Abstract:Across three studies, we provide a proof-of-concept evaluation of an integrative psychotherapeutic application of virtual reality (VR) technology. Study 1 (n = 36) evaluated an unguided "safe-place" imagery task, where participants were instructed "to create a safe space… [such as] a scene, item, design, or any visual representation that makes you feel safe" using either the Google Tilt Brush application (VR condition), the standard Microsoft Paint application (2-D condition), or via eyes-closed mental imagery alone (IMG condition). Study 2 (n = 48) evaluated a narrative episodic recall task, where participants viewed their childhood and adult homes and places of schooling either using either the Google Earth VR application (VR condition) or the standard Google Earth application (2-D condition) or recalled these places with their eyes closed via mental imagery alone (IMG condition). Finally, Study 3 (n = 48) evaluated a guided wilderness imagery task, during which different scripts were narrated, specifically, a trail walk in autumn, a spring meadow, and a hillside walk in snowy winter, while either these same scenes were visually presented using the Nature Treks VR application (VR condition), the scenes were presented using the same software but shown on standard computer monitor (2-D condition), or participants' eyes were closed (IMG condition). Order of intervention format was randomized across participants. Across all three studies, quantitative survey ratings showed that the VR format of intervention delivery produced greater positive affect and satisfaction and perceived credibility ratings as an intervention for trauma- and stressor-related disorders and psychological well-being as rated by university students who varied in traumatic and stressful life event history and symptoms of posttraumatic stress disorder, whereas qualitative findings revealed additional themes of experiential response including increased experience of presence and vividness in the VR condition. Future research directions and clinical applications are discussed.

Reality check. The issue of social plausibility in virtual reality therapy with addiction patients

Abstract:In France, virtual reality has been experimented since 2016 to address addictive behaviors. The existing literature insists on the "immersive" dimension of the technology as a significant factor in the efficacy of exposure. An ethnographic approach, however, suggests that the realism criterion should be reassessed. Based on a fieldwork conducted in the addictology department of a French hospital that made use of Virtual reality therapy (VRT), the article shows that it is a form of technical reiteration designed to develop reflexivity about one's actions, provoking the craving so that it can be domesticated. The patient's engagement in "as if" mode is facilitated by the therapist's work in mapping the patient's addictive practices, personalizing the exposure situations, and elaborating the simulation as being part of a practical experiment. While the question of graphic realism does not seem especially problematic, the question of social plausibility can put the simulation operation under strain.

Examining the potential of virtual reality to deliver remote rehabilitation

Abstract:Remote workers are particularly prone to mental health problems (Bowers et al., 2018). Unfortunately, it is often difficult for them to access the quality psychological help that they need. As a result, psychological treatment is increasingly being delivered to remote workers via telehealth (videoconferencing and telephone calls). However, the perceived remoteness of the therapist during such treatments can greatly hinder progress. This project examined the potential of virtual reality (VR) to deliver psychotherapy to workers located in remote locations (since it can make people separated by great distances feel that they are "present" in the same virtual space). The study compared the experiences of 30 'clients' who participated in both VR and Skype-based mock counselling sessions (delivered by trained psychotherapists). Overall, VR was found to outperform Skype: 1) as a therapeutic tool, 2) in terms of the perceived realism of the session; and 3), in terms of the degree of presence it generated in the clients and the therapists. Clients did not report feeling sick or stressed when using VR and found it as easy to use as Skype. These study findings (based on formal questionnaire data) were also confirmed by interviews with both the therapists and clients.

Abstract:Our access to computer-generated worlds changes the way we feel, how we think, and how we solve problems. In this review, we explore the utility of different types of virtual reality, immersive or non-immersive, for providing controllable, safe environments that enable individual training, neurorehabilitation, or even replacement of lost functions. The neurobiological effects of virtual reality on neuronal plasticity have been shown to result in increased cortical gray matter volumes, higher concentration of electroencephalographic beta-waves, and enhanced cognitive performance. Clinical application of virtual reality is aided by innovative brain–computer interfaces, which allow direct tapping into the electric activity generated by different brain cortical areas for precise voluntary control of connected robotic devices. Virtual reality is also valuable to healthy individuals as a narrative medium for redesigning their individual stories in an integrative process of self-improvement and personal development. Future upgrades of virtual reality-based technologies promise to help humans transcend the limitations of their biological bodies and augment their capacity to mold physical reality to better meet the needs of a globalized world.

