Abstract:Objective To evaluate the application of direct anterior approach for hip arthroplasty in treatment of femoral neck fracture in elderly patients with Parkinson's disease (PD). Methods The clinical data of 38 elderly PD patients (Hoehn-Yahr stage IV) with femoral neck fractures undergoing total hip arthroplasty in our hospital from October 2013 to October 2015 were retrospectively analyzed. The patients were divided into anterior approach and posterior approach groups with 19 cased in each group. Direct anterior or posterior hip arthroplasty, including total hip arthroplasty and bipolar arthroplasty, were performed. The patients were followed up for one month. The incision length, intraoperative blood loss, operation time, postoperative complications and incidence of dislocation were compared between the two groups. The clinical results were evaluated using modified Harris scores, X-ray films before and after surgery. Results The incision in the anterior approach group was shorter than that in the posterior approach group (P＜0.05). There was no significant difference in intraoperative blood loss and operation time between two groups. Harris scores at 1 month after surgery were significantly higher in the anterior group (82.4 ±1.7) than that in the posterior group (80.1 ±1.4, P＜0.05). X-ray examination showed that the hip joint prosthesis was stabilized 1 month after surgery in the anterior group. The rate of complications in the anterior group was 5.26％, which was significantly lower than that 15.79％ in posterior group (P＜0.05). In the direct anterior approach, there was 1 case of sensory hypoesthesia of the lateral thigh skin caused by the traction injury of the lateral femoral cutaneous nerve. Conclusion Direct anterior approach hip replacement for treatment of femoral neck fracture in elderly patients with Parkinson's disease has a good therapeutic efficacy and can significantly reduce the postoperative complications, especially hip dislocation.

Abstract:Displaced femoral neck fractures in the young are difficult to treat. The complexity of the fractures for closed or open reduction requires careful surgical planning and experience. Acceptable reduction criteria in this fracture is crucial and should be followed strictly in order to get the favorable outcomes. Various reduction techniques have been described ranging from closed reduction by traction table or closed reduction with minimal direct manipulation with instruments to direct open reduction. This manuscript describes the mini open reduction, Watson-Jones and Smith-Petersen approaches, and some modifications in terms of indications, advantages, and disadvantages of each approach for the decision-making in these complex fractures.

Abstract:Objectives: To summarize the clinical features of pediatric femoral neck fractures and analyze the risk factors for avascular necrosis of the femoral head. Methods: A retrospective analysis of the case data of pediatric femoral neck fractures treated in our hospital from January 2010 to December 2022, including gender, age, fracture type, causative factors, and surgical details. The occurrence of avascular necrosis of the femoral head was recorded and risk factors were analyzed. Results: From January 2010 to December 2022, a total of 45 cases of femoral neck fractures were treated in our hospital with a median age at onset of 93 months (IQR=81) and a median time from injury to surgery of 96 hours (IQR=46). Closed reduction was performed in 36 cases, while open reduction was performed in 9 cases. Avascular necrosis of the femoral head occurred in 29 cases postoperatively, while it did not occur in 16 cases. Increased time from injury to surgery and greater degree of fracture displacement were independent risk factors for avascular necrosis of the femoral head. The risk of avascular necrosis in Garden IV type femoral neck fractures was significantly higher than in Garden II and III type patients. An injury-to-surgery time exceeding 82.5 hours was identified as a critical threshold for the development of avascular necrosis of the femoral head. Conclusion: Pediatric femoral neck fractures have a low incidence rate and are mostly caused by high-energy trauma, often resulting in severe injuries. Therefore, actively maintaining stable vital signs and properly managing associated injuries, timely surgical intervention for femoral neck fractures, achieving good reduction and fixation of displaced fractures are crucial in the treatment of pediatric femoral neck fractures.

