Reliability of two alternative tricuspid annular plane systolic excursion measurement methods

Abstract:Introduction and aim Right ventricle dysfunction (RVD) is very frequent in intensive care units (ICU). Due to its ease, tricuspid annular plane excursion (TAPSE) is the most used parameter for RV systolic function. Conventional TAPSE is measured using M- mode imaging. However, some of the widely used hand-held ultrasonography devices (HHUD) do not have M- mode imaging properties and it is not always possible to obtain an apical-four chamber (A4C) view in critically ill patients (1) and a subcostal four-chamber view becomes the only window that allows the evaluation of heart function. This study aimed to evaluate the accuracy of two alternative TAPSE measurement methods Patients and methods This prospective observational study was conducted on 32 patients hospitalized in a 7-bed ICU. Conventional TAPSE from A4 view (TAPSE), subcostal TAPSE from subcostal view (SC-TAPSE), obtained from the distance of tricuspid valve annulus between systole and diastole, using cursor placement (Fig. A, B) (1), and B-Mode TAPSE from A4 view (BM- TAPSE), obtained from substruction of the distances between tricuspid valve annular plane and apex during systole and diastole (Fig. C) (2) were measured via a Vivid q device (General Electric Healthcare) equipped with a 3- MHz probe. Results In Bland-Altman's analysis, there was good agreement and there was no bias between TAPSE and SC- TAPSE, but although there was no bias, agreement was poor between TAPSE and BM- TAPSE. There was no significant difference between TAPSE (19 ± 4.8, media n = 19) and SC- TAPSE (18 ± 4.3, median = 18), where median SC- TAPSE was 1 cm under TAPSE (p = 0.40). BM- TAPSE (16 ± 4.4, median = 17) differed from TAPSE significantly, where median BM- TAPSE was 2 cm under TAPSE (p = 0.03). There was no significant difference between the TAPSEs in the evaluation of RVD. Conclusion Although SC-TAPSE can be used instead of the conventional TAPSE, it should be taken into account that it may measure 1 cm lower. References 1. Main AB, Braham R, Campbell D, et al.
