Abstract:OBJECTIVES:We identified the primary symptoms leading to Chinese patients presenting at hospital with suspected obstructive sleep apnea (OSA) and studied the prevalence and characteristics of OSA in confirmed cases. METHODS:We collected data on 350 consecutive patients (302 males and 43±11 years old) with suspected OSA who underwent overnight polysomnography (PSG). RESULTS:Among all patients, rankings of primary symptoms that led to the patients presenting at hospital for PSG were observed apnea (33%), snoring alone (29%), choking/gasping (13%), daytime sleepiness (5%) and other (20%). For severe OSA, prevalence rate was 61%, apnea hypopnea index (AHI) was 64±18, age was 44±10 years old, body mass index (BMI) was 28±3.5 kg/m(2), and hypertension rate was 28%. CONCLUSIONS:Self-awareness of symptoms led a majority of the patients to present at hospital in China. Compared to currently available case series studies, our results suggest that OSA patients in East Asian countries are characterized by higher prevalence and more severe apnea, younger age, poorer sleep quality, but less obesity and less comorbidity with hypertension, relative to countries in North America, South America and Europe.
Establishment and Application Evaluation of an Improved Obstructive Sleep Apnea Screening Questionnaire for Chinese Community: The CNCQ-OSA

Abstract:Objective: Obstructive sleep apnea (OSA) is a common sleep-disordered breathing disease. We aimed to establish an improved screening questionnaire without physical examinations for OSA named the CNCQ-OSA (Chinese community questionnaire for OSA). Methods: A total of 2585 participants who visited sleep medicine center and underwent overnight polysomnography were grouped into two independent cohorts: derivation (n = 2180) and validation (n = 405). The CNCQ-OSA was designed according to the baseline of patients in derivation cohort. We comprehensively analyzed the data to evaluate the predictive value of the CNCQ-OSA, compared to the GOAL questionnaire, STOP-Bang questionnaire (SBQ) and NoSAS questionnaire. Results: The CNCQ-OSA included seven variables: loud snoring, BMI ≥ 25 kg/m2, male gender, apnea, sleepiness, hypertension and age ≥30, with a total score ranging from 7 to 16.7 points (≥13.5 points indicating high risk of OSA, ≥14.5 points indicating extremely high risk). In the derivation and validation cohorts, the areas under the curve of the CNCQ-OSA were 0.761 and 0.767, respectively. In the validation cohort, the sensitivity and specificity of a CNCQ-OSA score ≥13.5 points for the apnea-hypopnea index (AHI) ≥5/h were 0.821 and 0.559, respectively (Youden index, 0.380), and the score ≥14.5 points were 0.494 and 0.887, respectively (Youden index, 0.375). The CNCQ-OSA had a better predictive value for AHI ≥ 5/h, AHI > 15/h and AHI > 30/h, with the highest Youden index, compared to the other questionnaires. Conclusion: The CNCQ-OSA can effectively identify the risk of OSA, which is appropriate for self-screening at home without physical examinations.
Development and validation of a simple clinical nomogram for predicting obstructive sleep apnea

Abstract:Obstructive sleep apnea is the most common type of sleep breathing disorder. Therefore, the purpose of our research is to construct and verify an objective and easy-to-use nomogram that can accurately predict a patient's risk of obstructive sleep apnea. In this study, we retrospectively collected the data of patients undergoing polysomnography at the Sleep Medicine Center of the First Affiliated Hospital of Guangzhou Medical University. Participants were randomly assigned to a training cohort (50%) and a validation cohort (50%). Logistic regression and Lasso regression models were used to reduce data dimensions, select factors and construct the nomogram. C-index, calibration curve, decision curve analysis and clinical impact curve analysis were used to evaluate the identification, calibration and clinical effectiveness of the nomogram. Nomograph validation was performed in the validation cohort. The study included 1035 people in the training cohort and 1078 people in the validation cohort. Logistic and Lasso regression analysis identified age, gender, diastolic blood pressure, body mass index, neck circumference and Epworth Sleepiness Scale as the predictive factors included in the nomogram. The training cohort (C-index = 0.741) and validation cohort (C-index = 0.745) had better identification and calibration effects. The areas under the curve of the nomogram and STOP-Bang were 0.741 (0.713-0.767) and 0.728 (0.700-0.755), respectively. Decision curve analysis and clinical impact curve analysis showed that the nomogram is clinically useful. We have established a concise and practical nomogram that will help doctors better determine the priority of patients referred to the sleep centre.

