Abstract:Abstract Study Objective To evaluate the practicability of transvaginal 4-dimensional (4D) hysterosalpingo-contrast sonography (HyCoSy) using SonoVue for screening of fallopian tube patency in infertile females. Design Prospective observational study (Canadian Task Force classification II-2). Setting The Second Affiliated Hospital of Zhejiang University. Patients 204 infertile females who underwent 4D-HyCoSy between July 2015 and June 2016. Intervention 4D-HyCoSy was performed to acquire dynamic images. If any of the fallopian tubes was not developing during the process, the procedure was repeated promptly, to exclude false-positive outcomes. Subsequently, tube status was confirmed by traditional 2-dimensional (2D)-HyCoSy. Finally, the results of the 4D-HyCoSy and the final 2D-HyCoSy were compared. Measurements and Main Results A total of 204 patients (408 tubes) were evaluated ith 4D-HyCoSy followed by 2D-HyCoSy. Of these 408 tubes, 385 demonstrated the same status on 4D-HyCoSy and 2D-HyCoSy, for an agreement rate of 94.4%. Conclusion A 4D technique combined with HyCoSy is a practicable screening method for assessing fallopian tube patency. It overcomes the disadvantages of 2D-HyCoSy and has a relatively high degree of agreement with the more difficult 2D-HyCoSy technique.

Abstract:Objective To assess the value of hysterosalpingo-contrast-sonography (HyCoSy) with the contrast agent SonoVue in the diagnosis of women infertility. Methods Thirty-six cases of infertility underwent HyCoSy using SonoVue and compared with laparoscopie hydrotubation. Resnlts Among 69 fallopian tubes,22 of them were diagnosed to be obstructive by HyCoSy. Compared with laparoscopic hydrotubation, the sensitivity and specificity of HyCoSy were 90. 5 % and 93.8 %, respectively. Conclusions HyCoSy is a convenient, non-invasive, non-radiative method,which may be regarded as an effective tool to assess the fallopian tubes in patients with infertility.

Abstract:Objective To investigate the clinical value of hysterosalpingo-contrast-sonography (HyCoSy) with high-pressure syringe injection and time-intensity curve analysis in the diagnosis of women infertility due to tube obstruction.Methods Fifty-six women with infertility underwent HyCoSy with compressive injection of SonoVue with a balloon catheter and high-pressure syringe.Time-intensity curve based on Autotracking Contrast Quatification (ACQ) software was analyzed in all patients.The arrival time (AT) of microbubbles from uterine cavity to tube fimbria was analyzed between partially obstructed tube group and control group.Results The AT in the group of partially obstructed tubes was (3.49 ± 1.70) s,while the AT in the group of freely patent tubes was (1.14-± 0.37)s,there was statistical difference of AT between the group of partially obstructed tubes and the group of freely patent tubes (P =0.000).According to ROC results,the optimization point of AT was 1.65 s,the sensitivity,specificity,positive predictive value and negative predictive value were 92.6％,93.3％,96.2％ and 87.5％,respectively.Conclusions High-pressure syringe injection in HyCoSy is useful for regular administration and high image quality.The AT could be considered as the diagnostic indices in the evaluation of fallopian tube patency for infertile women.
Clinical application of Sonovue sonohysterography in the monitoring of hydrotubation of 41 cases

Abstract:Objective:To explore the relationship between clinical judgement of hydrotubation and ultrasonic imaging under SonoVue sonohysterography.Methods:41 infertile women were selected and injected by SonoVue,the effect of hydrotubation was monitored, the results of laparoscope was golden standard.Results:11 fallopian tubes were diagnosed as complete obstruction and 56 fallopian tubes were diagnosed as narrow fallopian tubes by SonoVue sonohysterography, compared to the results of laparoscope,the sensitivity and specificity of SonoVue sonohysterography were 95.5% and 72.7%,respectively.The recanalization rate of fallopian tubes was 61.2% and gestation rate was 55.6% after recanalization.Conclusion:The recanalization rate of fallopian tubes and gestation rate are high in hydrotubation under SonoVue sonohysterography, the therapy is safe, convenient and combined with fewer complications.

