Abstract:<section class="article-section__content"><p>Signet‐ring cell carcinoma (SRCC) is an extremely rare subtype of adenocarcinoma with aggressive behavior that usually occurs in the gastrointestinal tract, prostate or breast. In this study, we described the clinicopathological and prognostic characteristics of, and the therapeutic strategies for seven patients with primary SRCC of the lung from Sichuan University West China Hospital to enhance our understanding of this kind of tumor. According to the information presented herein, it is believed that patients with primary pulmonary SRCC can live for a long time if the cancer is diagnosed early and treated actively. Abstract:A 56-year-old female patient presented with insidiously developed verruciform plaques on her trunk and extremities for one and a half year and hemialgia on her left side of head for 5 months.Signet-ring cell carcinoma was confirmed by histopathology of the skin lesion.Bone marrow biopsy showed infiltration of characteristic signet-ring cells.B-ultrasound demonstrated solid mass in right adnexa.The final diagnosis was signet-ring cell carcinoma with skin and bone metastases.The origin of the carcinoma was unknown.

Abstract:Introduction: Signet-ring cell carcinoma of the uterine cervix commonly occurs owing to metastasis. Today, the rarity of primary cervical origin is still making it a diagnostic challenge. This review aims to raise awareness to maintain early diagnosis and appropriate management. Case presentation: A 37-year-old patient presented with postcoital vaginal bleeding with a history revealing a negative Papanicolaou smear 2 years prior. Pelvic examination revealed a cervical mass and the biopsy was interpreted as a signet ring cell pattern. Detailed extrapelvic evaluation was made to rule out a possible extragenital primary tumor. The patient underwent a type-2 radical hysterectomy, pelvic and paraaortic lymphadenectomy. Following the histopatologic evaluation, the case was diagnosed and managed as primary cervical signet ring cell carcinoma (PCSRCC) with International Federation of Gynecology and Obstetrics 2018 stage IIIC. The patient received adjuvant chemoradiotherapy and is currently disease-free 24 months following surgery. Discussion: The small number of cases causes difficulty with an accurate diagnosis. Imaging and immunohistochemical (IHC) studies should be performed to distinguish possible primary sites. IHC studies are not yet close to refusing or confirming the diagnosis. Due to the lack of data, there is no consensus on the proper therapeutic strategy. Prognosis and survival appear to depend upon the stage of disease at diagnosis. Therefore, the awareness of such a histopathological kind of cervical cancer is crucial.
Primary signet ring cell carcinoma of the cervix: A case report with review of the literature

Abstract:Objective: Primary cervical signet ring cell carcinoma (PCSRCC) is extremely rare. In this paper, we describe a case presenting with PCSRCC. Case report: The 48-year-old woman visited the gynecological department because of postmenopausal bleeding. A cervical mass was discovered through pelvic examination, and the biopsy results indicated a poorly differentiated adenocarcinoma with a signet ring cell pattern. Colonoscopy revealed external compression of the rectum without intraluminal mucosal lesions. Abdominal computed tomography revealed a suspicious malignant lesion at the cervicorectal junction and multiple metastases. Debulking surgery was performed and the final pathology report documented a FIGO stage IVb PCSRCC involving multiple sites. Conclusion: Complete tumor survey and staging are critical to differentiate primary from metastatic signet cell carcinoma of the cervix. Immunohistochemical studies cannot provide precise information. Because cases are rare, it is difficult to determine the proper treatment guidelines and prognosis.