Abstract:Objectives: Snoring is a common symptom of obstructive sleep apnea (OSA) which is considered to be potential predictors of the obstruction site. Successful treatment of OSA depend on the determination the types of obstruction site. This study aimed to develop a machine learning-based model to detect obstruction site using snoring sound.Methods: Patients with OSA underwent drug-induced sleep endoscopy (DISE) and the snoring sounds were recorded simultaneously. We extracted acoustic features based on Mel-frequency cepstral coefficients (MFCC). A k-nearest neighbors (KNN) was used for snore classification.Results: Total 42 patients with OSA were enrolled. The accuracy of model was 85.55 %, F1 score was 85.04. With combined age, gender and Body Mass Index (BMI), the accuracy of model was 87.98 %, and F1 score was 87.96. The model exhibited accuracies of 83 %, 93 % and 92 %; an AUC of 85.88, 89.22 and 88.17 in detecting ret-ropalatal, retrolingual and multilevel obstructions.Conclusion: Our results suggest that combing snoring sound with age, gender and BMI, the machine learning based model can help automatically assess obstruction site. The model may have potential utility as a clinical tool to help for clinical decision-making.

Abstract:Objective: To analyze the correlation between drug-induced sleep endoscopy (DISE), results, polysomnography (PSG) indicators, and clinical parameters in patients with obstructive sleep apnea (OSA), and to establish and validate a predictive model for tongue base plane obstruction. Methods: This retrospective study analyzed 117 OSA patients diagnosed via PSG and treated at the Department of Otolaryngology-Head and Neck Surgery, Nanfang Hospital, Southern Medical University, between October 2014 and March 2022. The cohort comprised of 114 males and 3 females, with an age range of 20 to 54 years (mean age 38.1±8.4 years). Data on DISE results, PSG results, and clinical indicators were collected for all 117 patients. Logistic regression analysis was performed to identify relevant indicators, and a predictive model for tongue base plane obstruction was constructed and internally validated using the R programming language. Results: Univariate logistic regression analysis identified four independent risk factors for predicting tongue root plane obstruction: tonsil grading, N2, N3, and rapid eye movement sleep(REM) stage [OR:0.412(0.260~0.652),1.045(1.012~1.079),0.943(0.903~0.984),0.961(0.925~0.998),P <0.05]. Multivariate logistic regression analysis confirmed tonsil grading and N3 sleep stage (12.48±12.22%) as significant predictors. A nomogram model incorporating these factors demonstrated good predictive performance, with an area under curve(AUC) of 0.82 (95%CI: 0.548-1.000), an optimal cutoff of 0.519, a specificity of 80.0%, and a sensitivity of 86.7%. Internal validation of the model in the validation cohort yielded an AUC of 0.751 (95%CI: 0.625-0.876). Conclusions: Tongue base plane obstruction observed during DISE in OSA patients is associated with tonsil grading and N3 sleep stage duration. The predictive model developed for tongue base plane obstruction based on DISE demonstrates good efficacy, as evidenced by its internal validation.
Moderate to severe OSA screening based on support vector machine of the Chinese population faciocervical measurements dataset: a cross-sectional study

Abstract:Objectives Obstructive sleep apnoea (OSA) has received much attention as a risk factor for perioperative complications and 68.5% of OSA patients remain undiagnosed before surgery. Faciocervical characteristics may screen OSA for Asians due to smaller upper airways compared with Caucasians. Thus, our study aimed to explore a machine-learning model to screen moderate to severe OSA based on faciocervical and anthropometric measurements. Design A cross-sectional study. Setting Data were collected from the Shanghai Jiao Tong University School of Medicine affiliated Ruijin Hospital between February 2019 and August 2020. Participants A total of 481 Chinese participants were included in the study. Primary and secondary outcome (1) Identification of moderate to severe OSA with apnoea–hypopnoea index 15 events/hour and (2) Verification of the machine-learning model. Results Sex-Age-Body mass index (BMI)-maximum Interincisal distance-ratio of Height to thyrosternum distance-neck Circumference-waist Circumference (SABIHC2) model was set up. The SABIHC2 model could screen moderate to severe OSA with an area under the curve (AUC)=0.832, the sensitivity of 0.916 and specificity of 0.749, and performed better than the STOP-BANG (snoring, tiredness, observed apnea, high blood pressure, BMI, age, neck circumference, and male gender) questionnaire, which showed AUC=0.631, the sensitivity of 0.487 and specificity of 0.772. Especially for asymptomatic patients (Epworth Sleepiness Scale <10), the SABIHC2 model demonstrated better predictive ability compared with the STOP-BANG questionnaire, with AUC (0.824 vs 0.530), sensitivity (0.892 vs 0.348) and specificity (0.755 vs 0.809). Conclusion The SABIHC2 machine-learning model provides a simple and accurate assessment of moderate to severe OSA in the Chinese population, especially for those without significant daytime sleepiness.

