Abstract:Peripheral nerve damage can lead to loss of function or even complete disability, which brings about a huge burden on both the patient and society. Regulating nerve cell behavior and promoting nerve injury repair by bionic construction of a biological electric field to simulate natural nerve electrophysiological characteristics have attracted great attention, and many studies have shown that electroactive materials have a good repair effect on peripheral nerves. Here, we summarize the electroactive materials commonly used in peripheral nerve repair and their characteristics, especially for the design and application of a biodegradable non-invasive spontaneous electrical system. Besides, based on the fact that electroactive materials can provide electrical conductivity or a power generation performance, the specific role of electroactive materials in peripheral nerve repair was summed up from the following three aspects: cell behavior regulation, signaling pathway regulation and immune microenvironment regulation. This review provides a summary of the current research on the use of electroactive materials in peripheral nerve repair, which may help provide a more effective strategy for peripheral nerve injury (PNI) and allow more patients to benefit from artificial nerve catheter treatment.

Abstract:One of the most complex systems in the human body is the nervous system, which is divided into the central and peripheral nervous systems. The regeneration of the CNS is a complex and challenging biological phenomenon hindered by the low regenerative capacity of neurons and the prohibition factors in response to nerve injuries. To date, no effective approach can achieve complete recovery and fully restore the functions of the nervous system once it has been damaged. Developments in neuroscience have identified properties of the local environment with a critical role in nerve regeneration. Advances in biomaterials and biomedical engineering have explored new approaches of constructing permissive environments for nerve regeneration, thereby enabling optimism with regard to nerve-injury treatment. This article reviews recent progress in nanoengineered environments for aiding nerveinjury repair and regeneration, including nanofibrous scaffolds, functional molecules, and stem cells.

Abstract:Central and peripheral nerve injuries pose a great threat to people. Complications such as inflammation, muscle atrophy, traumatic neuromas and delayed reinnervation can bring huge challenges to clinical practices and barriers to complete nerve regrowth. Physical interventions such as electrical and magnetic stimulation show satisfactory results with varying parameters for acute and chronic nerve damages. The biological basis of electrical and magnetic stimulation mainly relies on protein synthesis, ion channel regulation and growth factor secretion. This review focuses on the various paradigms used in different models of electrical and magnetic stimulation and their regenerative potentials and underlying mechanisms in nerve injuries. The combination of physical stimulation and conductive biomaterial scaffolds displays an infinite potentiality in translational application in nerve regeneration.

Abstract:Peripheral nerve injury (PNI) is a relatively common disease caused by various circumstances, ultimately affecting the life quality of patients. Although existing medications and surgical interventions have particular benefits for nerve repair, more effective therapeutic strategies are urgently needed for various types of nerve injuries. Increasing investigations of nanomaterials have demonstrated their excellent biological properties, such as biocompatibility, permeability, degradability, high medicine loading efficacy, suitable mechanical properties, and broad applications in the biomedical field. Concerning peripheral nerve (PN) repair, nanomaterials with outstanding biological properties can be fitted as nerve conduits to provide support and guidance for PN regeneration and loaded with functional cells, cytokines, or specific medications to promote regenerative outcomes further. Almost all existing studies have focused on the application of different nanomaterials in PN repair, while the application of nanomaterials in different PN injuries has not been taken into account. This article outlines the application of nanomaterials in the medical field and the prevalent therapeutic strategies for PN repair. Importantly, it focuses on the application of nanomaterials in various PNI diseases, covering injuries of the sciatic nerve, cavernous nerve, facial nerve, median nerve, and more.

