Abstract:From the early days of spaceflight to current missions, astronauts continue to be exposed to multiple hazards that affect human health, including low gravity, high radiation, isolation during long-duration missions, a closed environment and distance from Earth. Their effects can lead to adverse physiological changes and necessitate countermeasure development and/or longitudinal monitoring. A time-resolved analysis of biological signals can detect and better characterize potential adverse events during spaceflight, ideally preventing them and maintaining astronauts' wellness. Here we provide a time-resolved assessment of the impact of spaceflight on multiple astronauts (n = 27) by studying multiple biochemical and immune measurements before, during, and after long-duration orbital spaceflight. We reveal space-associated changes of astronauts' physiology on both the individual level and across astronauts, including associations with bone resorption and kidney function, as well as immune-system dysregulation.

Abstract:Research on astronaut health and model organisms have revealed six features of spaceflight biology that guide our current understanding of fundamental molecular changes that occur during space travel. The features include oxidative stress, DNA damage, mitochondrial dysregulation, epigenetic changes (including gene regulation), telomere length alterations, and microbiome shifts. Here we review the known hazards of human spaceflight, how spaceflight affects living systems through these six fundamental features, and the associated health risks of space exploration. We also discuss the essential issues related to the health and safety of astronauts involved in future missions, especially planned long-duration and Martian missions.

Abstract:Abstract NASA’s plans for space exploration include a return to the Moon to stay—boots back on the lunar surface with an orbital outpost. This station will be a launch point for voyages to destinations further away in our solar system, including journeys to the red planet Mars. To ensure success of these missions, health and performance risks associated with the unique hazards of spaceflight must be adequately controlled. These hazards—space radiation, altered gravity fields, isolation and confinement, closed environments, and distance from Earth—are linked with over 30 human health risks as documented by NASA’s Human Research Program. The programmatic goal is to develop the tools and technologies to adequately mitigate, control, or accept these risks. The risks ranked as “red” have the highest priority based on both the likelihood of occurrence and the severity of their impact on human health, performance in mission, and long-term quality of life. These include: (1) space radiation health effects of cancer, cardiovascular disease, and cognitive decrements (2) Spaceflight-Associated Neuro-ocular Syndrome (3) behavioral health and performance decrements, and (4) inadequate food and nutrition. Evaluation of the hazards and risks in terms of the space exposome—the total sum of spaceflight and lifetime exposures and how they relate to genetics and determine the whole-body outcome—will provide a comprehensive picture of risk profiles for individual astronauts. In this review, we provide a primer on these “red” risks for the research community. The aim is to inform the development of studies and projects with high potential for generating both new knowledge and technologies to assist with mitigating multisystem risks to crew health during exploratory missions.

Abstract:In the coming decades, as humanity aims to establish a presence on Mars, there is a growing significance in comprehending, monitoring, and controlling the diverse health challenges arising from space exploration. The extended exposure to microgravity during space missions leads to various physical alterations in astronauts, such as shifts in bodily fluids, reduced plasma volume, loss of bone density, muscle wasting, and cardiovascular deconditioning. These changes can ultimately lead to orthostatic intolerance, underscoring the increasing importance of addressing these health risks. Astronauts are exposed to cosmic radiation consisting of high-energy particles from various sources, including solar cosmic rays and galactic cosmic rays. These radiations can impact the electrical signals in the heart, potentially causing irregular heart rhythms. Understanding the risks to the heart and blood circulation brought on by exposure to space radiation and the overall stress of spaceflight is essential and this article reviews the cardiovascular effects of space travel on astronauts.