Abstract:We will discuss the use of virtual reality (VR) across diverse settings and modalities. First, using VR as a self-guided and patient centric intervention with Veterans to support the management of chronic pain. The pilot study implemented VR in a community living center (CLC), offering it to Veterans with moderate to severe pain. Data collected included vital signs, pain scores, and qualitative feedback. The intervention aligns with complementary and integrative health practices and holds promise as a nonpharmacological pain management option for Veterans in various settings. Second, we will describe the utilization of VR during a workshop for inpatient and outpatient staff, as an educational tool to help learners increase awareness on inclusivity among transgender older adults. Learners embody "Eden" a transgender older woman, to increase confidence and knowledge of caring and relating to LGTBQ older adults. Third, we will explain how VR experiential training was offered as a one to one modality to geriatric inpatient mental health providers. They were provided with the full experience of embodying a person living with dementia from the patient's perspective with the goal of increasing empathy, confidence and satisfaction and improving on skills to non-pharmacologically manage behavioral and psychological symptoms of dementia.

Abstract:This study discusses the concept of virtual selves created in the virtual spaces [e.g. social network services or virtual reality (VR)]. It analyzes the activities in the different virtual spaces and claims that experience gained there can be transferred to real life. In respect to that, the effects of the VR treatment on the self as well as the concept of creating a life story are analyzed as interconnected. The research question which arises from these considerations is how to look at psychological trauma in order to explain the effectiveness of the usage of VR for treatment of traumatic disorders. The proposal in the study is to see trauma as a shift in the normal storyline of the narrative people create. With this concept in mind, it might be possible to support the claim that reliving traumatic events, regaining control over one’s life narrative, and creating new stories in the VR aids the treatment process in the search for meaning and resolution in life events. Considering the findings of researchers who argue in the field of self-narrative and traumatic treatment, as well as researchers on virtual selves, virtual spaces and VR, this study discusses the virtual as a possible medium to experience narratives and utilize those narratives as better explanatory stories to facilitate the therapeutic process of recovery and self-recreation. This study supports the idea that VR can be used to visualize patients’ narratives and help them perceive themselves as active authors of their life’s story by retelling traumatic episodes with additional explanation. This experience in the VR is utilized to form healthier narratives and coping techniques for robust therapeutic results that are transferred to real life.

Abstract:Virtual Reality (VR) technology offers new opportunities for the development of innovative clinical research, assessment, and intervention tools. VR-based testing, training, teaching, and treatment approaches that would be difficult, if not impossible to deliver using traditional methods are now being developed that take advantage of the assets that are available with VR technology. As research evidence continues to indicate clinical efficacy, VR applications are being increasingly regarded as providing innovative options for targeting the cognitive, psychological, motor, and functional impairments that result from various clinical health conditions. VR allows for the precise presentation and control of stimuli within dynamic multisensory 3-D computer generated simulations as well as providing advanced methods for capturing and quantifying behavioral responses. These characteristics support the rationale for the use of VR applications in clinical assessment, intervention, and training. This article begins with a brief review of the history and rationale for the use of VR with clinical populations. We then detail one use-case for the clinical application of VR—the exposure therapy treatment of anxiety disorders and post-traumatic stress disorder. While significant work is cited in other areas of Clinical VR (e.g., pain management, cognitive/physical assessment/rehabilitation, eating disorders, social skills/clinical training, etc.), a full overview of such a broad literature is beyond the scope of this article. Thus, we have opted to provide more in-depth analysis of one specific clinical area that clearly illustrates how VR has been successfully applied and is supported by an encouraging and evolving scientific literature.