Abstract:Key clinical message: Primary signet-ring cell carcinoma of the anal canal and rectum is an extremely rare and aggressive malignancy. The present case underscores the importance of considering primary signet-ring cell carcinoma in differential diagnoses for young patients with chronic anorectal symptoms. It highlights the need for a multidisciplinary treatment approach (including surgery, chemotherapy, and radiotherapy) and comprehensive follow-up for managing this challenging condition and improving long-term patient outcomes. Abstract: Primary signet-ring cell carcinoma of the anal canal and rectum is an exceedingly rare subtype of colorectal adenocarcinoma, often originating as an extension of rectal adenocarcinoma. This malignancy constitutes a small fraction of colorectal cancers and is scarcely reported in medical literature. We present the case of an 18-year-old male with a three-year history of progressively worsening hematochezia, anorectal pain, and defecation-associated prolapse. Initial conservative treatments failed, leading to further investigations that revealed a palpable, nodular anorectal mass. Imaging studies (including CT and MRI), and biopsy confirmed poorly differentiated adenocarcinoma with signet-ring cell morphology. The tumor exhibited extensive lymphovascular invasion and involved perirectal lymph nodes, and was staged as pT3, N2a. Immunohistochemical staining was positive for CK 7, CK 20, and SATB2, supporting the primary anorectal origin. The treatment regimen included initial diversion colostomies for symptom relief, followed by neoadjuvant chemotherapy with a modified 5-fluorouracil, leucovorin, irinotecan, and oxaliplatin (FOLFIRINOX) regimen and concurrent chemoradiation with Xeloda. The patient subsequently underwent an abdominoperineal resection (APR), which confirmed the diagnosis and achieved curative resection. Postoperative complications included transient ileus and wound infection, which were managed with supportive care. This case underscores the diagnostic and therapeutic challenges posed by primary signet-ring cell carcinoma of the anorectal region, highlighting the need for a high index of suspicion and comprehensive diagnostic workup in atypical presentations. The multimodal treatment approach, incorporating surgery, chemotherapy, and radiotherapy, was crucial in managing this locally advanced tumor. The rarity and aggressiveness of this carcinoma necessitate a tailored treatment strategy to improve patient outcomes. Long-term follow-up, including regular imaging and surveillance, is vital for monitoring disease recurrence and evaluating treatment effectiveness.

Abstract:PurposeTo investigate the clinicopathological features and prognosis of signet ring cell carcinoma of the stomach (SRC).MethodsA total of 1464 gastric cancer patients who underwent curative gastrectomy from 2000 to 2008 at a single center were evaluated. Signet ring cell carcinoma (SRC) was defined as the presence of at least 50% signet ring cells in the pathologic specimen. The clinicopatho-logical parameters and prognosis of SRC were analyzed by comparing with non-signet ring cell carcinoma (NSRC).ResultsOf 1464 patients, 138 patients (9.4%) were classified as SRC. There were significant differences in gender, age, tumor location, TNM stage, p21 expression, and p53 expression between SRC and NSRC. The 5-year survival rates of SRC and NSRC were 36.2% and 49.5%, respectively. The prognosis of SRC was poorer than that of NSRC (P < 0.001). Multivariate analysis showed that SRC histology was an independent factor for poor prognosis (P < 0.001).ConclusionPatients with SRC tend to present with a more advanced stage and poorer prognosis than patients with other types of gastric carcinoma.
Clinicopathologic and Molecular Features of Colorectal Adenocarcinoma with Signet-Ring Cell Component.

Abstract:BACKGROUND:We performed a retrospective study to assess the clinicopathological characters, molecular alterations and multigene mutation profiles in colorectal cancer patients with signet-ring cell component.METHODS:Between November 2008 and January 2015, 61 consecutive primary colorectal carcinomas with signet-ring cell component were available for pathological confirmation. RAS/BRAF status was performed by direct sequencing. 14 genes associated with hereditary cancer syndromes were analyzed by targeted gene sequencing.RESULTS:A slight male predominance was detected in these patients (59.0%). Colorectal carcinomas with signet-ring cell component were well distributed along the large intestine. A frequently higher TNM stage at the time of diagnosis was observed, compared with the conventional adenocarcinoma. Family history of malignant tumor was remarkable with 49.2% in 61 cases. The median OS time of stage IV patients in our study was 14 months. RAS mutations were detected in 22.2% (12/54) cases with KRAS mutations in 16.7% (9/54) cases and Nras mutations in 5.4%(3/54) cases. BRAF V600E mutation was detected in 3.7% (2/54) cases. As an exploration, we analyzed 14 genes by targeted gene sequencing. These genes were selected based on their biological role in association with hereditary cancer syndromes. 79.6% cases carried at least one pathogenic mutation. Finally, the patients were classified by the percentage of signet-ring cell. 39 (63.9%) cases were composed of ≥50% signet-ring cells; 22 (36.1%) cases were composed of <50% signet-ring cells. We compared clinical parameters, molecular and genetic alterations between the two groups and found no significant differences.CONCLUSIONS:Colorectal adenocarcinoma with signet-ring cell component is characterized by advanced stage at diagnosis with remarkable family history of malignant tumor. It is likely a negative prognostic factor and tends to affect male patients with low rates of RAS /BRAF mutation. Colorectal patients with any component of signet-ring cells, regardless of the extent, shared similar clinicopathological characteristics, molecular alterations and genetic profiles.