Abstract:Ionizing radiation and microgravity are two considerable health risks encountered during deep space exploration. Both have deleterious effects on the human body. On one hand, weightlessness is known to induce a weakening of the immune system, delayed wound healing and musculoskeletal, cardiovascular, and sensorimotor deconditioning. On the other hand, radiation exposure can lead to long-term health effects such as cancer and cataracts as well as have an adverse effect on the central nervous and cardiovascular systems. Ionizing radiation originates from three main sources in space: galactic cosmic radiation, solar particle events and solar winds. Furthermore, inside the spacecraft and inside certain space habitats on Lunar and Martian surfaces, the crew is exposed to intravehicular radiation, which arises from nuclear reactions between space radiation and matter. Besides the approaches already in use, such as radiation shielding materials (such as aluminium, water or polyethylene), alternative shielding materials (including boron nanotubes, complex hybrids, composite hybrid materials, and regolith) and active shielding (using fields to deflect radiation particles) are being investigated for their abilities to mitigate the effects of ionizing radiation. From a biological point of view, it can be predicted that exposure to ionizing radiation during missions beyond Low Earth Orbit (LEO) will affect the human body in undesirable ways, e.g., increasing the risks of cataracts, cardiovascular and central nervous system diseases, carcinogenesis, as well as accelerated ageing. Therefore, it is necessary to assess the risks related to deep space exploration and to develop mitigation strategies to reduce these risks to a tolerable level. By using biomarkers for radiation sensitivity, space agencies are developing extensive personalised medical examination programmes to determine an astronaut's vulnerability to radiation. Moreover, researchers are developing pharmacological solutions (e.g., radioprotectors and radiomitigators) to proactively or reactively protect astronauts during deep space exploration. Finally, research is necessary to develop more effective countermeasures for use in future human space missions, which can also lead to improvements to medical care on Earth. This review will discuss the risks space travel beyond LEO poses to astronauts, methods to monitor astronauts' health, and possible approaches to mitigate these risks.

Abstract:My journey to the stars began as I – along with the whole world – stood still and watched Neil Armstrong take those first small steps on the Moon. Fast forward 50 years and NASA astronauts Scott Kelly and Christina Koch each spend nearly a year in space aboard the International Space Station (ISS), a remarkable multinational collaborative project and floating U.S. National Laboratory that has supported continuous human presence in low Earth orbit for the past 20 years. Marking a new era of human space exploration, the first commercial rocket, SpaceX Falcon 9, recently launched NASA astronauts Doug Hurley and Bob Behnken in the Crew Dragon spacecraft Endeavor to the ISS and returned safely to Earth. NASA and its commercial partners are rapidly advancing innovative space technologies, and with the recently announced Artemis team of astronauts, plans to send the first woman and next man back to the moon and establish sustainable exploration by the end of the decade. Humankind will then be poised to take the next giant leap – pioneering human exploration of Mars.Historically, fewer than 600 individuals have participated in spaceflight, the vast majority of whom have been middle aged males (35-55 years) on short duration missions (less than 20 days). Thus, as the number and diversity of space travelers increase, a better understanding of how long-duration spaceflight affects human health is essential to maintaining individual astronaut performance during, and improving disease and aging trajectories following, future exploration missions. Here, I review findings from our NASA Twins Study and Telomeres investigations, highlighting potential mechanistic roles of chronic space radiation exposure in changes in telomere length and persistent DNA damage responses associated with long-duration spaceflight. Importantly, similar trends were observed in prostate cancer patients undergoing intensity-modulated radiation therapy (IMRT), additional support specifically for the role of radiation exposure. Individual differences in response were also observed in both cohorts, underscoring the importance of developing personalized approaches for evaluating human health effects and long-term outcomes associated with radiation exposures, whether on Earth or living in the extreme environment of space.

Abstract:The harsh environmental conditions in space, particularly weightlessness and radiation exposure, can negatively affect cardiovascular function and structure. In the future, preventive cardiology will be crucial in enabling safe space travel. Indeed, future space missions destined to the Moon and from there to Mars will create new challenges to cardiovascular health while limiting medical management. Moreover, commercial spaceflight evolves rapidly such that older persons with cardiovascular risk factors will be exposed to space conditions. This review provides an overview on studies conducted in space and terrestrial models, particularly head-down bedrest studies. These studies showed that weightlessness elicits a fluid shift towards the head, which likely predisposes to the spaceflight-associated neuro-ocular syndrome, neck vein thrombosis, and orthostatic intolerance after return to Earth. Moreover, cardiovascular unloading produces cardiopulmonary deconditioning, which may be associated with cardiac atrophy. In addition to limiting physical performance, the mechanism further worsens orthostatic tolerance after return to Earth. Finally, space conditions may directly affect vascular health; however, the clinical relevance of these findings in terms of morbidity and mortality is unknown. Targeted preventive measures, which are referred to as countermeasures in aerospace medicine, and technologies to identify vascular risks early on will be required to maintain cardiovascular performance and health during future space missions.