Abstract:Peripheral nerve injuries can cause serious health problems, even life-long disability. Although several engineering modalities have been developed for peripheral nerve regeneration, severe injuries that leave large gaps in peripheral nerves remain challenging to repair. Here, we report a biomaterial-based sono-electro-mechanical therapeutic system for long-gap peripheral nerve repair through harnessing both bio-electric and biomechanical modalities with neural regenerative potential. Applying implantable, biode-gradable piezoelectric nanotracts composed of polycaprolactone (PCL) and polyvinylidene fluoride (PVDF), we show significant regenerative performance by the synergy of ultrasound-activated electric stimulation and nanotopography-based contact guidance. Specifically, using cell culture models, we demonstrate sono-electro-mechanical promotion of pro-regeneration Schwann cell functions and neuronal growth in vitro. Mimicking long nerve gaps in humans, we use a rat model with a 15 mm sciatic nerve defect and show restoration of complex motor functions and axonal maturity by our sono-electro-mechanical therapeutic system at levels comparable with standard-of-care autograft treatment in vivo. Together, this study provides a previously unappreciated multimodal engineering strategy with great potential for clinical treatment of long-gap peripheral nerve injuries.(c) 2023 Elsevier Ltd. Abstract:Neuronal microenvironment imbalance is associated with successive and irreversible pathophysiological changes and insufficient functional restoration after peripheral nerve injury. Conventional neural-supporting scaffolds result in unsatisfactory curative effects due to lack of biomimetic nanotechnology designs and biochemical or physicochemical modifications. Consequently, they fail in rational and facile remodeling of the imbalanced growth microenvironment, and cannot recover neural structure and function. In recent years, with the increasing knowledge in neuronal injury-associated microenvironment, a number of novel strategies are applied in enhancing the biochemical and physicochemical natures of biomimetic nanomaterial-based scaffolds for nerve tissue engineering. These nanoscale scaffolds can trigger growth factor secretion and aggregation through surface modification, regulate ATP synthesis and hydrolysis, switch between oxidation and reduction states, and activate ion channels and stimulate electrical signals under certain biophysical cues. Consequently, they can determine neuronal cell fate by modulating their viability, development and cell cycles during the regeneration process. In this review, we systematically summarize the studies on the biomimetic scaffold design of functional nanomaterials, their basic topological, biochemical and physical properties, and nanotechnology-based restoration of a balanced nutritional microenvironment regarding four key neural regeneration factors, including immune response, intraneural vascularization, bioenergetic metabolism and bioelectrical conduction in order to provide ideas and inspiration for the nanomedicine-based neuronal regeneration therapy.

Abstract:Electroceuticals provide promising opportunities for peripheral nerve regeneration, in terms of modulating the extensive endogenous tissue repair mechanisms between neural cell body, axons and target muscles. However, great challenges remain to deliver effective and controllable electroceuticals via bioelectronic implantable device. In this review, the modern fabrication methods of bioelectronic conduit for bridging critical nerve gaps after nerve injury are summarized, with regard to conductive materials and core manufacturing process. In addition, to deliver versatile electrical stimulation, the integration of implantable bioelectronic device is discussed, including wireless energy harvesters, actuators and sensors. Moreover, a comprehensive insight of beneficial mechanisms is presented, including up-to-datein vitro, in vivoand clinical evidence. By integrating conductive biomaterials, 3D engineering manufacturing process and bioelectronic platform to deliver versatile electroceuticals, the modern biofabrication enables comprehensive biomimetic therapies for neural tissue engineering and regeneration in the new era.

Abstract:Peripheral nerve injury is a clinically common injury that causes sensory dysfunction and locomotor system degeneration, which seriously affects the quality of the patients' daily life. Long gapped defects in large nerve are difficult to repair via surgery and limited donor source of autologous nerve greatly challenges the successful nerve repair by transplantation. Significantly, remarkable progress has been made in repairing the peripheral nerve injury using artificial nerve grafts and a variety of products for peripheral nerve repair have emerged been approved globally in recent years. The raw materials of these commercial products includes natural/synthetic polymers, extracellular matrix. Despite a lot of effort, the desirable functional recovery still remains great challenges in long gapped nerve defects. Thus this review discusses the recent development of tissue engineering products for peripheral nerve repair and the design of bionic grafts improving the local microenvironment for accelerating nerve regeneration against locomotor disorder, which may provide potential strategies for the repair of long gaps or thick nerve defects by multifunctional biomaterials.

Abstract:Peripheral nerve tissues possess the ability to regenerate within artificial nerve scaffolds, however, despite the advance of biomaterials that support nerve regeneration, the functional nerve recovery remains unsatisfactory. Importantly, the incorporation of two-dimensional nanomaterials has shown to significantly improve the therapeutic effect of conventional nerve scaffolds. In this review, we examine whether two-dimensional nanomaterials facilitate angiogenesis and thereby promote peripheral nerve regeneration. First, we summarize the major events occurring after peripheral nerve injury. Second, we discuss that the application of two-dimensional nanomaterials for peripheral nerve regeneration strategies by facilitating the formation of new vessels. Then, we analyze the mechanism that the newly-formed capillaries directionally and metabolically support neuronal regeneration. Finally, we prospect that the two-dimensional nanomaterials should be a potential solution to long range peripheral nerve defect. To further enhance the therapeutic effects of two-dimensional nanomaterial, strategies which help remedy the energy deficiency after peripheral nerve injury could be a viable solution.