Abstract:Globally, gastrointestinal cancer is the most widespread neoplastic disease and the primary contributor to cancer-associated fatalities. Gastrointestinal signet ring cell carcinoma (SRCC) exhibits unique distinguishing features in several aspects when compared to adenocarcinomas (ACs). The scarcity of signet ring cell carcinoma has resulted in a heightened significance of related clinical and molecular investigations. However, a comprehensive and systematic review of the clinical, molecular, therapeutic, and research aspects of this disease is currently absent. This review provides an overview of the latest developments in our understanding of the clinical and molecular features of gastrointestinal signet ring cell carcinoma (SRCC). Additionally, we have compiled a list of potential therapeutic targets or biomarkers, as well as an examination of the current treatment options and the possible mechanisms of formation.
Primary Salivary Gland–Type Lung Cancer: Clinicopathological Analysis of 88 Cases from China

Abstract:INTRODUCTION:Salivary gland-type cancers are rare lung neoplasms involving mucoepidermoid carcinoma (MEC), adenoid cystic carcinoma (ACC), and epithelial-myoepithelial carcinoma (EMC). Their behavior and prognostic features are not clearly defined because of their low incidence. We retrospectively analyzed the clinicopathologic profiles of these tumors in a large series.METHODS:Eighty-eight patients confirmed as having primary salivary gland-type lung cancer between May 2001 and January 2013 were included from the archives of two thoracic oncology center institutions in China and retrospectively evaluated.RESULTS:Of the total 88 patients, 69 were MEC, 12 ACC, and seven EMC. Overall survival (OS) at 3, 5, and 10 years was 91.3%, 86%, and 80.6% in all cases, respectively, and disease-free survival (DFS) was 90.1%, 78.6%, and 55%, respectively. No significant difference was found among MEC, ACC, and EMC groups regarding OS (p = 0.518) and DFS (p = 0.082). Tumor-node-metastasis stage, lymph node involvement, intrathoracic invasion, and margin status were found to be related with OS (p = 0.000, 0.029, 0.000, 0.004) and DFS (p = 0.018, 0.042, 0.002, 0.002). Intrathoracic invasion was an independent predictor for OS (hazard ratio [HR], 1.129; p = 0.039) and DFS (HR, 1.071; p = 0.011). For patients with MEC, pathological grade also was an independent predictor of OS (HR, 0.045; p = 0.006) and DFS (HR, 0.067; p = 0.001).CONCLUSIONS:Salivary gland-type lung cancers are a group of low-aggressive entities with higher tendency to recurrence/metastasis. Intensive clinical, radiological, and pathological examinations are essential to estimation of the risk stratification and management.
Palliative Radiation Therapy For Metastatic Primary Signet Ring Cell Carcinoma Of The Lung: A Seer Analysis