Abstract:In recent years, the field of virtual reality (VR) has shown tremendous advancements and is utilized in entertainment, scientific research, social networks, artistic creation, as well as numerous approaches to employ VR for psychotherapy. While the use of VR in psychotherapy has been widely discussed, little attention has been given to the potential of this new medium for art therapy. Artistic expression in VR is a novel medium which offers unique possibilities, extending beyond classical expressive art mediums. Creation in VR includes options such as three-dimensional painting, an immersive creative experience, dynamic scaling, and embodied expression. In this perspective paper, we present the potentials and challenges of VR for art therapy and outline basic principles for its implementation. We focus on the novel qualities offered by this creative medium (the virtual environment, virtual materials, and unreal characteristics) and on the core aspects of VR (such as presence, immersivity, point of view, and perspective) for the practice of art therapy.

Abstract:Innovative technologies, such as virtual reality (VR), augmented reality (AR) and mixed reality (MR) are no longer only interesting for a niche of gamers but are increasingly being applied within healthcare. Possible applications for VR in care vary from exposure therapy for PTSD and anxiety disorders to aiding in cognitive and physical rehabilitation. VR has also acquired an important role in medical education. AR/MR are used predominantly within surgery, for instance for perioperative projection of patient information, holographic images and scans. Despite the absence of large randomized trials and cost-effectiveness studies, it seems just a matter of time before doctors embrace these innovative technologies. The digitalization of healthcare has, after all, already started.

Abstract:Virtual reality (VR) therapies have been used in health and mental health care settings. The applicability of VR expressive tools has not yet been examined adequately in the creative arts therapies. This pilot qualitative study reports on 17 participants' experiences with VR art-making tools to determine its relevance to art therapy practice and research. Findings indicate that VR-based self-expression is an embodied visual expression, generates novel artistic and imaginal responses, and requires a developmental trajectory in expression and mastery. The unique characteristics of art therapy in VR were identified as positive emotions, play and exploration, learning and mastery, access, and storage. VR has the potential to enhance psychological health and well-being through creativity, enhanced imagination, interactivity, and problem solving.

Abstract:This scoping review focuses on therapeutic interventions, which involve the creation of artworks in virtual reality. The purpose of this research is to survey possible directions that traditional practices of art therapy and therapeutic artmaking could take in the age of new media, with emphasis on fully immersive virtual reality. After the collection of papers from online databases, data from the included papers were extracted and analyzed using thematic analysis. The results reveal that virtual reality introduces novel opportunities for artistic expression, self-improvement, and motivation for psychotherapy and neurorehabilitation. Evidence that artmaking in virtual reality could be highly beneficial in therapeutic settings can be found in many aspects of virtual reality, such as its virtuality, ludicity, telepresence capacity, controlled environments, utility of user data, and popularity with digital natives. However, deficiencies in digital literacy, technical limitations of the current virtual reality devices, the lack of tactility in virtual environments, difficulties in the maintenance of the technology, interdisciplinary concerns, as well as aspects of inclusivity should be taken into consideration by therapy practitioners, researchers, and software developers alike. Finally, the reported results reveal implications for future practice.

Abstract:During life, many personal changes occur. These include changing house, school, work, and even friends and partners. However, the daily experience shows clearly that, in some situations, subjects are unable to change even if they want to. The recent advances in psychology and neuroscience are now providing a better view of personal change, the change affecting our assumptive world: (a) the focus of personal change is reducing the distance between self and reality (conflict); (b) this reduction is achieved through (1) an intense focus on the particular experience creating the conflict or (2) an internal or external reorganization of this experience; (c) personal change requires a progression through a series of different stages that however happen in discontinuous and non-linear ways; and (d) clinical psychology is often used to facilitate personal change when subjects are unable to move forward. Starting from these premises, the aim of this paper is to review the potential of virtuality for enhancing the processes of personal and clinical change. First, the paper focuses on the two leading virtual technologies - augmented reality (AR) and virtual reality (VR) - exploring their current uses in behavioral health and the outcomes of the 28 available systematic reviews and meta-analyses. Then the paper discusses the added value provided by VR and AR in transforming our external experience by focusing on the high level of personal efficacy and self-reflectiveness generated by their sense of presence and emotional engagement. Finally, it outlines the potential future use of virtuality for transforming our inner experience by structuring, altering, and/or replacing our bodily self-consciousness. The final outcome may be a new generation of transformative experiences that provide knowledge that is epistemically inaccessible to the individual until he or she has that experience, while at the same time transforming the individual's worldview.