Microfluidic hollow fiber with improved stiffness repairs peripheral nerve injury through non-invasive electromagnetic induction and controlled release of NGF

Abstract:Peripheral nerve injury can lead to paralysis, chronic pain, muscle atrophy and disability due to interrupted bioelectric signal communications between spinal cord and the innervated bodies. Bioelectric materials, particularly conductive hydrogels, have shown the increasing potentials in neural engineering because they serve as tissue scaffolds that can support cell growth and differentiation, and restore functional electric conduction. In this work, we fabricated size-tunable microfluidic hollow fibers (HF) with improved stiffness and elasticity based on a triple network called SCPAPPy, which was composed of sodium alginate/Ca2+ (SC), polyacrylamide (PA), and polypyrrole (PPy). This HF showed a higher conductivity of 0.32 S/m than that of native sciatic nerve. Based on finite element analysis and digital oscilloscope measurement, the conductive HF was demonstrated to generate electromotive forces through electromagnetic induction by using a figure-eight coil. This creative approach could non-invasively facilitate myelination by improving Schwann cell’s growth and migration. Meanwhile, the electromotive forces drove the secretion of endogenous nerve growth factor (NGF) from Schwann cells. Moreover, exogenous nerve growth factor-7S (NGF-7S) was loaded in the HF, which was released gradually and promoted the differentiation of neurons in a controlled manner. By applying the pulsed magnetic field (MF) and NGF-7S together, it was found that the rats with 5-mm sciatic nerve defects achieved a more rapid nerve regeneration and functional recovery. In a word, our study demonstrated the practical role of electromagnetic induction on nerve repair, and opened the window of synergistic therapy combining the bioelectricity and neurotrophic factors towards peripheral nerve injury.

Abstract:Peripheral nerve injury represents one of the most common types of traumatic damage, severely impairing motor and sensory functions, and posttraumatic nerve regeneration remains a major challenge. Electrical cues are critical bioactive factors that promote nerve regrowth, and bioartificial scaffolds incorporating conductive materials to enhance the endogenous electrical field have been demonstrated to be effective. The utilization of fully biodegradable scaffolds can eliminate material residues, avoid the need for a second surgery and circumvent the need for secondary retrieval procedures. Here, we propose a fully bioresorbable and conductive nerve scaffold integrating N‐type silicon (Si) membranes, which can deliver both structural guidance and electrical cues for the repair of nerve defects. The entire scaffold is fully biodegradable, and the introduction of N‐type Si can significantly promote the proliferation and production of neurotrophic factors in Schwann cells and enhance the calcium activity of dorsal root ganglion (DRG) neurons. The conductive scaffolds enable accelerated nerve regeneration and motor functional recovery in rodents with sciatic nerve transection injuries. This work sheds light on the advancement of bioresorbable and electrically active materials to achieve desirable neural interfaces and improved therapeutic outcomes, offering essential strategies for regenerative medicine. Abstract:Electroactive biomaterials have been shown useful for the repair of injured peripheral nerve. While for conventional conductive conduits, outer electrical stimulation device is unavoidably employed to exert electrical signals, and their rigid microstructures are usually incompatible with neural cells. Herein, a soft carbon methacryloyl/poly(L-lactic acid) (/PLLA) nerve tissue-engineering scaffold was fabricated, which provided an endogenous piezoelectric stimulation and conductive microenvironment. Based on amounts of in-vitro experiment data, such composite scaffold significantly improved adhesion and elongation of Schwann cells, and meanwhile promoted axonal outgrowth and neurites number of dorsal root ganglions. More interestingly, the scaffold was applied to a 10-mm sciatic nerve defect in rats and harvested at 12 weeks post-implantation. Immunohistochemical staining results indicated that our proposed graft significantly facilitated peripheral nerve regeneration by promoting myelination and axon outgrowth, meanwhile an enhanced motor functional recovery caused by the scaffold was also revealed due to the obviously-improved sciatic functional index and muscle weights. Overall, the soft, self-powered, and electroconductive /PLLA scaffold is a promising candidate for the treatment of peripheral nerve injuries.

