatologist considered calcium alone can not preserve bone mass;and only 5.9% considered calcitonin can not prevent the fracture induced by GIOP.The responders who have had CME about GIOP were prone to take appropriate management.Conclusion The survey demonstrates the lack of consensus in management of GIOP by the rheumatologist in China with the internationally accepted guideline.High quality CME may improve the decision-making ability of rheumatologist.

Retrospective study of side effects of low-dose glucocorticoids in rheumatoid arthritis

Abstract:Objective To investigate the side effects of low-dose glucocorticoids (GCs) in patients with rheumatoid arthritis (RA). Methods The adverse events associated with chronic use of low-dose GCs in 164 patients with RA were analysed. Results Among 164 patients, 68 (41.5%) patients were treated with GCs, 47 (28.7%) patients with low-dose GCs (≤10 mg/d) for at least 6 months and 21 (12.8%) patients with high-dose GCs. In comparison of low-dose GCs group with GCs free group, 14.9% (7/47) versus 17.7% (17/96) developed hypertension, 4.3% (2/47) versus 4.2% (4/96) developed diabetes mellitus, 4.3% (2/47) versus 3.1% (3/96) developed fracture, 10.6% (5/47) versus 4.2% (4/96) developed peptic ulcer, 31.9% (15/47) versus 13.5% (13/96) developed hyperlipoidemia, 4.3% (2/47) versus 1.0% (1/96) developed cataracts and 0 versus 1.0% (1/96) developed glaucoma. There were no significant differences in the incidences of hypertension and diabetes mellitus between the two groups (P0.05). The low-dose GCs group had higher incidences of hyperlipoidemia (P0.05). Conclusion Chronic use of low-dose GCs therapy appears to increase the risk of hyperlipoidemia, but not hypertension and diabetes mellitus in patients with RA.
Use of Glucocorticoids in Patients with COPD Exacerbations in China: a Retrospective Observational Study

Abstract:Background: Acute exacerbations of chronic obstructive pulmonary disease (AECOPD) are common in patients with underlying moderate to severe COPD and are associated with increased health and economic burden. International and Chinese guidelines recommend using glucocorticoids for the management of AECOPD because glucocorticoid therapy has been shown to benefit clinical outcomes. However, only scant data are available for current status of glucocorticoid therapy in hospitalized AECOPD patients in China. The aim of the study was to identify current use of glucocorticoids for the treatment of AECOPD in China. Methods: This retrospective, multicenter, noninterventional study evaluated the treatment pattern of AECOPD in patients hospitalized from January 2014 to September 2014 at 43 sites (41 tertiary hospitals and two secondary hospitals) in China. The endpoints of the study were the percentage of patients receiving glucocorticoids by different routes of administration, doses and duration, mortality, and the mean length of hospitalization. Results: A total of 4569 patients (90.17%) received glucocorticoids for AECOPD treatment. A combination of nebulized and systemic route was most frequently used (40.51%), followed by using nebulized route alone (38.00%), systemic route alone (15.45%), and inhaled route other than nebulization (6.04%). Furthermore, the most commonly prescribed glucocorticoids of the nebulized, intravenous, inhaled (other than nebulized) and oral route was budesonide (69.4%), methylprednisolone sodium succinate (45.31%), fluticasone propionate (19.54%), and prednisone acetate (11.90%), respectively. The in-hospital mortality rate was 1.24% and the mean length of hospitalization was 12.22 ± 6.20 days (± SD). Conclusions: Our study was the first study of the treatment pattern of glucocorticoids in the management of hospitalized AECOPD patients in China. Data indicates that there is a gap in the implementation of international guidelines for the treatment of AECOPD in China. Further studies are warranted to clarify the appropriate glucocorticoids strategy for the management of AECOPD to determine the optimal route of administration, dose and duration, and resulting clinical outcomes.
Asthma with Allergic Rhinitis Management in China: a Nationwide Survey of Respiratory Specialists at Tertiary Hospitals.