Abstract:Objective: Transgluteal approach for anatomical reduction of femoral neck fractures (extra-intraarticular) in children under preservation of the blood supply of the femoral head. Indications: Femoral neck fractures AO 31-M/2.1 I‑III; 31-M/3.1 I‑III; 31-M/3.2 I‑III. Contraindications: None. Surgical technique: Preparation of a muscular flap including the proximal insertion of the vastus lateralis muscle and approximately one third of the gluteus medius muscle. Elevation of the gluteus minimus muscle from the hip capsule without completely detaching it from its insertion. Exposure of the anterolateral hip capsule and capsulotomy followed by controlled reduction of the fracture fragments without compromising the retinacular vessels. Postoperative management: Touch-down weightbearing for 4-6 weeks (age dependent). To protect the healing of the abductors, active abduction or passive adduction prohibited for 4-6 weeks. Consolidation radiographs 4-6 weeks postoperatively. Results: Excellent results in 29 patients subsequently treated in the last 10 years by the transgluteal approach. No cases of avascular necrosis of the femoral head by this procedure.

Abstract:Background and objectives Treatment of benign osteolytic lesions in the femoral head and neck can be extremely challenging, particularly in children with open physis or for aggressive tumors with pathological fracture. There remains the difficult management decision as to whether to perform complete excision of the involved area or only curettage. Moreover, there is no agreed consensus on the optimal approach to lesion access when performing curettage, which included the transcervical, open and direct approach. The current systematic review aims to provide guidance for selection of surgical methods in clinical practice by comparing the advantages and drawbacks of different procedures. Methods A comprehensive literature search of PubMed, Embase and Web of Science databases were executed for human studies restricted to the English language. The search was filtered to include studies published from January 1980 to January 2020. Results A total of 33 articles including 274 patients were enrolled in the final analysis. The most common diagnosis was chondroblastoma (CBT) (104, 38.0%), followed by giant cell tumor (GCT) (56, 20.4%). There were 57 (20.8%) patients with pathological fracture. Intralesional curettage was performed in 257 (93.8%) patients with the local recurrence of 12.5% at the mean follow-up of 51.5 months. The patients who were presented with open physis or curetted via transcervical approach developed higher local recurrence in patients with CBT ( P < 0.001). The local recurrence rate of GCT is 33.3% after curettage, while 8 of 9 (88.9%) patients with fracture were treated successfully with joint preservation. Two of 45 (4.4%) patients developed avascular necrosis (AVN) of femoral head after surgical hip dislocation. The reported Musculoskeletal Tumor Society (MSTS) Score was comparable among patients with different approaches to curettage. Conclusion The majority of benign osteolytic lesions in the femoral head and neck can be treated with intralesional curettage with acceptable local tumor control and satisfactory function. The incidence of local recurrence might be decreased dramatically for lesion access under direct visualization. The native joint maintenance could be achieved even in patients with aggressive lesions presenting pathological fracture.

Abstract:Femoral neck fractures in physiologically young patients typically occur from high-energy axial loading forces through the thigh with the hip in an abducted position. These fractures have a high rate of associated head, chest, abdominal, and musculoskeletal injuries. High-energy hip fractures differ from traditional geriatric hip fractures regarding incidence, mechanism, management algorithms, and complications. After adequate resuscitation, goals of treatment include anatomic reduction and stable fixation while maintaining vascularity of the femoral head, which can be achieved through a variety of different techniques. Prompt recognition and treatment of these fractures is crucial to achieve a successful outcome because these injuries are often associated with complications such as osteonecrosis, fixation failure, and nonunion.

Comparison of DAA and PLA Approaches for Total Hip Replacement in the Treatment of Femoral Neck Fractures