Abstract:PURPOSE:There are upper airway abnormalities in patients with obstructive sleep apnea (OSA), and their speech signal characteristics are different from those of unaffected people. In this study, the severity of OSA was evaluated automatically by machine learning technology based on the speech signals of Chinese people.METHODS:In total, 151 adult male Mandarin native speakers who had suspected OSA completed polysomnography to assess the severity of the disease. Chinese vowels and nasal sounds were recorded in sitting and supine positions, and the accuracy of predicting the apnea-hypopnea index (AHI) of the participants using a machine learning method was analyzed based on features extracted from the speech signals.RESULTS:Among the 151 participants, 75 had AHI > 30 events/h, and 76 had AHI ≤ 30 events/h. Various features including linear prediction cepstral coefficients (LPCC) were extracted from the data collected from participants recorded in the sitting and supine positions and by using a linear support vector machine (SVM); we classified the participants with thresholds of AHI = 30 and AHI = 10 events/h. The accuracies of the classifications were both 78.8%, the sensitivities were 77.3% and 79.1%, and the specificities were 80.3% and 78.0%, respectively.CONCLUSION:This study constructed a severity evaluation model of OSA based on speech signal processing and machine learning, which can be used as an effective method to screen patients with OSA. In addition, it was found that Chinese pronunciation can be used as an effective feature to predict OSA.

Abstract:Objective: To develop a robust machine learning prediction model for the automatic screening and diagnosis of obstructive sleep apnea (OSA) using five advanced algorithms, namely Extreme Gradient Boosting (XGBoost), Logistic Regression (LR), Support Vector Machine (SVM), Light Gradient Boosting Machine (LightGBM), and Random Forest (RF) to provide substantial support for early clinical diagnosis and intervention. Methods: We conducted a retrospective analysis of clinical data from 439 patients who underwent polysomnography at the Affiliated Hospital of Xuzhou Medical University between October 2019 and October 2022. Predictor variables such as demographic information [age, sex, height, weight, body mass index (BMI)], medical history, and Epworth Sleepiness Scale (ESS) were used. Univariate analysis was used to identify variables with significant differences, and the dataset was then divided into training and validation sets in a 4:1 ratio. The training set was established to predict OSA severity grading. The validation set was used to assess model performance using the area under the curve (AUC). Additionally, a separate analysis was conducted, categorizing the normal population as one group and patients with moderate-to-severe OSA as another. The same univariate analysis was applied, and the dataset was divided into training and validation sets in a 4:1 ratio. The training set was used to build a prediction model for screening moderate-to-severe OSA, while the validation set was used to verify the model's performance. Results: Among the four groups, the LightGBM model outperformed others, with the top five feature importance rankings of ESS total score, BMI, sex, hypertension, and gastroesophageal reflux (GERD), where Age, ESS total score and BMI played the most significant roles. In the dichotomous model, RF is the best performer of the five models respectively. The top five ranked feature importance of the best-performing RF models were ESS total score, BMI, GERD, age and Dry mouth, with ESS total score and BMI being particularly pivotal. Conclusion: Machine learning-based prediction models for OSA disease grading and screening prove instrumental in the early identification of patients with moderate-to-severe OSA, revealing pertinent risk factors and facilitating timely interventions to counter pathological changes induced by OSA. Notably, ESS total score and BMI emerge as the most critical features for predicting OSA, emphasizing their significance in clinical assessments. The dataset will be publicly available on my Github.
Clinical evaluation in predicting childhood obstructive sleep apnea