Abstract:Nerve growth factor (NGF) is widely used for repairing peripheral nerve injury because of its capability in dominating the survival, migration, proliferation, and differentiation of nerve cells. Monosialoganglioside (GM1), as another kind of nerve growth factor, works for regulating NGF function. In this study, GM1 and NGF were incorporated into the Poly(l-lactic acid-co- epsilon-caprolactone)/silk fibroin (PLCL/SF) nanofibers by the coaxial electrospinning. The fibers morphology and core-shell structure were characterized by SEM and TEM. The scaffolds demonstrated high tensile stress with good flexibility. In vitro cell viability studies indicated that the scaffolds incorporating both GM1 and NGF played a synergistic effect to enhance Schwann cells (SCs) proliferation and Pheochromocytoma (PC12) cells differentiation, in comparison to the scaffolds only incorporating NGF. Subsequently, the nanofibrous conduit scaffolds (NCSs) were evaluated in vivo in a rabbit sciatic nerve defect model. The NGF/GM1 incorporated NCSs group performed better nerve function recovery than single incorporated group, in consideration of the compound muscle action potential (CMAP) and nerve conduction velocity (NCV) results. Furthermore, hematoxylin and eosin (H&E) staining, toluidine blue (TB) staining, and transmission electron microscope (TEM) analysis displayed better nerve regeneration of NGF/GM1 incorporated NCSs both quantitatively and qualitatively. Therefore, the results indicated the dual neurotrophins-incorporated NCSs had potentials for the application in peripheral nerve repairing.

Abstract:Peripheral nerve injury is a common clinical problem bringing heavy burden to patients, due to its high incidence and unsatisfactory treatment. Nerve guidance conduit (NGC) is a promising scaffold for peripheral nerve repair, and bioactive agents are applied for great functional recovery. Melatonin (MLT) and Fe3O4 magnetic nanoparticles (Fe3O4‐MNPs) are proven to inhibit oxidative stress, inflammation, and induce nerve regeneration. Herein, a multilayered composite NGC loaded with MLT and Fe3O4‐MNPs is designed for sequential and sustainable drug release, creating an appropriate microenvironment for nerve regeneration. The composite scaffold shows sufficient mechanical strength and biocompatibility in vitro, and evidently promotes morphological, functional, and electrophysiological recovery of regenerated sciatic nerves in vivo. This work proves that the multilayered conduits show great prospect in the long‐term nerve defects treatment due to easy manufacture and desired efficacy.

Abstract:Peripheral nerve injury (PNI) occurs frequently, and the prognosis is unsatisfactory. As the gold standard of treatment, autologous nerve grafting has several disadvantages, such as lack of donors and complications. The use of functional biomaterials to simulate the natural microenvironment of the nervous system and the combination of different biomaterials are considered to be encouraging alternative methods for effective tissue regeneration and functional restoration of injured nerves. Considering the inherent presence of an electric field in the nervous system, electrically conductive biomaterials have been used to promote nerve regeneration. Due to their singular physical properties, hydrogels can provide a three-dimensional hydrated network that can be integrated into diverse sizes and shapes and stimulate the natural functions of nerve tissue. Therefore, conductive hydrogels have become the most effective biological material to simulate human nervous tissue’s biological and electrical characteristics. The principal merits of conductive hydrogels include their physical properties and their electrical peculiarities sufficient to effectively transmit electrical signals to cells. This review summarizes the recent applications of conductive hydrogels to enhance peripheral nerve regeneration.

Tissue engineered nerve guidance conduits (NGCs) provide a promising platform for peripheral nerve repair through the integration of bioactive scaffolds, biological effectors, and cellular components. Herein, we firstly describe the pathogenesis of peripheral nerve injuries at different orders of severity to clarify their microenvironments and discuss the clinical treatment methods and challenges. Then, we discuss the recent progress on the design and construction of NGCs in combination with biological effectors and cellular components for nerve repair. Afterward, we give perspectives on imaging the nerve and/or the conduit to allow for the in situ monitoring of the nerve regeneration process. We also cover the applications of different postoperative intervention treatments, such as electric field, magnetic field, light, and ultrasound, to the well-designed conduit and/or the nerve for improving the repair efficacy. Finally, we explore the prospects of multifunctional platforms to promote the repair of peripheral nerve injury.

Abstract:Nanobiotechnology is an emerging field that has recently been explored for peripheral neural regeneration (PNR). Being a public-health problem, peripheral nerve injuries (PNIs) should be treated by the therapiesthat ensure swift functional recovery. The autologous nerve grafts (standard treatment for PNIs) are rarely available and also cause morbidity and neuroma formation at the harvest site, hence an alternative approach with minimum complications is required for the treatment of serious PNIs. Although nerve guidance conduits (NGCs) provide microenvironment for axonal regeneration but they are as yet imperfect solutions. Nanoparticles (e.g., metallic and metallic oxide nanoparticles) have properties which are interesting to include in biomaterials developed for peripheral nervous system regeneration including potential theranostic function. It is important to get an insight into the fundamental mechanisms of reconstruction of peripheral nerves for clinical translation of pre-clinical outcomes of the use of nanoparticles in PNR. Moreover, the combination of nanotechnological strategies is expected to provide transition from bed to bench-side and beyond to the patients, clinicians, and researchers.