Abstract:Primary signet ring cell (SRCC) of the lung is an extremely rare subtype of lung adenocarcinoma with a poor prognosis. Studies have shown a prevalence of distant metastases at initial diagnosis in lung SRCC. The role of palliative radiotherapy in lung SRRC with distant organ metastases needs to evolve. We analyzed SEER data to examine the role of palliative radiation in metastatic lung SRRC. Data were obtained from SEER database between 2010-2014.Patients of interest were identified by site recode C34.0-34.9 and ICD-O-3 code 8490/3 from SEER. We recorded metastatic information related to specific-organ metastases. Overall survival (OS) was calculated from the date of diagnosis using Kaplan Meier (KM) survival analysis and Cox regression model for comparisons of hazard ratio (HR) among subgroup. A two side p value <0.05 was regarded statistically significant. A total of 242 patients with metastatic lung SRCC were included. The median age was 64. 227 patients have a single-organ site of metastases while 15 patients have multi-organ metastases. For patients with single site metastases, the proportion of bone, brain, liver, lung metastasis was 41.0%, 20.7%, 12.8%, 25.6%, respectively. The mean survival was 10.3 months (95% CI=8.4-12.2), 1-yr OS ,2-yr OS was 28.50%, 14.40%, respectively. There was significant difference in OS between single site metastatic lung SRCC patients received palliative radiotherapy (yes vs no, Mean Survival (MS) 12.4 months vs 8.5 months) (p<0.001). For patients with isolated brain metastases at diagnosis, addition of palliative radiation improved OS significantly (MS 14.9 months; 1-yr OS 49.1%; 2-yr OS 18.9%;) compared to no radiation (MS 4.9 months, 1-yr OS 7.1%; log-rank p=0.005). OS was significantly different between patients with isolated-liver metastases treated with radiation (MS 11.3 months; 1-yr OS 36.4%; 2-yr OS 24.2%) versus no radiation (MS 7.5 months; 1-yr OS 18.8%; 2-yr OS 9.4%; log-rank p=0.04). For patients with isolated bone metastases , palliative radiation didn’t improve OS significantly (MS 9.9 months; 1-yr OS 24.7%; 2-yr OS 12.4%) compared to no radiation (MS 8.8 months; 1-yr OS 25.8%; 2-yr OS 8.2%; p=0.217). For isolated lung metastases patients underwent radiotherapy, MS was 12 months, 1-yr, 2-yr OS were 45.6%, 27.3%, which was significantly different versus no radiation (MS 3 months; 1-yr OS 22.5%; 2-yr OS 14.1%; p=0.02). Multivariable analysis showed that, for isolated brain, lung and liver metastases lung SRCC patients, palliative radiotherapy was associated with longer OS compared with observation. However, treatment of radiation couldn’t improve OS for lung SRCC patients with isolated bone metastases. For primary lung SRCC patients with isolated brain, liver or lung metastases, our analysis showed OS benefit of palliative radiation. Palliative radiotherapy should be recommended for metastatic lung SRCC in clinical management.

Prostate Adenocarcinoma with Signet-Ring Cells and Features of Mucin: A Clinical Case and Literature Review

Abstract:Introduction: Signet-ring cells are typically associated with mucin-secreting epithelium; thus, they are most commonly found in the gastrointestinal tract, but not exclusively. Primary signet-ring cell carcinoma of the prostate is a rare and poorly differentiated, aggressive acinar adenocarcinoma variant with a grim prognosis. Clinical Case: In June of 2023, a 54-year-old Caucasian male presented with a complaint of lower urinary tract obstructive symptoms with occasional macrohematuria, non-specific body aches, and shortness of breath. A prostate specimen obtained in transurethral resection of the prostate was sent for histopathological examination. After a series of extraprostatic diagnostic workups, including fibrogastroduodenoscopy, colonoscopy computed tomography imaging, and immunohistochemical studies, the patient was diagnosed with primary prostatic signet-ring cell adenocarcinoma stage IV. Unfortunately, due to the advanced stage of the disease, PE, and third-degree thrombocytopenia, the patient was not a candidate for chemotherapy and died of cardiopulmonary insufficiency later that week. Discussion: Prostatic signet-ring cell carcinoma accounts for 0.02% of all prostate adenocarcinoma cases. Due to its nature and epidemiology, a diligent extraprostatic investigation has to be carried out. The disease often presents with unremarkable clinical symptoms and variable serum prostate-specific antigen results, which may contribute to its late diagnosis. Inconsistent immunohistochemical findings and an unpredictable response to hormonal treatment together pose both diagnostic and therapeutic challenges that negatively affect the prognosis. Conclusions: This study highlights the importance of a multidisciplinary approach and the need for diagnostic and therapeutic consensus within the research community in search of the primary site of the disease, which may positively influence the prognosis.
A Minor (<50%) Signet-Ring Cell Component Associated with Poor Prognosis in Colorectal Cancer Patients: A 26-Year Retrospective Study in China