Abstract:Objectives : Femoral neck fractures are becoming more common within nowadays. This research is to explore the clinical effect of primary total hip replacement (THA) via direct anterior approach (DAA) and posterolateral approach (PLA) in the treatment of femoral neck fracture. Methods : Retrospective analysis of 100 cases of elderly patients with femoral neck fracture who underwent total hip arthroplasty admitted to Tianyou Hospital affiliated to Wuhan University of Science and Technology from January 2019 to January 2022. 50 patients treated with DAA approach were included in the observation group, and 50 patients treated with PLA approach were included in the control group. The operation indexes, postoperative acetabular abduction angle and anteversion angle, hip joint function, Harris score and complications were compared between the two groups. Result : The length of incision in the observation group was shorter than that in the control group, and the amount of intraoperative bleeding and postoperative hospital stay were shorter than those in the control group (P < 0.05); There was a statistically significant difference between the two groups in the ratio of acetabular abduction angle and its safe zone, and the length difference of both lower limbs (P < 0.05), while there was no statistically significant difference between the two groups in the ratio of acetabular anteversion angle and its safe zone, eccentricity, and its recovery rate (P > 0.05); Harris score of hip joint: 6 months after operation, the anterior approach group was significantly higher than the posterolateral approach group (P < 0.05), and there was no statistical difference between the two groups 12 months after operation (P > 0.05); The total incidence of complications in the observation group was lower than that in the control group, with a statistically significant difference (P < 0.05). Conclusion : DAA and PLA approaches for total hip replacement can restore the hip joint structure of patients with femoral neck fractures and achieve good results, but DAA approach has greater advantages in early postoperative recovery, improvement of hip joint function, small surgical injury, high application value, so it is recommended.
3D printing-based Ganz approach for treatment of femoral head fractures: a prospective analysis

Abstract:Background: Femoral head fractures are uncommon injuries. Open reduction and internal fixation (ORIF) of femoral head fracture is the preferred treatment for most patients. There are several surgical approaches and treatments for this difficult fracture. However, the optimal surgical approach for the treatment of femoral head fracture remains controversial. Meanwhile, the operation is difficult and the complications are numerous. We prospectively reviewed patients with femoral head fractures managed surgically through the 3D printing-based Ganz approach to define a better approach with the least morbidity. Patients and methods: Between 2012 and 2017, a total of 17 patients were included in this study. An exact 1:1 3D printing model of the injured hip side was fabricated for each patient and simulated surgery was finished preoperative. The surgical approach was performed as described by Ganz. Functional assessment was performed using the modified Merle d'Aubigne scores. The reduction of the fracture was evaluated according to Matta's criteria. The incidence of complications, such as heterotopic ossification (HO) and avascular necrosis (AVN), and the need for additional surgery were also documented. Results: Twelve of 17 patients (four females and eight males) were available for 2 years follow-up. The mean follow-up was 35 months (25-48 months). Average age for the 12 patients was 39.9 ± 12.2 years. According to the Pipkin classification, four patients were type I fracture, three patients were type II fracture, and five patients were type IV fracture. The mean operative time was 124.2 ± 22.1 min, and the estimated blood loss was 437.5 ± 113.1 ml. According to Merle d' Aubigne scores, excellent results were achieved in six of the 12 patients; four good and two poor results occurred in the rest of the patients. On the radiograph evaluation, fracture reduction was defined as anatomical in eight patients, and imperfect in four. Most patients had good outcomes and satisfactory hip function at last follow-up. Almost all great trochanteric osteectomy healed uneventfully. One patient developed symptomatic AVN of the femoral head and underwent THA at 3 years. After THA, she regained a good hip function with the ability to return to work and almost no reduction in sports activities. Heterotopic ossification was found in four cases (type I-1, type II-2, and type III-1). Conclusions: The 3D printing-based Ganz approach provides a safe and reliable approach and satisfactory results of treatment in femoral head fractures. Using 3D printed model for the fracture of the femoral head, the fracture can be viewed in every direction to provide an accurate description of fracture characteristics, which contributes to make a reasonable surgical plan for patients. In addition, the 3D printing-based Ganz approach can obtain excellent surgical exposure and protection of the femoral head blood supply, reduce the operation time and intraoperative blood loss, make the precise osteotomy, anatomically fix the intra-articular fragments, and effectively reduce postoperative complications. Abstract:Since its introduction by Smith and Robinson, the anterior approach to the subaxial cervical spine has become one of the standard procedures for numerous cervical spine pathologies, including, but not limited to degenerative disease, trauma, tumor, deformity, and instability. Along with its increasing popularity and improvements in anterior instrumentation techniques, a comprehensive knowledge of the surgical anatomy during the anterior exposure is critical for trainees and experienced spine surgeons alike to minimize the infrequent but potentially devastating risks associated with this approach. Understanding the anatomy and techniques to minimize damage to relevant structures can reduce the risks of developing notable postoperative complications and morbidity.
Free vascularised fibular graft for neglected femoral neck fractures in young adults