Abstract:BackgroundMany asthmatic patients have coexisting allergic rhinitis (AR). This study aims to investigate the compliance of physicians with respiratory medicine specialty (PRMs) to Global Initiative for Asthma (GINA) and Allergic Rhinitis and its Impact on Asthma (ARIA) guidelines during the management of their asthma-AR patients.MethodsThis cross-sectional questionnaire study surveyed the diagnostic methods and treatment patterns for asthma-AR comorbidity by PRMs from 98 hospitals across China.ResultsPRMs reported an estimated prevalence of asthma-AR comorbidity of >30% at their clinics. PRMs with greater work experience and a higher professional title estimated treating a significantly higher proportion of patients with AR within the previous month (p=0.002 and p<0.001, respectively); they also estimated a higher incidence of patients with AR in the autumn (p<0.001 and p<0.001). A higher percentage of PRMs with 11 years work experience prescribed nasal steroids for AR as compared to those with 1 to 10 years experience (56.9% vs 44.7%, p=0.002). A greater proportion of chief physicians used leukotriene modifiers and a lower proportion used antihistamine H-1-receptor blockers for AR as compared to residents (resident vs assistant chief: 27.5% vs 11.6%, p=0.002; and resident vs chief PRMs: 27.5% vs 9.5%, p=0.001).ConclusionPRMs in China demonstrated an up-to-date comprehension of asthma management (>90%); however, knowledge gaps existed in their concepts of AR and asthma-AR comorbidity. Thus, further education is warranted for PRMs regarding the importance of AR in asthma patients, definitive diagnosis (allergy tests), classifications of AR, and treatment guidelines for the asthma-AR comorbidity.

Abstract:Rheumatoid arthritis (RA) is an autoimmune disease characterized by synovial inflammation of the joints with high risk of disability. In recent years, remarkable progress has been made towards the diagnosis and treatment of RA, and the international RA guidelines have been also kept updated. Nevertheless, there are many challenges in China, especially inadequate number of rheumatologists and insufficient experience in the diagnosis and treatment of RA. Therefore, Chinese Rheumatology Association drafted the standardized diagnosis and treatment of RA based on the available evidence, so as to improve the management of RA patients in China.
Knowledge, attitudes, and practices of dietary management among patients with rheumatoid arthritis in China

Abstract:Background: This study aims to assess the knowledge, attitudes, and practices (KAP) regarding dietary management among patients with rheumatoid arthritis (RA). Methods: A cross-sectional survey was conducted at JiuJiang NO.1 People's Hospital from November 2023 to May 2024. Data were gathered using structured questionnaires that solicited demographic information and measured KAP scores related to dietary management in RA patients. Results: The survey yielded 504 valid questionnaires. Of the respondents, 306 (60.71%) had a Body Mass Index (BMI) within the optimal range of 18.5–23.9 kg/m 2 . The mean scores for knowledge, attitude, and practice were 10.13 ± 3.58 (possible range: 0–22), 31.38 ± 2.38 (possible range: 9–45), and 4.46 ± 2.30 (possible range: 0–12), respectively. Correlation analysis revealed significant relationships between knowledge and practice ( r = 0.294, p < 0.001) as well as between attitude and practice ( r = 0.178, p < 0.001). Multivariate logistic regression showed that knowledge score (OR = 1.165, 95% CI: [1.078, 1.259], p < 0.001) was independently associated with proactive practice. The structural equation model (SEM) results showed that knowledge had direct effects on attitude ( β = 0.291, p < 0.001) and practice ( β = 0.188, p < 0.001). Meanwhile, attitude had a direct impact on practice ( β = 0.081, p = 0.045). Conclusion: This study indicates that patients with RA generally demonstrate inadequate knowledge but hold positive attitudes toward dietary management, despite relatively inactive practices in implementing such dietary changes. Healthcare providers should prioritize educational interventions that not only enhance patient understanding but also actively support the implementation of dietary management strategies in clinical settings.