Abstract:Objective: To determine whether parents' observation, clinical examination, and lateral upper airway radiograph are useful in detecting clinically significant obstructive sleep apnea (OSA) in children. Method: We retrospectively reviewed data of 50 children aged 4 to 18 years who were consecutively referred to a sleep clinic for suspected OSA. All subjects underwent clinical assessments including standardized history collection, physical examination, and lateral neck radiograph for measurement of postnasal space. Each child underwent overnight polysomnography on the night of clinical assessments. Patients with clinically significant OSA, defined as apnea-hypopnea index (AHI) > 5, were compared with primary snorers, defined as AHI < or = 5. Results: Thirty-one children had clinically significant OSA, and 19 children were primary snorers. The prevalence of risk factors including allergic rhinitis, obesity, and craniofacial anomaly was similar between the two groups. Observable apnea during sleep, nocturnal enuresis, intrusive naps, mouth breathing, enlarged tonsils, and radiologic features of upper airway narrowing due to adenoid hypertrophy were found to be predictors for clinically significant OSA. Combining upper airway narrowing and mouth breathing or nocturnal enuresis had a sensitivity of 90.3%, and combining all six predictors had a sensitivity of 93.5% of detecting OSA. Conclusion: Combining clinical and radiologic findings might be helpful to screen for children with clinically significant OSA who need earlier investigation and intervention.
Validation of GOAL questionnaire as screening tool for clinical obstructive sleep apnea: A large sample study in China

Abstract:Background: Obstructive sleep apnea (OSA) is a serious disease with a high prevalence in the general population. The purpose of this study is to explore the effectiveness of the GOAL questionnaire in the clinical screening of OSA and compare it with other existing screening tools. Materials and methods: Outpatients and inpatients who underwent polysomnography (PSG) examination at the Sleep Medicine Center of the First Affiliated Hospital of Guangzhou Medical University from January 2013 to November 2016 were analyzed retrospectively. The basic data such as demographic, medical history, etc., and PSG data of the patients were collected, and the sensitivity, specificity, positive predictive value, negative predictive value and area under the curve (AUC) of GOAL and five other screening scales (the NoSAS score, Epworth Sleepiness Scale, the Berlin questionnaire, STOP, and STOP-Bang questionnaire) were calculated. Results: Data from 2,171 participants (1,644 male; 78%) were analyzed there were 1,507 OSA patients [Apnea Hypopnea Index (AHI) ≥ 5 events/h] among them, accounting for about 69.415%. No matter which cut-off point (AHI ≥ 5, 15 and 30 events/h), the AUC score reveals that GOAL questionnaire had comparable screening ability to the NoSAS and STOP-BANG, and performed better than the ESS, and the AUC scores of the STOP questionnaire and Epworth Sleepiness Scale (ESS) were both lower than 0.7. When the cut-off point of the AHI was 5 events/h, the AUC of GOAL was the highest at 0.799 (0.781-0.816), and its sensitivity was the highest at 89.1%. The sensitivity levels of the NoSAS score and STOP-Bang questionnaire were 67.4 and 78.8% respectively, while ESS and the Berlin questionnaire have higher specificity (70.2 and 72.3% respectively) but lower sensitivity (49.3 and 60.0% respectively). Conclusion: GOAL is a free, efficient and easy to manage tool with a screening ability comparable to NoSAS and STOP-Bang, and better than that of ESS.