Abstract:Neglected femoral neck fractures in young adults pose a great challenge to orthopedic surgeons because of the high risks of nonunion and osteonecrosis. We attempted to determine whether free vascularised fibular grafting through an anterior approach could enhance bone union, improve hip function and reduce complications in the treatment of such fractures. Thirteen patients with neglected femoral neck fractures were treated with a free vascularised fibular graft through an anterior approach between 2004 and 2008. The mean age was 30.9 years (range, 17-47 years). The average delay between injury and operation was 6.7 months (range, 1.5-22 months). All 13 cases had ununited fractures but without osteonecrosis of the femoral head on plain radiographs before coming under our care. The average follow-up was 51.2 months (range, 36-75 months). All patients had fracture union within an average of 4.8 months (range, 3-9 months). Postoperatively, 9 patients had coxa vara of 10 to 20 degrees and leg discrepancy between 0.5 to 1.5 cm. One patient had coxa vara deformity of 25 degrees and 2-cm leg discrepancy. No patients developed osteonecrosis of the femoral head or donor-site morbidity. The mean Harris hip score improved from 55.5 points preoperatively to 84.8 points postoperatively (p<0.01). This procedure may be useful and safe in the treatment of neglected femoral neck fractures in young adults, but further studies with a large number of patients are needed.
Precision reduction of femoral neck fractures: a novel strategy based on the femoral neck fracture morphology

Abstract:In femoral neck fractures the secondary damage caused by repeated multiple reductions needs to be prevented. Accordingly, the aim of this study was to achieve an anatomical reduction in the first manipulation of reduction in femoral neck fractures. We propose a new reduction strategy using the x-ray morphology of femoral neck fractures for preoperative planning. In the present study we compared this approach to conventional operation procedures for the treatment of femoral neck fractures. From 2020 to 2021, 35 patients with femoral neck fractures were operated with this reduction strategy. Those were compared with 34 patients treated without the proposed preoperative method. All patients were compared for consistency between the preoperative assessment and the intraoperative execution of the reduction approach, number of reductions, operative time, number of fluoroscopies, intraoperative bleeding, and reduction quality. The operative time of patients in the study group was significantly shorter (38.40 ± 10.26 min) than that of the control group (47.26 ± 9.09 min), and the number of reductions [1.0 (1.0, 1.0)] was significantly less than that of the control group [2.0 (1.75, 2.25)]. The number of fluoroscopies (10.27 ± 2.84) was also significantly less than that of the control group (13.53 ± 2.59) times. The KAPPA value = 0.886 shows the good agreement between the preoperative proposed protocol and the intraoperative protocol about the study group .The bleeding, quality of repositioning, Harris Hip score, MOS 12-item Short Form Survey (SF-12), and early complication rate were not statistically different between the groups (P > 0.05). The selection of the appropriate repositioning method based on the analysis of femoral neck fracture X-ray morphology can improve the efficiency and accuracy of preoperative planning. This reduces the secondary damage that may be caused by multiple reduction, shortens the operation time and reduces the exposure to radiation.
Impact of blade direction on postoperative femoral head varus in PFNA fixed patients: a clinical review and biomechanical research