Investigation of current situation of 694 Patients with Rheumatoid Arthritis in China

Abstract:Objective To investigate the current status of patients with rheumatoid arthritis in China.Methods Multicenter and cross-sectional investigation was made from March 15, 2007 to June 30,2007 in 16 centers in China, and 694 patients (114 men, 580 women, mean ±SD age 50±13 years, mean ±SD course 6±7 years) with rheumatoid arthritis were investigated who were diagnosed based on the classification criteria for rheumatoid arthritis published by American College of Rheumatology in 1987.Results In the total of 694 cases, 133 had systemic damages and 44 of them suffered with more than two systems damages.In all the 694 cases, the mean level of ESR and CRP was 44mm/h and 2.1mg/dl respectively.In addition, the mean number of VAS was 69.For the majority of patients, the therapy option was combined medications, wherein DMARDs (49.28%), NSAIDs (22.86%), steroid hormones (11.63%) and herbs (9.84%) were the major options, while biological agents were used only in a minority of patients (1.16%).Efficacy and side effects were still the most relevant factors for therapy.Conclusion There are many patients with moderate to severe rheumatoid arthritis.Some of them have systemic damage.The DMARDs, NSAIDs, steroid hormones and herbs are still the major therapeutic agents, Efficacy and side effects are the core factors that affects the choice of therapeutic regiomens.

In this two-stage, single-center, non-randomized controlled trial, we investigated the benefits of combining Chinese traditional herbal treatment with csDMARDs to aid GC discontinuation in terms of GC tapering, disease control, and safety. A total of 231 participants were enrolled, of which 150 eligible subjects were included in the first phase and allocated to three groups (control group, treatment group 1, and treatment group 2) based on their willingness to take traditional Chinese medicine and syndrome differentiation, in a 1:1:1 ratio. All groups received basic treatment consisting of methotrexate tablets (10 mg, qw), leflunomide (10 mg, qd), and stratified GC bridging therapy and tapering regimen (The intervention regimen was developed based on rigorous adherence to available evidence). Treatment group 1 received basic treatment combined with Juanbi Granule, while treatment group 2 received basic treatment combined with Yupingfeng Guizhi Decoction Granule. Efficacy was evaluated after a 12-week follow-up, with slightly adjustments to the treatment group based on efficacy and change of syndrome, followed by continued observation until 24 weeks to complete the study. The efficacy evaluation and data analysis were conducted in a blinded manner, including group label concealment, data cleaning, confounder and control regimen analysis, and outcome analysis. The characteristics and its contributing factors of refractory rheumatoid arthritis, view of the rheumatologists of China: results of a nationwide cross-sectional survey

Abstract:Objective: To explore the characteristics of refractory RA and its contributory factors based on the understanding of the rheumatologists of China. Methods: A national cross-sectional survey was performed in 32 provinces across China, and 1381 rheumatologists were recruited. Information about the sociodemographic background, refractory RA characteristics, and contributory factors was collected using a pre-made questionnaire including fourteen single-choice and four multiple-choice questions, respectively. Each of the single-choice and multiple-choice question had several items. Results: In response to the single-choice questions regarding the definition of refractory RA, 52.28% responded that "the 28 joint disease activity score-erythrocyte sedimentation rate (DAS28-ESR) > 5.1 with presence of signs suggestive of inflammatory disease" is characteristic of refractory RA, whereas 50.18% opined that "If target was not achieved with 2 conventional synthetic disease-modifying anti-rheumatic drugs (csDMARDs) combined with 1 biological or 1 targeted synthetic DMARD (b/tsDMARD), over a total of 3~6 months" should be considered refractory RA. In addition, 39.32% rheumatologists were in favor of inability to taper glucocorticoids (GCs) ≤ 10mg prednisone or equivalent daily. Additional features considered characteristics of refractory RA were extra-articular manifestations (84.94%), interfering comorbidities (76.32%), and radiographic progression (71.83%). Among the contributory factors, 92.03% considered infection, and interstitial lung disease was chosen by 90.95%, vasculitis by 82.26%, osteoporosis by 70.67%, and fibromyalgia syndrome by 70.38%. Conclusions: Our survey has demonstrated a variety of concepts of refractory RA among the rheumatologists of China. Disease activity assessment, treatment options, interfering comorbidities, and radiological progression all were associated with the characteristics of refractory RA. Key Points • This study demonstrated the definition, characteristics, and contributing factors of refractory RA from the rheumatologists' views in China. • The clarification of the insights and concepts on refractory RA will help to make comprehensive guidelines to treat this disease, further improving prognosis and reducing the societal burdens of RA.