Early identification of children with OSA, particularly those at a higher risk for severe symptoms, is essential for making informed clinical decisions and improving long-term outcomes. Therefore, we developed and validated a risk prediction model for severity in Chinese children with OSA to effectively identify children with moderate-to-severe OSA in a clinical setting. Patients and Methods: From June 2023 to September 2023, we retrospectively analyzed the medical records of 367 Children diagnosed with OSA through portable bedside polysomnography (PSG). Predictor variables were screened using the least absolute shrinkage and selection operator (LASSO) and logistic regression techniques to construct nomogram to predict the severity of OSA. Receiver operating characteristic curve (ROC), calibration curve, decision curve analysis (DCA), and clinical impact curve (CIC) were used to determine the discrimination, calibration, and clinical usefulness of the nomogram. Results: A total of 367 children with a median age of 84 months were included in this study. Neck circumference, ANB, gender, learning problem, and level of obstruction were identified as independent risk factors for moderate-severe OSA. The consistency indices of the nomogram in the training and validation cohorts were 0.841 and 0.75, respectively. The nomogram demonstrated a strong concordance between the predicted probabilities and the observed probabilities for children diagnosed with moderate-severe OSA. With threshold probabilities ranging from 0.1 to 1.0, the predictive model demonstrated strong predictive efficacy and yielded improved net benefit for clinical decision-making. ROC analysis was employed to classify the children into high and low-risk groups, utilizing the Optimal Cutoff value of 0.39. Conclusion: A predictive model using LASSO regression was developed and validated for children with varying levels of OSA. This model identifies children at risk of developing OSA at an early stage. Pediatric OSA is a challenging disease because OSA severity is associated with age, adenoid size, tonsil size (Hwang, 10 2013), and obesity. 11 Although nocturnal polysomnography (PSG) is considered the "gold standard" for diagnosing pediatric OSA, its application is constrained. 12–14 The American Academy of Sleep Medicine (AASM) 15 and the American Academy of Pediatrics 16 assert that Portable monitors (PM) are suitable for patients with high clinical suspicion of OSA. Abstract:Purpose: Snoring patients, as a high-risk group for OSA, are prone to the combination of severe OSA and face serious health threats. The aim of our study was to develop and validate a nomogram to predict the occurrence of severe OSA in snorers, in order to improve the diagnosis rate and treatment rate in this population. Patients and methods: A training cohort of 464 snoring patients treated at our institution from May 2021 to October 2022 was divided into severe OSA and non-severe OSA groups. Univariate and multivariate logistic regression were used to identify potential predictors of severe OSA, and a nomogram model was constructed. An external hospital cohort of 210 patients was utilized as an external validation cohort to test the model. Area under the receiver operating characteristic curve, calibration curve, and decision curve analyses were used to assess the discriminatory power, calibration, and clinical utility of the nomogram, respectively. Results: Multivariate logistic regression demonstrated that body mass index, Epworth Sleepiness Scale total score, smoking history, morning dry mouth, dream recall, and hypertension were independent predictors of severe OSA. The area under the curve (AUC) of the nomogram constructed from the above six factors is 0.820 (95% CI: 0.782-0.857). The Hosmer-Lemeshow test showed that the model had a good fit (P = 0.972). Both the calibration curve and decision curve of the nomogram demonstrated the corresponding dominance. Moreover, external validation further confirmed the reliability of the predicted nomograms (AUC=0.805, 95% CI: 0.748-0.862). Conclusion: A nomogram predicting the occurrence of severe OSA in snoring patients was constructed and validated with external data for the first time, and the findings all confirmed the validity of the model. This may help to improve existing clinical decision making, especially at institutions that do not yet have devices for diagnosing OSA.

Abstract:Study Objectives: To screen all of the obstructive sleep apnea (OSA)-characteristic pronunciations, explore the pronunciations which provide a better OSA classification effect than those used previously, and further clarify the correlation between speech signals and OSA.Methods: A total of 158 adult male Mandarin native speakers who completed polysomnography at the Sleep Medicine Center of Beijing Tongren Hospital from November 15, 2019, to January 19, 2020, were enrolled in this study. All Chinese syllables were collected from each participant in the sitting position. The syllables, vowels, consonants, and tones were screened to identify the pronunciations that were most effective for OSA classification. Results: The linear prediction coefficients of Chinese syllables were extracted as features and mathematically modeled using a decision tree model to dichotomize participants with apnea-hypopnea index thresholds of 10 and 30 events/h, and the leave-one-out method was used to verify the classification performance of Chinese syllables for OSA. Chinese syllables such as [leng] and [jue], consonant pronunciations such as [zh] and [f], and vowel pronunciations such as [ing] and [ai] were the most suitable pronunciations for classification of OSA. An OSA classification model consisting of several syllable combinations was constructed, with areas under curve of 0.83 (threshold of apnea-hypopnea index = 10) and 0.87 (threshold of apnea-hypopnea index = 30), respectively.Conclusions: This study is the first comprehensive screening of OSA-characteristic pronunciations and can act as a guideline for the construction of OSA speech corpora in other languages.