Abstract:[1] We review NASA's short-term and career radiation limits for astronauts and methods for their application to future exploration missions outside of low Earth orbit. Career limits are intended to restrict late occurring health effects and include a 3% risk of exposure-induced death from cancer and new limits for central nervous system and heart disease risks. Short-term dose limits are used to prevent in-flight radiation sickness or death through restriction of the doses to the blood forming organs and to prevent clinically significant cataracts or skin damage through lens and skin dose limits, respectively. Large uncertainties exist in estimating the health risks of space radiation, chiefly the understanding of the radiobiology of heavy ions and dose rate and dose protraction effects, and the limitations in human epidemiology data. To protect against these uncertainties NASA estimates the 95% confidence in the cancer risk projection intervals as part of astronaut flight readiness assessments and mission design. Accurate organ dose and particle spectra models are needed to ensure astronauts stay below radiation limits and to support the goal of narrowing the uncertainties in risk projections. Methodologies for evaluation of space environments, radiation quality, and organ doses to evaluate limits are discussed, and current projections for lunar and Mars missions are described.

Abstract:<span><br/>Over the past 60 years, our ability to live and work in space has evolved. From short sojourns in small spacecraft to landing on the moon and residing in an orbiting international space station, we have learned to adapt to an extreme environment and safely return home. Human missions to the Moon, Mars, and exploration of deep space are different. This paper summarizes the challenges of providing medical care, specifically mental health care during long-duration flights. Considerable information about challenges that crews bound for Mars will face is available. Literature regarding this issue is summarized. This manuscript provides a short historical summary of long-duration spaceflight to date; the challenges including limited communication with mission controllers on Earth; and, a summary of the behavioral impacts space flight has had on humans. A look at how the future autonomous systems might support physical and mental health when definitive care is millions of miles away, is also provided. Human spaceflight to Mars or other distant sites will require new approaches to mission preparedness and inflight medical support systems. Exploration class missions will be more autonomous than anything deployed until now. The concepts of telemedicine that have aptly supported crews from the 1960s to the present will no longer be in real-time. While communication between Earth and Mars is possible, it will be characterized by significant time delays. Mars-based crews will need to have systems onboard and on Mars to support all health and performance issues.<br/></span>

Abstract:SUMMARYThe human microbiota encompasses the diverse communities of microorganisms that reside in, on, and around various parts of the human body, such as the skin, nasal passages, and gastrointestinal tract. Although research is ongoing, it is well established that the microbiota exert a substantial influence on the body through the production and modification of metabolites and small molecules. Disruptions in the composition of the microbiota-dysbiosis-have also been linked to various negative health outcomes. As humans embark upon longer-duration space missions, it is important to understand how the conditions of space travel impact the microbiota and, consequently, astronaut health. This article will first characterize the main taxa of the human gut microbiota and their associated metabolites, before discussing potential dysbiosis and negative health consequences. It will also detail the microbial changes observed in astronauts during spaceflight, focusing on gut microbiota composition and pathogenic virulence and survival. Analysis will then turn to how astronaut health may be protected from adverse microbial changes via diet, exercise, and antibiotics before concluding with a discussion of the microbiota of spacecraft and microbial culturing methods in space. The implications of this review are critical, particularly with NASA's ongoing implementation of the Moon to Mars Architecture, which will include weeks or months of living in space and new habitats.

