Hb A2-Pontedera [δ93(F9) Cys > Trp; HBD: C.282T > G]: A New δ-Globin Chain Variant Detected by Screening Tests for Hemoglobinopathies

Abstract:Genetic variations in the δ-globin gene ( HBD MIM#142000) described to date number more than 140 and include deletions and point defects, variants, and thalassemias that, in each case and in various ways, qualitatively or quantitatively affect Hb A 2 synthesis. Hemoglobin (Hb) A 2 (α 2 δ 2 ) is the minor component of Hb available for oxygen transport in the adult subject with normal condition. The interest in accurate methods and procedures for measuring Hb A 2 is due to the fact that this parameter has important diagnostic relevance to the β-thalassemic trait [ Citation 1 , Citation 2 ]. Approximately 30% of the known Hb A 2 variants have been observed and described for the first time in Italian subjects or in some cases, residents of Italy [ Citation 3 , Citation 4 ]. A significant presence of these defects is particularly evident in geographic areas affected by thalassemia and where prevention must therefore consider the important parameter of Hb A 2 . Considering the presence of Hb A 2 variants themselves is never directly associated with any clinical problems, the identification of β-thalassemia carriers may seem to be the most important and virtually the only motivation that can be attributed to the quantification of Hb A 2 . However, we also know that the relative amount of Hb A 2 can help presume or rule out other conditions. It is known that some rare unstable β-chain variants contribute to an increase in Hb A 2 value [ Citation 1 ] while a decrease can be produced by iron deficiency, the presence of α-thalassemia defects, as well as electrophoretically or chromatographically silent variants of the delta gene. Therefore, it is clear that the diagnosis of β-thal trait cannot be based only on the measurement of Hb A 2 , and, as indicated by the guidelines, reference should always be made to erythrocyte parameters and iron levels [ Citation 5 ].