Abstract:Background We performed a retrospective study to determine the cancer-specific survival of colorectal cancer patients with a component of signet-ring cells or mucin comprising < 50% of the tumor mass. Methods A total of 2454 patients seen in our hospital from 1985 to 2011 were retrospectively studied. The patients were divided into five groups according to type of cancer: signet-ring cell carcinoma (with > 50% signet-ring cell, n = 36), partial signet-ring cell carcinoma (with < 50% signet-ring cell, n = 28), mucinous adenocarcinoma (with > 50% mucin lacking signet-ring cell, n = 267), partial mucinous adenocarcinoma (with < 50% mucin lacking signet-ring cell, n = 145), and classic adenocarcinoma (with absence of either mucin or signet-ring cell, n = 1978). Results Patients with > 50% or < 50% signet-ring cell had the lowest 5-year survival rates (35.5% and 29.7%, respectively), followed by patients with > 50% mucin (48.8%). Patients who had partial mucinous adenocarcinoma with < 50% mucin and classic adenocarcinoma patients had the highest 5-year survival rates (64.8% and 65.3%, respectively). Stratified and multivariate analysis showed that signet-ring cell carcinoma, partial signet-ring cell carcinoma and mucinous adenocarcinoma were independent predictors of decreased survival (hazard ratio 1.699, P = 0.016; hazard ratio 2.182, P = 0.005; hazard ratio 1.532, P < 0.001; respectively), and partial mucinous adenocarcinoma was not (hazard ratio 1.137, P = 0.431). Conclusions Patients with a component of signet-ring cells, regardless of the extent, had poor prognoses. Patients with mucinous adenocarcinoma containing >50% mucin had poor prognoses as well, whereas those with < 50% mucin had survival rates similar to those of classic adenocarcinoma patients. Therefore, in clinical practice, patients with a component of signet-ring cells, regardless of the extent, should be given significant clinical attention.
Morphologic Heterogeneity of Carcinoma with Signet Ring Cell Features at Different Primary Sites

Abstract:Introduction: Signet ring cells (SRCs) may be observed in carcinomas from multiple primary sites. Elucidating unknown primaries from metastases with SRCs represents a diagnostic challenge. This study examined morphologic characteristics of adenocarcinomas with SRCs from stablished primary sites and described objective features, which can aid in identifying the site of origin. Methods: The series encompasses 257 cases of adenocarcinomas with SRCs from gastroesophageal junction (GEJ, n = 38), stomach (n = 48), pancreatobiliary system (n = 16), colorectum (n = 40), appendix (n = 32), breast (n = 41), and lung (n = 42). H&E sections were examined and scored using architectural and cytologic criteria. Morphometric analysis was performed using QuPath software. Results: Extracellular mucin was more abundant in GEJ, colorectal, and appendiceal carcinomas. Poorly cohesive morphology was the most frequent pattern in gastric and breast carcinomas. The cytoplasmic mucin/vacuole was predominantly clear and targetoid in breast carcinomas. Breast and gastric carcinomas showed the highest nuclear to cytoplasmic (N/C) ratio, whereas appendiceal carcinoma the lowest. Conclusion: Morphological evaluation (extracellular mucin, architectural patterns, and the nature of cytoplasmic mucin/vacuole) represents an important step to determine the cancer site of origin in adenocarcinomas with SRCs and guides further ancillary studies. Cytological morphometry may help further refine morphological criteria and facilitate the construction of digital-pathology algorithms.