Abstract:A new echogenic contrast medium, SHU-454 is a new method for direct imaging of fallopian tube passage in tubal diagnostics using transvaginal B-mode echography. Clinical studies in over 1,600 patients have shown that this product offers a reproducible echogenic contrast, significantly amplifies Doppler signal and is well tolerated. SHU-454 is injected through the cervix into the uterus and fallopian tubes using the same technique as for radiologic hysterosalpingography. This examination is easily performed in 5 to 10 minutes. Results, compared with two conventional methods, hysterosalpingography and chromolaparoscopy, in 115 women show that HyCoSy is a reliable procedure for detecting tubal patency. HyCoSy results totally agree with the results from the conventional methods in case of tubal patency. This technique may be suitable as an easy, quick and low-risk outpatient examination. It does not involve radiation exposure and is unlikely to cause allergic reactions such as those known to occur with iodinated contrast media. Transvaginal HyCoSy can be used to assess tubal patency early in sterility diagnostics, before adnexal or uterine (myoma) surgery or when tubes and uterine appendages cannot be seen during chromolaparoscopy.

Abstract:Hysterosalpingo-contrast-sonography (HyCoSy) using saline and Echovist is a well tolerated outpatient technique that provides a significant amount of information of relevance to the infertile woman that is not obtainable at hysterosalpingogram (HSG) whilst avoiding exposure to X-ray irradiation. When performed by experienced operators, it serves as a valuable, first-line screening test for the more invasive procedures of laparoscopy and dye chromopertubation and hysteroscopy. If detailed diagnostic information is required in women in whom there is no clinical or ultrasound evidence of pelvic pathology, the surgical technique of fertiloscopy can be considered to be appropriate. This technique permits confirmation that the ovum pick-up mechanism is normal, the tubes are patent and the uterine cavity is normal, while salpingoscopy and microsalpingoscopy permit the assessment of the tubal lumen.

Abstract:In four-dimensional ultrasound hysterosalpinography micro bubble ultrasound contrast agent -sonovueis is injected into the uterine cavity to distend uterine cavity and fallopian for four-dimensional imaging .Four-dimensional ultrasound hysterosalpinography is mainly performed for the evaluation of uterine tubal patency and diagnosis of intrauterine lesions to analyze the causes of female infertility . Diagnosis of Fallopian Tube Patency by Combination of Four-Dimensional and Two-Dimensional Transvaginal Hysterosalpingo-Contrast Sonography

Abstract:Objective To evaluate the value of diagnosis in fallopian tube patency by using four dimensional transvaginal hysterosalpingo-contrast sonography (4D-HyCoSy) and two-dimensional transvaginal hysterosalpingo-contrast sonography(2D-HyCoSy) with SonoVue in infertile women.Methods First,fifty seven infertile women(113 fallopian tubes) were examined by 4D-HyCoSy,then examined by 2D-HyCoSy,and the results of examination were respectively recorded.Results By 4D-HyCoSy,54 fallopian tubes were patency,42 were unobstructed but unsmooth flow,17 were obstructed.By 2D-HyCoSy,48 of them were patency,40 of them were unobstructed but unsmooth flow,25 of them were obstructed.Three fallopian tubes were obstructed by 4D-HyCoSy,but by 2D-HyCoSy they were patency.Eleven fallopian tubes were patency by 4D-HyCoSy,but obstructed by 2D-HyCoSy.Nine fallopian tubes were patency by 4D-HyCoSy,but fimbriated extremity of these fallopian tubes were far awary from homolateral overys.They were unuseful.Eighteen patients underwent laparoscopy,30 fallopian tubes (91.4％) were concordant with laparoscopy using combination of 4D-HyCoSy and 2D-HyCoSy,the sensitivity,specificity,positive predictive value and negative predictive value were 86.7 ％,90.5 ％,86.7 ％ and 90.5 ％.The concidence rate,sensitivity,specificity,positive predictive value and negative predictive value were 85.7％,84.6％,90.5％,84.6％ and 86.4％ by 4D-HyCoSy and 71.4％,64.7％,70.0％,64.7％ and 77.8％ by 2D-HyCoSy,respectively.Accuraty of combination of 4D-HyCoSy and 2D-HyCoSy was similar with laparoscopy.Five patients appeared moderate pelvic pain,three patients appeared mild pelvic pain,two patients appeared nausea and vomiting.None need resuscitation.Conclusions Combination of 4D-HyCoSy and 2D-HyCoSy can more accurately diagnose fallopian tube patency,it can judge if fallopian tube is valuable or not and reduce misdiagnosing.It is a simple,safe,accurate,efficient method for evaluating the fallopian tube patency.