Abstract:Virtual reality (VR) can enhance mental health care. In particular, the effectiveness of VR-based exposure therapy (VRET) has been well-demonstrated for treatment of anxiety disorders. However, most applications of VRET remain localized to clinic spaces. We aimed to explore mental health therapists' perceptions of telehealth-based VRET (tele-VRET) by conducting semi-structured, qualitative interviews with 18 telemental health therapists between October and December 2022. Interview topics included telehealth experiences, exposure therapy over telehealth, previous experiences with VR, and perspectives on tele-VRET. Therapists described how telehealth reduced barriers (88.9%, 16/18), enhanced therapy (61.1%, 11/18), and improved access to clients (38.9%, 7/18), but entailed problems with technology (61.1%, 11/18), uncontrolled settings (55.6%, 10/18), and communication difficulties (50%, 9/18). Therapists adapted exposure therapy to telehealth by using online resources (66.7%, 12/18), preparing client expectations (55.6%, 10/18), and adjusting workflows (27.8%, 5/18). Most therapists had used VR before (72.2%, 13/18) and had positive impressions of VR (55.6%, 10/18), but none had used VR clinically. In response to tele-VRET, therapists requested interactive session activities (77.8%, 14/18) and customizable interventions components (55.6%, 10/18). Concerns about tele-VRET included risks with certain clients (77.8%, 14/18), costs (50%, 9/18), side effects and privacy (22.2%, 4/18), and inappropriateness for specific forms of exposure therapy (16.7%, 3/18). These results reveal how combining telehealth and VRET may expand therapeutic options for mental healthcare providers and can help inform collaborative development of immersive health technologies.

Abstract:Abstract Background Automated virtual reality therapies are being developed to increase access to psychological interventions. We assessed the experience with one such therapy of patients diagnosed with psychosis, including satisfaction, side effects, and positive experiences of access to the technology. We tested whether side effects affected therapy. Methods In a clinical trial 122 patients diagnosed with psychosis completed baseline measures of psychiatric symptoms, received gameChange VR therapy, and then completed a satisfaction questionnaire, the Oxford-VR Side Effects Checklist, and outcome measures. Results 79 (65.8%) patients were very satisfied with VR therapy, 37 (30.8%) were mostly satisfied, 3 (2.5%) were indifferent/mildly dissatisfied, and 1 (0.8%) person was quite dissatisfied. The most common side effects were: difficulties concentrating because of thinking about what might be happening in the room (n = 17, 14.2%); lasting headache (n = 10, 8.3%); and the headset causing feelings of panic (n = 9, 7.4%). Side effects formed three factors: difficulties concentrating when wearing a headset, feelings of panic using VR, and worries following VR. The occurrence of side effects was not associated with number of VR sessions, therapy outcomes, or psychiatric symptoms. Difficulties concentrating in VR were associated with slightly lower satisfaction. VR therapy provision and engagement made patients feel: proud (n = 99, 81.8%); valued (n = 97, 80.2%); and optimistic (n = 96, 79.3%). Conclusions Patients with psychosis were generally very positive towards the VR therapy, valued having the opportunity to try the technology, and experienced few adverse effects. Side effects did not significantly impact VR therapy. Patient experience of VR is likely to facilitate widespread adoption.

Virtual reality in rehabilitation and therapy

Abstract:This paper describes virtual reality and some of its potential applications in rehabilitation and therapy. Some aspects of this technology are discussed with respect to different problem areas (sensorimotor impairments, autism, learning difficulties), as well as previous research which investigated changes within some motor and motivation parameters in relation to rehabilitation of children with motor impairments. Emphasis is on the positive effects of virtual reality as a method in which rehabilitation and therapy can be offered and evaluated within a functional, purposeful and motivating context.