Abstract:Obstructive sleep apnea syndrome (OSAS) ,a common sleep disorder,is characterized by episodes of cessation of breathing during sleep lasting for more than 10 seconds, due to repetitive partial or complete obstruction of the upper airway. Surgical patients with OSAS may lead to peri-operative complications. An up-to-date "gold" diagnosis standard of OSAS remains polysomnogram. We review a variety of preoperative evaluation methods available for OSAS diagnosis,including questionnaires, portable devices,and other clinical screening tests. Abstract:Objective: This paper evaluates the application value of the STOP-Bang questionnaire combined with the Epworth Sleepiness Scale (ESS) in screening for obstructive sleep apnea (OSA) in the population. Method: Thousand-six hundred seventy-one patients with suspected OSA who visited the Sleep Medicine Center of the First Affiliated Hospital of Guangzhou Medical University from August 2017 to August 2020 were monitored by overnight polysomnography (PSG) after completing the ESS scale and STOP-Bang questionnaire. The sensitivity, specificity, positive predictive value, negative predictive value and receiver operating characteristic (ROC) curves of the two scales were calculated, and the accuracy in predicting OSA of the STOP-Bang questionnaire combined with ESS was analyzed. Results: With Apnea Hypopnea Index (AHI) cutoffs of ≥5, ≥15 and ≥30 events/h, the areas under the ROC curve scored by STOP-Bang were 0.724, 0.703 and 0.712, and those of ESS were 0.632, 0.634 and 0.695; the diagnostic odds ratio (DOR) values of STOP-Bang for OSA, moderate to severe OSA, and severe OSA were 3.349, 2.651 and 3.189, and those of ESS were 2.665, 2.279 and 3.289. The STOP-Bang score of three was used as the cut-off point for OSA diagnosis with higher sensitivity and lower specificity, while ESS had higher specificity. STOP-Bang (≥3) combined with ESS significantly improved its specificity for predicting OSA. Conclusion: The STOP-Bang questionnaire is a simple and effective new tool for screening patients for OSA, while a STOP-Bang score of ≥3 combined with ESS can further improve its specificity. Thus, we suggest further screening with ESS after a STOP-Bang score of ≥3 in suspected patients.

Abstract:Objective: By comparing the predictive value of the NoSAS (Neck circumference, Obesity, Snoring, Age and Sex) score combined with the Epworth Sleepiness Scale (ESS), STOP-Bang Questionnaire (STOP-Bang), STOP Questionnaire (STOP) and Berlin Questionnaire (Berlin), the application value of the NoSAS score combined with ESS in screening Obstructive sleep apnea hypopnea syndrome (OSAHS) in the population is evaluated. Method: 2560 suspected OSAHS patients visited the Sleep Medical Center of the First Hospital of Guangzhou Medical University between September 1, 2016 and October 31, 2020, and were monitored with a polysomnogram (PSG) after completing the NoSAS score, ESS, STOP-Bang, STOP and Berlin. The sensitivity, specificity, positive predictive value, negative predictive value and receiver operating characteristic (ROC) curve of each scale were calculated, and the accuracy in predicting OSAHS of the NoSAS score combined with ESS and each scale was analyzed. Results: The areas under the ROC curve scored by Berlin were higher than those of the other four questionnaires with Apnea Hypopnea Index (AHI) cutoffs of ≥5 and ≥ 10 events/h, while the area under the ROC curve scored by the NoSAS score was the highest with AHI cutoffs of ≥15, ≥20, ≥25 and ≥ 30 events/h. Among the five scales, the diagnostic odds ratio (DOR) of the NoSAS score was the highest. When a NoSAS score of ≥7 was used as the cutoff point for diagnostic NoSAS, it had higher sensitivity and specificity with a NoSAS score of ≥8 as the cutoff point for diagnostic NoSAS. A NoSAS score of ≥7 combined with ESS significantly improved its specificity for predicted OSAHS patients. Conclusion: The NoSAS score is a simple and effective new tool for screening patients for OSAHS, while a NoSAS score of ≥7 combined with ESS can further improve its specificity. Thus, we suggest further screening with ESS after a NoSAS score of ≥7 in suspected populations.