Abstract:Intertrochanteric femur fracture is a common type of osteoporotic fracture in elderly patients, and postoperative femoral head varus following proximal femoral nail anti-rotation (PFNA) fixation is a crucial factor contributing to the deterioration of clinical outcomes. The cross-angle between the implant and bone might influence fixation stability. Although there is a wide range of adjustment in the direction of anti-rotation blades within the femoral neck, the impact of this direct variation on the risk of femoral head varus and its biomechanical mechanisms remain unexplored. In this study, we conducted a retrospective analysis of clinical data from 69 patients with PFNA fixation in our institution. We judge the direction of blade on the femoral neck in on the immediate postoperative lateral X-rays or intraoperative C-arm fluoroscopy, investigating its influence on the early postoperative risk of femoral head varus. p < 0.05 indicates significant results in both correlation and regression analyses. Simultaneously, a three-dimensional finite element model was constructed based on the Syn-Bone standard proximal femur outline, exploring the biomechanical mechanisms of the femoral neck-anti-rotation blade direction variation on the risk of this complication. The results indicated that ventral direction insertion of the anti-rotation blade is an independent risk factor for increased femoral head varus. Complementary biomechanical studies further confirmed that ventral angulation leads to loss of fixation stability and a decrease in fixation failure strength. Therefore, based on this study, it is recommended to avoid ventral directional insertion of the anti-rotation blade in PFNA operation or to adjust it in order to reduce the risk of femoral head varus biomechanically, especially in unstable fractures. This adjustment will help enhance clinical outcomes for patients.

Abstract:Highlights • The porous structure directed neovessels into the encapsulated barren areas, facilitating bone regeneration. • Mechanical adaptation proceeded during material degradation and tissue regeneration. • Bio-adaptive reconstruction employing β-TCP system is a promising hip-preserving strategy for ANFH. A variety of techniques have been used for treating avascular necrosis of the femoral head (ANFH), but have frequently failed. In this study, we proposed a β-TCP system for the treatment of ANFH by boosting revascularization and bone regeneration. The angio-conductive properties and concurrent osteogenesis of the highly interconnected porous β-TCP scaffold were revealed and quantified through an in vivo model that simulated the ischemic environment of ANFH. Mechanical test and finite element analysis showed that the mechanical loss caused by tissue necrosis and surgery was immediately partially compensated after implantation, and the strength of the operated femoral head was adaptively increased and eventually returned to normal bone, along with continuous material degradation and bone regeneration. For translational application, we further conducted a multi-center open-label clinical trial to assess the efficacy of the β-TCP system in treating ANFH. Two hundred fourteen patients with 246 hips were enrolled for evaluation, and 82.1% of the operated hips survived at a 42.79-month median follow-up. The imaging results, hip function, and pain scores were dramatically improved compared to preoperative levels. ARCO stage II disease outperformed stage III in terms of clinical effectiveness. Thus, bio-adaptive reconstruction using the β-TCP system is a promising hip-preserving strategy for the treatment of ANFH. Assessment of the Α-Angle of Femoral Neck and Morphological Classification of the Anterior Femoral Head-Neck Junction on CT-based Images in Normal Chinese Adults