Abstract:Over the course of a mission to the International Space Station (ISS) crew members are exposed to a number of stressors that can potentially alter the composition of their microbiomes and may have a negative impact on astronauts' health. Here we investigated the impact of long-term space exploration on the microbiome of nine astronauts that spent six to twelve months in the ISS. We present evidence showing that the microbial communities of the gastrointestinal tract, skin, nose and tongue change during the space mission. The composition of the intestinal microbiota became more similar across astronauts in space, mostly due to a drop in the abundance of a few bacterial taxa, some of which were also correlated with changes in the cytokine profile of crewmembers. Alterations in the skin microbiome that might contribute to the high frequency of skin rashes/hypersensitivity episodes experienced by astronauts in space were also observed. The results from this study demonstrate that the composition of the astronauts' microbiome is altered during space travel. The impact of those changes on crew health warrants further investigation before humans embark on long-duration voyages into outer space.

Population analysis of space travelers

Abstract:Although many space missions have been completed in the last 60 years, space exploration is still technologically and medically challenging. While large-scale medical studies are impossible in space travelers, meta-analysis allows combining data from small crews that participated in space missions over several decades. Our primary objective was to examine space-travelers' sociodemographic characteristics and spaceflight activities, and their changes with time from the first spaceflight. Our secondary objective was to evaluate the publication practices to assess data availability for health-related meta-analytic studies. Based on state-funded space agencies used as primary sources, and third-party websites used as secondary sources, 565 humans (501 males/64 females) have currently completed spaceflight. The average age of space-travelers increased from 34±4 in the 1960s to 45±4 in the 2010s. While the duration of space missions has increased consistently, the number of missions per year varied in correlation with technological events. Using papers identified in the systematic review of bone health in astronauts, we examined the changes in reporting practices with time. The reported sample size varied from 1 to 58 people, in total providing data for 148 individuals. Data confidentiality significantly improved with time; however, the corresponding decrease in the availability of individual parameters did not allow stratification even by age, sex, and mission duration. Thus, space travelers represent a diverse population suitable for comparative studies, however, it is important to develop reporting practices that ensure consistent, transparent, and ethical presentation of outcomes to support meta-analyses that are critical for understanding the scope of spaceflight-induced health issues.
Understanding the Complexities and Changes of the Astronaut Microbiome for Successful Long-Duration Space Missions

Abstract:During space missions, astronauts are faced with a variety of challenges that are unique to spaceflight and that have been known to cause physiological changes in humans over a period of time. Several of these changes occur at the microbiome level, a complex ensemble of microbial communities residing in various anatomic sites of the human body, with a pivotal role in regulating the health and behavior of the host. The microbiome is essential for day-to-day physiological activities, and alterations in microbiome composition and function have been linked to various human diseases. For these reasons, understanding the impact of spaceflight and space conditions on the microbiome of astronauts is important to assess significant health risks that can emerge during long-term missions and to develop countermeasures. Here, we review various conditions that are caused by long-term space exploration and discuss the role of the microbiome in promoting or ameliorating these conditions, as well as space-related factors that impact microbiome composition. The topics explored pertain to microgravity, radiation, immunity, bone health, cognitive function, gender differences and pharmacomicrobiomics. Connections are made between the trifecta of spaceflight, the host and the microbiome, and the significance of these interactions for successful long-term space missions.
Medical Implications of Space Radiation Exposure Due to Low Altitude Polar Orbits

Abstract:Space radiation research has progressed rapidly in recent years, but there remain large uncertainties in predicting and extrapolating biological responses to humans. Exposure to cosmic radiation and Solar Particle Events may pose a critical health risk to future spaceflight crews and can have a serious impact to all biomedical aspects of space exploration. The relatively minimal shielding of the cancelled 1960's Manned Orbiting Laboratory program's space vehicle and the high inclination polar orbits would have left the crew susceptible to high exposures of cosmic radiation and high dose-rate SPEs that are mostly unpredictable in frequency and intensity. In this study, we have modeled the nominal and off-nominal radiation environment that a MOL-like spacecraft vehicle would be exposed to during a 30-day mission using high performance, multi-core computers. Projected doses from a historically large SPE (e.g. the August 1972 solar event) have been analyzed in the context of the MOL orbit profile, providing an opportunity to study its impact to crew health and subsequent <a class="link-external link-http" href="http://contingencies.It" rel="external noopener nofollow">this http URL</a> is reasonable to presume that future commercial, government, and military spaceflight missions in low-Earth orbit will have vehicles with similar shielding and orbital profiles. Studying the impact of cosmic radiation to the mission's operational integrity and the health of MOL crewmembers provides an excellent surrogate and case-study for future commercial and military spaceflight missions.