Abstract:Background/purpose: This study was designed to explore upper airway aerodynamic characteristics in individuals diagnosed with obstructive sleep apnea (OSA) and to evaluate correlations between these characteristics and other anatomical upper airway findings in these patients. Materials and methods: This was a retrospective study of 40 OSA patients (22 male, 18 female) who were stratified into groups with mild, moderate, and severe disease based upon overnight polysomnographic (PSG) recording results. Newtom5G cone-beam CT scans (CBCT) were conducted for all patients, and the resultant images were used to reconstruct threedimensional images of the upper airways which were used to calculate aerodynamic characteristics. Differences in these characteristics between groups were evaluated with one-way ANOVAs, while relationships between anatomical and aerodynamic characteristics were assessed through Pearson correlation analyses. Results: The aerodynamic of the upper airway has typical characteristic in severe group. There was a significant negative correlation in severe group between resistance during inspiration (Rin) and volume (V) (r = -0.693, P = 0.013), minimum axial area (MMA) (r = -0.685, P = 0.014), and lateral dimension (LAT) (r = -0.724, P = 0.008), resistance during expiration (Rex) and LAT (r = -0.923, P < 0.001). Conclusion: This study showed that airway resistance during inspiration and expiration are most closely associated with upper airway collapse in OSA patients, with repetitive collapse occurring during both of these breathing processes. Abstract:Objective To explore the possible obstructive site of the airway during sleep,so as to provide reference for the treatment of OSAHS patients.Methods We selected 30 OSAHS patients and 30 patients with simple snoring as controls to undergo electronic la-ryngoscopy.Roots of uvula,tonsil pole,epiglottis sink were observed and compared at quiet breathing and Muller respiratory status.Re-sults There was a significant difference in OSAHS group from control group.Airway narrowing of OSAHS patients appeared in multiple planes.Narrowing degrees of pharynx of OSAHS patients were significantly different at Muller respiratory status.Conclusions Elec-tronic laryngoscopy has clinical significance for observation of upper airway obstruction and change in OSAHS patients,which is a sim-ple and effective preoperative assessment of OSAHS patients.

Abstract:Age, gender, body mass index (BMI), and mean heart rate during sleep were found to be risk factors for obstructive sleep apnea (OSA), and a variety of methods have been applied to predict the occurrence of OSA. This study aimed to develop and evaluate OSA prediction models using simple and accessible parameters, combined with multiple machine learning algorithms, and integrate them into a cloud-based mobile sleep medicine management platform for clinical use. The study data were obtained from the clinical records of 610 patients who underwent polysomnography (PSG) at the Sleep Medicine Center of the Second Affiliated Hospital of Fujian Medical University between January 2021 and December 2022. The participants were randomly divided into a training-test group (80%) and an independent validation group (20%). The logistic regression, artificial neural network, naïve Bayes, support vector machine, random forest, and decision tree algorithms were used with age, gender, BMI, and mean heart rate during sleep as predictors to build a risk prediction model for moderate-to-severe OSA. To evaluate the performance of the models, we calculated the area under the receiver operating curve (AUROC), accuracy, recall, specificity, precision, and F1-score for the independent validation set. In addition, the calibration curve, decision curve, and clinical impact curve were generated to determine clinical usefulness. Age, gender, BMI, and mean heart rate during sleep were significantly associated with OSA. The artificial neural network model had the best efficacy compared with the other prediction algorithms. The AUROC, accuracy, recall, specificity, precision, F1-score, and Brier score were 80.4% (95% CI 76.7–84.1%), 69.9% (95% CI 69.8–69.9%), 86.5% (95% CI 81.6–91.3%), 61.5% (95% CI 56.6–66.4%), 53.2% (95% CI 47.7–58.7%), 65.9% (95% CI 60.2–71.5%), and 0.165, respectively, for the artificial neural network model. The AUROCs for the LR, NB, SVM, RF, and DT models were 80.2%, 79.7%, 79.2%, 78.4%, and 70.4%, respectively. The six models based on four simple and easily accessible parameters effectively predicted moderate-to-severe OSA in patients with PSG screening, with the artificial neural network model having the best performance. These models can provide a reliable tool for early OSA diagnosis, and their integration into a cloud-based mobile sleep medicine management platform could improve clinical decision making.