Abstract:Background: Glucocorticoids (GC) are currently recommended as bridging therapy in combination with csDMARDs in rheumatoid arthritis (RA) and should be tapered as rapidly as clinically feasible for safety concerns about their long-term use [1-3]. Objectives: To unravel the dynamical trajectory and characteristics of GCs tapering and discontinuation in RA patients commencing GCs with concomitant csDMARDs. Methods: We used data from longitudinal real-world TARRA (Treat-to-TARget in RA) cohort in Peking University First Hospital. RA patient who started GCs and contaminant csDMARDs therapy over 1-year follow-up were included. The changes in GCs dose and disease activity in the context of csDMARDs were evaluated. GCs discontinuation rate was analyzed using Kaplan-Meier analysis. The relapse profiles within 6 months after GCs discontinuation were also analyzed. Results: A total of 207 RA patients were included. During a median follow-up duration of 38.6 months, 124 (59.9%) patients discontinued GC. The median oral prednisolone dose of 10 (5-10) mg/d at initiation was reduced by 50% in the first 6 months and then more slowly reduced, finally to zero by 48 months. The cumulative probability of GCs discontinuation was 26.6% at year 1, 48.0% at year 2, 58.6% at year 3, with calculated median time of 27 months (Figure 1). Of the 124 patients who discontinued GCs, add of other csDMARDs or increment of current csDMARDs was required in 29.0% of them. Approximately half of 124 patients were in clinical remission at the time point of GCs discontinuation. Within 6 months after GCs withdrawal, 79.1% (91/115) of participants maintained relapse-free. Prediction of Flare Following Glucocorticoids Withdrawal in Rheumatoid Arthritis Patients with Continuation of Csdmards: a Real-Life Study.

Abstract:Background In patients with rheumatoid arthritis (RA), glucocorticoids (GC) should be firstly discontinued before considering tapering conventional synthetic disease-modifying anti-rheumatic drugs (csDMARD) or other biological agents. We aim to determine risk factors for flare after GC withdrawal in RA patients undergoing csDMARDs. Method RA patients who discontinued GC with continuation of csDMARD were selected from a longitudinal real-world cohort. Established RA was defined as disease duration over 12 months. Dissatisfied RA control was defined as the proportion of simplified disease activity index (SDAI)-based remission time to total time from GC initiation to discontinuation less than 50%. Logistic regression was used to analyze the independent risk factors for flare after GC discontinuation and results were expressed as odds ratio (OR). Results There were 115 eligible RA patients discounted GC with continuation of csDMARDs (methotrexate: 80%; hydroxychloroquine: 61%; csDMARDs combination: 79%). Of these, 24 patients experienced flare after GC discontinuation. Compared with relapse-free patients, flare patients were more likely to have established RA (75% vs. 49%, p=0.025), higher median cumulative prednisolone dosages (3.3g vs. 2.2g, p=0.004), and higher proportion of dissatisfied RA control during GC usage (66% vs. 33%, p=0.038). In multivariate analysis, significantly increased flare risk was predicted by established RA (OR 2.93 [1.02-8.43]), cumulative prednisolone dose > 2.5g (OR 3.69 [1.34-10.19]) and dissatisfied RA control (OR 3.00 [1.09-8.30]). Flare risk was increased with increases in number of risk factors, with highest OR of 11.56 in patients with three risk factors (p for trend=0.002). Conclusions Flare following GC withdrawal is not common in RA patients with undergoing csDMARDs therapy. Established RA, higher cumulative GC dose and dissatisfied RA control before GC discontinuation are important factors associated with flare after GC withdrawal.
Investigating the Safety and Compliance of Using Csdmards in Rheumatoid Arthritis Treatment Through Face-to-face Interviews: a Cross-Sectional Study in China.

Abstract:Rheumatoid arthritis (RA) significantly impacts the health of Chinese patients. Conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) are used as the standard treatment for patients with RA. However, Chinese patients with RA have reported poor compliance with csDMARDs. This study aims to better understand the safety and compliance of using csDMARDs in RA treatment. Face-to-face interviews were conducted by questionnaires on safety and compliance of csDMARDs in 400 patients with RA and 100 rheumatologists from 13 cities in China. Rheumatologists were from Tier 3 Class A hospitals with independent rheumatology departments, who admitted more than 30 patients with RA per week. All patients were diagnosed for > 3 months before the survey and had been treated with csDMARDs for > 3 months. The incidence of adverse events (AEs) that attributed to csDMARDs estimated by rheumatologists was lower than that reported by patients for all four prescribed csDMARDs. Also, types of common AEs in rheumatologist's perception differed from those in the patient's report. Only 86% (116/135) of patients claimed they notified their rheumatologist about AEs, and 40.8% (150/368) of patients did not strictly adhere to their prescribed treatment. Reasons why patients were not compliant with their treatment, other than AEs, included symptoms being less severe, travel, and busy working life/business trips. This study revealed gaps in perceptions of csDMARDs-related AEs and medication adherence between rheumatologists and patients. These findings suggested adequate doctor-patient communications, and considerations of multiple real-world situations may improve adherence in the treatment of RA patients. Key Points • This study identified gaps in rheumatologists' perception of the prevalence and type of AEs experienced by their patients, which could potentially help them improve their patients' compliance with treatment.