Women in space: A review of known physiological adaptations and health perspectives

Abstract:Exposure to the spaceflight environment causes adaptations in most human physiological systems, many of which are thought to affect women differently from men. Since only 11.5% of astronauts worldwide have been female, these issues are largely understudied. The physiological nuances affecting the female body in the spaceflight environment remain inadequately defined since the last thorough published review on the subject. A PubMed literature search yielded over 2200 publications. Using NASA's 2014 review series 'The effects of sex and gender on adaptation to space' as a benchmark, we identified substantive advancements and persistent knowledge gaps in need of further study from the nearly 600 related articles that have been published since the initial review. This review highlights the most critical issues to mitigate medical risk and promote the success of missions to the Moon and Mars. Salient sex-linked differences observed terrestrially should be studied during upcoming missions, including increased levels of inflammatory markers, coagulation factors and leptin levels following sleep deprivation; correlation between body mass and the severity of spaceflight-associated neuro-ocular syndrome; increased incidence of orthostatic intolerance; increased severity of muscle atrophy and bone loss; differences in the incidence of urinary tract infections; and susceptibility to specific cancers after exposure to ionizing radiation. To optimize health and well-being among all astronauts, it is imperative to prioritize research that considers the physiological nuances of the female body. A more robust understanding of female physiology in the spaceflight environment will support crew readiness for Artemis missions and beyond.

Abstract:[Formula: see text]There is a growing desire amongst space-faring nations to venture beyond the Van Allen radiation belts to a variety of intriguing locations in our inner solar system. Mars is the ultimate destination. In two decades, we hope to vicariously share in the adventure of an intrepid crew of international astronauts on the first voyage to the red planet.This will be a daunting mission with an operational profile unlike anything astronauts have flown before. A flight to Mars will be a 50-million-kilometre journey. Interplanetary distances are so great that voice and data communications between mission control on Earth and a base on Mars will feature latencies up to 20 min. Consequently, the ground support team will not have real-time control of the systems aboard the transit spacecraft nor the surface habitat. As cargo resupply from Earth will be impossible, the onboard inventory of equipment and supplies must be planned strategically in advance. Furthermore, the size, amount, and function of onboard equipment will be constrained by limited volume, mass, and power allowances.With less oversight from the ground, all vehicle systems will need to be reliable and robust. They must function autonomously. Astronauts will rely on their own abilities and onboard resources to deal with urgent situations that will inevitably arise.The deep space environment is hazardous. Zero- and reduced-gravity effects will trigger deconditioning of the cardiovascular, musculoskeletal, and other physiological systems. While living for 2.5 years in extreme isolation, Mars crews will experience psychological stressors such as loss of privacy, reduced comforts of living, and distant relationships with family members and friends.Beyond Earth's protective magnetosphere, the fluence of ionising radiation will be higher. Longer exposure of astronauts to galactic cosmic radiation could result in the formation of cataracts, impaired wound healing, and degenerative tissue diseases. Genetic mutations and the onset of cancer later in life are also possible. Acute radiation sickness and even death could ensue from a large and unpredictable solar particle event.There are many technological barriers that prevent us from carrying out a mission to Mars today. Before launching the first crew, we will need to develop processes for in-situ resource utilisation. Rather than bringing along large quantities of oxygen, water, and propellant from Earth, future astronauts will need to produce some of these consumables from local space-based resources.Ion propulsion systems will be needed to reduce travel times to interplanetary destinations, and we will need systems to land larger payloads (up to 40 tonnes of equipment and supplies for a human mission) on planetary surfaces. These and other innovations will be needed before humans venture into deep space.However, it is the delivery of health care that is regarded as one of the most important obstacles to be overcome. Physicians, biomedical engineers, human factors specialists, and radiation experts are re-thinking operational concepts of health care, crew performance, and life support. Traditional oversight of astronaut health by ground-based medical teams will no longer be possible, particularly in urgent situations. Aborting a deep space mission to medically evacuate an ill or injured crew member to Earth will not be an option. Future crews must have all of the capability and responsibility to monitor and manage their own health. Onboard medical resources must include imaging, surgery, and emergency care, as well as laboratory analysis of blood, urine, and other biospecimens.At least one member of the crew should be a broadly trained physician with experience in remote medicine. She/he will be supported by an onboard health informatics network that is artificial intelligence enabled to assist with monitoring, diagnosis, and treatment. In other words, health care in deep space will become more autonomous, intelligent, and point of care.The International Commission on Radiological Protection (ICRP) has dedicated a day of its 5th International Symposium in Adelaide to the theme of Mars exploration. ICRP has brought global experts together today to consider the pressing issues of radiation protection. There are many issues to be addressed: Can the radiation countermeasures currently used in low Earth orbit be adapted for deep space?Can materials of low atomic weight be integrated into the structure of deep space vehicles to shield the crew?In the event of a major solar particle event, could a safe haven shelter the crew adequately from high doses of radiation?Could Martian regolith be used as shielding material for subterranean habitats?Will shielding alone be sufficient to minimise exposure, or will biological and pharmacological countermeasures also be needed?Beyond this symposium, I will value the continued involvement of ICRP in space exploration. ICRP has recently established Task Group 115 to examine radiation effects on the health of astronaut crew and to recommend exposure limits. This work will be vital. Biological effects of radiation could not only impact the health, well-being, and performance of future explorers, but also the length and quality of their lives.While humanity has dreamed of travel to the red planet for decades, an actual mission is finally starting to feel like a possibility. How exciting! I thank ICRP for its ongoing work to protect radiation workers on Earth. In the future, we will depend on counsel from ICRP to protect extraterrestrial workers and to enable the exploration of deep space.