Abstract:Objective To investigate the normal range of the femoral neck α-angle in normal Chinese adults and classify the morphology of the anterior femoral head-neck junction on CT-based images. Methods Six hundred and fifty-two adult patients (Bilateral: 459 patients, unilateral:193 patients) with the total of 1111 hips (552 left; 559 right, and 654 male; 457 female) without known diseases affecting the proximal femur or symptoms of femoroacetabular impingement (FAI) underwent 64-slice CT scanning for medical purpose with the hip included in the scan range. The volume CT data was used for further analysis in this study. Oblique sagittal plane images paralleling to the axis of the femoral neck were reconstructed with the volume CT data, the image through the middle of the femoral neck was chosen to measure α-angle with AutoCAD2006 software. The morphology of the anterior femoral head-neck junction was classified. Analysis of variance and t-test were performed with SPSS 15.0. Results The mean value of α-angle of all 1111 hips was 38. 2° ±5.3°. The normal range of α-angle in Chinese adults was 28°to 49°.The mean value of left and right α-angles were 38. 0° ±5. 3°and 38.4° ±5. 3° ,respectively, and there was no statistically significant difference between both sides (t = - 1. 231 ,P ＞ 0. 05 ). Males have greater α-angles than females 39. 2° ±5. 8 vs 36. 8° ±4. 1°, with t = -8. 180, P ＜0. 01. There was no statistically significant differences of the α-angles of the seven age groups (F = 1. 765, P ＞ 0. 05 ). In all 1111 proximal femora, 168 were classified as concave type, accounting for 15. 1%, with a mean α-angle of 31.2° ±2. 0°,726 were smooth type, accounting for 65.4%, with a mean α-angle of 37.4° ± 2. 4°, and 217 were flat type, accounting for 19.5%, with a mean α-angle of 46. 4° ±3. 8°. The differences of the α-angles of the three groups reached statistical significance ( F = 1636. 107, P ＜ 0. 01 ). Conclusions The morphology of anterior femoral head-neck junction can be classified into three types: concave type (type Ⅰ ), smooth type (type Ⅱ ) and flat type (type Ⅲ ). This classification could directly reflect α-angle, which helps to recognize the variation of the offset of anterior femoral head-neck junction. This study offers basic data for the diagnosis of FAI.

Abstract:Avascular necrosis of the femoral head (ANFH) is a common orthopedic disease characterized by bone cell apoptosis due to impaired blood supply to the femoral head, structural changes in the bone trabeculae, bone collapse, femoral head deformation, and ultimately irreversible changes, such as bone destruction and joint space narrowing, occur. [ 1 , 2 ] Epidemiological studies have shown that in the United States, there are over 25 000 new cases of ANFH annually, accounting for 14.9% of the approximately 480 000 total hip replacement surgeries performed each year. [ 3 ] Early ANFH symptoms are usually not obvious, and some patients have mild hip pain, followed by progression to joint mobility disorders, walking difficulties, secondary osteoarthritis, and eventually even joint replacement surgery, which places a heavy burden on patients and families. [ 4 ] Therefore, early diagnosis of ANFH and timely intervention are particularly important for maintaining joint function, improving the quality of life, and enhancing the prognosis of the patient. [ 5 ]

Risk Factors for Avascular Necrosis of the Femoral Head in Pediatric Femoral Neck Fractures

Abstract:Introduction A neck of femur fracture is a rare injury in the pediatric population and is of foremost importance, as it is associated with a high rate of complications. It usually occurs due to high-velocity trauma or a fall from a height. There is a scarcity of data on risk factors and their role in the prognostication of avascular necrosis. The purpose of the study was to retrospectively analyze the association of various risk factors with avascular necrosis (AVN) of the femoral head in patients with a neck of femur fracture in the pediatric age group. Material and methods The study included 21 (13 males and 8 females) pediatric patients with a neck of femur fracture treated at a university-level hospital. The patients were followed for a minimum of one year and the clinico-radiological outcome was analyzed using Ratliff criteria. The association of AVN with age, gender, side, fracture type, and injury with treatment delay, type of reduction, and type of internal fixation used was studied. Results The mean age of the treated patients was 11 (±3.178) years (range=5-16 years). Avascular necrosis was seen in four patients and coxa vara occurred in two of them. A statistically significant association was seen between the Delbet fracture type and avascular necrosis, and three out of four cases of AVN were a Type I fracture (p-value=0.006). Three out of six patients having concomitant skeletal or other organ injuries developed AVN (p-value=0.022). The rate of AVN was higher in patients who were managed after 48 hours of initial injury but no statistically significant correlation was found (p-value=0.314). No statistically significant association with AVN was found between gender, age, type of reduction (closed/open), or the implant used (cannulated screws/k-wires). Conclusions Multiple independent factors may have a role in the development of AVN of the femoral head in children. Prognostication should not be based on a single factor. Statistically significant results in this study have shown that the type of fracture and concomitant skeletal or other organ injuries are important risk factors and should be kept in mind. All independent risk factors must be noted and should be considered while prognosticating the outcome of a child with a neck of femur fracture.