Abstract:Background: Cognitive behavioral group therapy alleviates depression by teaching patients to think and behave in more positive ways. Teletherapy (e.g., Zoom) is becoming more widely used, especially during the COVID-19 pandemic (where meeting in person is not safe). The current study explores the acceptability of taking teletherapy to the next level: Virtual Reality Group Therapy (VRGT). Methods: Semistructured interviews were conducted to explore stakeholder views on VRGT. Ten depressed patients and ten therapists watched a demonstration video of the proposed VRGT intervention and tested the VR application using a stand-alone VR headset. In VRGT, patients will use an avatar to interact with each other and with their therapist via networked multiparticipant VR. Results: Therapists and patients generally responded favorably to the idea of doing group therapy sessions in VR. Patients especially liked the idea of remaining anonymous via an avatar. Patients and therapists both indicated that the anonymity provided by avatars could increase patient’s willingness to make disclosures (to talk more freely and honestly), which could increase participation and could lead to better group cohesion. Conclusion: Although the findings suggested that VRGT may be more acceptable for some patients than for others, overall, the response of the patients and therapists was largely positive. Recommendations from this study could be used during the COVID-19 pandemic to deliver VRGTs. Finally, design ideas for creating a group VR world custom-designed for group therapy are discussed.
[The application of virtual reality in the treatment of mental disorders]

Abstract:Background: Virtual reality (VR) enables immersion in an interactive digital world with realistic experiences, that can be applied for controlled and personalized interventions. This review summarizes the current research on VR in the treatment of mental disorders. Methods: Selective literature search in PubMed and Google Scholar. Results: An increasing number of publications report the therapeutic application of VR for the treatment of mental disorders. Most VR applications are based on established therapy approaches, such as exposure therapy. According to meta-analytic data, virtual exposure therapy (VRET) for specific phobia and agoraphobia with panic disorder is as effective as traditional in vivo exposure therapy. VRET for the treatment of social phobia is significantly more effective than waitlist and placebo control groups with, however, currently inconsistent metanalytic results when compared to in vivo exposure therapy. VRET for the treatment of posttraumatic stress disorder (PTSD) is similar in effectiveness compared to active psychotherapy. For psychosis, positive results have been reported for the VR-based treatment of auditory verbal hallucinations. For patients with a substance use disorder, VR can induce craving, with still unverified diagnostic and therapeutic relevance. Conclusions: VRET can broaden the psychotherapy options for anxiety disorders. Encouraging results of VR-based treatments for psychosis and PTSD indicate the need for further research concerning its effectiveness and safety. In the field of substance use disorders, evaluation of clinical-orientated VR applications is needed.

Abstract:The narrative essence of human nature is that humans are storytellers, and this ability helps to constitute our identities. Challenges that disrupt this innate ability are adverse events that affect the human perception of the world and undermine the meaning one finds in reality. Such events might range from short-term stressors to long-term testing conditions such as pandemics. Tools for overcoming these negative effects and for achieving self-preservation might be sought in individual storytelling abilities in relation to self-constitution and identification. An example of an interactive digital narrative medium that provides a platform for such expression of complex issues is virtual reality (VR), which has been used as a visual narrative storytelling tool for decades. This study sets the notion of trauma in a new light, as a break in the individual story used to explain one’s life and as a story to be intertwined with and re-adapted to one’s overall lifetime. In the healthcare context, this experience can be most effectively presented in VR because it offers added meaning, potential choices, closure, and resolution as methods of utilization. Therefore, VR can be seen not only as a medium whereby traumatic events can be processed but also as an alternative viewpoint of the goal of self-(re)creation. This research discusses a theoretical proposal for ways of playing with self-construction mechanisms in the context of an immersive VR environment to create more opportunities to change one’s narrative and, hence, one’s real-life story.
Explorations in Designing Virtual Environments for Remote Counselling

Abstract:The advent of technology-enhanced interventions has significantly transformed mental health services, offering new opportunities for delivering psychotherapy, particularly in remote settings. This paper reports on a pilot study exploring the use of Virtual Reality (VR) as a medium for remote counselling. The study involved four experienced psychotherapists who evaluated three different virtual environments designed to support remote counselling. Through thematic analysis of interviews and feedback, we identified key factors that could be critical for designing effective virtual environments for counselling. These include the creation of clear boundaries, customization to meet specific therapeutic needs, and the importance of aligning the environment with various therapeutic approaches. Our findings suggest that VR can enhance the sense of presence and engagement in remote therapy, potentially improving the therapeutic relationship. In the paper we also outline areas for future research based on these pilot study results.