Abstract:Human spaceflight has historically been managed by government agencies, such as the NASA Twins Study 1 , but new commercial spaceflight opportunities have opened spaceflight to a broader population. In 2021, the SpaceX Inspiration4 mission launched the first-ever all civilian crew to low Earth orbit, which included the youngest American astronaut (age 29), novel in-flight experimental technologies (handheld ultrasound imaging, smartwatch wearables, and immune profiling), ocular alignment measurements, and new protocols for in-depth, multi-omic molecular and cellular profiling. Here we report the primary findings from the 3-day spaceflight mission, which induced a broad range of physiological and stress responses, neurovestibular changes indexed by ocular misalignment, and altered neurocognitive functioning, some of which match long-term spaceflight 2 , but almost all of which did not differ from baseline (pre-flight) after return to Earth. Overall, these preliminary civilian spaceflight data suggest that short-duration missions do not pose a significant health risk, and moreover present a rich opportunity to measure the earliest phases of adaptation to spaceflight in the human body at anatomical, cellular, physiologic, and cognitive levels. Finally, these methods and results lay the foundation for an open, rapidly expanding biomedical database for astronauts 3 , which can inform countermeasure development for both private and government-sponsored space missions.

Abstract:The International Space Station (ISS) allows research to be done in low earth orbit (LEO), offering a uniquely accelerating environment to model disease and study human health. Studies done in microgravity can simulate models of stress, inflammation, aging, immune dysfunction, and pre-malignant transformation of stem cells. The NASA Twins Study suggests that extended periods of spaceflight may affect hematopoietic stem cell and immune cell health and function through their multi-omics approach of one ground-based twin, Mark Kelly, and one on board the ISS, Scott Kelly (Garrett-Bakelman et al., Science, 2019). A recent study also suggests an adaptive response to immune cell activity related to pre- and post-flight long-term missions after transcriptomic analysis of astronaut blood samples (Stratis, et al., Frontiers in Immunology, 2023). These studies emphasize the importance of assessing astronaut hematopoietic stem cell and immune cell function to further understand the mechanisms of cellular and molecular changes associated with time spent in LEO. We designed a longitudinal study to collect (under an IRB-approved protocol) peripheral blood from Axiom Mission 2 (Ax-2) astronauts to assess their hematopoietic stem cell and immune cell health. Timepoints include Launch-45 days, Launch-2 days, inflight, Return+1 day, Return+50 days, and yearly for 5 years. Ax-2 was a 10-day mission which launched on May 21, 2023 and splashed down on May 31, 2023. This was a private crew-tended mission with a sample size of 4 astronauts. As part of the stem cell health and fitness assessment, we selected for CD34+ hematopoietic stem and progenitor cells (HSPCs) from the 4 private astronauts and performed functional survival and self-renewal assays from each timepoint. Preliminary results suggest that the self-renewal capacity of HSPCs decreases inflight, but upon Return+1 day, HSPCs regain their ability to clone themselves at a higher capacity than pre-flight. Furthermore, we collected HSPCs for whole genome sequencing, 10x Genomics single cell sequencing, and whole transcriptome sequencing, to analyze pre-, during, and post-flight genomic and transcriptomic changes. Analyses will be done to identify mutational burden and mutational signatures associated with each timepoint. These mutational signatures will be compared to previously identified C-to-T mutations to assess the pre-malignant status of HSPCs due to short-term spaceflight (Jiang et al., Cell Reports, 2021). In addition to assessing CD34+ HSPC health, we will also profile the immune landscape by sequencing in parallel the CD34- fraction and performing targeted FACS analysis based on initial sequencing results. We also seek to identify markers of inflammation and anti-viral response through analysis of cytokines from biobanked plasma. This investigation has the potential to provide valuable insight into stem cell and immune cell health and fitness associated with short-duration missions in LEO.