Abstract:OBJECTIVES: To determine the ability of transvaginal ultrasound, with or without saline infusion, to detect focally growing lesions in the uterine cavity in women with postmenopausal bleeding and endometrium > 5 mm, and to determine the accuracy of conventional ultrasound, saline contrast sonohysterography and diagnostic hysteroscopy under general anesthesia to diagnose endometrial polyps, submucous myomas and uterine malignancy.DESIGN: In a prospective study, 105 women with postmenopausal bleeding and endometrium > 5 mm underwent conventional ultrasound examination and saline contrast sonohysterography. Diagnostic and operative hysteroscopy under general anesthesia was then performed. The presence of focally growing lesions and the type of lesion (endometrial polyp, submucous myoma, malignancy or unclear focal lesion) were noted at ultrasound examination and at hysteroscopy.RESULTS: There was almost perfect agreement (96%) between saline contrast sonohysterography and hysteroscopy in the diagnosis of focally growing lesions. Saline contrast sonohysterography and hysteroscopy both had a sensitivity of approximately 80% with regard to diagnosing endometrial polyps (false-positive rates of 24% and 6%, respectively), whereas conventional ultrasound missed half of the polyps (sensitivity, 49%; false-positive rate, 19%). Hysteroscopy was superior to both saline contrast sonohysterography and conventional ultrasound with regard to discriminating between benign and malignant lesions (sensitivity, 84%, 44%, and 60%; false-positive rate, 15%, 6% and 10%, respectively). The risk of malignancy was increased seven-fold (odds ratio, 7.3; 95% confidence interval, 1.9-27.8) in women with distension difficulties at saline contrast sonohysterography, and two thirds of the women with a poorly distensible uterine cavity had a malignant diagnosis.CONCLUSION: Saline contrast sonohysterography is as good as hysteroscopy at detecting focally growing lesions in the uterine cavity in women with postmenopausal bleeding. However, neither hysteroscopy nor saline contrast sonohysterography can reliably discriminate between benign and malignant focal lesions. Distension difficulties at saline contrast sonohysterography should raise a suspicion of malignancy.

Abstract:Background: Infertility is a significant aspect of reproductive health and evaluating degree of tubal pathology is essential for determining appropriate management plans. Aims and objectives: To assess the role of hysterosalpingoscintigraphy (HSSG) as a tubal patency test in nuclear medicine and compare it with hysterosalpingography (HSG) in radiology in infertile women and study pain perception in both tests as well. Materials and methods: A prospective study was conducted on 50 infertility patients undergoing infertility evaluation at a tertiary care hospital. Both HSG and HSSG procedures were performed during proliferative phase of menstrual cycle. Results: Our study demonstrated the potential of HSSG as a tool for evaluating tubal patency in infertility workup. It showed good accuracy in detecting tubal patency compared to HSG. Conclusion: HSG is a radiological procedure valued for its ability to provide detailed anatomical information of uterus and patency of fallopian tubes. In contrast, HSSG provides dynamic information on the functional aspects of the reproductive system using nuclear medicine techniques. Both HSG and HSSG are vital tools in the diagnostic armamentarium for assessing female reproductive health, offering complementary information that aids in comprehensive patient management.