Abstract:Background Biological disease modifying antirheumatic drugs (bDMARDs) are frequently used in combination with other drugs. Very limited data are available on concomitant therapy with bDMARDs in China. Objectives To investigate the usage patterns and safety of concomitant drugs in Chinese RA patients receiving bDMARDs. Methods Patients from 15 Chinese hospitals were recruited in this cross-sectional study. Consenting patients (aged ≥18 years) diagnosed with RA receiving bDMARDs were included. Results Data collected from 802 patients with a mean (SD) age of 49.0 (13.9) years (81.3% women) were analyzed. Among these patients, 89.5%, 56.1%, 29.7%, and 19.1% were receiving concomitant conventional DMARDs (cDMARDs), nonsteroidal anti-inflammatory drugs (NSAIDs), glucocorticoids (GC), and drugs for local application (LA drugs), respectively. In total, 718 patients were receiving concomitant cDMARDs. The proportion of patients using 1, 2, and 3 concomitant cDMARDs was 49.3%, 41.2%, and 9.5%, respectively. The most common cDMARDs were methotrexate, hydroxychloroquine, leflunomide, and sulfasalazine; used by 65.9%, 41.8%, 41.5%, and 6.3% patients at a mean (SD) daily dose (mg) of 1.4 (0.4), 340.3 (96.3), 15.4 (5.2), and 1753.3 (693.3), respectively. The respective mean (SD) duration of treatment (days) was 443.7 (845.8), 261.3 (409.4), 413.4 (578.5), and 429.1 (1039.9). Among the 238 patients on concomitant GC, 73.1% and 23.1% patients were receiving oral prednisone and methylprednisolone at a mean (SD) weekly dose (mg) of 57.8 (31.9) and 59.7 (151.6), respectively. In total, 17.6% patients reported at least one GC-associated adverse event (AE) at a mean (SD) duration of treatment (weeks) of 12 (28.3); the most common AEs were moon face (13.9%) and weight gain (4.2%). Among the 450 patients receiving concomitant NSAIDs, 43.1%, 20.2%, and 12.0% patients were receiving celecoxib, meloxicam, and loxoprofen sodium at a mean (SD) daily dose (mg) of 306.2 (100.1), 12.8 (3.5), and 143.9 (45.3), respectively. Most NSAID users were receiving NSAIDs at a dose lower than the daily maximum, except 2 patients using diclofenac acid at higher than daily maximum dose (150 mg). Only 3.6% NSAID users reported at least one AE; the most common AE was gastrointestinal discomfort (3.1%). A total of 153 patients were receiving concomitant LA drugs. The most common LA drugs were ketoprofen, diclofenac acid, and a Chinese herb medicine “Jia Wei Shuang Bai San”, used by 30.1%, 20.9%, and 10.5% patients, respectively. The corresponding mean (SD) treatment duration (weeks) was 2.0 (4.1), 7.8 (26.4), and 16.9 (28.0), respectively. The predominant reason (62.9% patients) for not using LA drugs was the lack of physician-directed prescription. Conclusions The usage patterns of concomitant cDMARDs and NSAIDs in Chinese RA patients receiving bDMARDs are similar to those in Western countries. The 17.6% concomitant GC users who reported at least one AE are receiving GC for longer time (12 weeks on average). LA drugs including traditional Chinese medicine offer a broad option for Chinese patients but are less commonly used due to lack of prescription from physicians. These findings suggest that usage patterns of concomitant GC and LA drugs might still need to be optimized for Chinese RA patients receiving bDMARDs.