The effect of space travel on human reproductive health: a systematic review

Abstract:With increasing possibilities of multi-year missions in deep space, colonizing other planets, and space tourism, it is important to investigate the effects of space travel on human reproduction. This study aimed to systematically review and summarize the results of available literature on space travel, microgravity, and space radiation, or Earth-based spaceflight analogues impact on female and male reproductive functions in humans. This systematic review was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and Space Biomedicine Systematic Review methods. The search was performed using three databases: PubMed, Web of Science, and Medline Complete. During the database search, 364 studies were identified. After the study selection process, 16 studies were included in the review. Five studies included female participants, and the findings show an increased risk of thromboembolism in combined oral contraceptive users, decreased decidualization, functional insufficiency of corpus luteum, and decreased progesterone and LH levels related to space travel or its simulation. Male participants were included in 13 studies. In males, reproductive health considerations focused on the decrease in testosterone and sex hormone-binding globulin levels, the ratio of male offspring, sperm motility, sperm vitality, and the increase in sperm DNA fragmentation related to space travel or its simulation. Results of this systematic review highlight the need to focus more on the astronaut's reproductive health in future research, as only 16 studies were found during the literature search, and many more research questions related to reproductive health in astronauts still need to be answered.

Abstract:INTRODUCTION: Analysis of historical solar particle events (SPEs) provides context for some understanding of acute radiation exposure risk to astronauts who will travel outside of low-Earth orbit. Predicted levels of radiation exposures to exploration crewmembers could produce some health impacts, including nausea, emesis, and fatigue, though more severe clinical manifestations are unlikely. Using current models of anticipated physiological sequelae, we evaluated the clinical challenges of managing radiation-related clinical concerns during exploration spaceflight.METHODS: A literature review was conducted to identify terrestrial management standards for radiation-induced illnesses, focusing on prodromal symptom treatment. Terrestrial management was compared to current spaceflight medical capabilities to identify gaps and highlight challenges involved in expanding capabilities for future exploration spaceflight.RESULTS: Current spaceflight medical resources, such as those found on the International Space Station, may be sufficient to manage some aspects of radiation-induced illness, although effective treatment of all potential manifestations would require substantial expansion of capabilities. Terrestrial adjunctive therapies or more experimental treatments are unavailable in current spaceflight medical capabilities but may have a role in future management of acute radiation exposure.DISCUSSION: Expanded medical capabilities for managing radiation-induced illnesses could be included onboard future exploration vehicles. However, this would require substantial research, time, and funding to reach flight readiness, and vehicle limitations may restrict such capabilities for exploration missions.