Abstract:Lung cancer remains the leading cause of cancer-related mortality worldwide, with non-small cell lung cancer (NSCLC) constituting 85% of cases. Among NSCLCs, squamous cell carcinoma (SqCC) is strongly associated with smoking. However, lung cancer in never smokers (LCINS) represents approximately 25% of lung cancer cases globally and shows increasing incidence, particularly in East Asia. LCINS-SqCC is less well-characterized, especially regarding its genomic alterations and their impact on clinical outcomes. We conducted a retrospective analysis over a 20-year period (July 2003-July 2023) at two major tertiary centers in the UK. The cohort included 59 patients with LCINS-SqCC who underwent radical surgical resection. Data collected included demographic information, comorbidities, histopathological details, and outcome metrics such as disease-free and overall survival. Molecular sequencing of tumor specimens was performed to identify genomic aberrations. The cohort had a median age of 71 years (IQR 62-77) and a median BMI of 25.4 (IQR 22.8-27.8), with a slight male predominance (53%). The majority of patients (93%) had a preoperative MRC of 1-2. Recurrent disease was observed in 23 patients (39%), and 32 patients (54%) had died at a median follow-up of 3 years. Median disease-free survival was 545 days (IQR 132-1496), and overall survival was 888 days (IQR 443-2071). Preoperative creatinine levels were higher in patients who experienced recurrence (p = 0.037). Molecular analysis identified biallelic SMARCB1 loss in two younger patients, associated with rapid disease progression despite R0 resection. These patients' tumors were PDL1-negative, TTF-1-negative, and positive for cytokeratin, CD56, and p40. SMARCB1-deficient SqCC in never smokers represents a highly aggressive variant with poor disease-free survival, highlighting the importance of integrating advanced molecular diagnostics in clinical practice. This study underscores the necessity for personalized treatment strategies, including targeted therapies such as EZH2 inhibitors and immune checkpoint blockade, to address the unique molecular pathways in SMARCB1-deficient cancers. Further clinical trials are essential to optimize therapeutic approaches for this challenging subgroup of lung cancer.
Signet-Ring Cell Squamous Cell Carcinoma: A Biphenotypic Neoplasm of the Gastro-Esophageal Junction with Uncertain Biological Potential: Case Report and Literature Review

Abstract:The signet-ring cell variant of squamous cell carcinoma (SCC) is an extremely rare histological subtype, with only 24 cases (including the present case) reported in the Medline database: 15 affecting the external surface of the body, 3 in the lung, 2 affecting the uterine cervix, 1 involving the gingiva, another one affecting the esophagus and the present case that is the first reported at the gastro-esophageal junction (GEJ). In one case, the location of the lesion was not mentioned. A 59-year-old male patient underwent segmental eso-gastrectomy for carcinoma of the GEJ. The microscopic examination showed a pT3N1-staged SCC composed of solid nests admixed in over 30% of the tumor, with cells having eccentrically located nuclei and clear vacuolated cytoplasm. The signet-ring cells did not show mucinous secretion and were positive for keratin 5/6 and vimentin, with nuclear expression of β-catenin and Sox2 and focal membrane positivity for E-cadherin. Based on these features, the case was considered a signet-ring SCC with epithelial-mesenchymal transition. Thirty-one months after surgery, the patient was disease-free, with no local recurrence and no known distant metastases. In SCC, a signet-ring cell component might be an indicator of the dedifferentiation of tumor cells towards a mesenchymal molecular subtype.

Abstract: Metastasis to the testes is a rare occurrence, and identifying the primary site of origin can pose a significant challenge. Signet-ring cell carcinoma (SRCC) is an uncommon subtype of adenocarcinoma typically found in the stomach but can also occur in other organs. This case report presents a 44-year-old male with signet-ring cell adenocarcinoma metastasis in the right testis. The patient's initial clinical manifestation was testicular painful swelling, and subsequent immunohistochemical analysis using CK7, CK20, and CDX2 markers suggested a gastrointestinal origin. Normal upper and lower endoscopies rise suspicion of a small intestinal origin. The rarity of SRCC of the small intestine and the lack of clinical trials make treatment decisions difficult. This case highlights the importance of immunohistochemistry in determining the primary site of metastatic tumors and underscores the need for further research to establish optimal treatment strategies for rare malignancies like SRCC of the small intestine. As early diagnosis and appropriate treatment are critical for better patient outcomes.
The Characteristics and Prognostic Factors in Colorectal Cancer Containing Signet Ring Cell