Abstract:To investigate the value of real-time gray-scale contrast- enhanced ultrasound (CEUS) with SonoVue in the diagnosis of uterine tubal obstruction. Forty six females with infertility were inspected by transvaginal real-time gray-scale CEUS imaging during 5-10 days after the end of menses. After injection of contrast agent into the cavity of uterine, we observed the contrast agents affluxion through the uterine tube. All examination images and movies were stored in the local hard disk and compared with routine iodic X-ray salpingography. All CEUS procedures were performed successfully without side effects. Using X-ray salpingography as golden standard, the sensitivity of real-time gray-scale CEUS imaging in the diagnosis of fallopian tubal patency, occlusion and stenosis was 92.0%, 87.1%, 36.4% respectively; The specificity was 85.7%, 96.7%, 93.7%; The accurate was 89.1%, 93.5%, 84.7%; The Kappa was 0.78, 0.85, 0.16. SonoVue is very useful for sonosalpingography and there was no significant difference between real time gray-scale CEUS imaging and routine iodic X-ray salpingography in the diagnosis of uterine tubal occlusion.

Abstract:Nowadays, the prevalence of infertility is increasing. For infertile women, it is necessary to assess tubal patency. The gold standard of tubal patency assessment is laparoscopy with chromopertubation (LC). Hysterosalpingography (HSG), the most widely used technique, may cause iodine allergy and radiation exposure. Therefore, ultrasonography is expected as an alternative method of tubal patency assessment. At present, ultrasonography of fallopian tube consists of saline-infusion sonography (SIS), hysterosalpingo-contrast sonography (HyCoSy) and hysterosalpingo-foam sonography (HyFoSy), and HyCoSy is the most commonly used. Compared to traditional LC and HSG, HyCoSy shows accuracy and convenience while avoiding hospitalization, general anesthesia, radiation and iodine allgery, which makes it a promising technique in the future.
Transvaginal Saline Contrast Sonohystography to Investigate Postmenopausal Bleeding: A Systematic Review

Abstract:Transvaginal ultrasound (TVUS) is the initial investigation of choice for postmenopausal bleeding (PMB), followed by diagnostic hysteroscopy and endometrial sampling if abnormalities are detected. Saline contrast sonohysterography (SCSH) - injection of saline through the cervix into the uterine cavity prior to TVUS - allowed increased diagnostic accuracy in women with PMB in several small, heterogeneous studies. The objectives of the current study were to evaluate the diagnostic accuracy of SCSH in women with PMB, comparing findings with surgical and pathological reports, highlight the necessity of SCSH in guiding clinical decision-making, and establish if there is an increase/decrease in the number of hysteroscopies performed for PMB and, hence, the adherence of clinicians to imaging referral guidelines. The search strategy included formulating search terms identifying all synonyms of SCSH and postmenopause. The databases searched were MEDLINE, Embase, and the Cochrane Library. Only studies comparing SCSH to an alternative method were selected. The studies were screened and data analysis performed using content analysis. Data reduction was performed through systematic coding and the generation of themes We identified 18 studies, comprising 974 women, using SCSH to evaluate the endometrial cavity in women with PMB; most support SCSH improving diagnostic accuracy through delineating intracavitary structures. In effect, SCSH could be a first-line investigative modality to assess the uterine cavity once a larger, well-designed study has been conducted to clarify its specificity, sensitivity, and positive predictive value (PPV). Owing to its relatively non-invasive nature and potentially high diagnostic accuracy, SCSH could allow for more accurate decisions regarding the need for further investigation and subsequent management.