Abstract:Background: Colorectal signet ring cell carcinoma (SRCC) is a rare entity with poor prognosis. The aim of the study was to investigate the clinicopathological characteristics and identify the prognostic factors in colorectal cancer containing signet ring cell (CRC-SRC). Methods: We retrospectively analyzed 98 CRC-SRC patients confirmed by pathology from February 1987 to December 2015 in our institute. Age, gender, clinical characteristics, laboratory findings, pathologic grade and TNM staging were studied to identify prognostic factors associated with the overall survival (OS) and progression-free survival (PFS). Statistical analysis was conducted by SPSS 20.0. Results: Of the 98 patients in this study, 60 rectal patients accounted for 61.2%. Ninety-five (96.9%) patients were diagnosed as stage III or IV tumors, 76 (77.5%) patients had lymph node metastasis, and 27 (27.5%) patients had distant metastasis. The median survival time was 22.1 months (range, 1-165 months). The 1-, 3-, and 5-year OS rates of the patients were 74.5% (95%CI: 65.9%-83.1%), 44.8% (95%CI: 34.8%-54.8%), and 31.2% (95%CI: 21.4%-41.0%), respectively. Preoperative carcinoembryonic antigen (CEA), N staging, M staging, local relapse were associated with OS (χ2=4.163, 8.320, 11.199, 15.436, P < 0.05), while tumor nodules, postoperative chemotherapy, and postoperative radiotherapy had significant influence on PFS (χ2=10.039, 11.733, 7.116, P < 0.05). On multivariate analyses, preoperative CEA (HR = 2.514, 95%CI: 1.108-5.704) and distant metastases (HR = 2.127, 95%CI: 0.999-4.525) were independent predictors for survival. Based on the point of 30% and 80%, which was calculated by X-tile, we found that the proportion of signet ring cells in colorectal cancer had no significant influence on OS and PFS. Conclusions: Long-term follow-up of colorectal SRCC showed that it was a rare subtype of colorectal cancer with poor outcomes. Furthermore, the dismal prognosis was related to the presence of signet ring cells, rather than the proportion of signet ring cells. Preoperative CEA and distant metastases were hopeful to predict prognosis of CRC-SRC independently. Abstract:Introduction. Colorectal cancer is the third most common cancer worldwide. Signet ring carcinoma is an extremely rare subtype of colorectal cancer, with frequency ranges 0.3-4.6%. The diagnosis of this type of cancer is based on pathohistological analysis. Case outline. A 58-year-old patient was admitted due to abdominal pain and abdominal swelling. The physical findings indicated abdomen above the level of the chest, soft, painfully sensitive in the left hemiabdomen, with positive clinical signs of ascites. Laboratory analyzes indicated: positive inflammatory syndrome, elevation of D-dimer and CA-19.9. Ascites analysis showed the presence of malignant cells. Computed tomography revealed: hepatomegaly, liver steatosis, as well as multiple secondary deposits in the liver, ascites and peritoneal implants. Colonoscopy showed ulceration of the right colon, which was covered with fibrin. The pathohistological findings indicated poorly differentiated, invasive adenocarcinoma of the signet ring carcinoma type. The patient was treated with analgesics, diuretics, proton pump inhibitors, beta 2 blockers, angiotensin converting enzyme inhibitors, low molecular weight heparin, antibiotics and supportive therapy. The patient was discharged after 10 days of hospitalization. He was presented to the multidisciplinary team, which decided on further symptomatic therapy. Conclusion. Signet ring colon cancer is a rare, aggressive tumor with a poor prognosis. Although it is most often localized in the stomach, it is necessary to think about the colorectal localization of this tumor in the differential diagnosis of patients with colonic complaints, especially if they have ?alarm symptoms? and if they are younger.
